ABSTRACT

Objective: To evaluate the efficacy and safety of different medical treatment in advanced or unresectable angiosarcoma. Methods: This study was a single-center retrospective clinical study. Fifty-five advanced or unresectable angiosarcoma patients treated in Sun-Yat Sen University Cancer Center from January 2005 to August 2020 were enrolled. There were 34 patients who received first-line doxorubicin-based chemotherapy (doxorubicin group), 12 patients received first-line doxorubicin or liposome doxorubicin plus paclitaxel or albumin bound paclitaxel chemotherapy (combination therapy group), and 4 patients received first-line paclitaxel-based treatment (paclitaxel group). There were 6 patients who received anti-angiogenesis targeted therapy, another 2 patients received anti-PD-1 antibody plus anti-angiogenesis targeted therapy. Targeted therapy and immunotherapy plus targeted therapy included 5 cases of first-line therapy and 3 cases of second-line therapy. The therapeutic effect was evaluated by RECIST 1.1 standard. The adverse reactions were evaluated by CTCAE4.0 standard. Kaplan-Meier survival analysis was evaluated with Log rank test. Cox proportional hazard model was used to analyze the influencing factors. Results: There were 18 patients achieved partial response (PR) in 34 patients in the doxorubicin group, median progression-free survival (mPFS) was 4.5 months, and median overall survival (mOS) was 15 months. Four patients achieved PR in 12 patients in the combination therapy group, mPFS and mOS were 4 months and 19 months. Two patients achieved PR in 4 patients in the paclitaxel group, mPFS and mOS were 3 months and 9 months. However, only 1 in 6 patients achieved PR for anti-angiogenesis targeted therapy, mPFS and mOS were 3 months and 16 months. Two patients who received anti-PD-1 immunotherapy combined with anti-angiogenesis targeted therapy acquired PR for 17 months and more than 16 months. Median PFS (7.5 months) were longer in those with primary liver, lung and spleen angiosarcoma than in those with other primary site (3.0 months, P=0.028). The mOS (20 months) was longer in females than that in males (12 months, P=0.045). Primary tumor site, sex, age and treatment were not independent prognostic factors for angiosarcoma patients (P>0.05). Grade 3-4 cardiac toxicity was found in 2 patients in the combination therapy group. Conclusions: Doxorubicin-based and paclitaxel-based chemotherapy are the most important treatment for advanced angiosarcoma. Potential efficacy for targeted therapy combined with anti-PD-1 immunotherapy are showed in some patients with long duration of response and moderate adverse event.

ABSTRACT

A novel coronavirus pneumonia (NCP) epidemic has occurred in Wuhan, Hubei Province since December 2019, caused by a novel coronavirus (2019-nCoV) never been seen previously in human. China has imposed the strictest quarantine and closed management measures in history to control the spreading of the disease. However, severe trauma can still occur in the NCP patients. In order to standardize the emergency treatment and the infection prevention and control of severe trauma patients with hidden infection, suspected or confirmed infection of 2019-nCoV, Trauma Surgery Branch of Chinese Medical Doctors' Association organized this expert consensus. The consensus illustrated the classification of the NCP patients, severe trauma patients in need of emergency surgery, emergency surgery type, hierarchical protection for medical personnel and treatment places. Meanwhile, the consensus standardized the screening, injury severity evaluation, emergency surgical treatment strategy and postoperative management strategy of severe trauma patients during the epidemic period of NCP, providing a basis for the clinical treatment of such kind of patients.

