ABSTRACT

OBJECTIVE@#To propose and evaluate the clinical effect of midpiece facial nerve dissection through transparotid approach in regional parotidectomy.@*METHODS@#A total of 136 patients with benign parotid tumors were categorized into three groups according to the way of facial nerve dissection: anterograde dissection from main trunk (anterograde, n=70), retrograde dissection from distal branches (retrograde, n=34), and midpiece dissection through transparotid approach (middle dissection, n=32). Surgery duration, facial nerve injury, salivary fistula, earlobe sensation, Frey's syndrome, and aesthetic evaluation were compared.@*RESULTS@#The surgery duration in the middle dissection group was significantly shorter than that in the other two groups. The proportion of salivary fistula was higher in the anterograde group (9 cases, 12.9%; P<0.05) compared with that in the other groups. Postoperative facial nerve injury was similar between the middle dissection (1 case, 3.1%) and anterograde groups (3 cases, 4.3%) with lower injury rate compared with the retrograde group (7 cases, 20.6%). The anterograde group had more cases of hypoesthesia of the earlobe (12 cases, 17.1%; P<0.05) than the other two groups. Aesthetic score was higher in the anterograde and middle dissection groups compared with that in the retrograde group (P<0.05).@*CONCLUSIONS@#Midpiece facial nerve dissection is technically feasible and clinically viable in regional parotidectomy.

ABSTRACT

Objective To discuss the dilemma of establishing the hierarchical diagnosis and treatment system in China and the relevant policy recommendations.Method Game theory and Pareto improvement theory can be used to analyze and promote the implementation of the hierarchical diagnosis and treatment system.Results The difficulty of grading diagnosis and treatment lies in the failure to achieve Pareto improvement,that is,patients and the government can save costs in the grading system,but doctors have not been given corresponding incentives,therefore,they are lack of enthusiasm for subsidence.Conclusion The hierarchical diagnosis and treatment system can save the government subsidies for health insurance and medical costs.If the doctor can get part of the costs,they will sink to the grassroots medical institutions,which can realize the Pareto improvement.The order of medical treatment and classification will be formed.

ABSTRACT

OBJECTIVE: To analyze the rational behaviors of pharmaceutical companies when modifying drug instructions, and puts forward policies and suggestions for promoting these enterprises to actively modify drug instructions. METHODS: The game theory is used to analyze the game equilibrium between enterprises and the market (patients and their agents -doctors) or the relevant government departments when they modify the instructions of the pharmaceutical enterprises in China. RESULTS: Based on cost considerations, enterprises are unwilling to modify drug instructions voluntarily. CONCLUSION: The drug regulatory departments in China should improve the relevant laws and regulations, and formulate specific operational drug instructions to modify the program.These measures would reduce the cost of enterprises to take the initiative to modify the drug instructions, and encourage the patient (and also be a doctor) to actively report the problem of adverse drug reactions caused by the unmodified instructions and increase penalties for not actively modify the behavior of drug instructions.At last the pharmaceutical companies could timely and actively modify instructions.

ABSTRACT

Objective To explore the feasibility of open visiting among mothers' of very-low-birth-weight(VLBW) infants and observe effects.Methods Totally 74 eligible VLBW infants were recruited,those who were visited and cared daily by mothers were assigned into the experimental group(n=36) and those who were not visited by mothers were assigned into the control group(n=38).The experimental group received open visiting as well as individualized developmental nursing care,and the control group only received individualized developmental nursing care.Relevant regulations and procedures were designed,and clinical pathways of mothers' involvement in nursing care were developed and implemented.Incidence of infection,time to reach full oral feeding,weight gaining during hospitalization,rate of breast-feeding during hospitalization and 30 days after discharge,hospitalization time and rate of readmission 30 days after discharge were recorded and compared.Results There was no significant difference in incidence of infection between two groups (P>0.05).Time to reach full oral feeding in the experimental group was earlier than that of the control group,weight gaining during hospitalization in the experimental group was faster than that of the control group,rate of breast-feeding 30 days after discharge was higher in the experimental group,and the differences were significant (P<0.05).Hospitalization time in the experimental group was shorter than that of the control group,rate of readmission 30 days after discharge in the experimental group was lower,and the differences were significant (P<0.05).Conclusion Mother's open visiting for VLBW infants in NICU does not increase the incidence of infection,but can shorten time to reach full oral feeding,promote weight gaining,increase rate of breast-feeding 30 days after discharge,reduce hospitalization time,and decrease rate of readmission 30 days after discharge.

