ABSTRACT

Objective To evaluate the clinical efficacy of arthroscopic acromioplasty and rotator cuff repair for older patients with full-thickness rotator cuff tears(RCTs).Methods This was a prospective cohort study. From January 2013 to December 2017,a total of 142 older patients with fullthickness RCTs were enrolled.Based on treatment methods, patients were divided into an arthroscopic rotator cuff repair group(operation group)and a conservative treatment group(control group). The operation group had 70 patients, including 26 males and 44 females, with an average age of 74.5 years (aged 66-83 years),and was followed up for≥12 months after surgery. The DEORIO and COFIELD criteria revealed 61 cases of medium tears and 9 cases of large tears in the operation group. The control group had 62 patients, including 27 males and 35 females, with an average age of 74.2 years(aged 65-82 years),and was followed up for≥12 months. There were 41 cases of medium tears and 21 cases of large tears, according to the DEORIO and COFIELD criteria. The visual analogue scale(VAS)scores, range of motion,Simple Shoulder Test(SST),Constant score were recorded pre-treatment and at 1-year post-treatment and compared between the two groups. Results 132 older patients(70 in the operation group and 62 in the control group)were followed up for at least 1 year. There was no significant differences in VAS scores, range of motion,SST scores and the Constant score pre-treatment between the two groups(all P >0.05).However,the operation group was associated with significantly improved VAS scores, range of motion,SST scores and the Constant score at 1-year post-treatment,compared with the control groups (all P < 0.05).Conclusions Arthroscopic acromioplasty and rotator cuff repair can improve shoulder function in older patients with full-thickness RCTs.The long-term follow-up results will be studied further.

ABSTRACT

Objective To determine the incidence rate of HPV infection or multi-infections at different stages of cervical lesions in the development of cervical cancer , and the impact of specific types of HPV multi-infections on the risk of cervical cancer. Methods 103 samples of cervical tissues were detected and then divid-ed into ICC/HSIL group and LSIL/NILMF group according to the degree of pathological changes. HPV type was determined by PCR product sequencing. E6 nested multiplex PCR was performed to detect HPV multi-infections. Odds ratios were calculated to determinate the association between the sample category (LSIL/NILM or ICC/HSIL) and the specific types of HPV multi-infections. Results In HPV-positive samples, the rate of multi-in-fections had no significant differences between the two groups. Coinfection of HPV68 with HPV16 increased the risk of ICC/HSIL, as compared with HPV16 or HPV68 infection alone. Conclusions High-risk HPV coinfec-tions has a higher risk to induce ICC/HSIL than does HPV infection alone.

ABSTRACT

Objective:To evaluate the effect of down-regulation of Nav1.7 on the activation of astrocytes and microglia in DRG of rats with cancer pain, and explore the transmission of the nociceptive information. Methods:Lentiviral vector harboring RNAi sequence targeting theNav1.7gene was constructed, and Walker 256 breast cancer cell and morphine was injected to build the bone cancer pain model and morphine tolerance model in rats. Lentiviral vector was injected. Rats in each model were divided into 4 groups: model group, PBS group, vehicle group and LV-Nav1.7 group. The expression levels of GFAP and OX42 in dorsal root ganglia (DRG) were measured. Results: After the animal model was built, the level of Nav1.7, GFAP and OX42 was improved obviously with the time prolonged, which was statistically significant (P<0.05). The expression level of GFAP and OX42 in the DRG in the LV-Nav1.7 group declined obviously compared to the model group, PBS group and vehicle group (P<0.05).Conclusions:Intrathecal injection of Navl.7 shRNA lentiviral vector can reduce the expression of Nav1.7 and inhibit the activation of astrocytes and microglia in DRG. The effort is also effective in morphine tolerance bone cancer pain model rats.

ABSTRACT

Objective The article aimed to investigate the correlation of lipid profile levels with deoxyribonucleic acid (DNA) damage and total antioxidant capacity (TAC) levels in obese pregnant women. Methods Healthy pregnant women (n=120 ) who took routine prenatal care from August 2011 to August 2012 in our hospital were recruited .All the pregnant women were di-vided into normal weight group ( n =45 ) and obese group ( n=75 ) .The lipid profile levels , DNA damage and TAC levels of two groups were compared and analyzed , and the correlations among lipid profile levels , DNA damage and TAC levels were analyzed . Results Compared to normal weight group , obese group showed significantly higher levels of TC (P=0.000), TG(P=0.000), and LDL-c(P=0.004), but a lower level of HDL-c (P=0.006).The DNA damage and TAC level of obese group were obviously higher than those of normal weight group (P=0.000, P=0.000).The DNA damage was positively correlated with levels of TC , TG and LDL-c among obese pregnant women (r=0.23, P=0.026;r=0.26, P=0.008;r=0.19, P=0.032), and TAC level was positive-ly correlated with TG level (r=0.32,P=0.000). Conclusion Dyslipidemia, imbalance of prooxidant and antioxidant status al-ways occur to obese pregnant women .The DNA damage is positively correlated with levels of TC , TG and LDL-c among obese pregnant women, and TAC is positively correlated with TG level .

ABSTRACT

Objective To investigate the pathological features and clinical outcomes of the arthroscopic treatment for calcifying tendinitis of the supraspinatus. Methods From March 2009 to October 2010,34 patients with calcifying tendinitis of the supraspinatus were treated with shoulder arthroscopy. Based on size of calcifying deposits, they were divided into three groups: small (<10 mm), middle (≥ 10 mm, ≤20mm), and large (>20 mm). All patients underwent arthroscopic calcium excision and subacromial decompression, and acromioplasty of shoulder was performed in 8 cases, rotator cuff neoplasty in 10. The preoperative and postoperative clinical data such as Constant's score, Visual Analog Scale (VAS) pain score and radiographs were adopted for evaluation. Besides, histological and scanning electronic microscopic examinations of calcium deposits were also carried out. Results All patients were follwed up for average 11.5 months. The patients were regularly examined at a time point of 1 month preoperatively, 2 days preoperatively and of the latest follow-up. The average Constant score was 36.1 ±6.9, 55.6±12.4 and 89.7±2.7 respectively; the average VAS pain score was 8.2±0.8,7.03±0.7,1.7±0.3 respectively. Both scoring systems revealed a significant difference before and after surgery. However, the clinical outcomes of the calcified deposit (small, middle, and large) showed on significant difference among the 3 groups. Collagen fibers were not degenerated around the calcified deposit, and no apoptotic or dead cells could be found, therefore, it is not a pathological calcification. Conclusion Recognition of calcifying tendinitis and good timing of surgery are keys to good clinical results. Shoulder arthroscopy is a safe, effective and minimally invasive method for the treatment of calcifying tendinitis of the supraspinatus. The patients with various size of the calcium deposit treated with shoulder arthroscopy can lead to an excellent clinical outcome postoperatively.
