ABSTRACT

Objective:To explore the effect of the eradication rate and adverse reactions of different courses of Bifidobacterium combined with quadruple therapy containing furazolidone and tetracycline for rescue eradication of Helicobacter pylori.Methods:From December 2016 to May 2019, 348 patients with chronic gastritis who failed to eradicate Hp for the first time were selected from the outpatient department of digestive medicine of Kailuan General Hospital affiliated to North China University of Technology.Group A, B and C were randomly divided by Excel, Group A(116 patients): 14-day quadruple therapy (Eprazole+ colloidal bismuth pectin+ furazolidone + tetracycline); group B(116 patients): Bifidobacterium was given 14 days on the basis of treatment in group A; group C(116 patients): 7 days Bifidobacterium was given in the second week of treatment in group A 4 weeks after the end of eradication treatment, 14 C urea breath test was reexamined, to compare the eradication rate and the incidence of adverse reactions.Results:The eradication rates were 80.2%, 84.5% and 82.3% respectively according to intention treatment.The eradication rates were 88.6%, 90.7% and 89.7%, respectively according to perprotocol.There was no significant difference between the three groups(according to intention to treat analysis: χ 2=0.755, P=0.685; according to the perprotocol analysis: χ 2=0.271, P=0.873). The incidence of adverse reactions in three groups was 16.4%, 6.3% and 7.3% respectively, the difference was statistically significant(χ 2=7.561, P=0.023). Group B was significantly lower than group A(χ 2=5.570, P=0.017), the difference was statistically significant, but there was no statistically significant difference between group C and group A(χ 2=4.362, P=0.037). Conclusion:After 14 days of Bifidobacterium tetrad live tablet assisted with tetrad therapy containing furazolidone and tetracycline, the adverse reactions could be significantly reduced, but Bifidobacterium could not significantly improve the eradication rate.

ABSTRACT

Objective To investigate the effects of fasting serum triglycerides (TG) levels at different baseline on the risk of new-onset acute pancreatitis (AP) in in-service and retired employees of Kailuan Group.Methods A total of 125 178 in-service and retired employees of Kailuan Group who received health check-ups from 2006 to 2009 and had no AP history but had complete TG data were prospectively enrolled.According to quantile level,the baseline serum fasting TG level of study subjects were divided into <1.01 mmol/L group (n=42 128),1.01 to 1.64 mmol/L group (n=41 711) and > 1.64 mmol/L group (n=41 339).The incidence of new-onset AP of these three groups was analyzed.The survival curve was plotted by Kaplan-Meier method.The cumulative incidence rate was calculated and tested by log-rank method.And multivariate Cox proportional hazards regression model was performed to calculate hazard ratios (HR) of baseline fasting serum TG level for AP.Results After followed up for (7.36±1.23) years,a total of 193 cases of AP occurred.The incidences of AP in <1.01 mmol/L group,1.01 to 1.64 mmol/L group and > 1.64 mmol/L group were 1.43 events/10 000 person-years,2.37 events/10 000 person-years and 2.49 events/10 000 person-years,respectively.The cumulative incidence rates of AP in <1.01 mmol/L group,1.01 to 1.64 mmol/L group and >1.64 mmol/L group were 0.10% (44/42 128),0.18% (73/41 711) and 0.18% (76/41 339),respectively,and the difference was statistically significant (x2 =9.998,P=0.007).The results of multivariate Cox proportional hazards regression model analysis indicated that the risk of AP increased in 1.01 to 1.64 mmol/L group and > 1.64 mmol/L group compared with that of <1.01 mmol/L group,HR and 95% confidence interval (CI) were 1.56 (1.07 to 2.29) and 1.57 (1.06 to 2.32),respectively.After excluded onset AP within one year,with a control group of <1.01 mmol/L group,the results of multivariate Cox proportional hazards regression model analysis indicated that the HR and 95%CI for AP of 1.01 to 1.64 mmol/L group and > 1.64 mmol/L group were 1.70 (1.11 to 2.58) and 1.69 (1.10 to 2.60),respectively.Conclusion Baseline fasting serum TG levels over 1.01 mmol/L may increase the risk of AP.

ABSTRACT

Objective To evaluate the clinical feature of malignant ameloblastoma. Methods A review of 25 cases of malignant ameloblastoma was presented with respect to age, sex, primary site and times of recurrence, site and time of metastasis, results of follow up, pathological type. Results Male patients outnumbered female patients by 1.5 to1. The median age at which the primary tumor was first treated was 30.5 years. The primary sites were mandible(17), maxilla(5), tibia(1), rib(1), ulna(1). The most metastases were found in lung(80%), with cervical lymph nodes and liver each representing 24% and 12% of the metastatic sites. Other sites of metastasis included brain(1), femur(1), thoracic spine(1), spleen(1), myocardial(1), mediastinum(1). The median times of primary recurrence was 2.5 times. The median survival time after metastasis was 2 years. There were four pathological types which were follicular type(4), plexiform type(2), acanthomatous type(1), granular type(1). Conclusins Multiple recurrences developed in most of the 25 patients. The time between diagnosis of primary tumor and appearance of metastasis in 25 patients was long. The most frequent sites of metastasis was lung and cervical lymph nodes. Early adequate resection of the primary tumor to avoid repeated surgical intervention is advocated to prevent tumor spread.

