ABSTRACT

Squalene epoxidase (SQLE) is a potential target for the treatment of liver cancer. Bioinformatics analysis indicated that the high expression of SQLE was closely related to the clinical stage and poor prognosis of patients with liver cancer. However, the existing inhibitors against SQLE 195 tyrosine residue (Y195) cannot be used clinically due to severe side effects. In this study, 35 small-molecule compounds targeting SQLE 335 tyrosine residue (Y335) were selected by computer virtual screening. Combined with MTT assay, 3 candidate compounds (19#, 31# and 35#) with significant inhibitory effects on the proliferation of Huh7 cell line were obtained. Further studies showed that these 3 compounds could inhibit the migration of Huh7 cells, reduce the contents of total and free cholesterol, up-regulate the expression of tumor suppressor gene PTEN, and down-regulate the expression of PI3K and AKT proteins. The results showed that the novel inhibitors 19#, 31# and 35# targeting SQLE Y335 could reduce cholesterol content, inhibit the proliferation and migration of Huh7, thus playing an anti-liver cancer role.

ABSTRACT

Objective: To compare the safety and efficacy of transmesenteric vein extrahepatic portosystemic shunt (TEPS) and transjugular intrahepatic portosystemic shunt (TIPS) in the treatment of cavernous transformation of the portal vein (CTPV). Methods: The clinical data of CTPV patients with patency or partial patency of the superior mesenteric vein treated with TIPS or TEPS treatment in the Department of Vascular Surgery of Henan Provincial People's Hospital from January 2019 to December 2021 were selected. The differences in baseline data, surgical success rate, complication rate, incidence rate of hepatic encephalopathy, and other related indicators between TIPS and TEPS group were statistically analyzed by independent sample t-test, Mann-Whitney U test, and Chi-square test. Kaplan-Meier survival curve was used to calculate the cumulative patency rate of the shunt and the recurrence rate of postoperative portal hypertension symptoms in both groups. Results: The surgical success rate (100% vs. 65.52%), surgical complication rate (6.67% vs. 36.84%), cumulative shunt patency rate (100% vs. 70.70%), and cumulative symptom recurrence rate (0% vs. 25.71%) of the TEPS group and TIPS group were statistically significantly different (P < 0.05). The time of establishing the shunt [28 (2141) min vs. 82 (51206) min], the number of stents used [1 (12) vs. 2 (15)], and the length of the shunt [10 (912) cm vs. 16 (1220) cm] were statistically significant between the two groups (t = -3.764, -4.059, -1.765, P < 0.05). The incidence of postoperative hepatic encephalopathy in the TEPS group and TIPS group was 6.67% and 15.79% respectively, with no statistically significant difference (Fisher's exact probability method, P = 0.613). The pressure of superior mesenteric vein decreased from (29.33 ± 1.99) mmHg to (14.60 ± 2.80) mmHg in the TEPS group and from (29.68 ± 2.31) mmHg to (15.79 ± 3.01) mmHg in TIPS group after surgery, and the difference was statistically significant (t = 16.625, 15.959, P < 0.01). Conclusion: The best indication of TEPS is in CTPV patients with patency or partial patency of the superior mesenteric vein. TEPS improves the accuracy and success rate of surgery and reduces the incidence of complications.

