ABSTRACT

Objective:To investigate the imaging features of pancreatic mucinous cystic tumor (MCN) based on the European evidence-based guidelines on pancreatic cystic neoplasms and risk factors influencing tumor property.Methods:The retrospective case-control study was con-ducted. The clinicopathological data of 109 pancreatic MCN patients who were admitted to the First Affiliated Hospital of Naval Medical University (Changhai Hospital of Shanghai) from March 2011 to April 2021 were collected. There were 5 males and 104 females, aged (49±15)years. There were 97 cases with benign tumors and 12 cases with malignant tumors. Observation indicators: (1) clinical characteristics of MCN patients with different tumor properties; (2) imaging features of MCN patients with different tumor properties; (3) multivariate analysis of factors affecting evaluating tumor pro-perties of MCN. Measurement data with normal distribution were represented as Mean± SD, and comparison between groups was conducted using the t test. Measurement data with skewed distri-bution were represented as M(range), and comparison between groups was conducted using the Mann-Whitney U test. Count data were described as absolute numbers, and comparison between groups was performed using the chi-square test or Fisher exact probability. Comparison of ordinal data was analyzed using the non-parameter rank sum test. Statistically significant indicators in clinical and imaging characteristics were included in multivariate analysis. Multivariate analysis was performed by the Logistic regression model forward method. Results:(1) Clinical characteristics of MCN patients with different tumor properties. Gender(male, female), age, body mass index (BMI), cases with clinical symptoms (asymptomatically physical findings, abdominal pain and distension, emaciation, jaundice, pancreatitis, onset diabetes), cases with CA19-9 (<37 U/mL, ≥37 U/mL), cases with carcinoembryonic antigen (<5.0 μg/L, ≥5.0 μg/L), cases with surgical methods (pancreatoduo-denectomy, pancreatectomy of body and tail, segmental pancreatectomy), cases with tumor location (head of pancreas, tail of pancreas) were 4, 93, (47±14)years, (22±3)kg/m 2, 56, 35, 2, 1, 11, 5, 89, 8, 96, 1, 2, 90, 5, 4, 93 in the 97 cases with benign tumors, versus 1, 11, (59±17)years, (23±3)kg/m 2, 4, 4, 1, 0, 3, 2, 5, 7, 7, 5, 0, 12, 0, 0, 12 in the 12 cases with malignant tumors, showing significant differences in age, CA19-9 and carcinoembryonic antigen ( t=?2.69, χ2=22.57, 26.54, P<0.05) and showing no significant difference in gender, BMI, clinical symptoms, surgical methods and tumor location ( P>0.05) between them. (2) Imaging features of MCN patients with different tumor pro-perties. Of the 109 patients with pancreatic MCN, 85 cases underwent computed tomography (CT) plain and contrast-enhanced scan of pancreas, and 81 cases underwent magnetic resonance imaging (MRI) plain and contrast-enhanced scan of pancreas. There were 57 cases underwent both CT and MRI plain and contrast-enhanced scan of pancreas. Cases with tumor location (head of pancreas, tail of pancreas), cases with cyst morphology (circular, lobulated), cases with cyst diameter (<4 cm, ≥4 cm), diameter of cyst, cases with thickening of capsule wall, cases with calcification of capsule wall, cases with enhancing mural nodule of capsule wall, cases with pancreatic duct dilatation were 4, 93, 69, 28, 32, 65, 4.7(range, 3.3?6.8)cm, 38,20, 4, 13 in the 97 cases with benign tumors, versus 0, 12, 7, 5, 4, 8, 6.8(range, 3.3.?9.6)cm, 10, 2, 6, 4 in the 12 cases with malignant tumors, showing significant differences in thickening of capsule wall and enhancing mural nodule of capsule wall ( χ2=6.75, 21.75, P<0.05) and showing no significant difference in cyst morphology, cyst diameter, diameter of cyst, calcification of capsule wall and pancreatic duct dilatation ( P>0.05) between them. (3) Multivariate analysis of factors affecting evaluating tumor properties of pancreatic MCN. Result of multivariate analysis showed that age, carcinoembryonic antigen and mural nodule of capsule wall were independent factors affecting tumor properties of MCN ( odds Ratio=1.09, 19.67, 63.57, 95% confidence intervals as 1.01?1.18, 1.07?361.49, 4.07?993.49, P<0.05). Conclusions:Thickening of capsule wall and enhancing mural nodule of capsule wall are imaging features of patients with pancreatic MCN. Age, carcinoembryonic antigen and mural nodule of capsule wall are independent factors affecting tumor properties of pancreatic MCN.

