ABSTRACT

Guaranteeing the rights and safety of subjects is an important responsibility of all participants in the medical devices clinical trial, including medical institutions, sponsors and researchers. The legal disputes caused by serious adverse events in the clinical trial of medical devices are characterized by complex legal relationships, great difficulty in handling, and many points of dispute. Based on a typical case of medical device clinical trials, this paper discussed the litigation subject qualification, the treatment of contract breach and tort in medical device clinical trial, analyzed the responsibility of different subjects, and provided constructive suggestions on the risk management of medical device clinical trial.

ABSTRACT

Objective:To explore the mechanism of Banxia Xiexintang (BXXX) in preventing and treating chronic atrophic gastritis (CAG) through Kelch-like ECH-associated protein 1 (Keap1)/nuclear factor erythroid 2-related factor 2 (Nrf2)/antioxidant response element (ARE) signaling pathway. Method:SD rats were divided into a normal group (<italic>n</italic>=12) and an experimental group for CAG model induction. The model rats were then randomly divided into a model group, a vatacoenayme (VG) group (60 mg·kg<sup>-1</sup>), and high- (280 mg·kg<sup>-1</sup>), medium- (140 mg·kg<sup>-1</sup>), and low-dose (70 mg·kg<sup>-1</sup>) BXXX groups. The doses in the BXXX groups were equivalent to 28, 14, and 7 g·kg<sup>-1</sup> crude drugs. The rats in the normal group and the model group received distilled water at an equal volume, and those in the VG group and the BXXX groups were treated correspondingly by gavage. After 12 weeks of treatment, hematoxylin-eosin (HE) staining was carried out to observe pathological changes in the gastric mucosa of CAG rats. Western blot and real-time fluorescence-based quantitative PCR was used to detect the protein and mRNA expression levels of Nrf2, glutathione S-transferase (GST), and NAD (P)H:quinone oxidoreductase 1 (NQO1) in the gastric mucosa of CAG rats. Result:Compared with the normal group, the model group showed increased protein and mRNA expression levels of Nrf2, NQO1, and GST in the gastric mucosa of the rats (<italic>P</italic><0.05), atrophic gastric mucosa, and even intestinal metaplasia. The protein and mRNA expression levels of Nrf2, NQO1, and GST in the VG group and the high- and medium-dose BXXX groups were lower than those in the model group (<italic>P</italic><0.05), and gastric mucosa atrophy and intestinal metaplasia were significantly improved, especially in the high-dose BXXX group. However, the effect in the low-dose BXXX group was not significant. Conclusion:BXXX can blunt the transcriptional activity of Nrf2, shut down Nrf2 signaling pathway, and reduce the expression levels of NQO1 and GST to achieve normal oxidation-anti-oxidation balance, which may be one of its action mechanisms in the treatment of CAG.

ABSTRACT

Objective:To explore the effects of Yuanzhisan (YZS) containing Ginseng Radix et Rhizoma (YZSR) or Codonopsis Radix (YZSD) on memory disorder based on network and experimental pharmacology. Method:The active components and targets of YZS were retrieved from the component database and literature, and the targets of memory disorder from the disease databases. The intersection targets revealed by Veen diagram were subjected to pathway analysis. The common active components of YZSR and YZSD were molecularly docked onto the core targets. Scopolamine hydrobromide was used to establish the memory disorder model, which was employed in the behavioral experiments for evaluating the effect of YZSR and YZSD on memory disorder. Result:There existed 33 active components for Ginseng Radix et Rhizoma and 31 for Codonopsis Radix, with four common active components and 380 common targets. YZSR contained 85 active components and 790 drug targets, and YZSD 81 active components and 781 drug targets. The mapping of 425 memory disorder targets with those of YZSD and YZSD yielded 133 and 130 intersection targets, respectively. The metabolic pathways involved calcium ion signaling pathway, hypoxia-inducible factor-1 (HIF-1) signaling pathway, mitogen-activated protein kinase (MAPK) signaling pathway, etc. As revealed by molecular docking, the binding energy of common active components to the targets was negative, and the binding effect of frutinone A was the best. Behavioral experiment results showed that both YZSR and YZSD alleviated the memory disorder. In the step-down test, the number of errors in the YZSD group was significant lower than that in the model group (<italic>P</italic><0.01). In Morris water maze test, the movement distance of the YZSD group was remarkably shortened in comparison with that of the model group (<italic>P</italic><0.05). In the open field test, the movement distances of both the YZSR and YZSD group were shortened in contrast to that in the normal group (<italic>P</italic><0.05). Conclusion:YZS had a certain effect on memory disorder. There are similarities and differences between YZSR and YZSD in the treatment of memory disorder.

