ABSTRACT

Os cuidados em saúde na Amazônia possuem singularidades, não só pela diversidade de seu território, mas pela pluralidade de suas gentes. Nesse sentido, o objetivo deste estudo foi analisar o perfil dos usuários que acessam a atenção básica de saúde no município de Manicoré, Amazonas, descrevendo os seus fatores socioeconômicos e demográficos. Trata-se, assim, de um estudo transversal, do tipo descritivo com abordagem quantitativa sobre o perfil de saúde e condições de vida dessa população. Entre os resultados mais relevantes destacam-se: alto índice de gravidez na adolescência; elevado quantitativo de gestantes com as consultas pré-natais consideradas inadequadas; elevada taxa de partos cesáreos; elevado número de notificações de doenças infecciosas e parasitárias e a diminuição da cobertura da Estratégia de Saúde da Família. A análise levantada neste estudo teve como base os dados registrados nos sistemas de informações considerando-se os dados epidemiológicos da cobertura da Estratégia de Saúde da Família, bem como dos demais registros produzidos nas diversas portas de entrada da rede de saúde do município de Manicoré, os quais devem ser entendidos de forma ampla alinhados com as diretrizes dos programas e metas estabelecidas pelo Ministério da Saúde. O papel do controle social na discussão das prioridades deve estar, portanto, alinhado com as principais dificuldades enfrentadas no município, as quais devem ser materializadas nos planos de saúde.

ABSTRACT Objective: Our aim was to assess the ability of serum magnesium (Mg), measured on the first postoperative day (Mg1PO), to predict the need for calcium (Ca) replacement in patients undergoing total thyroidectomy (TT). Subjects and methods: Eighty patients undergoing TT, with Mg1PO and PTH dosage in the first (PTH1h) and eighth (PTH8h) hours after TT, were evaluated for the need for Ca replacement. Data were evaluated by uni/multivariate logistic regression and Receiver Operating Characteristic (ROC) curve. Results: 32 patients (40%) required Ca replacement. Median PTH1h, PTH8h and Mg1PO were higher in the no replacement group: 17 versus (vs) 3 pg/mL (p < 0.001), 18.2 vs 3.0 pg/mL (p < 0.001) and 2 vs 1.6 mg/dL (p < 0.001), respectively. Mg1PO was the isolated predictor for this replacement (odds ratio = 0.0004, 95% confidence interval: 0.000003-0.04; p = 0.001), with the cut-off value of 1.8 mg/dL showing sensitivity and specificity of 78.1% and 87.5%, respectively. Conclusions: In this group of patients, serum Mg1PO was the isolated predictor for the need for Ca replacement.

ABSTRACT

The aim of this study was to investigate the symptoms of anxiety resulting from social distancing and its impacts on the Brazilian population during the COVID-19 pandemic. The study consisted of a cross-sectional survey, with data constructed through an online questionnaire that included sociodemographic information and information about anxiety symptoms. The questions were constructed considering the concept of "mental health waves", as being the mental health demands presented at the respective moment of the pandemic. The sample consisted of 632 participants, and the prevalence of anxiety identified was 81%, with no statistically significant differences between genders. The main factors related to anxiety were loneliness (OR=2.18, 95% CI=1.16-4.09), sleeping difficulties (OR=2.59, 95% CI=1.57-4.28), irritability (OR= 1.85, 95% CI=1.11-3.09), financial difficulties (OR=1.85, 95% CI=1.01-3.40), and problems related to emotional isolation (OR=2.43, 95% CI=1.47- 4.01). In the context of the challenges for mental health, the results obtained contribute to deeper understanding of the changes related to anxiety that emerged with the advent of social distancing policies and the context of the pandemic.


O objetivo deste estudo foi investigar os sintomas de ansiedade decorrentes do distanciamento social e seus impactos sobre a população brasileira durante a pandemia COVID-19. O estudo consistiu em uma pesquisa transversal, com dados construídos por meio de questionário online que incluía informações sociodemográficas e informações sobre sintomas de ansiedade. As questões foram construídas considerando o conceito de "ondas de saúde mental", como sendo as demandas de saúde mental apresentadas no respectivo momento da pandemia. A amostra foi composta por 632 participantes, e a prevalência de ansiedade identificada foi de 81%, sem diferenças estatisticamente significativas entre os sexos. Os principais fatores relacionados à ansiedade foram solidão (OR=2,18, 95% IC =1,16-4,09), dificuldades para dormir (OR=2,59, 95% IC =1,57-4,28), irritabilidade (OR= 1,85, 95% IC =1,11%-3,09), dificuldades financeiras (OR=1,85, 95% IC =1,01-3,40) e problemas relacionados ao isolamento emocional (OR=2,43, 95% IC =1,47-4,01). No contexto dos desafios para a saúde mental, os resultados obtidos contribuem para uma compreensão mais profunda das mudanças relacionadas à ansiedade que surgiram com o advento das políticas de distanciamento social e o contexto da pandemia.

