ABSTRACT

Neoadjuvant chemotherapy (NAC) has shown promising results in patients with locally advanced penile cancer. However, no consensus exists on its applications for locally advanced penile cancer. Thus, it is unclear which kind of chemotherapy regimen is the best choice. Consequently, a systematic search of PubMed, Web of Science, and EMBASE was performed in March 2021 to assess the efficacy and safety of NAC for the treatment of patients with locally advanced penile cancer. The Newcastle-Ottawa Scale was used to assess the risk of bias in each study. This study synthesized 14 published studies. The study revealed that patients who achieved an objective response to NAC obtained a better survival outcome compared with those who did not achieve an objective response. In addition, the objective response rates (ORRs) and pathological complete response (pCR) rates were 0.57 and 0.11, respectively. The incidence of grade ≥3 toxicity was 0.36. Subgroup analysis found that the ORR and pCR of the taxane-platinum (TP) regimen group performed better than those of the nontaxane-platinum (NTP) regimen group (0.57 vs 0.54 and 0.14 vs 0.07, respectively). Moreover, the TP regimen group had more frequent toxicity than the NTP regimen group (0.41 vs 0.26). However, further studies were warranted to confirm the findings.

ABSTRACT

OBJECTIVE@#Ganoderma lucidum spore (GLS) is gaining recognition as a medicinal part of G. lucidum and has been reported to possess various pharmacological properties, such as antitumor activity. In this work, wall-broken GLS powder (BGLSP) and wall-removed GLS powder (RGLSP), two kinds of GLS powder with different manufacturing techniques, were compared in terms of contents of active constituents and in vivo and in vitro antitumor effects.@*METHODS@#The ultraviolet and visible spectrophotometry method was used to determine the contents of polysaccharides and total triterpenoids in BGLSP and RGLSP. Seventeen individual triterpenoids were further quantified using ultra-high-performance liquid chromatography and quantitative analysis of multi-components by single marker. The antitumor effects of BGLSP and RGLSP were evaluated using in vitro cell viability assay against human gastric carcinoma SGC-7901, lung carcinoma A549 and lymphoma Ramos and further validated by in vivo zebrafish xenograft models with transplanted SGC-7901, A549 and Ramos.@*RESULTS@#The results showed that the contents of polysaccharides, total triterpenoids and individual triterpenoids of RGLSP were significantly higher than those of BGLSP. Although both BGLSP and RGLSP inhibited the three tumor cell lines in vitro in a dose-dependent manner, the inhibitory effects of RGLSP were much better than those of BGLSP. In the in vivo zebrafish assay, RGLSP exhibited more potent inhibitory activities against tumors transplanted into the zebrafish compared with BGLSP, and the inhibition rates of RGLSP reached approximately 78%, 31% and 83% on SGC-7901, A549 and Ramos, respectively.@*CONCLUSION@#The results indicated that the antitumor effects of GLS were positively correlated with the contents of the polysaccharides and triterpenoids and demonstrated that the wall-removing manufacturing technique could significantly improve the levels of active constituents, and thereby enhance the antitumor activity.

ABSTRACT

OBJECTIVE@#To study the therapeutic effects of the ultramicro needle-knife combine with cervical spine fine adjusting on youth cervical curvature abnormality case.@*METHODS@#From November 2016 to October 2018, 88 young patients with abnormal curvature of cervical spine were treated. Due to loss of follow up, 86 cases were actually completely including 37 males and 49 females, ranging in age from 20 to 40 years old, with an average of (30.55±5.21) years old, and the course of disease ranged from 1 to 42 months, with a mean of (14.21±7.38) months. All the patients were divided into two groups:treatment group (44 cases) and control group (42 cases). The patients in the treatment group were treated with ultramicro needle-knife and cervical spine fine adjusting, and the patients in the control group were treated with conventional acupuncture and manipulation. The treatments were done 1 time per week in the treatment group while 3 times per week in control group every week, with a duration of 3 weeks for both groups. Before treatment, 3 weeks after treatment, and at the end of 1 month follow-up, the score of neck pain questionnaire(NPQ), range of the motion(ROM) in the cervical region and the D values of cervical physiological curvature were recorded. The efficacy at the end of treatment and in the follow up was evaluated. During the treatment, the patients were also required to correct the bad posture in daily life, to sleep in a low pillow position, and put a moderately columnar pillow behind the neck for 0.5 hours every morning and evening.@*RESULTS@#Two patients in the control group were dropped out after 3 weeks treatment. No adverse reactions were found in the 2 groups during the treatment period. Compared with those before treatment, all scores at all the observation time points were significantly improved between two groups after treatment(all 0.05). There was statistical difference in ROM scores between different time points, there was time effect (=240.32), =0.000). There was no interaction between time factor and group factor (=0.311, =0.734). The effective rate of the treatment group and control group were 90.91%(40 / 44) and 80.95%(34 / 42) respectively, the treatment group was more effective than the control group (0.05) in the follow-up duration.@*CONCLUSION@#The method of needle knife combined with cervical spine fine adjusting has a better therapeutic efficiency than conventional acupuncturecombined with manipulation in treating youth cervical curvature abnormality patients. Because this novel method can recover the cervical curvature, relieve the neck pain, and improve cervical mobility.

