ABSTRACT

Objective:To compare the efficacy of transcatheter arterial embolization (TAE) with laparotomy in the treatment of severe liver injury.Methods:A retrospective cohort study was conducted to analyze the clinical data of 48 patients with severe liver injury admitted to 909th Hospital of Joint Logistics Support Force (Affiliated Dongnan Hospital of Xianmen University Medical College) from December 2013 to June 2020, including 28 males and 20 females; aged 16-75 years [(45.7±6.2)years]. There were 25 patients with grade III, 15 grade IV and 8 grade V according to the American Association for the Surgery of Trauma (AAST) classification. After general treatments such as infusion and hemostasis, TAE was performed in 26 patients (TAE group) and laparotomy in 22 patients (laparotomy group). The operation time and length of hospital stay were compared between the two groups. Erythrocyte, hemoglobin and serum creatinine were compared before operation and at postoperative 1 day. Alanine aminotransferase (ALT) and aspartate aminotransferase (AST) were analyzed before operation and at postoperative 1, 3, 7 days. Complications were observed.Results:All patients were followed up for 12-60 months [(17.1±9.1)months]. The operation time and length of hospital stay were (65.7±9.2)minutes and (21.6±6.6)days in TAE group, significantly shorter than (162.5±28.1)minutes and (31.5±7.4)days in laparotomy group ( P<0.05 or 0.01). There was no significant difference between the two groups referring to erythrocyte, hemoglobin and serum creatinine before operation and at postoperative 1 day (all P>0.05). There was no significant difference in ALT and AST between the two groups before operation (all P>0.05). TAE group showed ALT level of 1 154(884, 1 698)U/L, (975.3±400.9)U/L and (403.4±232.9)U/L at postoperative 1, 3, 7 days, significantly lower than 2 053(1 965, 2 132)U/L, (1 604.1±188.2)U/L and (915.3±160.5)U/L in laparotomy group (all P<0.05). TAE group showed AST level of (1 313.2±542.0)U/L, 525(302, 971)U/L and 174(84, 324)U/L at postoperative 1, 3, 7 days, significantly lower than (1 962.9±245.4)U/L, 1 478(1 089, 1 677)U/L and 837(674, 1 006)U/L in laparotomy group ( P<0.05 or 0.01). The complication rate was 26.9% (7/26) in TAE group, significantly lower than 59.1% (13/22) in laparotomy group ( P<0.05). Conclusion:For severe liver injury, TAE can significantly shorten operation time and length of hospital stay, accelerate the recovery of liver function and reduce the complication rate in comparison with laparotomy.

ABSTRACT

Antipsychotic medication is the primary treatment for schizophrenia, which is effective on ameliorating positive symptoms and can reduce the risk of recurrence, but it has limited efficacy for negative symptoms and cognitive dysfunction. The negative symptoms and cognitive dysfunction seriously affects the life quality and social function for the patients with schizophrenia. Currently, there is plenty evidence that antipsychotic drugs combined with adjuvant therapy drugs can effectively improve the negative symptoms and cognitive dysfunction. These drugs include anti-oxidants, nicotinic acetylcholine receptors and neuro-inflammatory drugs (anti-inflammatory drugs, minocycline), which show potential clinical effects.

