ABSTRACT

OBJECTIVE@#To explore the effect of intermittent pneumatic compression(IPC) combined with 3M thermometer on the prevention of deep venous thrombosis(DVT) in patients with femoral intertrochanteric fracture.@*METHODS@#From March 2016 to August 2019, 127 patients with femoral intertrochanteric fractures who underwent proximal femoral nail antirotation(PFNA) were retrospectively analyzed. They were divided into two groups according to different methods of thrombus prevention and treatment. Among them, 63 patients in group A did not use IPC and 3M thermometer;64 cases in group B were treated with IPC combined with 3M thermometer. Color Doppler ultrasound was used to dynamically monitor the DVT and changes of lower limbs during perioperative period. The venous thrombosis of lower limbs was monitored at 0, 24, 72 h and > 72 h after operation(recheck every 3 days until discharge).@*RESULTS@#Occurrence of DVT of lower limbs after PFNA operation in two groups:there were 5 cases (7.8%) in group B and 20 cases (31.7%) in group A, there was significant difference between two groups (P=0.001). There was no significant difference in lower limb DVT between two groups at 0, 72 and > 72 h after operation(P>0.05), but the formation rate of group A was significantly higher than that of group B at 24 h after operation (P=0.049). There was no significant difference in DVT formation between group A and group B(P>0.05). However, the formation of DVT in group A was significantly higher than that in group B(P=0.012).@*CONCLUSION@#Intraoperative IPC combined with 3M thermostat can effectively prevent DVT of lower limbs in patients undergoing PFNA surgery.

ABSTRACT

@#AIM: To observe the improvement of ocular hemodynamics before and after intraocular pressure(IOP)intervention in patients with non-arteritic anterior ischemic optic neuropathy(NAION).<p>METHODS: Retrospective case series. Totally 92 patients(92 eyes)with NAION were admitted to the Department of Ophthalmology, Xi'an Fourth Hospital from July 2012 to September 2018. Forty-six patients received only basic treatment without IOP lowering treatment as the conventional treatment group. The other 46 patients were treated with brinzolamide eye drops combined with brimonidine eye drops to lower IOP on the basis of conventional treatment, as the IOP intervention group. Before and after treatment, the blood flow rate of the ophthalmic artery and central retinal artery were measured by color Doppler ultrasound. The IOP, best corrected visual acuity(BCVA), mean visual field defect(MD), retinal nerve fiber layer thickness(RNFLT), ophthalmic artery and central retinal artery peak systolic blood velocity(PSV), end-diastolic blood velocity(EDV), pulse index(PI)and resistance index(RI)were compared.<p>RESULTS: Before treatment, there were no significant differences in IOP, BCVA(LogMAR), MD, RNFLT, PSV, EDV, PI and RI between the two groups(<i>P</i>>0.05). After 14d of treatment, IOP, BCVA(LogMAR), MD and RNFLT of the two groups were significantly improved compared with those before treatment(<i>P</i><0.05), and the improvement effect of IOP intervention group was better, and the difference between the two groups was statistically significant(<i>P</i><0.05). The PSV, EDV, PI of ophthalmic artery and central retinal artery in the two groups were increased compared with those before treatment, and RI was decreased compared with before treatment; and the changes of PSV, EDV, RI, PI of ophthalmic artery and PSV, EDV, RI of central retinal artery in the IOP intervention group were more significant than those in the conventional treatment group(<i>P</i><0.05). <p>CONCLUSION: IOP intervention can significantly improve the ocular hemodynamic indexes and improve the visual acuity of patients with NAION.

ABSTRACT

Objective:To analyze the epidemiological characteristics of an aggregational gastroenteritis and determine the genotypes of sapovirus, and to provide scientific basis for formulating effective control strategies. Methods:Unified case definition, active case search and descriptive epidemiology were used to analyze the epidemic. Feces or anal swabs of untreated students, teachers, canteen staff as well as canteen environment samples were collected. Norovirus and sapovirus nucleic acid tests were conducted by real-time fluorescent RT-PCR, and sapovirus nucleic acid was amplified by conventional RT-PCR. The gene region of capsid protein was analyzed by MEGA7.0 software and phylogenetic tree was constructed. Results:A total of 12 cases were reported in the epidemic, and the incidence rate was 44.44%. All reported cases, with vomiting symptoms, were found in the same class. The epidemic showed a point-based outbreak. The first case became the source of infection in class, and the epidemic lasted for 8 days. Real-time fluorescent RT-PCR assay confirmed that five children's feces were positive for sapovirus nucleic acid, and the first-episode children's feces were positive for sapovirus and GII norovirus nucleic acid. Sequence alignment result showed that the sapovirus strains belonged to GI.1 type with homologous genes. Conclusion:Based on the clinical manifestations, field epidemiological investigation and laboratory test results, we confirm that the first case of the epidemic in class is caused by GI.1 sapovirus infection. The epidemic is effectively controlled by comprehensive measures such as case isolation and disinfection.

