ABSTRACT

ABSTRACT Objective To investigate the prevalence of factors related to healthcare-associated infections, caused by multidrug-resistant bacteria, in a pediatric intensive care unit. Methods A retrospective case-control study conducted from January 1, 2007 to December 31, 2018, in São Paulo (SP), Brazil. The study was carried out at the pediatric intensive care unit of a high-complexity, tertiary care general hospital. The study included patients aged 1 month to 19 years, admitted to the pediatric intensive care unit, diagnosed as healthcare-associated infections. Results There was significant evidence of infection by multidrug-resistant bacteria associated with immunosuppressed patients (p<0.001), in whom the likelihood of multidrug-resistant bacteria infection was estimated to be nine-fold higher than among non-immunosuppressed patients (OR 8.97; 95%CI 2.69-29.94). In the analysis of multiple logistic regression model, we observed that immunosuppressed patients had an 8.5-fold higher chance of multidrug-resistant bacteria infection when compared to non-immunosuppressed patients (OR 8.48; 95%CI 2.54-28.35; p=0.001). There is evidence of association between the Case Group and presence of Gram-positive (p=0.007), coagulase-negative Staphylococcus (p<0.001), and Gram-negative (p=0.041) microorganisms. Conclusion The immunocompromised-state variable is a factor related to healthcare-associated infections caused by multidrug-resistant bacteria, and the Case Group presented higher proportions of Gram-positive microorganisms and coagulase-negative Staphylococcus.

Resumo Fundamento O maior risco de se desenvolver diabetes com o uso de estatinas é um desafio para a segurança do uso dessa classe de medicamentos em longo prazo. No entanto, poucos estudos analisaram essa questão durante síndromes coronarianas agudas (SCA). Objetivos Investigar a associação entre início precoce da terapia com estatina e níveis de glicemia em pacientes admitidos com SCA. Métodos Este foi um estudo retrospectivo de pacientes hospitalizados por SCA. Pacientes que nunca haviam usado estatinas foram incluídos e divididos segundo uso ou não de estatina nas primeiras 24 horas de internação. O desfecho primário foi a incidência de hiperglicemia na internação (definida como pico de glicemia > 200mg/dL). Modelos de regressão logística e modelos lineares multivariados foram usados para ajuste quanto a fatores de confusão e um modelo de pareamento por escore de propensão foi desenvolvido para comparações entre os dois grupos de interesses. Um valor de p menor que 0,05 foi considerado estatisticamente significativo. Resultados Um total de 2357 pacientes foram incluídos, 1704 deles alocados no grupo que receberam estatinas e 653 no grupo que não receberam estatinas nas primeiras 24 horas de internação. Após os ajustes, uso de estatina nas primeiras 24 horas foi associado com uma menor incidência de hiperglicemia durante a internação (OR ajustado = 0,61, IC95% 0,46-0,80; p < 0,001) e menor necessidade de uso de insulina (OR ajustado = 0,56, IC 95% 0,41-0,76; p < 0,001). Essas associações mantiveram-se similares nos modelos de pareamento por escore de propensão, bem como após análises de sensibilidade, como exclusão de pacientes que desenvolveram choque cardiogênico, infecção grave ou pacientes que foram a óbito durante a internação hospitalar. Conclusões Entre os pacientes internados com SCA que não receberam estatinas previamente, a terapia precoce com estatina associou-se independentemente com menor incidência de hiperglicemia durante a internação. Abstract Background Increased risk of new-onset diabetes with statins challenges the long-term safety of this drug class. However, few reports have analyzed this issue during acute coronary syndromes (ACS). Objective To explore the association between early initiation of statin therapy and blood glucose levels in patients admitted with ACS. Methods This was a retrospective analysis of patients hospitalized with ACS. Statin-naïve patients were included and divided according to their use or not of statins within the first 24 hours of hospitalization. The primary endpoint was incidence of in-hospital hyperglycemia (defined as peak blood glucose > 200 mg/dL). Multivariable linear and logistic regression models were used to adjust for confounders, and a propensity-score matching model was developed to further compare both groups of interest. A p-value of less than 0.05 was considered statistically significant. Results A total of 2,357 patients were included, 1,704 of them allocated in the statin group and 653 in the non-statin group. After adjustments, statin use in the first 24 hours was associated with a lower incidence of in-hospital hyperglycemia (adjusted OR=0.61, 95% CI 0.46-0.80; p < 0.001) and lower need for insulin therapy (adjusted OR = 0.56, 95% CI 0.41-0.76; p < 0.001). These associations remained similar in the propensity-score matching models, as well as after several sensitivity analyses, such as after excluding patients who developed cardiogenic shock, severe infection or who died during index-hospitalization. Conclusions Among statin-naïve patients admitted with ACS, early statin therapy was independently associated with lower incidence of in-hospital hyperglycemia. ABSTRACT

