ABSTRACT

OBJECTIVE@#To explore the relationship between physiological parameters changes and severe heatstroke induced by 5-km armed cross-country training.@*METHODS@#A total of 521 male officers and soldiers from a special team who participated in the summer training of 5-km armed cross-country training from year 2016 to 2017 were enrolled. All trainees participated in 5-km armed cross-country training in high temperature and humidity environment of ambient temperature > 32 centigradeand (or) relative humidity > 65%. The trainees were divided into two groups according to the incidence of severe heatstroke in the course of training. The age, enlistment time, constitution score, body mass index (BMI), external environment (ambient temperature, relative humidity, wind speed, heat index) of trainees of the two groups, and the change rates of arterial blood oxygen saturation (SaO2), body temperature, pulse and blood pressure within 5 minutes after the 5-km armed cross-country training were compared between the two groups. The risk factors of severe heatstroke were screened by two classified Logistic regression analysis, and the predictive value of various risk factors of severe heatstroke was analyzed by the receiver operator characteristic curve (ROC).@*RESULTS@#In 521 trainees of 5-km armed cross-country training, 29 trainees suffered from severe heatstroke accounting for 5.57%. There was no significant difference in the age, enlistment time, constitution score, BMI, or external environment during 5-km armed cross-country training between severe heatstroke group and non-severe heatstroke group. Compared with those without severe heatstroke, the descending rates of body temperature, pulse, blood pressure and SaO2 increased rate within 5 minutes after 5-km armed cross-country training of severe heatstroke trainees were significantly decreased [temperature descending rate: (0.67±0.30)% vs. (1.43±1.28)%, pulse descending rate: (7.53±5.21)% vs. (13.48±8.07)%, blood pressure descending rate: (9.28±6.84)% vs. (19.42±7.73)%, SaO2 increased rate: (0.51±0.39)% vs. (1.50±1.43)%, all P < 0.01]. Two classification Logistic regression analysis showed that the temperature descending rate [odds ratio (OR) = 0.485, 95% confidence interval (95%CI) = 0.289-0.817], pulse descending rate (OR = 0.903, 95%CI = 0.845-0.965), blood pressure descending rate (OR = 0.841, 95%CI = 0.790-0.896), and SaO2 increased rate (OR = 0.421, 95%CI = 0.250-0.711) were the risk factors for severe heatstroke during 5-km armed cross-country training (all P < 0.01). ROC curve analysis showed that temperature descending rate [area under ROC curve (AUC) = 0.659, 95%CI = 0.604-0.714], pulse descending rate (AUC = 0.730, 95%CI = 0.762-0.900), blood pressure descending rate (AUC = 0.831, 95%CI = 0.659-0.801), SaO2 increased rate (AUC = 0.711, 95%CI = 0.655-0.767) could be used for the incidence of severe heatstroke prediction during 5-km armed cross-country training (all P < 0.01), and the predicted value was the same.@*CONCLUSIONS@#Under the same conditions, the severe heatstroke during 5-km cross-country training is closely related to the descending rates of body temperature, pulse, and blood pressure as well as SaO2 increased rate within 5 minutes after the training, whose predictive values for severe heatstroke were the same.

ABSTRACT

Objective Bone defect repair remains a challenge in regenerative medicine, which has triggered a research upsurge on improving the bone repairing effect using heterogeneous bone combined with growth factors.The aim of this study was to evaluate local bone formation following surgical implantation of coralline hydroxyapatite (CHA), a heterogeneous bone, in combination with the concentrated growth factor (CGF).Methods This randomized prospective study included 24 New Zealand rabbits, which were equally divided into a CHA/CGF+CHA, a CHA/CGF+autograft and a CHA+autograft group.A defect 10 mm in diameter was made in the parietal bone of each animal and filled with CHA/CGF, CHA or autograft.At 6 and12 weeks after the operation, we observed the bone formation by micro-CT and histological examination.Results The bone volume (BV) was significantly higher in the CHA/CGF+CHA than in the CHA+autograft group both at 6 weeks ([39.00±7.61] vs [32.12±6.55] mm3, P<0.05) and at 12 weeks after the operation ([49.75±2.36] vs [39.45±7.02] mm3, P<0.05), and so was the bone mineral density (BMD) ([308.30±29.82] vs [256.85±151.25] mg/mL, P<0.05;[389.00±31.87] vs [302.53±127.05] mg/mL, P<0.05).Histological examination showed that the new bone was distributed throughout the CHA scaffold in the CGF/CHA group at 6 weeks, and the new bone was observed only in the periphery region of the CHA scaffold in the CHA group.The bone defects in the CGF/CHA group were fully repaired at 12 weeks, while those in the CHA group were partly repaired with bone and fibrous tissue in the central region of the defects.Conclusion Combination of CHA with CGF could effectively enhance bone healing.CHA/CGF compound artificial bone is an ideal substitute in bone transplantation.

