ABSTRACT

La tormenta tiroidea es un estado crítico y poco frecuente que condiciona la disfunción de múltiples órganos por el efecto del exceso de las hormonas tiroideas, esta disfunción endócrina tiene una elevada mortalidad y genera manifestaciones típicas como la taquicardia, fiebre, alteraciones gastrointestinales, cardiovasculares y del sistema nervioso central. El embarazo se ha asociado con un incremento en la incidencia de arritmias. Necesitan un tratamiento inmediato con drogas antiarrítmicas, cardioversión eléctrica o cesárea de urgencia. El WPW es una anormalidad cardiaca congénita que consiste en la presencia de un haz anómalo (Haz de Kent) que evita el sistema normal de conducción uniendo directamente aurículas y ventrículos. Veremos el caso de una gestante de 32 semanas que presenta un cuadro de tormenta tiroidea y múltiples episodios de taquicardia paroxística supraventricular (TPS), de tórpida y sombría evolución clínica mediada por un haz anómalo de Kent intermitente. Es evidente que la tormenta tiroidea en el contexto de la gestación produjo cambios en las propiedades electrofisiológicas del haz anómalo de Kent intermitente lo cual propició el desarrollo de múltiples taquicardias paroxísticas supraventriculares refractarias a la cardioversión eléctrica y farmacológica. Tampoco mejoró con la tiroidectomía total, solamente cedió por completo con la ablación por catéter de radiofrecuencia del haz anómalo de Kent.


Thyroid storm is a critical and infrequent state that conditions the dysfunction of multiple organs due to the effect of excess thyroid hormones. This endocrine dysfunction has a high mortality and generates typical manifestations such as tachycardia, fever, gastrointestinal, cardiovascular and heart disorders, and the central nervous system. Pregnancy has been associated with an increased incidence of arrhythmias. They need immediate treatment with antiarrhythmic drugs, electrical cardioversion, or emergency caesarean section. WPW is a congenital cardiac abnormality that consists of the presence of an abnormal bundle (Kent bundle) that prevents the normal conduction system, directly joining the atria and ventricles. We will see the case of a 32-week pregnant woman who presented symptoms of thyroid storm and multiple episodes of paroxysmal supraventricular tachycardia (PST), with a torpid clinical course mediated by an abnormal intermittent Kent bundle. It is evident that the thyroid storm in the context of pregnancy produced changes in the electrophysiological properties of the intermittent Kent bundle, which led to the development of multiple PST refractory to electrical and pharmacological cardioversion. Moreover, it also did not improve with total thyroidectomy, only resolved completely with radiofrequency catheter ablation of the Kent bundle.

Abstract Purpose: The Tpeak-Tend interval of the T wave has emerged as a new electrocardiographic marker of increased transmural dispersion of ventricular repolarization. We aimed to determine the presence of cardiac conduction system disorders in patients with systemic arterial hypertension (SAH) who have altered Tpeak-Tend interval of the T wave. Methods: The 67 patients with SAH were divided into two groups. Those with prolonged (≥ 77 ms) Tpeak-Tend intervals, 21 (31%) patients were in the study group. Those with normal (< 77 ms) Tpeak-Tend intervals, 46 (69%) patients were in the control group. Alteration of ventricular repolarization manifested as a prolongation of the Tpeak-Tend interval was detected by computerized electrocardiographic analysis tools. Results: The median value of QRS complex duration was significantly wider in the study group as compared to the control group (110 ± 12 ms vs. 94 ± 8 ms p < 0.001). There was a significantly greater incidence of left anterior hemiblock in the study group (14% vs. 0% p < 0.04). The median value of the QTc interval was significantly greater in the study group (440 ± 26 vs. 422 ± 15 p < 0.01). There was a significantly greater incidence of patients with prolonged QTc interval in the study group (33% vs. 11% p < 0.02). The median value of the Tpeak-Tend interval was significantly greater in the study group (84 ± 5 ms vs. 65 ± 4 ms p < 0.001), as well as, the Tpeak-Tend/QTc ratio in the study group (0.19 ± 0.1 vs. 0.16 ± 0.1 p < 0.001). Conclusion: There is a significantly greater ventricular repolarization disorders and abnormalities of the cardiac conduction system in SAH patients who possess altered Tpeak-Tend interval of the T wave.


