ABSTRACT

OBJECTIVE@#To investigate the risk factors of elbow stiffness after open reduction and internal fixation of intercondylar fracture of humerus.@*METHODS@#From March 2015 to February 2019, 120 patients with humeral intercondylar fractures were treated with open fixation including 59 males and 61 females, aged from 25 to 77 years with an average of(53.5±3.2) years. According to the occurrence of elbow stiffness after operation, 120 patients were divided into stiffness group(37 cases) and control group(83 cases). The related factors of elbow stiffness were analyzed by single factor analysis, and the risk of elbow stiffness after internal fixation of humeral intercondylar fracture was analyzed by logistic regression factor.@*RESULTS@#There were 37 cases of elbow stiffness(stiff group), and 83 cases had no elbow stiffness(control group). The incidence of joint stiffness was 30.83%. There were significant differences between the stiffness group and the control group in age, injury energy, fracture to operation time, AO classification of fracture, open injury and postoperative premature or hyperactivity. Multivariate logistic regression analysis showed that age>50 years old, high energy injury, AO classification of fracture, open fracture and postoperative premature or hyperactivity were risk factors for elbow stiffness after internal fixation of humeral intercondylar fracture. The postoperative mobility and Mayo elbow performance score(MEPS) scores of the postoperative stiffness group were lower than those of the non-stiffness group with statistical significance(P<0.05). There were no significant differences in postoperative mobility and MEPS scores between flexion stiffness and rotation stiffness after humeral intercondylar fracture(P>0.05).@*CONCLUSION@#In view of the risk factors of elbow stiffness after internal fixation of humeral intercondylar fracture, reasonable operation plan and rehabilitation strategy should be formulated before operation to minimize the incidence of elbow stiffness.

ABSTRACT

With the deepening of research on intestinal microbiota, more and more evidence indicates that intestinal microbiota affects various physiological processes of the human body and is closely related to tumors, Researches show that intestinal microbiota can promote tumorigenesis, enhance the antitumor effect of chemotherapeutic drugs, mediate chemoresistance, regulate the effect of immunotherapy, and reduce intestinal damage during radiotherapy. The phenomena and mechanisms revealed in the above studies are incredible. Based on the above correlation, the genus found in the study are expected to become the new target of tumor prevention and treatment. The evidence obtained from current animal and clinical trials is refreshing, however, in view of the complexity of human intestinal microecology and the limitations of human understanding, its potential role in tumorigenesis and treatment still needs systematic and in-depth research.

ABSTRACT

The decline in skeletal muscle mass and function with age is referred as sarcopenia. It is characterized by the muscle fiber's quality, strength, muscle endurance and metabolic ability decreasing as well as the fat and connective tissue growing. Previous studies have shown that sarcopenia in itself features decreased number and cross-sectional area of muscle fibers and the net degradation of protein, which results from the joint effects of multiple factors such as the exacerbation of inflammation, oxidative stress injury, mitochondrial dysfunction, abnormal autophagy and dysregulation of muscle quality regulatory factors. In this review, we systematically displayed the molecular mechanism of sarcopenia, which will be helpful to deepen our understanding of sarcopenia and provide potential targets for the prevention and treatment of sarcopenia.

ABSTRACT

Under normal condition, there are a few lipid droplets in skeletal muscle. But in skeletal muscle acute injury, muscular dystrophy, muscle atrophy, obesity, diabetes and other pathological conditions, the fat deposition in skeletal muscle increases, which implicate that the fat deposition may play an important role in the pathogenesis of these diseases. However, the mechanisms of development and regulation of fat deposition in skeletal muscle are not clear. Clarifying the key signaling pathways and regulatory factors that affect fat deposition in skeletal muscle, and exploring new ways to improve the fat deposition in skeletal muscle will not only help to deepen our understanding of the pathogenesis of these diseases, but also provide new ideas for the treatment of these diseases. This paper reviews the research progresses and main mechanisms of fat deposition in skeletal muscle.

