ABSTRACT

Objective:To explore the effect of scientific research group-based model on the improvement of nursing scientific research ability in a county-level hospital.Methods:A nursing research group was established, the composition of team members and work contents of the group were clarified, tiered focused scientific research training was implemented through progressive research content and flexible training methods, and individual and progressive research guidance was provided through research counseling application forms, while literature reading activities were conducted through article reports and literature debriefings. Retrospective analysis of the nursing research practice, the number of scientific research outputs, and research capacity were applied to evaluate the effect of the nursing research group-based model.Results:Within 2 years of the establishment of the nursing research group, 37 training programs were conducted, 33 scientific research materials were pushed to the WeChat public account, 23 scientific research projects were tutored, and 6 research projects were successfully declared. The scientific research ability score of nursing staff participating in literature reading debriefing increased from 44.24±19.12 to 53.57±13.86( P<0.001). Conclusions:The application of the nursing research group-based model can improve the nursing scientific research ability of the county-level hospital.

ABSTRACT

Objective:To improve the problems of catheter occlusion, low comfort of patients and low satisfaction of nurses, the pre-filled SS-Y valve fixation method was used for catheter nursing.Methods:Traditional method of arterial catheter nursing were performed in 54 patients with malignant tumors of digestive tract treated by transcatheter arterial infusion chemotherapy (TAI) (control group) from April 1 to December 31, 2017 in the Department of Radiation Intervention, the Affiliated Second Hospital of Medical College of Zhejiang University. Pre-filled SS-Y valve fixation method was used for arterial catheter nursing in 72 patients treated with TAI (observation group) from January 1 to September 30, 2018. The fixation, comfort and satisfaction of nurses were compared between the two groups.Results:The rate of arterial catheter displacement, slippage and satisfaction of nurses were 0, 0 and 97.22% (70/72) in observation group, and 13.0% (7/54), 5.6% (3/54), 77.78% (42/54) in control group. Significant statistic differences were found between two groups in rate of arterial catheter displacement and comfort scores (χ 2=9.882, 11.813, P<0.01). There was no significant difference in slippage between the two groups ( P>0.05). The comfort scores of the patients in observation group at 5 time points were better than those in control group, and the difference was statistically significant ( Z=-6.462--3.611, P < 0.01). Conclusions:The method of pre-filled SS-Y valve fixation is reliable with a low displacement rate and a higher satisfaction for both patients and nurses. This method can be popularized in clinical nursing.

Objective To explore the advantages of the three-finger method of pinching the needle infusion port. Methods A total of forty three patients who underwent intracranial vein infusion port implantation between January 2018 and June 2018 were enrolled. The self-control method was divided into the test group and the control group, and both groups of patients used a safe butterfly wing non-destructive needle. The control group used the traditional needle insertion method from July 1st to July 31st. The experimental group used the three-finger needle-to-needle method from August 1st to August 31st to compare the two groups of patients in one-time injection. Different results in terms of local skin bleeding and patient satisfaction. Results The one-time injection rate and non-bleed rate of the control group were both 25.6%(11/43) and the satisfaction rate was 20.9%(9/43). There was one case of pedicle turnover, one case of clipping syndrome, and one case of local infection; no other complications. The one-time injection rate and non-bleed rate of the test group were both 90.7%(39/43) and the satisfaction rate was 90.7%(39/43); there was one case of clipping syndrome; no other complications. The difference of one-time injection rate, local non-bleed rate and satisfaction rate between the two groups was statistically significant (χ2=37.45, 37.45,42.43, both P<0.05). Conclusions The three-finger method of pinching the needle can improve the one-time injection rate of the nurse, improve the patient's satisfaction and reduce the complications. This aspect deserves to be promoted in the clinic.

ABSTRACT

Objective To explore the effects of application of one-hand double-lumen simultaneous locking tech-nique on reduction of occlusion using high pressure resistant double-lumen PICCs. Methods Convenience sampling method was used to recruit 73 patients assigned into the experimental group,using one-hand double-lumen simulta-neous locking technique;66 patients were assigned into the control group,using routine locking method. The rate of PICC occlusion was recorded. Results The rate of incomplete occlusion was 8.22% in the experimental group,and 17.42% in the control group,the rate of complete occlusion in the experimental group was 0,and 0.76% in the control group,and the difference was statistically significant(P<0.05). Conclusion The one-hand double-lumen simul-taneous locking technique is easy and convenient to conduct. It can effectively reduce occlusion using high pres-sure resistant double-lumen PICCs.