Resumen Objetivo: El intervalo Tpico-Tfinal de la onda T es un marcador electrocardiográfico de la dispersión transmural aumentada de la repolarización ventricular. Investigamos la presencia de trastornos del sistema de conducción cardíaca en pacientes con hipertensión arterial sistémica (HA) que poseen alterado el intervalo Tpico-Tfinal de la onda T. Métodos: Los 67 pacientes con HA fueron divididos en dos grupos. Aquellos con intervalos de Tpico-Tfinal prolongados (≥ 77 ms), 21 (31%) pacientes (grupo de estudio). Aquellos con intervalos normales (< 77 ms) Tpico-Tfinal, 46 (69%) pacientes (grupo control). Los intervalos Tpico-Tfinal fueron medidos por herramientas de análisis electrocardiográfico computarizado. Resultados: El valor mediano de la duración del complejo QRS fue significativamente más amplio en el grupo de estudio (110 ± 12 ms vs. 94 ± 8 ms p < 0.001). Hubo una incidencia significativamente mayor de hemibloqueo anterior izquierdo en el grupo de estudio (14% vs. 0% p < 0.04). El valor mediano del intervalo QTc fue significativamente mayor en el grupo de estudio (440 ± 26 vs. 422 ± 15 p < 0.01). Hubo una incidencia significativamente mayor de pacientes con intervalo QTc prolongado en el grupo de estudio (33% vs. 11% p < 0.02). El valor mediano del intervalo Tpico-Tfinal fue significativamente mayor en el grupo de estudio (84 ± 5 ms vs. 65 ± 4 ms p < 0.001), así como el cociente Tpico-Tfinal/QTc (0.19 ± 0.1 vs. 0.16 ± 0.1 p < 0.001). Conclusión: Existe una alteración de la repolarización ventricular significativamente mayor y anomalías del sistema de conducción cardíaca en pacientes con HA que poseen alteración del intervalo Tpico-Tfinal de la onda T.

ABSTRACT Objective. To describe the mortality trends of diabetes mellitus (DM) in Colombia, by sex and age group, from 1979 to 2017. Methods. We carried out an ecological study using mortality data from the Colombian National Administrative Department of Statistics. Crude and age-standardized annual mortality rates per 100 000 people were estimated. Trends of standardized rates were described by sex and age groups. Joinpoint regression models were performed to study mortality trends. Results. Throughout the whole period, the total number of DM recorded deaths in Colombia was 200 650, 58% (116 316) in women (p<0.05). The age-standardized mortality rates (ASMR) by sex increased from 13.2 to 26.6 deaths per 100 000 in women and from 10.1 to 22.7 in men from 1979 to 1999. We observed a decrease from 26.6 to 15.4 per 100 000 in women, and from 22.7 to 15.9 in men for the period 1999-2017. The joinpoint regression analysis showed that the average annual percentage change of the period did not vary in both sexes (men: -0.2%, 95% CI -1.0 to 1.4%; women: 0.7%, 95% CI -0.1 to 1.6%). Conclusions. The DM mortality showed a decreasing trend after 2000 in women and 2004 in men. Primary and secondary prevention programs must continue to be strengthened for an earlier diagnosis of diabetes.


RESUMEN Objetivo. Describir las tendencias de la mortalidad por diabetes mellitus (DM) en Colombia, por sexo y grupo de edad, entre 1979 y 2017. Métodos. Estudio ecológico con datos de mortalidad del Departamento Administrativo Nacional de Estadística de Colombia. Se estimaron las tasas de mortalidad anuales brutas y ajustadas por edad por cada 100 000 personas. Se describieron las tendencias de las tasas ajustadas por sexo y grupos de edad. Se realizaron modelos de regresión joinpoint para estudiar las tendencias de la mortalidad. Resultados. En el período del estudio, el número total de muertes por DM registradas en Colombia fue de 200 650, el 58% (116 316) en mujeres (p<0,05). Las tasas de mortalidad ajustadas por edad, en cada sexo, aumentaron de 13,2 a 26,6 muertes por 100 000 en las mujeres y de 10,1 a 22,7 en los hombres entre 1979 y 1999. En el período 1999-2017 se observó una disminución de 26,6 a 15,4 por 100 000 en las mujeres y de 22,7 a 15,9 en los hombres. El análisis de regresión joinpoint demostró que el cambio porcentual anual medio del período no varió en ambos sexos (hombres: -0,2%, IC 95% -1,0 a 1,4%; mujeres: 0,7%, IC 95% -0,1 a 1,6%). Conclusiones. La mortalidad por DM mostró una tendencia decreciente después del año 2000 en las mujeres y del 2004 en los hombres. Es necesario seguir fortaleciendo los programas de prevención primaria y secundaria a fin de alcanzar un diagnóstico más temprano de la diabetes.

ABSTRACT

Gymnogeophagus labiatus and G. lacustris have been long recognized as sister species exhibiting different ecological requirements. Gymnogeophagus labiatus occurs in rock bottom rivers in the hydrographic basins of Patos Lagoon (HBP) and Tramandaí River (HBT), while G. lacustris is exclusive from sand bottom coastal lagoons of the HBT. In this study, we used molecular markers, morphological measurements and data from nuptial male coloration to investigate the evolutionary relationship between these species in each hydrographic basin. We found, for all data sets, a closer relationship between G. labiatus and G. lacustris from the HBT than between G. labiatus populations from HBT and HBP. In particular, lip area had a large intraspecific plasticity, being uninformative to diagnose G. lacustris from G. labiatus. Molecular clock-based estimates suggest a recent divergence between species in the HBT (17,000 years ago), but not between G. labiatus from HBP and HBT (3.6 millions of years ago). Finally, we also found a divergent G. labiatus genetic lineage from the Camaquã River, in the HBP. These results show that the current taxonomy of G. labiatus and G. lacustris does not properly represent evolutionary lineages in these species.(AU)


