ABSTRACT

Objective:To study the clinical characteristics and differences of severe hyperbilirubinemia caused by hemolytic disease of the newborn (HDN) and glucose-6-phosphate dehydrogenase (G6PD) deficiency.Methods:From January 2014 to December 2021, newborns (gestational age ≥ 35 weeks and postnatal age ≤ 28 d) admitted to the Department of Neonatology of Hunan Children's Hospital with severe hyperbilirubinemia caused by HDN or G6PD deficiency were retrospectively analyzed. According to the etiology of hyperbilirubinemia, they were assigned into HDN group and G6PD deficiency group. The general conditions, clinical manifestations, laboratory results, treatment and prognosis of the two groups were compared using SPSS 26.0 software.Results:A total of 532 cases were in the HDN group and 413 cases in the G6PD deficiency group. The HDN group reached peak hyperbilirubinemia earlier than the G6PD deficiency group [3(2,5) d vs. 6(4,8)d, P<0.05]. The HDN group had lower peak value of total serum bilirubin [379.5(345.6,426.7) μmol/L vs. 486.4 (413.5,577.4) μmol/L] and lower incidence of anemia [37.4% (199/532) vs. 55.0% (227/413)]than the G6PD deficiency group.The incidence of anemia with elevated reticulocyte percent(Ret%) in the HDN group was higher than the G6PD deficiency group[66.3%(132/199) vs. 5.7%(13/227), P<0.05]. Compared with the G6PD deficiency group, the incidences of exchange transfusion and repeated (≥2 times) exchange transfusion, acute bilirubin encephalopathy(ABE) and the mortality rate after withdrawal of treatment in the HDN group were significantly lower ( P<0.05). Conclusions:Neonatal severe hyperbilirubinemia caused by HDN has early onset. G6PD deficiency caused hyperbilirubinemia has higher incidences of anemia, more severe jaundice and ABE, without increased Ret%.

ABSTRACT

BackgroundPerinatal depression seriously affects the physical and mental health of pregnant women, such as affecting their role transition, identity, and family relationships, etc. In severe case, it can even lead to suicidal behavior, causing a heavy burden on pregnant women and their families. A hierarchical management model centered on pregnant women, involving collaboration of families, communities, and hospitals, facilitates comprehensive and dynamic management of perinatal depression. ObjectiveTo evaluate the application effect of "Internet +" hospital-community-family trinity linkage management model on perinatal depression in pregnant women, in order to provide a reference for the clinical intervention. Methods80 pregnant women who established medical records from the Department of Obstetrics and Gynecology of Mianyang Third People's Hospital from January to December 2022, with Edinburgh Postnatal Depression Scale (EPDS) score>9, were selected as the research objects. According to the random number table method, they were divided into a study group and a control group, each group consisting of 40 cases. Both groups received routine nursing intervention in the pregnant women's school of obstetrics and gynecology outpatient department before delivery, and in the study room of the department of obstetrics and gynecology after being admitted to the hospital for delivery. After discharge, they received routine follow-up until 42 days postpartum. The study group received the "Internet +" hospital-community-family ternary linkage management on the basis. EPDS, Pittsburgh Sleep Quality Index (PSQI), Generic Quality of Life Inventory (GQOLI-74) and Nursing Satisfaction questionnaire were assessed before intervention and 42 days postpartum. ResultsAfter intervention, the EPDS score and PSQI score of the study group were lower than those of the control group (F=42.823, 60.453, P<0.05), GQOLI-74 score and nursing satisfaction were higher than those of the control group (F=198.902, χ2=5.165, P<0.05) . Conclusion"Internet +" hospital-community-family trinity linkage management model may help to improve the severity of perinatal depression symptoms, improve the quality of sleep and life, and increase the satisfaction of pregnant women.[Funded by Mianyang Health Scientific Research Commission Project (number, 202134)]