ABSTRACT

Objective To construct an adenovirus vector expressing small interfering RNA ( siRNA) targeting to rat Raf-1 gene and identify its function in cardiomyocytes.Methods The siRNA containing DNA sequence targeting to Raf-1 and its negative control sequence were designed, synthesized, annealed and subcloned into adenoviral shuttle vector pAdTrack-CMV.The recombinant adenovirus vector pAd-siRaf-1 was obtained by homologous recombination with pAdTrack-siRaf-1 linearized by PmeI and pAdeasy-1 in bacteria BJ5183, then transfected into HEK293 cells to package the adenovirus.Cardiomyocytes were infected with the adenovirus pAd-siRaf-1, and the expressions of Raf-1 and NF-κB protein were detected by Western blotting.[ 3 H ]-leu incorporation was evaluated by scintillation.The surface area of cardiomyocytes was measured using a HJ2000 image analysis system.Results The adenovirus vectors were verified by enzyme digestion and DNA sequencing.Compared with the Ang II group, Raf-1 and NF-κB expression, the surface area and [ 3 H]-leu incorporation of cardiomyocytes were significantly decreased in cardiomyocytes infected with the adenovirus PAd-siRaf-1.Conclusions A recombinant adenovirus vector containing rat siRaf-1 gene is successfully constructed.It can effectively reduce Raf-1 and NF-κB expression and cardiomyocyte hypertrophy induced by Ang II.

ABSTRACT

Objective To study the psychological stress level of medical nurses when the hospitalized cancer patients died and to investigate the related factors.Methods Toally 72 medical nurses on duty who encountered cancer patient's death between May 2013 to November 2015 were enrolled in the study.SCL-90 was used to investigate the mental stress levels in the medical nurses on days 3 and 7 after the patient's death,which were compared with the norm.The correlations between psychological stress level and related factors were studied.Results The score by SCL-90 on day 3 was significantly higher than that on day 7 (147.15±29.09 vs.133.62±20.37,t=6.64,P=0.00).Compared to the norm,the scores on 9 dimensions of SCL-90 were significantly different 3 days after the patient's death;dimensions of SCL-90 were significantly different 7 days after parternt's death (P<0.05).Professional working was factor for medical nurse's acute stress disorder.Conclusion Cancer patients' deaths render huge impact on medical nurses,with acute stress disorder most manifest.In order to improve the the psychological quality of nurses,especially the young and inexperienced ones,we have to enhance the death education,helping them to seek for social support.

ABSTRACT

Objective To explore the protective effect of liraglutide ( Li) on the proximal tubular cells ( PTECs) of rats induced by model control. Methods The PTECs of rats were obtained by primary culture and divided into normal control group ( NC) , model control group ( HG) , HG+low-dose Li group, HG+medium-dose Li group, and HG+high-dose Li group ( n-6 in each group) . After 72 h of incubation, the activity of Na+/K+-ATPase in PTECs was measured by the liquid scintillation counter. Protein expression of Toll-like receptor 4 ( TLR4 ) was measured by Western blotting. Interleukin-6 ( IL-6 ) , tumor necrosis factor-α (TNF-α) and plasminogen activator inhibitor-1 (PAI-1) in culture supernatants were measured by the enzyme linked immunosorbent assay (ELISA). Results Compared with NC, activity of Na+/K+-ATPase [(2 737. 88±317. 22)μmol·g-1 ·h-1 ] and the protein expression of TLR4 in PTECs, IL-6, TNF-α and PAI-1 in culture supernatants were significantly increased in HG (P<0. 05, or P<0. 01). Compared with HG, activity of Na+/ K+-ATPase and the protein expression of TLR4 in cells, IL-6, TNF-αand PAI-1 in culture supernatants reduced significantly in HG+medium-or high dose Li groups (P<0. 05, or P<0. 01). Conclusion Liraglutide can inhibit the expression of inflammatory cytokines and stabilize the Na+/ K+-ATPase’ s activity in PTECs in high glucose environment. Therefore, it may play a role in kidney protection.

