ABSTRACT

AIM: To investigate the effects of agkis-trodon halys venom anti-tumor component (AHVAC-) on the biological behavior of gastric cancer MKN-28 cells. METHODS: Gastric cancer MKN-28 cells were treated with the experimental concentrations (5, 10, 15 μg/mL) of AHAVC- for 24 h. Cell proliferation and toxicity assay (cell counting kit-8, CCK-8) was used to detect the inhibition rates of the cells in different concentrations of AHVAC-. The migration ability of the cells was evaluated by wound-healing and Transwell assay. The apoptosis were observed by laser confocal microscopy with annexin V-mCherry/DAPI double staining, and the apoptosis rates were analyzed by flow cytometry with annexin V-FITC/PI double fluorescence staining. The protein level of Caspease-3 was determined by Western blot. RESULTS: Compared with normal control group, the results of AHVAC- concentration groups showed that with the increase of AHVAC- concentration, the proliferative activity of MN-28 cells decreased gradually (P<0.01), the cell migration ability decreased gradually (P<0.01), and the cell apoptosis rate increased (P<0.05). The expression of apoptosis-related protein Caspease-3 was up-regulated (P<0.01). CONCLUSION: AHVAC- inhibits proliferation and migration of gastric cancer MSN-28 cells and induces apoptosis.

ABSTRACT

Objective:To analyze the prognostic factor of arteriovenous fistula (AVF) maturation and provide the theoretical basis for the hemodialysis patients' AVF surgery.Methods:Retrospective investigation was conducted on patients that underwent AVF surgery in Wuhan Central Hospital from January 2017 to June 2019 as study subjects to investigate the prognostic factor of AVF maturation. The general information, hematological indicators, preoperative and postoperative ultrasound data of patients were recorded. Patients were divided into mature group and immature group according to the physical examination status, functional status and postoperative color doppler ultrasound data after AVF surgery, and the differences between the two groups were compared. Logistic regression analysis was used to analyze the prognostic factors of AVF maturation, and receiver operating characteristic (ROC) curve was drawn to evaluate the predictive value of each factor for AVF maturation.Results:A total of 164 patients were included, including 110 patients in the mature group and 54 patients in the immature group. There were statistically significant differences between the two groups in surgical vein diameter, surgical artery diameter, brachial artery flow, cardiac ejection fractions, systolic blood pressure, diastolic blood pressure and history of diabetes (all P<0.05), while there were no statistically significant differences in other indicators such as gender, age, etc. Logistic regression analysis showed that preoperative surgical artery diameter (every increase by 0.1 mm, OR=1.402, 95% CI 1.159-1.697, P=0.001), surgical vein diameter (every increase by 0.1 mm, OR=1.341, 95% CI 1.176-1.528, P<0.001), cardiac ejection fraction (every increase by 5%, OR=1.184, 95% CI 1.087-1.289, P<0.001), systolic blood pressure (every increase by 10 mmHg, OR=1.407, 95% CI 1.103-1.796, P=0.006), brachial artery flow (every increase by 10 ml/min, OR=1.047, 95% CI 1.004-1.091, P=0.034) and history of diabetes ( OR=0.271, 95% CI 0.103-0.715, P=0.008) were the independent influencing factors for AVF maturation. ROC curve showed that preoperative surgical artery diameter ( AUC=0.728, 95% CI 0.643-0.839, P<0.001), surgical vein diameter ( AUC=0.762, 95% CI 0.686-0.839, P<0.001), cardiac ejection fraction ( AUC=0.711, 95% CI 0.626-0.796, P<0.001) were the important indicators for predicting AVF maturation. Conclusions:Preoperative surgical artery diameter, surgical vein diameter, cardiac ejection fraction and diabetes mellitus are the prognostic factors of AVF maturation. In preoperative vascular assessment, attention should be paid to the diameter of target arteries and veins, as well as to the cardiac function and diabetes mellitus of patients.

