ABSTRACT

Background: Tuberculosis (TB) caused by Mycobacterium tuberculosis (Mtb) is still a major public health concern around the world. Prompt detection of active tuberculosis cases helps in timely therapeutic intervention and reduces community transmission. Despite limited sensitivity, conventional microscopy is still used to diagnose pulmonary tuberculosis in high-burden nations such as India. This study, therefore, was aimed at assessing the diagnostic performance of microscopy by Ziehl Neelsen (ZN) and auramine (AO) staining in the diagnosis of pulmonary tuberculosis. Materials and methods: A prospective comparative study was done on the sputum samples of 2,395 adult patients from November 2018 to May 2020 suspected of having pulmonary tuberculosis visiting the Designated Microscopic Centre of SGT Medical College, Budhera, Gurugram. Each sample was subjected to ZN staining, and AO staining as per NTEP guidelines. Results: Out of the 2,395 samples studied, 161 (6.76%) and 224 (9.35%) were positive by ZN and AO staining methods respectively. Pauci-bacillary cases detected by AO were more than ZN staining. There were 63 more sputum samples detected by AO staining which were missed by ZN microscopy. Conclusion: When compared to conventional ZN staining, the auramine staining technique is more sensitive and takes less time to diagnose pulmonary tuberculosis

ABSTRACT

Methods@#Twenty patients (age, 14–60 years) with healed spinal tuberculosis with thoracic kyphosis >50° were included. Lateral-view radiography of the whole spine, including both hips, was performed for assessment of kyphotic angle (K angle), sagittal balance, lumbar lordosis, and spinopelvic parameters. Pulmonary function was assessed by measuring the forced vital capacity (FVC), forced expiratory volume in 1 second (FEV1), and their ratio (FEV1/FVC) by spirometry. @*Results@#A positive correlation between severity of kyphosis and sagittal imbalance was noted, with compensatory mechanisms maintaining the sagittal balance in only up to 80° of dorsal kyphosis. In >80° of kyphosis, FVC was found to be markedly decreased (mean FVC=50.6%). The mean K angle was lower in subjects with lower thoracic kyphosis. In lower thoracic kyphosis, due to short lordotic and long kyphotic curves, both lumbar lordosis and pelvic retroversion worked at compensation, whereas, in middle thoracic kyphosis, due to long lordotic curve, only lumbar lordosis was required. Normal pulmonary function (mean FVC, 83.0%) and lesser kyphotic deformity (mean K angle in adolescents, 69.8°; in adults, 94.4°) were found in adolescents. @*Conclusions@#In >80° of thoracic kyphosis, there is sagittal imbalance and a markedly affected pulmonary function. Such patients should be offered corrective surgery if they are symptomatic and medically fit to undergo the procedure. However, whether the surgical procedure would result in improved pulmonary function and sagittal balance needs to be evaluated by a follow-up study.

ABSTRACT

Objective To investigate the prognostic value of the platelet-lymphocyte ratio (PLR) in non-small cell lung cancer patients treated with PD-1/PD-L1 inhibitors. Methods PubMed, EMBASE, Web of Science, Medline, Cochrane Library, China National Knowledge Infrastructure, Chinese Biomedical Literature Database, VIP, WanFang and other databases were searched online for eligible studies about evaluating the relation between PLR and the prognosis of NSCLC patients treated with PD-1/PD-L1 inhibitors from the establishment of database to April 2020. The relevant data of literatures that met the inclusion criteria were extracted. Pooled estimates of HR and 95%CI were calculated using Stata 15.0. Results We included six studies involving 551 patients. Elevated PLR was associated with worse OS and PFS of NSCLC patients treated with PD-1/PD-L1 inhibitors. The subgroup analysis of OS showed the prognostic value of high PLR in Caucasian race, cutoff value ≤169.05, PLR cutoff value determination according to previous literature and multi-center retrospective study (P < 0.05). Subgroup analysis of PFS showed the prognostic value of high PLR in East Asian race, cutoff value ≤169.05, PLR cutoff value determination according to previous literature and single-center retrospective study (P < 0.05). Conclusion Among NSCLC patients treated with PD-1/PD-L1 inhibitors, elevated blood PLR is associated with shorter OS and PFS, indicating that it may be a potential biomarker for PD-1/PD-L1 treatment on NSCLC patients.