ABSTRACT

OBJECTIVE: To explore the relationship among social support, work engagement and mental health in preschool teachers. METHODS: By cluster random sampling method,348 preschool teachers from cities and countryside in Henan Province were selected as study subjects. A questionnaire survey was conducted using the Social Support Rating Scale, the Utrecht Work Engagement Scale and the Symptom Checklist-90( SCL-90) to analyze the relationship among social support, work engagement and mental health. RESULTS: The social support total score, the dimension scores of objective support, subjective support and support utilization were( 40. 90 ± 10. 48),( 8. 93 ± 2. 87),( 23. 79 ± 5. 85) and( 8. 17 ± 1. 94),respectively. The work engagement total score, the vitality, devotion and concentration dimension scores were( 71. 69 ±20. 18),( 24. 91 ± 9. 06),( 21. 44 ± 6. 93) and( 25. 34 ± 7. 58),respectively. The SCL-90 total score was( 1. 85 ±0. 63). Each dimension score of social support was positively correlated with each dimension score of work engagement( P < 0. 01). The SCL-90 total score was negatively correlated with each dimension score of social support and work engagement( P < 0. 01). The 3 dimensions of social support including objective support, subjective support and support utilization had significant direct effect( the standard path coefficients were- 0. 21,- 0. 27 and- 0. 23,respectively,P < 0. 01) on mental health. They also had indirect effect(- 0. 13,- 0. 16 and- 0. 12,respectively) on the mental health through work engagement among preschool teachers which accounted for 38. 00%,37. 00% and 34. 00% of their total effects(- 0. 34,- 0. 43 and- 0. 35,respectively). CONCLUSION: The social support and work engagement are important factors influencing the mental health of preschool teachers. The work engagement acts as a partial mediator between social support and mental health.

ABSTRACT

<p><b>BACKGROUND</b>Candidal esophagitis is the primary infection among all digestive tract opportunistic ones in acquired immunodeficiency syndrome (AIDS) cases. X-ray manifestation reports of it are still rare. This study aimed to conduct a retrospective analysis on the X-ray data of 6 AIDS cases complicated with candidal esophagitis, and to study the X-ray characteristics of it combined with the findings from gastroscopy.</p><p><b>METHODS</b>Among 6 cases in this series, all cases were confirmed by Shenzhen Center for Disease Control and Prevention (CDC) to be HIV positive and all of them had CD4 cell counts less than 150 × 10(6)/L. All cases underwent X-ray and gastroscopy, and mycelium were found in the mucous membrane of the esophagus.</p><p><b>RESULTS</b>In this series, the findings of the X-ray were as follows: (1) Affected areas: Four cases in the whole esophagus, 2 cases in the middle and lower part of esophagus; (2) Abnormal motivity: Six cases had decreased tension, loose walls, weakened peristalsis, decreased number of peristalsis waves and delayed emptying of barium; (3) Abnormal contour: Six cases had the sign of "decorative border" or "brush", two cases had narrowed canal; (4) Abnormal membrane and "cobblestone sign": Six cases had thickened membrane and "cobblestone sign" on the surface of the abnormal membrane. The hyperemia of mucosa was covered tightly with yellow-white pseudomembrane spots. This was in accordance with the small cobblestone-like filling defect found by X-ray.</p><p><b>CONCLUSIONS</b>If the AIDS cases have dysphagia, and X-ray shows that more than two sections of the esophagus are affected, with decreased motility, the walls in the sign of "brush" or "decorative edges", thickened membrane with "cobblestone sign", candidal esophagitis is highly possible.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the effects of Tongmai decoction on the perioperative changes of serum concentrations of tumor necrosis factor alpha (TNF-alpha) and interleukin (IL)-6 in patients with femoral fractures, and conform the effectiveness of Tongmai decoction on inflammatory factors in patients with femoral fractures, providing the theoretical evidence for the clinical use of Tongmai decoction.</p><p><b>METHODS</b>From October 2007 to May 2009, 60 patients with closed traumatic femoral fractures were selected according to the inclusion criterias and exclusion criterias. All the patients were randomly divided into three groups (group A, group B and group C). Twenty patients in group A (Tpanax Notoginseng pill group), 13 patients were male and 7 patients were female; ranging in age from 20 to 45 years, averaged 32.0 years; the disease course ranged from 2.0 to 26.0 h, with an average of 9.5 h. Twenty patients in group B (Tpanax Notoginseng pills and Lornoxicam injection group),12 patients were male and 8 patients were female; ranging in age from 23 to 42 years, averaged 31.0 years; the disease course ranged from 3.5 to 25.0 h, with an average of 13.6 h. Twenty patients in group C (Tpanax Notoginseng pill, Lornoxicam injection and Tongmai decoction group), 14 patients were male and 6 patients were female; ranging in age from 21 to 44 years, averaged 31.5 years; the disease course ranged from 4.6 to 29.0 h, with an average of 13.3 h. Among all the patients, 42 patients with fractures were fixed with femoral intramedullary nailing, and other 18 patients with femoral locking plate fixation. The patients in group A took Tpanax Notoginseng pills orally, 4 g each time and twice daily; the patients in group B took Tpanax Notoginseng pills orally as group A, and at the same time received intramuscular injection of Lornoxicam, 8 mg each time and once daily; the patients in group C took Tpanax Notoginseng pills orally and received intramuscular injection of Lornoxicam as group B, and at the same time took Tongmai decoction (R ) orally, 200 ml each time and twice daily. The above medications were administered to the three groups on the second day after admission to hospital. Peripheral blood samples were taken for determination of pro-inflammatory cytokines of TNF-alpha and IL-6 in blood serum on the 2nd and 6th days before operation and on the 8th and 13th days after operation. And all the patients were evaluated liver and kidney function at the 2nd and 7th days after admission. Analysis of variance and least significant difference-test were done with the help of SPSS 17.0 statistic software.</p><p><b>RESULTS</b>The differences among three groups of TNF-alpha and IL-6 in blood serum at the 2nd day after admission and 2 days after operation had no statistical significance (P > 0.05). The TNF-alpha and IL-6 levels among 3 groups had statistical differences at the 7th day after admission and at the 7th day after operation (P < 0.05, P < 0.01). There were significant differences of TNF-alpha and IL-6 levels between the 7th day after admission and the 2nd day after admission, the 7th day after operation and the 2nd day after admission (P < 0.01). There were also significant differences of TNF-alpha and IL-6 levels between group C compared with group A and B at the 7th day after admission and the 7th day after operation(P < 0.05, P < 0.01).</p><p><b>CONCLUSION</b>The serum concentrations of TNF-alpha and IL-6 level significantly increased in perioperative period. The results indicate that the Tongmai decoction may play an important role in inhibiting the release of TNF-alpha and IL-6 into the blood stream and decreasing the incunabula complication at early traumatic stage.</p>