BACKGROUND Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) was confirmed in Brazil in February 2020, the first cases were followed by an increase in the number of cases throughout the country, resulting in an important public health crisis that requires fast and coordinated responses. OBJECTIVES The objective of this work is to describe the isolation and propagation properties of SARS-CoV-2 isolates from the first confirmed cases of coronavirus disease 2019 (COVID-19) in Brazil. METHODS After diagnosis in patients that returned from Italy to the São Paulo city in late February by RT-PCR, SARS-CoV-2 isolates were obtained in cell cultures and characterised by full genome sequencing, electron microscopy and in vitro replication properties. FINDINGS The virus isolate was recovered from nasopharyngeal specimen, propagated in Vero cells (E6, CCL-81 and hSLAM), with clear cytopathic effects, and characterised by full genome sequencing, electron microscopy and in vitro replication properties. Virus stocks - viable (titre 2.11 × 106 TCID50/mL, titre 1.5 × 106 PFUs/mL) and inactivated from isolate SARS.CoV2/SP02.2020.HIAE.Br were prepared and set available to the public health authorities and the scientific community in Brazil and abroad. MAIN CONCLUSION We believe that the protocols for virus growth and studies here described and the distribution initiative may constitute a viable model for other developing countries, not only to help a rapid effective pandemic response, but also to facilitate and support basic scientific research.

ABSTRACT

A doença de Parkinson (DP) é a segunda doença neu-rodegenerativa mais comum. Caracteriza-se clinicamente por tremor de repouso, rigidez, bradicinesia e instabilidade postural. O presente estudo busca caracterizar clinicamente uma população com DP em nosso meio. Objetivos: Descrever o perfil clínico de pacientes com DP de um hospital terciário do Rio de Janeiro, com foco nos sinais e sintomas motores da doença, para caracterização local da mesma. Métodos: Análise retrospectiva de 115 prontuários de pacientes com DP acompanhados no ambulatório de Distúrbios do Movimento do Hospital Universitário Pedro Ernesto da Universidade do Estado do Rio de Janeiro. Os parâmetros avaliados incluíram: idade, sexo, tem-po de doença, sinais motores predominantes, dimídio mais afetado, complicações motoras e tratamento utilizado. Resultados: Foram selecionados 95 participantes com DP, sendo a média de idade da amostra de 64,7 anos, com tempo médio de doença de 9,9 anos, pre-dominando no sexo masculino, em uma proporção de 2.5 homens para 1 mulher. Em relação ao tratamento utilizado, predominou o uso da levodopa /inibidor da dopa-descarboxilase, seguindo-se o pramipexole em monoterapia ou a associação de ambos. Conclusão: Apesar da grande variação na faixa etária, observou-se maior acome-timento da doença entre 60 e 80 anos, chamando atenção o tremor como sinal predominante. Entre as complicações motoras destaca-mos o "wearing off", seguido pelas discinesias. (AU)