Gymnogeophagus labiatus e G. lacustris vêm sendo consideradas espécies irmãs que possuem diferentes exigências ecológicas. Gymnogeophagus labiatus ocorre em rios de fundo de pedra nas bacias hidrográficas da Laguna dos Patos (HBP) e do rio Tramandaí (HBT), enquanto G. lacustris é exclusivo da HBT, ocorrendo em lagoas costeiras de fundo de arenoso. Nesse estudo, foram usados marcadores moleculares, medidas morfológicas e dados sobre a coloração nupcial em machos para investigar a relação evolutiva entre estas espécies em cada bacia hidrográfica. Para todos os conjuntos de dados foi observada uma relação mais próxima entre G. labiatus e G. lacustris da HBT do que entre as populações de G. labiatus da HBP e HBT. Em particular, a área do lábio teve uma grande plasticidade intraespecífica, não sendo informativa para diagnosticar G. lacustris de G. labiatus. Estimativas baseadas no relógio molecular sugeriram uma divergência recente entre as espécies da HBT (17.000 anos atrás), mas não entre as populações de G. labiatus da HBP e HBT (3,6 milhões de anos atrás). Finalmente, também foi encontrada uma linhagem genética de G. labiatus divergente no rio Camaquã, na HBP. Esses resultados mostram que a taxonomia atual de G. labiatus e G. lacustris não representa adequadamente as linhagens evolutivas nessas espécies.(AU)

RESUMEN Objetivo: Estimar la carga económica asociada a la enfermedad renal crónica (ERC) en Colombia, para el periodo 2015-2016. Materiales y métodos: Se estimó la carga económica de la ERC en Colombia a partir de datos de terceros pagadores. Se realizó un costeo de los estadios de la ERC mediante la técnica de macro-costeo o top-down, con el fin de conocer el impacto económico de cada estadio en la carga global de la enfermedad. Se construyó un modelo de Markov que simulara la historia natural de la enfermedad. Se utilizó una tasa de descuento del 5% y ciclos anuales. A cada estado de la ERC se le asignó un costo relacionado con la atención, con lo cual se obtuvo un estimado del costo y de la carga de enfermedad de todos los pacientes con ERC. Resultados: Al final de los ciclos de todos los pacientes desde el estadio 1-5, incluyendo el trasplante, se estimó una carga económica que asciende a COP 10,5 billones (IC 95% COP 8,7- COP 14,4). En el escenario con tasa de descuento del 5%, la carga económica asciende a COP 7,8 billones. Conclusión: La ERC representa una alta carga económica al sistema de salud colombiano, la cual oscila entre COP 8,7 y COP 14,4 billones, que representarían entre el 1,6% y el 2,7% del producto interno bruto de Colombia en el 2015.


ABSTRACT: Objective: Estimate the economic burden due to chronic kidney disease (CKD) in Colombia, for 2015-2016. Methods: We estimated the economic burden of CKD through third payer's data. A top-down technique was used to estimate the costs related to CKD as input to estimate the economic burden of the disease. A Markov model was conducted to simulate the natural history of the disease. A 5% discount rate and annual cycles were used. Each stage of the disease had a cost associated with the disease, producing and estimation of the economic burden of all patients with CKD. Results: At the end of the modeling from stages 1-5, including transplantation, the economic burden of CKD was COP $10.5 billion (IC 95% $8.7-$14.4). In the discount rate scenario, the economic burden was COP $7.8 billons. Conclusion: CKD represents a high economic burden on the Colombian health system, which ranges from COP $ 8.7- $ 14.4 billion that would represent between 1.6% and 2.7% of Colombia's Gross Domestic Product, 2015.

Abstract Introduction: Acute otitis media is the main cause of consultation, antibiotic use, and ambulatory surgery in developed countries; besides, it is associated with an important economic burden. However, non-medical indirect costs of acute otitis media, which are relevant in this pathology, have been underestimated. Objective: To estimate the costs of acute otitis media in pediatric patients in Cartagena, Colombia. Materials and methods: We conducted a prospective study of micro-costing between 2014 and 2015. The direct and indirect costs of acute otitis media were determined through forms applied to parents or caregivers. Loss of productivity was estimated based on the monthly legal minimum wage of 2014 (COP $616.000) (USD $308). Results: A total of 62 episodes of acute otitis media occurred. The total economic costs attributed per episode was COP $358,954 (standard deviation: SD ± COP $254,903, i.e., USD $179). The total economic burden was COP $22,503,141 (USD $11,250), the indirect costs per episode were COP $101,402 (USD $51), and the average care time spent by parents was 3.7 days. Conclusion: The estimated costs of acute otitis media in this study were lower than the costs estimated in a review of high-income countries and similar to those of low-income countries such as Nigeria. Information on total costs (direct and indirect) of acute otitis media is necessary for public health decision-making and for full cost-effectiveness assessments.


