ABSTRACT

OBJECTIVE: To establish an HPLC method for the determination of total(Ct) and free(Cf) concentrations of teicoplanin (TEIC) in plasma. METHODS: For determing the free concentration, the plasma samples were prepared by ultrafiltration. A C18 column was used, with acetonitrile -0.01 mol•L-1 sodium dihydrogen phosphate (25∶75, pH adjusted to 3.3 by phosphoric acid) as the mobile phase, the detection wavelength was set at 240 nm, the column temperature was maintained at 35℃, and the flow rate was 1.0 mL•min-1. The specificity, linearity, the lower limit of quantitation, precession, recovery and stability of the developed method were validated. RESULTS: The free concentration was linear with in 0.5-50 μg•mL-1, the lower limit of quantification was 0.5 μg•mL-1, and the method recovery rate was 94.63%-103.72%. The intra-day and inter-day precision(RSD) were all less than 4.00; the linearity was good with in the total concentration of 1.562 5-100 μg•mL-1, the lower limit of quantification was 1.562 5 μg•mL-1, the method recovery rate was 94.55%-99.59%. The intra-day and inter-day precision(RSD) were all less than 4.00. The TEIC was all stability at the conditions of maintaining at room temperature for 10 h, after freeze-thaw cycle and keeping at 4℃ for 72 h. CONCLUSION: The method is simple, accurate, and sensitive, and is suitable for the clinical determination of total and free concentrations of teicoplanin and the study of pharmacokinetics.

ABSTRACT

The present study is to establish a quantitative analysis of multi-components by single marker(QAMS) for determining contents of seven compositions in Alismatis Rhizoma, alismoxide, alisol C 23-acetate, alisol A, alismol, alisol B, alisol B 23-acetate and 11-deoxy-alisol B. Six relative correction factors(RCFs) of alismoxide, alisol C 23-acetate, alisol A, alismol, alisol B and 11-deoxy-alisol B were established in the UPLC method with alisol B 23-acetate as the internal standard, which was to calculate the mass fraction of each. The mass fraction of seven effective constituents in Alismatis Rhizoma was calculated by the external standard method(ESM) at the same time. Compared with the content results determined by the ESM and QAMS, the feasibility and accuracy of QAMS method were verified. Within the linear range, the RCFs of alismoxide, alisol C 23-acetate, alisol A, alismol, alisol B, 11-deoxy-alisol B were 0.946, 4.183, 0.915, 1.039, 0.923 and 1.244, respectively, with good repeatability in different experimental conditions. There was no significant difference between the QAMS method and ESM method. Then, QAMS method was applied to determination of the different degree Alismatis Rhizoma from different areas. As a result, the concentrations of 7 components have differences in different areas, but no significant differences in different grades. The QAMS method is feasible and accurate for the determination of the seven chemical compositions, and which can be used for quality control of Alismatis Rhizoma.

ABSTRACT

Aim To investigate the effect of ginseng polysaccharides(WGP) on renal fibrosis in mice with diabetic nephropathy(DN) and its possible mecha-nism. Methods Diabetic mice were induced by intra-venous injection with 120 mg·kg-1streptozotocin (STZ) and were randomly divided into the following four groups with 10 mice per group: model group, low-, medium-, high-dose(25,50,100 mg·kg-1) of WGP groups. Other 10 normal mice were treated as normal control group. The mice were administered o-rally for 12 weeks. After that,the levels of fasting blood-glucose(FBG),microalbuminuria(mAlb),serum creatinine(Scr) and blood urea nitrogen(BUN) were detected. The expression of collagen fibers and the ex-pression of α-SMA protein in renal cortex were detec-ted using Masson staining and immunohistochemical staining. The cAMP content in renal cortex was detec-ted by radioimmunoassay. The protein expressions re-lated to extracellular matrix composition and PKA/CREB in renal cortex were detected by Western blot.Results WGP could obviously reduce the contents of FBG,mAlb,Scr and BUN in DN mice,meanwhile de-creasing the expressions of α-SMA,LN and FN protein by inhibiting the activation of cAMP/PKA/CREB sig-nal pathway,which led to the alleviation of degree of glomerular sclerosis and interstitial fibrosis in kidney. Conclusion WGP has inhibitory effect on renal fibro-sis in DN mice model,which might be related with the suppression of cAMP/ PKA /CREB signaling pathway.