ABSTRACT

Objective: To investigate the differentiated expressions of EphB2 protein in colorectal carcinoma at different clinicopathologic staging and in different colorectal carcinoma cell lines with varying metastasis potential and explore the effect of EphB2 on colorectal cancer and its action mechanism. Methods: Immunohistochemistry was used to detect the expression of EphB2 and microvascular density (MVD) in colorectal carcinoma tissues (n = 43) and normal colon tissues (n = 10) was detected. Four colorectal carcinoma cell lines with varying metastasis potential, SW480, HT29, HCT116, and LoVo cells, were cultured in vitro. The expression of EphB2 protein was measured by Western blotting and the transcription of EphB2 mRNA was determined by RT-PCR. Results: EphB2 was expressed in the cytoplasm of the most colon carcinomas. EphB2 receptor expression was negatively related with tumor infiltration depth and distant metastasis; EphB2 receptor expression was positively related with the differentiation degree of colon cancer. Expression of EphB2 protein had positive correlation with MVD in tumor tissues; the expression level of EphB2 was decreased in high-metastatic HCT116 and LoVo cells compared with low-metastatic SW480 and HT29 cells. Conclusion: Decreased expression of EphB2 may promote the carcinogenesis and progression of colorectal carcinoma. One of the explanations for the effect of EphB2 is its regulaion on tumor angiogenesis.

