ABSTRACT

Objective To explore the safety and efficacy of dezocine preemptive analgesia in the reconstruction of nasal bone fracture.Methods From September 2016 to May 2018,63 patients with simple nasal bone fracture admitted to the 908th Hospital of PLA Joint Logistics Support Force were selected in the study.The patients were divided into two groups by random number table method :dezocine group ( n=32) and control group ( n=31).The dezocine group was injected with dezocine before surgery ,while the control group was injected with 0.9% sodium chloride solution.The differences of MAP,HR,SpO2 at different time points between the two groups were observed. And the difference of VAS/RSS during operation and 2 hours after surgery were also observed.Results There were no statistically significant differences in MAP ,HR and SpO2 between the two groups at different time points (10 min before and 10 min after intramuscular injection ) ( all P>0.05).During operation and 20 min after operation,the MAP values of the control group were (110.1 ±16.38) mmHg and (105.28 ±14.81) mmHg,respectively,which were higher than those of the dezocine group [(102.35 ±14.25) mmHg and (101.25 ±13.12) mmHg)], the differences were statistically significant ( t =2.259,2.153, all P <0.05).There were no statistically significant difference in HR between the two groups at 10 min before and 10 min after intramuscular injection(all P>0.05).The HR in the control group during operation and at 20min after operation were (90.81 ±19.52) times/min,(77.25 ± 18.25)times/min,respectively,which were faster than those in the dezocine group [(85.16 ±17.25) times/min, (71.36 ±16.15)times/min],the differences were statistically significant ( t=2.074,2.047,all P<0.05).There were no statistically significant differences in SpO 2 between the two groups at different time points ( all P>0.05).In the surgery,the RSS score of dezocine group [(4.3 ±0.6) points] was better than that in control group [(2.1 ± 0.4)points],the difference was statistically significant (t=17.008,P<0.05).The VAS scores in the control group during operation and 20 min after operation were (2.3 ±0.5) points,(1.5 ±0.8) points,respectively,,which were lower than those in the control group [(5.5 ±0.7) points,(2.1 ±0.6) points] (t=20.572,3.277,all P<0.05). Conclusion Application of dezocine preemptive analgesia in the reconstruction of nasal bone fracture is effective and safe,the hemodynamics is stable and the analgesic effect is definite in the surgery .

ABSTRACT

Objective To study the reason of concurrent styloid process syndrome after tonsillectomy and the clinical effect of styloidectomy.Methods Twelve patients with styloid process syndrome after tonsillectomy receiving styloidectomy were retrospectively analyzed.The clinical effect was evaluated.Results Ten patients were operated on unilateral styloid,two patient was operated on bilateral styloid.All patients were followed -up for 6 months to 5 years after operation.All of them were cured.Conclusion The mechanism of styloid process syndrome after tonsillectomy might be related to the stimulation of scar formation in the tonsillar fossa.Styloidectomy ia an effective method of treatment.

Hepatocellular carcinoma (HCC) is a malignant tumor with high morbidity and high mortality.Early diagnosis is the key for HCC treatment.DNA methylation is one of the important mechanisms of epigenetic genetics.To investigate the mechanism of DNA methylation can provide a theoretical basis for the diagnosis of HCC,and the methylation level of the specific genes can also be used as the biomarkers of HCC for the early diagnosis and prognosis evaluation.

ABSTRACT

Purpose To study the clinicopathological and immunohistochemical features, carcinogenesis and differential diagnosis of female pelvic serous carcinoma. Methods The clinical data, macroscopic and microscopic features and immunostaining results of 20 patients with pelvic serous carcinoma were studied, and some associated literatures were reviewed. Results 20 cases of PSC aged from 23 to 87, with the mean age being 58. 9. PSC may occur in fallopian tube, ovary and peritoneum, while they were referred to hospital because of abdominal distention, abdominal pain or pelvic mass. The tumor often invasive pelvic organs diffusely when they were diag-nosed, so, the primary site were difficultly determined. Usually, the primary focus of serous carcinoma of fallopian tube is small and easily planted in pelvic. The patients with ovarian serous carcinoma or peritoneal serous carcinoma had serous tubal intraepithelial car-cinoma at the mean time. Conclusions The tubal epithelial cells may be the major source of PSC. About the specimen of PSC, we need check the fallopian tube carefully to determine the primary site, and make differential diagnosis with peritoneal malignant mesothe-lioma and metastatic carcinoma from other sites than pelvic when it diffusely invasive peritoneum.