ABSTRACT

Endoscopic resection is becoming the preferred primary treatment for early gastric cancer with a low incidence of lymph node metastasis. There have been published guidelines and consensus on the standardized endoscopic diagnosis and treatment in recent years. As part of the standardized endoscopic process, risk stratification regarding gastric cancer should be performed prior to gastroscopy. This is followed by a systematic gastroscopic mapping. The pre-malignant mucosal changes should be noted during screening. Chromoendoscopy and Image-Enhanced Endoscopy are recommended to assist the preoperative diagnosis for any suspicious lesion. Indication for endoscopic resection is according to the pathologic findings post biopsy. There are two main goals of a successful endoscopic resection: en bloc resection and negative margins. The final pathological diagnosis should be comprehensive and systematic. Follow-up surveillance or additional surgery should be based on the eCure system.

ABSTRACT

Objective To establish a simple and practical regression equation for estimating maximal oxygen uptake(VO2max),so as to provide a base and reference for epidemiological studies. Methods Totally 128 freshmen(88 males and 40 females) were randomly recruited majoring in social sports and physical education to conduct the correlation analysis between VO2max and the weight, body surface area, body mass index(BMI) and vital capacity. Then 34 freshmen(17 males and 17 females) were chosen from them randomly to compare whether there was a significant difference between the two VO2max relative values measured by Astrand estimation and direct measurement respectively.Based on the results, the multiple linear regression analysis was conducted:independent variables entered theregression equation in turn according to the Forward method and the significance test of the regression equation, the regression coefficients and the fitting degree was conducted. We got the regression equation finally according to the Durbin-Watson and VIF values.Another 17 male freshmen were recruited to verify the efficient of the regression equation. Results Significant differences were observed in the relative VO2max value of females, being 45.14 ± 8.08 ml/kg/min and 32.63 ± 5.87 ml/kg/min by Astrand and direct measuring respectively, but not in that of the male, being 47.24 ± 6.46 ml/kg/min and 51.62 ± 6.91 ml/kg/min accordingly. There was very significant correlation between VO2max and weight, body surface area,BMI and vital capacity of the male freshmen(P<0.01),but not of the female freshmen (P>0.05).The regression equation of estimating the relative VO2max of the boy freshmen was y=34.812+0.015×vital capacity(ml)-40.403×body surface area(m2).No significant differences were found between the value calculated using this equation and that measured directly (P>0.05).Conclusions The regression equation found in this study is simple, practical and efficient, and it can be used to estimate the relative VO2max of male freshmen majoring in physical education and social sports in epidemiologic study,so as to evaluate their cardiopulmonary fitness.