ABSTRACT

Objective To explore the feasibility of extracorporeal membrane oxygenation (ECMO) in protecting the donor liver in donation after citizen's death. Methods Clinical data of 16 donors and recipients undergoing liver transplantation using ECMO to protect the donor liver were retrospectively analyzed. The effect of ECMO on different indicators of the donors was evaluated. The liver function and clinical prognosis of the recipients after liver transplantation were observed. Results Compared with the time before ECMO, the heart rate, total bilirubin (TB), alanine transaminase (ALT) and aspartate transaminase (AST) of the donors after ECMO were significantly reduced, whereas the systolic blood pressure, diastolic blood pressure and partial pressure of arterial oxygen (PaO2) were remarkably increased (all P < 0.05). The liver function of the recipients was properly recovered after liver transplantation, and gradually restored normal at postoperative 7 to 28 d. Postoperative complications occurred in 3 recipients, including delayed liver function recovery in 1 case, biliary tract stenosis in 1 case and portal vein thrombosis in 1 case. Among them, the patient with portal vein thrombosis died after secondary operation, and the other 2 patients were recovered and discharged after symptomatic treatment. Conclusions The hemodynamics, liver function and other indicators of donors from donation after citizen's death are significantly improved after ECMO, and the liver function of the recipients also recover well.

ABSTRACT

To investigate the effects of different wavelet filters on correlation and diagnostic performance of radiomics features.
 Methods: A total of 143 colorectal cancer (CRC) patients (64 positive in lymph node metastasis and 79 negative) with contrast-enhanced CT examination were recruited. After labeling the tumor area by experienced radiologists, radiomics wavelets features based on 48 different wavelets were extracted using in-house software coded by Matlab. The correlation coefficients of the features with same names between different wavelets were calculated and got the distribution of high-correlation features between each wavelet. The least absolute shrinkage and selection operator (LASSO) was used to build signatures between lymph node metastasis and wavelet features data set based on different wavelets. The numbers of features in signatures and diagnostic performance were compared using Delong's test.
 Results: With the difference of wavelet order increased, the number of high-correlation features between two wavelets decreased. Some features were prone to high correlation between different wavelets. When building radiomics signature based on single wavelet, signatures built from 'rbio2.2', 'sym7' and 'db7' did well in predicting lymph node metastasis. The signature based on Daubechies wavelet feature set had the highest performance in predicting lymph node metastasis, while the signature from Biorthogonal wavelet features was worst. Improvement was significant in diagnostic performance after excluding the high-correlation features in the whole features set (P=0.004).
 Conclusion: In order to reduce the data redundancy of features, it is recommended to select wavelets with large differences in wavelet orders when calculating radiomics wavelet features. It is necessary to remove high correlation features for improving the diagnostic performance of radiomics signature.

ABSTRACT

To develop and validate a fat-suppressed (T2 weighted-magnetic resonance imaging, T2W-MRI) based radiomics signature to preoperatively evaluate the histologic grade (grade I/II VS. grade III) of invasive breast cancer.
 Methods: A total of 202 patients with MRI examination and pathologically confirmed invasive breast cancer from June 2011 to February 2017 were retrospectively enrolled. After retrieving fat-suppressed T2W images and tumor segmentation, radiomics features were extracted and valuable features were selected to build a radiomic signature with the least absolute shrinkage and selection operator (LASSO) method. Mann-Whitney U test was used to explore the correlation between radiomics signature and histologic grade. Receiver operating characteristics (ROC) curve was applied to determine the discriminative performance of the radiomics signature [area under curre (AUC), sensitivity, specificity, and accuracy]. An independent validation dataset was used to confirm the discriminatory power of radiomics signature. 
 Results: Eight radiomics features were selected to build a radiomics signature, which showed good performance for preoperatively evaluating histologic grade of invasive breast cancer, with an AUC of 0.802 (95% CI 0.729 to 0.875), sensitivity of 78.7%, specificity of 70.3% and accuracy of 73.7% in training dataset and AUC of 0.812 (95% CI 0.686 to 0.938), sensitivity of 80.0%, specificity of 73.3% and accuracy of 76.0% in the validation dataset.
 Conclusion: The fat-suppressed T2W-MRI based radiomics signature can be used to preoperatively evaluate the histologic grade of invasive breast cancer, which may assist clinical decision-maker.

