ABSTRACT

Los cereales integrales son una categoría de alimentos fundamental para la dieta humana y representan una fuente invaluable de carbohidratos, proteínas, fibras, fitoquímicos, minerales y vitaminas. Muchos estudios han demostrado que el consumo de cereales integrales está relacionado con un menor riesgo de enfermedades crónicas no transmisibles. Sin embargo, varios de sus efectos positivos para la salud parecen desaparecer cuando se refinan los cereales. La pregunta de investigación es cuál sería el efecto de los cereales integrales en la prevención de enfermedades crónicas no transmisibles. Este artículo describe los efectos positivos del consumo de cereales integrales en la prevención de estas enfermedades. Para ello, se realiza una descripción narrativa que revisa los antecedentes disponibles sobre: a) situación epidemiológica en Chile y su relación con la alimentación; b) consumo de cereales integrales en el mundo y en Chile; y c) el efecto del consumo de cereales integrales en la reducción del riesgo de enfermedad cardiovascular. obesidad, diabetes tipo 2 y cáncer. Los antecedentes generales son muy alentadores sobre el efecto positivo del consumo de cereales integrales sobre determinadas enfermedades, especialmente las de origen metabólico. Sin embargo, la incertidumbre de algunas asociaciones negativas merece mayor atención.


Whole grains are a fundamental food category for the human diet and represent an invaluable source of carbohydrates, proteins, fibers, phytochemicals, minerals and vitamins. Many studies have shown that consumption of whole grains is linked to a lower risk of chronic non-communicable diseases. However, several of its positive health effects seem to disappear when grains are refined. The research question is what would be the effect of whole grains in the prevention of Chronic Non-Communicable Diseases. This article describes the positive effects of whole grain consumption in these diseases.To this end, a narrative description is made that reviews the available background on: a) epidemiological situation in Chile and its relationship with diet; b) consumption of whole grains in the world and in Chile; and c) the effect of whole grain consumption on reducing the risk of cardiovascular disease. obesity, type 2 diabetes, and cancer. The general background is very encouraging for a positive effect of whole grain consumption on certain diseases, especially those of metabolic origin. However, the uncertainty of some negative associations deserves further attention.

ABSTRACT

Antecedentes: La pandemia de COVID-19 en Guatemala provocó el rebalse de los ya colapsados hospitales. Los servicios de otras especialidades además de Medicina Interna tuvieron que emplearse de lleno en atender la pandemia. En el Hospital General San Juan de Dios, el servicio de operados de emergencia atendió durante 4 meses pacientes con esta enfermedad, a cargo del personal quirúrgico, residentes de infectología y de medicina de emergencias para apoyar a la especialidad de Medicina Interna; mostrando la necesidad de la intervención de todas las especialidades en la atención a la pandemia. Material y Métodos: Estudio descriptivo transversal sobre la experiencia del servicio de operados de Emergencia en la atención a pacientes con enfermedad COVID-19 de julio a octubre del 2020. Resultados: Se atendieron 178 pacientes con COVID-19, con predominio de sexo masculino 57%, con edades de 16 a 91 con una media de 65 años.70 pacientes eran hipertensos, 69 diabéticos y 13 con enfermedad renal. 169 casos tuvieron neumonía por SARS-COV2, 2 con choque y fallo orgánico múltiple. La mortalidad fue de 13 casos (7 %). Los medicamento más utilizados fueron las heparinas de bajo peso molecular. El 86 % recibió terapia con esteroides, 50% con anticuerpos monoclonales, especialmente tocilizumab. El antibiótico más usado fue la Ceftriaxona (32 %). 6 pacientes recibieron remdesivir (3%). CONCLUSIONES: Este estudio evidencia la experiencia del servicio de operados de emergencia durante la pandemia de COVID19, donde médicos no expertos en enfermedades virales infecciosas, aprendieron sobre la marcha el manejo de esta enfermedad. Un significativo número de pacientes fueron beneficiados con la implementación de estos servicios. El aprendizaje, las competencias y los servicios que presta el médico dedicado a urgencias debe adaptarse para responder a las necesidades de la población (AU)


