ABSTRACT

Objective:To analyze the effects of β-receptor blockers on all-cause mortality in elderly patients with mild-moderate chronic heart failure(CHF)and chronic obstructive pulmonary disease(COPD), and to analyze risk factors relevant to death.Methods:This was a multicenter retrospective cohort study from January 2013 to December 2017.The 400 elderly patients with CHF and COPD treated in Rizhao People's Hospital(187 cases), Shandong Provincial Hospital(122 cases)and Beijing Anzhen Hospital(91 cases)were enrolled.The patients receiving β-receptor blockers were included as β-receptor blockers group(n=200), and the patients matching for similar age and cardio-pulmonary function, not receiving β-receptor blockers were selected as the control group(n=200). All patients were followed up until December 31, 2019.The primary endpoints were all-cause mortality.The risk factors for all-cause death were compared and analyzed.Results:Among 400 patients, the average age was(72.2±11.7)years with 226 males(56.5%). There was no significant difference in baseline data such as age, gender ratio, body mass index, heart function, lung function, and treatment regimen after matching between the two groups(all P>0.05). At end of 3-years follow-up, risks of all-cause mortality( χ2=7.284, P<0.01), and re-hospitalization risk due to worsening heart failure( χ2=6.782, P<0.01), acute exacerbation of COPD( χ2=6.921, P<0.01)were significantly reduced in β-receptor blockers group versus control group.Multivariate Cox regression analysis showed that age ≥75 years( HR=2.142), diabetes( HR=1.929), ratio of baseline forced expiratory volume in the first second / forced vital capacity <66.8%( HR=1.114), baseline glomerular filtration rate <72.0 ml/min( HR=3.572)and left ventricular ejection fraction <46.4%( HR=2.294)at end of 3-year follow-up were risk factors for mortality, whileβ-receptor blockers( HR=0.745)was a protective factor(all P<0.05). Conclusions:β-receptor blockers can significantly reduce the mortality and re-hospitalization rate in elderly patients with CHF and COPD.

ABSTRACT

Objective To screen and identify mRNAs targeted by human cytomegalovirus-encoded miR-US33-1-5p (HCMV miR-US33-1-5p) for understanding the biological functions of HCMV miR-US33-1-5p. Methods Potential target mRNAs of HCMV miR-US33-1-5p were screened out by using hybrid-PCR, a simple and effective method. Dual-Luciferase Reporter Assay System was used to identify the binding abili-ties of HCMV miR-US33-1-5p to 3′-UTRs of these potential target mRNAs. Results Twelve potential mRNAs targeted by HCMV miR-US33-1-5p were screened out in human embryo lung fibroblast cells infected with HCMV. Luciferase activities of 3′-UTRs of seven target mRNAs were significantly down-regulated. Conclusion Proteins encoded by these target mRNAs are involved in virus replication,immune system reg-ulation,protein synthesis,cell cycle regulation,energy metabolism and so on. Identification of mRNAs tar-geted by HCMV miR-US33-1-5p is helpful for further studies on biological functions of miR-US33-1-5p.

ABSTRACT

Objective To investigate the effect of gefitinib on mucus hypersecretion by inhibiting epidermal growth factor receptor (EGFR) activity in chronic obstructive pulmonary disease (COPD).Methods Human airway epithelail cell lines 16HBE cells were exposed to cigarette smoke extraction (CSE) to establish the COPD model.EGFR activity was inhibited by tyrosine kinase inhibitor gefitinib.The mRNA expressions of EGFR and MUC5AC were detected by real-time PCR.EGFR,p-EGFR and MUC5AC protein levels were determined by Western blot and ELISA.Results EGFR mRNA level was increased by 12.7% in CSE and 8.6% in gefitinib group,but had no significant differences among CSE,gefitinib group and control group (all P> 0.05).MUC5AC mRNA levels were enhanced by 141.7%,26.4% in CSE group and gefitinib group respectively,and there were significant differences among CSE,gefitinib group and control group (all P<0.05).EGFR protein levels were (600.34±64.58) μg/mg,(632.58±72.94) μg/mg,(584.57±67.39) μg/mg,in control,CSE and gefitinib groups,respectively,and there were no significant differences between groups (all P>0.05).p-EGFR protein levels were (338.62±45.28) μg/mg,(679.43±78.23) μg/mg,(292.74±59.17) μg/mg in control,CSE and gefitinib groups,respectively.MUC5AC protein levels were(72.80±6.25)μg/mg,(187.00±±10.26)μg/mg,(92.57±8.32)μg/mg in control,CSE and gefitinib groups respectively.Compared with control group,p-EGFR and MUC5AC protein levels were increased significantly in CSE group (both P<0.05),and had no significant differences in p EGFR and MUC5AC protein levels between control group and gefitinib group.Conclusions CSE may lead to mucus hypersecretion through activating the EGFR-mediated signaling pathways.Gefitinib may inhibit mucus hypersecretion by inhibiting EGFR tyrosine kinanse activity.EGFR may serve as a potential target for COPD.