ABSTRACT

Puerarin is one of the most important effective components of Pueraria lobata which exhibited classic estrogen-like biological activities and had remarkable cardiovascular protections in vivo and in vitro experiments. These protections of puerarin are mainly exhibited on improving the myocardial cells membrane potential and arrhythmia based on effecting the Na+, K+ , and Ca2+ channels,resisting myocardial fibrosis damage, diastolic effect on blood vessels, promoting angiogenesis, resisting calcification and atherosclerosis, improving blood flow, antiplatelet aggregation, reducing lipid and resisting diabetes. The main mechanisms are to improve the membrane potential and reduce cardiovascular damage caused by inflammation, oxidative stress and apoptosis, and the main regulated signal pathways are the PI3K/Akt, the NF-kappa B and the caspases.

ABSTRACT

Traditional Chinese ancient prescriptions have been used for treatment of liver cancer for a long history and the scientific and rational compatibility is a great wealth for modern research and development (R&D) of new drugs. The research and development of new drugs are often accompanied with a large investment, a long cycle and a high risk, especially for the anti-tumor drugs R&D which are facing more risks and lower successful rate. In this research, the regularity of compatibility of drugs was analyzed from 124 anti-hepatoma ancient prescriptions by computer program. The results can offer help to the R&D of anti-hepatoma new drugs and reduce the risk of drug screening. In addition, we surveyed 22 companies in this field from six provinces such as Beijing, Shanghai, Tianjin and so on and obtained 240 risk assessment questionaires. Then we used qualitative analysis method to interpret the greatest impacts for the risks in the process of R&D, production and sales of anti-hepatoma new drugs. The study provides a basis for anti-liver cancer drugs R&D researchers, who can take effective measures to reduce the R&D risks and improve successful rate.

ABSTRACT

<p><b>OBJECTIVE</b>To compare the difference in the efficacy on low back pain of cold-damp pattern between electric-thermal Bian-stone therapy and moxibustion box therapy.</p><p><b>METHODS</b>Forty-one cases of low back pain of cold-damp pattern were randomized into an electric-thermal Bian-stone therapy group (group A, 26 cases) and a box moxibustion therapy group (group B, 15 cases). In the group A, the electric-thermal Bian-stone was placed over Shenshu (BL 23) and Weizhong (BL 40). The temperature of stone was adjusted in accordance with patient's comfort. In the group B, moxibustion box was used over Shenshu (BL 23) and Weizhong (BL 40). The treatment was given once every day or every two days. Ten treatments made one session. The symptom and physical signs score of low back pain and the score of cold-damp syndrome were observed before and after treatment in the patients.</p><p><b>RESULTS</b>The symptom and physical signs score of low back pain and the score of cold-damp syndrome were all improved in the two groups (all P < 0.01). The changing rate of symptom and physical signs and syndrome were (37.04 +/- 32.68)% and (22.85 +/- 29.95)% in the group A, and were (47.29 +/- 22.08)% and (23.89 +/- 22.53)% in the group B, respectively, without significant difference in comparison between the two groups (all P > 0.05).</p><p><b>CONCLUSION</b>The efficacy on low back pain of cold-damp pattern treated with the electric-thermal Bian-stone therapy is similar to that of moxibustion box therapy. This therapy is characterized as more convenient, safer operation and less pollution.</p>

ABSTRACT

Contemporary pharmacological research has demonstrated that puerarin, the most important phytoestrogen extracted from Pueraria lobata(Willd.) Ohwi, has protecting functions on the cardiovascular system, nervous system, osteoporosis, liver injury, and inflammation in vivo and in vitro. Most of these research studies focused on inhibiting oxidative stress and apoptosis through regulating various bioactivators and signal pathways. Among these, superoxide dismutase (SOD), endothelial nitric oxide synthase (eNOS) and malondialdehyde (MDA), and PI3K/Akt, MAPK, and NF-κB are of great importance. The data cited in this review were mainly obtained from articles listed in PubMed and Elsevier SDOL published from 1959 to 2013, and the search term used was "puerarin".