Parkinson's disease (PD) is the second most common neurodegenerative disease. The main clinical features are rest tremor, rigidity, bradykinesia and postural instability. The present study aims to characterize clinically a population with PD in our commu-nity. Objective: To describe the clinical profile of PD patients from a tertiary hospital in Rio de Janeiro, with emphasis on motor signs and symptoms, for its local characterization. Methods: Retrospective analysis of 115 files from PD patients followed at the Movement Disorders Outpatient Clinic at Pedro Ernesto University Hospital from the State University of Rio de Janeiro. Parameters evaluated included: age, sex, disease duration, predominant motor signs, most affected side, motor complications and treatment prescribed. Results: Ninety five PD patients were selected, with an average age of the sample of 64.7 years, average disease duration of 9.9 years, male predominance, in a proportion of 2.5 men:1 woman. Regarding treatment, levodopa/dopa decarboxylase inhibitor was the most common drug prescribed, followed by pramipexol monotherapy or both in association. Conclusion: In spite of the wide age range, the disease affected mainly individuals between 60 and 80 years old. Tremor was the pre-dominant sign. Among motor complications, wearing off and dyskinesias should be highlighted. (AU)

ABSTRACT Objective To describe the microbiological characteristics and to assess the risk factors for mortality of ventilator-associated tracheobronchitis in a case-control study of intensive care patients. Methods This case-control study was conducted over a 6-year period in a 40-bed medical-surgical intensive care unit in a tertiary care, private hospital in São Paulo, Brazil. Case patients were identified using the Nosocomial Infection Control Committee database. For the analysis of risk factors, matched control subjects were selected from the same institution at a 1:8.8 ratio, between January 2006 and December 2011. Results A total of 40 episodes of ventilator-associated tracheobronchitis were evaluated in 40 patients in the intensive care unit, and 354 intensive care patients who did not experience tracheobronchitis were included as the Control Group. During the 6-year study period, a total of 42 organisms were identified (polymicrobial infections were 5%) and 88.2% of all the microorganisms identified were Gram-negative. Using a logistic regression model, we found the following independent risk factors for mortality in ventilator-associated tracheobronchitis patients: Acute Physiology and Chronic Health Evaluation I score (odds ratio 1.18 per unit of score; 95%CI: 1.05-1.38; p=0.01), and duration of mechanical ventilation (odds ratio 1.09 per day of mechanical ventilation; 95%CI: 1.03-1.17; p=0.004). Conclusion Our study provided insight into the risk factors for mortality and microbiological characteristics of ventilator-associated tracheobronchitis.


RESUMO Objetivo Descrever as características microbiológicas e avaliar os fatores de risco para mortalidade na traqueobronquite associada à ventilação mecânica em um estudo caso-controle de pacientes de terapia intensiva. Métodos Estudo realizado ao longo de 6 anos em uma unidade de terapia intensiva médico-cirúrgica de 40 leitos, em um hospital privado e de nível terciário em São Paulo, Brasil. O Grupo Caso foi identificado usando o banco de dados da Comissão de Controle de Infecção Hospitalar. O Grupo Controle foi pareado na proporção de 1:8,8 entre janeiro de 2006 e dezembro de 2011. Resultados Quarenta episódios de traqueobronquites associadas à ventilação foram avaliados em 40 pacientes na unidade de terapia intensiva, e 354 pacientes não apresentaram traqueobronquite Grupo Controle. Foram identificados 42 microrganismos (dos quais 5% foram infecções polimicrobianas), sendo que 88,2% de todos os microrganismos eram bactérias Gram-negativas. Usando um modelo de regressão logística, encontramos os seguintes fatores de risco independentes para mortalidade em pacientes com traqueobronquites associadas à ventilação: pontuação da Acute Physiology and Chronic Health Evaluation I (odds ratio 1,18 por uma unidade de pontuação; IC95%: 1,05-1,38; p=0,01) e duração da ventilação mecânica (odds ratio 1,09 por dia de ventilação mecânica; IC95%: 1,03-1,17; p=0,004). Conclusão Nosso estudo forneceu informações sobre os fatores de risco para mortalidade e características microbiológicas da traqueobronquite associada à ventilação mecânica.

