ABSTRACT

OBJECTIVE:To provide refere nce f or the quality control and evaluation of rice-wine processed Coptidis Rhizoma decoction piece. METHODS :Taking 17 batches of rice-wine processed Coptidis Rhizoma decoction piece from different manufacturers as samples ,HPLC method was adopted to determine the contents of 4 kinds of alkaloids as epiberberine ,coptisine, palmatine and berberine. The compound weights of epiberberine ,berberine,palmatine and berberine were calculated by the subjective and objective combination weighting method (AHP combined with variation coefficient ). Then the quality evaluation method was used to evaluate the quality of decoction pieces combined with the appearance of decoction pieces and the contents of 4 alkaloids. The percent mass constant was calculated and the grade of rice-wine processed Coptidis Rhizoma decoction piece was classified. RESULTS :According to the results of content determination of 4 kinds of alkaloids ,among 17 batches of samples ,a total of 13 batches of samples met the requirements of 2015 edition of Chinese Pharmacopoeia (part Ⅳ). Mass constants of 13 batches of qualified samples were 10.03-26.96,and the percent mass constants were 37.20%-100%. If the percent mass constant ≥ 80% of the sample was listed as the first-class product ,the sample with the percent mass constant between 50%-<80% was classified as the second-class product ,and the rest was listed as the third-class product ,therefore the quality constant of first-class product was ≥21.57,that of second-class product was 13.48-<21.57,and that of third-class product was <13.48. According to the grading standard ,3 batches of 13 batches of qualified samples are classified as first-class products ,6 batches are classified as second-class products ,4 batches are classified as third-class products. CONCLUSIONS :The established subjective and objective combination weighting method and quality constant method can more scientifically and reasonably classify rice-wine processed Coptidis Rhizoma decoction piece.

ABSTRACT

Artemisinin is the antidote to malaria, and has made hundreds of millions patients get rid of the disease since application in clinic. At present, the main accesses to artemisinin are directly extracted from Artemisia annua,chemical synthesis and biosynthesis. By comparing the differences of artemisinin production process between China and abroad, this research analyzes the reasons of the problem, and put forward their views and suggestions, so as to provide references for Artemisia annua further research.

ABSTRACT

Objective To analyze clinical features and prognosis of primary intestinal T cell lymphoma (ITCL)which was misdiagnosed as Crohn′s disease (CD),and summarized the key points of differentiation between ITCL and CD.Methods From January 2003 to January 2014,clinical data of patients with ITCL once misdiagnosed as CD were retrospectively analyzed,which included demographic,clinical,pathological and prognostic data.The data of 177 patients diagnosed as CD from January 2012 to January 2014 were collected. The demographic,clinical,pathological and prognostic data of these two groups were analyzed and compared. The continuous variables were compared with t test or Mann-WhitneyU test,and the differences of classification variables between two groups were analyzed by Chi-square test or Fisher exact probability method.Results A total of 18 patients (17 males and one female)with ITCL misdiagnosed as CD were enrolled in the study,and the median age at diagnosis was 38.5 (28.8 to 42.5)years and the median duration of diagnosis was 6.00 (3.75 to 13.25)months.The common primary symptoms were abdominal pain (12/18),diarrhea (13/18)and anemia (13/18).Intestinal perforation was primary symptom in two cases (2/18).However,B symptoms of lymphoma was observed in 16 patients,which included fever in 13 patients,weight loss in 16 patients and night sweat in one patient.One or more serious complications appeared in 12 patients,which included intestinal perforation in nine patients,severe gastrointestinal bleeding in seven patients and intestinal obstruction in two patients.In 177 patients with CD,104 patients were male (58.8%),and the median age at diagnosis was 22.0 (18.0 to 29.0) years.The primary symptoms were abdominal pain (88.7%,157/177),diarrhea (55.9%,99/177),anemia (63.8%,113/177),fever (33.3%,59/177)and weight loss (59.9%,106/177).During the disease course,30 patients (16.9%)had intestinal perforation (mainly chronic),12 patients (6.8%)had intestinal obstruction and seven patients (4.0%)had severe gastrointestinal bleeding.Compared to CD patients,male patients were more common in ITCL (χ2 =8.837,P <0.01),age at diagnosis was older (χ2 =314.5,P <0.01),the disease course was shorter (U=385.0,P <0.01),weight loss (χ2 =5.867,P <0.05)and fever (χ2 =10.609,P <0.01)were more common in clinical symptoms and intestinal perforation and severe gastrointestinal bleeding were more common in complications (χ2 =9.185,24.908,both P <0.01).The lesions of ITCL were multiple lesions, small bowel involved in eight cases,colon involved in 14 cases and one case with esophagus involved.Under endoscopy examination,most lesions appeared as ulcerations and were segmentally distributed.Compared to CD, lymphocyte proliferation was more common in the intestinal histopathological findings of ITCL (17/18 vs 19.7%(35/177);χ2 =42.844,P <0.01)and granuloma was rare (0 vs 42.8%(76/177),χ2 =12.665,P <0.01). Among 18 patients with ITCL,nine received chemotherapy and the median survival time was two months. Conclusions Primary ITCL had non-specific symptoms and was easily misdiagnosed as CD.More attention should be paid to the differential diagnosis of the two disease.