ABSTRACT

INTRODUCTION@#It is vital to diagnose and treat pulmonary tuberculosis (PTB) early, in the interests of the patient and public health. At the Singapore Tuberculosis Control Unit (TBCU), it was not uncommon to discover abnormal chest radiographs (CXRs) predating their PTB diagnosis by months to years in the electronic medical records (EMRs) of sputum acid-fast bacilli smear-positive patients. Our study explored this observation.@*METHODS@#The EMRs of sputum smear-positive PTB patients treated at the TBCU from January to July 2014 were viewed for abnormal CXRs preceding their PTB diagnosis. Information in the EMRs pertaining to the indication for the CXRs, radiological reports (including whether the possibility of PTB was stated) and action taken was captured.@*RESULTS@#Of the 254 sputum smear-positive patients, 108 had previous CXRs in their EMRs, of whom 39 (36.1%) had previous CXRs compatible with PTB. Most of these were performed in tertiary institutions and for reasons unrelated to PTB. No action was taken in response to these CXRs in 24 (61.5%) patients. 27 (69.2%) patients had abnormal CXRs dating back more than six months, with 12 (30.8%) dating back more than 30 months before the PTB diagnosis. Patients aged ≥ 55 years were significantly more likely to have previous CXRs that were compatible with PTB.@*CONCLUSION@#PTB patients may be asymptomatic or have chronic indolent disease and remain undiagnosed for years. There is a need for vigilance within our healthcare system to seize opportunities for early diagnosis and treatment to prevent the spread of this infectious disease in Singapore.

ABSTRACT

Objective To understand the status of sexually transmitted infection and seeking medical treatment behavior, and influencing factors of Vietnamese cross-border female sex workers (FSWs) in Hekou County, Yunnan, and to provide a basis for promoting reproductive health and preventing and controlling the spread of sexually transmitted diseases (STD) in Vietnamese cross-border FSWs. Methods The snowball sampling method was used to recruit research subjects in entertainment venues in Hekou County, Yunnan, to conduct a questionnaire survey and collect blood, vaginal secretions and cervical swab specimens for HIV/STD testing. Results A total of 262 Vietnamese cross-border FSWs were investigated. The total infection rate of sexually transmitted diseases and HIV was 35.8% (94/262). The positive rate of genital herpes simplex, fungal vaginitis, trichomoniasis, wet warts and chlamydia were 23.5%, 8.0%, 2.7%, 1.5% and 1.5%, respectively. The positive rate of HIV was 1.5%. In the past year, 116 Vietnamese cross-border FSWs had symptoms related to sexually transmitted diseases. Among them, 34.5% chose to go to the hospital or private outpatient clinic, 44.8% bought medicines by themselves, and 20.7% did not receive any treatment. The FSWs who reported having sexually transmitted diseases testing in the past year (OR=3.54, P<0.05), came from medium and high-end places (OR=3.94, P<0.05), had more than two symptoms (OR=3.88, P<0.05), and self-perceived high risk of sexually transmitted infection were more likely to seek medical treatment. Conclusion The Vietnamese cross-border FSW population in Hekou County of Yunnan Province had a high rate of sexually transmitted infections. The proportion of seeking medical treatment among FSWs having symptoms related to sexually transmitted diseases was low. It is necessary to guide the Vietnamese cross-border FSWs to seek formal medical treatment.