ABSTRACT

Several species of black fungi have been reported as agents of subcutaneous phaeohyphomycosis. Although most of these fungi are considered opportunistic pathogens, they play an important role in phaeohyphomycosis, a disease considered an emergent mycosis among solid organ recipients. We report a case of phaeohyphomycosis caused by Alternaria infectoria of the left hand and the 4th finger of the right hand of a 68-year-old male who underwent a renal transplant 35 months before. The lesion was treated with surgical excision. One year later, the patient presented a new lesion on the 5th finger of the right hand, but this time caused by Colletotrichum gloeosporioides that was also removed surgically. Both lesions did not relapse after being removed. Antifungal susceptibility testing was performed against five antifungal drugs (amphotericin B, itraconazole, flucytosine, fluconazole and voriconazole). Alternaria infectoria was resistant to all five drugs and C. gloeosporioides was sensitive only to amphotericin B and voriconazole. We emphasize the need of histopathologic and microbiologic studies of new lesions of phaeohyphomycosis, since in this case the same patient was infected twice by two different fungi.


Diversas especies de hongos negros han sido descritos como agentes de feohifomicosis subcutáneas. A pesar que la gran mayoría de estos se consideran como oportunistas, juegan un papel importante en la feohifomicosis, enfermedad considerada una micosis emergente en pacientes trasplantados de órganos sólidos. Reportamos un caso de feohifomicosis subcutánea causada por Alternaria infectoria en el dorso de la mano izquierda y 4° falange de la mano derecha de un paciente de sexo masculino de 68 años de edad, que había sido sometido a un trasplante de riñón 35 meses antes. Se realizó escisión quirúrgica de la lesión. Después de un año, el paciente presentó una nueva lesión, esta vez en la 5° falange de la mano derecha causada por Colletotrichum gloeosporioides, lesión que también fue removida quirúrgicamente. El paciente no presentó nuevas lesiones después de las cirugías. Las pruebas de susceptibilidad antifúngica fueron realizados utilizando cinco antifúngicos (anfotericina B, itraconazol, 5-fluorcitosina, fluconazol y voriconazol). Alternaria infectoria presentó resistencia a los cinco antifíngicos y C. gloeosporioides fue susceptible solamente a anfotericina B y voriconazol. Enfatizamos la necesidad de realizar estudios histopatológicas y microbiológicos de nuevas lesiones de feohifomicosis, ya que el mismo paciente fue infectado por dos hongos diferentes.

ABSTRACT

Ochrobactrum anthropi infection in newborn patients is rare, and the treatment is challenging because of its widespread and unpredictable resistance to antimicrobial agents and discrepancies between in vitro susceptibility and in vivo efficacy. We report the clinical and microbiological characteristics of Ochrobactrum anthropi bacteremia in a preterm patient.

Fundamentos: A valva nativa pode ser substituída por uma prótese mecânica ou biológica e a escolha do tipo de substituto valvar é passível de discussão, pois implica morbimortalidade dos pacientes operados. Objetivo: Comparar a evolução dos pacientes que receberam próteses biológicas e mecânicas. Métodos: Estudo retrospectivo realizado em prontuários de pacientes submetidos à primeira cirurgia de troca de valva (mitral e/ou aórtica), no período de 1995 a 2000, em hospital terciário da rede pública. Foram selecionados 303 pacientes: 173 constituíram o grupo com prótese mecânica. As variáveis analisadas foram sexo, idade na época da cirurgia, comorbidades, tipo de cirurgia realizada, fração de ejeção ventricular no pré-operatório e no pós-operatório, ritmo cardíaco antes e pós-procedimento e desfechos: óbito, reoperação, acidente vascular encefálico (AVE), e endocardite infecciosa. Resultados: O tempo médio de seguimento foi 10 anos. A mortalidade tardia foi 3,46% (bioprótese) e 5,38% (mecânica), p=0,568. A reoperação foi maior no grupo bioprótese (43,4%) vs. grupo prótese mecânica (4,6%) - OR=15,83 (IC 95% 6,61-7,85; p>0,001). A prevalência de AVE foi semelhante (11% - bioprótese vs.9,5% - prótese mecânica, p=0,561), assim como a prevalência de endocardite (11,6% - bioprótese vs. 6,2% - prótese mecânica, p=0,115). Conclusões: A mortalidade, a prevalência de AVE e a endocardite foram estatisticamente semelhantes entre os grupos (prótese biológica e prótese mecânica) e o número de reoperações foi significativamente maior no grupo com bioprótese, tanto em posição mitral como em posição aórtica.