ABSTRACT

The aim of present study was to evaluate the linkage disequilibrium (LD) of p.R72P, PIN3 Ins16bp, p.P47S, p.R213R and r.13494g[a polymorphism of TP53 and their haplotypes association with oesophageal cancer risk in patients from Punjab, northwest India. A total of 466 samples, including 233 oesophageal cancer patients and 233 healthy individuals were analysed. Data analysis revealed the gender specific association. In female group, arginine–proline (RP) genotype (P = 0.08) and P allele (P = 0.07) of p.R72P polymorphism was marginally associated with increased risk of oesophageal cancer. A1A2 genotype (P = 0.06) and A2 allele (P = 0.07) of PIN3 Ins16bp polymorphism was marginally associated with decreased risk of oesophageal cancer in male group. A1A2–GA genotype combination (P = 0.04) of PIN3 and r.13494g[a polymorphisms was significantly associated with decreased risk of oesophageal cancer in male group. In female group, PP–GA genotype combination (P = 0.02) of p.R72P and r.13494g[a polymorphisms and RP–A1A1–GG genotype combination (P = 0.04) of p.R72P, PIN3 and r.13494g[a polymorphisms was significantly associated with increased risk of oesophageal cancer. We observed moderate LD between two intronic polymorphisms PIN3 Ins16bp and r.13494g[a (D0 = 0.90; r 2 = 0.68). Haplotype analysis revealed that none of the haplotype combination was associated with oesophageal cancer risk when both the genders were considered. Stratification on the basis of gender showed that P-A2-P-A-A haplotype of p.R72P, PIN3 Ins16bp, p.P47S, p.R213R and r.13494g[a polymorphisms was marginally associated with reduced oesophageal cancer risk in male group (P = 0.08). Replication of these findings in independent cohorts may be insightful for the role of TP53 in oesophageal cancer pathogenesis.

ABSTRACT

Background: Chronic neck pain is observed to be commonly kindred with forward head posture (FHP). Rib cage mechanics is found to be altered that decreases thoracic mobility. This reduced mobility of thorax reduces the effectiveness of diaphragm, intercostals, and abdominal muscles in terms of ventilation. Therefore this study was done to evaluate the effectiveness of exercises meant for enhancing the stability of the neck with feedback on neck stabilization exercises with feedback in improving the respiratory status. Methods: This was an experimental study. Based on inclusion & exclusion criteria, 100 subjects (54 males and 46 females) enrolled for the study, which was further allotted into Experimental and Control groups. The experimental group was given Cervical Stabilization Exercise with feedback in addition to routine Physiotherapy treatment. Control group was given only regular Physiotherapy treatment for six weeks. The digital camera assessed the FHP by measuring of Craniovertebral Angle (CVA). Spirometry assessed pulmonary function (FEV1) and Micro RPM assessed inspiratory muscle strength (PImax). All measurements were taken on the day of study, on 3rd and 6th week Results: Significant reduction in forward head posture measured by improvement in Craniovertebral angle, improvement in Inspiratory muscle strength (PIMax) and pulmonary functions (FEV1) were found in the group that received cervical stabilization exercises with feedback along with the conventional Physiotherapy (p< 0.05). Therefore it is suggested that cervical stabilization exercises correct the head posture and helps to improve the biomechanics of respiratory muscles. Conclusion: Cervical stabilization exercise is an effective approach to correct the forward head posture, and it should be included in the intervention measures of patients with forward head posture.