However, the mechanism underlying this dependence is largely unknown. In this study, we used the withdrawal symptoms precipitated by naloxone to characterize opioid dependence in mice. We determined the functional role of mu-opioid receptors (MORs) expressed in different subpopulations of neurons in the development of morphine withdrawal. We found that conditional deletion of MORs from glutamatergic neurons expressing vesicular glutamate transporter 2 (Vglut2) largely eliminated the naloxone-precipitated withdrawal symptoms. In contrast, conditional deletion of MORs expressed in GABAergic neurons had a limited effect on morphine withdrawal. Consistently, mice with MORs deleted from Vglut2 glutamatergic neurons also showed no morphine-induced locomotor hyperactivity. Furthermore, morphine withdrawal and morphine-induced hyperactivity were not significantly affected by conditional knockout of MORs from dorsal spinal neurons. Taken together, our data indicate that the development of morphine withdrawal is largely mediated by MORs expressed in Vglut2 glutamatergic neurons.

ABSTRACT

The chemical constituents from ethyl acetate extract of Gleditsiae spina were isolated and purified by various chromatographic methods such as MCI gel CHP-20, ODS, Sephadex LH-20, silica gel and semi-preparative HPLC. Seven lignans were isolated and identified by spectroscopic data analyses as (7R,8S,7'E,7''S,8''R)-buddlenol P (1), (+)-syringaresinol (2), (+)-isolariciresinol (3), (7S,8R)-cedrusin (4), (7S,8R)-4,9,9'-trihydroxy-3,3'-dimethoxy-7,8-dihydrobenzofuran-1'-propylneolignan (5), 3',4-O-dimethylcedrusin (6), balanophonin (7). Among them, compound 1 is a new lignan, compounds 2-7 are isolated from the Gleditsia L. for the first time. MTT method was used to investigate the effect of compounds 2-7 on LPS-induced injury of NRK-52e cells. As a result, compounds 2, 3 and 7 exhibit protective effects against LPS-induced damage to NRK-52e cells.

ABSTRACT

This study was designed to study the chemical constituents from bulbil of Dioscorea opposite Thunb.. Four compounds were isolated by silica gel column chromatography. On the basis of physic-chemical characters and spectroscopic data analysis, these compounds were identified as lyzalkaloid (3,4-dihydro-6-hydroxy-4-methyl-6H-pyrido[6,5-b]indol-5(1H)-one) (1), anoectochine (2), ginsenine (3), and 2-hydroxy-3-(1H-indol-3-yl) propanoic acid methyl ester (4). Compound 1 is a new indole alkaloid, named as lyzalkaloid. Compounds 2-4 were isolated from this plant for the first time. The cytotoxic activities were assessed by MTT assay. All compounds exhibited the cytotoxic activity against HepG2 and MDA-231 with IC50 values of over 100 μmol·L-1, respectively. All compounds show no significant cytotoxic activities against HepG2, MDA-231 cancer cell.