ABSTRACT

OBJECTIVE: To investigate the prenatal ultrasonographic manifestations of fetal congenital omphalo-enteric fistula with intestinal prolapse and improve the understanding and diagnosis of this disease.METHODS: A retrospective analysis of the prenatal ultrasonographic features was performed in 2 cases of fetal congenital omphalo-enteric fistula with intestinal prolapse comnfirmed after surgery between July 2016 and September 2017 in First Affiliated Hospital of Fujian Medical University,and the ultrasonographic manifestations were compared with postnatal looks and intraoperative conditions.Characteristics of prenatal ultrasonographic manifestations were summarized.RESULTS: Both cases were misdiagnosed as omphalocele by prenatal ultrasonography.Congenital omphalo-enteric fistula with intestinal prolapse was verified by surgery after delivery.A narrow pedicle,special bowel-shape and incomplete capsule were its prenatal ultrasonic characteristics.CONCLUSION: Fetal congenital omphalo-enteric fistula with intestinal prolapse is rare,but has particular prenatal ultrasonographic manifestations. Strengthening the understanding of the disease,reducing misdiagnosis and improving prenatal ultrasound diagnosis will provide help for prenatal counseling and clinical choice for time of delivery.

ABSTRACT

Objective To analyze the correlation between antimicrobial use density (AUD) and change in antimicrobial resistance rate of Stenotrophomonas maltophilia (SM), and explore the influencing factors of antimicrobial resistance of SM. Methods Antimicrobial resistance rate of SM and AUD of commonly used antimicrobial agents in patients in a hospital from 2012 to 2017 were summarized, correlation was analyzed with Pearson correlation method. Results A total of 23 994 strains of gram-negative bacteria were isolated, of which 1 331 strains (5.55%) were SM, mainly from sputum (54.02%) and distributed in intensive care unit (21.49%). Resistance rates of SM to ceftazidime, levofloxacin, and compound sulfamethoxazole were 21.79%, 7.66%, and 13.37% respectively, resistance rates to levofloxacin showed an increasing trend year by year (P<0.05). Resistance rate of SM to levofloxacin was positively correlated with the use intensity of β-lactamase inhibitors, carbapenems, fluoroquinolones, and oxazolidinones (all P<0.05); resistance rate to compound sulfamethoxazole was positively correlated with the use intensity of macrolides (P<0.05).Conclusion Change in resistance rates of SM to levofloxacin and compound sulfamethoxazole are positively correlated with the use intensity of some commonly used antimicrobial agents, reducing AUD is beneficial to the control and reducing of the resistance of SM.

ABSTRACT

Recently,research on microbes has been explored ex-plosively. Microbial-related studies have become the forefront of anti-cancer research. A large number of microorganisms are uni-versal in the viscera, skin, nose, mouth and reproductive sys-tem. Probiotics is a kind of microorganisms beneficial to human health. Although most studies have proven the tumor-promoting effects of bacterial flora,antitumor effects of probiotics have also been observed. Studies have confirmed that probiotics can pre-vent the onset of early stage colon cancer as well as non-colorec-tal cancer mainly via several mechanisms,such as adjustment of intestinal flora,regulation of immune response,anti-inflammato-inactivation of metabolic carcinogen, antioxidant and induction of tumor cell apoptosis. However,the specific underlying molecular mechanism still needs to be further studied. The latest studies on human have shown that intestinal microbiota combining with diet can alter the risk of special colorectal cancer. As a cancer chemoprevention strategy, more pre-clinical and clinical research are in need for the study of the effects of probiotics on cancer prevention and treatment.