ABSTRACT

OBJECTIVE:To establish the fingerprint of Artemisia apiacea to identify common model and evaluate the quality coherence of the commercially available A. apiacea decoration pieces in Chongqing. METHODS:HPLC method was performed on the column of Waters XTerra C18 with the mobile phase consisted of methanol-0.1% phosphoric acid(gradient elution)at the flow rate of 1.0 ml/min. The detection wavelength was 220 nm,column temperature was 30 ℃ and the sample size was 10 μl. The simi-larity of 12 batches of samples was evaluated by TCM Chromatogram Fingerprint Similarity Evaluation System(2004A edition);a common model of fingerprints was identified by principal component analysis(PCA)and cluster analysis(CA). RESULTS:Total-ly 11 batches of samples were screened to establish common model of fingerprints and 16 common peaks were obtained,among which,3 common components were identified;RSDs of precision,stability and reproducibility tests were lower than 2.53%. The similarity of 11 batches of A. apiacea and reference chromatogram was more than 0.990. CONCLUSIONS:The method is stable and easy,and can provide reference for the scientific quality evaluation and control of A. apiacea;the quality of commercially available A. apiacea decoration pieces is generally good,however,there is still a little number of batches with poor quality. The production procedures and supervision of A. apiacea decoration pieces should be strengthened.

ABSTRACT

Objective To inspect the efficacy and mucosa healing condition of infliximab with azathioprine combination therapy in Crohn's disease (CD) and its correlation with prognosis.Methods A total of 20 active CD patients who received infliximab and azathioprine combination therapy at The First Affiliated Hospital of Sun Yat-sen University were objects of this study.The clinical efficacy was evaluated at 10 weeks,30 weeks,54 weeks and 2 years respectively according to CD activity index.The efficacy was evaluated under endoscopy at 10 weeks,30 weeks,54 weeks and 2 years respectively according to mucosal response situation under endoscopy.The data were analyzed by analysis of variance or Fisher's exact test between two groups.The factor affecred mucosal healing was analyzed by Logistic regression analysis.Results The clinical remission rate of patients without steroid at week 10,30,54 and 2 year were 12/20,16/20,15/20 and 15/20 respectively.Mucosal healing rate at week 10,30 and 54 weeks were 8/20,12/20 and 10/20 respectively.Logistic regression analysis indicated that age was the only factor affected mucosal healing at 30 weeks (OR =0.774,95% CI:0.630 to 0.950).There was significant differences in clinical remission between mucosa response patients and invalid under endoscopy at 30 weeks and 2 years without steroid (at 30 weeks:14/14 vs 2/6; at 2 years:14/14 vs 1/6; all P<0.01).Infliximab were withdrawn in 4 of 16 patients who was in non-steroid clinical remission at 30 weeks,and the other 12 patients were continued with infliximab therapy.There was no significant difference in non-steroid clinical remission rate (4/4 vs 11/12) and mucosa healing rate (2/4 vs 7/12) between withdrawal and continue of infliximab therapy (all P>0.05).Conclusions Infliximab with azathioprine combination therapy can effectively promote and maintain mucosa healing in CD.The mucosa response patients can maintain long time non-steroid clinical remission.