ABSTRACT

Objectives:To analyze the correlation between systolic cardiac insufficiency and ECG parameters of patients with triple-vessel disease(left anterior descending artery, left circumflex artery and right coronary artery showed ≥ 70% of diameter stenosis), but without history of myocardial infarction. Methods: A total of 96 triple-vessel disease patients without prior myocardial infarction who underwent coronary angiography examination between 2017-03-01 and 2017-07-05 in Zhongshan hospital were recruited in this study. According to LVEF, patients were divided into the normal cardiac function group (78 patients with LVEF ≥ 50%) and the reduced LVEF group (18 patients with LVEF < 50%). The Receiver Operating Characteristic (ROC) curve was applied to test optimal cut-off value of the ECG parameters and logistics regression analysis was utilized to determine the correlation between ECG indices with cardiac insufficiency. Results: A small percentage (18.8%) triple-vessel disease patients without prior myocardial infarction developed cardiac insufficiency. QRS duration, QTc duration were all significantly increased in patients with cardiac dysfunction) compared with patients with normal cardiac function (P < 0.05). ROC curve indicated good predictive efficacy to systolic cardiac insufficiency with HR > 70.5 bpm (sensitivity 81.3%, specificity 58.9%), QRS > 97.5 ms(sensitivity 82.4%, specificity 67.5%), QTc > 425 ms (sensitivity 93.8%, specificity 41.1%). Logistic multivariate regression analysis showed that QRS >97.5 ms(OR=7.577, 95%CI:1.094~52.490,P =0.030) was significantly correlated with systolic cardiac insufficiency. Conclusions: For triple-vessel disease patients without prior myocardial infarction, wider QRS in the resting ECG may indicate cardiac insufficiency.

ABSTRACT

The present study aims to investigate the lignans from the flower buds of Magnolia biondii. The isolation and purification of the compounds were performed by column chromatographies on Diaion HP-20, silica gel, and Sephadex LH-20, combined with semi-preparative HPLC. Their structures were elucidated on the basis of spectral data and physiochemical properties. Eighteen compounds were isolated and identified as magnolin (1), epimagnolin (2), eudesmin (3), kobusin (4), aschantin (5), lirioresinol B dimethyl ether (6), pinoresinol monomethy ether (7), (+)-de-O-methylmagnolin (8), isoeucommin A (9), syringaresinol 4-O-β-D-glucopyranoside (10), phillygenin (11), lariciresinol-4'-O-β-1-D-glucoside (12), conicaoside (13), (7'S, 8'R)-dihydrodehydrodiconiferylalcohol (14), 7R*, 8S*-dihydrodehydrodiconiferyl alcohol 4-O-β-D-glucopyranoside (15), 7S, 8R-erythro-7, 9, 9'-trihydroxy-3, 3'-dimethoxy-8-O-4'-neolignan 4-O-β-D-glucopyranoside (16), 7S, 8R-erythro-4, 9, 9'-trihydroxy-3, 3'-dimethoxy-8-O-4'-neolignan-7-O-β-D-glucopyranoside (17), and (+)-isolariciresinol (18). Compounds 7-18 are isolated from this plant for the first time.

ABSTRACT

To observe the clinical efficacy of Huazhuo Jiedu formula in treating chronic erosive gastritis (CEG) patients with syndrome of accumulation of turbidity and toxicity, explore its mechanism by observing the changes in expression levels of hypoxia inducible factor (HIF-1α), vascular endothelial growth factor (VEGF) in serum and gastric mucosa tissues after treatment, and provide theoretical basis for the clinical application of Huazhuo Jiedu formula in treating chronic erosive gastritis. All 70 patient of CEG were randomly divided into control group and treatment group, 35 cases in each group. The patients in control group received Alatan Wuwei Wan, bid, 1 bag/time; while the patients in treatment group were given with Huazhuo Jiedu formula, 1 dose/day. The course of the treatment was 6 months in both groups. The changes in clinical symptoms, gastroscopic signs, pathology and the expression levels of HIF-1α, VEGF, and phosphatase and tensin homolog deleted on chromosome ten (PTEN) in serum and gastric mucosa tissues were observed in both groups. The results showed that treatment group was better than control group in clinical efficacy, gastroscopic efficacy and pathological effect after treatment (<0.05); the levels of HIF-1α and VEGF in serum of treatment group were lower than those in the control group after treatment (<0.05), while the level of PTEN in serum of treatment group was higher than that in the control group after treatment (<0.05); the levels of HIF-1α and VEGF in gastric mucosa tissues in the treatment group were lower than those in the control group after treatment, while the level of PTEN in gastric mucosa tissues in treatment group was higher than that in the control group after treatment (<0.05), with statistically significant differences between these two groups (<0.05). Huazhuo Jiedu formula can improve the clinical symptoms, gastroscopic signs and pathological conditions in CEG patients with syndrome of accumulation of turbidity and toxicity, and the mechanism may be associated with decreasing the expression level of HIF-1α, VEGF and increasing the expression level of PTEN.