Abstract The thickness and shade of a restoration will affect the transmission of light from the light-curing unit (LCU). This study determined the power (mW), spectral radiant power (mW/nm), and beam profile of different LCUs through various thicknesses and shades of a CAD-CAM resin composite (BRAVA Block, FGM). Five thicknesses: 0.5; 0.75; 1.0; 1.5, and 2.0 mm, in three shades: Bleach; A2 and A3.5 of a CAD-CAM resin (n = 5). Two single-peak LCUs: EL, Elipar DeepCure-S (3M Oral Care); and OP, Optilight Max (Gnatus), and one multiple-peak LCU: VL, VALO Grand (Ultradent), were used. The LCUs were positioned touching the surface of the BRAVA Block. The power and emission spectrum were measured using a fiberoptic spectrometer attached to an integrating sphere, and the beam profiles using a laser beam profiler. The effect of the material thickness on the light attenuation coefficients was determined. VL and EL delivered more homogeneous beam profiles than OP. The type of the BRAVA Block had a significant effect on the transmitted power, and wavelengths of transmitted light (p < 0.001). There was an exponential reduction in the power and emission spectrum as the thickness of the BRAVA Block increased (p < 0.001). The light transmission through the A2 shade was least affected by the thickness (p < 0.001). The attenuation coefficient was higher for the violet light and higher for A3.5 than the A2 or Bleach shades. No violet light from the VL could be detected at the bottom of 2.0 mm of the BRAVA Block.

Resumo: O objetivo do estudo foi analisar a tendência de atividade física dos escolares brasileiros e as associações com variáveis demográficas, socioeconômicas e comportamentais, por meio da Pesquisa Nacional de Saúde do Escolar (PeNSE) em suas quatro edições - 2009, 2012, 2015 e 2019. Foram usados dados dos escolares (13-17 anos) participantes das quatro edições da PeNSE (n = 392.922). Descrevemos o percentual de ativos, a média e valores percentuais da atividade física de intensidade moderada à vigorosa em minutos/semana. A regressão de Poisson foi ajustada para sexo, idade, cor da pele, escore de bens e comportamento sedentário (≥ 2 horas/dia para assistir TV e ≥ 3 horas/dia de tempo sentado). Como limitação, a amostra da PeNSE/2009 refere-se apenas às capitais brasileiras. O percentual de ativos reduziu de 43,1% em 2009 para 18,2% em 2019. A média em atividade física de intensidade moderada à vigorosa da PeNSE/2009 (média = 318,4 minutos/semana; IC95%: 313,4-323,4) reduziu 50% em 2019. Na educação física, a média semanal em atividade física de intensidade moderada à vigorosa das meninas foi menor que 50 minutos, ao passo que a dos meninos foi maior que 60 minutos, nas quatro edições da PeNSE. Ainda, 22,7% das meninas relataram (PeNSE/2019) não ter tido aulas de educação física, enquanto o mesmo é relatado por 19,7% dos meninos. O comportamento sedentário sofreu redução no hábito de assistir TV, porém o tempo sentado aumentou de 50,1% (IC95%: 48,9-51,3) para 54% (IC95%: 53,1-54,9) entre a PeNSE/2009 e a PeNSE/2019. Como consequência da queda nos níveis de atividade física, são necessárias políticas públicas que promovam a atividade física, como aumentar as aulas de educação física na escola para, no mínimo, três vezes por semana.


Abstract: This study aimed to analyze the physical activity trend of Brazilian schoolchildren and the associations with demographic, socioeconomic, and behavioral variables by using the Brazilian National Survey of School Health (PeNSE) in its four editions - 2009, 2012, 2015, and 2019. Data from students (13-17 years old) participating in the four editions of the PeNSE (n = 392,922) were used. We describe the percentage of active, mean, and percentile values of moderate to vigorous intensity physical activity in minutes/week. Poisson's regression was adjusted for gender, age, skin color, goods score, and sedentary behavior (≥ 2 hours/day watching TV and ≥ 3 hours/day sitting time). As a limitation, the PeNSE/2009 sample refers only to the Brazilian capital cities. The percentage of active students decreased from 43.1% in 2009 to 18.2% in 2019. The mean moderate to vigorous intensity physical activity of PeNSE/2009 (mean = 318.4 minutes/week; 95%CI: 313.4-323.4) decreased 50% in 2019. In physical education, the weekly average in moderate to vigorous intensity physical activity of girls is less than 50 minutes and boys is greater than 60 minutes in the four editions of PeNSE, also 22.7% of girls reported (PeNSE/2019) not having taken physical education classes whereas the same thing is reported by 19.7% of boys. Sedentary behavior reduced regarding time watching TV, but sitting time increased by 50.1% (95%CI: 48.9-51.3) and 54% (95%CI: 53.1-54.9) between PeNSE/2009 and PeNSE/2019. As a consequence of the drop in physical activity levels, public policies that promote physical activity are necessary, including increasing physical education classes at school to at least three times a week.


Resumen: El objetivo de este estudio fue analizar la tendencia de la actividad física del alumnado brasileño y sus asociaciones con las variables demográficas, socioeconómicas y de comportamiento mediante la Encuesta Nacional de Salud del Escolar (PeNSE) en sus cuatro ediciones (de 2009, 2012, 2015 y 2019). Se utilizaron los datos del alumnado (13-17 años) que participó en las cuatro ediciones de la PeNSE (n = 392.922). Se describieron el porcentaje de valores activos, la media y los valores percentiles de actividad física de intensidad moderada a vigorosa en minutos/semana. La regresión de Poisson se ajustó según sexo, edad, color de la piel, puntuación de activos y comportamiento sedentario (≥ 2 horas/día para ver televisión y ≥ 3 horas/día sentado). Como limitación, la muestra de la PeNSE/2009 se refiere únicamente a las capitales brasileñas. El porcentaje de activos disminuyó del 43,1% en 2009 al 18,2% en 2019. La media de actividad física de intensidad moderada a vigorosa de la PeNSE/2009 (media = 318,4 minutos/semana; IC95%: 313,4-323,4) tuvo una reducción de un 50% en 2019. En educación física, el promedio semanal en actividad física de intensidad moderada a vigorosa de las niñas fue menos de 50 minutos, y el de los niños llegó a 60 minutos en las cuatro ediciones de la PeNSE, además, el 22,7% de las niñas y el 19,7% de los niños (PeNSE/2019) informaron no haber tomado clases de educación física. Hubo una disminución en el comportamiento sedentario de ver televisión, pero el tiempo sentado aumentó del 50,1% (IC95%: 48,9-51,3) al 54% (IC95%: 53,1-54,9) entre la PeNSE/2009 y la PeNSE/2019. El descenso de los niveles de actividad física lleva a la necesidad de desarrollar políticas públicas que promuevan la actividad física, incluido el incremento de las clases de educación física en las escuelas al menos tres veces por semana.