ABSTRACT

Objective:To understand the status quo of perception control, self-management and cancer-related fatigue (CRF) in breast cancer patients receiving chemotherapy, and to investigate the impacts of perception control and self-management on CRF.Methods:In this study, 246 breast cancer patients undergoing chemotherapy from Shandong Cancer Hospital and Institute were investigated by means of cross-sectional survey. Cancer experience and efficiency scale was used to evaluate the patients′ perception control; cancer patients′ self-management scale was used to assess their self-management; and Piper Fatigue Scale was used to evaluate their CRF. Finally, the scores of CRF among the patients with different demographic data were compared, and the effects of perception control and self-management behavior on CRF were analyzed.Results:In univariate analysis, there were statistically significant differences of CRF scores resulting from different educational backgrounds ( F=3.392, P=0.019), sources of medical cost ( F=4.368, P=0.005), disease stages ( F=4.376, P=0.005), chemotherapy periods ( F=3.865, P=0.010) and courses of disease ( F=3.094, P=0.028). The differences in each dimension of perceived control cancer experience ( F=7.248, P=0.001), control efficacy ( F=96.595, P<0.001), self-management level of cancer patients ( F=65.009, P<0.001) and CRF ( F=130.973, P<0.001) were statistically significant. Cancer experience in perception control was positively correlated with CRF ( r=0.467, P<0.001); control effectiveness and self-management of cancer patients were negatively correlated with CRF ( r=-0.505, P<0.001; r=-0.564, P<0.001). Multiple linear regression showed that source of medical expenses (setting commercial insurance as the reference group), chemotherapy cycle (setting chemotherapy cycle ≥ 6 cycles as the reference group), cancer experience, control effectiveness, and self-management were entered in regression models, which could explain 55.5% of the total variation in CRF scores, and there was a significant linear relationship ( F=17.100, P<0.001). Conclusion:Medical staff should focus on CRF in patients at their own expense and in the 2-5 chemotherapy cycles. Cancer experience is positively correlated with CRF. Control effectiveness and self-management behavior are negatively correlated with CRF.

Objective@#To investigate the current status of picky eating among children and family-related factors, to provide scientific basis for diet intervention for children.@*Methods@#A total of 1 829 children were selected from 5 kindergartens in Shenzhen and investigated by electronic questionnaire survey from March 18 th-20 th in 2020.The data was analyzed by SPSS 21.0.@*Results@#The reported rate of picky eating behavior among children was 43.8%. Parents-picky eating behavior, children s preference to eating snacks, parental worries on children s appetite, parents attitude toward the food disliked by children were positively correlated to children s picky eating behaviors[OR(95%CI)=3.86(2.74-5.43), 1.44(1.15-1.80), 3.21(2.45-4.22), 2.21(1.65-2.95)]; however, eating various food under the instruction of their parents was negatively correlated to children s behaviors[OR(95%CI)=0.38(0.29-0.49)](P<0.05).@*Conclusion@#The reported rate of picky eating behaviors of children on holiday is lower than usual, but parents should to help them develop good dietary habits, prevent picky eating among children.

ABSTRACT

The negative symptoms and cognitive symptoms of schizophrenia patients are still clinical problems to be solved. Schizophrenia patients are abnormal in oxidative stress, immune regulation, and anti-histone deacetylase (HDAC), while sulforaphane plays a role in anti-oxidative stress, anti-inflammation, and anti-HDAC. Therefore, the sulforaphane could improve the negative symptoms and cognitive deficits of schizophrenia.

ABSTRACT

Bone marrow mesenchymal stem cells (BMSCs) have a strong self-renewal ability and multi-directional differentiation potential, so they are ideal cells for cell transplantation, and are widely used in the treatment of diabetes, osteoarthritis and other diseases. BMSCs could increase insulin and C peptide levels in patients by a variety of mechanisms, and a good blood glucose control could be achieved without exogenous insulin therapy. Recently, a number of clinical studies confirmed the safety and effectiveness of BMSCs transplantation in the treatment of diabetes. BMSCs transplantation has also achieved good results for treating complications of diabetes, such as diabetic nephropathy, diabetic peripheral vascular disease, diabetic cardiomyopathy, diabetic bone disease and others. However, due to the lack of standardized transplantation regimens, BMSCs transplantation has been limited in the clinical application. This review summarized the status and research progress of BMSCs transplantation in the treatment of diabetes mellitus and its complications.

ABSTRACT

Objective To establish a SYBR Green real-time PCR detection method with tissue-specific miRNAs and explore a novel approach for forensic body fluid identification. Methods The frequently reported 6 standard miRNAs were synthesized to establish a SYBR Green method, and verify with body fluid. The relative expression data for the 6 miRNAs were obtained using SYBR Green real-time PCR method in peripheral blood, menstrual blood, saliva and semen. Results The assays showed that miRNA205 permitted the unequivocal identification among different fluids. miRNA451 and miRNA144 could be used to distinguish blood from non-blood. Menstrual blood or peripheral blood could be identified through miRNA214. miRNA888 and miRNA891 was highly expressed in semen. Conclusion The results of this study indicate that miRNA SYBR Green profiling may provide a feasible and effective approach to body fluid identification for forensic casework.

