ABSTRACT

OBJECTIVE@#To investigate whether the Runx2 gene can induce the differentiation of human amniotic mesenchymal stem cells (hAMSCs) to ligament fibroblasts in vitro and promote the tendon-bone healing in rabbits.@*METHODS@#hAMSCs were isolated from the placentas voluntarily donated from healthy parturients and passaged, and then identified by flow cytometric identification. Adenoviral vectors carrying Runx2 gene (Ad-Runx2) and empty vector adenovirus (Ad-NC) were constructed and viral titer assay; then, the 3rd generation hAMSCs were transfected with Ad-Runx2 (Ad-Runx2 group) or Ad-NC (Ad-NC group). The real-time fluorescence quantitative PCR and Western blot were used to detect Runx2 gene and protein expression to verify the effectiveness of Ad-Runx2 transfection of hAMSCs; and at 3 and 7 days after transfection, real-time fluorescence quantitative PCR was further used to detect the expressions of ligament fibroblast-related genes [vascular endothelial growth factor (VEGF), collagen type Ⅰ, Fibronectin, and Tenascin-C]. The hAMSCs were used as a blank control group. The hAMSCs, hAMSCs transfected with Ad-NC, and hAMSCs were mixed with Matrigel according to the ratio of 1 : 1 and 1 : 2 to construct the cell-scaffold compound. Cell proliferation was detected by cell counting kit 8 (CCK-8) assay, and the corresponding cell-scaffold compound with better proliferation were taken for subsequent animal experiments. Twelve New Zealand white rabbits were randomly divided into 4 groups of sham operation group (Sham group), anterior cruciate ligament reconstruction group (ACLR group), anterior cruciate ligament reconstruction+hAMSCs transfected with Ad-NC-scaffold compound group (Ad-NC group), and anterior cruciate ligament reconstruction+hAMSCs transfected with Ad-Runx2-scaffold compound group (Ad-Runx2 group), with 3 rabbits in each group. After preparing the ACL reconstruction model, the Ad-NC group and the Ad-Runx2 group injected the optimal hAMSCs-Matrigel compunds into the bone channel correspondingly. The samples were taken for gross, histological (HE staining and sirius red staining), and immunofluorescence staining observation at 1 month after operation to evaluate the inflammatory cell infiltration as well as collagen and Tenascin-C content in the ligament tissues.@*RESULTS@#Flow cytometric identification of the isolated cells conformed to the phenotypic characteristics of MSCs. The Runx2 gene was successfully transfected into hAMSCs. Compared with the Ad-NC group, the relative expressions of VEGF and collagen type Ⅰ genes in the Ad-Runx2 group significantly increased at 3 and 7 days after transfection ( P<0.05), Fibronectin significantly increased at 3 days ( P<0.05), and Tenascin-C significantly increased at 3 days and decreased at 7 days ( P<0.05). CCK-8 detection showed that there was no significant difference ( P>0.05) in the cell proliferation between groups and between different time points after mixed culture of two ratios. So the cell-scaffold compound constructed in the ratio of 1∶1 was selected for subsequent experiments. Animal experiments showed that at 1 month after operation, the continuity of the grafted tendon was complete in all groups; HE staining showed that the tissue repair in the Ad-Runx2 group was better and there were fewer inflammatory cells when compared with the ACLR group and the Ad-NC group; sirius red staining and immunofluorescence staining showed that the Ad-Runx2 group had more collagen typeⅠ and Ⅲ fibers, tending to form a normal ACL structure. However, the fluorescence intensity of Tenascin-C protein was weakening when compared to the ACLR and Ad-NC groups.@*CONCLUSION@#Runx2 gene transfection of hAMSCs induces directed differentiation to ligament fibroblasts and promotes tendon-bone healing in reconstructed anterior cruciate ligament in rabbits.

ABSTRACT

Objective To investigate influencing factors of serum small and dense low-density lipoprotein cholesterol (sd LDL-C) levels in patients with chronic diseases in Zhoujiadu community, and to provide a scientific basis for the treatment of sd LDL-C related chronic diseases. Methods A total of 200 patients with common chronic diseases who were admitted to our hospital from November 2017-November 2019 were selected. Basic data including height, weight, past medical history and family history of the patients were collected. Biochemical indicators were measured to understand dietary structure. The influencing factors of serum sd LDL-C were analyzed by multiple stepwise regression. Results The levels of serum sd LDL-C in patients with advanced age, increased BMI, thickened IMT, fatty liver and coronary heart disease were significantly increased (P<0.05). Pearson linear correlation analysis showed that serum sd LDL-C level of patients with chronic diseases was positively correlated with TC, TG, LDL, ALT and AST, and negatively correlated with HDL (P<0.05). Multiple linear regression showed that TG, BMI, IMT, fatty liver, oil heating (frying and frying) and the use of mixed oil were independent risk factors of sd LDL-C level in patients with common chronic diseases, while HDL-C was a protective factor (P<0.05). Conclusion The levels of serum sd LDL-C in patients with common chronic diseases in Zhoujiadu community were correlated with dyslipidemia, BMI, thickening of IMT, fatty liver, oil heating (frying and frying), and use of mixed oil.