ABSTRACT

@#Objective     To systematically evaluate the safety, efficacy, and economics of intracardiac echocardiography (ICE) versus transesophageal echocardiography (TEE) in left atrial appendage occlusion (LAAO). Methods     PubMed, EMbase, The Cochrane Library, CBM, CNKI, VIP and WanFang Database were systematically  searched to collect relevant studies on comparing ICE and TEE-guided LAAO from inception to June 15th, 2022. Two reviewers independently screened the literatures, extracted the data, and assessed the risk of bias of the included studies. Meta-analyses were performed using RevMan 5.3 and R 4.0.3. Retrospective cohort studies were excluded for sensitivity analysis. Subgroup analyses were performed based on the types of occluder and ICE catheter. Results     A total of 14 studies with 6 599 patients were included. Meta-analyses showed no statistical differences in technical success rate, overall complications, device embolization, peri-device leakage, device-related thrombus, stroke, vascular complications, bleeding, operation time, fluoroscopy time, or contrast agent volume between the ICE and TEE-guided LAAO. The total in-room time (MD=–33.47 min, 95%CI –41.20 to –25.73, P<0.000 01) and radiation dosage (MD=–170.20 mGy, 95%CI –309.79 to –30.62, P=0.02) were lower in the ICE group than those in the TEE group, whereas the incidence of pericardial effusion/tamponade was higher than the TEE group (RR=1.57, 95%CI 1.01 to 2.45, P=0.048). Except for pericardial effusion/tamponade, subgroup analyses and sensitivity analysis showed similar results. The analysis based on the cost data from the United States showed comparable or even lower total costs for ICE versus TEE, but comparative domestic cost studies were lacking. Conclusion     Current evidence suggests that ICE-guided LAAO can reduce radiation dosage and total in-room time, and there is no statistical difference in the overall complication rate between the two groups. Owing to the limitations of sample size and quality of the included studies, the conclusion still needs to be verified by large sample size and high-quality randomized controlled trials.

ABSTRACT

ObjectiveTo analyze the effects of mindfulness-based childbirth on procedural fear, pain and pregnancy outcome in pregnant women, so as to provide references for clinical application of mindfulness-based childbirth. MethodsA total of 100 pregnant women in the Third Hospital of Mianyang from January 2018 to January 2019 were selected, and divided into experimental group and control group by random number table method, each with 50 cases. All pregnant women received routine nursing, based on this, experimental group received an 8-week mindfulness-based childbirth intervention. Then pregnant women were evaluated with Childbirth Attitudes Questionnaires (CAQ) and Numerical Rating Scale (NRS), meantime, the labour course and mode of delivery were recorded. Thereafter, the two groups were compared in terms of fear, pain, labor course and delivery mode. ResultsThe scores of CAQ [(30.46±9.27) vs.(43.24±14.54), t=-5.242, P<0.01] and NRS [(6.64±0.80) vs.(7.32±0.77), t=-4.331, P<0.01] yielded statistical differences between experimental group and control group. The labor course of experimental group was shorter than that of control group [(10.10±1.91)h vs.(13.28±3.28)h, t=-5.921, P<0.01]. The vaginal delivery rate was 84.00% in experimental group and 60.00% in control group, and the cesarean section rate was 16.00% and 40.00%, respectively, with statistical difference (χ2=7.143, P<0.01). ConclusionMindfulness-based childbirth may alleviate the labor fear and pain, shorten the labor course and promote vaginal delivery.