ABSTRACT

Objective To explore the changes of cardiovascular system of military personnel during highly intensive training.Methods One hundred and seventy officers and soldiers were as research subjects,including 100 individuals in high-intensity group and 70 in control group.The levels of serum cortisol (COR),high sensitivity C-reactive protein (hs-CRP),cardiacspecific Troponin T (cTnT) and human heart fatty acid binding protein (H-FABP) were measured when military training ended.All subjects were tested with fatigue assessment instrument (FAI).The data of blood pressure and electrocardiogram were collected from 40 individuals randomly selected from high-intensity group and 36 from control group before and after training for analyzing the blood pressure,arrhythmia and heart rate variability (HRV).Results The levels of COR (indicator related to stress) and hs-CRP (indicator related to inflammation) were significantly higher in high intensity group than in control group (P<0.05).Highly intensive training can lead to the emergence of myocardial micro-injury,the levels of cTnT and H-FABP were obviously higher than those in control group (P<0.01),and the mean blood pressure and the severity of fatigue status were significantly higher than those in control group (P<0.05).The incidence of severe ventricular arrhythmia was lower in both groups (P=1.000).The average heart rate,total heart beats,total number of atrial premature beat,total number of ventricular premature beat,the incidence of sinus arrhythmia and intermittent second degree type Ⅰ atrioventricular block were significantly higher in high intensity group than in control group (P<0.05).The HRV of high intensity group was obviously decreased (P<0.01).Conclusion Highly intensive training may induce the military personnel into the state of acute stress and inflammation,which may lead to myocardial injury,increase severity of fatigue status,accompanied with the rise of blood pressure,low HRV and increased incidence of various arrhythmias.

ABSTRACT

Objective To investigate the anti-inflammatory effect of chlorogenic acid( CGA) on BV2 microglia in-fected by herpes simplex virus ( HSV)-1.Methods BV2 cell was stimulated by HSV-1 to create a cell model of vi-rus encephalitis, then CGA was processed with different concentrations. MTT method was used to measure cell via-bility. Real-time quantitative PCR and ELISA were used to detect the content and expression of tumor necrosis fac-tor alpha ( TNF-alpha) and interleukin ( IL)-6. Results After CGA treatment, the cell survival rate significantly improved, and after HSV-1 infected BV2 cells, CGA inhibited the significant increase in mRNA levels of TNF-al-pha and IL-6. In addition, CGA can reduce the release of TNF-alpha and IL-6 induced by HSV in the cells.The mRNAs of TNF-alpha and IL-6 were also significantly inhibited by CGA.Conclusions CGA inhibits inflammatory response in HSE.CGA can be used as an anti-inflammatory agent to treat HSE new strategies.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate whether RhoA/Rho-kinase contributes to the occurrence of chronic post-thoracotomy pain (CPSP) by up regulation of glutaminase 1 (GLS1) expression in the spinal dorsal cord.</p><p><b>METHODS</b>Twenty five male Sprague Dawley (SD) rats were divided into control group (n=5) and model group (n=20). The rats in the model group were randomized into two sub groups (n=10) for observation on day 10 and day 21 after thoracotomy, and each group was further divided into CPSP and non CPSP groups according to the behavioral test results. All the rats were sacrificed after behavioral test for examination of GLS1 and RhoA expressions in the spinal cord using Western blotting and RT PCR. We also compared the effect of the Rho kinase inhibitor fasudil and saline, both injected intraperitoneally daily at 10 mg/kg for 7 consecutive days following thoracotomy, on CPSP and GLS1 expression in 30 male SD rats on day 21 after thoracotomy.</p><p><b>RESULTS</b>Compared with the control group, the rats with CPSP showed significantly increased expressions of GLS1 and RhoA mRNA in the spinal cord on both day 10 and day 21 following thoracotomy (P<0.01), but the rats without CPSP did not show obvious changes in GLS1 and RhoA expressions. In fasudil treated rats, the mechanical pain threshold was obviously increased and the expressions of GLS1 and RhoA were significantly reduced as compared with those in saline treated rats (P<0.01).</p><p><b>CONCLUSION</b>RhoA plays an important role in the occurence of CPSP by up-regulating the expression of GLS1 in the spinal dorsal cord of rats.</p>

Objective:To investigate the inhibitory effect of Shenqi Fuzheng injection on the expression of VEGF gene in human hepatocellular carcinoma cell line MHCC97L,and to elucidate the effect of Shenqi Fuzheng injection on tumor invasiveness,and to validate it through siRNA technology.Methods:After 24 hours of addition of Shenqi Fuzheng Injection (MHC) in the culture system of MHCC97L,Real-time PCR and Western-blot were used to detect the expression of MMP-9 and MMP-9 in the control group (Shenqi Fuzheng),negative control group (NaCI) And the expression of VEGF and RNA in the blank control group,and to determine the effect of Shenqi Fuzheng injection on the expression of VEGF and RNA.The effect of Shenqi Fuzheng Injection on the invasiveness of MHCC97L cells was detected by Transwell assay.The lentiviral vectors of MHCC97L cells were transfected into 293T cells and transfected into MHCC97L cells.The transfected cells were transfected into 293T cells by Lipofectamine 2000,and transfected into MHCC97L cells.The Effect of Shenqi Fuzheng Injection on the Invasion of VEGFi Tumor by Transwell.Results:Shenqi Fuzheng injection could significantly decrease the expression of VEGF mRNA and protein in normal MHCC97L cells (P<0.05),and decrease the invasiveness of tumor cells (P<0.05).VEGF RNA interference was successful.After intervention,Shenqi Fuzheng injection could not decrease the invasiveness of MHCC97L tumor (P>0.05).Conclusion:Shenqi Fuzheng injection can reduce the invasion of MHCC97L cells by inhibiting the expression of VEGF gene.