Abstract Objective The use of a fiber glass post (FGP) type and choice of FGP diameter to restore endodontically treated incisors without ferrule is controversial. This study evaluated survival rate and failure mode of severely compromised central incisors without ferrule rehabilitated using resin-based composite (RBC) with or without FGP with different diameters. Methodology A total of 60 decoronated bovine incisors without a ferrule were endodontically treated and prepared for 1.4, 1.6, and 1.8 mm diameter FGPs (Whitepost System DC 0.5, Fit 0.4, and DCE 0.5; FGM). Half of the teeth received FGPs cemented using dual-cure resin cement (Allcem Core; FGM), the other half were filled using only bulk-fill RBC (OPUS Bulk Fill; FGM). The crowns were directly restored with RBC. The roots were embedded in polystyrene resin and the periodontal ligament was simulated with polyether impression material. Fatigue testing was conducted under 5 Hz cyclic loading at 30 degrees to the incisal edge, beginning at 50 N (5,000 cycles) as a warmup. After, the load was increased 100 N every 15,000 cycles until fracture occurred. All specimens were subjected to transillumination, micro-CT analysis, and digital radiography before and after fatigue testing. Fracture mode was classified according to severity and repair potential. Data were analyzed with Kaplan-Meier survival test and post hoc log-rank test (α=0.05) for pairwise comparisons. Results Using FGP significantly increased the number of cycles to failure, irrespective of FGP diameters (p=0.001). The FGP diameters had no statistically significant effect on cycles to failure or failure mode. Conclusion Using FGP without ferrule improved survival rate of structurally severely compromised central incisors compared with rehabilitation without FGP. The diameter of the FGPs had no effect on the survival rate and failure mode.

ABSTRACT BACKGROUND: Neuroendocrine tumors are rare neoplasms of uncertain biological behavior. The liver is one of the most common sites of metastases, occurring in 50% of patients with metastatic disease. AIMS: To analyze a clinical series in liver transplant of patients with neuroendocrine tumors metastases. METHODS: A retrospective descriptive study, based on the review of medical records of patients undergoing liver transplants due to neuroendocrine tumor metastases in a single center in northeast Brazil, over a period of 20 years (January 2001 to December 2021). RESULTS: During the analyzed period, 2,000 liver transplants were performed, of which 11 were indicated for liver metastases caused by neuroendocrine tumors. The mean age at diagnosis was 45.09±14.36 years (26-66 years) and 72.7% of cases were females. The most common primary tumor site was in the gastrointestinal tract in 64% of cases. Even after detailed investigation, three patients had no primary tumor site identified (27%). Overall survival after transplantation at one month was 90%, at one year was 70%, and five year, 45.4%. Disease-free survival rate was 72.7% at one year and 36.3% at five years. CONCLUSIONS: Liver transplantation is a treatment modality with good overall survival and disease-free survival results in selected patients with unresectable liver metastases from neuroendocrine tumors. However, a rigorous selection of patients is necessary to obtain better results and the ideal time for transplant indication is still a controversial topic in the literature.


RESUMO RACIONAL: Os tumores neuroendócrinos são neoplasias raras de comportamento biológico incerto. O fígado é um local comum de metástase, ocorrendo em 50% dos pacientes com doença metastática. OBJETIVOS: Analisar casuística de transplante hepático por metástases de tumores neuroendócrinos. MÉTODOS: Estudo descritivo retrospectivo com revisão de prontuários de pacientes submetidos a transplante hepático por metástases de tumores neuroendócrinos em um único centro no Nordeste do Brasil durante 20 anos (janeiro de 2001 a dezembro de 2021). RESULTADOS: Durante o período analisado, foram realizados 2.000 transplantes hepático, sendo 11 indicados por metástases hepáticas de tumores neuroendócrinos. A média de idade ao diagnóstico foi de 45,09±14,36 anos (26-66 anos) e 72,7% dos casos eram do sexo feminino. O local do tumor primário mais comum foi o trato gastrointestinal (64% dos casos). Após detalhada investigação, três pacientes não tiveram o local do tumor primário identificado (27%). A sobrevida global um mês e após um ano do transplante foi de 90 e 70%, respectivamente. A sobrevida após 5 anos foi de 45,4%. A taxa de sobrevida livre de doença foi de 72,7% no primeiro ano e 36,3% em cinco anos. CONCLUSÕES: O transplante hepático é uma modalidade de tratamento com bons resultados de sobrevida global e sobrevida livre de doença, em pacientes selecionados com metástases hepáticas irressecáveis de tumores neuroendócrinos. No entanto, a seleção rigorosa dos pacientes é necessária para obter melhores resultados e o momento ideal para a indicação do transplante ainda é um tema controverso na literatura.