ABSTRACT

Objective:To establish an HPLC-RID method for the simultaneous determination of taurine and aminocaproic acid in compound taurine eye drops. Methods:A Waters NH2column(250 mm ×4.6 mm,5 μm) was used, the mobile phase was water-acetonitrile (40:60) at a flow rate of 0.8 ml·min-1, the column temperature was 35.0℃, and the temperatures of the detector reference cell and the sample cell were both set at 35.0℃,The injection volume was 20 μl. Results:The linear ranges of taurine and aminocaproic acid were 0.05-2.00 mg·ml-1(r = 0.999 9). The average recovery was 98.57% (RSD = 1.07%,n= 9) and 100.69% (RSD =0.75%,n =9),respectively. Conclusion:The established method is accurate,simple and rapid, and suitable for the quality control of compound taurine eye drops.

ABSTRACT

BACKGROUND:Anterior dislocation of the shoulder joint is mainly due to trauma caused by anterior labral capsular ligaments complex functional deficiency. At present, the main clinical use is conservative treatment, but the long-term effect is poor, and cannot achieve the desired results. OBJECTIVE:To study the biomechanical characteristics of dual fixed anchors and their effects in recurrent anterior dislocation of the shoulder with severe bone defect. METHODS:According to the treatment, 88 patients with recurrent anterior dislocation of the shoulder with severe bone defect were divided into conservative treatment group (n=44) and dual fixed anchor group (n=44). Japan AG-ISOKN material tensile tester was used to measure biomechanical properties of dual fixed anchors. Conservative treatment group received closed manipulative reduction. Dual fixed anchor group received dual fixed anchor fixation. The repair effects were compared between the two groups. RESULTS AND CONCLUSION:(1) Biomechanical parameters:in dual fixed anchor bending test, limit strength was (3 534±321) N;limit displacement was (21.12±2.50) mm;the yield load was (2 342±154) N;yield shift was (13.10±53) mm. Maximum axial pul out force was (809±25.2) N. The maximum spin torque was (1.2±0.3) N?m. (2) Clinical test results:Total effective rate was 96%in the dual fixed anchor group and 86%in the conservative treatment group (P<0.05). Ranges of motion of anteflexion and lift and shoulder joint stability score were significantly higher in the dual fixed anchor group than in the conservative treatment group during final fol ow-up (P<0.05). Range of external rotation motion was significantly smal er in the dual fixed anchor group than in the conservative treatment group (P<0.05). The complication rate was lower in the dual fixed anchor group (11%) than in the conservative treatment group (25%) (P<0.05). (3) Results verified that dual fixed anchors have good biomechanical characteristics, may have ideal repair effect in the treatment of recurrent anterior dislocation of the shoulder with severe bone defect, and can improve functions and activities of the shoulder joint. The low complication rate is more conducive to shoulder reconstruction.

