ABSTRACT

Objective:To explore the efficacy of Tuina combined hot compress in the treatment of lumbar disc herniation (LDH).Methods:A total of 90 patients with LDH low back and leg pain in Shijiazhuang Traditional Chinese Medicine Hospital from January 2019 to June 2020 were divided into two groups, 45 in each group. The control group was treated with Aceclofenac Sustained-release Tablets and Mecobalamin, and the observation group was treated with Tuina combined hot compress. Both groups were treated for 1 month and followed up 6 months. The Visual Analogue Scale (VAS) was used to evaluate the degree of pain. The Oswestry Dysfunction Index (ODI) was used to evaluate the recovery of lumbar function, and the improvement of daily activities of patients after treatment was evaluated. The conduction velocity of tibial nerve motor conduction velocity (TNMCV) and common peroneal nerve motor conduction velocity (CPNMCV) were measured by electromyography. The clinical effect was evaluated.Results:After 6 months of follow-up, the response rate of the observation group was 51.1% (23/45) and that of the control group was 28.9% (13/45) ( χ2=3.98, P=0.046). The VAS score of the observation group was significantly lower than that of the control group ( t=11.26, P<0.01). The ODI scores at 1 month after treatment and 6 months after follow-up were significantly lower than those in the control group ( t values were 7.19, 18.30, respectively, all Ps<0.001). The observation group 1 month after CPNMCV treatment and 6 months of follow-up ( t=2.08, 1.99), 1 month after TNMCV treatment and 6 months of follow-up ( t values were 2.46, 2.12, respectively) were significantly higher than those in the control group ( P<0.05). Conclusion:Tuina combined with hot compress can improve the nerve conduction velocity of lower limbs, relieve the symptoms of low back and leg pain in patients after LDH operation.

ABSTRACT

Objective@#To investigate the clinical effect of different surgical methods in the treatment of diabetic patients with hard nuclear cataract.@*Methods@#From May 2016 to May 2018, 104 patients with diabetic hard core cataract were selected in Hangzhou Dajiangdong Hospital.According to the surgical methods, 53 cases were divided into small incision group and 51 cases in ultrasound group.The small incision group was treated with small incision cataract extraction, and the ultrasound group was treated by phacoemulsification.The changes of visual acuity and intraocular pressure preoperation and postoperative 1 week and 4 weeks were compared between the two groups.The levels of inflammatory factors preoperation and postoperative 3d, and the occurrence of postoperative complications were compared.@*Results@#The visual acuity of the small incision group at 1 week[(0.49±0.12)] and 4 weeks[(0.67±0.16)] after operation was higher than that before operation[(0.14±0.05)](t=19.600, P=0.000, t=23.018, P=0.000), and the visual acuity at 4 weeks after operation was higher than that at 1 week after operation (t=6.552, P=0.000). The visual acuity of the ultrasound group at 1 week[(0.46±0.15)] and 4 weeks[(0.64±0.14)] after operation was higher than that before operation [(0.15±0.07)](t=19.600, P=0.000, t=23.018, P=0.000), and the visual acuity at 4 weeks after surgery was increased compared with that at1 week after operation (t=6.329, P=0.000). The intraocular pressure of the small incision group at 1 week[(29.83±2.65)mmHg] and 4 weeks [(15.47±3.38)mmHg] after operation was lower than that of the ultrasonic group at 1 week [(35.39±3.24)mmHg] and 4 weeks after operation [(21.97±2.06)mmHg](t=9.596, P=0.000, t=11.787, P=0.000). In the small incision group, 3d after operation, the serum TNF-α [(52.41±6.28)μg/L]], IL-1β [(24.64±4.15)μg/L] and IL-6 [(34.12±5.32) μg/L] levels were lower than those in the ultrasound group[TNF-α (78.93±10.42)μg/L, IL-1β (38.92±4.86)μg/L and IL-6 (56.39±7.19)μg/L] (t=15.789, P=0.000, t=16.135, P=0.000, t=18.003, P=0.000). The incidence rate of postoperative complications in the small incision group [13.21%(7/53)] was lower than that in the ultrasound group [39.22%(20/51)] (χ2=9.146, P=0.000).@*Conclusion@#The effect of small incision cataract extraction in the treatment of patients with diabetic hard nuclear cataract is better than phacoemulsification, and it causes small inflammatory reaction and less complications.It is worthy of clinical reference.

ABSTRACT

OBJECTIVE:To establish the method for rapid determination of ginsenoside Rg1,Re,Rb1 in Panax quinquefolius crude slices.METHODS:HPLC method was adopted to determine the total contents of ginsenoside Rg1,Re,Rb1 (as reference value).NIRS combined PLS algorithm were adopted to establish total quantitative correction model of ginsenoside Rg1,Re,Rb1.According to the reference,62 samples were collected.The spectrum was pretreated with multivariate scattering correction method combined with first order derivative method.The optimal ranges of wave band for ginsenoside Rg1,Re,Rb1 were 7 664.23-5 236.05 cm-1.RESULTS:Methodology validation for total content determination of ginsenoside Rg1,Re,Rb1 was in line with the requirements.For total quantitative correction model of ginsenoside Rg1,Re,Rb1,related correction set coefficient was 0.991 03,corrected mean square deviation 0.010 26.CONCLUSIONS:The method is rapid,accurate,simple and free of contamination.It can be used for rapid determination of ginsenoside Rg1,Re,Rb1 in P quinquefolius crude slices.

ABSTRACT

Objective To explore the clinicopathological features of osteofibrous dysplasia (OFD). Methods All the clinicopathological information of 17 cases of OFD were reviewed,including sex,age,course and radiography or histopathology features. The expression of Cytokeratin(CK) was examined by SP. Results Age of the patients ranged from 2-15 years, and the course of the lesion ranged from 1 month to 7 years. The mean age and the mean course was 7.2 years and 2.1 years respectively. Male to female ratio was 1∶1.4. The lesions all occured in tibia, presentating with localized masses. Only one invloved with both tibia and fibula. Radiographs showed there were cystic lesions in the thickened anterior cortex of the tibia with light marginal sclerosis. Histopathology showed a zonal architecture. Abundant osteoblasts in the vicinity of the trabeculae. Only two cases were positive for CK(11.8%). 8 of 10(80%) patients recurred. Conclusion OFD is predominately involved with the tibia in children. The history is long. It has a characteristic histopathologic image and clinical progress. Its a independent entity, not the same of adamantinoma.