ABSTRACT

Objective To explore the clinical application of renal replacement therapy in renal transplan-tation from donation after citizen's death(DCD).Methods A total of 41 cases of the patients with renal replace-ment therapy after renal transplantation from DCD from January 2013 to December 2016 were involved,of which 14 cases received peritoneal dialysis,21 intermittent hemodialysis(IHD)and 6 continuous renal replacement ther-apy(CRRT).The therapeutic effect and complications of three renal replacement therapies were retrospectively ana-lyzed. Results After dialysis treatment,the concentration of blood BUN,Crea,and potassium was significantly lower than that before the treatment(P<0.05);the difference of Crea and BUN before and after the treatment in IHD and CRRT group was higher than that in peritoneal dialysis group(P<0.05)but there was no statistically sig-nificant difference between IHD and CRRT group(P > 0. 05). Conclusion The renal replacement after kidney transplantation from DCD should be based on the patient's condition,which is the key to protect their kidneys even to save patients'lives.

ABSTRACT

Purpose Lymph-vascular invasion (LVI) is a risk factor for the prognosis of colorectal cancer, and it is of great value to evaluate the status of lymphatic vessels before treatment. This study aims to predict colorectal cancer LVI preoperatively based on radiomics. Materials and Methods Radiomics features were extracted from preoperative CT images of colorectal cancer retrospectively collected and radiomics labels were constructed. The predictive efficacy of radiomics labels were assessed and internally verified. Joint predictive factors were established by combining clinical factors with independent predictive efficacy and radiomics labels, and their predictive efficacy was evaluated. Results Radiomics labels consisted of 58 radiomics features were correlated with LVI status (P<0.0001)with the former showing good discrimination ability[C-index 0.719,95% CI:0.715-0.723]and classification ability(sensitivity 0.726, specificity 0.628) with internal validation (C-index 0.720). Joint predictive factors containing radiomics labels and carcino-embryonic antigen further enhanced the predictability of radiomics labels (C-index 0.751, sensitivity 0.788, specificity 0.667). Conclusion The radiomics labels built in this study can provide individualized prediction of LVI status of patients with colorectal cancer before surgery. Joint predictive factors in combination with clinical risk factors further improved predictive efficacy.

ABSTRACT

Objective To explore the influence of psychological intervention on the immune function of patients with nasopharyngeal carcinoma. Methods Forty nasopharyngeal carcinoma patients with concomitant radiochemotherapy were divided into intervention group and control group by random digits table method with 20 cases each.Among concomitant radiochemotherapy nasopharyngeal carcinoma patients, intervention group were treated with comprehensive psychological behavior, and control group received only concurrent radiotherapy and chemotherapy. Both groups were investigated by Self-rating Anxiety Scale(SAS)and Self-rating Depression Scale(SDS).Natural killer cells activity were measured by flow cytometry. Results There were no significantly difference in score of SDS and SAS between two confirmed groups(P>0.05).After treatment,the score of SDS and SAS in intervention group were(45.26± 3.83), (42.57 ± 3.75) points, which were significantly lower than those of control group (52.68 ± 3.66), (51.16 ± 3.62) points, the difference was statistically significant(t =6.264, 7.370, P<0.05). Meanwhile, natural killer cell activity between two groups showed no significantly difference (P>0.05). However, the activity of natural killer cells in intervention group was 26.96±9.01,which was higher than that of control group 22.84 ± 7.19, there were statistically significant difference between two groups (t=2.120, P<0.05).There was a positive correlation between the value difference of SDS scores,SAS scores and natural killer cell activity between two groups (r=0.238, P=0.006; r=0.527, P=0.003). Conclusions Comprehensive psychological intervention can significantly not only improve the depression and anxiety of nasopharyngeal carcinoma patients,but also enhance the activity of natural killer cells,which help maintain the leukocyte level of patients.