ABSTRACT

Objective To investigate the prevalence and genotype distribution of human papillomavirus (HPV) in the region of Guangxi.Methods We retrospectively analyzed 15774 individuals from the Outpatient and inpatient Unit as well as Physical Examination Departments of the People's Hospital of Guangxi Zhuang Autonomous Region between May 2010 and March 2017.Exfoliated c ellsor swab specimens were collected,and the genotypes of HPV were determined by Polymerase Chain Reaction (PCR) and reverse blot assay.Results The prevalence of HPV infection was 38.54% (6080/15774).The infection rate of single genotype and multiple genotypes were 25.60% (4038/15774) and 12.95% (2042/15774),respectively.In single infection patterns,the most common genotypes included HPV 6 (10.54%,1663/15774),52 (3.91%,616/ 15774),16 (2.16%,340/15774),11 (2.14%,338/15774),and 58 (2.00%,316/15774).While among the multiple infections patterns,HPV 52+53 (4.26%,87/2042) and 52+58 (3.23%,66/ 2042) were common,and followed by HPV 16+43 (2.40%,49/2042),16+52(2.30%,47/2042),6+42 (2.15%,44/2042) and 6+43 (2.15%,45/2042).HPV 52,16,58,51,53 and 18 were the top six high risk (HR) genotypes of HPV,accounting for 26.77% (4222/15774,95% CI =26.08-27.46);while HPV 6,11 and 43 were the leading low-risk (LR) genotypes of HPV,accounting for 27.77% (4380/15774,95%CI =27.07-28.47).The overall ratio of single infection to multiple infections was 1.98 (4038 vs.2042).Conclusion HPV 6,52,16,11,58,18 were the main HPV infection genotypes,and 52+53 and 52+58 were common infection combinations in Guangxi Zhuang Autonomous region.The HPV multiple infections were increased.Apparently,more HPV low risk genotypes were seen in male patients that should be aware.

ABSTRACT

Objective: To observe the difference of the effect of low-frequency electroacupuncture (EA) on blood lipids between male and female patients with simple obesity due to damp induced by spleen deficiency. Methods: Eighty patients with simple obesity were recruited, including 37 males and 43 females, to receive low-frequency EA by selecting Yinlingquan (SP 10), Sanyinjiao (SP 6), Zusanli (ST 36), Fenglong (ST 40), Quchi (LI 11), Tianshu (ST 25), Zhongwan (CV 12), Shuifen (CV 9), Qihai (CV 6) and Guanyuan (CV 4), with needles retained for 30 min. The treatment was given once a day, 10 sessions as a treatment course, for 2 courses in total. The contents of body fat percentage (F%), total cholesterol (TC), triglyceride (TG), high-density lipoprotein (HDL), low-density lipoprotein (LDL), blood glucose (Glu) and adiponectin (ADPN) in serum were observed to see the changes, and the two groups were compared and analyzed. Results: After the treatment, F%, and serum contents of TC, TG, LDL, Glu and ADPN dropped significantly in the two groups (P<0.05 or P<0.01) and the serum content of HDL increased significantly in male group (P<0.05). The decrease of F% in female group was more significant than that in male group (P<0.01); the decrease of ADPN in male group was more significant than that in female group (P<0.05). Conclusion: EA can regulate the disordered blood lipids in male and female patients with simple obesity, with certain differences between genders. The decrease of subcutaneous fat content is more significant in females than that in males, while the decrease of ADPN is more significant in males.

ABSTRACT

The diffusion coefficient ((4. 11±0. 78)×10-11 m2/s) and hydrated radius ((6. 12±1. 21) nm) of FITC-DSA ( fluorescein isothiocyanate conjugate dog albumin ) were investigated and measured by combining Taylor dispersion analysis with laser induced fluorescence in microchip. The influence of size of gold nanoparticles (15, 30 and 50 nm) on the interactions between FITC-DSA and gold nanoparticles was studied. The preliminary result demonstrated that the binding of protein and gold nanoparticle displayed a size-dependent relationship. It was interesting that the 50-nm gold nanoparticles could enhance the FITC-DSA fluorescence in microchannel driven with pressure. This method was simple, fast, low sample consumption and high throughput and could be used to study the interaction of nanoparticles and proteins. Systematical study using this method would enable us to have a deep understanding of the toxicity of nanomaterials, and promote the development of safe nanomedicine.