BACKGROUND: In recent years, genetic haploidentical peripheral blood stem cell transplantation has been gradually improved, and haploid allogeneic hematopoietic stem cell transplantation has become an important treatment choice for malignant hematopoietic disease. OBJECTIVE: To observe the clinical efficacy of genetic haploidentical peripheral blood stem cell transplantation for myelodysplastic syndrome. METHODS: The clinical data of 21 myelodysplastic syndrome cases undergoing genetic haploidentical peripheral blood stem cell transplantation were retrospectively analyzed. Modified BU/CY+ATG administration was performed as a pretreatment strategy for haploidentical peripheral blood stem cell transplantation, and the combined use of cyclosporine A+mycophenolate mofetil+short-range methotrexate±basiliximab was adopted to prevent graft-versus-host disease (GVHD). RESULTS AND CONCLUSION: (1) The 21 cases were followed for an median of 333 days (22-1 222 days), with 76% (16/21) infection of granulocyte lack period, 100% (21/21) neutrophil reconstruction, the median implantation time of 12 days (7-17 days), 81% (17/21) platelet engraftment, and the median implantation time of 14 days (7-68 days). (2) The accumulative incidence of GVHD was 52.4% (11/21), including 29% (6/21) of acute GVHD and 24% (5/21) of chronic GVHD. The incidence of hemorrhagic cystitis was 38.1% (8/21). The recurrence rate after transplantation was 4.8% (1/21). (3) The 2-year non-relapse mortality was 48% (10/21), and the 2-year disease-free survival rate was 46.8%. These results show that in the absence of HLA-identical related donors and unrelated donor, genetic haploidentical peripheral blood stem cell transplantation is a safe, effective, feasible and alternative treatment option for myelodysplastic syndrome.

ABSTRACT

BACKGROUND: Recurrence of acute leukemia after hematopoietic stem cell transplantation is one of the major problems affecting the long-term survival of patients. Early intervention to prevent ALL recurrence after transplantation can improve disease-free survival, overall survival and reduce post-transplant mortality. Monitoring of minimal residual disease (MRD) by flow cytometry and PCR-based molecular biology techniques is a widely reliable and practicable method. OBJECTIVE: To dynamically monitor the MRD level of acute lymphoblastic leukemia after peripheral blood haploidentical hematopoietic stem cell transplantation and to explore its implications for predicting early relapse. METHODS: A retrospective study was conducted in 53 patients with acute lymphoblastic leukemia who had underwgone peripheral blood haploidentical hematopoietic stem cell transplantation at the First Affiliated Hospital of Zhengzhou University from June 2011 to June 2017. The patients were followed up for postoperative 1, 3, 6, 12 months to observe the relation between MRD levels and relapse after transplantation. RESULTS AND CONCLUSION: (1) The disease-free survival rate of MRD positive group and MRD negative group were 20.0% and 65.8%, respectively; and the overall survival rates were 50.8% and 68.9% in the two groups, respectively. There were significant differences between two groups. (2) Among 16 MRD positive patients accepting clinical intervention after transplantation, 4 patients presented with MRD negative and had no recurrence. (3) Eleven hematologic recurrence patients were given tyrosine kinase inhibitor-targeted therapy, chemotherapy, donor lymphocytes Infusion and secondary transplantation, but they eventually died. The median time from the discovery of MRD positive to hematologic recurrence was 100 (7-190) days, and during this period. Clinical intervention was confirmed to extend the recurrence time. In this study, one case refused clinical intervention, and eventually died of recurrence. Our findings indicate that dynamic monitoring of the MRD level in acute lymphoblastic leukemia patients after peripheral blood haploidentical transplantation can predict recurrence, by which the patients can be given early intervention to reduce the risk of recurrence and improve disease-free survival and overall survival.