Background: The native valve may be replaced by a mechanical or biological prosthesis, with this choice often open to discussion, as it raises morbidity and mortality issues for operated patients. Objective: To compare the progress of patients receiving biological and mechanical prostheses. Methods: Retrospective study of the medical records of patients undergoing first valve replacements (aortic and/or mitral), from 1995 to 2000 at a tertiary hospital in the National Health network. Of the 303 records selected, 173 constituted the biological group and 130 the mechanical prosthesis group. The variables analyzed included gender, age when operated, comorbidities, type of surgery, ventricular ejection fraction and cardiac rhythm before and after surgery, and outcones: death, reoperation, stroke and infective endocarditis. Results: The median follow-up time was 10 years, with delayed mortality at 3.46% (bio-prothesis) and 5.38% (mechanical), p=0.568. There were more reoperations in the bio-prosthesis group (43,3%) than in the mechanical group (4.6%), with and OR of 15.83 (CI 95% 6.61 - 7.85; p<0.001). The stroke rate was similar (11% for bio-prosthesis vs 8.5% mechanical) p=0.561, with endocarditis at 11.6% for bio-prosthesis vs 6.2% mechanical (p=0.115). Conclusions: The mortality, stroke and endocarditis rates were statistically similar between the groups, but with significantly more reoperations in the mitral and aortic bio-prosthesis groups.

The purpose of this study was evaluate the occurrence of displacement on the apical and cervical thirds of artificial curved root canals with 30° of curvature, comparing the use of stainless steel hand and oscillatory files with nickel-titanium (NI-TI) hand, rotary and oscillatory files. Methods: Seventy artificial curved root canals were divided into 7 groups (n=10). All canals were prepared with sizes 15to 40 instruments at the working length according to a crown-down technique. The amount of material removed in the curved portion of the simulated canals at2 mm level outside and 11 mm inside was measured. Data were analyzed statistically by ANOVA and Tukey’s test (p<0.05). Results: There was statistically significant differences (p<0.05) at 2 mm level between NI-TI and steel stainless groups. At 11 mm, statistically significant differences (p<0.05) were observed between the groups that used rotary systems (5 and 6) and the remaining groups. Conclusions: The most centered preparations were those belonging to groups 5 and 6, at both levels. Root canal preparation with NI-TI files yielded better results than the stainless steel files especially when mechanically activated.

El objetivo de este trabajo "in vitro" fue evaluar la resistencia de unión de dos sistemas adhesivos utilizando el ensayo de microtracción, variando el tipo de substrato dentinario. Fueron utilizados 32 terceros molares humanos recientemente extraídos, los cuales fueron divididos en cuatro grupos de acuerdo con la orientación de los túbulos dentinarios (paralelos o perpendiculares) y con el tipo de sistema adhesivo utilizado (convencional o auto-acondicionante). Además de evaluar la resistencia de unión, todas las muestras fueron examinadas visualmente para determinar el tipo de fractura. Posteriormente, los resultados fueron sometidos a análisis estadística, permitiendo concluir que los adhesivos autoacondicionantes poseen un mejor comportamiento, mostrando mayor resistencia de unión a la dentina cuando los túbulos dentinarios están expuestos paralelamente al largo del eje del elemento dentario. Sin embargo cuando los túbulos dentinarios están perpendiculares al eje del diente, los sistemas adhesivos convencional y autoacondicionantes poseen resistencia adhesiva semejante. En cuanto al tipo de fractura de los cuerpos de prueba, las fallas adhesivas son predominantes, independiente del sistema adhesivo utilizado y del tipo de substrato dental


The aim of this in vitro study was to evaluate the microtensile bond strength, for two adhesive systems, varying the dentin substrate. Thirty-two recently extracted third molars human were used, and divided in four groups according to the dentin tubules orientation (parallels and perpendiculars) and the adhesive systems (total-etching and self-etching). The specimens were examined by optical microscope for determined the fracture type. After, the statistical analysis was evaluated, allowing to conclude that the self-etching adhesive systems showed better behavior and greater bond strength for dental substrate, when the dentin tubules were parallel exposited to dental axis. Nevertheless, when the dentin tubules were perpendiculars, the total-etching and self-etching adhesive systems showed similar statistical bond strength values. For the fracture type of the specimens, adhesive fractures were predominant, independent of the adhesive systems and the type of dental substrate used