ABSTRACT

Objective:To investigate the effects of different doses of Lycium barbarum polysaccharides (LBP) and aerobic exercise on rats with non-alcoholic fatty liver disease (NAFLD) and its mechanisms. Methods:SD rats of 8 weeks old were randomly divided into normal control group and high fat diet (HFD) group. The normal control group was given normal diet, and the HFD group was given HFD for 10 weeks to construct a NAFLD rat model. After successful modeling, the rats with NAFLD were assigned into four groups with eight in each, namely: HFD group, LBP-a group (50 mg/kg), LBP-b group (100 mg/kg) and aerobic exercise group (20 m/min, 60 min/d). After the intervention for 6 weeks, fasting for 12 hour and fasting blood glucose level was measured. Blood samples and liver tissues of rats were collected after intraperitoneal anesthesia with 10% chloral hydrate. The levels of glutamate pyruvic transaminase (GPT), glutamic oxaloacetic transaminase (GOT), total cholesterol (TC), triacylglycerol (TAG), low density lipoprotein (LDL), high density lipoprotein (HDL), malondialdehyde (MDA), superoxide dismutase (SOD) and irisin in rats were measured according to corresponding protocols of kits. The expressions of peroxisome proliferators-activated receptor-γ coactivator-1 α (Pgc1-α) and fibronectin type III domain-containing protein 5 (Fndc5) in liver were detected by using reverse transcription polymerase chain reaction. Results:Compared with the HFD group, the levels of GPT, GOT, TC, TAG, MDA and homeostasis model assessment of insulin resistance (HOMA-IR) were significantly decreased in LBP-a group, LBP-b group and aerobic exercise group (P<0.05). The levels of HDL and SOD were significantly increased in three intervention groups (P<0.05). The levels of LDL were markedly decreased and the levels of irisin were significantly increased in LBP-b group and aerobic exercise group compared with the HFD group (P<0.05). The expressions of Pgc1-α mRNA in LBP-b group and aerobic exercise group were up-regulated as compared with the HFD group (P<0.05). The expressions of Fndc5 mRNA in three intervention groups were up-regulated as compared with the HFD group (P<0.05). Conclusion:LBP and aerobic exercise can significantly improve lipid metabolism disorder and decrease liver oxidative stress in rats with NAFLD, which may be related to the up-regulated expressions of Pgc1-α mRNA and Fndc5 mRNA.

ABSTRACT

Aim To explore the inhibitory effect of Euonymus alatus on hepatic fibrosis induced by carbon tetrachloride (CCl4) in mice and its mechanism. Methods Eighty C57BL/6 male mice were randomly divided into eight groups: normal group, CCl4model group, Euonymus alatus(EA) ethanol extracts groups in early stage(EAE), EA ethanol extracts groups in later stage(EAL),two drug groups with low/medium/high dose(EAE-L/M/H, EAL-L/M/H), with 10 mice in each group. Fibrosis model was established by injecting CCl4in peritoneal cavity,and the study lasted for 30 days. Different doses of drugs were given from 1 st day to 15 th day in EAE while from 16 th day to 30 th day in EAL,then all mice were sacrificed to for the observation of the morphological changes and collage-nous fiber by HE and Masson staining. Liver index, ALT,AST and TNF-α were tested by ELISA. The ex-pressions of α-SMA and CollagenⅠwere measured by immunohistochemistry and Western blot. Results Compared to normal group, liver index, ALT, AST, TNF-α, α-SMA and CollagenⅠ in EA groups were lower than those in model group in a dose-dependent manner(P<0.01 or P<0.05). Liver morphology and collagenous fiber in EAE and EAL were better than those in model group in a dose-dependent manner. The effect of EAE were superior to that of the EAL in HE, Masson, α-SMA, Collagen Ⅰ indexes(P <0.05). Conclusions Euonymus alatus may inhibit the process of hepatic fibrosis in mice with dose-effect de-pendence, and drug treatment in early stage performs better,which may be related to the decrease of TNF-α that affects the expression of α-SMA and Collagen Ⅰ.