Objective@#To study the correlation of the gene expressions of Chk1 and Chk2 with sperm concentration and motility.@*METHODS@#According to sperm concentration and motility (percentage of progressively motile sperm), we divided 80 semen samples into four groups of equal number: normal control, oligozoospermia (OS), asthenospermia (AS), and oligoasthenozoospermia (OAS). We detected the sperm DNA fragmentation index (DFI) and viability and determined the expressions of Chk1 and Chk2 in the sperm by RT-PCR and Western blot.@*RESULTS@#Statistically significant differences were not found in sperm DFI among the control, OS, AS, and OAS groups (21.24±6.93, 19.67±7.64, 21.52±6.92, and 19.28±11.55, P>0.05), but observed in sperm concentration, progressive motility, and viability between the DFI >30% and DFI ≤30% groups (P<0.01). Compared with the normal control, sperm viability was remarkably decreased in the OS, AS, and OAS groups ([83.48±9.87]% vs [63.86±9.16]%, [50.45±16.99]%, and [39.21±15.74]%, P<0.05). RT-PCR showed remarkable differences among the control, OS, AS, and OAS groups in the relative expression level of Chk1 mRNA (0.73±0.22, 0.62±0.14, 1.03±0.39, and 0.92±0.071, P<0.01), which was correlated positively with sperm concentration (b = 80.661, P<0.01) but negatively with sperm motility (b = -19.275, P < 0.01), as well as in that of Chk2 mRNA (0.66±0.30, 0.27±0.09, 0.59±0.19, and 0.42 ± 0.11, P<0.01), which was correlated negatively with sperm concentration (b = -90.809, P<0.01) but positively with sperm motility (b = 27.507, P <0.01). The relative expression levels of the Chk1 protein were significantly different among the four groups (0.63±0.05, 0.42±0.03, 1.13±0.08, and 0.87±0.07, P<0.01), which was correlated positively with sperm concentration (b = 55.74, P<0.01) but negatively with sperm motility (b =-22.649, P<0.01), and so were those of the Chk2 protein (1.23±0.36, 0.37±0.16, 0.87±0.08, and 0.68±0.12, P<0.01), which was correlated negatively with sperm concentration (b =-53.001, P<0.01) but positively with sperm motility (b = 16.676, P < 0.01).@*CONCLUSIONS@#Chk1 and Chk2 are significantly expressed in human sperm. In case of sperm DNA damage, up-regulated Chk1 expression may enhance sperm apoptosis and lead to asthenospermia, while increased Chk2 expression may inhibit spermatogenesis and result in oligospermia.

ABSTRACT

Objective To investigate the phenolic constituents in the fruit of Gardenia jasminosides var. radicans. Methods The phenolic constituents were isolated and identified by chromatography on silica gel, Toyopearl HW-40, Sephadex LH-20, MCI gel CHP-20, ODS, and RP-HPLC. Their structures were elucidated on the basis of physicochemical properties and spectral analyses. Results Seventeen compounds were isolated from 50% aqueous acetone extract from the fruit of Gardenia jasminosides var. radicans and identified as 3,3',5-trimethoxy-9,9'-epoxylignane-4,4'-diol (1), 3,4-divanillyltetrahydrofuran (2), marphenol C (3), sinapaldehyde (4), hydroxycinnamic acid (5), isoferulic acid (6), caffeic acid (7), isofraxidin (8), isoscopoletin (9), 6-methoxy-7-hydroxycoumarin (10), 6,7-dihydroxycoumarin (11), syringic acid (12), 3-hydroxy-4-methoxybenzoic acid (13), 3,4-dihydroxybenzoic acid (14), p-hydroxybenzoate (15), 4,5-dihydroxy-3-methoxy-benzoic acid (16), and gallic acid (17). Conclusion All 17 compounds are isolated from this plant for the first time.