ABSTRACT

Objective To investigate the clinical efficacy of porous calcium phosphate bioceramics (PTCP)for treating stage FicatⅢ non-traumatic osteonecrosis of the femoral head(ONFH)in the patients aged below 60 years old.Methods A total of 41 patients (63 hips)with non-traumatic ONFH in our hospital from March 2006 to July 2009 were collected and divided into two groups.The group A (n=1 6)was performed the core decompression only,while the group B (n=25)was performed the core de-compression plus through the head-neck junction of the femur to opening the window light bulb shape focus debridement and impac-ting bone grafting combined with PTCP implantation.The various clinical evaluation indicators were observed and evaluated.Results No statistically significant differences in PRI were found at each time point in the group A.PRI at postoperative 1 month in the group B was significantly decreased than before operation,hereafter,the stable status was maintained and slightly risen again until postoperative 3 years(P =0.033).The Harris scores at postoperative all time points in the group A were significantly lower than those in the group B.The necrosis areas at postoperative 24 months,3,4,5 years in the group B were significantly decreased com-pared with before operation and lower than those in the group A.Conclusion PTCP implantation has satisfactory effect for the treatment of stage FicatⅢ non-traumatic ONFH in the patients aged below 60 years old.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effects of different tooth preparations on three-dimensional adaption of computer aided design and computer aided manufacturing (CAD/CAM) crowns based on the reverse engineering.</p><p><b>METHODS</b>The tooth preparation model of the left maxillary first molar was scanned to build five different tooth preparations using the NX Imageware 13.2 software. The resin cores (n=8) were designed with the exocad software. The scanning data of the inner surface of each resin core were used to analyze the three-dimensional adaption with the Geomagic Qualify 12 software.</p><p><b>RESULTS</b>According to the color-coded deviation images, the deviation of the heavy chamfer shoulder was the smallest and the most uniform, whereas the other groups with sharp lines showed large deviation. After statistical analysis, the total deviation of the heavy chamfer shoulder, 135° shoulder, feather shoulder, 90° shoulder with lipped margins, and sharp marginal ridges preparation were (16.88±2.83), (26.88±3.61), (53.56±4.30), (51.38±4.46), and (47.19±4.62) μm. A statistical significance was observed between the heavy chamfer and 135° shoulders (P<0.05). The other groups fitted poorly, without statistical significance between each group (P>0.05).</p><p><b>CONCLUSION</b>Three-dimensional analysis using a computer is a preferable method to study the three-dimensional adaption of crowns. The heavy chamfer shoulder and round line preparation are clinically recommended. However, feather shoulder, 90° shoulder with lipped margins, and sharp marginal ridge preparation should be avoided.</p>

ABSTRACT

In order to improve the result of clinical practice for undergraduate students of sto-matology, the standardized training program was carried out before clinical practice by the teaching and researching Office of Periodontology of Affiliated Stomatological Hospital of Chongqing Medical University, in this program, based on our own realities, the structure and content of teaching was designed to guide the transformation of students to doctors, collective teaching and personalized teach-ing was combined to reform the conventional model of teaching and various student-oriented teaching methods were explored, including mimic forum of disciplines, intractable case discussion, clinical practice summary, course completion test and so on, thus promoting the result of clinical practice. Be-sides, the construction of teaching staff was strengthened, and the evaluation and management system was established to ensure the teaching quality, thus summarizing a set of efficient and effective clinical teaching methods presented in this paper.

ABSTRACT

Objective To discuss the indications and efficacy for treating tibia insertion avulsion fracture of the posterior cruciate ligament(PCL)by the mean of the end of ligament suture via arthroscopy.Methods Twenty-one cases of tibia insertion avulsion fracture of PCL were treated with suture the end of ligament via arthroscopy from June 2007 to June 2009,including 14 males,7 females,with a mean age of 31.5 years(range,13-52 years).The insertion avulsion fragment was fixed to the tibia with two strands of 5Ethibond threads via arthroscopy.Two bone tunnels(2.5 mm in width)were made from the anterior part of the tibia to the posteromedial(at 4:30 toward)and posterolateral(at 7:30 toward)of the tibia bone bed respectively.Then the sutures were pulled out through the two bone tunnels and tied to the door-nail.Reduction and healing of the avulsion fragment,relaxation and mobility degree of knee,and the influence of osteoepiphysis to teenage patients were observed after surgery.KT-2000 value and Lysholm score were calculated.Results All cases were followed up from 10 to 24 months,with an average of 13.5 months.All the fragments were united without displacement in all cases according to X-ray films.One case showed posterior drawer test(+),but the tibia insertion was healed.The knee extension in all cases was unlimited,and 10° to 15° flexion limitation was found in 2 cases.The average range of flexion was 140.5°±3.8°.On average,KT-2000 value was(1.2±0.4)mm,Lysholm score was 95.2±2.7.Conclusion For treating the avulsion fracture of PCL,suture the end of ligament via arthroscopic transfixion can get a reliable fixation and a satisfied function of the knee,especially,which is suitable for the small avulsion fracture fragments,the adolescent patients with premature epiphysis.

ABSTRACT

Anterior cruciate ligament (ACL) has two important functional roles, including biomechanics and proprioceptive sense. A groat propr!oceptive sense is a basis to exactly obtain highly functional exercise. ACL injury may block perception and conduction information and seriously influence motion and daily activities. A complete rehabilitation of ACL function depends on rehabilitation of both mechanical structure and proprioceptive sense. Therefore, reconstruction of joint function following ACL injury is focus on reconstruction of both joint biomechanical stability and proprioceptive sense. This study was designed by distribution and function of proprioceptive sense to analyze the altemation of propdoceptive sense following ACE injury, and to investigate the related factors and ways for rehabilitation of propriocaptive sense following ACL reconstruction.