ABSTRACT

Objective To evaluate the efficacy and safety of azathioprine (AZA) in long term treatment of patients with active Crohn's disease (CD) in China. Methods Sixty patients with active CD,who needed to be treated with systemic steroids, were recruited. All patients initially received AZA combined with steroids therapy and AZA was maintained for treatment after withdrawal of steroids. Clinical efficacy, endoscopic healing of mucosa and adverse events were assessed at the end of the 12th, 24th, 48th, 72th and 96th weeks. Results The complete remission (CR) of the patients at the 12th, 24th, 48th, 72th and 96th weeks was 55.0%, 66. 7%, 61. 7%, 53. 3% and 53. 3%,respectively. Endoscopic examination was performed in 25 patients before treatment and at the end of the 48th week. Eight of them achieved mucosal healing that was kept to the end of 96th week (8/8).Whereas only 9 out of 17 patients without mucosal healing achieved CR at the end of 96th week (9/17,P=0. 026). The clinical features were compared between CR group and non-CR group at the end of 48th week. Logistic regression analysis showed that regaining of hs-CRP was the only independent factor for maintaining remission by AZA treatment ( P= 0. 009,OR 10.1,95 % CI 1.8 ~ 57.9). Sixteen patients (26.7 % ) had adverse events. Ten (16.7 % ) of them had to halt treatment because of serious adverse events. Leucopenia was the most common adverse event and could be occurred at any time during the treatment. Conclusion AZA combined with steroid therapy can effectively induce remission of active CD. Long term steroid-free remission is also effectively maintained by AZA treatment. The most common adverse event is leucopenia and some patients can get mucosal healing. Those who get mucosal healing may have longer duration of remission.

ABSTRACT

Objective To evaluate the effect and mechanism of 5-aminosalicylic acid (5-ASA) on bone marrow suppression caused by thiopurines, and to explore the proper dosage of thiopurines when combined with 5-ASA for inflammatory bowel diseases (IBD) patients.MethodsThe clinical data of IBD patients who took thiopurines were retrospectively analyzed. Thiopurine methyltransferase (TPMT) activity and 6-thioguanine nucleotide (6-TGN) concentration were tested.In prospective study, patients firstly treated with azathioprine (AZA) of 50 mg/d for 4 weeks, then combined with 5-ASA of 3 g/d for another 4 weeks.The concentration of 6-TGN in red blood cells (RBC) was analyzed at the end of 4th and 8th week.Results In retrospective study, there were 45 cases in AZA/6-mercaptopurine (MP) combined with 5-ASA group, 94 patients were in AZA/6-MP.alone group.The incidence of bone marrow suppression in these two groups were 46.7% and 16.0%, respectively.Multivariates regression analysis indicated co-administration of 5-ASA was the only risk factor of increasing bone marrow suppression incidence (OR=3.45,95% CI 1.31 ~ 9.04).There was no significant difference of TPMT activity between AZA/6-MP combined with 5-ASA group and AZA/6-MP alone group(t=-0.351 ,P=0.734).The 6-TGN concentration was significantly higher in AZA/6-MP combined with 5-ASA group than that of AZA/6-MP alone group (the median concentration was 384.9 pmol/8× 108 RBC and 286.4 pmol/8× 108 RBC,F=29.15,P=0.00).Prospective study was completed in 8 patients.After treated with AZA of 50 mg/d for 4 weeks, the 6-TGN concentration of 7 patients was lower than 230 pmol/8 × 108 RBC.After added with 5-ASA of 3 g/d for another 4weeks, the 6-TGN concentration of 7 patients was over 230 pmol/8 × 108 RBC, three patients of those was even higher than 420 pmol/8 × 108 RBC, and bone marrow suppression occurred in 2 patients.ConclusionsThe incidence of bone marrow suppression increased in Chinese IBD patients treated with recommended routine dossage of AZA/6-MP when conbined with 5-ASA.The mechanism may be related with the increased concentration of 6-TGN in RBC.To reduce the AZA dosage may possibly keep the efficacy while decrease the incidence of bone marrow suppression.