ABSTRACT

The chemical constituents of Dioscorea opposite Thunb. were isolated and purified by Diaion HP-20, Sephadex LH-20, Toyopearl HW-40, MCI Gel CHP-20, ODS, silica gel column chromatography and semi-preparative-HPLC. Nine compounds were found and their structures were elucidated by spectral data and physicochemical properties, which were identified as 2-(1',2',3'-trihydroxybutyl-4'-O-α-D-glucopyranoside)-6-(2",3",4"-trihydroxybutyl)-pyrazine (1), uracil (2), xanthine (3), hypoxanthine (4), thymine (5), adenosine (6), uridine (7), inosine (8), and 5'-deoxy-5'-methylsulphinyladenosine (9). The compound 1 is a new pyrazine derivative, and the compound 3-5, 8, 9 are found in this plant for the first time.

ABSTRACT

A HPLC-UV-ELSD method was established for simultaneous determination of six components in two intermediates of Shenqi Fuzheng injection (SFI) and the feasibility of establishing quantitative analysis of multi-components by single marker (QAMS) methods on different detectors was further explored. Calycosin-7-O-β-D-glucoside and astragloside Ⅳ were selected as internal reference substances for respectively flavonoids and saponins, and relative correlation factors (RCF) of formononetin-7-O-β-D-glucoside, 9, 10-dimethoxypterocarpan-3-O-β-D-glucopyranoside, 2'-dihydroxy-3', 4'-dimethoxyisoflavan-7-O-β-D-glucopyranoside and astragloside Ⅱ were calculated. Eventually, quantitative results of the 14 samples were compared between QAMS and external standard method. The sample concentrations calculated by QAMS were similar with concentrations calculated by external standard method, and the absolute values of relative deviations were generally less than 5% according to the UV detection of flavonoids. On the basis of ELSD detection for saponins, however, the absolute values of relative deviation of the two methods ranged from 0.48% to 23.17%. The QAMS method built on ultraviolet (UV) detectors was stable and can be used as a substitute method to reduce the consumption of standard compounds; meanwhile, the accuracy of QAMS method built on evaporative light scattering detector (ELSD) was inferior to that of external standard method, and the working principle of ELSD and feasible concentration range remain to be further studied.

ABSTRACT

<p><b>Objective</b>To evaluate the safety and effect of transurethral holmium laser enucleation of the prostate (HoLEP) in comparison with bipolar transurethral plasmakinetic prostatectomy (TUPKP) in the treatment of benign prostatic hyperplasia (BPH).</p><p><b>METHODS</b>We searched the databases of PubMed, SCI, Ovid, The Cochrane Library, CNKI, CBM, VIP, and Wangfang Data for controlled clinical trials about HoLEP versus TUPKP in the treatment of BPH published up to April 2016. The studies were screened according to the inclusion and exclusion criteria, the data extracted, and their quality evaluated by 2 reviewers independently, followed by a meta-analysis using the RevMan 5.3 software.</p><p><b>RESULTS</b>A total of 7 studies were included, involving 2031 cases. In comparison with TUPKP, HoLEP showed significantly longer operation time (WMD = 24.61, 95% CI 11.88, 37.34, P lt; 0.001), shorter hospital stay (WMD =-1.91, 95% CI -3.74, -0.07, P = 0.04), shorter bladder irrigation time (WMD = -21.50, 95% CI -34.95, -8.06, P = 0.002), shorter catheter-indwelling time (WMD = -27.60, 95% CI -48.17, -7.03, P = 0.009), less hemoglobin loss (WMD = - 0.42, 95% CI -0.78, -0.07, P = 0.02); lower postvoid residual urine (PVR) at 3 months (WMD = -3.35, 95% CI -4.46, -2.23, P<0.001) and 6 months after surgery (WMD =-1.11, 95% CI -2.18, -0.05, P = 0.04); higher maximum urinary flow rate (Qmax) (WMD = 0.42, 95% CI 0.04, 0.80, P = 0.03) and fewer urinary tract irritation symptoms (OR =0.58, 95% CI 0.41, 0.81, P = 0.002) at 12 months after surgery. No statistically significant differences were found between the two groups in the volume of resected tissue, serum sodium reduction, urethral stricture, erectile dysfunction, retrograde ejaculation, or transient urinary incontinence (P>0.05), or in the improvement of the quality of life (QoL) at 1, 3 and 12 months, International Prostate Symptom Score (IPSS) at 1, 3, 6 and 12 months, Qmax at 1, 3 and 6 months, or International Index of Erectile Function-5 (IIEF-5) at 6 months after surgery (P>0.05).</p><p><b>CONCLUSIONS</b>HoLEP is preferred to TUPKP in clinical application for its advantages of higher Qmax at 12 months after surgery, lower PVR at 3 and 6 months, higher peri-operative safety, faster recovery, and fewer urinary tract irritation symptoms. However, for the quantity and quality limitations of the included publications, our findings are to be further supported by large-sample, multi-center, and high-quality prospective controlled clinical studies.</p>