ABSTRACT

Objective Toinvestigateserumalanineaminotransferase(ALT)andtotalbilirubin(TB)concentrationsofchildrenin Panyu district of Guangzhou ,and establish the reference ranges for ALT and TB tests for healthy children of different gender and different age .Methods 4 065 children were chosen randomly ,and their serum ALT and TB were determined .After excluding the outliers ,the results were analyzed statistically .Results After excluding outliers ,a total of 4 000 children were enrolled in the stud-y ,including 2 241 males ,1 759 females ,aged 2- <9 years old .According to statistical analysis ,reference ranges were established . For male the ALT reference ranges were as follows ,2- <3 years old :5 .0-36 .0 U/L ,3- <4 years old :7 .0-21 .0 U/L ,4- <8 years old :6 .0-21 .0 U/L ,8- <9 years old :5 .0-24 .0 U/L ;TB reference ranges of male were as follows ,2- <3 years old :3-12μmol/L ,3- <7 years old :4-15 μmol/L ,7- <8 years old :4-16 μmol/L ,8- <9 years old :7-12 μmol/L .ALT reference range of female were as follows ,2- <3 years old :6 .0-30 .0 U/L ,3- <4 years old:7 .0-22 .0 U/L ,4- <9 years old :6 .0-19 .0 U/L ;female TB reference range ,2- <3 years old :3 .0-12 .0 μmol/L ,3- <4 years old :3 .0-13 .0 μmol/L ,4- <9 years old:4 .0-16 . 0 μmol/L .Conclusion Established the reference ranges of ALT and TB test for healthy children in Panyu district .

ABSTRACT

Objective To understand the children health care and the prevention of lead pollution in Panyu in the future by re-search on the detection result of blood lead from Panyu children during 2011 and 2012.Methods The detection of whole blood lead was by atomic absorption spectrometer from body examination in 6 308 children in Panyu Center Hospital be-tween 2011 and 2012:①The blood lead result from assigned children were divided into five age groups:infancy group (1~12 months,1 511 cases),toddler age group (1~2 years,1 558 cases),preschool age group (3~6 years,1 704 cases),school age group (7~11 years,1 076 cases)and adolescence group (12~18 years,459 cases).②Based on different age group,each group was separated by male and female.③According to the detection year,each group was also put into 2011 and 2012 re-spectively.Results The overall level of children blood lead in Panyu,Guangzhou in 2011 and 2012 was 37.01±29.18μg/L, and the total poisoning rate was 1.2 9%.Along with the growth of age,the overall level of children blood lead was in up-wards trend but the poisoning rate kept in a stable level beginning from infant stage.The blood lead level of boys in Panyu was 38.87±30.18μg/L,and the poisoning rate was 1.91%;the blood lead level of girls was 34.13±27.32μg/L,and the poisoning rate was 1.29%.The blood lead levels and the poisoning rates of boys were higher than girl’s in most of age groups.Conclusion Compared with other big cities like Guangzhou,the blood lead level of children in Panyu was declined but it’s not the standard performance,and there’s a certain extent of lead pollution.So in addition to reinforce pollution con-trol from government,parents and children should enhance the knowledge,education and precaution of lead poisoning.

Diabetes mellitus has become the third threaten diseases second only to cardiovascular disease and tumor.Many re-searches demonstrated that the microRNAs(miRNAs)in blood related to the occurrence and development of diabetes melli-tus.Because of the non-invasion and repeatability of circulating miRNAs,the circulating miRNAs have a broad application prospect of diagnosis and prognosis evaluation for various types of diabetes.This overview arms to introduce the source and characters of circulating miRNA,and also the progress of circulating miRNA related to diabetes mellitus,so as to lay a foun-dation for further research.

ABSTRACT

Objective To detect the opening of the myocardial mitochondrial permeability transition pores (MPTP) after intrauterine asphyxia in neonatal rats,and to explore the mechanism of the myocardial hypoxic-ischemic and reperfusion injury caused by the opening of MPTP after asphyxia.Methods Cesarean sections were undertaken in female SD rats at the 21st day after pregnancy.The uterine arteries were clamped for 30 minutes followed by releasing for 1 hour and the pups were allocated into the asphyxia group.The uterine arteries were isolated but not clamped and the pups were allocated into the control group.There were 30 neonatal rats in either group and all of them were sacrificed 24 h after birth.Serum cardiac troponin Ⅰ (cTn Ⅰ) levels were detected by enzyme linked immunosorbent assay.The opening degree of MPTP was detected by fluorospectro-photometry.Myocardial ischemic areas were detected by TTC staining.Tissues from the cardiac apex were taken and the pathologic changes of the myocardium were explored by hematoxylin-eosin staining.SPSS for Windows 13.0 was used for statistic analyses.Results In HE staining slices,the myocardial cells in asphyxia group were disarranged and edematous.In control and asphyxia group,the serum cTn Ⅰ levels were (0.08 ±0.04) μg/L and (0.40 ±0.29) μg/L (P <0.01),the myocardial ischemic areas were (8.01 ±3.48) % and (42.50 ± 15.90)% (P <0.01),and the opening degrees of MPTP were (118.10 ± 19.10) RFU and (79.40 ± 10.57) RFU (P < 0.01) respectively.The serum levels of cTn Ⅰ,the myocardial ischemic areas,and the opening degrees of MPTP were significantly increased in asphyxia group compared with control group.The serum level of cTn Ⅰ was positive correlated with the opening degree of MPTP in either group(r =-0.384,P < 0.01).Conclusion There are myocardial injuries in neonatal rats after asphyxia,which represent as high level of serum cTn Ⅰ,myocardial ischemia and necrosis.Opening of the myocardial MPTP is one of the causes of myocardial injury.