OBJECTIVE:To establish the method for simultaneous determination of 6 residual organic solvents in Xingnaojing injection,such as methanol,ethanol,isopropanol,n-butanol,ethyl acetate and acetonitrile.METHODS:Headspace GC method was adopted.The determination was performed on DB-624 capillary column by temperature programming with the injector temperature of 200 ℃;flame ionization detector was adopted with the temperature of 250 ℃;carrier gas was nitrogen with flow rate of 25 mL/min and split ratio of 35 ∶ 1;headspace sampling size was 1 mL,and heating temperature of headspace sampling was 80 ℃;equilibrium time was 15 min.RESULTS:The linear ranges of methanol,ethanol,isopropanol,n-butanol,ethyl acetate and acetonitrile were 15.00-240.00 μg/mL (r =0.999 9),25.00-400.00 μg/mL (r =0.999 9),25.00-400.00 μg/mL (r =0.999 9),25.00-399.99 μg/mL(r=0.999 9),25.00-399.99 μg/mL(r=0.999 8) and 5.00-80.00 μg/mL(r=0.999 9).The LOQ were 5.98,3.94,2.05,2.13,1.39,1.24 μg/mL,and the LOD were 2.01,2.11,1.18,1.56,1.15,0.01 μg/mL,respectively.RSDs of precision tests were all less than 2.0%,stability and repetitive tests only ethyl acetate was detected,RSD<2.0%;the recoveries were 93.59%-99.02% (RSD=2.62%,n=6),92.42%-98.40% (RSD=2.43%,n=6),94.81%-104.64% (RSD=3.47 %,n=6),94.56%-106.73% (RSD=4.21%,n=6),97.04%-106.33%(RSD=3.50%,n=6)and 98.40%-107.97% (RSD=3.37%,n=6).CONCLUSIONS:The method is specific,rapid,simple and accurate,and can be used for simultaneous determination of 6 residual organic solvents in Xingnaojing injection.

ABSTRACT

Objective To develop and validate a CT-based radiomics predictive model for preoperative predicting the stage of non-small cell lung cancer (NSCLC). Methods In this retrospective study, 657 patients with histologically confirmed was collected from October 2007 to December 2014.The primary dataset consisted of patients with histologically confirmed NSCLC from October 2007 to April 2012, while independent validation was conducted from May 2012 to December 2014.All the patients underwent non-enhanced and contrast-enhanced CT images scan with a standard protocol. The pathological stage (PTNM) of patients with NSCLC were determined by the intraoperative and postoperative pathological findings,and were divided into early stage(Ⅰ,Ⅱstage)and advanced stage(Ⅲ,Ⅳstage).A list of radiomics features were extracted using the software Matlab 2014a and the corresponding radiomics signature was constructed. Multivariable logistic regression analysis was performed with radiomics signature and clinical variables for developing the prediction model. The model performance was assessed with respect to discrimination using the area under the curve (AUC) of receiver operating characteristic(ROC) analysis. Results The discrimination performance of radiomics signature yielded a AUC of 0.715[95% confidence interval (CI):0.709 to 0.721] in the primary dataset and a AUC of 0.724(95% CI:0.717 to 0.731) in the validation dataset. On multivariable logistic regression, radiomics signature, tumor diameter, carcinoembryonic antigen (CEA) level, and cytokeratin 19 fragment (CYFRA21-1) level were showed independently associated with the stage ( Ⅰ,Ⅱ stage vs. Ⅲ, Ⅳ stage) of NSCLC. The prediction model showed good discrimination in both primary dataset (AUC=0.787, 95%CI:0.781 to 0.793;sensitivity=73.4%, specificity=72.2% ,positive predictive value=0.707,negative predictive value=0.868) and independent validation dataset (AUC=0.777, 95% CI:0.771 to 0.783,sensitivity=91.3% ,specificity=67.3% ,positive predictive value=0.607, negative predictive value=0.946). Conclusion The radiomics predictive model, which integrated with the radiomics signature and clinical characteristics can be used as a promising and applicable adjunct approach for preoperatively predicting the clinical stage (Ⅰ,Ⅱ stage vs. Ⅲ,Ⅳ stage) of patients with NSCLC.