Background: The COVID-19 pandemic in Guatemala collapsed hospitals due to overflow. The services of other specialties in addition to Internal Medicine had to be fully used to attend the pandemic. At the San Juan de Dios General Hospital, the emergency surgery service treated patients with this disease for 4 months, in charge of surgical personnel, infectology residents and emergency medicine support the Internal Medicine specialty; showing the need for the intervention of all specialties in the care of the pandemic. METHODOLOGY: Cross-sectional descriptive study on the experience of the Emergency Service in caring for patients with COVID-19 disease from July to October 2020. Results: A total of 178 patients with COVID-19 were treated, with a male predominance of 57%, aged from 16 to 91 with an average of 65 years. In addition to COVID-19 disease, 70 patients were hypertensive, 69 diabetics, and 13 with kidney disease. 169 cases had SARS-COV2 pneumonia, 2 with shock and multiple organ failure. Mortality was 13 cases (7%). Because most of the cases required oxygen and were moderate, severe and severe, drug treatment was used with high frequency. The drug that was used the most was heparins in all cases, most of them low molecular weight. 86% received steroid therapy, 50% with monoclonal antibodies, especially tocilizumab. Antibiotics were used frequently, the majority (32%) received Ceftriaxone. 6 cases received remdesivir (3%). CONCLUSIONS: The COVID-19 pandemic has demanded that doctors from all specialties join the response and fight against this disease. This study shows that all specialties have the capacity to handle patients with COVID-19 since internal medicine and related services have exceeded their capacity to care (AU)

ABSTRACT

Las características psíquico-motoras y físicas presentes en los pacientes con síndrome de Down le confieren una mayor susceptibilidad para sufrir lesiones traumáticas en la cavidad bucal, que causan afectaciones desde el punto de vista funcional, estético y psíquico. La atención estomatológica a estos pacientes debe ser diferenciada, por cuanto requieren técnicas innovadoras, preparación, agilidad y destreza por parte del estomatólogo, quien además debe integrarse a la familia y relacionarse con ellos desde edades tempranas. Por tales razones se presenta el caso de un paciente de 14 años de edad con síndrome de Down y una fractura no complicada de corona, en el que el tratamiento estético y restaurador logró desarrollarse de manera convencional.


The physical and psycho-motor characteristics present in Down syndrome patients provides a higher possibility to suffer from traumatic lesions in the oral cavity which cause affections from the functional, aesthetic and psychic point of views. Dental care to these patients should be differentiated since they require innovative techniques, training, agility and ability on the dentist part. This professional, in addition, should integrate to the family and be in contact with them since early ages. For these reasons a 14 year old Down syndrome patient is presented and a non-complicated crown fracture in which aesthetic and rehabilitating treatment could be conventionally developed.

Background: The decline of estrogen levels increases cardiovascular risk in women. Platelets express estrogen receptors and 17β-estradiol- (E2) can produce a protective effect on thrombus formation. The hydroxylation of cholesterol generates several sterols and 27-hydroxycholesterol (27HC) predominates in circulation. Aim: To evaluate the effect of 27HC as an endogenous antagonist of the anti-aggregating properties of E2 in platelets of postmenopausal women. Material and Methods: Platelet function of postmenopausal women was evaluated ex-vivo. Platelets pre-incubated with 27HC in the presence or absence of E2, were stimulated with collagen. Aggregation was evaluated using turbidimetry using a Chrono-log aggregometer. Results: Collagen-stimulated platelet aggregation was significantly inhibited by E2. The inhibitory effect of E2 on collagen-stimulated platelet aggregation was significantly reversed in the presence of 27HC. Conclusions: The suppressive effect of E2 on platelet aggregation is inhibited by 27HC, which could contribute to increase cardiovascular risk in postmenopausal women.

ABSTRACT

Background: Accumulated exposure to high levels of estrogen is associated with an increased incidence of breast cancer. Thus, factors such as early puberty, late menopause and hormone replacement therapy are considered to be risk factors, whereas early childbirth, breastfeeding and puberty at a later age are known to consistently decrease the lifetime breast cancer risk. Epidemiological studies suggest that consumption of isoflavones correlates with a lower incidence of breast cancer. Data from human intervention studies show that the effects of isoflavones on early breast cancer markers differ between pre- and post-menopausal women. The reports from experimental animals (rats and mice) on mammary tumors are variable. These results taken together with heterogeneous outcomes of human interventions, have led to a controversy surrounding the intake of isoflavones to reduce breast cancer risk. This review summarizes recent studies and analyzes factors that could explain the variability of results. In mammary tissue, from the cellular endocrine viewpoint, we analyze the effect of isoflavones on the estrogen receptor and their capacity to act as agonists or antagonists. On the issue of puberty timing, we analyze the mechanisms by which girls, but not boys, with higher prepuberal isoflavone intakes appear to enter puberty at a later age.
