ABSTRACT

OBJECTIVE To mine the focus and emotional attitude of the public on rare diseases, and to provide decision- making reference for relevant departments to formulate and implement relevant policies, systems and strategies for medical security of rare diseases. METHODS Latent Dirichlet allocation (LDA) topic model and sentiment analysis method were used to analyze the comment text of short videos related to the “nosinasine” medical insurance admission event on the Bilibili video website, mine the theme and sentiment tendency of the text, and put forward relevant strategy suggestions. RESULTS A total of 8 videos with tens of thousands of playback and 7 109 text data were obtained. According to the LDA analysis, online public paid attention to 9 topics related to rare diseases (the price of rare disease drugs, the inclusion of rare disease drugs in medical insurance, the status quo of research and development of rare disease drugs, the scope of medical insurance and the medical security system, the plight of rare disease patients, the prevention and screening of rare diseases, the value of rare disease drugs in medical insurance, the Chinese and western medical methods of rare diseases, and the supply and demand status of rare disease drugs), which could be summarized into 4 categories according to the objects of concern (rare disease drugs, rare disease medical insurance, rare disease medical and health services and rare disease patient groups). On the whole, the emotional tendency of the Internet public towards each topic showed a low positive and high negative tendency. CONCLUSIONS The public paid more attention to the price, research and development, supply and demand of rare disease drugs, and was deeply worried about the current medical status of rare diseases in China. To strengthen and improve the medical security for patients with rare diseases, such as actively carrying out public health services for rare diseases, strengthening cooperation in the diagnosis and treatment of rare diseases, and researching rare disease drugs, etc. Chinese medical and health departments can make concerted efforts in medical and health services, drug supply security, medical security and other social security.

ABSTRACT

OBJECTIVE: To systematically review the efficacy and safety of colesevelam hydrochloride combined with other hypoglycemic drugs in the treatment of T2DM, and to provide evidence-based reference for clinical treatment of T2DM. METHODS: Retrieved from PubMed, Embase, Medline, Cochrane Library, CJFD, VIP and Wanfang database during database establishement-Jul. 2019, randomized controlled trials (RCT) about the efficacy and safety of colesevelam hydrochloride combined with other hypoglycemic drugs (trial group) vs. placebo or other hypoglycemic drugs (control group) in the treatment of T2DM were collected. After extracting data from clinical studies that met the inclusion criteria, the quality of the studies was evaluated by Cochrane System Evaluator Manual 5.1.0. Meta-analysis was conducted by using Rev Man 5.3 statistical software in respects of the levels HbA1c, FPG, LDL-C, the incidence of total ADR, incidence of hypoglycemia and incidence of gastrointestinal ADR. RESULTS: A total of 11 RCTs were included, involving 2 625 patients. Results of Meta-analysis showed that HbA1c levels [MD=-0.37, 95%CI(-0.51, -0.22),P<0.001], FPG level [MD=-0.47, 95%CI(-0.88, -0.07), P=0.02] and LDL-C level [MD=-0.38, 95%CI(-0.49, -0.28), P<0.001] in trial group were significantly lower than control group, with statistical significance. In terms of safety, the incidence of total ADR [OR=1.24, 95%CI(1.06, 1.45), P=0.007] and gastrointestinal ADR [OR=1.78,95%CI(1.05, 3.02),P=0.03] in trial group were significantly higher than control group, with statistical significance. There was no significant difference in the incidence of hypoglycemia [OR=1.03, 95%CI(0.62,1.72),P=0.90]. CONCLUSIONS: Colesevelam hydrochloride combined with other hypoglycemic drugs can effectively reduce the levels of HbA1c, FPG and LDL-C in T2DM patients, but attention should be paid to the occurrence of gastrointestinal ADR.

ABSTRACT

Objective To investigate the relationship between p53,COX-2,Bax,c-myc genes and colorectal carcinoma complicated with chronic schistosomiasis. Methods One hundred and sixty patients with colorectal carcinoma were selected and divided into two groups;a schistosomiasis group(colorectal carcinoma complicated with chronic schistosomiasis,n=80)and a non-schistosomiasis group(colorectal carcinoma uncomplicated with chronic schistosomiasis,n=80). The tissue microarray tech-niques and immunohistochemistry method were used in all the patients to detect the expressions of p53,COX-2,Bax and c-myc proteins. Results The positive rate and level of p53 protein expression in the schistosomiasis group were lower than those in the non-schistosomiasis group, but there were no significant differences between the two groups(both P>0.05). The COX-2 protein in both groups was positive, but the positive expression level of COX-2 in the schistosomiasis group was higher than that in the non-schistosomiasis group, and there was a significant difference between the two groups(P0.05). The positive rate of c-myc expression in the schistosomiasis group was higher than that in the non-schistosomiasis group, with a significant difference(P<0.01),but the positive expression level was lower than that in the non-schistosomiasis group, and there was a significant difference between the two groups(P<0.01). Conclusions Schistosome infection may impact on the deficiency of p53 of human colorectal cancer cells. It may promote the excessive expression of COX-2 protein, which is an indirect carcinogenic factor. The expression of Bax gene has no correlation with schistosome infection. The schistosome chronic infection may cause a persistent low level expression of c-myc gene.