ABSTRACT

Objective:To explore the effect of hyperbaric oxygen (HBO) on the blood-brain barrier via the silent information regulator 1 (SIRT1)/Forkhead box O1(FoxO1) signaling pathway after cerebral ischemia and reperfusion using a rat model.Methods:Forty Wistar rats were randomly assigned into sham, cerebral ischemia-reperfusion (CIR), CIR+ HBO and CIR+ HBO+ EX527 groups, each of 10. The cerebral ischemia-reperfusion model was established in all groups except the sham group by right middle cerebral artery occlusion using the modified thread-occlusion method. The sham group was not ligated. Both the CIR+ HBO and CIR+ HBO+ EX527 groups were given HBO 1, 9, 21, 45 and 69 hours after the reperfusion. The CIR+ HBO+ EX527 group was additionally injected with 5mg/kg of EX527(a SIRT1inhibitor) peritoneally 4, 12, 24, 48 and 72 hours after the reperfusion. Then 2% Evens blue (EB) was injected into the tail vein an hour before the rats were sacrificed. The content of EB and the expression of SIRT1, FoxO1, ZO-1, Occludin, Claudin-5 mRNA and their proteins were determined using spectrophotometry, reverse transcription-polymerase chain reactions and Western blotting.Results:The average EB content of the hippocampal brain tissue from the CIR, CIR+ HBO and CIR+ HBO+ EX527 rats was significantly greater than the Sham group′s average 72h after reperfusion. The average expression of SIRT1, FoxO1, ZO-1, Occludin and Claudin-5 mRNA and their proteins was significantly lower, with the CIR + HBO + EX 527 group′s average significantly lower than that of the CIR+ HBO group.Conclusions:HBO can increase the expression of tight junction protein via the SIRT1/FoxO1 pathway. It helps to protect the blood-brain barrier in CIR injury situations.

ABSTRACT

Objective:To explore the whole genome sequencing of carbapenem resistant Klebsiella pneumoniae (CRKP), reveal the prevalence of drug-resistance in CRKP, and guide the prevention and control of nosocomial infection. Methods:The CRKP isolates in Bengbu Third People′s Hospital from November 2018 to March 2019 was collected, and the strain identification and drug susceptibility testing were carried out using VITEK-2 Compact analyzer. The whole genome of selected CRKP was sequenced by Illumina Hiseq 2500 platform, spliced with spades-2.0 sequence, analyzed and extracted the core gene and drug-resistant genotype by cgMLST, and constructed the bacterial association map and transmission map by Scotti software.Results:Among the 19 CRKP strains, 13 were CT3176 genotype, 2 were CT1313 and 2 were CT1689, and 2 were other CRKP strains. CT3176 was distributed mainly in the Department of critical care medicine and was in epidemic status. The strains mainly carried blaKPC-2 (19/19), blaCTX-M-65 (19/19), blaSHV-11 (16/19), blaTEM-1B (14/19), and aminoglycosides. Among them, aadA2 (14/19), rmtB (14/19), AAC(3)-Ⅱd (12/19), armA (11/19), mph (E) (11/19), msr (E) (11/19), mph (A) (10/19), fosA6 (19/19) and other resistant genes were detected. Software analysis showed that the strains numbered 7 and 29, and strains numbered 12 and 17 had 42% and 37% transmission rate repectively, but that of the other strains remain low.Conclusions:By using the whole genome sequencing of microorganisms, strain typing, drug resistance genes, and the homology of strains can be determined, strain association map and transmission map can be built. That can effectively guide the development of nosocomial infection prevention and control in hospitals.