ABSTRACT

Objective We make a retrospective analysis , to compare to COPD group and discuss the risk factors and the clinical features in acute exacerbation in patients with ACOS to follow-up the exacerbating frequency after regular treatment in both two groups in one year. Methods There were 56 patients with ACOS and 80 patients with COPD from 2013 to 2015 in our hospital in 30%≤FEV1<80% in the stable phase. The common data of the enrolled patients included the age,sex,smoking, and allergic rhinitis or other allergic diseases in family. We analyzed laboratory index including PaO2,PaCO2,CRP,white blood cells,IgE of serums and compared the proportion of antibiotics,system used of glucocorticoid and noninvasive ventilation in hospitalization of acute exacerbation and followed up exacerbating frequency after using ICS united LABA/LAMA. Results The age and smoking index in the ACOS group were lower than the COPD group (P < 0.05). The allergic rhinitis or other familial allergic diseases,lower age of 60,the high IgE in serum were risk factor, in ACOS. In acute exacerbation, the PaCO2,IgE and WBC in serum were higher than that of the COPD group(P <0.05). The midian length of stay in hosipital was 12 days in the ACOS group and 8 days in the COPD group. The proportion of antibiotics,systemic administration of glucocorticoid and noninvasive ventilation in hospitalization of acute exacerbation in the ACOS group were higher than that of the COPD group (P < 0.05). The exacerbating frequency was decreased after using ICS united LABA/LAMA(1.2±0.6 vs 3.8±1.3,P < 0.05)in both ACOS and COPD groups. Conclusions The allergic diseases may participate in ACOS, in which it has familial tendency. In acute exacerbation, ACOS patients had even more inflammation and faster course than COPD patients. Using ICS united LABA/LAMA can reduce exacerbating frequency in ACOS.

Objective To analyze the epidemic characteristics of imported malaria in Liyang City,so as to provide the evi?dence for formulating appropriate control strategies and measures. Methods The malaria epidemic situation reports,case epi?demiological investigations and the follow?up materials of counterparts returned together with the cases were collected and ana?lyzed in Liyang City from 2007 to 2014. Results Totally 53 imported malaria cases were reported and they were all laboratory confirmed in Liyang City from 2007 to 2014. Forty ? nine cases(92.5%)were imported from African countries,37 cases (69.8%)were Plasmodium falciparum infection,and 37 cases(69.8%)had overseas incidence history. Forty cases(75.5%) were aged between 30 and 49 years,and 52 cases(98.1%)were male,42 cases(79.2%)were farmers. These cases were dis?tributed in all the 10 towns and there were no obvious seasonal variation for onset time. The median time from onset to seeing a doctor was 3 d,and the median time from seeing a doctor to being diagnosed was 0 d. The first visit health facilities by the pa?tients were various,and the diagnostic health facilities were all the medical institutions at the county level or above,and the standardized treatment rate was 100%. Conclusion During the last 5 consecutive years,there have been no local infected ma?laria cases in Liyang City,and the remarkable results have been achieved in malaria elimination. However,there are still many imported malaria cases,and the infected species are diverse. Therefore,the imported malaria is still the key of malaria preven?tion and control.

Objective:To discuss the effect and significance of reasonable medication review mode for intravenous medical advice on the platform of pharmacy intravenous admixture service( PIVAS) . Methods:The effect of prescription review interference in intrave-nous drugs on the platform of PIVAS was studied through analyzing irrational prescriptions before and after the performance of PIVAS for 25 departments in our hospital. Results:The real-time review and interference procedure for intravenous transfusion was established, which could significantly increase the percentage of reviewed medical advice and the correction rate of irrational medical advice, and re-duce the percentage of irrational medical advice. The medical advice review quality was also increased, which could guarantee the med-ication safety in patients. Conclusion:The real-time review and interference procedure for intravenous transfusion on the platform of PIVAS can significantly improve the rational medication level of intravenous drugs and avoid the hidden danger of irrational medical ad-vice to the maximum extent to ensure the medication safety in patients, which is also beneficial to the provision of medication service and new platform establishment for the development of clinical pharmacy in hospitals.