ABSTRACT

O objetivo desta pesquisa experimental in vitro foi analisar comparativamente as deformações no canal radicular apóso uso do sistema rotatório de NiTi ProTaper Universal, ProDesign e técnicas híbridas utilizando os dois sistemas. Foram utilizados e instrumentados, setenta e cinco blocos com canais radiculares artificiais, divididos em cinco grupos(n=15). Para o grupo 1, os preparos foram realizados pelo sistema ProTaper Universal, utilizando a lima F3 no preparo apical. Para o grupo 2, utilizou-se o sistema ProDesign com preparo apical usando a lima 30/0,2. Para o grupo 3,os preparos foram realizados pelo sistema ProTaper Universal,utilizando a lima F2 no preparo apical. Para o grupo 4, os sistemas ProDesign e ProTaper foram utilizado sem conjunto com preparo realizado com a lima F2. Para o grupo 5, os sistemas ProDesign e ProTaper foram utilizados em conjunto com preparo apical realizado com as limas F1 e F2. As brocas de Gates-Glidden #5, #4, #3, #2 e#1 realizaram o preparo da porção reta de todos os canais,caracterizando o preparo cervicoapical. Foram analisados a diferença e o quociente da quantidade de resina removida nos seis milímetros de curvatura, aferidos tanto na parte interna quanto na parte externa. Zip e elbow foram quantificados pela forma final, após instrumentação em cada grupo testado. Para análise dos dados utilizou-se os testes paramétricos (ANOVA p<0,05), não paramétrico Kruskal--Wallis (p<0,05) e qui-quadrado (p<0,05). Para diferença e quociente, verificou-se melhores formas para os grupos 2 e3. Através da análise quantitativa e qualitativa, observou-se melhores preparos com o sistema ProDesign e ProTaperUniversal com preparo apical utilizando lima F2.


The aim of this in vitro experimental analysis was to comparethe changes in canal shape after the use of ProTaper UniversalNiTi rotary system, ProDesign system, and a hybridtechnique using both systems. A total of seventy-five simulatedroot canals were prepared and divided into five groups(n = 15). For Group 1, the ProTaper Universal System withapical preparation file F3 was used. The difference and thequotient the amount of removed resin were analyzed withinsix millimeters of the canal curvature, measured for both innerand outer walls. The amount of zip and elbow apical formationand mean final shape for each type tested were analyzed.Data were analyzed using parametric tests (ANOVA p<0.05),non-parametric test Kruskal-Wallis (p<0.05) and Chi-squaretest (p<0.05). When difference, quotient and final mean shapewere analyzed, the best preparations were observed in groups2 and 3. Through qualitative and quantitative analysis, thebest preparations were obtained with ProDesign System andProTaper Universal System with apical preparation file F2.

Objective: To describe a norovirus outbreak in a Brazilian long-term care facility from July 8 to 29, 2005. Methods: In the first 48 to 72 hours after onset of symptoms in inpatients and employees, the main infection control strategies were staff education, emphasis on hand washing, implementing contact precautions up to 48-72 hours after resolution of symptoms, complete cleaning of the rooms and exclusion of symptomatic employees from work until 48-72 hours after resolution of their symptoms. Epidemiological and clinical characteristics of the norovirus infections were described based on chart review. Results: The incidence among inpatients and employees was 41.3% and 16.25%, respectively. The main symptom was diarrhea, affecting 100% of inpatients and employees. Forty-four percent of specimens were positive by RIDASCREEN® Norovirus analyses, and identified as norovirus genogroup GII. Seventy percent of inpatients were women and their age range was 51-98 years. Inpatients had in average two comorbid conditions - 87.3% with cardiovascular or chronic pulmonary condition and 47.6% with dementia. There was not relapse or death. Conclusions: The early infection-control measures associated to surveillance are required to keep long-term care facilities free of noroviruses and to protect those who are most vulnerable.