OBJECTIVE: To establish a simple and sensitive method for simultaneous detection of chlorpyrifos( CPF),and its metabolite 3,5,6-trichloro-2-pyridinol( TCP) in plasma samples by ultra performance liquid chromatography( UPLC).METHODS: The 0. 5 m L of blood sample was extracted by ethyl acetate,then separated by C18 column using acetonitrile /water as a mobile phase and detected by diode-array detector under the ultraviolet spectrum of 298 nm( 0. 0-2. 0 min) and289 nm( 2. 0-5. 0 min) through UPLC. RESULTS: The method showed that a good linear range was 1. 00-50. 00 mg / L of both CPF and TCP,and the correlation coefficients was 0. 999 9. The limit of detection was 0. 30 mg / L and the lower limits of quantitation was 1. 00 mg / L of both CPF and TCP. The recovery rates of CPF and TCP were 87. 20 %-103. 08 %and 86. 20 %-99. 28 %,respectively. The within-run relative standard deviation( RSD) of CPF and TCP was 5. 28 %-6. 17 % and 2. 32 %-4. 43 %,and the between-run RSD was 6. 62 %-7. 53 % and 3. 55 %-5. 24 %. Samples can be kept in4 degrees Celsius refrigerator for 7 days. CONCLUSION:s The proposed method is simple,reliable and sensitive. It can be applied for simultaneous detection of CPF and its metabolite TCP in plasma samples.

ABSTRACT

Objective To investigate the values and characteristics of 75 g oral glucose tolerance test (OGTT) in women with different pre-pregnancy body mass index (BMI) and to evaluate the risk of gestational diabetes mellitus (GDM).Methods Medical records of 9 803 pregnant women attending the Peking University First Hospital and delivered between July 1, 2011 and December 31, 2013 were retrospectively analyzed.The frequency of adverse pregnancy outcomes across different degrees pre-pregnancy BMI was calculated.We divided participants into 4 groups based on pre-pregnancy BMI, underweight: BMI<18.5 kg/m2 (1 221 cases), normal weight: 18.5-23.9 kg/m2 (6 594 cases), overweight: 24-27.9 kg/m2 (1 549 cases), obesity: ≥28.0 kg/m2 (439 cases).The diagnosis of GDM was made when any one of the values was met or exceeded in 75 g OGTT.The characteristics of 75 g OGTT and the incidence of GDM were analyzed.Results (1) The average age, pre-pregnancy weight, height and pre-pregnancy BMI of the participants was (30.5±3.7) years, (57.7±9.0) kg, (162.8±4.9) cm and (21.8±3.2) kg/m2, respectively.All the values of 75 g OGTT were presented normal distribution.(2)There was statistical difference in the glucose levels among women with different pre-pregnancy BMI.The fasting, 1-hour, 2-hour glucose were (4.55± 0.34), (7.31 ± 1.54), (6.38± 1.23) mmol/L in underweight women, (4.65 ±0.38), (7.70± 1.59), (6.70± 1.27) mmol/L in normal weight women, (4.82±0.47),(8.29±1.67),(7.04±1.29) mmol/L in overweight women and (4.94±0.48), (8.56 ± 1.64), (7.10 ± 1.35) mmol/L in obesity women (P<0.01).(3) The incidence of GDM was 21.76%(2 133/9 803) in our study.There were 1 374 cases (64.42%, 1 374/2 133)with only one abnormal OGTT value while 759 cases (35.58%, 759/2 133)with two or more abnormal values.The incidence of GDM in women with underweight, normal weight, overweight and obesity was 12.53%(153/1 221),19.71%(1 300/ 6 594),32.73% (507/1 549) and 39.41% (173/439), respectively (P<0.05).Meanwhile, the incidence of women with two and more abnormal OGTT value in GDM was increased as the pre-pregnancy increasing.Conclusion The risk of GDM is increased as pre-pregnancy BMI increasing, and the risk of GDM increases significantly in women with pre-pregnancy overweight or obesity.