Resumen Introducción. La otitis media aguda es la principal causa de consultas médicas, de uso de antibióticos y de cirugías ambulatorias en los países desarrollados. Está asociada con una significativa carga económica, pero sus costos indirectos no médicos, los cuales son relevantes en esta enfermedad, se han subestimado. Objetivo. Estimar los costos de la otitis media aguda en pacientes pediátricos en Cartagena, Colombia. Materiales y métodos: Se hizo un estudio prospectivo de microcosteo entre el 2014 y el 2015. Se determinaron los costos directos e indirectos de la otitis media aguda mediante encuestas a los padres o cuidadores. La pérdida de productividad se estimó con base en el salario mínimo legal vigente mensual del 2014(COP$616.000) (USD$308). Resultados. Se presentaron 62 episodios de otitis media aguda. Los costos económicos totales por episodio fueron de COP $358.954 (desviación estándar, DE: ± $254.903) (USD $179). La carga económica total fue de COP $22'503.141 (USD $11.250), los costos indirectos por episodio fueron de COP $101.402 (USD $51) y el tiempo promedio empleado por los padres en el cuidado fue de 3,7 días. Conclusiones. Los costos estimados de la otitis media aguda en el presente estudio fueron menores a los estimados en países con ingresos altos y similares a los de países con ingresos bajos como Nigeria, según una revisión bibliográfica. La información sobre los costos totales directos e indirectos de la otitis es necesaria para la adopción de decisiones en salud pública y para hacer evaluaciones económicas completas de costo-efectividad.

RESUMEN Objetivo Estimar el gasto de bolsillo y la probabilidad de gasto catastrófico de los hogares y sus determinantes socioeconómicos en Cartagena, Colombia. Materiales y Métodos Estudio transversal en una muestra poblacional estratificada aleatoria de hogares de Cartagena. Se estimaron dos modelos de regresión cuyas variables dependientes fueron gasto de bolsillo y probabilidad de gasto catastrófico en salud de los hogares. Resultados El gasto de bolsillo promedio anual en hogares pobres fue 1 566 036 COP (US$783) (IC95% 1 117 597-2 014 475); en hogares de estrato medio 2 492 928 COP (US$1246) (IC95% 1 695 845-3 290 011) y en hogares ricos 4 577 172 COP (US$2 288) (IC95% 1 838 222-7 316 122). Como proporción del ingreso, el gasto de bolsillo en salud fue de 14,6% en los hogares pobres, de 8,2% en los hogares de estrato medio y de 7,0% en los hogares ricos. La probabilidad de gasto catastrófico en salud de los hogares pobres fue 30,6% (IC95% 25,6-35,5%), de los de estrato medio del 10,2% (IC95% 4,5-15,9%) y de los hogares de estrato alto del 8,6% (IC95% 1,823,0%). El estrato socioeconómico, la educación y la ocupación fueron los principales determinantes del gasto de bolsillo en salud y de la probabilidad de incurrir en gasto catastrófico en salud. Conclusiones En el sistema de salud persisten desigualdades en la protección financiera de los hogares contra el gasto de bolsillo y la probabilidad de gasto catastrófico. El presente estudio genera evidencia para revisar la política de protección social de los hogares socioeconómicamente más vulnerables.(AU)


ABSTRACT Objective To estimate out-of-pocket (OOP) health expenditure and the probability to incur in catastrophic health expenditure, as well as the socio-demographic determinants of households in Cartagena, Colombia. Materials and Methods Population-based cross-sectional study on a randomized stratified sample of Cartagena households. Two regression models were developed using OOP health expenditure and the probability to incur in catastrophic health expenditure as dependent variables. Results The average annual OOP health expenditure was $1 566 036 COP (US$783) (95%CI: $1 117 597 - $2 014 475) in poor households, $2 492 928 COP (US$1 246) (95%CI: $1 695 845 - $3 290 011) in middle class households, and $4 577 172 COP (uS$2 288) (95%CI: $1 838 222 - $7 316 122) in upper class households. Regarding the household income ratio, the OOP health expenditure was 14.6% in poor households, 8.2% in middle class, and 7.0% in upper class households. The probability to incur in catastrophic health expenditure was 30.6% (95%CI: 25.6% - 35.5%), 10.2% (95%CI: 4.5%-15.9%) and 8.6% (95%CI: 1.8%-23%) in the low, middle- and high-class households, respectively. Educational attainment, socioeconomic strata and employment were the main determinants of OOP and the probability to incur in catastrophic health expenditure. Conclusions The health system has persistent inequalities regarding the financial protection of households related to out-of-pocket expenditure and the probability of catastrophic expenditure. This study provides evidence to review the social protection policy for the most socio-economically vulnerable households.(AU)