ABSTRACT

OBJECTIVE:To compare the clinical efficacy and safety of salmeterol-fluticasone alone versus fluticasone propio-nate combined with montelukast sodium in the treatment of children with moderate persistent asthma. METHODS:275 children with moderate persistent asthma were randomly divided into inhaled corticosteroids(ICS)+ long acting β2 receptor agonist(LABA) group (139 cases) and ICS+ leukotriene receptor antagonists (LTRA) group (136 cases). ICS+LABA group was given Salmeter-ol-fluticasone powder inhalant,1 inhalation,bid;ICS+LTRA was given Fluticasone propionate inhalation aerosol,bid+Montelu-kast sodium chewable tablet 5 mg,before bedtime,qd. Both groups received more than 12 weeks of treatment. Body indexes of 2 groups were observed after treatment,such as asthma control degree,asthma score,the percentage of peak expiratory flow(PEF) in estimated value,aberration rate of PEF,the levels of eosinophi cationic protein(ECP)and leukotrienes E4(LTE4)in peripheral blood,the times of taking short acting β2 receptor agonist (SABA) during treatment,asymptomatic days,compliance,the inci-dence of ADR,etc. RESULTS:Total effective rate of ICS+LABA group(86.33%)after 4 weeks treatment was higher than that of ICS+LTRA group(58.09%),with statistical significance(P0.05). The day and night asthma score of 2 groups decreased significantly and the percentage of PEF in estimated value increased significantly after treatment,there was statisti-cal significance compared to before treatment(P0.05). The levels of ECP and LTE4 decreased significantly in 2 groups after treatment,there was statistical signif-icance compared to before treatment(P0.05). CONCLUSIONS:Salmeterol-fluticasone alone versus fluticasone propionate combined with montelukast sodi-um in the treatment of children with moderate persistent asth-ma both have good therapeutic efficacy and safety,while thelatter one has stronger inhibition effect on ECP and LTE4,and to control asthma symptoms more rapidly.

ABSTRACT

Objective To investigate the effect of rosiglitazone on the epithelial-mesenchymal transition process of renal tubular epithelial cell induced by transforming growth factor-β(TGF-β). Methods The human renal tubular epithelial cell line HK-2 was cultured in vitro and treated with TGF-β at the presence or absence of rosiglitazone, and the morphological changes of HK-2 cells were observed. Then PPARγ,SMAD family number 2/ 3,E-cadherin and Vimentin were detected by western blot, and the mRNA expression of PPARγ, E-cadherin, Vimentin, zinc finger transcription factor Snail and Slug were measured by realtime quantitative PCR. Finally,dual luciferase reporter assay was performed to detect the effect of TGF-β and rosiglitazone on the tran-scription activity of E-cadherin promoter. Results TGF-β could induce the EMT process of HK-2, including morphological changes such as longer pseudopod and broader cell space. TGF-β also activated the SMAD2/ 3 signaling pathway, with expression changes of E-cadherin and Vimentin. Rosiglitazone reversed the morphological changes of HK-2 cells induced by TGF-β, with shor-ter pseudopod and narrower cell space. Rosiglitazone also inhibited the mRNA and protein expressions of Vimentin, and restored the mRNA and protein expression of E-cadherin by up-regulating PPARγ. The activity of E-cadherin promoter was enhanced under the treatment of rosiglitazone. Conclusion Rosiglitazone can antagonize the EMT process of renal tubular epithelial cell induced by TGF-β, through the activation of PPARγ and stimulating transcription and protein expression of E-cadherin.