ABSTRACT

Anaerobic ammonium oxidation (ANAMMOX), as its essential advantages of high efficiency and low cost, is a promising novel biological nitrogen elimination process with attractive application prospects. Over the past two decades, many processes based on the ANAMMOX reaction have been continuously studied and applied to practical engineering, with the perspective of reaching 100 full-scale installations in operation worldwide by 2014. Our review summarizes various forms of ANAMMOX processes, including partial nitritation-ANAMMOX, completely autotrophic nitrogen removal over nitrite, oxygen limited autotrophic nitrification and denitrification, denitrifying ammonium oxidation, aerobic deammonification, simultaneous partial nitrification, ANAMMOX and denitrification, single-stage nitrogen removal using ANAMMOX and partial nitritation. We also compare the operating conditions for one-stage and two-stage processes and summarize the obstacles and countermeasures in engineering application of ANAMMOX systems, such as moving bed biofilm reactor, sequencing batch reactor and granular sludge reactor. Finally, we discuss the future research and application direction, which should focus on the optimization of operating conditions and applicability of the process to the actual wastewater, especially on automated control and the impact of special wastewater composition on process performance.

ABSTRACT

To study the effect of preservation temperature on the characteristics of anaerobic ammonium oxidation (Anammox) granules and optimize the preservation temperature of Anammox granules, the Anammox granules were cultivated in an upflow anaerobic sludge bed reactor through adjusting the hydraulic retention times, and the inorganic carbon with KHCO3/NaHCO3 was alternately supplied. Subsequently, the enriched Anammox granules were preserved at -40, 4 and 35 °C, and ambient temperature of (27 ± 4) °C. NaHCO3 can be used as the inorganic carbon for the growth of anaerobic ammonium oxidizing bacterium (AnAOB). The best preservation temperature was 4 °C for maintaining Anammox biomass, Anammox activity, settleability, and the integrity of the Anammox granule and AnAOB cell structure. During the preservation period, the first-order exponential decay model can simulate the decay of Anammox biomass and activity, and the decay coefficients (bAN) of Anammox biomass and activity had positive correlation with the degree of AnAOB cell lysis. Meanwhile, the rate of Anammox biomass decay was larger than that of Anammox activity. The ratio of protein to polysaccharide in extracellular polymeric substances and heme c cannot effectively indicate the changes of Anammox granules settleability and activity, respectively, and the bioactivity has a negative association with the degree of AnAOB cell lysis.

ABSTRACT

Objective To explore the application value of Mammotome biopsy system in diagnosis and treatment of breast lesions. Methods Summed up the experience of 318 breast lesions in 192 patients who were treated with mammotome. Results 316 lesions were re-sected safely and completely. Skin hematoma was found in 17 patients and ecchymosis was found in 8 patients after operation. All patients had no recurrence and residual. Conclusion The Mammotome biopsy system has many advantages for the diagnosis and treatment of breast lesions, including accurate localization, minimal invasion, safety and good cosmetology.

