ABSTRACT

Aim To study the correlation between oxidative stress and neurotoxicity induced by deguelin, providing the mechanism basis for the structural modification and drug combination about deguelin. Methods SH-SY5Y cells were exposed to deguelin ranging from 1. 56-100 jimol • L"1 for 24 to 72 h, whose survival rate was determined by CCK-8 assay. The content of LDH, MDA levels, GPx and antioidant enzyme activities of SOD were determined using the assay kits ac-cording the manufacturer's protocol. And the content of ROS was determined by flow cytometry. The expressions of MEK, EGF, ERK and CREB were determined by immunoblotting. Results Deguelin obviously inhibited the growth of SH-SY5Y cells in a concentra-tion-and time-dependent manner ( P < 0. 05 ). After the cells were injured by deguelin, the content of ROS, LDH and MDA in SH-SY5Y cells increased obviously (P <0. 05), while the vitality of GPx and SOD decreased obviously ( P < 0. 05) in a dose-dependent manner. Furthermore, the protein expression levels of MEK, EGF, RAS and CREB in MAPK/ERK signaling pathway decreased obviously when treated with deguelin in a dose-dependent manner. (P < 0. 05). Conclusions The trauma of SH-SY5Y cells induced by deguelin is closely related to oxidative stress, in which MAPK/ERK signaling pathway may play a mediating role.

ABSTRACT

Objective: To investigate the clinical features and prognosis of intracranial embryonal tumors in children. Methods: Clinical data of 27 patients with intracranial embryonal tumors who were admitted to Beijing Shijitan Hospital, Capital Medical University be-tween May 2011 and December 2018 were retrospectively analyzed. Results: The study included 27 patients, comprising 17 male and 10 female children, with a median age of 7 years. Twelve patients underwent gross total resection, and 15 patients underwent subto-tal resection. After surgery, all 27 patients underwent craniospinal irradiation. The dose of craniospinal irradiation was 27-30.6 Gy, and the dose of the tumor bed was 55.8-60 Gy. Twenty-two patients underwent chemotherapy regimen composed of irinotecan, vtncris-tine, etoposide, nedaplatin after radiotherapy. The median follow-up period was 22 (4-93) months. Nine patients are alive without lo-cal or distant recurrence. Eighteen patients died; the causes of death were intracranial recurrence and spinal cord metastasis. The 1-, 3-, and 5-year survival rates were 79.4%, 50.2%, and 36.5%, respectively. Conclusions: Intracranial embryonal tumors in children are rare and malignant and can metastasize along the neuroaxis. Surgery, adjuvant radiotherapy, and chemotherapy are important for pa-tients with intracranial embryonal tumors. Moreover, the prognosis is poor.

ABSTRACT

Psoriasis is a common,chronic autoimmune/inflammatory disease directed by abnormal activation of skin immune system.It is characterized by erythematous plaques and slivery scales in the patients'lesional sites.Dendritic cells,macrophages,gamma delta T cells and group 3 innate lymphocytes are great contributors to the progress of the disease,as they are the main source of proin-flammatory cytokines like TNF-α,IL-1β,IL-6,IFN-α,IL-23 and IL-17.Particularly,as the initiator of the inflammation,DCs are indispensable of the progress.There are different subpopulations of dendritic cells in the skin,in this review,we are trying to elaborate their unique location and function based on the published articles.

ABSTRACT

In this experiment, rat nasal mucosa absorption characteristics of prim-O-glucosylcimifugin and 5-O-methylvisammioside were studied to provide a basis for drug delivery of Toutongning nasal spray. The nasal mucosa absorption test in rats was conducted with in situ nasal perfusion method after pH 6 buffer solution was used to prepare high, medium and low concentrations of prim-O-glucosylcimifugin, 5-O-methylvisammioside mixed solution as liquid circulation in nasal cavity. Then the concentrations of the circulating liquid compositions to be measured were determined by HPLC, and the absorption rates of prim-O-glucosylcimifugin and 5-O-methylvisammioside under different pH conditions were also investigated. According to the results, the absorption rate constant was (0.588±0.041)×10⁻³, (0.547±0.023)×10⁻³, (0.592±0.063)×10⁻³ min⁻¹ for prim-O-glucosylcimifugin high, middle and low concentrations, and (0.438±0.041)×10⁻³, (0.407±0.023)×10⁻³, and (0.412±0.063)×10⁻³ min⁻¹ for 5-O-methylvisammioside high, middle and low concentrations. There was no significant difference among high, middle and low concentration groups, and the absorption under pH 6 was better than that under other pH conditions. Therefore, we can get the conclusion that the main active ingredient of Toutongning nasal sprays can be absorbed through the nasal mucosa, and it is feasible to make nasal spray; in addition, pH 6 of nasal spray is scientific and reasonable.