ABSTRACT

Aim: To understand the relative diagnostic sensitivity, specificity and accuracy of each modality.Introduction: Internal de-arrangement of knee requires utmostdiagnostic accuracy especially in professional sportspersonnel’s. MRI is the most advanced modality to date forsuch injuries and there is a trend among most of the patientsand treating young doctors to order for an MRI, in almost all theinjuries, obvious and occult, with the result that there is a heavyrush of workload on MRI and it is difficult to cope with the workload, resulting in a long waiting list.Method: In view of the above situation and conflicting reports, regarding the efficiency of ultrasound and total neglect ofclinical diagnosis this comparative study of clinicalexamination, ultrasonic diagnosis and MRI was conducted inour tertiary care centre, from March 2017 to March 2018, as aprospective double blind study. Results: Among the 104 patients who participated in this study106 lesions were diagnosed clinically, where in 10 lesions werefalse positives as confirmed by MRI, in only 96 cases. Resultsof ultrasound analysis were quite encouraging as it coulddiagnose IDK in 95 cases meaning only one case as falsenegative. Lateral meniscus (LM) was diagnosed clinically in 16cases, sonographically in 17 cases and by MRI in 18 cases.Medial Meniscus (MM) was diagnosed as torn clinically in 38cases, ultrasonographically and MRI in 36 cases. ACL wasdetected as torn clinically in 32 cases while 28 and 27 by USGand MRI while as PCL was detected as torn clinically in 1 andin 2 cases by USG and MRI. Medial collateral (MC) wasdetected as torn clinically in 7 cases and by USG and MRI In10 cases while as Lateral Collateral (LC) was detected as tornclinically in 2 cases while 3 by USG and MRI.Overall, sensitivity, specificity and accuracy (SSA) of clinicalexamination was 75.7%, 79.6% and 78.1%. Mean SSA ofultrasound was 96.8%, 98.4% and 97.4%. The study provesthat ultrasound is a reasonably sensitive, specific and accuratein expensive diagnostic tool and is underutilized, as in experthands its results match that of MRI.

ABSTRACT

Introduction: Fractures of distal third of tibia pose greaterchallenge to the treating surgeon, in view of its relative lack ofvascularity, as it is devoid of muscular envelope and is coveredonly by skin and tendons. Its proximity to the ankle joint makesit further complex than any diaphyseal fracture. A variety oftreatment modalities are available including non operative, external fixation, IM nailing and plating. Whereas, closedreduction and casting is known for redisplacement withresultant malunion and joint stiffness because of prolongedimmobilisation, external fixators are of utility only for compoundfractures of mid third and are known for pin track infections.Material and Methods: A Comparative prospective study oftwo procedures, for displaced fractures of distal third tibia inadults was conducted in Govt. Medical College (GMC),Jammu. 30 patients (Group A) were treated by ClosedReduction and fixation with IMIL Nailing and another 30 (GroupB) were treated by closed reduction and fixation using DistalTibial Locking Plate with MIPPO Technique. The patients werefollowed for a minimum of one year wherein the clinicalparameters, radiological progress and final functional resultswere evaluated as per American Orthopaedic Foot and AnkleSociety Score (AOFAS).Results: There was no significant difference in fracture healingtime or ambulation time. Good to Excellent results of 60% werefound in Group A and 73.3% in Group B. There were 13.3% ofpoor results in Group A as compared to 6.7% of Group B.Overall complication rate was higher in Group A. IMIL Nailinghad to be abandoned in 2 cases (6.7%) and switched on toDTLCP, due to per operative finding of medullary canal beingnarrower than 8 mm in one and curved in another. IMIL GroupA, also had 2 cases (6.7%) bent nail, 2 cases (6.7%) brokennail and another 3 cases (10%) of bent/broken locking bolts. No such implant related complications were observed inGroup B.Conclusion: IMIL Nailing is a technically demandingprocedure, requiring a thorough pre- operative work up and askilled experienced surgeon with all sizes of nails, reamers andalternative options available before contemplating it. However,DTLCP by MIPPO technique is easier to learn, is equallybiological, based on sound AO principles of indirect reductionand stabilization by extra periosteal fixed angle construct. Theprocedure has fewer per operative and postoperativecomplications and gives comparable or better results than IMILNailing. The choice of the procedure should be guided by thesite and geometry of fracture and individual experience of thetreating surgeon.