Methods: A total of 532 retired hypertension patients elder than 65 years in Guangzhou military region were enrolled. The patients were divided into 2 groups: Hypertension group, n=414 and Hypertension combining OH (H+OH) group, n=118. The patient's age (65-79、≥ 80), hypertension grade (Grade 1-3) and complication status were studied. The risk factors for H+OH prevalence were analyzed by multivariate Logistic regression analysis. Results: The incidence rate of H+OH was 22.2% (118/532). In H+OH group, the ratios of elderly and very elderly patients were 6.7% and 23.1%, P<0.05 and the ratios of OH occurrence for hypertension grade 1, 2 and 3 were 12.6%, 23.3% and 25.2% respectively, P<0.05. Multivariate Logistic regression analysis presented that systolic blood pressure (BP) in supine position, BP at immediate standing, heart rate in supine position, heart rate after 2 minutes standing and chronic cardiac insufficiency were the impact factors for H+OH occurrence, P<0.05. Conclusion: In elderly hypertension patients, incidence of OH was increasing with age elevating; H+OH has been related to age, severity of hypertension and chronic cardiac insufficiency.

Objective : The aim of this study was to explore the clinical characteristics of renal metastatic cancer, the methods for its detection by radioiodine (131I), and the response to 131I treatment in fourteen patients with renal metastases from differentiated thyroid carcinoma (DTC).Subjects and methods : DTC patients (n = 2,955) that received treatment with 131I were retrospectively analyzed. Scans (131I-WBS, 31I-SPECT/CT and/or 18F-FDG-PET/CT) were performed after an oral therapeutic dose of 131I. Therapeutic efficacy was evaluated based on changes in Tg and anatomical imaging changes at renal lesions.Results : Among these 14 patients, 11 had avidity for 131I, but three patients did not accumulate 131I after 131I treatment. In the 11 131I-positive renal lesions, 10 cases were detected by 131I-SPECT/CT combined with another imaging modality and one case by 131I-WBS combined with ultrasonography (US). In the three 131I-negative renal lesions, two cases were detected by 18F-FDG-PET/CT and one case by computed tomography (CT). In 11 patients with 131I-avid renal metastases, Serum Tg levels in 81.82% (9/11) patients showed a gradual decline, and 18.18% (2/11) of the patients showed a significant elevation. There was no marked difference in serum Tg before the last 131I treatment (Z = 0.157; p = 0.875). Only one patient presented partial response, eight patients exhibited stable disease, and renal metastases progressed in two patients showing progressive disease. No patients reached complete response.Conclusion : 131I-SPECT/CT, combined with another imaging modality after 131I-WBS, can contribute to the early detection of renal metastases of DTC. 131I therapy is a feasible and effective treatment for most DTC renal metastases with avidity for 131I. Objetivo : O objetivo deste estudo foi analisar as características clínicas de metástases renais, os métodos para sua detecção por radioiodo (131I) e a resposta ao tratamento com 131I em 14 pacientes com metástases renais de carcinoma diferenciado da tireoide (DTC).Sujeitos e métodos Pacientes com DTC (n = 2.955) que receberam tratamento com 131I foram analisados retrospectivamente. 131I-PCI, 31I-SPECT/CT e/ou 18F-FDG-PET/CT foram feitos após uma dose terapêutica oral de 131I. A eficácia terapêutica foi baseada nas alterações da Tg e nas imagens anatômicas das lesões renais.Resultados : Dos 14 pacientes, 11 apresentaram lesões ávidas por 131I, mas três pacientes não acumularam 131I depois do tratamento com 131I. Nas 11 lesões renais positivas para 131I, 10 casos foram detectados por 131I-SPECT/CT combinado com outra modalidade de exame de imagem e um caso por 131I-WBS combinado com US. Nas três lesões renais negativas para 131I, dois casos foram detectados por 18F-FDG-PET/CT e um caso por tomografia computadorizada (TC). Em 11 pacientes com metástases renais ávidas por 131I, os níveis séricos de Tg em 81,82% (9/11) dos pacientes mostraram um declínio gradual e 18,18% (2/11) apresentaram uma elevação significativa. Não houve diferenças marcadas na Tg sérica antes do último tratamento com 131I (Z = 0,157; p = 0,875). Apenas um paciente apresentou resposta parcial, oito pacientes apresentaram doença estável e as metástases renais progrediram em dois pacientes que apresentaram doença progressiva. Nenhum dos pacientes apresentou resposta completa.Conclusão : 131I-SPECT/CT, combinada com outra modalidade de diagnóstico por imagem após 131I-PCI, pode contribuir para a detecção precoce de metástases renais de DTC. ABSTRACT