ABSTRACT

<p><b>OBJECTIVE</b>To compare the impacts on skin blood flow between moving cupping following the meridian running direction and that against the running direction.</p><p><b>METHODS</b>JLG-2 meridian cupping drainage instru ment was used for moving cupping on the back along the Bladder Meridian running course in either single direction for 20 times. The cupping device was Bian stone cup, 44 mm in inner diameter, negative pressure from -0.03 to -0.04 MPa. PeriScan PIM II laser Doppler perfusion imager was used to observe the changes in skin blood flow on the running course of the Bladder Meridian with cup moved up and down and in the same region on the contralateral Bladder Meridian. Blood flow was measured before cupping, at the immediate time after cupping and 10 min after cupping separately. Fourteen healthy volunteers received the test. The measuring region was subdivided into a moving cupping area, an upstream area, a downstream area, a contralateral moving cupping area, a contralateral upstream area and a contralateral downstream area. The mean blood flow was calculated in each area.</p><p><b>RESULTS</b>Blood flow was increased significantly in each area and was more apparently increased in the moving cupping area. In comparison of the changing rate of blood flow between cupping following the meridian running direction and that against the running direction, it was only found that the changing rate in the upstream area of moving cupping against the running direction was significantly higher than that following the running direction (P < 0.05). The differences were not statistically significant in comparison among the other areas. Additionally, the changing rates of blood flow in the upstream and downstream area of the Bladder Meridian were increased significantly as compared with the contralateral Bladder Meridian.</p><p><b>CONCLUSION</b>The local effects are similar between moving cupping following the meridian running direction and that against the running direction. The abscopal effect of moving cupping against the running direction is superior to that following the running direction. It is suggested that the dual-directional moving cupping is applicable for the treatment of local disorders and the abscopal effect is better with moving cupping against the meridian running direction.</p>

ABSTRACT

Background : Fatty acid synthase (FASN) is overexpressed in a variety of human cancers, and may be involved in cancer metastasis. Hence, the strategies targeted on FASN may have therapeutic potential for treating cancer metastasis. Objectives : The aim of this study is to investigate the correlation of FASN expression with metastasis in human osteosarcoma. Materials and Methods : Human osteosarcoma cell lines U2-OS and osteosarcoma biopsy specimens were employed in this study. The expression of FASN protein in osteosarcoma specimens was detected by IHC (immunohistochemistry) and the relationship with metastasis was analyzed. We performed the cerulenin, an inhibitor of FASN, to inhibit FASN expression in U2-OS cells. Western blot and RT-PCR were performed to investigate the expression of FASN in U2-OS cells. Cells mobility was detected by wound healing and Transwell assays. Results : Results showed that the FASN expression level in the cases with pulmonary metastases was significantly higher than in those without metastasis. In vitro, the invasion and migration of U2-OS cells were suppressed by inhibiting FASN. Our findings suggested that FASN may be involved in osteosarcoma metastasis

ABSTRACT

<p><b>BACKGROUND</b>Reports on mood regulating circuit (MRC) indicated different activities between depressed patients and healthy controls. The functional networks based on MRC have not been described in major depression disorder (MDD). Both the anterior cingulate cortex (ACC) and thalamus are all the key regions of MRC. This study was to investigate the two functional networks related to ACC and thalamus in MDD.</p><p><b>METHODS</b>Sixteen patients with MDD on first episode which never got any medication and sixteen matched health controls were scanned by 3.0 T functional magnetic resonance imaging (fMRI) during resting-state. The pregenual anterior cingulate cortex (pgACC) was used as seed region to construct the functional network by cortex section. The thalamus was used as seed region to construct the functional network by limbic section. Paired-t tests between-groups were performed for the seed-target correlations based on the individual fisher z-transformed correlation maps by SPM2.</p><p><b>RESULTS</b>Depressed subjects exhibited significantly great functional connectivity (FC) between pgACC and the parahippocampus gyrus in one cluster (size 923) including left parahippocampus gyrus (-21, -49, 7), left parietal lobe (-3, -46, 52) and left frontal lobe (-27, -46, 28). The one cluster (size 962) of increased FC on thalamus network overlapped the precuneus near to right parietal lobe (9, -52, 46) and right cingulate gyrus (15, -43, 43) in health controls.</p><p><b>CONCLUSIONS</b>Abnormal functional networks exist in earlier manifestation of MDD related to MRC by both cortex and limbic sections. The increased functional connectivity of pgACC and decreased functional connectivity of thalamus is mainly involved in bias mood processing and cognition.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To observe clinical therapeutic effect of tendon-muscle picking therapy on myofascitis of back.</p><p><b>METHODS</b>One hundred and twelve cases were randomly divided into a tendon-muscle picking therapy group and an electroacupuncture (EA) group, 56 cases in each group. The tendon-muscle picking therapy group were treated by picking pain tendon-muscle tubercles on the back, and the EA group by EA at Dazhu (BI. 11), Feishu (BL 13), Xinshu (BL 15), Tianzong (SI 11) and Jiaji (EX-B 2). Their therapeutic effects were compared after treatment for 2 courses.</p><p><b>RESULTS</b>The total effective rate was 89.3% in the tendon-muscle picking therapy group and 78.6% in the EA group with a significant difference between the two groups (P<0.05).</p><p><b>CONCLUSION</b>Tendon-muscle picking therapy has a good therapeutic effect on myofascitis of back.</p>