ABSTRACT

Objective This study was to evaluate the clinical outcomes of orbital blowout fracture repair using the individual titanium mesh bent by computer-aided rapid prototyping . Methods The CT imaging data of 11 cases of orbital blowout fracture were analyzed.The subciliary approach was used for the exposure of the fractures .An appropriate 3D-printed titanium mesh pre-bent by computer-aided rapid prototyping was selected and implanted according to the characteristics of a given defect .The surgical results were analyzed by evaluating diplopia , eyeball movement , enophthalmos , and the position of the titanium mesh and comparing the orbital vol-ume before and after surgery . Results All the operations were successfully accomplished .No remolding or trimming was needed for the pre-bent titanium mesh and implanted titanium mesh was tightly fixed to the orbital wall .The patients were followed up for 3 -12 months, which revealed no symptoms of diplopia in any of the patients . Volumetric analysis showed a significantly reduced orbital volume difference from (2.60 ±0.43) mL preoperatively to (-0.07 ±0.62) mL postoperatively (P<0.01).Enophthalmos was desirably correc-ted, with the enophthalmos difference decreased from (2.35 ±0.81)mm preoperatively to (-0.10 ±0.52) mm postoperatively (P<0.01).No extra-ocular muscle limitation was observed . Conclusion The individual titanium mesh bent with computer-aided rapid prototyping technology can be applied to orbital blowout fracture repair , which may achieve a high success rate of anatomic restoration of the orbital volume and effective prevention of enophthalmos .

ABSTRACT

Objective Carotid body tumor is closely related to carotid artery .How to deal with carotid artery is the guaranty of successful surgery .The aim of this study was to summary the experience of keeping carotid artery continuity in the treatment of 15 carotid body tumor cases . Methods Clinical data of 15 cases with 17 carotid body tumors , in which 2 patients had bilateral tumors , were collected.The 17 carotid body tumors were classified as Shamblin I (6 cases),Ⅱ(6 cases) andⅢ(5 cases).Fifteen tumors were dissected simplely.Among these 15 tumors, three of Shamblin Ⅱand two of Shamblin Ⅲunderwent repair of the break of carotid artery.The rest two of Shamblin Ⅲunderwent tumor and external carotid artery excision and internal carotid artery repairment .Matas pressing test of carotid artery or carotid artery balloon occlusion test was not performed in any patient . Results All 17 tumors were successfully excised .None of the 17 tumors underwent anastomosis or reconstruction of internal carotid artery or arteria carotis com -mun.The continuity of carotid artery or internal carotid artery was kept in all the patients after tumor excision .Hoarseness and bucking induced by the injury of vagus nerve occurred in 13 cases, and bucking recovery in all the cases after 2 years follow up, and only one case remained mild hoarseness .Hypoglossal nerve injury occurred in 4 cases with tongue deviation , and the symptom recovery after 6 months follow up.No one had recurrent by the follow-up period ranging from 1 to 9 years. Conclusion Most carotid body tumors could be completely resected by tumor dissection method with the accurate treatment , thereby the continuity of carotid artery could be kept.The Matas pressing test of carotid artery or carotid artery balloon occlusion test is not necessarily to every patient .