ABSTRACT

BACKGROUND:We have successfuly prepared digital coraline hydroxyapatite artificial bone scaffold in previous experiments, and it has been confirmed that it has the necessary physical and chemical properties of bone tissue engineering scaffolds. OBJECTIVE: To evaluate the sensitization of digital coraline hydroxyapatite artificial bone scaffold. METHODS:A total of 32 guinea pigs were randomly divided into saline group (negative control group, n=8), 5% formaldehyde group (positive control group,n=8), experimental A group (the mass ratio of 3:1,n=8), and experimental B group (the mass ratio of 4:1,n=8). Sensitization test at the maximal dosage was performed according toBiological Evaluation of Medical Devices-Part 10: Tests for Irritation and Delayed-Type Hypersensitivity, including intracutaneous induction, local induction, and provocation. Patch was removed after 24 and 48 hours, and the skin response was classified according to Magnusson and Kligman criteria. Patch was removed after 48 hours, and the skin was performed with biopsy, stained with hematoxylin-eosin, and observed under optical microscope. RESULTS AND CONCLUSION: Sensitization response was not tested in the negative control group, experimental A group and experimental B group at 24 and 48 hours after patch removal; however, moderate erythema was observed in the positive control group. Optical microscope demonstrated that spongiosis, edema, diffuse or perivascular mononuclear infiltration was not observed, and only a smal number of basicytes were seen in the experimental A and B groups. These findings indicate that the digital coral hydroxyapatite artificial bone scaffolds, with the mass ratio of 3:1 and 4:1, are biologicaly safe for sensitization.

ABSTRACT

Objective To investigate the role of sitting-up-straight position in correction of peripherally inserted central catheter(PICC)heterotopia.Method Eighteen cases with PICC heterotopia were instructed to take the position of sitting up straight. Result All of the patients had the PICC heterotopia corrected.Conclusions The sitting-up-straight position can effectively correct the PICC catheter heterotopia.This method is simple and feasible and thus worthy of clinical application.

ABSTRACT

Objective To observe the effect of recombinant human tumor necrosis factor receptor Fc fusion Protein (rhTNFR:Fc) treatment on IgM-RF,IgG-RF,IgA-RF of patients with rheumatoid arthritis.Methods A randomized,active-comparator controlled,parallel group study were conducted.110 patients were enrolled and were randomly divided to the treatment group,in which patients were treated with twice weekly subcutaneous injection of rhTNFR:Fc (25 mg) (rhTNFR:Fc treatment group,n=55),and the MTX froup,in which MTX (the mean dosage was 15 mg/week) (MTX group,n=55) for 24 weeks.Blood routine,IgM-RF,IgG-RF,IgA-RF,and disease activity score 28 (DAS28) were monitored.Student's t-test was used for statistical analysis.Results The level of IgM-RF decreased significantly in the rhTNFR:Fc treatment group 24 week(29±16) U/ml later.However,the level of IgG-RF(145±20) U/ml,IgA-RF(153±34) U/ml increased significantly in the rhTNFR:Fc group,and the level of IgG-RF (62±14) U/ml,IgA-RF (66-±19) U/ml decreased significantly in the MTX group.Conclusion Although rhTNFR:Fc,is effective in treating the clinical symptoms of RA,it seems to affect RF producing-B cells either directly or indirectly.

ABSTRACT

Objective To study the correlation between blood lactic acid level and PCIS scroing system.To study the relationship between dynamic lactic acid monitoring indexes and prognosis of critical patients.Methods Differences of mortality,incidence of MODS and shock,PCIS score between two groups were compared.The patients were divided into 3 groups by PCIS score to compare pathogenetic conditions prognosis, then to study the were divided into death group and survival group by the hospital prognosis. Differences of their lactic indexes and other clinical indicators which reflected organ/system status were compared, prognostic-significant lactic indexes were selected by mu fti-variable logistic analysis. Results Patients with elevated lactic acid level had more incidence of shock and MODS, higher PCIS scores and higher mortality, the differences had statistical significance compared with the normal lactic acid group. There were significant linear correlation between ICU lactic acid level, peak lactic acid level and PCIS scores respectively. For the patients with elevated lactic acid level, peak lactic acid level, 12h lactic clearance had significant correlation with prognosis. Conclusion PCIS scoring system had a significant linear correlation with lactic acid level. For the patients with elevated lactic acid level,peak lactic acid level,12h lactic clearance were good indicators to evaluate patients' prognosis.