ABSTRACT

Among the management and control tactics of the coffee berry borer Hypothenemus hampei (Ferrari), there is the use of entomopathogenic fungi. Due to the importance of prospecting isolates of entomopathogenic fungi for the control of the coffee berry borer, the objective of this study was to evaluate the efficiency of 26 isolates of entomopathogenic fungi in the control of this insect pest in the laboratory. The coffee berry borers were immersed in a solution adjusted to the concentration of 1 to 3 × 108 conidia/mL of each isolate and the control treatment (sterilized water). After seven days total mortality and confirmed mortality were evaluated. The isolates that caused the highest mortality and two commercial isolates were selected for evaluation of lethal concentration (LC50 and LC90) and lethal time (LT50 and LT90). Coffee berry borers were treated at different conidia concentrations for lethal concentration to assess total and confirmed mortality. For a lethal time, the coffee berry borers were treated at the concentration of 108 conidia/mL of the selected isolates, and, after two days and every 24 hours until the eighth day, the number of dead individuals was verified. Among the 26 isolates evaluated, 24 presented mortality higher than the control treatment, and three presented mortality higher than 85%. In the LC50 and LC90 assays, the IBCB 353 and IBCB 364 isolates were more lethal to H. hampei. In the LT50 and LT90 assays, the IBCB 66 and IBCB 353 isolates caused lethality in a shorter time.

ABSTRACT Objective: Cytological analysis and Bethesda classification of thyroid nodules is the standard method of diagnosing differentiated thyroid carcinoma (DTC). However, even for nodules with a non-malignant cytological diagnosis, there is a not insignificant risk of cancer. There are doubts whether this lack of certainty would influence patient prognosis. Our aim was to compare patients with DTC, classified according to the preoperative cytological diagnosis, regarding their evolution. Subjects and methods: A retrospective study was carried out with 108 DTC patients submitted to total thyroidectomy (TT) between 2009 and 2015, divided into three groups according to preoperative cytological diagnosis (Bethesda classification): classes I/II, III/IV, and V/VI. Groups were compared for evolution considering response to treatment at last evaluation as well as time disease free. Statistical analysis used ANOVA, chi squared, and Kaplan-Meier curves with p<0.05 considered significant. Results: Groups differed for time between nodule puncture and TT [in months; V/VI (2.35 ± 2.48) < III/IV (7.32 ± 6.34) < I/II (13.36 ± 8.9); p < 0.0001]. There was no significant difference between groups for evolution at final evaluation (disease free status; classes I/II: 71.4%; classes III/IV: 60%; classes V/VI: 66.6%; p = 0.7433), as well as time disease free (in months; classes I/II: 34.57 ± 25.82; classes III/IV: 38.04 ± 26.66; classes V/VI: 30.84 ± 26.34; p = 0.3841). Conclusions: DTC patients classified according to preoperative cytological diagnosis did not differ for evolution. Although patients with non-malignant cytological diagnoses were submitted to TT later, this did not affect the evolution of the cases.

Resumo O carcinoma anaplásico da tireoide corresponde ao câncer de menor incidência e de pior prognóstico. Neste relato, descrevemos o caso de uma paciente de 64 anos submetida a tireoidectomia total associada a esvaziamento cervical nível VI devido a carcinoma papilífero de tireoide. No seguimento, apresentou sinais de recidiva regional e foi submetida a esvaziamento cervical ampliado e esofagectomia cervical. No intraoperatório, observou-se ausência de plano de clivagem entre o tumor e a artéria carótida comum (ACC) e procedeu-se com implante de shunt carotídeo, resseção em bloco incluindo segmento da ACC e do esôfago acometidos e ponte vascular com interposição da veia safena magna. Foi evidenciado carcinoma anaplásico em revisão de lâmina do exame anatomopatológico. A paciente foi submetida ao tratamento adjuvante e não apresenta sinais de recidiva locorregional. Diante da possibilidade de proceder uma cirurgia curativa com ressecção em bloco, o cirurgião vascular deve estar apto para as opções cirúrgicas.


Abstract Of all thyroid cancers, anaplastic thyroid carcinoma (ATC) has the lowest incidence and worst prognosis. In this report, we describe a 64-year-old female patient who underwent total thyroidectomy and level VI neck dissection for papillary thyroid carcinoma. During follow-up, she showed signs of regional recurrence and underwent extended neck dissection and cervical esophagectomy. Intraoperatively, there was no cleavage plane between the tumor and the common carotid artery (CCA), so a carotid shunt was implanted and en bloc resection, including the affected CCA and esophagus segments was performed followed by vascular bypass with interposition of a great saphenous vein graft. A pathology review found evidence of anaplastic carcinoma. The patient underwent adjuvant treatment and has no signs of locoregional recurrence. Presented with the possibility of carrying out curative surgery with en bloc resection, the vascular surgeon must be prepared for the surgical options.