ABSTRACT

Objective To explore the risk factors of urate deposition on ultrasound in patients with gout.Methods All the gout patients who visited our center between February 2015 and February 2017 and underwent ultrasound examination of bilateral knees,ankles and first metatarsophalangeal joints (MTP1) were enrolled.Subgroup analysis was done depending on whether double contour sign (DCS) or tophus was found on ultrasound.Main statistical analysis methods were t test,chi-square test and logistic regression model.Results One hundred and twenty-six patients were included.DCS was found in 50(39.7%) patients and tophus was found in 48 (38.1%) patients.The serum uric acid (SU) level of the DCS positive group was signi-ficantly higher than the DCS negative group [(602±79) μmol/L vs (538±101) μmol/L,t=3.998,P=0.044].The hyperuicemia duration of the two groups were (186±87) months and (130±77) months,which was significantly different (t=3.330,P=0.002).The hyperuicemia duration of the tophus positive group was significantly higher than tophus negative group [(175±102) months vs (138±96) months,t=2.003,P=0.045].The SU level and hyperuicemia duration were independent risk factors of positive DCS in gout patients [OR =1.006,95% CI (1.002,1.01 1);OR=1.028,95%CI (1.013,1.042)].The hyperuicemia duration was independent risk factor of positive tophus in gout patients [OR=1.004,95%CI (1.000,1.007)].Receiver operating characteristic curve (ROC) curve showed gout patient whose hyperuricemia duration was longer than 94months and SU level was higher than 505.5 μmol/L were more likely to have positive DCS in joints;meanwhile,patient whose hyperuricemia duration was longer than 137 months were more likely to have positive tophus in joints.Conclusion Gout patients who have positive DCS and tophus on ultrasound have longer hyperuicemia duration.Positive DCS is also related with patients' higher serum levels.The hyperuicemia duration is an independent risk factor of urate deposition on ultrasound in patients with gout.

ABSTRACT

Stem cell research has become a frontier in the field of life sciences, and provides an ideal model for exploring developmental biology problems such as embryogenesis, histiocytosis, and gene expression regulation, as well as opens up new doors for clinical tissue defective and inheritance diseases. Among them, menstrual blood-derived stem cells (MenSCs) are characterized by wide source, multi-directional differentiation potential, low immune rejection characteristics. Thus, MenSCs can achieve individual treatment and have the most advantage of the clinical application. The central nervous system, including brain and spinal cord, is susceptible to injury. And lethality and morbidity of them tops the list of all types of trauma. Compared to peripheral nervous system, recovery of central nervous system after damage remains extremely hard. However, the treatment of stem cells, especially MenSCs, is expected to solve this problem. Therefore, biological characteristics of MenSCs and their treatment in the respect of central nervous system diseases have been reviewed at home and abroad in recent years, so as to provide reference for the treatment of central nervous system diseases.

ABSTRACT

Background The deposition of advanced glycation end products (AGEs) in lens is the risk factor of diabetic complications.Researches revealed that AGEs has autofluorescence.Crystallin is a longevity protein.AGEs accumulation is probably associated with diabetic retinopathy (DR).Objective This study was to evaluate the association of AGEs autofluorescence intensity with diabetes and with DR.Methods A cross-sectional study was carried out under the approval of Ethic Committee of Shenyang He Eye Hospital and informed consent of each patient.One hundred eyes of 100 patients with age-related cataract aged 50-70 years were included in He Eye Hospital from September to December 2015.The patients were divided into non-diabetes group (40 patients) and diabetes group (60 patients),and then the patients in diabetes group were subdivided into non-DR (NDR) group,non-proliferating DR (NPDR) group and proliferating DR (PDR) group according to the DR grading criteria,20 patients for each.Glycosylated henoglobin A1c (HbA1c) and fasting plasma glucose (FPG) were detected for each subject,and the lens autofluorescence was assayed with lens fluorescence biomicroscope (Clearpath DS120).The association of lens autofluorescence intensity with serum HbA1c level or DR severity was analyzed.Results The age and diabetes course were matched among the non-diabetes group,NDR group,NPDR group and PDR group (F=2.587,2.899,both at P>0.05),and the FBS and HbA1c level were evidently higher in the NDR group,NPDR group and PDR group than those in the non-diabetes group (all at P<0.01).The autofluorescence intensity of lens was (0.159±0.032),(0.256±0.024),(0.319 ±0.013) and (0.394±0.035) cd in the non-diabetes group,NDR group,NPDR group and PDR group,respectively,showing a significant difference among the groups (F =90.265,P =0.000).The autofluorescence intensity of lens in the NDR group,NPDR group and PDR group was significantly increased in comparison with the non-diabetes group and the autofluorescence intensity of lens was gradually increased with the severity of DR (all at P<0.01).A positive linear correlation was found between autofluorescence intensity of lens and serum HbA1 c level in diabetes patients (r =0.654,P < 0.05).Conclusions The autofluorescence intensity of AGEs in lens appears to be associated with the severity of DR and HbA1 c.The autofluorescence intensity of AGEs in the lens of diabetes patient is probably one of the evaluation indexes of early stage of DR.