ABSTRACT

AIM:To explore the expression level of tristetraprolin(TTP)in rats after subarachnoid hemor-rhage(SAH)as well as the potential role of TTP in the early brain injury(EBI)after SAH in rats.METHODS:In the first experiment setting, total 56 adult male SD rats were randomly divided into sham group and SAH group. The SAH mod-el was performed by endovascular perforation. The brain tissues were taken out after SAH at 5 different time points(0,12, 24,48,72 h and 1 week). The expression of TTP in the brain tissues was detected by Western blot.In the second experi-ment,a total of 60 SD rats were divided into 4 groups: sham group, SAH group, SAH +vector group and SAH +TPP group.Neurological score, brain water content and blood-brain barrier were evaluated at 48 h after SAH.TUNEL staining was performed to detect cell apoptosis in the rat brain tissue.ELISA method was used for quantitative detection of interleu-kin-6(IL-6)and tumor necrosis factor-α(TNF-α).The protein levels of TTP,Bax,Bcl-2 and cleaved caspase-3 in the rat brain tissue were detected by Western blot.RESULTS:The protein expression of TTP in the brain was downregulated markedly from 12 h after SAH,reached the lowest level at 48 h,and then had an upward trend. After modeling for 48 h, Garcia neurological score was significantly reduced, and brain water content and Evans blue(EB)content of the brain tis-sue of the rats in SAH group were significantly higher than those in sham group(P<0.05).SAH induced significant in-creases in IL-6 and TNF-αlevels in the brain tissue(P<0.05).The number of TUNEL-stained cells was increased in the subcortical brain region after SAH compared with sham group. In addition,a lower level of Bcl-2 and higher levels of Bax and cleaved caspase-3 in the rat brains were observed at 48 h after SAH.However, the neurological deficit score was signif-icantly increased, and the brain water content and EB content in the rat brains were significantly reduced in SAH +TTP group in comparison with SAH +vector group(P<0.05).Over-expression of TTP dramatically suppressed the levels of IL-6 and TNF-αin the rat brains, and reduced the number of TUNEL positive cells.Furthermore, upregulation of TTP signifi-cantly decreased the levels of cleaved caspase-3 and Bax, and evidently enhanced the expression of Bcl-2(P<0.01). CONCLUSION:The expression of TTP is significantly decreased in early period after SAH, and enhancing the level of TTP effectively inhibits EBI following SAH in rats.

ABSTRACT

BACKGROUND: Stem cells are the potentially immortal cells capable of self-renewal, which are essential to the mystery of human development and aging, and are also the core of research for regenerative medicine. OBJECTIVE: To summarize the biological characteristics of embryonic stem cells, adult stem cells and induced pluripotent stem cells, to review the clinical and commercial applications in stem cell therapy and drug screening, and to analyze the problems and prospects in stem cell industry. METHODS: We searched relevant articles about stem cell models in PubMed and CNKI databases during 1995 to 2017 on internet, and took"stem cells, embryonic stem cells, adult stem cells, induced pluripotent stem cells, stem cell therapy, drug screening" as the keywords in English and Chinese, respectively. RESULTS AND CONCLUSION: According to the origin, stem cell models are divided into three types: embryonic stem cells, adult stem cells and induced pluripotent stem cells. Different types of stem cells have their unique biological advantages. Embryonic stem cells can generate all somatic cell types, but the application is limited by ethical disputes. As for adult stem cells, there are the most extensive and in-depth, studies as the well as the most prevalent and mature applications. Induced pluripotent stem cells have similar characteristics as embryonic stem cells, and furthermore their use avoids source restriction, moral and ethical controversies, bringing new opportunities for stem cell application. Stem cell-based cell therapy has shown successful achievement. There have been a few commercial products about adult stem cells-based cell therapy; in the meanwhile both embryonic stem cells and induced pluripotent stem cells are making their way into clinical trials. In addition, pluripotent stem cells hold great promise for the specific drug screening because they enable scientists to establish a variety of cell and disease models in vitro.

ABSTRACT

BACKGROUND:Due to the differences of sport events and individual metabolic characteristics of athletes, it is difficult to establish uniform biological indexes for training monitoring. Current individual evaluation is a longitudinal analysis relying on experience or numerical variation width, which is less objective.OBJECTIVE: To explore the individual function assessment and monitoring by monitoring blood biochemical indexes and brain function indexes in elite gymnasts, in order to accurately reflect the body changes resulting from training loads. METHODS:Thirty gymnasts from the Chinese national gymnastics team preparing for London Olympic Games were enroled in this study and monitored from the last winter training until the London Olympic Games. The blood biochemical indicators and brain function indicators were measured and assessed individually according to according to the principle of quality control (alert value=mean±SD, and controled variable=mean±2SD). RESULTS AND CONCLUSION:Under the relatively uniform test conditions, it is feasible to individualy assess the blood biochemical and brain function indexes of elite gymnasts in accordance with the principle of quality control, which is more accurate and objective to reflect the body changes under training load and the current state of athletes. In addition, the combined monitoring of blood biochemical indexes and brain function indexes is more comprehensive to evaluate the body function and status of elite gymnasts.