ABSTRACT

<p><b>OBJECTIVE</b>To analyze the clinical characteristics of cytomegalovirus(CMV) infection after allogenic hematopoietic stem cell transplantation(allo-HSCT) and the effect of preemptive therapy.</p><p><b>METHODS</b>A total of 134 patients who underwent allo-HSCT from March 2010 to March 2015 in the Department of Hematology of our hospital were enrolled in this study. The CMV infection rate, the median time of CMV infection occurence, and the risk factors for CMV infection after allo-HSCT, the response rate of preemptive treatment and the median time of CMV-DNA turning negative were analyzed. Five-year overall survival rate was compared between the patients with or without CMV infection.</p><p><b>RESULTS</b>The incidence of CMV viremia was 55.2%(74/134), and the median time for the CMV with CMV-DNA positive for the first time was 34 days(14-283) after allo-HSCT.Both univariate and multivariate analysis showed that the thymoglobulin(ATG) used in conditioning regimen and Ⅱ-Ⅳ grade of aGVHD were the risk factors for CMV viremia. After preemptive treatment the 85.1% of patient with CMV viremia turned negative, and the median time of CMV-DNA turning negative were 15 days(5-82), only 2 patients died of CMV pneumonia. Five-year overall survival rate of the patients with or wihout CMV viremia was 49% and 66.3% respectively, and the difference between the 2 groups was significant(P=0.041).</p><p><b>CONCLUSION</b>The ATG used in conditioning regimen and Ⅱ-Ⅳ grade of aGVHD may increase the incidence of CMV infection after allo-HSCT, and the preemptive thrapy can effectively prevent the CMV viremia turning to CMV disease.</p>

ABSTRACT

Despite its dual role in determining cell fate in a wide array of solid cancer cell lines, autophagy has been robustly shown to suppress or kill acute myeloid leukemia cells via degradation of the oncogenic fusion protein that drives leukemogenesis. However, autophagy also induces the demise of acute leukemia cells that do not express the known fusion protein, though the molecular mechanism remains elusive. Nevertheless, since it can induce cooperation with apoptosis and differentiation in response to autophagic signals, autophagy can be manipulated for a better therapy on acute myeloid leukemia.

ABSTRACT

<p><b>OBJECTIVE</b>To study the adaptive response mechanisms in high background radiation area (HBRA) among Yangjiang local people through gene and protein expression of receptor for advanced glycation end products (RAGE) and S100A6 in peripheral blood and sputum in inhabitants of HBRA.</p><p><b>METHODS</b>A total of 53 male inhabitants were selected from HBRA in Yangjiang as the exposure group, while 53 male inhabitants were selected from Enping (control area, CA)as the control group. The content of RAGE and S100A6 gene and protein were detected by RT-PCR and Western blotting assay. Thermo luminescent dosemeter(TLD) assay was used to measure the outside dose and estimate the effective dose.</p><p><b>RESULTS</b>The effective dose in CA and HBRA was respectively 1.95 mSv and 6.24 mSv, which was 3 fold difference. Compared with CA, RAGE and S100A6 expression were significantly reduced in both gene and protein level in HBRA. The relative median mRNA expression of RAGE and S100A6 in peripheral blood were respectively 0.28, 1.06 and 0.16, 0.79 in CA and HBRA group, there was significance (with analysis Z values of -2.587 and -2.328 respectively, P < 0.05) with Wilcoxon rank test. For the protein of sputum, the relative median expression were respectively 2.98, 2.25 and 0.53, 0.47 with significant difference (with analysis Z values of -2.201 and -2.366 respectively, P < 0.05) by Wilcoxon rank test.</p><p><b>CONCLUSION</b>The low expression of RAGE and S100A6 in HBRA group might be correlated with the adaptive response and the low mortality of cancer in HBRA.</p>