ABSTRACT

Objective To investigate the protective effects of Radix Paeoniae Rubra(RPR)extracts on reticulum stress and macrophage infiltration on diabetic cardiomyopathy rats and the mechanism. Methods Sprague ̄Dawley rats were randomly divided into 5 groups: normol control, model control (induced by streptozotocin),RPR extracts at low dose (i.p.15 g.kg-1 ), middle dose (i.p.30 g.kg-1 ), and high dose (i.p.60 g.kg-1 ).The artery intubation was carried out in all rats for detecting cardiac function 10 weeks later.The myocardial structure was detected by masson staining.The level of myocardial tissue glucose regulatory proteins GRP78,caspase ̄12 and caspase ̄3 expression were measured by Western blot.The ED ̄1 protein was tested by immunohistochemical staining. Results Compared with the normal controls, the model rats presented decreased LVSP, +dp/dtmax ,-dp/ dtmax , LVEF, increased expressions of GRP78, caspase ̄12,caspase ̄3, ED ̄1 and the synthesis of collagen fiber(P<0.05 or P<0.01).RPR extract inhibited the above indexes of model control rats in a concentration ̄dependent manner(P<0.05 or P<0.01). Conclusion The RPR obviously ameliorates the cardiac function and myocardial fibrosis of diabetic cardiomyopathy rats,which may be associated with suppressing the expression of GRP78,caspase ̄12,caspase ̄3 and macrophage infiltration.

ABSTRACT

Objective To evaluate the application effect of clinical nursing pathways in acute myocardial infarction patients by using the Meta-analysis.Methods Published randomized controlled trials (RCT) in acute myocardial infarction patients were searched and screened in CNKI,VIP,Wanfang database under present standards.The quality of the included studies was evaluated by certain standards.The Review Manager 5.0 software was taken for analysis.Results Totally 18 studies including 1877 cases were eligible to the criteria (894 in the experimental group and 983 in the control group) altogether.The Meta-analysis showed there was significant difference between the experimental group and the control group in hospitalization days,patients' satisfaction degree and rate of mastering knowledge.Significant difference existed between the experimental group and the control group in bedridden time,complication rate and the recurrence rate of myocardial infarction.Conclusions Application of clinical nursing pathway in acute myocardial infarction patients can shorten hospitalization days,increase patients' satisfaction degree and rate of mastering knowledge,decrease bedridden time,complication rate and the recurrence rate of myocardial infarction.

ABSTRACT

Objective To assess stress degree of osteosarcoma caregivers and explore the impact factors of stress response. Methods We used CRA scale to investigate stress state of the primary caregivers of osteosarcoma patients, the demographic characteristics, status of patients, the objective amount of care and scores for each latitude underwent multiple regression analysis. Results The pressure of caregivers from the city was relatively light, annual income more than 50 thousand yuan and with help from the families to take care patients could effectively alleviate the pressure. The caregivers came from rural areas, household income less than 20 thousand yuan, caregivers were the patients' parents, had their own job presently, with no one to help were high - pressure groups. Conclusions Caregivers of osteosarcoma patients with high-pressure need focused intervention, more help and support should be given to these population.

Objective To investigate the effect of aspirin on endothelin-1 induced proliferation of cardiac fibroblasts (CFs). Methods Isolated and cultured CFs from neonatal Sprague-Dawley rats (SD) were randomly divided into 7 groups: control, ET-1, aspirin, ET-1 +aspirin 1 mmol/L, ET-1 +aspirin 2 mmol/L, ET-1 +aspirin 5 mmol/L and ET-1 + aspirinl 10 mmol/L. CFs were counted by MTr assay. Cell cycle distribution was determined with a flow cy-tometer (FCM). [~3H]-Proline incorporation was evaluated by scintillation counting. Nitric oxide (NO) was meas-ured by colorimetry. Results 10~(-7)mol/L ET-1 significantly increased the number of CFs and [~3H]-Pro incorporation and decreased NO_2~-/NO_3~--secretion compared with the control group (P <0.01).1 ~ 10 mmol/L Aspirin inhibited the effects of ET-1 on CFs in a concentration-dependent manner (P <0.05 vs ET-1). In the ET-1 group, the percent-age of cells in the S phase was higher than that of the control, which was inhibited by 10 mmol/L Aspirin (P <0.01 vs ET-1 and control). Conclusion Aspirin can restrain the proliferation and collagen synthesis of cardiac fibroblasts induced by endothelin-1, and this effect may be partially mediated by NO.