ABSTRACT

Objective:To study the effect of Wenjing Huayu Zhitong therapy (Xiangyan Zhitong prescription, XZP) on the mitogen activated protein kinase (MAPK)/extracelluar regulated protein kinase (ERK) signaling pathway of primary dysmenorrhea (PD) rats, and explore the pathogenesis of PD and the mechanism of action of Wenjing Huayu Zhitong therapy. Method:Forty-eight female SPF-grade Wistar rats were randomly divided into blank group, model group, western medicine group, low-dose XZP group, medium-dose XZP group, and high-dose XZP group, with 8 rats in each group. In addition to the blank group, dysmenorrhea rat model with cold coagulation and blood stasis syndrome was established by cold stimulation combined with estradiol benzoate and oxytocin. The rats in the blank group, model group, western medicine group, low-dose XZP group, medium-dose XZP group, and high-dose XZP group were given distilled water, distilled water,0.06 g·kg-1 ibuprofen, 6.55 g·kg-1 XZP, 13.09 g·kg-1 XZP, and 26.18 g·kg-1 XZP, respectively, by gavage for 6 days. The writhing latency and writhing frequency of rats were recorded within 30 min after oxytocin injection.Western blot was used to detect the protein expression of B-Raf, mitogen activates extracellular regulated kinases1/2 (MEK1/2), extracellular regulated kinases1/2 (ERK1/2), p-MEK1/2, p-ERK1/2, c-Jun, and cyclooxygenase-2 (COX-2) in rat uterus. Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR) was employed to detect the mRNA expression of B-Raf, MEK1, MEK2, ERK1, ERK2, c-Jun, and cyclooxygenase-2 (COX-2) in rat uterus. Result:Compared with the model group, the treatment groups showed insignificantly prolonged writhing latency and significantly reduced writhing frequency (P<0.01). On the 6th day of modeling, there was no significant difference in the quantitative scores of symptoms and signs among the treatment groups. On the 12th day of modeling, the scores changed little in the western medicine group and the low-dose XZP group and decreased significantly in the medium- and high-dose XZP groups (P<0.01) compared with those in the model group. Compared with those in the blank group, the protein and mRNA levels of p-MEK1/2, p-ERK, B-Raf, c-Jun, and COX-2 in the model group were significantly up-regulated (P<0.01). Compared with those in the model group, the protein and mRNA levels of p-MEK1/2, p-ERK1/2, B-Raf, c-Jun, and COX-2 in the western medicine group, medium-dose XZP group, and high-dose XZP group were significantly down-regulated (P<0.05,P<0.01). Conclusion:The mechanism of Wenjing Huayu Zhitong therapy in treating PD with cold coagulation and blood stasis syndrome may be related to the down-regulation of MAPK/ERK signaling pathway.

ABSTRACT

Objective:To observe the clinical efficacy of the modified Buzhong Yiqitang combined with Erxian decoction in treating stress urinary incontinence (SUI) of perimenopausal women due to spleen and kidney Qi deficiency. Method:One hundred and six patients were randomly divided into a control group (52 cases) and an observation group(54 cases). Patients in both groups received lifestyle intervention and pelvic floor muscle training (PFMT). On this basis, patients in the observation group were further treated with the modified Buzhong Yiqitang combined with Erxian decoction, 1 bag/day, while those in the control group were provided with Suoquan pills, 6 g/time, 2 times/day, for eight weeks. Following the international consultation on incontinence questionnaire-short form (ICIQ-SF) scoring before and after treatment, the urodynamic parameters such as maximum urinary flow rate (Qmax), maximum urethral closure pressure (MUCP), residual urine volume (RUV), abdominal pressure leakage point pressure (ALPP), and bladder capacity (BC) were measured. The number of incontinence episodes per 24 h, the degree of urinary incontinence, the amount of 1 h urine leakage, and the spleen and kidney Qi deficiency syndrome score were recorded before and after treatment. The levels of estradiol (E2), follicle stimulating hormone (FSH), pituitary adenylate cyclase activating peptide (PACAP), and vasoactive intestinal peptide (VIP) were measured before and after treatment. Result:The ICIQ-SF sub-scores of the urinary incontinence frequency, severity, and impact on quality of life as well as the total score in the observation group were all lower than those in the control group (P<0.01). Qmax, MUCP, ALPP and BC in the observation group were elevated in contrast to those in control group (P<0.01), while the RUV declined (P<0.01). Compared with the control group, the observation group exhibited a decreased number of incontinence episodes per 24 h, milder degree of urinary incontinence, reduced amount of 1 h urine leakage, and lower spleen and kidney Qi deficiency syndrome score (P<0.01). The E2, PACAP, and VIP in the observation group were up-regulated as compared with those in the control group (P<0.01), whereas the FSH was down-regulated (P<0.01). The cure and effective rates of the observation group were (29/50) 58.00% and (47/50)94.00%, respectively, significantly better than (18/48)37.50% and (38/48)79.17% of the control group (χ2=4.124, χ2=4.683, P<0.05). Conclusion:On the basis of the lifestyle intervention and PFMT, the modified Buzhong Yiqitang combined with Erxian decoction obviously alleviates urinary incontinence, adjusts sex hormones, PACAP and VIP, ameliorates urodynamic parameters, and enhances the quality of life of patients with SUI due to spleen and kidney Qi deficiency. The resulting cure and effective rates are superior to those of the positive control.