ABSTRACT Objective: To validate the cross-cultural adaptation of the English version of theYoung Spine Questionnaire (YSQ) to Portuguese. The questionnaire enables the evaluation, in children, of back pain prevalence and frequency, restrictions on activity, care-seeking behavior, and the influence that the parents' manifestations of back pain can have. Method: The research was conducted in a school setting, with a sample of 128 children aged 9 to 12 years (63 males and 65 females). The questionnaire was completed at two different moments. Test-retest reliability was used to evaluate the degree of reproducibility, and the intraclass correlation coefficient (ICC) between the two sets of observed values was estimated for p<0.05. Cronbach's alpha coefficient was used to assess the degree of reliability (internal consistency) of the questionnaire. Results: For most of questions, the ICC ranged between 0.527 and 0.870, with reasonable to excellent correlation to the reproducibility index. The exceptions were foritem c) in two of the first three questions, where the ICC values (0.149 and 0.277) were lower. The value of Cronbach's alpha calculation was 0.8, indicating high internal consistency. Conclusion: It was demonstrated that the cross-cultural adaptation of the YSQ had content validity, was well-understood, and suitable for use in studies with Portuguese children between 9 and 12 years of age. RESUMO: Objetivo: Objetivo: Validar a adaptação transcultural da versão inglesa do "The Young Spine Questionnaire" (YSQ), para a língua portuguesa. O questionário permite avaliar, em crianças, a taxa de prevalência e a frequência de dor na coluna, as restrições de atividade, o comportamento de procura de cuidados e a influência que pode ter a dor de coluna manifestada pelos pais. Método: A pesquisa foi realizada em contexto escolar, com uma amostra de 128 crianças dos 9 aos 12 anos (63 do sexo masculino e 65 do sexo feminino). O questionário foi preenchido em dois momentos distintos. Para avaliar o seu grau de reprodutibilidade utilizou-se o Teste-Reteste, estimando-se o coeficiente de correlação intraclasse (ICC) entre os dois conjuntos de valores observados, para p<0,05. Também se recorreu ao coeficiente Alfa de Cronbach, para encontrar o grau de confiabilidade (consistência interna) do questionário. Resultados: Na maioria das questões o intervalo do ICC situou-se entre 0,527 e 0,870, correspondente a uma correlação situada entre razoável a excelente para índice de reprodutibilidade. A exceção foram dois dos itens c) das três primeiras questões, em que o ICC apresentou valores inferiores (0,149 e 0,277). O valor do cálculo do Alfa de Cronbach resultou em 0,8, apresentando elevada consistência interna. Conclusão: Ficou demonstrado que a adaptação transcultural do YSQ tem validade de conteúdo e é bem compreendida para ser utilizada em estudos com crianças portuguesas dos 9 aos 12 anos de idade. RESUMEN: Objetivo: Validar la adaptación transcultural de la versión inglesa del "The Young Spine Questionnaire" (YSQ) al portugués. El cuestionario permite evaluar, en los niños, la tasa de prevalencia y frecuencia del dolor de espalda , las restricciones de actividad, el comportamiento de búsqueda de atención y la influencia que puede tener el dolor de espalda manifestado por los padres. Método: La investigación se llevó a cabo en un entorno escolar, con una muestra de 128 niños de 9 a 12 años (63 niños y 65 niñas). El cuestionario se completó en dos momentos diferentes. Para evaluar su grado de reproducibilidad, se utilizó el "Test-Retest", estimando el coeficiente de correlación intraclase (CCI) entre los dos conjuntos de valores observados, para p <0,05. También se utilizó el coeficiente Alfa de Cronbach, para conocer el grado de confiabilidad (consistencia interna) del cuestionario. Resultados: En la mayoría de las preguntas, el CCI osciló entre 0,527 y 0,870, lo que corresponde a una correlación situada entre razonable y excelente para el índice de reproducibilidad. Las excepciones fueron dos de los ítems c) de las tres primeras preguntas, donde el ICC mostró valores más bajos (0,149 y 0,277). El valor del cálculo del Alfa de Cronbach dio como resultado 0,8, lo que demuestra una alta consistencia interna. Conclusión: Se demostró que la adaptación transcultural del YSQ tiene validez de contenido y es adecuada para ser utilizada en estudios con niños portugueses de entre 9 y 12 años. Abstract This study evaluated the effect of the use of glass ionomer cement (GIC) and flowable bulk-fill resin composite (BFRC) for filling pulp chambers and the type of high-speed handpiece light used on dentin removal during access preparation for endodontic retreatment in molar teeth. Twenty maxillary molars were treated endodontically. BFRC (Opus Bulk Fill Flow APS, FGM) was used to fill the pulp chamber and replace coronal dentin (n = 10). In the remaining teeth, the pulp chamber was filled with GIC (Maxion R, FGM). Conventional resin composite (Opallis, FGM) was used to restore the enamel layer in all teeth. The samples in each group were divided into two subgroups, and the root canals were reaccessed using a handpiece with white or ultraviolet light. The teeth were scanned using micro-CT before and after root canal reaccess. The dentin volume removed was calculated and analyzed using 2-way analysis of variance and Tukey's test (α = 0.05). The crown and pulp chamber locations with dentin removal are described using frequency distribution. During the access, fewer pulp chamber walls were affected and a lower volume of dentin was removed from the pulpal floor in the group restored with GIC than in the group restored with BFRC. No effect was observed on the coronal dentin walls with respect to the filling protocols and type of light used. For dentin removal from the pulp chamber, handpieces with white light performed better than those with ultraviolet light, irrespective of the filling protocol used. The use of GIC to fill the pulp chamber and use of white handpiece light reduced dentin removal from the pulpal floor and resulted in fewer affected dentin walls.