Inflammasome is a vital part of innate immunity, and NLRP3 inflammasome is by far the most thoroughly studied inflammasome complexes that have been described. NLRP3activating signals include toxins secreted by pathogens, crystalline moleculesand endogenous danger signals. Activation of NLRP3 inflammasome needs two steps: priming and activating. The priming step affects NLRP3 at the transcriptional and posttranscriptional modification levels * the activating step is associated with ion flowing, mitochondria and lysosomes. In this paper we also reviewed the negative regulation of NLRP3 inflammasome at the expression, assembly and activation levels.

ABSTRACT

The present study was aimed to isolate the active compounds from the fermentation products of Fusarium oxysporum, which had hepatitis C virus (HCV) NS3 protease inhibitory activity. A bioactive compound was isolated by reverse-phase silica-gel column chromatography, silica-gel column chromatography, semi-preparative reverse-phase High Performance Liquid Chromatography (HPLC), and then its molecular structure was elucidated based on the spectrosopic analysis. As a result, the compound (H1-A, 1) Ergosta-5, 8 (14), 22-trien-7-one, 3-hydroxy-,(3β, 22E) was isolated and identified. To the best of our knowledge, this was the first report on the isolation of H1-A from microorganisms with the inhibitory activity of NS3 protease.

ABSTRACT

Objective:To investigate the serum Vitamin A level of 2-6 years old children in Shenyang kindergarden. Method:A total of 2 000 children aged 2-6 years old in Shenyang kindergarden were selected. And fluorescence micro spectrophotometry was used to detect. Results:Serum Vitamin A level of 2 000 children fluctuated at 0.700-1.751μmol/L,the mean was (1.074±0.257)μmol/L. There was no vitamin A deficiency (VAD) and sub-Vitamin A deficiency (SVAD) . The incidence of suspect SVAD was 52.7%. Conclusions:VAD and SVAD may be eradicated in 2-6 years old in Shenyang kindergarden, but there is increasing tendency in suspect SVAD. Decreasing and eradicating susptect SVAD is important work of our current and next step.

ABSTRACT

Objective: To investigate the differential expression of ST3GalⅤ in human hepatic carcinoma cell lines, HepG?2 and SMMC?7721 and normal hepatic cell line, L?02 by sialyltransferase DNA microarray. Methods: Expression of ST3Gal family was measured with DNA microarray. Western blot was used to verify DNA microarrays data. Result:ST3GalⅤ downregulated in HepG?2 and SMMC?7721 cell lines, compared with control cell line. Conclusion: The downregulation of ST3GalⅤ in HepG?2 and SMMC?7721 cell lines may result in the reduction of GD1a and influence the hepatocyte proliferations.

ABSTRACT

Objective:To test the effect of ginseng polysaccharides on anti?tumor activity. Method: The cell inhibitory rates were detected by MTT assay. Results: The starch?like glucan ( WGPN) and the AG type pectin ( WGPA?2?RG) had no effect on tumor cells, the AG+HG type pectin ( WGPA?4?RG) and the HG type pectin ( WGPA?3?HG, WGPA?1?HG) showed stronger effect on tumor cells. Conclusion:Different types of ginseng polysaccharides have different antiproliferation. A neutral fraction WGPN has no inhibition, while ginseng pectin exertes variable inhibition.
