ABSTRACT

Varicocele is one of the infertility factors in men and has negative effects on sperm parameters. Using varicocelectomy that is the most effective method for treatment of varicocele, there will be significant improvement in sperm parameters. The purpose of this study was to survey sperm parameters before and after varicocelectomy and fertility rate after surgery. This interventional study was done on 118 infertile couples from 2003 to 2004 that referred with male factor and varicoceles disease to Fatemeh Zahra infertility center. The sperm parameters were compared before and after varicocelectomy and the pregnancy results were compared with each other. The type of varicoceles was unilateral in 72 persons [61%] and bilateral in 46 [39%]. The mean number [ +/- SD] of sperm reached from 42.3 +/- 26.9 millions before surgery to 48.4 +/- 27.1 million after surgery. Also, sperm motility reached from 50.8 +/- 18 percent to 54.8 +/- 17.9 percent and round cells reached from 1.9 +/- 4.7 to 0.6 +/- 3.3 percent that this process is meaningful in each three cases [p<0.007]. The mean normal morphology of sperms showed an increase after surgery but wasn't meaningful. After varicocelectomy, the rate of pregnancy was seen in 42% of persons that increased to 48.3% in persons who used different methods of ART. According to the results of this study, varicocelectomy can increase sperm parameters. Also, with increasing normal sperm morphology, almost half of the operated persons experienced their fertility

ABSTRACT

Varicocele is one of the causes of male infertility. It changes with temperature and drainage of testicular veins that due to spermatogenesis disturbance and decrease of fertility in these patients. However varicocelectomy is a treatment for varicocele, but there are other methods of treating this disease including assisted reproductive technology [ART], Sperm processing and swim up. This interventional study was performed on 155 cases in fertility and infertility Center of Babol University of medical sciences in 2004. Sperm parameters in varicocele patients was assessed before and after sperm processing. The mean age of patients was 31.8 years. Sperm motility before sperm processing in grade III and IV was 53.9% and 12.9%, respectively but after sperm processing in grade III and IV changed to 78.7% and 56.8%, respectively. Total motility before sperm processing was 40% and changed after processing to 80%. The results show that with sperm processing and collection of appropriate sperms, we are able to increase fertility

ABSTRACT

Infertility as an obvious problem causes different social problems in family. One of the most common methods to solve the problem of infertile couples with azoospermia is capture via testis or epididymo biopsy. Use of anesthetic drugs is unavoidable for pain relief. Since these drugs can cause the reduction of sperm motility so it is necessary to determine the concentration of drugs that used for topical anesthetic. In this experimental study, semen samples of 30 men were studied. Post swim up samples [65ml] in Ham's F10 culture were put in six separated tubes and 10% of semen was mixed with xylocaine [20%, 40%, 60%, 80% and 100%]. Sample one was considered as control group and sperm motility and grading at 3 different times [0, 2, and 24 hours] were recorded. Data was statistically analyzed. Mean of sperm motility in control group was 63.20 +/- 5.31 and in test group decreased from 45.50 +/- 11.54 [xylocaine 20%] to 4.9 +/- 5.72 [xylocaine 100%] [p= 0.000]. Prolonged contact causes more decrease in motility and grading of sperm [p=0.001]. Increase of xylocaine concentration caused a decrease in sperm motility, so the effect of xylocaine on sperm parameters depends on dosage and duration of contact
