ABSTRACT

@#AIM: To evaluate the efficacy and safety of preoperative intravitreal injection of ranibizumab(IVR)and panretinal photocoagulation(PRP)combined with conventional trabeculectomy without drainage device in patients with neovascular glaucoma(NVG).<p>METHODS: The charts of 27 patients(27 eyes)who presented with NVG in our hospital between August 2015 and November 2018 were reviewed. All eyes were treated using the standard protocol of IVR accompanied by PRP, followed by conventional trabeculectomy without drainage device. The main outcomes were intraocular pressure(IOP)control, change in best-corrected visual acuity(BCVA), and surgical complications.<p>RESULTS: The postoperative follow-up periods for all patients were at least 18(mean 21.7±4.3)mo. The IOP significantly decreased from 45.7±5.1 mmHg(baseline)to 18.4±3.6 mmHg(last visit), and BCVA(LogMAR)improved from 2.42±0.68(baseline)to 1.77±0.93(last visit). The success rate was higher in eyes with peripheral anterior synechiae(PAS)≤50% than in those with PAS >50% after 3mo postoperatively. The main surgical complications were hyphema(11.1%)and hypotony with a shallow anterior chamber(14.8%)during the early postoperative stage, along with encapsulated bleb(37.0%)during the mid-late postoperative stage. Serious intraoperative or postoperative complications were not observed.<p>CONCLUSION: IVR and PRP combined with conventional trabeculectomy without drainage device is a safe and effective way in NVG treatment, especially for eyes with PAS ≤50%. Preoperative IVR and PRP appear to improve the success rate.

ABSTRACT

OBJECTIVE@#To study the clinical efficacy and complications of Endobutton titanium plate and clavicle hook plate in the treatment of acromioclavicular dislocation.@*METHODS@#Total 48 patients with Rockwood Ⅲ to Ⅴ acromioclavicular joint dislocation from March 2015 to May 2019 were retrospectively divided into two groups according to different surgical methods. Among the patients, 23 patients were treated with Endobutton loop titanium plate fixation (observation group), including 15 males and 8 females, ranging in age from 23 to 59 years old, with an average of(36.2±8.1) years old;Rockwood type Ⅲ in 6 cases, type Ⅳ in 11 cases and type Ⅴ in 6 cases. Twenty-five patients were treated with clavicular hook plate(control group), including 17 males and 8 females, ranging in age from 22 to 54 years old, with an average of (34.7±6.4) years old; Rockwood type Ⅲ in 6 cases, type Ⅳ in 14 cases and type Ⅴ in 5 cases. The operation time, intraoperative bleeding, hospitalization time, visual analogue scale (VAS) of pain, Constant-Murley score of shoulder function and postoperative complications were compared between the two groups.@*RESULTS@#All the patients were followed up, and the duration ranged from 24 to 51 months, with a mean of (30.5±6.5) months. The amountof bleeding and hospitalization time in the observation group were (71.9±4.0) ml and(8.2±1.6) d respectively;and those in the control group were (97.6±13.4) ml and (12.8±1.2) d respectively. There was significant difference between the two groups (@*CONCLUSION@#Compared with clavicular hook plate internal fixation in the treatment of acromioclavicular joint dislocation, Endobutton loop titanium plate internal fixation technology has the advantages of less surgical bleeding, shorter hospitalization time, less postoperative pain, good recovery of shoulder joint function and less complications.

