ABSTRACT

Objective:To explore the impact of nurses' hierarchical management on nurses' professional life quality, nursing quality and nursing risk in emergency department. Methods:A total of 40 nurses who were on duty in the Emergency Department of Tangshan Gongren Hospital from February 2018 to May 2020 were selected for the study. Our hospital performed routine management of emergency department nurses from February 2018 to February 2019. After a 3-month wash-out period, nurses were managed hierarchically from May 2019 to May 2020. According to the practical experience and educational background of nurses, they were divided into 5 levels, namely N0 level assistant nurse ( n=12), N1 level registered nurse ( n=9), N2 level responsible nurse ( n=10), N3 level specialist nurse ( n=5), N4 expert level ( n=4). Before and after the hierarchical management of nurses in the emergency department, the Quality of Nursing Work Life (QNWL) scale was used to evaluate the nurse′s quality of life, and the nursing quality was assessed by the Nursing Service Quality Questionnaire. Analyze the incidence of errors in the nursing process to assess the nursing risk. Results:The scores of work environment, work family balance, social environment, workload score and QNWL total score before the implementation of hierarchical management were 46.33 ± 8.52, 30.01 ± 6.07, 21.45 ± 4.60, 94.52 ± 8.06, 192.31 ± 12.46, respectively. After implementation, the scores were 51.29 ± 4.75,34.62 ± 4.68, 25.89 ± 3.58, 109.43 ± 6.15 and 221.23 ± 10.03, respectively. The scores after implementation were higher than those before ( t value was 3.216-11.435, P<0.05). The scores of human resource allocation, ward management, operating procedures, basic nursing, nurse training, nursing efficiency, nursing record writing, and first-aid goods management were 84.29 ± 9.75, 80.39 ± 12.46, 83.02 ± 5.63, 80.93 ± 9.84, 87.14 ± 6.12, 85.91 ± 8.46, 88.20 ± 6.76 and 83.51 ± 7.18, respectively. After the implementation, the scores were 92.38 ± 4.66, 94.67 ± 3.89, 91.25 ± 6.37, 94.78 ± 3.56, 95.01 ± 3.78, 93.81 ± 4.11, 95.13 ± 3.07, 93.57 ± 4.62, respectively. The scores after implementation were higher than those before ( t value was 4.735-8.371, P<0.05). After the implementation of nurses' hierarchical management, the nursing error rate was 4.44% (8/180), which was significantly lower than 10.00% (18/180) before implementation ( χ2 value was 4.146, P<0.05). Conclusions:The application of hierarchical management of nurses in emergency department can improve nurses' professional life quality, improve nursing quality, and reduce the incidence of nursing errors. It is worthy of popularization and application in emergency department.

ABSTRACT

Objective@#To explore the degree of social support in patients after Coronary Artery Bypass Grafting(CABG) and its influence on Acute Stress Disorder (ASD), in order to provide theory evidence for nursing intervention.@*Methods@#From June 2017 to Mar 2018, 359 patients after CABG in Tangshan Workers Hospital were recruited for the study. Their general information were collected, SASRQ, SSRS and PSS-Fa were used for professional evaluation.@*Results@#The degree of social support in patients after CABG was at medium level, the total score of social support was (41.69±0.28), higher than the normal domestic value[(34.56±3.73)score, n=128]; character (F=29.652), employment (t=6.526)and the type of health insurance (F=20.547)impacted on the degree of social support (P<0.05). Total score of social support had negative correlation with SASRQ total score (r=-0.528, P<0.05); character (β=0.262), family support (β=-0.281) and social support (β=-0.267) were picked in the regression equation (P<0.05), the results showed that introversion was a risk factor for ASD in patients after CABG, high degree of family support and social support were protective factors for ASD in patients after CABG.@*Conclusions@#Social support of patients after CABG needs to be improved, so as to reduce the incidence of acute stress disorder.

ABSTRACT

BACKGROUND: Acute coronary syndrome(ACS) are a result of the rupture of unstable atherosclerotic plaques and thrombosis. Matrix metalloprotei nases (MMPs) are believed to play an important role in vascular remodeling and plaque instability by degrading the extracellular matrix.OBJECTIVE: To investigate the possible impact of polymorphisms in matrix metalloproteinase-3 and -12 genes on the risk of ACS in Chinese Han population.DESIGN: A case-control study based on ACS patients as subjects and healthy population as the control group.SETTING: Department of clinical tests of a university hospital and the department of biochemistry and molecular biology of a medical college PARTICIPANTS: A total of 103 patients with ACS(outpatients and inpatients received and hospitalized from March 2002 to December 2003 at the People' s Armed Police Hubei General Hospital,Zhongnan Hospital of Wuhan University and Liyuan Hospital of Huazhong University of Science and Technology) were involved and were divided into ACS group and the normal control group. For the ACS group, there were 73 males and 30 females with an average age of(53±10) years, including 47 cases of acute cardiac infarction and 56 cases of unstable cardiac angina. For the normal control group, there were 100 healthy subjects as controls, including 67 males and 33 females with an average age of(52±12) years. All the subjects were Chinese Han Population in Hubei Province, and they were voluntary to participate in the study.METHODS: Polymerase chain reaction(PCR) and restriction fragment length polymorphism(RFLP) techniques were used to examine the MMP-3-1612 5A/6A and MMP-12 -82 A/G gene polymorphisms respectively.MAIN OUTCOME MEASURES: The genotypes of and the allele frequencies in the patients and the individuals in the control group.RESULTS: The prevalence of 5A/6A +5A/5A genotype(41.7% vs 24%,x2 =6. 447,P<0.05) and5A allele (21.8% vs 13%,χ2 =4. 906,P<0.05) of MMP-3 gene were significantly more frequent in the ACS group than in the control subjects. The odds ratio of the 5A/6A +5A/5A was 2. 269(95% CI,1.241 to 4. 149,P<0.01). No significant difference was detected between the patients and the control group in genotype and allele distribution of the MMP-12 -82 A/G gene polymorphism(P>0.05).CONCLUSION: These findings indicate that the 5A/6A polymorphism of MMP-3 gene is associated with the risk of ACS,and might play an important role in the development of coronary atherosclerosis..This conclusion provides experimental data for gene study of the coronary disease and primary intervention for rehabilitation.

ABSTRACT

Objective To investigate the preventive measurement of vagal reflex occurrence on patients with percutaneous transluminal coronary angioplasty (PTCA).Methods On the reason of vagal reflex occurrence,to 75 patients with PTCA adopt the measurement to prevent hypovolemia, control the excessive rebound of cavity viscera, adjust the patients mental state, and so on.Then to observe the level of patients with vagal reflex occurrence, and compare with the control group of 75 patients under common care. Results The prevent group level of patients with vagal reflex occurrence is 2.61%;the control group is 28%.Conclusion To patients with PTCA adopt the measurement to prevent hypovolemia, control the excessive rebound of cavity viscera, adjust the patients mental state, and so on.The effect to decrease the level of vagal reflex occurrence is great.

ABSTRACT

Some of the SARS patients' rights( such as self-determination, privacy, personal freedom and body eminent domain) and social commonweal principle conflict, which restricts a SARS patient's rights. Meanwhile the SARS patients must carry out their obligations to safeguard public interests. It's revealed from one aspect that the public not only pay attention to the patients' legal rights and interests but also understand the patients' rights and obligations are relative and flexible.