ABSTRACT

Uno de los factores condicionantes del peso corporal es la percepción que la persona tiene de su imagen corporal. El objetivo fue conocer la percepción de imagen corporal según el índice de masa corporal (IMC) en 107 Médicos Residentes del Hospital de Clínicas.Se utilizó un diseño observacional descriptivo de corte transverso. Se exploraron las variables edad, sexo, circunferencia abdominal, peso, talla, IMC, percepción del peso corporal. La circunferencia abdominal fue de riesgo elevado en 37% de los varones y en 16% de las mujeres, de riesgo muy elevado en 13% de varones y en 29% de las mujeres (p=0,009). En cuanto al IMC la media fue de 25,7±4,6; estando en peso normal el 45% y en sobrepeso el 43%. Se encontró más mujeres con peso normal (63% vs 28%). Los varones presentan más sobrepeso (61% vs 28%) (p=0,0009). En cuanto a la percepción del peso corporal se perciben con obesidad 23%; con sobrepeso 47% y con peso normal 28%. Cruzando los datos con los IMC verdaderos, tenemos que el 43% no tiene la percepción real de acuerdo a su IMC. La concordancia en cuanto a IMC real y la percepción es del 63%. Casi la mitad de los residentes estudiados no tiene la percepción real de acuerdo a su IMC. La percepción de obesidad es mayor en mujeres que en hombres acorde a su IMC real. Se requiere de más investigaciones en referencia al tema aplicándolo a toda la población del hospital para crear medidas educativas de oportuna intervención en la salud del personal sanitario(AU)


The perception of their own body image that a person has is a conditioning factor of the body weight. We aimed to know the perception of body image assessing the body mass index (BMI) in 107 medical residents of the Clinical Hospital. We utilized a descriptive observational design to evaluate multiple variables like gender, age, abdominal diameter, weight, height, body mass index, and perception of body weight. The abdominal diameter was of high risk in 37% of males and 16% in females, and of very high risk in 13% of males and in 29% of females (p=0,009). The medium BMI was 25,7±4,6. There was a normal BMI in 45% of the subjects and overweight in 43%. There were more female with normal weight than male (63% vs 28%), and there were more overweight male than female (61% vs 28%) (p=0,0009). Considering the perception of body weight, 23% perceive themselves to be obese, 47% with overweight, and 28% with normal body weight. Comparing this perception with real BMI, we observed that 43% of the subjects do not have a real perception according to the BMI. The agreement between real BMI and the image perception is 63%. Almost half of the medical residents studied do not have a real body weight perception according to BMI. The perception of obesity is greater in women than in men according to the real BMI. There is a necessity of larger studies including the total population of the hospital to create educational measures for timely intervention in the health of hospital personal(AU)

Abstract In spite of many genetic studies that contributed for a deep knowledge about the peopling of the Americas, no consensus has emerged about important parameters such as the effective size of the Native Americans founder population. Previous estimates based on genomic datasets may have been biased by the use of admixed individuals from Latino populations, while other recent studies using samples from Native American individuals relied on approximated analytical approaches. In this study we use resequencing data for nine independent regions in a set of Native American and Siberian individuals and a full-likelihood approach based on isolation-with-migration scenarios accounting for recent flow between Asian and Native American populations. Our results suggest that, in agreement with previous studies, the effective size of the Native American population was small, most likely in the order of a few hundred individuals, with point estimates close to 250 individuals, even though credible intervals include a number as large as ~4,000 individuals. Recognizing the size of the genetic bottleneck during the peopling of the Americas is important for determining the extent of genetic markers needed to characterize Native American populations in genome-wide studies and to evaluate the adaptive potential of genetic variants in this population.

Introducción y objetivo: La rosácea es una enfermedad crónica de etiología multifactorial que genera principalmente una respuesta inflamatoria. El objetivo del presente estudio fue describir la presencia de anticuerpos antinucleares (ANAS) en pacientes diagnosticados con rosácea en un centro dermatológico de Bogotá. Métodos: Estudio prospectivo que describe la presencia de ANAS por IFI en pacientes con rosácea de cualquier edad que consultaron a FUNINDERMA. Mediante la estimación de frecuencias absolutas y relativas, se caracteriza-ron variables como la edad, sexo, presencia de ANAS, diluciones de los positivos y duración de la enfermedad. Los análisis estadísticos y la recopilación de la información se realiza-ron en Microsoft Excel 2016. El presente estudio se califica sin riesgo para los pacientes. Resultados: En total, se analizaron 42 pacientes con rosácea, 73,8% mujeres. La edad pro-medio fue de 46,6 años (rango 15-98).