ABSTRACT

Objective To evaluate the reasonable individual program for upper urinary tract calculi in minimally invasive treatment. Methods From January 2013 to June 2016, 147 patients (sides) with upper urinary calculi who had some characteristics received laparoscopic nehprectomy or corresponding surgical treatment. The number of cases of postoperative stagnation, the average hospitalization time and the average cost of treatment were compared with those of 147 patients (lateral) who underwent PCNL and URSL with the similarity, shape and load of stones before June 2016, respectively. Results The removal rate of stage I was 100.00% (147/147) in laparoscopic group, which was significantly higher than that in PCNL and URSL group (91.84%, 135/147), the difference was statistically significant (P = 0.001); Laparoscopic group postoperative blood transfusion (0/147) and interventional hemostasis (0/147) were significantly lower in 6 cases (6/147) and 4 cases (4/147) in PCNL and URSL groups,the differences were statistically significant (P = 0.013, P = 0.044). There was no postoperative severe infection in laparoscopic group (0/147), which has no significant difference (P = 0.156) in postoperative severe infection between PCNL and URSL group (2/147). There were 9 cases of 134 cases of postoperative (9/134) fever at ≥ 38℃ in laparoscopic group, which was significantly lower than that in PCNL and URSL group (28/147), the difference was statistically significant (P = 0.002); Laparoscopic group of postoperative urinary tract stenosis in 3 cases (3/114), which was significantly lower than that of PCNL and URSL group (9/101), the difference was statistically significant (P = 0.045). The average length of stay in laparoscopic group was (10.12 ± 0.29) d, which was significantly lower (P = 0.011) than that in PCNL and URSL group (13.97 ± 0.38) days. The average cost of treatment in laparoscopic group (12541.84 ± 181.54) yuan was significantly lower than that in PCNL and URSL group (18124.65 ± 302.32) yuan, the difference was statistically significant (P = 0.018). Conclusion In some cases, when the upper urinary tract calcuci are suitable for 'cut out', the use of laparoscopic treatment is more secure; when there is a need for surgical treatment of complications, can be treated simultaneously. Laparoscopic technique is one of the important methods of minimally invasive treatment for upper urinary calculi.

ABSTRACT

Objective To evaluate the prognostic factors and treatment results of intracranial ependymomas (EPs).Methods Thirty-one intracranial EPs patients who received postoperative radiotherapy in Beijing Shijitan Hospital between January 2009 and June 2012 were analyzed retrospectively.Twenty-two males and 9 females had an average age of 18 years (range 3-60 years).Seventeen patients received gross total resection (GTR) while fourteen received subtotal resection (STR).Median total radiation dose was 53.9 Gy (48.6-60 Gy).The three-year and five-year progress-free survival (PFS) and overall survival (OS) were estimated with the Kaplan-Meier method.Univariate analysis was performed using eight clinical and dosimetric factors by Log-Rank testing.Cox proportional hazards model was used to identify the independent prognostic factors correlated to EPs.Results The median time of follow-up was 51 months.At the endpoint of the follow-up period,7 patients experienced tumor recurrence:5 had a local recurrence (LR) and 2 had both LR and distant recurrence.Six patients died,4 of which had cases of pediatric infratentorial anaplastic EPs.The three and five-year progress-free survival (PFS) were 80.6% and 75.9%.Overall survival (OS) at three-year and five-year were 83.9% and 76.2%,respectively.Univariate analysis showed a more favorable prognoses in terms of three-year PFS,five-year PFS and OS for GTR compared to STR (x2 =4.685,6.311,4.238,P < 0.05).Besides,a more favourable univariate outcome in terms of five-year PFS was evident in patients when the total radiotherapy dose was > 55 Gy compared to ≤55 Gy (x2 =4.210,P < 0.05),and no severe radiotherapy complications occurred.Conclusions Surgery is the major treatment method,while adjuvant radiotherapy is important for subtotal resection and anaplastic EPs patients.Surgical resection and radiotherapy dose were relevant to prognosis.

