ABSTRACT

OBJECTIVE To investigate the role of clinical pharmacists in the treatment of a patient with Epstein-Barr (EB) virus encephalitis. METHODS Clinical pharmacist participated in drug diagnosis and therapy for a patient with EB virus encephalitis. According to the physiological characteristics of the disease and the pharmacokinetic-pharmacodynamic characteristics of antibiotics, clinical pharmacists suggested that the dose should be adjusted as ceftriaxone 2 g, q12 h+meropenem 2 g, q8 h. Based on the uncontrolled infection of the patient, pharmacists suggested that ceftriaxone should be stopped and vancomycin 1 million U and q12 h should be used as alternative therapy. According to the results of etiology, pharmacists suggested that acyclovir should be discontinued and replaced with ganciclovir 5 mg/kg, q12 h. The electrolyte disturbance of the patient may be adverse drug reactions caused by Mannitol injection, it was recommended to stop the drug. RESULTS The clinician followed the advice of the clinical pharmacists. After treatment, the patient improved and was discharged. CONCLUSIONS Clinical pharmacists can carry out pharmaceutical care for patients with EB virus encephalitis, assist physicians in optimizing the treatment plan of patients, and ensure the effectiveness and safety of drug treatment.

ABSTRACT

OBJECTIVE:To investigate the omeprazole off-label use and provide reference for rational drug use in clinic. METHODS:In retrospective survey,1838 omeprazole prescriptions were selected from outpatient department during Jun. 2014 to May 2017 by random number table. According to the latest drug instruction, whether the off-label use or not was determined. Off-label use was determined by reviewing domestic and foreign guidelines, literatures and Micromedex database. RESULTS:Among 1838 pre-scriptions,there were 1750 prescriptions of off-label drug use (95.21%),involving 13 items and 3 types of off-label drug use. Main type of off-label drug use was over-indication medication, involving 1747 prescriptions (96.47%). There was no statistical significance in the incidence of off-label drug use between digestive department and non-digestive department or among physicians at different levels(P>0.05). Among 13 items of off-label drug use,7 items were supported by evidence-based medicine evidence of domestic and foreign guideline, expert consensus/suggestion or literature reports, etc.;among which 1 item of off-label drug use were included in Micromedex grading system and other 6 items had no evidence-based medicine evidence. CONCLUSIONS:The phenomenon of omeprazole off-label use is widespread in outpatient department of the hospital, and some off-label drug use are sup-ported by evidences. There are differences in the quality of those evidences. It is suggested to standardize the off-label drug use to avoid legal risks and guarantee the safety of drug use for patients.

ABSTRACT

Objective:To explore the effects of Chinese medicine clinical pharmacists participating in the formulation and imple-mentation of traditional Chinese medicine clinical pathway in orthopaedics department. Methods: Totally 60 cases of radius fracture patients were selected from 2009 to 2011 as the control group, and 54 cases of radius fracture patients were selected as the observation group in our hospital during 2015 and 2016. The observation group was instructed by both Chinese medicine clinical pharmacists and doctors;meanwhile, Chinese medicine clinical pharmacists were responsible for the entire pharmaceutical care. The clinical curative effect of the two groups, the rational use rate of traditional Chinese medicines, the average length of stay, the average cost of traditional Chinese medicines and the incidence of ADR between the groups were compared. The effect of Chinese medicine clinical pharmacists participating in the formulation and implementation of clinical pathway was evaluated. Results:After the formulation and implementa-tion of clinical pathway, the total effective rate was improved from 80. 0% to 92. 6%, the rational use rate of traditional Chinese medi-cines increased significantly from 60. 0% to 85. 2%, and the differences were statistically significant (P<0. 05). The average length of stay decreased significantly from (14.0 ±2.3) d to (11.0 ±3.1) d and the difference was statistically significant (P<0.05). Meanwhile, the average cost of traditional Chinese medicines decreased from (887. 07 ± 186. 44) yuan to (437. 14 ± 127. 32) yuan and the difference was statistically significant (P<0. 05). Moreover, the incidence of ADR decreased from 16. 7% to 11. 1%. Con-clusion:Chinese medicine clinical pharmacists should participate in the whole process of clinical pathway, which can play an important role in ensuring the implementation of clinical pathway and standardizing the rational drug use. It can significantly make the clinical medication safer, and more effective and economical. It can shorten the hospital stay and reduce the hospitalization costs as well.

ABSTRACT

Objective To detect CT appearances of different stomach diseases and evaluate the value of CT in the diagnosis of gastric diseases. Methods 62 cases of stomach diseases were detected by CT.Results 49 cases of gastric cancer,7 cases of stomach leiomyoma,3 cases of stomach ulcer,1 cases pylorus fleshy were examined,2 cases of stomach epineurial tumor. There was 91% correspondent rate of CT with pathologic examination. Conclusion CT imaging analysis is not only important to determine to gastric cancer, but also is great valuable to diagnose and differentiate the different stomach diseases.