Methods: A Chiralpak ASH (250 mm ×4. 6 mm, 5 μm) column was used with the mobile phase of n-hexane-ethanol-diethylamine (75 ∶ 25 ∶ 0. 1). The flow rate was 1. 0 ml·min-1 . The wavelength was set at 225 nm. The column temperature was 35℃. Results: The resolution of safinamide mesilate and the enantiomer was above 2. 0. The linear range of them was 1. 007-2 517. 500 μg·ml-1 and 0. 909-2273. 200 μg·ml-1 ,respectively(r = 0. 999 0). The average recovery of the enantionmer was 104. 9% with RSD of 2. 3% (n = 9). Conclusion: The method is accurate and rapid, and suitable for the determination of the enantiomer in safinamide mesilate.

Conclusion:The method is accurate and rapid, and suit-able for the determination of the enantiomer in rivaroxaban.

ABSTRACT

Objective To observe the treatment difference of using acupiont application alone, pricking Sifeng (EX-UE10) alone, and combination of the two therapies in treating infantile chronic cough. Method Ninety patients were randomized into 3 groups (acupoint application group, pricking Sifeng group, and a combining group), 30 in each group. Same points were selected for the three groups:Feishu (BL13), Pishu (BL20), Shenshu (BL23), and Sifeng (EX-UE 10). By follow-up research of the symptom scale, the therapeutic efficacies were compared. Result The total effective rate was 83.3%in the acupoint application group, 80.0%in the pricking Sifeng (EX-UE 10) group, and 93.3%in the combining group;meanwhile, the symptom scores were significantly changed after intervention in all three groups (P<0.05). Conclusion Separate use and combining use of acupoint application and pricking Sifeng (EX-UE 10) all can improve the condition of infantile chronic cough, while the combining use can produce a significantly higher therapeutic efficacy, thus it’s a safe, effective, and easy-to-operate external treatment in traditional Chinese medicine.

ABSTRACT

Objective To explore the effect of weightlessness simulation on processing speed of mental rotation of males in 72 h head-down tilt,in order to further reveal human performance changes caused by weightlessness.Methods During 72 h weightlessness simulation (9 am day 1 to 9 pm day 4,head-down tilt started from 9 pm day 1),16 male subjects were processed speed of rotation data byhand picture mental rotation (test 8 times),using self cross-references design,and the variation of processing speed of mental rotation by duration was analyzed.Results The linear regression analysis showed,the reaction time of each time point for each subject as the independent variable,the angle of rotation as the dependent variable,with the extension of simulated weightlessness time.The intercept(constant term of the regression equation),indicating the processing speed of non-rotation,were gradually increased with HDT time,but didn(t) reached significantly difference (F(3,14) =0.551,P =0.650) ;the slope(Regression coefficient of the regression equation),indicating the processing speed of rotation.were gradually increased with HDT time,and reached the significantly difference (F(3,14) =4.338,P =0.009).The value of intercept and slope was like upside-down U in three days,and reached the highest value at day 2.Conclusion Weightlessness simulation have affection on the speed of mental rotation,especially on the rotation speed.