Resumo Este estudo avaliou os efeitos do cimento de ionômero de vidro (GIC) e da resina composta fluida bulk fill (BFRC) usados como preenchimento da câmara pulpar; e o tipo de iluminação das turbinas de alta rotação na remoção dentinária após cavidades de acesso para retratamento endodôntico em dentes molares. Vinte molares superiores foram tratados endodonticamente. Dez dentes foram restaurados usando BFRC (Opus Bulk Fill Flow APS, FGM) para preencher a câmara pulpar e dentina coronária; e resina composta convencional (Opallis, FGM) para restaurar a camada de esmalte. Os outros dentes foram restaurados usando GIC (Maxion R, FGM) para preencher a câmara pulpar e resina composta (Opallis, FGM). As amostras foram divididas em dois grupos e os canais radiculares foram novamente acessados com turbina de alta-rotação com iluminação branca ou ultravioleta. Os dentes foram escaneados usando micro-CT antes e após o novo acesso ao canal radicular. O volume de dentina removida foi calculado e os dados foram analisados por ANOVA bidirecional e teste de Tukey (α=0,05). As regiões na coroa e na câmara pulpar que apresentaram dentina removida no acesso dos canais foram descritas por meio de distribuição por frequência. A reabertura do canal radicular com GIC resultou em menos paredes afetadas da câmara pulpar e menor volume de dentina removida no assoalho. Nenhum efeito foi observado nas paredes de dentina coronária considerando aos protocolos de preenchimento. A turbina de alta rotação com iluminação branca reduziu a remoção de dentina da câmara pulpar, independentemente do protocolo de restauração utilizado. O uso de turbina de alta rotação com iluminação branca e GIC para preencher a câmara pulpar reduziram a remoção de dentina do assoalho e afetaram menos paredes dentinárias.

ABSTRACT Objective: Thyrotropin-stimulated thyroglobulin (STg) after total thyroidectomy is a prognosis marker for differentiated thyroid carcinoma (DTC). As Tg level is influenced by thyrotropin (TSH), perhaps the STg/TSH ratio is also a prognosis marker for these tumours. We aimed to compare STg/TSH ratio and first STg level in differentiated thyroid carcinoma patients for their ability to predict the long-term response to initial treatment. Subjects and methods: This retrospective study evaluated data from 181 DTC patients for first (1st) STg and STg/TSH ratio, at 1-3 months post-total thyroidectomy and before iodine-131 therapy, according to response to initial therapy [Excellent/Indeterminate or Incomplete (Biochemical/Structural)] observed at final evaluation, and with the survival time with excellent/indeterminate response. Results: Cases with incomplete response presented higher STg level [225.13 ± 585.26 ng/mL versus (vs) 20.4 ± 192.9 ng/mL; p < 0.001] and STg/TSH ratio (3.01 ± 7.8 vs 0.27 ± 2.58; p < 0.001). Cutoffs of 5 ng/mL for STg and 0.085 for STg/TSH displayed sensitivities of 76.7% and 76.9%, and specificities of 79.2% and 82.6%, respectively, in predicting response to therapy. Values below these cutoffs were associated with longer survival time in excellent/indeterminate response (140.4 vs 15.9 and 144.6 vs 15.9 months, respectively). Conclusion: STg/TSH ratio has a similar performance to the 1st STg in predicting long-term response to initial therapy.

ABSTRACT

This study aimed to analyze the shear bond strength (SBS) of ceramic orthodontic brackets bonded with two different adhesive systems to intact and eroded teeth. Ceramic brackets were bonded to 72 bovine central incisors divided into four groups, defined by two study factors: enamel condition (control group, kept in artificial saliva; and experimental group, eroded by using immersion cycles in Coke™ for 90 seconds, every six hours for five days), and adhesive system type (Transbond™ XT or Transbond™ Plus Color Change). Polycrystalline ceramic brackets were adhesively fixed on all specimens using the same light curing protocol. SBS was tested using 0.5 mm/min and the failure mode was classified. SBS data was analyzed using two-way ANOVA followed by Tukey test. The adhesive remnant index (ARI) scores were analyzed using Kruskal-Wallis test with Dunn's post-hoc pairwise comparison (α=0.05). Percentages of ARI scores between the groups were compared by Fisher's exact test. Spearman's correlation coefficient was applied to investigate the correlation between ARI scores and SBS values. Only the adhesive system factor had significant effect on SBS (p=0.014), Transbond™ Plus Color Change showing higher values. No significance was found for enamel condition (p=0.665) or the interaction between adhesive system and enamel condition (p=0.055). ARI scores frequencies differed between groups (p<0.001). The median ARI scores were statistically different for most comparisons among the groups. However, no significant correlation was found between ARI scores and SBS. In conclusion, the type of adhesive system affected the SBS of ceramic brackets to dental enamel, but the enamel condition, intact or eroded, had no significant effect. There was no correlation between ARI scores and SBS values, although eroded enamel tended to retain more adhesive after bracket removal.

Abstract: Three-point bending test is the most common mechanical test used for quantifying the biomechanical quality of bone tissue and bone healing in small animals. However, there is a lack of standardization for evaluation of bone repair by cortical perforation. The aim of this study was to determine the influence of bone defect position in the proximal metaphysis of rat tibias during load application and different span configuration on the three-point bending test outcomes. Cortical defects with 1.6 mm diameter were created at a standardized location on the medial surface of 60 tibias of male Wistar rats. The animals were euthanized 7 days after surgery. Five specimens were used to create 3D models for finite element analysis using high-resolution micro-CT images. Two spans (6 and 10mm) and three positions of the bone defect in relation to the load application (upward, frontal and downward) were evaluated experimentally (n = 10) and in finite element analysis (n = 5). Maximum load (N) and stiffness (N/mm) were statistically analyzed with 2-way ANOVA and Tukey test (α = 0.05). The results demonstrated that span and orientation of the bone defect significantly influenced the fracture pattern, stress distribution and force versus displacement relation. Therefore, reliable outcome can be achieved creating the bone defect at 8 mm from the extremity of the proximal epiphysis; placing a 10 mm distance span and downward facing defect position to allow a better distribution of stress and more fracture patterns that reached the bone defect target area with less intra-group variability.