Objective To investigate the clinical effects of phacoemulsification with foldable posterior cham-ber intraocular lens implantation in the treatment of primary angle closure glaucoma with cataract .Methods 82 patients(82 eyes) with primary angle closure glaucoma with cataract were selected as the research objects ,and taken the treatment of combination of phacoemulsification and posterior chamber foldable intraocular lens implanta -tion.c The vision,intraocular pressure ,anterior chamber depth and angle opening changes before and after 6 months of operation were ompared ,and the postoperative complications were observed .Results 54 eyes achieved best corrected visual acuity of more than 0.5,12 eyes achieved best corrected visual acuity of 0.4,6 eyes achieved best corrected visual acuity among 0.1 to 0.3 and 6 eyes achieved best corrected visual acuity of less than 0.1.The difference between postoperative best corrected visual acuity and that before operation had significant statistical differences (χ2 =43.244,P<0.01).After having the operation ,the intraocular pressure of acute closed angle glaucoma pre clinic , onset period and chronic angle closure glaucoma had significantly improved compared to pre -operation ( t=7.955, 4.741,5.638,all P<0.01).The chamber depth of the three groups had also improved substantially compared to that of pre-operation(t=13.080,11.890,16.124,all P<0.01).The degree of angle opening of acute closed angle glau-coma and chronic angle closure glaucoma were better than that of pre-operation,and postoperatively ,the degree of an-gle opening of the former turned out to be wider than that of the latter (χ2 =9.246,P<0.05).There were no postop-erative intraocular lens clamping,corneal decomposition,severe complication of retinal detachment and macular cys-toids edema .Conclusion The phacoemulsification combined with intraocular lens implantation for the treatment of primary angle closure glaucoma with cataract can effectively improve visual acuity ,control intraocular pressure ,deepen the anterior chamber ,open angle ,and the adverse reactions occurred less ,which is worthy of clinical application .

Objective To compare the feasibility,effectivity and security of modified trabeculectomy in advanced stage angle-close glaucoma with cataract.Methods One hundred and forty-five patients (151 eyes) of advanced stage angle-close glaucoma with cataract were enrolled in this study.Among them,85 patients(91 eyes,observation group) were treated with modified trabeculectomy with the resection of deep scleral resection and 60 patients (60 eyes,control group) were treated with routine trabeculectomy.The visual acuity,intraocular pressure (IOP),type of filtering bleb and postoperative complication were compared.Results There was significant difference in the visual acuity < 0.1,0.4-0.5,> 0.5 between after operation for 1 month and before operation in two groups (P < 0.05).but there was no significant difference in the visual acuity 0.1-0.3 (P > 0.05).The IOP before operation in observation group was (29.6 ± 4.1) mmHg (1 mmHg =0.133 kPa),12 months after operation was (13.1 ± 4.9) mmHg,the degree of decrease was (16.6 ± 5.0) mmHg.The IOP before operation in control group was (30.4 ± 6.3) mmHg,12 months after operation was (18.9 ± 3.0) mmHg,the degree of decrease was (10.4 ± 4.0) mmHg.The IOP after operation and the degree of decrease between two groups had significant difference (P < 0.05).The functional filtering bleb(Ⅰ type and Ⅱ type) in observation group was 77 eyes(84.6%,77/91),and 51 eyes(85.0%,51/60) in control group,there was no significant difference (P > 0.05).After operation for 1 month,there was 8 eyes appeared transient hypotony in observation group and 5 eyes in control group,there was no significant difference (P > 0.05).Conclusion The modified trabeculectomy in advanced stage angle-close glaucoma with cataract can reduce IOP more effectively and not increase the intraoperative risk and postoperative complication.