Objective This present study was to examine the prevalence and determinants of one-night-stand behavior among young men who have sex with men (YMSM).Methods A total of 403 YMSM aged 16 to 25 were recruited through internet promotion,extending activity and HIV VCT in Wuhan.Data was gathered through anonymous questionnaire.Binary logistic regression was used to examine factors associated with one-night-stand behavior.Results Of the 398 YMSM,48.99% (195/ 398) reported having had casual sex in the last 6 months.Of the ones having had casual sex,34.29% (60/175) and 28.65% (49/171) reported using condoms consistently during anal or oral sexual contacts,respectively.These figures were lower than those of YMSM not having casual sexual contacts [with anal sex as 49.08% (80/163) and oral sex as 38.85% (61/157)].76.80% (149/194) of the YMSM reported having had multiple sexual partners,with the figure higher than those without [33.15% (60/ 181)] (P<0.01).Results from the logistic regression analysis showed that the following factors seemed to be associated with casual sex activities among YMSM,including:often using intemet,(OR=4.89,95% CI:1.90-12.54),taking illegal drugs (OR=2.72,95% CI:1.60-4.63).Conclusions YMSM who bad engaged in casual sex,practicing unprotected sex or having multiple sexual partners,were recognized as high risk population.Targeted intervention programs are needed to decrease the one-night-stand behavior.Internct intervention strategy seemed an important method to serve the purpose.

Objective To understand the spatial and temporal distribution of HIV/AIDS in Hubei province,and provide scientific evidence for the prevention and control of AIDS.Methods GeoDa software was used for autocorrelation analysis,SatScan 9.2 software was used for statistical analysis of spatial scanning,and finally geographic information system was used for visualization.Results A total of 6 952 HIV/AIDS cases were reported during 2010-2013 in Hubei,and the spatial autocorrelation analysis showed that Global Moran's I index was 0.266 (P<0.05),indicating that there was a positive spatial autocorrelation of HIV/AIDS.Global Moran's I index increased year by year (P< 0.05),indicating that the increased spatial aggregation of HIV/AIDS during 2010-2013.The local Moran's I index showed that "high-high" clustering areas were in Wuhan,and the number of "high-high" clustering areas increased during 2010-2013.Moreover,the "high-high" clustering areas expanded from Wuhan to surrounding areas.Spatial and temporal scan analysis revealed that 19 counties in Wuhan,Huangshi,Ezhou,Xianning with a radius of 60.01 km (LLR=625.14,RR=3.23) were the main spatial and temporal clustering area during 2012-2013.Conclusion The spatial changes of HIV/AIDS seemed to be regular from 2010 to 2013 in Wuhan,spatial correlation at provincial level decreased and the "high-high" clustering areas gradually expanded from Wuhan to surrounding areas,indicating that it is necessary to strengthen the AIDS prevention and control in these areas in Hubei.

Objective To investigate the post-traumatic stress disorder (PTSD) and its influence factors among people living with HIV /AIDS (PLWHA) ,and provide evidence for developing relevant interventions and treatment methods .Methods Anony-mously completed questionnaires ,including self-designed questionnaire and the post-traumatic checklist civilian version (PCL-C) , were received from PLWHA in Suizhou from December 2014 to March 2015 .Descriptive analysis and multivariate logistic regres-sion analysis were used to examine factors associated with PTSD .Results Among 243 qualified PLWHA ,106 of them were positive with PTSD (43 .62% ) ,and the prevalence of PTSD among male and female were 37 .88% and 50 .45% ,respectively .Among the three characteristic symptoms ,repeated flashback was the highest (78 .60% ) ,then hyper-vigilance (64 .20% ) ,and the positive rate of avoidance/numbness was the lowest (57 .20% ) .Multivariate logistic regression analysis showed that female was more suscepti-ble to PTSD than male (OR = 1 .960 ,95% CI :1 .025 - 3 .747) ,discrimination or unfair treatment would increase the risk of PTSD (OR= 2 .967 ,95% CI :1 .498 - 5 .876) ,and the frequency of fear/tension was positive correlated with PTSD (occasionally OR =2 .074 ,95% CI :1 .008 - 4 .265 ;always OR = 6 .690 ,95% CI :2 .708 - 16 .531) .Conclusion PLWHA had a high level of PTSD ,and fe-male PLWHA and those who felt fear/tension or experienced discrimination/unfair treatment were more likely to suffer from PTSD .