RESUMEN Objetivo: Estimar los costos económicos de las infecciones respiratorias agudas (IRA) en el municipio de AquitaniaBoyacá, 2014. Métodos: Evaluación económica parcial, tipo descripción de costos desde la perspectiva del sistema de salud. Se utilizó la técnica de micro-costeo para estimar los costos asociados a las IRA en Aquitania-Boyacá. Los costos se discriminaron por los siguientes rubros: consultas, exámenes de laboratorio e imágenes, medicamentos, materiales e insumos y estancia hospitalaria. Se utilizaron medidas de tendencia central para resumir la información con sus respectivas medidas de dispersión. Los análisis estadísticos se realizaron en Microsoft Excel 2013®. Resultados: Los 1.576 pacientes, representaron 2.471 atenciones por IRA en la Empresa Social del Estado (ESE) Salud Aquitania, de las cuales el 15,6% fueron en el servicio de urgencias, 81,4% en consulta externa y 3% en el servicio de hospitalización de la ESE Salud Aquitania. El promedio de estancia hospitalaria fue de 2,5 días, (máximo de cinco días). El costo promedio de un paciente con IRA hospitalizado fue $759.437 (RIC$639.132-$879.742); para mujeres fue de $753.879 (RIC$548.862-$877.895) y en hombres de $764.397 (RIC$618.523-$910.271). El rubro que representó un mayor peso en el total del costo por caso hospitalizado fue la estancia hospitalaria, con el 64%. Conclusión: las IRA representan una elevada carga económica al municipio de Aquitania Boyacá. La carga económica para el sistema de salud de la atención de pacientes ambulatorios en Aquitania-Boyacá en 2014 fue de $78.9 millones de pesos y la relacionada con la hospitalización fue de $56.198.338 (IC95% $ 47.295.768 - $65.100.908).


ABSTRACT Objective: To estimate the economic costs related to Acute Respiratory Infections (ARI) in a Colombian municipality. Methods: A retrospective cost-of-illness study was conducted to estimate economic costs due to ARI in Aquitania-Boyacá. The micro-costing (bottom-up) technique was used to estimate the costs associated with ARI in the municipality of Aquitania-Boyacá. Costs were divided by the following items: consultations, laboratory and images, drugs, materials and supplies and the hospital stay. Measures of central tendency were used to summarize the information with their respective dispersion measures. Statistical analyzes were performed in Microsoft Excel 2013®. Results: The 1,576 patients accounted for 2,471 ARI in the Empresa Social del Estado (ESE) Salud Aquitania, of which 15.6% were in the emergency department, 81.4% in the outpatient department and 3% in the hospitalization service. The average length of stay was 2.5 days (maximum of 5 days). The mean cost of a patient with hospitalized ARI was $ 759,437 (ICR $639,132- $ 879,742); for women was $ 753,879 (ICR $548,862- $ 877,895) and for men $ 764,397 (ICR $618,523- $ 910,271). Length of stay was the largest share of total cost per case, with 64%. Conclusion: ARI represented a high economic burden to the municipality of Aquitania-Boyacá. The economic burden for the health care system of outpatient in the municipality in 2014 was $78,9 million pesos and that related to hospitalization was $ 56,198,338 (IC95% $ 47,295,768- $ 65,100,908).

Abstract Domestic turkeys present several color phenotypes controlled by at least five genetic loci, but only one of these has been identified precisely: the bronze locus, which turned out to be the melanocortin-1 receptor (MC1R) gene. MC1R variation is important for breeders interested in maintaining or developing different color varieties. In this study, we sequenced most of the MC1R gene from 16 White Holland (the main commercial turkey variety) and 19 pigmented turkeys from southern Brazil with two purposes. The first was to describe the MC1R diversity in White Holland turkeys, which may serve as reservoirs of genetic diversity at this locus. The second was to test whether the traditional color classification used by Brazilian breeders is related to previously known MC1R alleles. White Holland turkeys had four different haplotypes corresponding to the bronze (b+) and black-winged bronze (b1) alleles. Pigmented turkeys also had four haplotypes corresponding to the b+ and b1 alleles, but different haplotypes represent the most common b+ allele in these two groups. The black (B) allele was absent from our samples. Overall, our results suggest that white and pigmented individuals form two different populations, and that the traditional color classification used by Brazilian breeders cannot accurately predict the genotypes at the bronze locus.

ABSTRACT The aim of this study was to evaluate the possible protective of C. guianensis oil against MMC and CP, which are direct- and indirect-acting chemical mutagens, using the micronucleus test. Three experiments were performed. First the C. guianensis oil was co-administered to mice at doses of 250, 500 and 1000 mg/kg bw with 4 mg/kg bw MMC or 50 mg/kg bw CP. Second, the mutagenic drug (CP) was administered ip 50 mg/kg bw and after 6 and 12 hours 250 and 500 mg/kg bw of C. guianensis oil were administered. In the last, C. guianensis oil was administrated (250 and 500 mg/kg bw) during five days and after it was administered ip 50 mg/kg bw CP. The results obtained showed that the C. guianensis oil is not cytotoxic neither genotoxic to mouse bone marrow. Regarding the antimutagenic effect, all doses of C. guianensis oil were significantly (p < 0.05) effective in reducing the frequency of micronucleated polychromatic erythrocytes, when compared with MMC or CP alone. Based on these results, our results suggest that the C. guianensis oil shows medicinal potential as an antimutagenic agent, modulating the mutagenicity caused by both direct- and indirect-acting chemical mutagens, in a mammalian model.

ABSTRACT

Abstract Soil salinity is a major abiotic stress that results in considerable crop yield losses worldwide. However, some plant genotypes show a high tolerance to soil salinity, as they manage to maintain a high K+/Na+ ratio in the cytosol, in contrast to salt stress susceptible genotypes. Although, different plant genotypes show different salt tolerance mechanisms, they all rely on the regulation and function of K+ and Na+ transporters and H+ pumps, which generate the driving force for K+ and Na+ transport. In this review we will introduce salt stress responses in plants and summarize the current knowledge about the most important ion transporters that facilitate intra- and intercellular K+ and Na+ homeostasis in these organisms. We will describe and discuss the regulation and function of the H+-ATPases, H+-PPases, SOS1, HKTs, and NHXs, including the specific tissues where they work and their response to salt stress.