ABSTRACT

Objective To construct small hairpin RNA (shRNA) expression plasmid targeting rat opticin gene. Methods Four pairs of opticin oligonucleotides were synthesized and inserted into the plasmid vector, resulting into four plasmids: shRNA-1, shRNA-2, shRNA-3 and shRNA-4. Then the four constructed shRNA expression vectors and empty vector were transfected into rat ciliary non-pigment epithelium (NPE) cells by lipofectmaine 2000. Non-transfected NPE cells were set as control group. The expression of opticin mRNA and protein were measured by Reverse transcriptase polymerase chain reaction (RT-PCR) and Western blot respectively. Results The opticin mRNA expression of the shRNA-1,shRNA-2, shRNA-3, shRNA-4 group were decreased compared with the control group (F = 10. 239, P = 0. 000);the inhibitory rate were 85.7% ,62. 87% ,54.87% and 48.77% respectively. The opticin protein expression of the shRNA-1,shRNA-2, shRNA-3, shRNA-4 group were also decreased compared with the control group (F=17.870, P= 0.000);the inhibitory rate were 78.7%, 34.6%, 31.1% and 16.8% respectively.Conclusions The shRNA-1 expression plasmid has most potent inhibitory effect on opticin expression in rat ciliary NPE cells.

ABSTRACT

Objective To investigate the effects of hyperbaric oxygen (HBO) on the expression of caveolin2 and matrix-metalloproteinase-9 (MMP-9) in brain tissues and the blood brain barrier (BBB) after cerebral ischemia and reperfusion (I/R) in rats. Methods Three hundred and seventy male Wistar rats were randomly assigned into sham,ischemia-reperfusion,HBO,and I/R + HBO groups. After creating cerebral I/R models,oxygen at 0.25 MPa was administered 5 times,and 2% Evans blue (EB) was injected into the tail veins 1 h before the rats were sacrificed.The permeability of the BBB,the expression of caveolin-2 and MMP-9,and EB content were determined by Western blotting,immunohistochemistry,and spectrophotometery,respectively. Results In the I/R group,the EB content increased steadily to a peak at 4 hours.EB content in the IR + HBO group was significantly lower than in the I/R group.Caveolin-2 and MMP-9 were significantly augmented by I/R injury at the 24th,48th and 72nd hours.Compared to the I/R group,HBO intervention decreased their expression levels. Conclusion HBO intervention can reverse the increase of caveolin-2 and MMP-9 caused by I/R injury,which suggests a mechanism for protective effects of HBO on the permeability of the BBB in I/R injury.

ABSTRACT

Objective: To observe the clinical effect of acupuncture treatment for infantile autism. Methods: After being treated with the needling technique for enhancing wisdom and opening the orifice, 32 cases of sick children were assessed by Autism Treatment Evaluation Checklist (ATEC) depending upon their clinical symptoms. Results: After acupuncture treatments, language communication, interpersonal relationships, movements and behavior, and personal ability had all improved. Among them, the remarkable rate of language communication showed a statistical significance in comparison with the other three items (P<0.05). In the comparison of the ATEC scores before and after the treatments, the language items (P<0.01), social relationships, perception and behavior items, showed statistically significant differences (P<0.05). Conclusion: Acupuncture therapy can effectively improve the clinical symptoms of sick children with autism, and the interference effect on language function is especially remarkable.

ABSTRACT

OBJECTIVE:To study the separation and purification of polysaccharides sulfate from Ulva lactuca,and to analyze its structure.METHODS:The separation and purification method of polysaccharides sulfate from U.lactuca was optimized.The content of polysaccharides in polysaccharides sulfate was determined by phenyl-sulfuric acid method.The molecular amount of polysaccharides was determined by HPGC.The structure of polysaccharides was analyzed by IR and 13C-NMR.RESULTS:The optimal extraction condition of polysaccharides sulfate from U.lactuca were crashed materials 20 mesh sieve,20-fold water,decocting 3 times at 85 ℃,2 hours each time.The content of polysaccharides was 22.64% and molecular amount was 1 156 112.Soluble polysaccharides mainly contained ?-glucuronic acid,rhamnose and plenty of sulfate radical.CONCLUSION:The method is simple and stable,which provides theoretic basis for production of polysaccharides sulfate from U.lactuca.