ABSTRACT

The efficacy for rectal medication of methyl carprost suppository combined with oxytocin in preventing and controlling hemorrhage after cesarean section is observed and analyzed. The 180 parturients who are conducted cesarean


section in our hospital are selected as research subjects. All parturients are entitled to the right to know and randomly divided into research group and control group with 90 cases in each group. Among them, the parturients in control group are given oxytocin to prevent and control hemorrhage during the operation, while the research group combines with rectal medication of methyl carprost suppository to prevent and control postoperative hemorrhage base on the ways of control group. The condition of hemorrhage in two groups is observed and compared. By comparing with the time of the third stage of labor in two groups, the results don’t demonstrate significant difference [p>0.05]. Compared with the amount of hemorrhage during the operation, 2h and 24h after delivery, the amount of hemorrhage in research group is less than control group with obvious difference [p<0.05]; through recording the change of blood pressure before and after medication in two groups, the result shows that there is no significant difference [p<0.05], which indicates higher safety. In preventing and controlling hemorrhage after cesarean section, rectal medication of methyl carprost suppository combining with oxytocin can achieve good efficacy, significantly reduce the amount of postpartum hemorrhage and will not affect the blood pressure index

ABSTRACT

<p><b>OBJECTIVE</b>To explore the formation of pre-metastatic niche in the mouse lung and to study the underlying molecular mechanisms whereby primary breast carcinoma-derived factors mediate recruitment of bone marrow-derived cells (BMDCs) and affect the formation of pre-metastatic lung environment before the arrival of tumor cells.</p><p><b>METHODS</b>Mammary carcinoma 4T1 cells were inoculated into the mammary gland to construct mouse model of breast cancer. Confocal microscopy was used to detect the recruitment of BMDCs in the pre-metastatic lungs. The expression of factors in the mouse sera and 4T1 cell culture media was assayed using RayBio Custom mouse cytokine antibody array kit. The mice were injected daily with recombinant VEGF for 7 consecutive days to observe the effect of VEGF on BMDCs recruitment in the mouse lung.</p><p><b>RESULTS</b>No BMDCs were observed in the lungs of control and 4T1-tumor-bearing mice on day 0. On day 7 and 14, clusters of BMDCs observed in the lungs of 4T1-tumor-bearing mice were 8.7±2.2/objective field and 48.8±3.2/objective field, respectively, significantly higher than those in the control mice (1.1±0.8/objective field and 3.1±1.7/objective field) (P<0.05 for both). Confocal microscopic observation found that metastatic breast cancer cells preferentially facilitate BMDCs recruitment sites in the pre-metastatic mouse lungs. The levels of VEGF, GM-CSF, and IL-6 in the serum of 4T1-tumor-bearing mice were significantly increased compared with those in the control group (P<0.05 for all). However, VEGF was detected only in the culture media of 4T1 cells. The amount of BMDCs in the mouse lung tissue was (22.8±3.6)/objective field in the VEGF group and (3.1±0.4)/objective field in the control group (P<0.05). There were 36.8±5.4 metastatic foci in the lung tissue of VEGF group and 12.6±2.2 in the control group (P<0.05).</p><p><b>CONCLUSIONS</b>The results of this study demonstrate that primary breast cancer cells can alter the lung microenvironment during the pre-metastatic phase and induce the formation of pre-metastatic niche. Primary tumor cell-derived VEGF may be a crucial factor responsible for the formation of pre-metastatic niche.</p>

CD40-CD40 ligand (CD40-CD40L) cross-linking,an important cell signaling transduction pathway in immune response,is involved in the regulation of cellular and humoral immune.CD40-CD40L signaling pathway abnormalities can lead to immune system disorders and incur immune pathological reaction.A variety of common pediatric diseases,such as immunodeficiency diseases,inflammatory diseases and autoimmune diseases,are associated with CD40-CD40L signaling transduction pathway abnormalities.The relationship between CD40-CD40L and the above-mentioned diseases is of clinical significance.