ABSTRACT

Bryconamericus lethostigmus is the type-species of the monotypic genus Odontostoechus, diagnosed in part based on the presence of a unique tooth series in the premaxilla. Recently a new proposal of classification of the Stevardiinae placed Odontostoechus as a junior synonym of a monophyletic genus Bryconamericus sensu stricto, a genus characterized by the presence of two tooth series. Bryconamericus lethostigmus is redescribed herein and the single tooth series in the premaxilla is demonstrated to originate from merging of the external tooth row with the inner row during ontogeny refuting primary hypothesis of homology between the mouth morphology of B. lethostigmus and the genera Bryconacidnus, Ceratobranchia, Monotocheirodon, Othonocheirodus, Rhinopetitia and Rhinobrycon. A phylogeographic analysis indicated that the pattern described for the sympatric species Diapoma itaimbe is not mirrored by B. lethostigmus. The results also do not support the hypothesis of a new species in the rio Araranguá drainage.(AU)


Bryconamericus lethostigmus é a espécie tipo do gênero monotípico Odontostoechus, diagnosticado em parte pela presença de uma única série de dentes na pré-maxila. Recentemente uma nova proposta de classificação de Stevardiinae considerou Odontostoechus como sinônimo júnior do gênero monofilético Bryconamericus sensu stricto caracterizado pela presença de duas séries de dentes. Bryconamericus lethostigmus é redescrito e demonstra-se que a única série de dentes no premaxilar se origina pela junção da série externa de dentes com a série interna durante a ontogenia, refutando a hipótese de homologia primária entre a morfologia da boca de B. lethostigmus e os gêneros Bryconacidnus, Ceratobranchia, Monotocheirodon, Othonocheirodus, Rhinopetitia e Rhinobrycon. A análise filogeográfica indica que o padrão descrito para a espécie simpátrica Diapoma itaimbe não se repete em B. lethostigmus. Os resultados também não suportam a hipótese de uma nova espécie para a bacia do rio Araranguá.(AU)

ABSTRACT

SOFTEL, es la empresa encargada por el Ministerio de las Comunicaciones de informatizar las entidades de Salud Pública. Para ello se han desarrollado varias aplicaciones, para ser desplegadas en hospitales, clínicas, policlínicos y bancos de sangre, fundamentalmente. Específicamente los laboratorios constituyen un área muy sensible debido a que en ellos se realizan todos los medios diagnósticos, que resultan de gran utilidad el almacenamiento de esta información para futuros estudios de los pacientes. Este trabajo expone la experiencia obtenida en el despliegue de dicho módulo, como una primera experiencia, conocida como despliegue de piloto o prueba beta. Esta experiencia sirve de base para las futuras instalaciones en otros clientes, reportó una serie de inconformidades que se resolvieron antes de concluir el mismo y finalmente quedo concluido el despliegue. Este tuvo complicaciones relacionadas con la existencia de laboratorios fuera del área de cobertura del Instituto y la preparación de las interfaces con los diferentes auto analizadores(AU)


SOFTEL is the enterprise commissioned by the Ministry of Communications to computerize public health institutions. For that has been developed several applications to be deployed in hospitals, clinics and blood banks, basically. Specifically, laboratories are a very sensitive area because in them all the diagnostic tools are used, so it is useful storage of this information for future studies of patients. This paper describes the experience gained in the deployment of the module, as a first experience, known as pilot or deployment of beta testing. This experience, which forms the basis for future installations on other customers, reported a series of disagrees which were resolved before the end of the same and finally the deployment was completed. This had complications related to the existence of laboratories outside the coverage area of the Institute and the preparation of the interfaces with the different analyzers(AU)

ABSTRACT

Este trabalho foi desenvolvido com o objetivo de avaliar a resistência aos ectoparasitas em bovinos jovens da raça Crioula Lageana e meio-sangue Angus em condições naturais. Foram utilizados 10 machos castrados em cada grupo, recém desmamados (6-8 meses) e mantidos sob as mesmas condições de manejo, em pastagens cultivadas de inverno e verão, no município de Monte Castelo - SC, sofrendo infestação natural por ectoparasitas. A cada 28 dias, fêmeas de carrapatos (Rhipicephalus (Boophilus) microplus) com tamanho acima de 4 mm foram quantificadas nos dois lados do corpo de cada animal, bem como os nódulos com larvas de Dermatobia hominis. A espessura da capa do pelame foi também avaliada a cada coleta e os animais foram classificados quanto à coloração do pelame. Os animais da raça Crioula Lageana foram mais resistentes às infestações por D. hominis e R. microplus. Não houve diferença entre a disposição de carrapatos e bernes conforme os lados direito e esquerdo dos animais. Os animais de pelagem escura albergaram a maior porcentagem de ectoparasitas nos grupos avaliados.