ABSTRACT

Objective To investigate the evaluation standard and proper time point of anti-tuberculosis trial for differential diagnosis between intestinal tuberculosis (ITB) and Crohn's disease (CD). Methods Clinical data and endoscopic changes of 28 patients with confirmed ITB and 11 with confirmed CD,who underwent anti-tuberculosis trail, were retrospectively analyzed. Results No significant difference could be found in clinical characteristics of ITB and CD patients on baseline, such as active pulmonary tuberculosis, strong positive skin test and anal fistula/perianal abscess. Clinical symptoms were relieved in both groups right after anti-tuberculosis treatment. After 3 months of treatment, the no-improvement rate in ITB group was 0, whereas that of CD group was 27.3% (P =0. 004). The disappearance rate plus improvement rate of ulcer in ITB group was 90. 9% (20/22) plus 9. 1% (2/22) and 100% ( 28/28 ) plus 0 at 3 and 6 months of treatment, respectively. The disappearance rate plus improvement rate of nodular lesion was 58. 8% (10/17) plus 41.2% (7/17) and 76. 5% (13/17) plus 23.5% (4/17), respectively. There was no obvious improvement of active ulcer or nodular transformation in CD group at any time point ( P < 0. 01 ).Conclusion With deficiency of special index for differential diagnosis of ITB and CD, some cases hard to differentiate still have to accept anti-tuberculosis treatment. Three months of anti-tuberculosis treatment is a proper time point to evaluate the efficacy. Disappearance of active ulcer and nodular transformation, together with cure or obvious improvement in clinic are taken as effective for treatment trail.

ABSTRACT

[Objective] To investigate the change of liver histology, proliferation of BDEC, and expression of TGF-β in different stages of liver chelestusis. [Methods] ①Rat cholestatic livers were induced by common bile duct ligation (BDL) and separated into 3 groups, namely control group (DO), 7 days after BDL group (DT), and 18 days after BDL group (D18). ② Histological changes of livers in different groups were evaluated based on Knodell HAI score. ③ Real-time PCR was used to detect the expression of TGF-β in liver tissue and isolated BDEC in different groups. ④ Statistically analyzing the correlation between Knedell HAl score and the levels of TGF-β_1 mRNA. ⑤ In vitro study was performed to investigate the effect of TGF-β_1 on an immortalized mouse intrahepatic bililary epithelial cell line (mIBEC). [Results] (I) Knedell HAI score and the proliferation of intrahepatic bile ducts increased as the liver cholestasis aggravated. ② The levels of TGF-β_1, TGF-β_2, and TGF-β_3 mRNA were significantly up-regulated in liver tissues and BDEC as the liver cholestasis aggravated. ③ Positive correlation was found between Knedell fibrotic score and the levels of TGF-β_1 mRNA in liver tissues and BDEC(r=0.9376, P<0.05 and r=0.9682, P < 0.01). ④ In vitro study showed that TGF-β_1 inhibited the proliferation of mIBEC. [Conclusion] ① Liver injury, biliary proliferation, and the levels of TGF-β mRNA expression increased as liver cholestasis aggravated. ② The interaction of TGF-β_1 and BDEC plays an important role in the pathogenesis of BDL induced cholestatic liver disease. ④ Up-regulated expression of TGF-β_1 mRNA in the proliferated BDEC participates in the formation of BDL induced cholestatic liver fibrosis.

ABSTRACT

<p><b>OBJECTIVE</b>Study the effect of self-microemulsifying drug delivery system(SMEDDS) on the solubility and absorption of tanshinones to guide the selection of composition of tanshinone SMEDDS.</p><p><b>METHOD</b>The solubility of tanshinones in the solution of SMEDDS was determined by UV-spectrometer and the absorption of tanshinone SMEDDS was determined by HPLC as the detection method.</p><p><b>RESULT</b>The solubility of tanshinones in solution of SMEDDS was 10 times in water and 2.5 times in micelle solution. The solubility of tanshinones in solution of SMEDDS was increased with the increasing of oil (MCT) in composition of tanshinone SMEDDS. The absorption constants (Ka) in SMEDDS and micelle solution was 0.479 h(-1) and 0.326 h(-1) respectively, and the absorption half life (t1/2) was 1.44 h and 2.12 h respectively. The absorption was increased with the oil increasing in composition of tanshinone SMEDDS.</p><p><b>CONCLUSION</b>SMEDDS can increase the solubility and absorption of tanshinones significantly and the increasing of oil content (MCT) in SMEDDS composition promote the dissolution and absorption of tanshinones.</p>