ABSTRACT

Objective To study the relation between stroke and both plasma leptin level and polymorphism of leptin receptor gene. Methods Ninty-nine patients with stroke and 97 patients with other diseases (controls), admitted to our hospital from January 2007 to December 2008, were chosen in the experiment. Polymerase chain reaction-restriction fragment length polymorphism (LEPR) examination was performed to detect the leptin receptor gene (Gln223A rg gene) polymorphism in these patients. The level of serum leptin was analyzed by enzyme linked immunosorbent assay. Results The genotype frequencies of GG, GA and AA in Gln223Arg gene in patients with stroke were 66.67%, 20.20% and 13.13%, respectively, while those in controls were 78.35%, 15.46% and 6.19%, respectively; those in these 2 groups were not significantly different (P>0.05). The allele frequencies of A and G in patients with stroke were 76.77% and 25.23%, while those in controls were 86.08% and 13.92%; those in the 2 groups were significantly different (P<0.05). The level ofleptin in patients with stroke was markedly higher that that in controls (P<0.05). Binary regression analyses showed that history of hypertension, elevated fasting blood glucose and cholesterol, increased plasma leptin level were associated with stroke. Conclusion The allele frequencies of A in Gln223A rg gene, hypertension, elevated fasting blood glucose and cholesterol and increased plasma leptin level might be correlated with the increased risk of stroke.

ABSTRACT

Objective To obtain the normative data of Nenrobehavioral Cognitive Status Examination (NCSE) in healthy Chinese adults and assess the effects of age, educational level, and gender on the examination results. Methods A total of 100 normal healthy Chinese adults ranging from 18 to 85 years of age were examined with NCSE to obtain the scores of NCSE and the functional items. Rank sum test was used to analyze the variation of the scores between the different groups blocked by educational level, age, and gender. Results Good agreement was found between the normative data provided by NCSE and the measurements in healthy Chinese adults. Significant differences were noted between the young, middle-aged and old people subjects in the scores of the functional items such as comprehension, repetition, memory and judgment (P<0.05). The average total score of NCSE and scores for some functional items (orientation, comprehension, repetition, memory, calculation and similarities) differed significantly between the groups with different education levels (P<0.05), but the gender did not affect the total or item scores of NCSE (P>0.05). Conclusion The NCSE can be applied in examining healthy Chinese adults using the standards provided by the scale, but the impact of age and education level of the subjects should be considered when interpreting the results.

ABSTRACT

Objective To study the incidence of visuospatial impairment in patients with Parkinson's disease(PD)and its related factors.Methods Visuospatial impairment was assessed in 107 PD patients from 6 hospitals in Guangzhou during the period from April to June 2007 using cube copying test.The motor,neuropsyehiatric,and cognitive symptoms and the activities of daily life(ADL)of the patients were evaluated using Hoehn-Yahr scale,Unified Parkinson's Disease Rating Score-Ⅱ (UPDRS-Ⅱ),UPDRS-Ⅲ,UPDRS-Ⅳ,Hamilton Anxiety Scale(HAMA),Hamilton Rating Scale of Depression(HRSD),Fuld Object-Memory Evaluation(FONt),Rapid Verbal Retrieval Test(RVR),Wechsler Adult Intelligence Scale Block Design(WISC-BD),Wechsler Adult Intelligence Scale Digid Span(WAIS-DS),Mini-Mental State Examination(MMSE),and Neuropsychiatric Inventory(NPI).Spearman correlation analysis was performed to analyze the bivariate correlation between the visual-spatial ability and the related factors,and binary logistic regression analysis was used to identify the factors related to visual-spatial disorder.Results Visuospatial impairment was found in 59 of the 107 patients(55.14%).Logistic regression analysis identified the Hoehn-Yahr stage and duration after disease onset as the risk factors,while RVR and WISCoBD as the protective factors for visuospatial impairment.Conclusion The Hoehn-Yahr stage,duration from disease onset,speech fluency,and image recognition and construction ability are related to visuospatial impairment in the PD patients.The visuospatial ability of the PD patients can be improved by relieving their motor symptoms and trainings for speech and image recognition and construction abilities.

ABSTRACT

Aim To evaluate the effect and safety of azithromycin injection produced in China and cefazolin in the treatment of respiratory tract infections.Methods 50 patients with respiratory tract infections were divided randomly into two groups. Patients in the test group recieved azithromycin in the dosage of 250 mg?d-1,qd for 5 d iv with double dosages at the first time and patients in the control group recieved cefazolin in the dosage of 2.0 g,bid,iv for 5~10 d. Results No statistical significant differences of overall clinical effect were observed between two groups .The fully recovery,effective and side reaction rates for the test group were 46.67%,90% and 3.3%,respectively,and for control group the data were 60%,95%,and 0%, respectively.Conclusion Azithromycin injection produced in China is effective and safe in the treatment of respiratory infections.