Abstract: The aim of this study was to investigate the effects of different post space irrigation protocols for removing residual filling material from dentin walls, by using microcomputed tomography (micro-CT), and the influence of these protocols on dentin microhardness. Bovine incisors (n = 35) were filled with the single-cone technique and MTA Fillapex (Angelus, Londrina, PR, Brazil). Post space preparation (PSP) was performed 7 days after filling, using the Odous Touch electrical system (Odous De Deus Ind. e Com., Belo Horizonte, MG, Brazil), followed by post space irrigation using manual irrigation, passive ultrasonic irrigation, or Easy Clean, together with 2.5% sodium hypochlorite (NaOCl), or with 2.5% NaOCl and 17% EDTA (NaOCl/EDTA). Micro-CT scans were performed at three time points. The residual filling material was evaluated at three levels: cervical, middle and apical. The Knoop test was measured with four indentations around the canal lumen at three dentin depths: X (100 μm), Y (200 μm) and Z (400 μm). Statistical analysis was performed using ANOVA (p < 0.05). The effects of the activation method (p < 0.001), and the root level (p = 0.013), as well as the interaction between the irrigant and the activation method (p = 0.041), led to different percentages of residual filling material. Lower amounts of residual filling material were observed at the cervical versus the middle and apical levels (p < 0.05). No significant differences were observed in dentin microhardness (p > 0.05). The best removal of the residual filling material was performed using the Easy Clean tip and NaOCl/EDTA, regardless of the activation methods.

Abstract: To evaluate the effect of selective or nonselective carious tissue removal and the use of a resin-modified glass ionomer (RMGIC) liner under bulk fill resin composite restoration on the stress at the pulp chamber, the elastic moduli of hard, firm, soft and intact dentin were calculated using nanoindentation. Post-gel shrinkage of the bulk fill resin composite and RMGIC were determined using the strain-gauge method. Six finite element models were created by using digital radiography with the combination of two study factors: a) carious tissue removal: selective removal or nonselective removal of carious tissue, and b) use of RMGIC liner: with or without 1.0 mm of RMGIC liner. The modified von Mises stresses (mvm) (MPa) were extracted on the nodes of the internal wall of the pulp ceiling chamber at 100 N occlusal loading. Data were analyzed descriptively and recorded quantitively. Both study factors influenced the stress distribution. The mvm stress during the restorative procedure was higher for nonselective carious tissue removal without RMGIC (25.9 MPa) and lower for selective carious tissue removal associated with RMGIC (13.5 MPa). The dentin elastic modulus increased from soft carious (3.6 ± 0.3 MPa) to firm carious (5.2 ± 1.0 MPa) to hard carious (10.9 ± 1.2 MPa) to intact dentin (22.7 ± 3.0 MPa). Molars with carious lesions showed high mvm stress at the pulp ceiling (89.6 MPa) and at fragilized coronal structure remaining. Selective carious tissue removal followed by restoration using a Vitrebond liner and Tetric N-Ceram Bulk fill reduced the stress at the pulp chamber ceiling.

ABSTRACT The IgG4-related disease (IgG4-RD) is a systemic disease recently characterized as an inflammatory condition generally related to the increase in serum IgG4 levels, a subclass of immunoglobulins (IgG) which corresponds to less than 6% of the total serum IgG, with singular histopathological features. The involvement of the central nervous system is rare and may be isolated or associated with other organs, mimicking tumors. Commonly, it involves the hypophysis, presenting hypophysitis as the main manifestation, but it can also affect the dura mater, presenting as IgG4-related hypertrophic pachymeningitis (IgG4-RHP). Neurological manifestations occur as a result of mass effect, typically due to vascular or nervous structures compression, resulting in functional deficits according to the anatomical site of the lesion. The main histopathological features are dense lymphoplasmacytic infiltrate, fibrosis arranged, at least focally, in a storiform pattern, and obliterative phlebitis, associated with increased numbers of IgG4+ plasma cells or an increased IgG4/IgG ratio in tissue. In this disease, the serum IgG4 levels are usually increased. The objective of this article is to report the case of a 37-year-old male patient who presented a pulsatile headache associated with diplopia and blurred vision. After radiological, histopathological and immunohistochemical studies, the diagnosis of IgG4-RHP was confirmed, besides presenting a literature review about IgG4-RD and IgG4-RHP.


RESUMEN La enfermedad relacionada con inmunoglobulina G4 (ER-IgG4) es una enfermedad sistémica recientemente caracterizada como condición inflamatoria que se relaciona con el aumento sérico de IgG4, una subclase de inmunoglobulinas (IgG) que incluye menos de 6% del total de IgG séricas, con características histopatológicas propias. El acometimiento del sistema nervioso central (SNC) es raro y puede ser aislado o asociado a otros órganos; es muy confundido con tumores. Es más común en la hipófisis, siendo la hipofisitis su principal manifestación, pero puede acometer también la duramadre, manifestándose como paquimeningitis hipertrófica relacionada con IgG4 (PHR-IgG4). Las presentaciones neurológicas se atribuyen al efecto de masa, tipicamente por compresión de estructuras vasculares o nerviosas, produciendo déficits funcionales según la ubicación anatómica de la lesión. Los hallazgos histológicos más comunes son infiltrado linfoplasmocítico denso, fibrosis (disposta, por lo menos focalmente, en un patrón estoriforme) y flebitis obliterante, además de elevación del número de células plasmáticas IgG4+ o de la proporción IgG4/IgG en el tejido. En esa enfermedad, los niveles séricos de IgG suelen estar elevados. El objetivo de este trabajo es reportar el caso de un paciente masculino con cuadro de cefalea pulsátil asociada a diplopía y visión borrosa. Luego de estudio radiológico, histopatológico y inmunohistoquímico, se obtuvo el diagnóstico de PHR-IgG4. Aún presentamos una revisión de literatura sobre la ER-IgG4 y la PHR-IgG4.