ABSTRACT

Objective To evaluate the hematological toxicity of craniospinal irradiation,and determine the short-term clinical efficacy and prognostic factors in medulloblastoma.Methods Eightseven patients who underwent craniospinal irradiation were retrospectively analyzed with respect to the changes in hematology during craniospinal irradiation.The effect of sex,age,tumor location,interval between surgery and radiation,interval time during radiation and radiation sequence on survival were also studied.Results The 1,2,3-year overall survival (OS) and progress-free survival (PFS) rate were 95.0%,92.4%,84.9% and 93.7%,89.8%,80.8%,respectively.The incidence of 2-3 grade leucopenia was 90.8%,while the incidence of 1-2 grade thrombocytopenia was 70.1%,and the incidence of 3 grade thrombocytopenia was 1.1%.The incidence of 1-2 grade hemoglobin reduction was 16.1%.No patient had grade 3-4 hemoglobin reduction.Kaplan-Meier analysis shows that more favorable prognoses in terms of 3-year PFS were evident for 0-1 grade thrombocytopenia compared with 2-4 grade thrombocytopenia (x2 =3.936,P < 0.05).And 3-year PFS and 3-year OS were evident for 0 grade hemoglobin reduction compared with 1-4 grade hemoglobin reduction (x2 =10.269,9.336,P < 0.05).The 3-year PFS between interval time during radiation < 3 days and ≥ 3 days was 84.6% and 68.6% (x2 =4.413,P < 0.05).Conclusions Hematological toxicity during craniospinal irradiation and the interval time during radiation were prognostic factors.

ABSTRACT

objective:To investigate the clinical features and prognosis of pinealoblastoma in children. Methods:The clinical data of 10 patients with pinealoblastoma were retrospectively analyzed. These patients were admitted to the Department of Radiation Oncology, Beijing Shijitan Hospital, Capital Medical University from December 2011 to December 2015. Results:This study included 10 patients, with 8 males and 2 females, with a median age of 7 years. Of the 10 patients, 5 underwent gross total resection and 5 underwent sub-total resection. The 10 patients were exposed to craniospinal irradiation after surgery. The median doses of craniospinal irradiation and tumor bed were 30.6 (25.5-36) Gy and 55.8 (50.4-60) Gy, respectively. Of the 10 patients, 4 underwent chemotherapy 1 month af-ter radiotherapy. The median follow-up period was 16.5 (1.5-49) months. The 10 patients survived. No local and distant recurrences were observed. Conclusion:Pinealoblastomas are rare, malignant, and pineal regional lesions that can metastasize along the neuroax-is. Surgery combined with adjuvant radiotherapy and chemotherapy should be applied to patients with pinealoblastoma. Prognosis is also favorable.

ABSTRACT

Objective To investigate the application value of CT/MR image fusing in gross tumor volume (GTV) delineation of esophageal squamous cell carcinoma.Methods Twenty-nine patients with esophageal squamous cell carcinoma to be treated with radical surgery underwent routine CT scanning,MR T2-weighted imaging (T2WI) and diffusion weighted imaging (DWI) before surgery.Diffusion-sensitive gradient b values were taken at 400,600,and 800 s/mm2.GTVs were delineated on the CT image,CT/ MR T2WI,and CT/MR DWI respectively.The MR T2W1 image was used as the intermediary for the fusion of the CT image and MR DWI image.The length of GTVs measured under different imaging conditions were compared with the length of the resected specimen of esophagus.Results The GTV length was (44.94 ± 18.46) and (45.05 ±21.47) mm on the CT images and CT/MR T2WI images respectively.When the b values were 400,600,and 800 s/mm2,the esophageal carcinoma GTV length on CT/MR DWI images was (42.12 ± 17.79),(41.18 ± 17.17) and (39.77 ± 17.66) mm,respectively.The coefficient between the esophageal carcinoma GTV lengths on CT/MR DWI images and the pathological lesion lengths was 0.928,0.926 and 0.927 respectively.Conclusions CT/MR DWI images displays esophageal carcinoma GTV length more accurately,thus guiding the delineation of GTV effectively.

Objective To evaluate the difference between CT and CT/MR DWI images for gross tumor volume (GTV) delineation in precision radiotherapy for esophageal squamous cell carcinoma.Methods 20 patients with pathologically confirmed as esophageal squamous cell carcinoma were selected for target delineation.The GTV based on CT and CT/MR DWI images of the patients were delineated by six radiation oncologists.The mean,standard deviation,coefficient of variation (CV =standard deviation/mean) of GTV volume,the ratio of minimum value and maximum value (ratio =maximum value/minimum value) of the GTV volume were calculated.And the CV and ratio of the GTV by the two methods were compared.Results The biggest difference of GTV volume on CT and on CT/MR DWI images was 55.71 and 13.89 cm3 (F =12.80,P < 0.05).The CV on CT and CT/MR DWI images were 0.30 ± 0.08,0.11-±0.04 (Z =-3.92,P < 0.05),and the ratio of GTV volume were 2.38 ± 0.62,1.34 ± 0.13,respectively (Z =-3.92,P < 0.05).Conclusions CT/MR DWI images could display GTV more directly,which may help to increase consistency of GTV for esophageal squamous cell carcinoma delineation among different radiation oncologists.