ABSTRACT

Aim: To provide the foundation for reasonable utilization by analyzing the essential oils of Sabina pingii var. wilsonii.. Methods: The essential oils were extracted using steam distillation and separated with GC capillary column. The components were quantitatively determined with normalization method, and were preliminarily identi-fied by GC-MS. Results: 79 Components were identified, which took up 93. 78% of the essential oils. Conclusion: The main components of essential oils were a-terpineol( 16. 70%); α-fenchene( 14. 98%); 2,6,10-dodecatrien-1-ol, 3,7, 11-trimethyl-( 6.49%); 1, 4-cyclohexadiene, 1-methyl-4-[1-methylethyl]-( 4. 74%); kaura-5, 16-dien-18 [orl9]-ol(3. 62%); naphthalenemethanol, 1, 2, 3, 4, 4a, 5, 6, 7-octahydro-α, α, 4a, 8-tetramethyl-, [2R-cis]-(3. 04%); [ + ]-4-carene ( 2. 81%); D-limonene ( 2. 71%); β-myrcene ( 2. 59%); cedrol (2.40%); a-pinene (2. 36%); 7-isopropyl-1, 1, 4a-trimethyl-1 ,2,3,4,4a, 9,10,10a-octahydrophenanthrene(2. 32%).

ABSTRACT

Objective To characterize the clinical features and outcome of massive lower gastrointestinal hemorrhage in patients with Crohn's disease.Methods The clinical data form 13 patients Who identified as Crohn's disease accompanied by massive lower gastrointestinal hemorrhage between 1998 and 2007 were retrospectively analyzed.Results The ratio of men to women was 3.3 : 1.Bleeding was the first symptom of disease in foor patients.The lesion was involved in small howel in 10 cases.The origin of bleeding was identified in 8 cases.Four patients received surgical operation and none of them died,while 9 patients received medical treatment and 3 of them died.Totally 10 patients were followed up for 6 to 94 months.Eight patients had recurrent massive hemorrhage,and two of them subsequently required surgery.One patient died after surgery.All patients who had recurrent massive bleeding and still survival were characterized by recurrent bleeding as only or major clinical feature,and they were all with small bowel involvement.Conclusions Diagnostic and therapeutic approach of massive lower gastrointestinal hemorrhage in Crohn's disease is very challenging.It is possible that there exists a particular phenotype of Crohn's disease that presents a tendency of bleeding recurrence,whose bleeding probably related to small bowel lesions.

ABSTRACT

OBJECTIVE: To analyze the chemical constituents of the volatile oil from the stem and leaves of Sarcandra glabra.METHODS: The volatile oil was extracted from the stem and leaves of Sarcandra glabra by steam distillation.The components of the volatile oil were separated and identified by GC-MS.The relative content of each component was determined by area normalization.RESULTS: 29 components in the volatile oil from the stem of Sarcandra glabra were identified,and their content representing 75.88% of the total volatile oil.64 components in the volatile oil from the leaves of Sarcandra glabra were identified,with their content representing 87.98% of the total volatile oil.CONCLUSION: The volatile from leaves and that from stems of Sarcandra glabra varied in the content of low molecular weight of chemical constituents,almost no low molecular weight of chemical constituent contained in the volatile oil in the stems.These data provide scientific bases for exploitation of Sarcandra glabra.

OBJECTIVE:To establish a method for the determination of the content of total Flavonoid in Fructus Psoraleae by UV spectrophotometry.METHODS:The determination was performed using Bavachin standard as control substance,and sodium hydroxide as color-developing agent,with the wavelength set at 440nm.RESULTS:The absorbency and concentration of Bavachin have good linearity in the range of 4.10~ 12.30? g? mL-1(r=0.999 8).The average recovery rate was 100.5%(RSD=1.57%,n=9).CONCLUSION:The method is easy to operate,highly accurate,reproducible and can be applied to determine the content of total Flavonoid in Fufang Psoralen corylifolia sustained-release tablets.