ABSTRACT

<p><b>OBJECTIVE</b>The aim of this study was to observe whether vasoactive intestinal polypeptide (VIP) participated in the attack of trigeminal neuralgia, and to understand further its pathogenetic mechanism.</p><p><b>METHODS</b>Sixteen patients were studied. During the attacks the venous blood was sampled from both the external jugular vein and cubital fossa vein ipsilateral to the pain, and after operations the venous blood from the external jugular vein was sampled again, the external jugular vein blood of eleven normal volunteers was collected as the control. Plasma levels of VIP were determined using sensitive radioimmunoassays.</p><p><b>RESULTS</b>During the attacks of trigeminal neuralgia the levels of VIP in the external jugular vein blood were significantly higher than that in the cubital fossa vein blood, postoperative external jugular vein blood and the external jugular vein blood of normal volunteers.</p><p><b>CONCLUSION</b>VIP indeed participates in the attack of trigeminal neuralgia in the local region, the role of VIP may be relating to enhancing the effects of substance P in local neurogenic inflammation.</p>

ABSTRACT

Objective: To summarize the experience of repairing or al and maxillofacial defects with radial forearm free flap. Methods: The clinical records of 110 cases received oral and maxillofacial reconstruction with radial forearm free flap were analyzed.Results: Forearm free flap healed well in 10 5 out of the 110 cases.with the overall success rate of 96.4%.Failure was in 4 c ases of cancer.One patient died from cerebral infarction 19 days after operation .Post operation infection did not result in the necrosis of the flaps because o f corresponding treatment in 5 cases.Conclusion: The radia l forearm free flap characterized by the anatomical consistency,easy fold, ideal thickness, flexibility and long pedicle, is feasible in the reconstruction of soft tissue defects in oral and maxillofacial region.

ABSTRACT

砄bjective: To observe whether substance P(SP), calcitonin gene related peptide(CGRP), vasoactive intestinal polypeptide(VIP),? endorphin(? EP) are related to the pathogenesis of trigeminal neuralgia. Methods: During the attacks of trigeminal neuralgia, SP,CGRP,VIP,? EP in plasma from both external jugular and cubital fossa vein blood ipsilateral to the pain were assessed by sensitive radioimmunoassay in 16 ptients and 10 health controls, after operations the venous blood from the external jugular vein of the patients was sampled again; the external jugular vein blood of normal volunteers was collected as control. SP like and CGRP like immunoreactive positive granules of pain and painless branches were labelled using ABC immunohistochemistry methods; quantitative analysis was performed using HPIAS 1000 system. Results: During the attacks of trigeminal neuralgia, the levels of SP,CGRP,VIP in the external jugular vein blood were significantly higher than those in the cubital fossa vein blood, postoperative external jugular vein blood and the external jugular vein blood of normal volunteers. The plasma levels of ? EP in patients with trigeminal neuralgia were significantly lower than that in normal volunteers.The number and area of SP like ,CGRP like immunoreactive positive granules in the pain branches was much more and larger than those in the painless branches . Conclusion: Abnormal synthesis and release of neuropeptides in patients may be related to the pathogenesis of trigeminal neuralgia.

Objective:To determine the effectiveness and validity of morphometric analysis made from dental panoramic tomograms(DPTs) for predicting mandibular bone mineral density(BMD) while taking the result of dual energy X-ray absorptiometry(DXA) as gold standard.Methods: Twenty healthy subjects(8 male,12 female) with intact dentition,aged from 1126 years old,were included in the present study.The BMD of each patient was measured at the lumbar spine(L1-L4) by DXA;the results were obtained based on the density of surface(g/cm2).Dental panoramic radiography was carried out using dental panoramic tomography(RTG230/ENR,made in Italy).Mandibular cortical thickness(MCT) and panoramic mandibular index(PMI) values were calculated and their relationship with DXA measurements were subjected to correlation analysis.The validity of MCT and PMI measurements were assessed by sensitivity and specificity.Results: We found mandibular BMD was positively correlated with MCT(r=0.268,P