ABSTRACT

<p><b>BACKGROUND</b>In the process of hepatic fibrosis, the accumulation of collagen fibers is strongly related to the hepatic function. The aim of this study was to investigate the three-dimensional architecture of the collagen network in the liver of rats with hepatic fibrosis.</p><p><b>METHODS</b>Healthy adult male Wistar rats (n = 32) were randomly divided into a control group (n = 16) and a hepatic fibrosis group (n = 16). In the control group, the rats were treated with peanut oil while the rats in hepatic fibrosis group were treated for 10 weeks with 60% CCl(4) diluted in peanut oil. The quantity of collagen fibers was detected by Western blotting; distribution of the collagen was detected by sirius red staining and polarized microscope; the three-dimensional architecture of collagen in the liver was observed under the scanning electron microscope after fixed tissues were treated with cell-maceration using NaOH. Statistical analysis was performed using the u test.</p><p><b>RESULTS</b>The quantity of collagen fibers increased significantly in the hepatic fibrosis group. With the aggravation of hepatic fibrosis, collagen fibers gradually accumulated. They interlaced the reticulation compartment and formed a round or ellipse liver tissue conglomeration like a grape framework that was disparate and wrapped up the normal liver lobule. The deposition of collagen fibers was obvious in adjacent hepatic parenchyma, especially around the portal tracts.</p><p><b>CONCLUSION</b>Our experiment showed the collagen proliferation and displays clearly the three-dimensional architecture of collagen fibers in rat liver with hepatic fibrosis by scanning electron microscope. It can provide a morphological foundation for the mechanisms of changed haemodynamics and portal hypertension in hepatic fibrosis.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the relationship between genetic polymorphisms of glutathione S-transferase (GST) M1, T1 and susceptibility to mountain sickness.</p><p><b>METHODS</b>Forty-three soldiers with acute mountain sickness and 80 healthy soldiers matching with sex/age and training under the same condition were divided into case group and control group. A multiple polymerase chain reaction method was used to detect GSTM1 and GSTT1 genes in genomic DNA isolated from peripheral blood cells from both cases and controls.</p><p><b>RESULTS</b>The frequency of the GSTT1 positive genotype was significantly higher in cases (69.8%) than in controls (42.5%) (P = 0.004, OR = 3.12, 95% CI 1.42 approximately 6.86). The frequency of GSTM1 negative genotype was also higher in cases (72.1%) than in controls (52.5%) (P = 0.03, OR = 2.34, 95% CI 1.05 approximately 5.02). Persons with both GSTM1 and GSTT1 negative genotypes had 5-fold more risk than those with GSTT1 negative and GSTM1 positive genotypes in developing mountain sickness (OR = 5.04, 95% CI: 1.00 approximately 25.3).</p><p><b>CONCLUSION</b>Genetic polymorphisms of glutathione S-transferase M1, T1 may be the risk factors in the development of mountain sickness.</p>

ABSTRACT

Objective To Analyze the imaging characteristics of intraparenchymal schwannoma and the related pathology,in order to improve the accuracy of diagnosis and be in favor of the clinics and the prognosis.Methods Four cases were confirmed to be intraparenchymal schwannoma by pathological and immunohistochemistry examination.One case was examined with precontrast and enhanced CT scanning,one with unenhanced MRI scanning,two with unenhanced and enhanced CT and MRI scanning.Their images were retrospectively analyzed.Results Of the four cases,three patients were less than 30 years old,with tumors located supratentorially.Cysts were found in all cases,with nodules on the wall in 3 cases.The nodules were enhanced markedly in two cases and moderately in one ease.In addition,calcification was detected in one case and prominent peritumoral edema existed in 1 case.The picture of the pathology demonstrated Antoni type A and Antoni type B.Immunostaining showed intense immunoreactivity for S-100 protein and Vim and negative immunoreactivity for GFAP and EMA.Conclusions Intraparenchymal schwannoma mostly occurred in juvenile,which located supratentorially in most cases.The presence of a cyst and peritumoral edema together with the tumor appears to be characteristic of intraparenchymal schwannoma.Calcification or the enhanced nodule is the helpful sign for the diagnosis.Combining the imaging findings with the pathology and immunohistochemistry results can gain the accurate diagnosis.