ABSTRACT

Objective Parotidectomy and superficial parotidectomy are traditional surgical treatment to the malignant parotid gland tumour although they have many defects.The aim of this study was to analyze the feasibility of individualized treatment to patient according to tumor`s size and TNM stage. Methods Thirty three patients with low grade malignant parotid gland tumours from Dec 2005 to Dec 2010 were recruited in the present study.All the patients received the territorial surgical resection firstly.Then, 9 cases re-ceived the expanded territorial surgical resection (1 case had recurrent probability, accepted adjuvant radiotherapy).Sixteen cases un-derwent parotidectomy with complete tumor resection ( 2 case had envelope invasion, accepted adjuvant radiotherapy) .Eight cases ac-cepted adjuvant radiotherapy(4 cases on T1 and T2 stage preserved facial nerve although the nerves adhering to tumors, and other 4 ca-ses on T4a stage accepted facial nerve excision ) . Results Follow up ranged from 3.5 to 8.5 years and no recurrences were docu-mented.9 patients who accepted territorial surgical resection showed fine facial symmetry.8 cases of them without adjuvant radiotherapy had good salivary secretion.Nine cases of other 24 patients showed temporary facial nerve injury and recovered within 1 to 4 months. Four cases with facial nerve excision showed permanental facial paralysis. Conclusion To protect the function of parotid gland to the greatest extent, the small low grade malignant parotid gland tumors without facial nerve invasion could have territorial surgical resection. Combined territorial surgical resection with postoperative adjuvant radiation therapy is useful to avoid the tumors recurrence and en-croachment of facial nerve to protect the facial nerve function.

ABSTRACT

<p><b>OBJECTIVE</b>To construct and identify a lentiviral vector of RNA interference targeting human Notch-1 gene.</p><p><b>METHODS</b>To determine the Notch-1 gene sequences, three RNAi target sequences (shRNA1-3) were designed in accordance with the RNAi sequence design principles and cloned into the lentiviral vector pLenOR-THM by endonuclease BamH I restriction, EcoR I double digestion, and T4 DNA-ligase ligation. After the transformation into competent DH5alpha bacteria, the candidate clones were identified by Kpn I and EcoR I double digestion and DNA sequencing. The recombinant and three packaging plasmids were co-transfected into human embryonic kidney cell line 293T cells by lipofectamine to produce the lentiviral particles. The viral titer was determined. The 293T cells were infected by the lentiviral particles obtained, and transfection efficiency was assessed using a fluorescent microscope. The lentiviral vector particles were also transfected into ACC-M cells. The Notch-1 expression in the transfected cells was assayed by quantitative reverse transcription polymerase chain reaction (QRT-PCR) and Western blot analysis.</p><p><b>RESULTS</b>The lentiviral RNAi vector pLenOR-THM-Notchl for Notch-1 gene was constructed successfully. Strong green fluorescence was observed in the 293T cells under fluorescent microscope after co-transfection of the cells with the four plasmids of lentiviral vector. The virus in the supernatant reached a titer of 5.8 x 10(8) TU x mL(-1). The transfection efficiency of the collected virus exceeded 90% in 293T cells with 1 as a multiplicity of infection. The third lentiviral vector was found to significantly inhibit the Notch-1 expression at the mRNA and protein levels.</p><p><b>CONCLUSION</b>The lentiviral RNAi vector of Notch-1 has been successfully constructed and identified.</p>

ABSTRACT

Objective:To explore the effect of damage control surgery on polytraumatism with severe oral maxillo-facial trauma.Methods: We retrospectively analyzed 32 cases of polytraumatism with severe oral maxillo-facial trauma treated by damage control surgery.Results: The principles of damage control surgery were successfully applied to the treatment.Of the 32 cases,31 survived,with their polytraumatism sequentially managed,and only 1 died.Conclusion: Damage control surgery helps to raise the success rate in the treatment of polytraumatism with severe oral maxillo-facial trauma.

ABSTRACT

Platelet rich plasma is an autologous concentration of platelet that has a 4 to 5-fold or more increase over baseline platelet number,and it is a concentration of the growth factors proved to be actively secreted by platelets to initiate all wound healing.PRP enhances bone marrow stromal cells and osteoblasts proliferation in vitro.PRP combined with ?-tricalcium phosphate,bioceramic or autogenous bone enhance the bone healing considerably.PRP is effective in accelerating bone regeneration for the treatment of periodontal defect after surgical extraction of an impacted mandibular third molar.PRP combined with autogenous bone,bovine-derived xenograft or hydroxyapatite improve the osteogenesis of alveolar bone grafting in alveolar clefts or periodontal intrabony defect patients.Culture-expanded bone marrow stromal cells and autologous PRP injected into the distracted callus can shorten the treatment period by acceleration of bone regeneration during distraction osteogenesis.PRP works via the degranulation of the ? granules in platelets,which contain the synthesized and prepackaged growth factors.Little benefit from PRP may be due to use of damaged platelets or non-critical-sized bone defect models.The value of PRP is its proven effectiveness,its safety,its cost effectiveness,and its availability in an easy-to-develop manner.