ABSTRACT

Necroptosis is a tightly regulated form of necrosis that requires the activation of receptor-interacting protein (RIP) kinases RIPK1 and RIPK3, as well as the RIPK3 substrate mixed lineage kinase domain-like protein (MLKL). Because of membrane rupture, necroptotic cells release damage-associated molecular patterns (DAMPs) that evoke immune responses. Necroptosis is emerging as an important cellular response in the modulation of cancer initiation, progression, and metastasis. Necroptosis of cancer cells is considered to be an immunogenic cell death capable of activating anti-tumor immunity. Necroptosis also participates in the promotion of myeloid cell-induced adaptive immune suppression and thus contributes to oncogenesis. In addition, necroptosis of endothelial cells and tumor cells is conducive to tumor metastasis. In this review, we summarize the current knowledge of the complex role of necroptosis in cancer and discuss the potential of targeting necroptosis components for cancer therapies.

ABSTRACT

Background and aim Ventricular tachycardia (VT) represents the most frequent cause of sudden cardiac death. Information on clinical characteristics, acute management and outcome of patients with sustained VT is limited in our part of world. The aim of this study was to analyze the demography, hemodynamics, ECG features, underlying disease, mode of termination and outcome of patients presenting with VT. Methods This single center cohort study represents total of 107 patients of VT enrolled over 45 months. Results Mean age was 45 years and 59 of the patients were males. Thirty three of these patients were hemodynamically unstable (31%) and 74 were stable (69%) Coronary artery disease was the most common etiological factor accounting for 39% of patients followed by non-ischemic cardiomyopathy. Determinants of hemodynamic instability were VT in course of acute myocardial infarction (8 out of fourteen) and polymorphic pattern of VT (13 out of 26). Spontaneous termination of VT occurred in seven patients, antiarrythmic drugs terminated VT in 53 of 67 patients and in remaining 45 patients VT was terminated with direct current (DC) cardioversion. Total of twenty three patients died during the hospital stay. Factors that contributed to mortality were old age, hemodynamic instability and low ejection fraction. Conclusion Ischemic heart disease remains the leading cause of VT. Hemodynamically unstable VT occurs more frequently in acute myocardial infarction and polymorphic VT. Most effective method of VT termination is DC cardioversion. Old age, hemodynamic instability and ejection fraction contribute to overall mortality in VT.

ABSTRACT

Cupressus torulosa D. Don, known as the Himalayan or Bhutan cypress, is one of the medicinal plants commonly used in the Indian System of Medicine for various ailments. The present study evaluates the chemical composition and inhibitory potential of the essential oil and three different extracts (chloroform, methanol and aqueous) of aerial parts of C. torulosa. Chemical composition of essential oil was determined by GC-MS that showed the presence of four major components viz., α-pinene (45.44%), 3-carene (38.34%), terpinolene (5.36%) and aromadendrene (6.32%). Essential oil showed significant inhibitory activity against Bacillus subtilis, Pseudomonas alcaligenes, Micrococcus luteus and Bacillus cereus in comparison to the different extracts. Essential oil also showed good antifungal activity against the three fungal pathogens viz., Alternaria alternata, Curvularia lunata and Bipolaris specifera. TLC-bioautography was used to screen the antibacterial components of the essential oil. Analysis showed the presence of four zones of inhibition on bioautography plate at Rf values 0.80, 0.70, 0.61 and 0.46. This study has demonstrated the presence of four potential antibacterial compounds in the essential oil of Cupressus torulosa.