RESUMO A doença relacionada com a imunoglobulina G4 (DRIgG4) é uma enfermidade sistêmica recentemente caracterizada como condição inflamatória que se relaciona com o aumento sérico de IgG4, uma subclasse de imunoglobulinas (IgG) que compreende menos de 6% do total de IgG séricas, com características histopatológicas singulares. O acometimento do sistema nervoso central (SNC) é raro e pode ser isolado ou associado a outros órgãos; é muito confundido com tumores. É mais comum na hipófise, sendo a hipofisite sua principal manifestação, mas pode acometer também a dura-máter, manifestando-se como paquimeningite hipertrófica associada à IgG4 (PH-DRIgG4). As apresentações neurológicas são creditadas ao efeito de massa, tipicamente por compressão de estruturas vasculares ou nervosas, propiciando déficits funcionais de acordo com o sítio anatômico da lesão. Os achados histológicos mais comuns são infiltrado linfoplasmocitário denso, fibrose (em arranjo, pelo menos focalmente, de padrão estoriforme), e flebite obliterativa, além de elevação do número de plasmócitos IgG4+ ou da razão IgG4/IgG no tecido. Nessa doença, os níveis séricos de IgG4 geralmente estão elevados. O objetivo deste artigo é relatar o caso de um paciente do sexo masculino com quadro de cefaleia pulsátil associada à diplopia e turvação visual. Após estudo radiológico, histopatológico e imuno-histoquímico, foi obtido o diagnóstico de PH-DRIgG4. Apresentamos, ainda, uma revisão de literatura sobre DRIgG4 e PH-DRIgG4

Abstract The aim of this study was to analyze the influence of orthodontic bracket type (metallic or ceramic) and mouthguard on biomechanical response during impact. Two-dimensional plane-strain models of a patient with increased positive overjet of the maxillary central incisor was created based on a CT scan, simulating the periodontal ligament, bone support, gingival tissue, orthodontic brackets (metallic or ceramic) and mouthguard. A nonlinear dynamic impact finite element analysis was performed in which a steel object hit the model at 1 m/s. Stress distributions (Von Mises and Modified Von Mises) and strain were evaluated. Stress distributions were affected by the bracket presence and type. Models with metallic and ceramic bracket had higher stresses over a larger buccal enamel impact area. Models with ceramic brackets generated higher stresses than the metallic brackets. Mouthguards reduced the stress and strain values regardless of bracket type. Mouthguard shock absorption were 88.37% and 89.27% for the metallic and ceramic bracket, respectively. Orthodontic bracket presence and type influenced the stress and strain generated during an impact. Ceramic brackets generated higher stresses than metallic brackets. Mouthguards substantially reduced impact stress and strain peaks, regardless of bracket type.


Resumo O objetivo deste estudo foi analisar a influência da presença e tipo de bráquete ortodôntico (metálico ou cerâmico), e a presença de protetor bucal na resposta biomecânica durante impacto. Modelos bidimensionais em estado plano de deformação de paciente com incisivo central superior com overjet positivo acentuado foram criados baseados em tomografia computadorizada, simulando ligamento periodontal, suporte ósseo, tecido gengival, bráquetes ortodônticos (metálico e cerâmico) e o protetor bucal. Análise de elementos finitos não-linear de impacto foi realizada na qual uma esfera de aço atingiu o modelo a 1m/s. A distribuição de tensões (Von Mises e Von Mises modificado) e a deformação foram avaliadas. As distribuições de tensões foram afetadas pela presença e tipo de bráquete. Modelos com bráquete metálico e cerâmico produziram maiores valores de tensões sobre maior área do esmalte vestibular. Modelos com bráquetes cerâmicos geraram maiores tensões do que metálicos. O protetor bucal reduziu as tensões e deformações geradas independentemente do tipo de bráquete. A capacidade de absorção de choques foi de 88.37 e 89.27% para os bráquetes metálicos e cerâmicos, respectivamente. A presença e o tipo de bráquete influenciou a distribuição de tensões e deformações durante o impacto. Bráquetes cerâmicos geraram maiores valores de tensão do que metálicos. Protetor bucal reduziu significativamente os picos de tensão e deformação.

Abstract The aim of this study was to analyze the structural, morphological and mechanical properties of two different lithium disilicate glass-reinforced ceramics for CAD-CAM systems (IPS e.max CAD and Rosetta SM). Five methodologies were used for both ceramics: microstructure (n = 2) was analyzed using x-ray diffraction (XRD); morphological properties (n = 2) were analyzed by scanning electron microscopy (SEM), with and without hydrofluoric etching; porosity (n = 3) was assessed using 3D micro-computed tomography (micro-CT); flexural strength was measured (n =1 0) using the three-point bending test; and bond strength was determined with self-adhesive resin cement (n = 10), using a microshear bond test. After performing all the tests, the data were analyzed using t-Student test and two-way ANOVA. All the tests used a significance level of α = 0.05. High peak positions corresponding to standard lithium metasilicate and lithium disilicate with similar intensities were observed for both ceramics in the XRD analysis. Morphological analysis showed that the crystalline structure of the two ceramics studied showed no statistical difference after acid etching. Additionally, no significant differences were recorded in the number or size of the pores for the ceramics evaluated. Moreover, no differences in flexural strength were found for the ceramic materials tested, or in the bond strength to ceramic substrates for the resin cements. Based on the study results, no significant differences were found between the two CAD-CAM lithium disilicate glass-reinforced ceramics tested, since they presented similar crystalline structures with comparable intensities, and similar total porosity, flexural strength and bond strength.