Objetivo: Descrever um surto de norovírus ocorrido em uma instituição de longa permanência no Brasil, de 8 a 29 de julho 2005. Métodos: Nas primeiras 48 a 72 horas após o início dos sintomas entre moradores da instituição de longa permanência e funcionários, as principais estratégias de controle da infecção foram: educação da equipe, reforço na higienização das mãos, implementação de precauções de contato até 48 a 72 horas após o término de sintomas, limpeza "terminal" dos quartos dos moradores e afastamento dos funcionários sintomáticos até 48 a 72 horas após o término dos sintomas. As características clínicas e epidemiológicas das infecções por norovírus foram descritas baseadas nos dados dos prontuários. Resultados: A incidência foi 41,3% entre moradores e 16,25% entre funcionários. O principal sintoma foi diarreia, acometendo 100% dos casos; 44% das amostras de fezes foram positivas pela análise RIDASCREEN® Norovirus, com a identificação do norovírus genogrupo II. Setenta por cento dos moradores eram do sexo feminino, com idade de 51 a 98 anos. Os moradores tinham, em média, duas comorbidades, sendo 87,3% com doenças cardiovasculares ou pulmonares e 47,6% com demência. Não houve recidiva do surto ou óbitos. Conclusões: As medidas precoces de prevenção associada à vigilância são estratégias que mantém as instituições de longa permanência livres de infecções por norovírus e protegem aqueles indivíduos mais vulneráveis.

Aim: The aim of this study was to evaluate the micro shear bond strength of different ceramic systems – IPS Empress 2, Cergogold, In-Ceram Alumina and Cercon - and a dual luting agent. Methods: Twelve specimens of each ceramic were fabricated and divided according different surface treatments: Group 1: No additional treatment was applied to the ceramic surface; Group 2: Ceramics were etched with 9.5% hydrofluoric acid; Group 3: specimens treated with airborne particle abrasion for each ceramic system in accordance with manufacturer’s instructions (n=20). The tests were performed after 24 h or after water storage for 6 months. Data were then assessed statistically using the 3-way ANOVA and the Tukey’s test (P<0.05). Results: For Cergogold and IPS Empress 2 systems, the treatments performed with airborne particle abrasion and hydrofluoric acid showed no significant differences from each other, and both were superior to the groups without treatment. For Cercon and In-Ceram ceramics, no differences were found among the groups (P<0.05). When the surface was treated with hydrofluoric acid, the highest bond strength was found to IPS Empress 2 in the 6-month storage period (P<0.05). Conclusion: Lower bond strength values were only observed with IPS Empress 2 ceramic for the control group in the 6-month storage (P<0.05).

ABSTRACT

O objetivo deste trabalho foi avaliar as características dos sistemas adesivos disponíveis atualmente considerando a aplicação ou não do condicionamento prévio com ácido fosfórico. O conhecimento das estruturas e o tratamento adequado do substrato dental são fundamentais para utilização dos adesivos convencionais. Os sistemas adesivos autocondicionantes possuem técnica simplificada dispensando o condicionamento ácido prévio. O profissional deverá ter conhecimento dos mecanismos de adesão ao substrato dental, experiência clínica e domínio da técnica de aplicação do adesivo escolhido. O condicionamento ácido prévio não deve ser considerado indispensável na Odontologia adesiva atual.

This is a epidemiological paper concerning the use of cigarettes by medicine students and also a literature review based on several different articles. According to the World Health Organization (WHO), smoking is the main cause of preventable diseases in the planet and is directly responsable for about 8 millions deaths a year. ABSTRACT

O objetivo deste trabalho foi aplicar dois métodos de mensuração da infiltração marginal da resina composta com intuito de verificar qual delas permite maior confiabilidade dos resultados. Foram confeccionadas cavidades de classe V, com margem cervical em dentina, restauradas com o sistema restaurador Single Bond/Z250. As medidas de infiltração dos grupos foram realizadas por meio de dois métodos: análise por escores e análise por programa de computador específico (Image Tool), através do qual foram realizadas medidas para obtenção de valores em milímetros. A análise estatística demonstrou que não houve compatibilidade dos resultados obtidos através das duas técnicas.