ABSTRACT

Objective To investigate the clinical and immunological features,therapeutic response as well as prognosis of adult onset Still's disease (AOSD).Methods AOSD was diagnosed in 137 patients referred to our department.Clinical and immunological data were retrospectively analyzed.Therapeutic response and prognosis were systemically reviewed during the follow-up period.Intergroup incidence divergence was analyzed by chi-square test.Cox regression analysis was adopted to determine factors related with relapse.Results Fever,rash and arthritis were the cardinal clinical features of AOSD patients.Elevated inflammatory indices including ferritin (128 suhjects,97.1% ) along with neutrophilia and liver dysfunction were the main laboratory findings.Ninety-eight patients were followed up and 75% (73 subjects) had achieved complete remission after 4 weeks treatment.Forty-one patients (42%) who had achieved remission relapsed during follow-up period.Combination of glucocorticoid steroid and disease modifying antirheumatic drugs (DMARDs) were more effective than glucocorticoid steroid only in inducing remission and preventing relapse.More patients received glucocorticoid combined with methotrcxate and hydroxychloroquine achieved remission (8 of 8 patients) than patients who were treated with glucocorticoid and methotrexate (25 of 28 patients,89% ) and those treated with glucocorticoid and hydroxychloroquine (14 of 16 patients,88% ).Patients with glucocorticoid were more likely to suffer disease recurrence than those who took glucocorticoid combined with DMARDs (61% vs 29%,P=0.004).Cox regression analysis suggested that methotrexate had protec-tive effect against recurrence [RR=0.418,95%CI (0.192-0.909),P=0.028].5% of patients were diagnosed to other diseases during the follow up period.Conclusion Initial treatment with combined glucocorticoid and DMARDs is beneficial to induce remission and prevent reoccurrence.Methotrcxate has a protective effect against recurrence.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the intima-media thickness and cardiac remodeling in patients with prehypertension and impaired glucose tolerance.</p><p><b>METHOD</b>Three-hundred patients were divided into four groups: normal control (NC, n = 61), prehypertension (PH, n = 83), impaired glucose tolerance (IGT, n = 91), prehypertension and impaired glucose tolerance (PH + IGT, n = 65). Intima-media thickness (IMT) was measured by doppler ultrasonography. Left ventricle mess (LVMI), midwall fractional shortening (mFS), peakE/peakA (E/A) were measured by echocardiography.</p><p><b>RESULTS</b>(1) IMT was significantly increased in PH, IGT and PH + IGT groups than in NC group [(0.7 ± 0.1) mm, (0.7 ± 0.1) mm and (1.0 ± 0.1) mm vs.(0.6 ± 0.1) mm, all P < 0.01], which was significantly high in PH + IGT group than in PH or IGT group. Regression analysis demonstrated that high-sensitivity C-reactive protein (hs-CRP), 2-hour postprandial blood glucose (2hPBG), systolic pressure (SBP), diastolic blood pressure (DBP) were independent predictors to increased IMT. (2) LVMI was higher in PH and PH + IGT groups than in NC group [(97.0 ± 3.3) g/m(2), (97.1 ± 2.8) g/m(2) vs.(87.0 ± 2.0) g/m(2), (87.9 ± 1.5) g/m(2), all P < 0.01], and positively related with SBP, DBP. (3) mFS was lower in PH and PH + IGT groups [(14.0 ± 0.8)%, (14.0 ± 0.8)% vs. (18.3 ± 1.0)%, (18.2 ± 0.5)%, P < 0.01], negatively related with SBP and DBP. E/A was similar among groups and lower E/A was associated with higher SBP.</p><p><b>CONCLUSION</b>Vascular and left ventricular structure remodeling and systolic dysfunction could be detected in prehypertension patients. Impaired glucose tolerance could enhance vascular remodeling in prehypertension patients.</p>

ABSTRACT

Objective To study the therapeutic effect of cardura on lower urinary tract symptoms (LUTS) and ejectile dystunction(ED) in men with benign prostatic hyperplasia (BPH).Methods One hundred and one cases with BPH and typical LUTS were investigated using the International Prostate Symptom Score (IPSS) and International Index of Erectile Function 5 (IIEF-5)before and after treatment. The patients were given cardura 4 mg, once a day for 8 weeks. The LUTS and ED were was analyzed after treatment. Results The IPSS was scores ( 14.3 ± 4.0) after treatment, and it was lower than before (mean scores: 19.9±6.5, t= 7.534, P<0.001). The IIEF2-5 was(14.9±8.8)scores after treatment, and it was higher than before (mean scores: 9.5±8. 5, t = 5. 335, P< 0. 001 ), respectively. There was statistically significant correlation between IIEF2-5 and IPSS score (r= -0. 327, P<0. 001 ). Various indexes were significantly improved after cardura treatment as compared with pretreatment (P<0.001). Conclusions LUTS is dangerous factor for sexual dysdysfunction, and the severity of LUTS is closely related to the development of sexual dysfunction. Cardura can at once improve the sexual function and LUTS of BPH patients.