ABSTRACT

Objective::To investigate the effect of Wenshen Zhuyang Tiaozhou method on sex hormones, insulin and ovulation in polycystic ovary syndrome (PCOS) patients with kidney-Yang deficiency syndrome based on phosphatidylinositol 3-kinase (PI3K)/protein kinase B (Akt) signaling pathway. Method::Totally 60 PCOS patients with kidney-Yang deficiency syndrome treated at the First Affiliated Hospital of Guizhou University of Traditional Chinese Medicine from February 2017 to July 2018 were selected and divided into observation group and control group according to the random number table method, with 30 cases in each group. The control group was orally treated with ethinylestradiol and cycloproterone acetate tablets, in addition to this therapy, the observation group also received Wenshen Zhuyang Tiaozhou method. Both groups were followed up after three months of treatment. Sex hormone levels, fasting plasma glucose (FPG) and fasting insulin (FINS) levels were measured in all patients. Homeostasis model assessment-insulin resistance(HOMA-IR) was calculated by homeostasis model assessment method, and the expression levels of Akt and p-Akt proteins in endometrium were detected by Western blot. Result::After treatment, the levels of FSH and LH in both groups decreased, and the level of E2 sex hormone increased (P<0.05). After treatment, the levels of FSH and LH in observation group were lower than those in control group, whereas the level of E2 sex hormone was higher than that in control group (P<0.05). After treatment, the levels of FPG and FINS in two groups were significantly lower (P<0.05), while the levels of FPG and FINS in observation group were significantly lower than those in control group (P<0.05). After treatment, the ovarian volume and the number of sinusoidal oocytes in two groups decreased significantly, while the endometrial thickness increased (P<0.05). After treatment, the ovarian volume and the number of sinusoidal oocytes in observation group were significantly lower than those in control group, and the endometrial thickness was significantly lower than that in observation group (P<0.05). After treatment, the ovarian volume and the number of sinusoidal oocytes in observation group were significantly lower than those in control group, and the endometrial thickness was larger than that of control group (P<0.05). After treatment, the Akt and p-Akt proteins in the endometrium of observation group and control group were significantly decreased, while the Akt and p-Akt proteins in the endometrium of observation group were significantly lower than those of control group (P<0.05). Conclusion::Wenshen Zhuyang Tiaozhou method can effectively ameliorate the sex hormone level, regulate insulin and promote the recovery of ovarian function in PCOS patients with kidney-Yang deficiency syndrome. This may be related to the blocking of the phosphorylation of Akt and the inhibition of the activation of PI3K/Akt signaling pathway.

ABSTRACT

Objective:To observe the clinical efficacy of Wenjing Huayu Zhitong therapy in treating primary dysmenorrhea (PD) with cold coagulation and blood stasis, and to explore its immune mechanisms on PD. Method:The 108 PD patients with cold coagulation and blood stasis syndrome were collected and randomly divided into traditional Chinese medicine (TCM) group, ibuprofen group and placebo group according to the random number table method, with 36 cases in each group. All patients received corresponding medicines three days before menstruation. The patients in TCM group were treated with TCM and ibuprofen sustained release capsule simulator. The patients in ibuprofen group were treated with ibuprofen sustained-release capsule and TCM simulator. The patients in placebo group were treated with TCM simulator and ibuprofen sustained-release capsule simulator. Treatment lasted for 6 consecutive days for three menstrual cycles, and follow-up was conducted for three menstrual cycles after drug withdrawal. The visual analogue score (VAS), total time of abdominal pain and TCM symptom scores in each menstrual cycle were recorded. The levels of CD3+, CD4+, CD8+ and the ratio of CD4+/CD8+ in peripheral blood before and after treatment were detected by flow cytometry. Result:After treatment for three menstrual cycles, both the TCM group and ibuprofen group were better than placebo group in reducing VAS score and reducing total abdominal pain time (P<0.01). The long-term follow-up effect after drug withdrawal in TCM group was significantly better than that in ibuprofen group (P<0.01). The total effective rate was 91.43% (32/35) in TCM group, 66.67% (10/33) in ibuprofen groups, and 30.30% (10/33) in placebo group . The efficacy of the TCM group was better than that of the ibuprofen group (χ2=-2.971, P<0.01), and the efficacy of the ibuprofen group was better than that of the placebo group (χ2=-2.371, P<0.05). After treatment, the levels of CD3+, CD4+ and CD4+/CD8+ in TCM group were significantly increased and the levels of CD8+ were decreased significantly as compared with those before treatment (P<0.01). After treatment, the levels of CD4+ and CD4+/CD8+ in TCM group were higher (P<0.05,P<0.01),while the levels of CD8+ were significantly lower than those in ibuprofen group and placebo group (P<0.01). Conclusion:Wenjing Huayu Zhitong therapy can reduce the VAS score and accumulative time of abdominal pain, and improve the dysmenorrhea symptoms in patients with PD. Reversal of the T cell subsets disorder may be one of its mechanisms in treating PD with cold coagulation and blood stasis.