ABSTRACT

Worldwide, peroral endoscopic myotomy (POEM) has achieved remarkable initial outcomes in the treatment of achalasia. In China, POEM has developed very quickly since the first case was performed in our center in August 2010. With experience, we have successfully performed POEM for special cases (such as pediatric patients, patients with sigmoid-type esophagus, and patients with recurrent symptoms after previous surgery) and have altered our technique to achieve long-term symptom remission and simplify the POEM procedure. These changes include posterior wall incision, full-thickness myotomy, a "push-and-pull" technique for myotomy, and water-jet assisted POEM. In this article, our experiences in POEM are summarized, including changes in technique, applications of the procedure, and the management of possible complications.

ABSTRACT

Ac-Phe-Lys-PABC-DOX (PDOX) is a smart doxorubicin (DOX) prodrug designed to decrease toxicities while maintaining the potent anticancer effects of DOX. This study was aimed at elucidating the effectiveness and toxicities of DOX and PDOX in patient-derived MCF-7 breast cancer cells in vitro. The MCF-7 cells were exposed to both PDOX and DOX, and cytotoxicities, cell cycle and P53/P21 signaling alterations were studied. Abundant cathepsin B was found in the MCF-7 cells, and treatment with PDOX and DOX triggered dose- and time-dependent cytotoxicity and resulted in a significant reduction in cell viability. The IC50 of PDOX and DOX was 3.91 and 0.94 μmol/L, respectively. Both PDOX and DOX caused an up-regulation of the P53/P21-related signal pathway, and PDOX significantly increased expression of P53 and caspase 3, and arrested the cell cycle at the G1/G2 phase. As compared with DOX, PDOX reduced toxicities, and it may have different action mechanisms on breast cancer cells.

ABSTRACT

Objective: To establish a RP-HPLC method for the simultaneous determination of 13 amino acids in Asini Corii Colla. Methods: Asini Corii Colla was hydrolyzed for 3 h at 120°C using 6 mol/L hydrochloric acid, and the amino acids were determined by RP-HPLC after being derived with phenyl isothiocyanate. HPLC was performed on an Eclipse AAA column (75 mm × 4.6 mm, 3.5 μm) with acetonitrile-110 mmol/L ammonium acetate solution (1:99) as mobile phase A and acetic acid-acetonitrile-water (0.05:80:20) as mobile phase B in the gradient mode at a flow rate of 0.6 mL/min. The column temperature was 42°C, and the detection wavelength was set at 254 nm. Results: The 13 amino acids were separated within 36 min with a good linearity (r > 0.9977) in the range of the test concentration. The average recoveries (n = 3) were 95.2%-104.4% and the RSD values were less than 5.0%. Conclusion: The method is simple, accurate, and repeatable, which could be available for the quality control of Asini Corii Colla. It may also serve as a good reference for the determination of amino acids in other pharmaceuticals.

<p><b>OBJECTIVE</b>To compare the clinical value of narrow band imaging (NBI) and iodine staining for margin determination of early esophageal cancer during endoscopic submucosal dissection (ESD).</p><p><b>METHODS</b>Clinical data of 87 patients with early esophageal cancers undergoing endoscopic submucosal dissection (ESD) were analyzed retrospectively. Patients were assigned to NBI group and iodine staining group according to the staining method before ESD operation. Clinicopathological features, esophageal spasm ratio, operation time, en bloc resection rate, complications, local recurrence, and distant metastases were compared between the two groups.</p><p><b>RESULTS</b>There were 37 patients in NBI group while 50 patients in iodine staining group. Location and size of the lesions between two groups were not significantly different. The ratio of moderate-severe esophageal spasm in NBI group was significantly lower as compared to iodine staining group [10.8%(4/37) vs. 32.0%(16/50), P<0.05]. The average operation time in NBI group was significantly shorter than that in iodine staining group [(42.2±19.5) min vs. (53.3±30.9) min, P<0.05). All the tumors were resected in an en bloc fashion and the R0 resection rate was 100%. Perforations in 2 patients and delayed bleeding in 1 patient were successfully treated by endoscopic methods. Esophageal strictures occurred in 3 patients of NBI group and 4 patients of iodine staining group, who were treated by endoscopic dilation and retrievable stents. During mean 13.2 months (range 4 to 20 months) follow-up periods, local recurrence occurred in 2 patients of NBI group and 2 patients of iodine staining group. These patients received ESD or other surgery.</p><p><b>CONCLUSION</b>Compared with iodine staining, using NBI for margin determination of early esophageal cancer during ESD is more convenient and fast because of distinctly lower degree of esophageal spasm.</p>
