ABSTRACT

Objective To evaluate the reliability and validity of the brief version of dysfunctional beliefs and attitudes about sleep(DBAS-16).Methods Participants were out-patients who with insomnia or insomnia comorbided depression disorder or anxiety disorder. All of them were assessed by DBAS-16,Pittsburgh sleep quality index (PSQI),self-rating depression scale (SDS),self-rating anxiety scale (SAS).30 of them were evaluated by DBAS-16 two weeks later. Results 230 patients were assessed,212 pieces of questionnaire with integral answers were collected,The Cronbach α Coefficient of DBAS-16 was 0.786,the Cronbach α coefficients of four subscales were 0.424-0.703.Split-half reliability coefficient of total score was 0.775,split-half reliability coefficient of four subscales were 0.424-0.719.Item-total correlation coefficients ranged from 0.602 to 0.283(all P<0.05).The testretest reliability coefficient was 0.928,and coefficient of each subscales ranged from 0.709 to 0.907 (all P<0.05).Confirmatory factor analysis was used to estimate the construct validity. The fit indices:x2/dfwas 1.623,the adjusted goodness of fit index (AGFI) was 0.882,root mean square error of approximation (RMSEA) was 0.054,incremental fit index (IFI)was 0.899,non-normed fit index (NNFI) was 0.871,comparative fit index (CFI) was 0.895.Total DBAS-16 score significantly correlated with PSQI(r=-0.311,P<0.01),SDS(r=-0.192,P<0.01),SAS(r=-0.162,P<0.01).Conclusion The reliability and validity of the brief version of DBAS-16 are adequate.

ABSTRACT

BACKGROUND: The most commonly used rod-screw posterior spinal fixation system has some biomechanical drawbacks. The screw-plate system is more preferred for patients.OBJECTIVE: To prepare a new posterior spinal fixation system based on imaging results of distance between two pedicles, radian of spine flexion, extension and the height of intervertebral space of Chinese population.METHODS: The thoracolumbar data of 129 normal people were measured. According to the imaging measurements and relative documents, a device of gear-distraction plate (GDP) for spine reduction and fixation was designed. Eighteen fresh calf lumbar specimens were randomly divided into 3 groups, the test group was fixed by GDP and the other groups were fixed by CD and Steffee plate, respectively. The results of the displacement, strain, strength, stiffness and ultimate strength were measured under the states of vertical compression, flexion, extension and lateral bending respectively, and the results were statistically analyzed.RESULTS AND CONCLUSION: GDP could meet the need of strength and stiffness of human bodies. It showed superior to the CD and Steffee plate groups in strength and stiffness (P < 0.05), with 13% or 14% torsion intensity greater than that of the CD or Steffee plate groups. The ultimate mechanical performance test showed that, the load bearing of the GDP group was greater than that of the CD and Steffee plate groups (P < 0.05). The findings demonstrated that GDP for Chinese human presented with good biomechanical stability, which can promote vertebrae fracture union and prevent kyphosis relapses or vertebral height loses.

ABSTRACT

To explore the relationship of mieroglia activation, motoneuronal loss in the ventral horn of spinal cord and sciatic nerve regen-eration after the sciatic nerve injury,Sprague-Dawley rats were prepared a modal of the fight sciatic nerve crash injury. The immunoreactiv-ity(-ir)ofmicroglia and number of the motoneurons inthe ventral horn of spinal cord were detected at 3 and7 days, light and electron mi-croscopic detection of sciatic nerve degeneration and regeneration were performed at 4 weeks after the nerve injury. The results showed:(1)At 3 days after the sciatic nerve injury,OX-42-irinthe ventral horn of spinal cord begantoincrease significantly(P<0.05);(2)The number ratio of motoneurons in ventral horn of spinal cord in ipsilateral to contralateral for injury decreased markedly(P<0.05),in-dicating the numbers of ipsilateral motoneuroanl survival decreased;(3)Histological assessment showed the poor regeneration of the in-jured nerves;(4)Simvastatin(an inhibitor of cholesterol biosynthesis, having potential immunomodulatory capacities)facilitated the mi-eroglial activation.the motoneuronal survival and sciatic nerve regeneration were better than non-simvastatin-treated vehicle rats. The pres-ent results suggest that mieroglia activation in the ventral horns ofthe spinal cord may play an important protective role in the nerve regener-ation after peripheral nerve injury of the rat.

ABSTRACT

[Objective]To study the effect of Simvastatin on the regeneration of sciatic nerve of rats and its mechanism of immunoregulation.[Method]Twenty eight female Sprague-Dawley rats were used and the rats experienced the sciatic nerve crush injury.Animals were randomized into: Simvastatin-treated crush injury animals,vehicle-treated crush injury animals and sham-operated animals.Simvastatin(20 mg/kg) was given once a day over a time period of 14 day sby oral gavage via a pharyngeal tube. After surgery, the functional evaluation of nerve recovery, electrophysiologic assessment, histological assessment, serum IL-6 and TNF-? assessment were performed.[Result]The toe spread index of Simvastatin-treated crush injury animals was higher significantly than that of vehicle rats at d5 and d8(P

ABSTRACT

Objective To evaluate surgical indications and method for regional pancreatoduodenectomy combined with blood vessel reconstruction. Methods Forty-four patients underwent pancreatoduodenectomy combined with superior mesenteric vein and portal vein(smv-pv) resection and reconstruction between April 1994 and March 2001.Among them superior mesenteric artery (SMA) and hepatic artery (HA) were reconstructed in 4 and 2 cases, respectively. Partial resection of the anterior wall of the inferior vena cava was performed in one case for tumor invasion. Results The overall mortality was 7.1%,with no complications. The resected endothelium or margins of the blood vessel and pancreas were microscopically tumor free in all cases. Histological specimen examinations demonstrated adenocarcinoma of pancreas head in 43 cases, neuroendocrine adenocarcinoma was diagnosed in one case. Patients were followed-up from 3~87 months with 2 cases lost after PV/SMV for pancreatoduodectomy. Six patients have survived more than 5 yearsand 21 cases more than 3 years. Conclusion Regional pancreatoduodenectomy combined with reconstructionof blood vessel could increase tumor resection rate in properly selected patients and could be performed safely without increased morbidity and mortality.