This study was carried out to evaluate the breed resistance against ectoparasites infestations in Crioula Lageana and crossbred Angus male calves under natural condition. Ten weaned and neutered male calves (6-8 months) of each group were kept together in winter and summer pastures in Monte Castelo, Santa Catarina State. Every 28 days, female ticks (Rhipicephalus (Boophilus) microplus) more than 4 mm long were counted on the right and left side of the body of each animal, as well as the D. hominis nodules. Coat thickness also was measured at each sampling and all animals were classified according coat color. Crioula Lageana group was less infested by D. hominis than crossbred Angus in five times and by ticks in the infestation peak (P<0.05). There were no differences in the distribution of the ticks and grubs between animal body sides. Dark-haired animal in both groups were the most susceptible.

ABSTRACT

The discovery and characterization of informative intraspecific genetic markers is fundamental for evolutionary and conservation genetics studies. Here, we used nuclear ribosomal ITS sequences to access intraspecific genetic diversity in 23 species of the genus Passiflora L. Some degree of variation was detected in 21 of these. The Passiflora and Decaloba (DC.) Rchb. subgenera showed significant differences in the sizes of the two ITS regions and in GC content, which can be related to reproductive characteristics of species in these subgenera. Furthermore, clear geographical patterns in the spatial distribution of sequence types were identified in six species. The results indicate that ITS may be a useful tool for the evaluation of intraspecific genetic variation in Passiflora.

OBJECTIVES: To study the utilization pattern and the cost of sedatives, analgesics and neuromuscular blocking agents in a multidisciplinary intensive care unit (ICU) METHODS: A prospective observational study was conducted in the ICU of the Eric Williams Medical Sciences Complex, Trinidad and Tobago, for a period of twelve weeks. All patients admitted to the ICU were enrolled. No interventions were done. Data collected included demographics, diagnoses on admission, length of stay in the ICU, status of mechanical ventilation, patient outcome, quantity of sedatives, analgesics and neuromuscular blocking agents used in every patient and their cost. RESULTS: There were 333 patient-days encountered from 34 patients studied. Midazolam, fentanyl and cisatracurium were the most commonly used sedative, opioid and neuromuscular blocking agents respectively. The total cost of drugs used for sedation, analgesia and neuromuscular blockade was approximately US$ 19 600 per annum. Cost for this treatment alone accounted for more than 50% of the total ICU drug costs. The costs were significantly higher in patients who stayed more than two weeks in the ICU when compared to those who stayed less than two weeks (p < 0.001). CONCLUSIONS: The study highlights the utilization pattern and financial burden of sedation, analgesia and neuromuscular blockade in the delivery of critical care.


OBJETIVOS: Estudiar el patrón de utilización y costo de los sedantes, analgésicos, y agentes de bloqueo neuromuscular en una Unidad Multidisciplinaria de Cuidados Intensivos (UCI). MÉTODOS: Se llevó a cabo un estudio prospectivo en la UCI del Complejo de Ciencias Médicas Eric Williams, en Trinidad y Tobago, por un período de doce semanas. Todos los pacientes ingresados a la UCI fueron enrolados. No se realizaron intervenciones. Los datos recogidos incluyeron demografías, diagnósticos de ingreso, longitud de la estadía en la UCI, estatus de ventilación mecánica, evolución del paciente, así como la cantidad y el costo de los sedantes, analgésicos y agentes de bloqueo neuromuscular usados en cada paciente. RESULTADOS: Se encontraron 333 días-pacientes a partir de 34 pacientes estudiados. La midazolama, el fentanil y el cisatracurio fueron los agentes sedativos, opioides y de bloqueo neuromuscular más comúnmente usados. El costo total de los medicamentos usados para la sedación, la analgesia y el bloqueo neuromuscular fue de aproximadamente $19 600 USD por año. El costo sólo para este tratamiento representó más del 50% del total de los costos de medicamentos de la UCI. Los costos fueron significativamente más altos en pacientes que permanecieron más de dos semanas en la UCI, en comparación con aquellos que permanecieron menos de dos semanas (p < 0.001). CONCLUSIONES: El estudio pone de relieve el patrón de utilización de la carga financiera de la sedación, analgesia y bloqueo muscular a la hora de ofrecer atención crítica.

ABSTRACT

To assess the extent of genetic variability of rDNA intergenic spacer (IGS) in Metarhizium sp., 34 strains (27 isolated in Brazil) were sequenced and analyzed together with an additional 20 Metarhizium anisopliae var. anisopliae sequences retrieved from GenBank. Overall, the global nucleotide diversity for the region under study was of 0.090, while for the Brazilian isolates it was only 0.016. Phylogenetic analyses showed four well-supported groups (A, B, C, and D), one of which (D) has not been previously identified. All but one of the Brazilian strains cluster in this novel D phylogroup, suggesting that the genetic variation found in Brazil is a subset of the worldwide M. anisopiliae var. anisopliae variation.