Objective: To study the influence of occlusal force changes on apoptosis related gene Bcl-2 and Bax in rat periodontal ligament fibroblasts (PDLF). Furthermore, to explore the possible mechanism in remodeling of periodontal ligaments. Methods: Animal model of occlusal force changes was established by extracting the right first, second and third maxillary molars in male SD rats. Rats were sacrificed at 6,12 hours and 1, 2, 3, 5, 7, 14 and 28 days after teeth extraction(n=6), and their alveolar bone tissues were harvested. HE staining was used to observe the morphologic changes of alveolar bone tissues and immunohistochemistry were used to detect Bcl-2 and Bax expressions in PDLF. The rats of normal occlusal forces were used as control(n=6). The results were assigned a mean score based on the expressions and the intensity of cell positively stained for Bcl-2 and Bax. Results: Animal model of occlusal force changes was successfully established. HE staining showed that the control group had more pycnotic periodontal ligament, orderly aligned fibres, and flatter alveolar bone than model group. ABSTRACT

Objective:To study the role of tetrodotoxin-resistant(TTX-R)human sodium channel of hNa v1.8 mRNA in trigeminal neuralgia (TN). Methods:The samples of affected trigeminal division(inferior alveolar nerve) and non-affected division(lingual nerve) were surgically obtained from six idiopathic TN patients to whom the drug therapy was not effective.The expression of hNa v1.8 mRNA was assessed by semi-quantitative reverse transcription polymerase chain reaction (RT-PCR) with ?-actin as an internal control. A sample of great auricular nerve (from a patient who had to accept the combined radical neck dissection with glossectomy and hemimandibulectomy) and a sample of infraorbital nerve (from a patient who had to accept subtotal resection of maxilla) were used as the negative controls. Resoults:The relative amount of hNa v1.8 mRNA in the affected trigeminal divisions(1.233?0.324) was higher than that in the non-affected divisions(1.018?0.249), t=3.919,P=0.011. Conclusion:The higher expression of hNa v1.8 mRNA in the affected nerves of TN patients may play an important role in TN.

Objective:To evaluate the biocompatibility of Ti-75 alloy(TA) as a substrate of composite implant (CI) and the role of the shear strength of TA-coating in the CI-bone interface. Methods:A transcortical implant model was used. A total of 32 implants of CI and TA were implanted into bilateral femur of 4 adult mongrel dogs. the animals were killed 3 and 6 months after surgery. The implant-bone interfaces were observed by morphological examination and the interface shear strength was measured by push-out test. ABSTRACT

The tongue and its surrounding structures have special anatomical and functional characters and how to restore its shape and function after glossectomy (especially total glossectomy) still remains a difficult problem. The authors devised a new method of repair by using a forearm free flap with results in 16 out of 17 patients. The selection and shaping of the flap, the evaluation of the recipient site and its blood vessels, the prevention of the oral infection,the monitoring of the circulatory status of the flap and the evalution of the operative result were discussed.

ABSTRACT

Objective:To observe whether there is abnormal expression of tetrodotoxin-resistant (TTX-R)hNa v1.8 channel protein in demyelinated fibers of the affected nerves in patients with trigeminal neuralgia(TN), and to explore the relationship between ectopic discharges and TN.Methods: Six affected inferior alveolar nerves obtained from patients with idiopathic TN were studied. One great auricular nerve and one normal inferior alveolar nerve were used,which were obtained from patients undergoing combined radical neck dissection with glossectomy (negative control) and mandibulectomy (normal controls) . One rat spinal nerve was used as positive control. Immunohistochemical method and electron microscope were used to observe the expression of TTX-R hNa v1.8 channel protein in all groups. Results: Strong expression of hNa v1.8 channel protein was found in the demyelinated fibers of the affected nerves in patients with TN, weak expression in the neuraxon of rat spinal nerve,and none in the normal inferior alveolar nerve and the great auricular nerve. Conclusion: Abnormal expression of hNa v1.8 channel protein in the demyelinated fibers in patients with TN may play an important role in the pathogenesis of TN.

ABSTRACT

Objective: To study the effects of capsaicin applied locally to infraorbital nerve on primary afferent nerve inadult rats. Methods:Capsaicin solution (2% ) was directly applied onto one lateral intraorbital nerve, and the contralateralnerve was used as self-control. Both experimental and control infraorbita1 nerves were observed under electron tnicroscope on3, 7, 14, 21, 28, 35 or 42 d after local administration. Substance P(SP)--like immunoreactive positive granules in both lateralnucleus caudalis spinalis nervi trigemini (CNV) were examined by immunohistochemical method. Results: Degenerated un-myelinated fibers in capsaicin-treated nerves were found under electron microscope, and no myelinated fibers changes werefound. The gray scale value of SP-like immunoreactive positive granules in the experimental side was markedly lower thanthat in the control side (P<0. 01), while no significant difference was observed between different time groups. Conclusion:Local administration of capsaicin can produce selective destruction of unmyelinated fibers in primary afferent nerves, and canreduce the levels of SP in regions of CNV. The results suggest that local administration of capsaicin has potential therapeuticvalue for trigeminal neuralgia.