ABSTRACT

Tumor is one of the diseases that seriously endanger human health, and how to treat tumor effectively is still one of the important problems in the field of medicine. At present, most of the radiotherapies and chemical drugs for cancer have serious side effect despite of an obvious efficacy. With a unique syndrome differentiation treatment system and overall concept, traditional Chinese medicine has become the key research and development object of antitumor drugs due to many advantages, such as multiple channels, multiple levels, multiple links, multiple targets and less toxicity, and could can fully mobilize the immune and epidemic prevention mechanism of the body. A large number of studies have shown that Xiao Xianxiongtang and its effective ingredients have obvious antitumor effect. Many doctors have applied Xiao Xianxiongtang and modified formulas in clinical treatment of tumors, and relevant pharmacological studies have also confirmed the effectiveness of this formula, but with a lack of systematic summary of its effective ingredients and its mechanism of action. Now, with alkaloids, ketones, sterols and phenols in Xiao Xianxiongtang as the starting point, this study mainly focuses on inhibition of tumor cell proliferation, invasion and migration, induction of tumor cell apoptosis and autophagy, inhibition of tumor cell cycle, enhancement of tumor cell sensitivity, inhibition of tumor angiogenesis and regulation of immunosuppressive tumor microenvironment from two ways to sort out composition, function and mechanism of drugs. In this paper, effective components, main targets and mechanism of intervention in the tumor development of Xiao Xianxiongtang were reviewed, in order to provide a new idea for subsequent antitumor research and development of this prescription.

ABSTRACT

PURPOSE: Plasma cells and immunoglobulins (Igs) play a pivotal role in the induction and maintenance of chronic inflammation in nasal polyps. During secondary immune responses, plasma cell survival and Ig production are regulated by the local environment. The purpose of the present study was to investigate the presence of long-lived plasma cells (LLPCs) and specific survival niches for LLPCs in human nasal polyps.METHODS: Nasal mucosal samples were cultured with an air-liquid interface system and the Ig levels in culture supernatants were analyzed by enzyme-linked immunosorbent assay. The characteristics of LLPCs in nasal polyps were determined by immunohistochemistry and immunofluorescence. The expression of neurotrophins as well as their receptors was detected by quantitative real-time polymerase chain reaction, immunohistochemistry, immunofluorescence, and Western blotting.RESULTS: The numbers of CD138⁺ total plasma cells and BCL2⁺ plasma cells were increased in both eosinophilic and non-eosinophilic nasal polyps compared with those in normal tissues. The production of IgG, IgA, and IgE was detected in culture supernatants even after a 32-day culture of nasal polyps. Although the total numbers of plasma cells were decreased in nasal polyps after culture, the numbers of BCL2⁺ plasma cells remained stable. The expression of nerve growth factor (NGF) as well as tropomyosin receptor kinase (Trk) A, a high-affinity receptor for NGF, was upregulated in both eosinophilic and non-eosinophilic nasal polyps. In addition, BCL2⁺ plasma cell numbers were positively correlated with NGF and TrkA mRNA expression in nasal mucosal tissues. Polyp plasma cells had the expression of TrkA.CONCLUSIONS: Human nasal polyps harbor a population of LLPCs and NGF may be involved in their prolonged survival. LLPCs may be a novel therapeutic target for suppressing the local Ig production in nasal polyps.