ABSTRACT

ObjectiveTo observe the effects of salmeterol fluticasone on the glucose metabolism and bone density of the elderly patients with chronic obstructive pulmonary disease (COPD) combined with type 2 diabetes mellitus (T2DM).MethodsThirty-one patients with COPD combined with T2DM were divided into 2 groups by random digits table,14 cases in control group were given conventional therapy,17 cases in experimental group were given conventional therapy and fluticasone propionate 50/500 μg,twice a day,for 3 months.The fasting blood glucose,postprandial 2 hours glucose,glycosylated hemoglobin,plasma cortisol and bone density respectively before and after treatment were detected.ResultsIn experimental group,the fasting blood glucose,postprandial 2 hours glucose,glycosylated hemoglobin and bone density before treatment was (5.25 ± 0.21 ) mmol/L,(7.14 ± 0.33 ) mmol/L,(5.58 ± 0.26 )%,( 1.96 ± 0.11 ) g/cm2,and after treatment was(5.31 ± 0.27 ) mmol/L,(7.22 ± 0.29 ) mmol/L,(5.67 ± 0.23 )%,(2.03 ± 0.15 ) g/cm2,there was no significant difference (P > 0.05).In control groups,the fasting blood glucose,postprandial 2 hours glucose,glycosylated hemoglobin and bone density before treatment was (5.33 ± 0.35) mmol/L,( 7.26 ± 0.29 ) mmol/L,( 5.62 ± 0.19 )%,( 1.88 ± 0.20 ) g/cm2,and after treatment was ( 5.36 ± 0.31 ) mmol/L,(7.30 ± 0.35 ) mmol/L,( 5.69 ± 0.26 )%,( 1.98 ± 0.17 ) g/cm2,there was no significant difference (P > 0.05 ).There was no significant difference in plasma cortisol before and after treatment in two groups (P >0.05).ConclusionInhaling salmeterol fluticasone for elderly patients with COPD combined with T2DM is safe.

ABSTRACT

<p><b>BACKGROUND AND OBJECTIVE</b>Transbronchial needle aspiration (TBNA) and endobronchial ultrasound-guided TBNA (EBUS-TBNA) have been applied to the diagnosis for mediastinal lymph nodes. The aim of this study is to evaluate the clinical value and safety of TBNA and EBUS-TBNA on hilar and mediastinal lymph nodes of lung cancer patients.</p><p><b>METHODS</b>Two hundred fifty patients with suspected lung cancer were enrolled. All petients with hilar and/or m lymphoadenopa-ediastinal thy found by CT scan received TBNA, biopsy and brushing. EBUS-TBNA was performed in 15 patients among them.</p><p><b>RESULTS</b>Lung cancer were confirmed in 180 patients by TBNA, biopsy and brushing. The positive rates were 82.86%, 51.24% and 45.45%. Fifteen patients after EBUS-TBNA had a positive rate of 91.67%.</p><p><b>CONCLUSION</b>TBNA and EBUS-TBNA were proved to be safe procedure with a high yield for the diagnosis ofhilar and mediastinal lymph nodes in lung cancer patients.</p>

ABSTRACT

Objective:To observe the effects of Astragalus polysaccharides on antigen presentation of dendritic cells(DC).Methods:DC and CIK cells were generated by culturing PBMC of healthy blood donor.The typical DC and DC pulsed by Astragalus polysaccharides were co-cultured with CIK cells,respectively.Then the changes in the phenotypes of the DCs pulsed with Astragalus polysaccharides were determined.Cell surface markers were analysed by FACS method.IFN-? and IL-12 secreted by co-cultured cells were detected by ELISA.The cytotoxicity of effector cells on A549 cells in vitro were measured by MTT assays.Results:Astragalus polysaccharides could increase the expression of CD40,CD80 and HLA-DR on DC surface.The Astragalus polysaccharides pulsed DC-CIK cells resulted in an enhanced killing activity to A549 cells than that of unpulsd DC-CIK cells(P