ABSTRACT

Objective To understand the HIV infection status in middle aged and old men and the association between HIV infection and female sex workers in a city in Hubei province and provide evidence for the targeted prevention and control of HIV infection.Methods Ten communities/villages were selected through stratified cluster sampling from the suburban and rural areas of the city.A total of 634 men aged ≥50 years were surveyed by using self-designed questionnaire in December,2014.Results The awareness rate of HIV/AIDS related knowledge was 13.3% (68/513).Among the men surveyed,63.2% (371/587) thought they couldn't be infected with HIV,9.1% (52/573) believed that extramarital sex is acceptable,and 32.4% (189/583) considered that commercial sexual behavior is just as well.Among the surveyed men reporting sexual behavior in the past 6 months,6.6% (15/228) had sex with regular sexual partners,in which 63.6% (7/11) never used condoms,21.8%(128/586) reported that some friends had commercial sexual behaviors.Among 601 men receiving HIV testing,7 were HIV positive (1.2%).The HIV detection rates among the men living at 3 communities near red light district were 4.2% (5/119),1.1% (1/95) and 0.5% (1/192) respectively;while no HIV infections were found in the men living at 2 towns far away from red light district.Seventy seven of 581 men surveyed (13.2%) reported to meet visiting female sex workers and they had higher HIV positive rate (5.2%,4/77) than those without such experiences (0.6%,3/504) (P=0.007).Conclusion The middle aged and old men in this city had low awareness of HIV/AIDs related knowledge,and high-risk sexual behaviors existed among them.HIV positive rate was relatively high in this population,and HIV infection might be associated with sex service.

ABSTRACT

Objective To explore the association between sexual sensation seeking and risk sexual behaviors among young men who have sex with men (YMSM) who were under 25 years old.Methods 403 YMSM were recruited through online publicity,organization of outdoor activities together with HIV voluntary counseling and testing services from May to November in 2013.A total number of 375 (93.05%) valid questionnaires were received.Results Data showed that,among the 375 YMSM,37.9% had sex with men for the first time without using a condom,55.7% had multiple sexual partners,51.7% had one-night stand,and 5.6% engaged in group sex activities.Results from correlation analysis showed that sexual sensation seeking was positively correlated with four risk sexual behaviors (r=0.247,0.218,0.296 and 0.252,respectively).Data from the Unconditional logistic regression analysis showed that YMSM with high sexual sensation seeking were more likely to engage in risk sexual behaviors.Conclusion Sexual sensation seeking was associated with risk sexual behaviors among YMSM.More knowledge and behavior education based on the sexual sensation seeking should be emphasized,as well as increasing the awareness on HIV-related risk sexual behaviors.

ABSTRACT

<p><b>OBJECTIVE</b>To assess the effects of dexmedetomidine (Dex) on propofol dosage in target-controlled infusion (TCI) and hemodynamics in patients undergoing laparoscopic surgery under general anesthesia.</p><p><b>METHODS</b>Sixty patients undergoing laparoscopic surgery under general anesthesia were randomly divided into control group (n=30) and the Dex group (n=30). The patients in Dex group received a loading dose of Dex (1 µg/kg, infused within 10 min) before the surgery followed by continuous infusion at the rate of 0.3 µg·kg(-1)·h(-1) till the end of the surgery, and the control patients received saline infusion in the same manner. Heart rate, blood pressure, bispectral index (BIS), and propofol dose in TCI were recorded during induction and maintenance of anesthesia. The incidence of hypotension and bradycardia were observed during and after the surgery.</p><p><b>RESULTS</b>No difference was found in the incidence of hypotension and bradycardia between the control group and Dex group (P>0.05), but heart rate and blood pressure were lower in Dex group during extubation (P<0.05). The dose of propofol in TCI was significantly less in Dex group than in the control group (P<0.05).</p><p><b>CONCLUSION</b>Dex can reduce hemodynamic abnormalities caused by extubation and decrease the dosage of propofol in TCI, and may serve as an ideal adjuvant drug for general anesthesia.</p>