ABSTRACT

Non-alcoholic steatohepatitis (NASH) is a chronic metabolic syndrome and the CFLAR-JNK pathway can reverse the process of NASH. Although silibinin is used for the treatment of NASH in clinical, its effect on CFLAR-JNK pathway in NASH remains unclear. This study aimed to investigate the effect of silibinin on CFLAR-JNK pathway in NASH models both and . The study was performed using male C57BL/6 mice fed with methionine- choline-deficient diet and simultaneously treated with silibinin for 6 weeks. The study was performed by using mouse NCTC-1469 cells which were respectively pretreated with oleic acid plus palmitic acid, and adenovirus-down for 24 h, then treated with silibinin for 24 h. After the drug treatment, the key indicators involved in CFLAR-JNK pathway including hepatic injury, lipid metabolism and oxidative stress were determined. Silibinin significantly activated CFLAR and inhibited the phosphorylation of JNK, up-regulated the mRNA expression of and , reduced the activities of serum ALT and AST and the contents of hepatic TG, TC and MDA, increased the expression of NRF2 and the activities of CAT, GSH-Px and HO-1, and decreased the activities and expression of CYP2E1 and CYP4A . These effects were confirmed by the experiments. Silibinin prevented NASH by regulating CFLAR-JNK pathway, and thereby on one hand promoting the -oxidation and efflux of fatty acids in liver to relieve lipid accumulation, and on the other hand inducing antioxidase activity (CAT, GSH-Px and HO-1) and inhibiting pro-oxidase activity (CYP2E1 and CYP4A) to relieve oxidative stress.

ABSTRACT

In this study, the effects of honokiol (HN) treatment for 24 h on lipid synthesis was examined in HepG2 cells. The parameters include intracellular lipid droplet and the expression of SREBP-1c and PNPLA3, glucose uptake, and oxidative stress including the expression of CYP2E1 and CYP4A in normal, TO901317 (TO)- and oleic acid (OA)-treated HepG2 cells. The lipid droplets were detected by oil red O staining. The glucose uptake was measured by fluorescence spectrophotometry using[2-(N-(7-nitrobenz-2-oxa-1,3-diazol-4-yl) amino)-2-deoxyglucose, 2-NBDG] as probe. The expression levels of target genes were detected by quantitative PCR and Western blot. The results showed that:① TO (5 μmol·L−1) and OA (0.5 mmol·L−1) treatment increased the levels of intracellular lipid accumulation and the mRNA and protein expression of SREBP-1c and PNPLA3. After HN (10, 20, 40 μmol·L−1) treatment for 24 h, the lipid accumulation and the expression of SREBP-1c and PNPLA3 were all decreased in the tested cells. ② OA treatment significantly suppressed glucose uptake, while HN treatment dose-dependently increased the glucose uptake in OA-treated cells. ③ Compared with control group, CYP2E1 protein level significantly decreased in the three tested cells, and CYP4A protein level significantly decreased only in OA-treated cells following HN treatment. The above results suggest that HN may attenuate lipid accumulation by suppressing the expression of SREBP-1c and PNPLA3, and reduce lipid peroxidation and insulin resistance by down-regulation of the protein levels of CYP2E1 and CYP4A in HepG2 cells with steatosis.

ABSTRACT

Objective: To explore the effect of arecoline hydrobromide (AH) on the expression of rat hepatic and renal transporters. Methods: The effect of AH on the mRNA expression of 13 hepatic and renal transporters was studied after orally giver AH (0.8, 4, and 20 mg/kg/d) to rats for 21 d. Results: The results from the real-time PCR indicated that, AH treatment at low dose significantly decreased the mRNA levels of hepatic MRP2 and MDR1A, while significantly increased renal MRP5 mRNA level. On the other hand, AH treatment at high dose significantly inhibited the mRNA expression of hepatic OCT2, OAT2, OCTN2, OATP1A1, OATP1A4, OATP2B1, MRP2, and MDR1A, as well as renal MRP2, BCRP, and MDR1A. However, the mRNA expression of renal OCTN2, OATP1A1, OATP1A4, and MRP5 were significantly up-regulated following the treatment of high dose of AH. And the AH-induced effect on the above transporters was dose dependent in some extent. Conclusion: Due to the drug interaction caused by the alteration in expression and function of hepatic and renal transporters, it is suggested that the betel nut addicts should be paid more attention in case of adverse drug interactions.