ABSTRACT

Objective To compare and analyze the causes that occurred in psychiatric and general department using SHEL models, in order to provide evidence of preventing nursing adverse events.Methods We analyzed causes of 136 nursing adverse events using SHEL models, and compared the classification, causes, incidence of nursing adverse events between psychiatric and general departments. Results The first three nursing adverse events in psychiatric department were medication error,going outside, suicide and self harm, and those in general department were medication errors, specimens being wrong, improper operation and pipeline off. 91.18% of nursing adverse events accounted for quality of nursing services, 43.38% accounted for nursing environment, 41.18% accounted for clinical environment,and 57.35% accounted for parties and others. Incidence of nursing adverse events in psychiatric department were higher than that in general department. Conclusions SHEL model can objectively and comprehensively analyze the causes of nursing adverse events, and make nursing managers take the appropriate management strategy and the best safeguard against the risk of development of response.

ABSTRACT

<p><b>OBJECTIVE</b>To study the risk factors of hepatitis B virus (HBV) intrauterine infection.</p><p><b>METHODS</b>Risk factors of HBV intrauterine infection were analyzed by nested case control study.</p><p><b>RESULTS</b>Data from univariate analysis revealed that risk factors of HBV intrauterine infection were positive results on HLA-DR3 (OR = 4.71, 1.62-13.66), HBV DNA (OR = 6.59, 2.72-15.97) and HBeAg (OR = 4.53, 1.93-10.64) in pregnant women, HLA-DR3 (OR = 3.91, 1.18-12.94) in newborn, HLA-I) R3 (OR = 5.96, 1.14-31.15) both in pregnant women and her newborns and HBV infection in placentas (OR = 2.51,1.12-5.60). Results from Multivariate unconditional logistics regression analysis showed that the risk factors of HBV intrauterine infection were positive in both HLA-DR3 (OR = 4.65, 1.44-15.05) and HBV DNA (OR = 6.56, 2.65-16.23) in pregnant women. However, there was no interaction between the two factors. The exposure rate of other factors did not reveal the difference in the two groups. With the increase of HBV DNA in pregnant women, the risk of HBV intrauterine infection was rising (chi2 = 16.74, P < 0.05).</p><p><b>CONCLUSION</b>Risk factors of HBV intrauterine infection were HLA-DR3 positive and HBV DNA positive in pregnant women but there was no interaction between the two factors. The risk of HBV intrauterine infection was increased along with the increase of HBV DNA in pregnant women.</p>

ABSTRACT

Objective To summarize the management of scrotal gangrene.Methods Totally,14 male patients (mean age,48 years;age range,27-74 years) with scrotal gangrene were treated from August 2003 to June 2005.Among these patients,scrotal gangrene was complicated by perianal and penile gangrene in 2 cases;by perianal,penile and inguinal gangrene in 10 cases;by perianal,penile,inguinal gangrene and abdominal subcutaneous gangrene in 2 cases.Three cases had concomitantly necrosis of the penis;and 5 ca- ses,orchitis.Bacterial cultures were performed in all 14 cases with the positive results.All patients underwent surgical treatment,including PhaseⅠincisions,aggressive debridement,drainage,irrigation,and scrotoplas- ty.After operation they received antibiotic therapy and supporting treatment.Three cases of penile necrosis underwent complete penis amputation and transplanting the urethra before the anus.Five cases of orchitis un- derwent unilateral orchiectomy.Two cases of abdominal gangrene underwent surgical subcutaneous treatment, including incision,aggressive debridement,drainage,and irrigation.Results Twelve cases undergoing scrotoplasty achieved PhaseⅠhealing.Two cases undergoing urethral fistulation recovered well.Ten cases of penile skin gangrene received penile dermatoplasty 1 month later,and then recovered.Two cases who had al- ready had AIDS died 2 or 3 d after operation.Conclusions The management of scrotal gangrene primarily consists of early and aggressive debridement,antibiotic therapy and supporting treatment.The PhaseⅠde- bridement and scrotoplasty as well as drainage are effective for the treatment of scrotal gangrene.