ABSTRACT

AIM: To study the influence of intravitreal injection of bevacizumab combined with photon dynamic treatment ( PDT ) in the treatment of central serous choroiretinopathy (CSC). METHODS:We selected 82 CSC patients treated in the hospital between February 2012 and April 2015, who were diagnosed by optical coherence tomography (OCT) and fundus fluorescein angiography (FFA) and all got the disease monocularly, and divided them into two groups called control group (n=35) and research group (n=47) by different methods, the patients of control group were treated by PDT method, while the patients of research group were treated by vitreous bevacizumab injection combined with PDT. Then compare the number of treatment times, the rate of flat pigment epithelial detachment(PED), macular subretinal fluid absorption, OCT indexes such as volume of the macular central hub, average thickness of central macular with diameter of 6mm and thickness of central macular region below 1mm range, the difference of corrected vision and clinical efficacy of two groups before and after the treatment. RESULTS:There were 32 eyes which were treated once, and 3 eyes which were treated twice in the control group, the average treatment times was 1.25士0.20; however, there were 9 eyes which were treated three times,10 eyes twice,28 eyes once, the average treatment number was 1.48士0.22, the treatment times of two groups had no statistical difference (t= 1.209, P>0.05). On the other hand,13 eyes' PED (37%) was flat in the control group,22 eyes (63%) which macular retinal subepithelial serous absorption;30 eyes' PED (64%) was flat in the research group,17 eyes (36%) which macular retinal subepithelial serous absorption. The rate of flat PED and macular retinal subepithelial serous absorption of two groups had obvious statistical difference (P<0.05). The OCT indexes such as volume of the macular central hub, average thickness of central macular with diameter of 6mm and thickness of central macular region bellow 1mm of two groups had no statistical difference before the treatment (P>0.05), in contrast these OCT indexes were all much lower than control group after the treatment, and the results had obvious statistical difference (P < 0. 05). Besides,these OCT indexes after treatment were all much lower than that of two groups before treatment respectively,and it also had obvious statistical difference (P< 0. 05). After 1 - month's treatment, the LogMAR corrected vision of two groups had no statistical difference (P>0.05). While the LogMAR corrected vision of research groups had statistical difference with control group after 3mo and 6mo (P<0.05). There were 6 invalid cases, 15 improvement cases, 8 excellence cases and 6 healing cases in the control group; the total effective rate of control group was 83%. While there were 3 invalid cases, 8 improvement cases, 20 excellence cases, 16 healing cases in the research group, the total effective rate of research group was 94%. The effective rate of two groups had obvious statistical difference (P<0.05). CONCLUSION: In the treatment of chronic CSC with PED patients, the method of vitreous bevacizumab injection combined with low dose PDT has better effect on PED flat, promoting macular subretinal serous absorption, reducing macular thickness and improving visual acuity.

ABSTRACT

PIWI-interacting RNAs (piRNAs) are a class of small non-coding RNAs of 24-32 nucleotides (nt) in length, which were first found in 2006. The piRNAs in germ cells interact with reproduction-specific PIWI proteins and play an important role in the development of the reproductive system. In the reproductive system of the male mouse, piRNAs arise from long single stranded precursor transcripts expressed in discrete genomic regions. The productions of piRNAs are termed primary piRNA biogenesis and secondary piRNA biogenesis. Mature piRNAs can silence the genome instability threat posed by mobile genetic elements called transposons. Furthermore, piRNAs regulate the expression of mRNAs transcriptionally and post-transcriptionally in spermiogenesis. This review summarizes the biogenesis and key functions of piRNAs in the reproductive system of male mice.

ABSTRACT

Spermatogenesis is a unique process of cell differentiation, which involves the regulation of a series of complicated post-transcriptional expressions. MicroRNAs (miRNAs) are a class of none-coding RNAs that play important roles in regulating post-transcriptional gene silencing. MiRNAs are expressed in a cell-specific manner in spermatogenesis and participate in the maturation and differentiation of male germ cells. The specifically altered seminal plasma miRNA is closely related with spermatogenic dysfunction and therefore can be used as a novel biomarker for the diagnosis of male infertility. A deeper insight into these specific miRNAs may point a new direction in the studies of the molecular mechanisms of spermatogenesis and spermatogenic dysfunction.