ABSTRACT

<p><b>BACKGROUND</b>Few studies were reported on the comparison of clinical outcomes between intensity-modulated radiotherapy (IMRT) and three-dimensional conformal radiotherapy (3D-CRT) in the treatment of glioblastoma multiforme (GBM). This study aimed to determine whether IMRT improves clinical outcomes compared with 3D-CRT in patients with GBM.</p><p><b>METHODS</b>The records of 54 patients with newly-diagnosed GBM from July 2009 to December 2010 were reviewed. The patients underwent postoperative IMRT or 3D-CRT with concurrent and adjuvant temozolomide. Kaplan-Meier method and log rank test were used to estimate differences of patients' survival.</p><p><b>RESULTS</b>The median follow-up was 13 months. Of the 54 patients, fifty (92.6%) completed the combined modality treatment. The 1-year overall survival rate (OS) was 79.6%. The pattern of failure was predominantly local. A comparative analysis revealed that no statistical difference was observed between the IMRT group (n = 21) and the 3D-CRT group (n = 33) for 1-year OS (89.6% vs. 75.8%, P = 0.795), or 1-year progression-free survival (PFS) (61.0% vs. 45.5%, P = 0.867). In dosimetric comparison, IMRT seemed to allow better sparing of organs at risk than 3D-CRT did (P = 0.050, P = 0.055). However, there was no significant difference for toxicities of irradiation between the IMRT group and the 3D-CRT group.</p><p><b>CONCLUSIONS</b>Our preliminary results suggested that delivering standard radiation doses by IMRT is unlikely to improve local control or overall survival for GBM compared with 3D-CRT. Given this lack of survival benefit and increased costs of IMRT, the utilization of IMRT treatment for GBM needs to be carefully rationalized.</p>

ABSTRACT

<p><b>BACKGROUND</b>Wound infection following spinal instrumented surgery is a frequent complication. The optimal treatment of acute deep wound infection following spinal instrumentation fusion remains controversial because of variability in cohort identification, definition of an infection, and the instrument used to measure outcomes. This retrospective study evaluated the clinical curative effect for postoperative spinal infection after instrumented spine fusion with extensive debridement, or implant removal.</p><p><b>METHODS</b>From January 2004 to October 2009, 851 patients were identified who underwent surgical treatment of spinal diseases. The medical records of patients who developed infections were reviewed in detail.</p><p><b>RESULTS</b>Of 851 patients, 41 (4.9%) developed an infection. Thirty-three were acute, and eight were delayed. Acute infected cases were managed with antibiotic therapy, and aggressive debridement of the wound and soft tissues leaving all instrumentation in situ in all but one patient. The most common symptoms of acute infection included: posterior incisional drainage (26 of 33 patients), back pain (22 of 33 patients) and fever (13 of 33 patients). Among patients with delayed onset infection, five of eight patients had local pain, four of eight patients had incision drainage, and one patient had a prolonged period of intermittent fever. The most frequent causative organism for postoperative spinal infection following spine surgery is Staphylococcus aureus. Pseudarthrosis was noted in long-term follow-up in four of 41 patients.</p><p><b>CONCLUSIONS</b>We recommend irrigation and debridement, no instrumentation removal, and, if necessary, repeat debridement followed by delayed primary closure for the treatment of acute deep infection with instrumentation.</p>