ABSTRACT

This study is to investigate the effect of rhein lysinate on inducing human breast cancer cell line SK-Br-3 apoptosis and the role of HER-2 signal pathway in the apoptosis. MTT assay was used to detect SK-Br-3 cell proliferation. Cell cycle and apoptosis were analyzed by flow cytometry. The protein expression and the protein phosphorylation of HER-2 signal pathway were detected by Western blotting. The level of HER-2 mRNA was detected by RT-PCR and the level of HER-2 expression was also detected by immunofluorescence cytochemical methods. The results showed that rhein lysinate remarkably inhibited breast cancer SK-Br-3 cell proliferation. The IC50 value for 48 h treatment was 85 micromol x L(-1). Apoptosis in SK-Br-3 cells was induced by rhein lysinate in a dose dependent manner. The protein expressions of HER-2, NF-KB, and the protein phosphorylation of HER-2 were downregulated, however the protein expression of p53 and p21 was upregulated after rhein lysinate treatment. The level of HER-2 mRNA decreased by using RT-PCR assay and the level of HER-2 expression was also decreased by using immunofluorescence cytochemical assay after rhein lysinate treatment. It can be concluded that rhein lysinate could inhibit SK-Br-3 cell proliferation and induce apoptosis. HER-2/NF-kappaB/p53/p21 signal pathway might be involved in this process. Rhein lysinate has a good prospect to be an adjuvant chemotherapeutic drug.

ABSTRACT

<p><b>OBJECTIVE</b>By means of phage-display technique, to screen polypeptides that specifically bind to human gastric cancer with high metastatic potential to peritoneum.</p><p><b>METHODS</b>Two human gastric cancer cell lines were used: GC9811-P with high metastatic potential to peritoneum and its wild type parental GC9811, to carry out subtractive screening with a phage display-12 peptide library.</p><p><b>RESULTS</b>After three rounds of screening, 40 phage clones bond to GC9811-P cells were randomly selected. When injected into the peritoneal cavity of nude mice, 6 of the 40 clones did not bind to mouse peritoneum as examined by immunohistochemical staining. They were considered to be capable of binding specifically to GC9811-P cells. Sequence analysis revealed two different exogenous peptides: TLNINRLILPRT and SMSI(X)SPYI(XXX).</p><p><b>CONCLUSION</b>Two peptides have been obtained that specifically bind to a gastric cancer cell variant GC9811-P, which easily disseminates to the peritoneum. Whether or not they could block GC9811-P metastasis to peritoneum in vivo remains to be determined.</p>

<p><b>OBJECTIVE</b>To determine the value of medical adhesive products for decreasing abdominal effusion after clearing up lymph nodes in patients with gastric cancer.</p><p><b>METHODS</b>Seventy patients with gastric cancer and undergoing D(2 ) or D(2 )(+) radical excision were randomly divided into group A and B based on envelope method from June 2002 to December 2003. In group A (n=35),medical adhesive products was dripped on the retroperitoneal regions where perigastric lymph nodes were cleared up, while not in group B(n=35). Four items including the volume of intraperitoneal drainage, the loss of protein, erythrocyte and hemoglobin were compared between group A and B from the first day to the 5th day after operation.</p><p><b>RESULTS</b>No patients died in group A and B. The volume of intraperitoneal drainage and loss of protein in group A were less than those in group B (P< 0.01) from the first day to the 3th day after operation, but there was no significant difference at the 4th day and 5th day after operation(P > 0.05). The total volume of intraperitoneal drainage and loss of protein in group A were less than those in group B (P< 0.01) from the first day to the 5th day after operation. The loss of erythrocyte and hemoglobin reached the peak at the first day after operation both in group A and B, then gradually decreased, so there was no significant difference between group A and B later.</p><p><b>CONCLUSION</b>The medical adhesive products can decrease abdominal effusion effectively after clearing up perigastric lymph nodes and facilitate patients' recovery from operation.</p>

ABSTRACT

Objective: Our aim was to investigate the alteration of p16 gene in human pancreatic carcinoma. Methods: A total of 66 human pancreatic tissue specimens, comprising 51 with pancreatic carcinomas and 15 normal pancreatic tissue specimens, were examined for homozygous deletion and mutation of p16 gene by using PCR-SSCP method. Results: No mutation and deletion was detected in 15 normal pancreatic tissue samples. Of 51 pancreatic carcinoma specimens, only one was found mutation for p16 gene in PCR-SSCP assay, and the deletion of the p16 gene in 23 samples were confirmed by using PCR, with a 45% p16 gene deletion rate. Conclusion: These data suggest that p16 gene alterations may play a role in the progression of human pancreatic carcinoma.