ABSTRACT

<p><b>BACKGROUND</b>Minimally invasive video-assisted thyroidectomy (MIVAT) has received increasing attention for malignant thyroid diseases. The aim of this study was to compare the outcomes of MIVAT with conventional open thyroidectomy (CT) for papillary thyroid microcarcinoma (PTMC).</p><p><b>METHODS</b>Thirty-one patients were treated with MIVAT and 37 with CT. Their pathological characteristics, surgical complications, 5-year postoperative thyroglobulin (TG) and ultrasonic results were followed up.</p><p><b>RESULTS</b>All the patients took levothyroxine for suppressing thyroid stimulating hormone (TSH) after surgery, and were followed up with measurement of serum TG and neck ultrasonography at intervals of 6 or 12 months. There was no statistically significant difference between the CT and MIVAT groups for sex ratio, operation time, positive lymph nodes, complications and prognosis, but the MIVAT group had better cosmetic results.</p><p><b>CONCLUSIONS</b>MIVAT did not differ significantly from CT for PTMC after 5 years follow-up, but it did have better cosmetic results. MIVAT is a safe and valid surgical technique for selected cases.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To compare the modified and conventional methods in orotracheal intubation of GlideScope videolaryngoscope.</p><p><b>METHODS</b>Sixty patients aged 21-53 years with ASA physical status I-II were scheduled for elective abdominal operation under general anesthesia. After routine anesthesia induction orotracheal intubation was performed with GlideScope videolaryngoscope; the patients were randomly divided into two groups (n=30 in each): Group M received modified orotracheal intubation and Group C received conventional orotracheal intubation. Noninvasive blood pressure and heart rate were recorded before and after anesthesia induction, at intubation, 1 and 3 min after intubation.</p><p><b>RESULTS</b>The time of intubation procedure was significantly longer in Group C than in Group M. BP and HR significantly decreased after intubation, there were no differences between two groups.</p><p><b>CONCLUSION</b>The modified method can improve the readiness of the procedure but it shows no advantages in prevention of adverse hemodynamic responses during the orotracheal intubetion of videolaryngoscopy.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To set up a stable primary culture system of Leydig cells with higher purity.</p><p><b>METHODS</b>We separated Leydig cells from other testicular cells, such as Sertoli and germ cells, by enzymatic digestion in combination with Percoll density gradient centrifugation and identified Leydig cells by 3beta-HSD staining.</p><p><b>RESULTS</b>The purity achieved by this method was above 95% and the total number of Leydig cells obtained from one testicle was about 1 x 10(6). The cytoplasm of Leydig cells was stained in deep blue by 3beta-HSD staining, and these cells possessed testosterone-secreting capability.</p><p><b>CONCLUSION</b>Leydig cells can be separated by enzymatic digestion combined with Percoll density gradient centrifugation, and 3beta-HSD staining to identify Leydig cells is simple and feasible with high purity.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effect of nonylphenol (NP) on testosterone secretion of Leydig cells.</p><p><b>METHODS</b>A primary culture system of Leydig cells was set up, followed by identification of Leydig cells with 3beta-HSD staining. After treatment with different concentrations of NP, testosterone secretion was detected and morphological examination was performed.</p><p><b>RESULTS</b>Following treatment with NP, morphologic changes of Leydig cells were detected, with decreased cell density at high doses of NP. Testosterone increased at lower concentrations of NP while decreased at high concentrations of NP.</p><p><b>CONCLUSION</b>Lower doses of NP can stimulate the secretion of testosterone, but increased exposure to NP will inhibit testosterone secretion of Leydig cells. Besides, high concentrations of NP can cause death of Leydig cells in vitro.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To detect the toxic effects of pentachlorophenol (PCP) on cultured rat Sertoli cells.</p><p><b>METHODS</b>The viability of Sertoli cells was detected and morphological examination was performed, followed by flow cytometric assay to evaluate the toxic effect of PCP on rat Sertoli cells.</p><p><b>RESULTS</b>MTT assay showed that PCP induced a concentration- and time-dependent decrease in Sertoli cell viability. Flow cytometric assay revealed that the number of dead Sertoli cells grew along with increased exposure to PCP.</p><p><b>CONCLUSION</b>PCP, with obvious cytotoxic effects, can cause necrosis of Sertoli cells in vitro.</p>

ABSTRACT

The research on endocrine disrupters has become one of the key directions in the field of environmental medical science. Nonylphenol ethoxylates (NPEs) are widely used in industry and family washing as one of the non-ionic surfactants. NPEs can be degraded biologically to nonylphenol (NP), whose estrogenic effects or toxicity may do harm to the reproductive systems of male fish, amphibians and mammals. Thus it is an urgent affair to develop perfect assays, evaluate the harm of these chemicals to human beings, find the exact mechanism of action and restrict the use of NPEs.