ABSTRACT

Objective:To summarize the experience of reconstruction of mandibular defects with vascularized fibular osteocutaneous flap.Methods:Segmental mandibular resection and immediate reconstruction with vascularized fibular osteocutaneous flap were performed in 8 patients with tumor.The fibula flaps were cut into 2-4 segments and bent to the contour of the mandibles,and they were fixed precisely by the titanium plate.The skin flaps were used for reconstruction of soft tissue defects or as observation windows of the blood circulation.Results:In all 8 patients,transplanted vascularized fibular osteocutaneous flaps survived well,and restoration of mandibular contour and function was satisfactory. Conclusion:The fibular osteocutaneous flap can be cut into segments and shaped according to location and contour of mandibular defect.Vascularized fibular osteocutaneous flap may be an ideal selection for the reconstruction of mandibular defect.

ABSTRACT

Objectives: To study the histopathological grading and the clinical-staging system on the surgical prognosis in patients with mucoepidermoid carcinoma(MEC) of the salivary glands.  Methods: Analysis were made to the primary sites, clinical findings and the correlation between the histopathological-grading/clinical-staging system and the surgical prognosis in 43 patients with pathological evidence of MEC.  Results: Recurrence or regional lymph node metastasis were diagnosed in 6 of the 36 MEC patients, among which 4 were well differentiated and 2 were poorly differentiated, 1 was in stage Ⅰ to Ⅱ, and 5 were in the stage of Ⅲ or Ⅳ, there were 23 survivors. Thirteen of the 36 patients died, 4 of whom from MEC and 9 from reasons not related to MEC. All the died were in stage Ⅲ or Ⅳ.  Conclusions: Recurrence or metastasis of MEC correlates with clinical-staging, but not related to the histopathological grading, this suggested that ablative surgery of the primary tumor and dissection of the regional lymphatic system are required in cases of Ⅲ and Ⅳstage MEC. Postoperative irradiation is proposed in the event of inadequate resection.

ABSTRACT

Objectives:To evaluate the efficiency of packing jaw defects with a composite of coral and bone marrow. Methods:Twelve patients (9 with jaw cyst and 3 with ameloblastoma) underwent enucleation of jaw lesion and packing with a composite of coral and bone marrow.Repair of the bone defects was evaluated at 1 week,1,6 and 12 months postoperatively by clinical examination and X-ray films. Results:Wound healing after the operations on 10 patients was uneventful,and definite ossification around the implanted material could be detected at 1 month postoperatively.A lot of bone formation and partial resorption of coral were observed at 6 months postoperatively.Complete resorption of coral and complete bone repair were obtained at 12 months postoperatively.Wound breakdown was observed on two other patients,and the composite had to be removed completely. Conclusions:A composite of coral and bone marrow may enhances bone healing in jaw defects after cyst or ameloblastoma removal.

ABSTRACT

Objectives:The effects of pingyangmycin(PYM) in the treatment of cavernous hemangioma, strawberry hemangioma and mixed hemangioma in the oromaxillofacial and intraoral regions is evaluated. Methods:450 cases of hemangioma in the oromaxillofacial regions from January 1993 to January 1999 were reviewed. PYM was injected into the cavity or in the circumference of the hemangioma, and the injection may be repeated every 7~10 days for 3~5 times. Results:450 patients were followed-up for 6 months~6 years. 86.89% were cured and nearlly cured,and 12% were improved. The total efficiency rate was 98.86%. The cure and elementary cure rates of cavernous hemangioma, strawberry hemangioma and mixed hemangioma were 91.04%, 86.08% and 78.01%, respectively, but the elementary cure rate of the wine color stainscapillary hemangioma was 14.29% only. Conclusions:This method may be a safe, simple and effective therapy for cavernous hemangioma, strawberry hemangioma and mixed hemangioma in the oromaxillofacial regions.