ABSTRACT

Objective To investigate the effect of Fructus Psoraleae on the proliferation and differentiation of cultured osteoblast isolated from neonatal rat's calvarium.Methods Calvarium osteoblasts of new born SD rat were cultured and exposed to different concentrations of Fructus Psoraleae extract,then the content of alkaline phosphatase(ALP)in the cell was measured and the cell proliferation was detected by tetrazoline colorimetry(MTT).Results Ethyl acetate extract and alcohol extract from Fructus Psoraleae can improve the activity of ALP and the proliferation of rat's cultured osteoblasts,the optimum concentration was 2.0?10-3 mg/mL.Conclusion Ethyl acetate extract and alcohol extract from Fructus Psoraleae can stimulate the differentiation and proliferation of osteoblasts in vitro.

RESULTS: The result showed that there was good linear relation between the area and the concentration of psoralen、isopsoralen、isobavachin、bavachin、corylin、corylifolinin、bvachinin、psoraidin and bavachalcone within 0.4-152.0,26.4-132.0,13.6-66.0,27.2-136.0,31.2-156.0,48.8-244.0,50.4-252.0,36.0-180.0,(8.8)-44.0 ?g/mL.The average recovery of psoralen and bavachin is 98.7% and 97.6%. CONCLUSION: This method is convenient,reliable,and can be used to control the quality of Compound Buguzhi Sustained-Release Tablet.

ABSTRACT

AIM: To explore the optimal separation of the total flavonoid from Radix Puerariae by selecting appropriate macroporous resins and to study systematically the factors which affect separation. METHODS: Static and dynamic adsoption desorption methods were adopted, and evaluated for separating efficiency by measuring the concentration of total flavonoid in Radix Pueraria with UV spectrophotometer. RESULTS: The SP70 macroporous resin had the best separating efficiency when the flavonoid content in the liquid was 0.5g per 1mL equivalemt to raw material. pH5~6. the volume of drug 60BV (resin bed volume) with the adsorption power 2BV/h. and the volume of 70%(mL?mL -1 ) ethanol as eluant 4BV with desorption power 2BV/h. In result. total flavonoid's purity could reach 80%. CONCLUSION: This method is simple and feasible with good effect of separation, which can meet industrial requirements.

ABSTRACT

Objective: By studying the pharmacokinetics in dogs' vivo of Gansu Capsule (sustained release), evaluate slow release effect in vivo of Gansu Capsule (sustained release). Methods: To take gallic acid as marker component, which is one of effective components, After taking Gansu Capsule (sustained release) and Gansu Capsule (common), to adopt high performance liquid chromatography to determinate the blood concentration of acid pyrogallol in dogs at different time. Results: Both Gansu capsule (sustained release) and Gansu Capsule (common) coincide one compartment model. It suggested that Gansu Capsule (sustained release) had remarkable effects of slow release and could maintain higher blood concentration in vivo longer the slow release effect of Gansu Capsule (sustained release) in vivo that could be evaluated by accumulative releasing percentage in vito. Conclusion: Gansu Capsule (sustained release) had good slow release effect in vivo and vitro.

ABSTRACT

AIM: To establish the quality standard for Yanqingsong Granule (Radix P uerariae, Rhizoma Chuanxiong, Radix Paeoniae Alba, Radix Angelicae Dahuricae, et c.). METHODS: Radix Puerariae; Rhizoma Chuanxio ng; Radix Paeoniae Alba; Radix Angelicae Dahuricae. in Yanqingsong Granule were identified by TLC, and the content of puerarin was determined by HPLC. RESULTS: Radix Puerariae; Rhizoma Chuanxio ng; Radix Paeoniae Alba; Radix Angelicae Dahuricae could be identified by TLC. P uerarin showed a good linear relationship at a ran ge of 0.1716~0.8580?g,r=0.9999. The average recovery was 97.49% (RSD= 1.77% and n=6). CONCLUSION: The methods are available with a reproducibility and ca n control the quality of this granule effectively.