ABSTRACT

Prostacyclin (PGI2) is a member of the prostaglandins family and the main product of arachidonic acid metabolism by cyclooxygenase and prostaglandin synthase. PGI2 mediates cellular activity through binding to G-protein coupled receptor, prostacyclin receptor, therefore leading to activation of adenylyl cyclase, accumulation of cAMP and activation of PKA. PGI2 plays an important role in vascular biology such as regulating vasodilation, platelet aggregation, vascular permeability, and vascular smooth muscle cell proliferation. At high concentrations, PGI2 is able to interact with peroxisomal proliferator-activated receptor, a class of nuclear receptors, and modulate specific cellular functions such as angiogenesis. This paper reviews the research advances in PGI2 synthesis and signal pathway.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the phenotype, frequency and function of CD4+ T cell subsets and the relevant cytokines, as well as the relationship between these cells and appearance of pneumonia of novel (H1N1) influenza A patients.</p><p><b>METHODS</b>68 healthy people, 53 confirmed novel A(H1N1) influenza patients without pneumonia and 16 confirmed severe novel A (H1N1) influenza patients with pneumonia were enrolled in this study. Viral load in nasopharyngeal swabs specimens was measured by real time PCR assay. The phenotype and percentage of CD4+ T cell subsets including Th1, Th2, Th17, and Treg cells were measured by Flow cytometry analysis. The relevant cytokines in plasma including TGF-beta, IL-6 and IFN-gamma were measured by ELISA. Data was analyzed by one way ANOVA.</p><p><b>RESULTS</b>It was found that peak viral load and viral shedding period of severe patients with pneumonia was significantly increased compared with mild patients without pneumonia (P < 0.05). The percentage of Th17 cells of severe patients with pneumonia was significantly diminished compared to that of healthy subjects and mild patients without pneumonia (P < 0.05). However, Th1, Th2, Treg cells frequencies had no significant differences (P > 0.05) among these three groups. The level of TGF-beta in plasma for the severe patients with pneumonia was also significantly decreased compared to that of healthy subject and mild patients without pneumonia (P < 0.05). The viral shedding period inversely correlated with the frequency of Th17 cells (r = - 0.38, P < 0.05).</p><p><b>CONCLUSION</b>H1N1 influenza A virus can inhibit Th17 cells to differentiate, particularly more extent in patients with pneumonia. Impaired Th17 cells may correlate with viral clearance and pneumonia of novel H1N1 influenza A patients.</p>

ABSTRACT

Objective To observe the efficacy and acute toxicity in non-myeloablative low-dose tatal body irradiation. Methods From January 2006 to January 2008, 27 cases of hematopoietic stem cell transplantation patients received non-myeloablative pretreatment program involving low-dose whole body irradiation. Pretreatment program: the total dose of TBI was 2 Gy, once completely. According to primary disease, physical condition, age, organ function and HAL coincide situation, the patients received different chemotherapy, including FUL, CTX, Ara-C, melphalan and so on. To prevent the graft-versus-host disease (GVHD), CSA/MMF was used. Results Hematopeietic reconstruction: the peripheral blood WBC in all 27 cases reduced to (0.05-0.9) × 109/L in 4 to 8 days, with the neutrophil count > 0.5 × 109/L in 8 to 22 days (median + 10.5 days), the platelet count > 30 × 109/L in 11 to 28 days (median + 14.5 days). The transplant rate was 85.2% (23/27). The fever rate was 65% and the infection rate was 37% (10/27). Hemorrhagic cystitis,hepatic veno-occlusive disease (HVOD), and other complications did not occurred. 4 cases developed aGVHD (15 %) and 5 cases developed cGVHD (19 %) out of 27 cases. Follow-up 3 to 22 months, 21 eases are still alive (77.8%). Conclusions Pretreatment with low-dose whole body irradiation of non-myeloablative hematopeietic stem cell transplantation proved to be simple and safe with fewer complications and broad indications. Program of TBI with total dose of 2 Gy (once completely) is a safe, less toxic and effective method. It could achieve the effectiveness of immune suppression.

OBJECTIVE To investigate the status of the management of nosocomial infection in Anhui Province so as to help to improve it.METHODS Twenty-four hospitals were on-the-spot investigated in a way of questioning and sampling using uniform questionnaire by trained investigators.RESULTS 95.83% Of all involved hospitals have built independent department of nosocomial infection management.These departments from 70.83% hospitals play an important role in infection management and 45.83% of them have suitable structure of staff.There are some problems in the key regions of nosocomial infection control such as operating room,blood dialyzing center and sterilization supply department: small work area,old equipment,irrational arrangement,sterilizing inadequately and so on.CONCLUSIONS The work of administration of nosocomial infections needs to be strengthened.

To promote the normalization,standardization and systematization of hospital infection control,the Coordination and Advisory Group for Hospital Infection Control of the province carried out an investigation in thirty-five 2nd grade hospitals of fourteen prefectures and cities in regard to the orga- nization and management of hospital infection control,necessary infection monitoring and sterilization devices against highly infectious areas through questionaires,medical theory tests and on-the-spot inves- tigations.Analysis is also made of the present conditions and pending problems in the field of infection control of the 2nd grade hospitals in the province.Relevant solutions are suggested.
