ABSTRACT

Background Kunming is a plateau city with sufficient sunshine, high ultraviolet intensity, and strong radiation. In recent years, ozone (O3) pollution has gradually become the primary problem of air pollution in the city. Objective To evaluate the health effects of atmospheric O3 exposure on non-accidental deaths in Kunming. Methods The data of meteorological variables (average temperature, average relative humidity, average air pressure, and average wind speed), air pollutants (PM2.5, PM10, SO2, CO, and O3) and non-accidental deaths (NAD) of residents were collected in Kunming from 2017 to 2019. A generalized additive model was adopted to conduct time-series analyses on the current-day (lag0), single-day (lag1-lag3), and cumulative lag (lag01-lag03) effects of O3 on NAD; furthermore, hierarchical analyses by gender, age, and season (warm and cold) were conducted. Results The average concentration of O3-8h from 2017 to 2019 was (84.3±32.3) μg·m−3. For every 10 μg·m−3 increase in O3-8h concentration, the NAD risks of lag0, lag01, and lag02 of total population increased by 0.70% (95%CI: 0.11%-1.29%) 0.79% (95%CI: 0.14%-1.44%), and 0.75% (95%CI: 0.08%-1.43%), respectively; for women, the NAD risks of lag2 and lag02 increased by 0.80% (95%CI: 0.08%-1.53%) and 1.05% (95%CI: 0.09%-2.03%) respectively; for the residents over the age of 65, the associated NAD risks of lag0, lag01, and lag02 increased by 0.82% (95%CI: 0.16%-1.48%), 0.93% (95%CI: 0.20%-1.67%), and 0.96% (95%CI: 0.20%-1.73%), respectively; in the warm season, the NAD risks of lag0, lag01, and lag02 increased by 0.91% (95%CI: 0.12%-1.70%), 0.98% (95%CI: 0.12%-1.86%), and 1.00% (95%CI: 0.07%-1.93%), respectively; After introducing PM2.5, PM10, SO2, NO2, and CO to the model, the effects of O3 exposure level on resident’s NAD was not statistically significant. Conclusion An increase of O3 exposure level associates with an increase of NAD risk in residents, and there is a lag effect. Residents over the age of 65, women, and all residents in warm season may be more sensitive to O3 exposure.

ABSTRACT

Objective:To analyze the burden and benefit finding of illness among home caregivers of patients with cognitive impairment after stroke, so as to provide references for formulating targeted intervention strategies.Methods:Using the intentional sampling method, 15 home caregivers of patients with cognitive impairment after stroke treated in the First Affiliated Hospital of Air Force Medical University from July 2019 to December 2019 were selected as research objects. The semi-structured interview was conducted by phenomenological method, and the interview data were analyzed by Colaizzi analysis method.Results:The burden of home caregivers of patients with cognitive impairment after stroke was mainly manifested as excessive psychological pressure, impaired health, heavy financial burden, and separation from social life. The benefit finding of illness was mainly manifested in the respect for life, the improvement of health behavior ability, and the enhancement of family and social responsibility.Conclusions:Home caregivers of patients with cognitive impairment after stroke have both a negative sense of burden and a positive sense of benefit finding of illness. Medical workers should make a dialectical analysis to help resolve the burden of negative feelings, actively cultivate a sense of benefit finding of illness, and improve their behavioral ability of care.

ABSTRACT

Objective To investigate the effects of enteral nutrition support with different doses of glutamine on nutritional status and gastrointestinal function in elderly critically ill patients.Methods Totally 90 critically ill patients were randomly divided into three group:the control group without glutamine (group A),enteral nutrition with low glutamine [0.3 g/(kg· d)] (group B),and high glutamine [0.6 g/(kg· d)] groups (group C).Fasting blood samples were collected on days 1,7,and 14 after the initiation of study,and then the indicators including serum diamine oxidase,glutamine,transferin (TF),prealbumin (PA),hemoglobin (HGB),albunin,and nitrogen balance were determined.In addition,the gastrointestinal complications including bloating,diarrhea,and gastric retention were recorded.Results On the 7th and 14th days,the serum glutamine levels in groups B and C were (1 886.8 ±420.5),(2 228.6 ±440.2) μg/L;(1 889.6 ± 436.1),(2 174.3 ± 440.8) μg/L,respectively,which were significantly higher than those in group A [(1 612.0 ±493.5),(1 869.7 ±559.8) μg/L] (P =0.027,P =0.008).The serum diamine oxidase levels were also significantly lower in group B [(2 310 ± 1 271),(1 602.5 ± 1 137.9) U/L] and group C [(2 076.3 ±567),(1 586.3 ±530.9) U/L] than in group A [(3 250 ±923),(2 476 ±862) U/L] (P =0.000,P =0.000).The serum TF and PA levels were significantly increased on the 7th day (P =0.023,P =0.047),and the HGB was significantly higher on 14th day (P =0.003),The number of patients who had undergone successful implementation of enteral nutrition were 4 and 3 in groups B and C,and the number of patients who had achieved the target volume of enterral nutrition was 25 and 27 in groups B and C,which were significantly larger than those in group A (12 and 13,respectively) (P =0.008,P =0.000).The gastrointestinal complication was also significantly decreased after the administration of enterral nutrition with glutamine.Conclusions Enteral nutrition with glutamine can improve intestinal mueosa barrier and nutritional status in elderly critically ill patients.Thus,it helps to reduce gastrointestinal complications and transition time of total enteral nutrition.

ABSTRACT

Objective To observe the clinical curative effect of Semen Cassiae application on navel in treating essential hypertension. Methods Totally 1674 cases of hypertension, according to visiting sequence, were randomly divided into treatment group (864 cases) and control group (810 cases). The control group was given hydrochloride enalapril 10 mg orally, 2 times a day. The treatment group was treated with Semen Cassiae application on navel, replaced per 24 h. The two groups were treated with moderate exercise, reasonable diet, and the course was 6 months. The clinical symptoms, blood pressure, blood lipid and blood rheology of the two groups were measured before and after treatment. Results To compare the symptoms efficacy, total effectiveness of treatment group was 95.14%(822/864), the control group was 71.11%(576/810), there was a significant difference (P<0.01). To compare antihypertensive effects, the total effective rate of the treatment group was 95.60% (826/864), the control group was 73.46% (595/810), there was a significant difference (P<0.01). The change of blood pressure in the treatment group was significantly better than the control group (P<0.01). TC, TG, HDL-C, LDL-C and blood rheology of the treatment group was significant different between before and after treatment (P<0.01). Conclusion On the basis of western medicine, Semen Cassiae application on navel played a double effect of acupoint and drug. It is more effective in lowering blood pressure, improving the clinical symptoms, blood lipid and blood rheology indexes.