Objective To explore the clinical effects of phacoemulsification with intraocular lens implantation for cataract patients who have had glaucoma and have undergone trabeculectomy before.Methods Phacoemulsification with intraocular lens implantation was performed on 33 patients(38 eyes)with cataract after trabeculectomy.The postoperative visual acuity,intraocular pressure(IOP),filtering bleb,corneal endothelium and complications were observed.Results No posterior capsular rupture occurred and all the lenses were inserted into the capsular bags.Follow up for 3 months at least.The correct visual acuities were≥4.7 in 30 cases(78.9%),≥5.0 in 6 cases(15.8%).Postoperatively,the IOP of all eyes was below 20mmHg and there was no change in the filtering bleb.The loss rate of the corneal endothelial cells was 15.07%after the operation.The density of the postoperative corneal endothelial cells was statistically significant different from those of the preoperative(t=4.582,P<0.01).The complications occurred more than that of regular phaeoemulsification.Conclusions Phacoemulsification is convenient and effective in patients with cataract after trabeculectomy.The incision should be made without damaging the filtering bleb.The surgery requires skillfully and actively enlarging the small pupils and protecting the corneal endothelium.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the epidemic distribution of Mycobacterium tuberculosis isolates from Beijing, Guangdong and Ningxia, and to determine M. tuberculosis strains of the "Beijing Family".</p><p><b>METHODS</b>Two hundred and six IS6110 DNA fingerprinting patterns of M. tuberculosis strains from three provinces (city) were transferred to digital data, compared with the world M. tuberculosis DNA fingerprinting database, and then clustered by Gel compare 4.1 software. The clustering values in different patients with tuberculosis were compared by chi(2) test. Risk factors for recent transmission were calculated using odd ratios.</p><p><b>RESULTS</b>No M. tuberculosis strains were found the same as those of DNA fingerprint database. 56.8% (117/206) fingerprinting patterns of M. tuberculosis shared by least two-thirds of the IS6110 fragments and their Spoligotyping fingerprinting patterns were consistent with those of M.tuberculosis strains of the "Beijing Family". There were significant differences between female and male, different age groups (< 42 years old) and older (>or= 42 years old) (P < 0.05). Odd ratio was 5.06 in the group younger than 42 years old (95% CI: 1.00 - 34.34) and was 4.43 (95% CI: 0.94 - 28.76) in males.</p><p><b>CONCLUSION</b>M. tuberculosis strains of "Beijing Family" were popular in Beijing, Guangdong and Ningxia. Men and younger age group (< 42) were shown to be infected by identical strains more often than women and older aged which might play an important role in the recent transmission of tuberculosis in these areas. IS6110 DNA fingerprinting of M. tuberculosis could be used to trace the source of tuberculosis infection.</p>

ABSTRACT

Aim To study pharmacokinetics and bioavailablity of domestic penicillin V dispersion tablet in healthy volunteers. Methods According to the crossover design, each volunteer in two groups was orally given a single dose ( 0.75 g ) of domestic penicillin V dispersion tablet or imported penicillinV tablet alternately and the plasma concentrations were determined by RP HPLC. The pharmacokinetic parameters were obtained by using ATPK program and calculated on the basis of open single compartment model. Results After a single oral dose( 0.75 g ), the t 1/2(ke) was ( 0.75 ? 0.10 ) h and ( 0.70 ? 0.14 ) h ,the c max was( 8.44 ? 2.40 ) mg?L -1 and ( 8.75 ? 3.04 ) mg?L -1 at ( 0.56 ? 0.11 ) h and ( 0.63 ? 0.17 ) h and AUC 0~4 was( 8.44 ? 2.40 ) mg?h?L -1 and ( 8.75 ? 3.04 ) mg?h?L -1 for two formulations, respectively. Relative bioavailability of domestic penicillin V dispersion tablet was ( 90.50 ? 8.84 )%. Conclusion The result shows that the two formulations are bioequivalent.