ABSTRACT

<p><b>OBJECTIVE</b>To compare the advantages and disadvantages between exercise challenge test (ECT) and methacholine challenge test (MCT) in the measurement of airway hyperresponsiveness (AHR), in order to identify a better and safer method to measure AHR.</p><p><b>METHODS</b>Forty-seven children with controlled asthma after regular treatment were enrolled. ECT and MCT were performed for each child successively, and sensitivity was obtained through comparison with the golden standard (PD20). The occurrence of bronchospasm symptoms during the two tests was recorded.</p><p><b>RESULTS</b>Taking PD20 as the gold standard, in children with moderate or severe AHR, the sensitivity of MCT (61%) for the measurement of AHR was significantly higher than that of ECT (9%) (P<0.05). The consistency between MCT results and PD20 was relatively high (κ=0.614), while the consistency between ECT results and PD20 was relatively low (κ=0.006). However, in the MCT, the incidence of bronchospasm symptoms was high and positively correlated with the incidence of cough and chest distress (P<0.05).</p><p><b>CONCLUSIONS</b>MCT has a higher sensitivity for the measurement of AHR, but has a higher incidence of adverse events, compared with ECT in children with controlled asthma after regular treatment.</p>

ABSTRACT

<p><b>BACKGROUND</b>Hepatic ischemia-reperfusion (I/R) injury occurs in many clinical procedures. The molecular mechanisms responsible for hepatic I/R injury however remain unknown. Sphingolipids, in particular ceramide, play a role in stress and death receptor-induced hepatocellular death, contributing to the progression of several liver diseases including liver I/R injury. In order to further define the role of sphingolipids in hepatic I/R, systemic analysis of sphingolipids after reperfusion is necessary.</p><p><b>METHODS</b>We investigated the lipidomic changes of sphingolipids in a rat model of warm hepatic I/R injury, by delayed extraction matrix-assisted laser desorption ionization time-of-flight mass spectrometry (DE MALDI-TOF-MS).</p><p><b>RESULTS</b>The total amounts of ceramide and sphingomyelin and the intensity of most kinds of sphingolipids, mainly sphingomyelin, significantly increased at 1 hour after reperfusion (P < 0.05) and reached peaks at 6 hours after reperfusion (P < 0.01) compared to controls. Six new forms of ceramide and sphingomyelins appeared 6 hours after reperfusion, they were (m/z) 537.8, 555.7, 567.7, 583.8, 683.5 and 731.4 respectively. A ceramide-monohexoside (m/z) 804.4 (CMH(d18:1C22:1+Na)(+)) also increased after reperfusion and correlated with extent of liver injury after reperfursion.</p><p><b>CONCLUSIONS</b>Three main forms of sphingolipids, ceramide, sphingomyelin and ceramide-monohexoside, are related to hepatic I/R injury and provide a new perspective in understanding the mechanisms responsible for hepatic I/R injury.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To understand the trend of gastric cancer mortality based on the exploration of the death data from 1970 to 2004 in Zhaoyuan County.</p><p><b>METHODS</b>The death data of 1970-1974 were collected from the first malignant tumor death survey of China, and the data of 1985-2004 were derived from the cancer registration network of Zhaoyuan. The appreciation indexes were crude mortality rate and standardized mortality rates by the percentages of Chinese population in 1982 and the percentages of world population in 1966.</p><p><b>RESULTS</b>The crude mortality rates in the five periods of 1970-1974, 1985-1989, 1990-994, 1995-1999, and 2000-2004 were 34.94, 44.34, 40.05, 48.73, and 40.18 per 100,000, respectively; the standardized mortality rates by Chinese population were 30.82, 30.35, 27.78, 31.63, and 20.10 per 100,000, respectively. The gastric cancer mortality remained stability from 1970 to 1999, however, the standardized mortality rate of gastric cancer in 2000-2004 declined by 34.8% compared with that in 1970-1974. The study also observed that the highest age-specific gastric cancer mortality, in the past 35 years, has been postponed by 10 years. And, the gastric cancer mortality ranked from top first to top three.</p><p><b>CONCLUSION</b>There is a decreasing trend for the incidence of stomach cancer and this might be associated with the changes of dietary consumption and pattern.</p>