ABSTRACT

Objective To evaluate the efficacy and safety of tulobuterol patch in the treatment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD) in the elderly. Methods Totally 165 elderly patients with moderate to severe AECOPD were divided into trial group and control group randomly.83 patients of trial group were treated with tulobuterol patch,2 mg once a day,and fluticasone inhalation 250 μg twice a day; 82 patients of control group were treated with inhalation of salmeterol/fluticasone 250 μg/50 μg twice a day.Basic therapy was the same in two groups. Results After 10-14 days' treatment,forced expiratory volume in one second(FEV1),peak expiratory flow (PEF),6-min walking distance and symptom scores were ( 1.30 ± 0.31 ) L,(245.3 ± 56.1 ) L/min,(263.0±53.2)m,(33.2±12.1)scores in trial group,and (1.21±0.23)L,(213.9±58.4) L/min,(230.0±45.6)m,(37.8± 14.5) scores in control group,respectively.The lung function,6 minute walk distancc and symptom score were improved (t=2.120,3.521,4.279,2.212,all P<0.05).The frequencies of rescue medication,waking-up suffocating at night and the days of hospital stay were deceased significantly in trial group as compared with control group[(2.5 ± 0.6) time/d,( 1.8 ± 0.5)time/week,(12.9±1.6)dvs.(2.90.8) time/d,(2.2±0.7) time/week,(14.1±1.8) d,t=3.610,4.219,4.524,all P<0.05].The incidence of adverse reactions was not significant difference between two groups[8 cases(9.6%)vs.7 cases(8.5%),P>0.05] and the adverse reactions were mild.Conclusions Tulobuterol patch is a newly formulated,effective and safe medication for the treatment of acute exacerbation of AECOPD.

ABSTRACT

Objective To investigate the effects of tumor necrosis factor-α ( TNF-α ) on the resensitization of human lung cancer cell lines A549/DDP to cisplatin (DDP) and to explore the relationship between the expression of TNF-αand resistance-related protein (LRP) in lung tissue.Methods The cytotoxic effects of combinational treatment by TNF-α and cisplatin on A549/DDP were measured by MTT assay.The expression of LRP was assessed by immunocytochemistry methods.Results The IC50 of A549/DDP to cisplatin were decreased from 7.12 ng/L to 5.02 ng/L,4.41 ng/L respectively by 250 U/ml and 1000 U/ml TNF-α treatment ( P < 0.01 ),with the sensitivity of A549/DDP to cisplatin increased by 1.42 and 1.62 fold respectively.LRP was overexpressed in A549/DDP cell.250 U/ml or 1000 U/ml TNF-α plus cisplatin treatment down-regulated the expression of LRP with the positive rates of ( 60.14 ± 6.54 ) % and ( 57.23 ± 5.98 ) %respectively,which were significantly lower than that of cisplatin alone treatment ( 75.97 ± 5.32 ) % and control group (79.63 ± 4.78 ) % ( both P < 0.01 ).Conclusion Tumor necrosis factor-α can reverse the resistance of A549/DDP to cisplatin,which may be partially attributed to down-regulating LRP expression.

ABSTRACT

Objective To investigate the value of medical thoracoscopy in diagnosis of pleural effusion of unknown aetiology in aged people.Methods The patients aged 65 years and over,with exudative pleural effusion of unknown aetiology,were enrolled in this study.And they underwent medical thoracoscopy for diagnosis.Results The 49 patients,33 males and 16 females,aged 65-82years (at average age of 70.5 yeas),were enrolled.The 83.7% (41 cases) of pleural effusion was unilateral,and 16.3% (8 cases) was bilateral.The 28.6% (14 cases) of them suffered from tuberculosis,16.3 % (8 cases) malignant tumor.The pathology results of 16 cases showed nonspecific inflammation and normal pleural tissue.The other 10 patients showed a normal pleuracy or abnormal pleuracy undergoing a failure biopsy.Considering the clinical data of the 27 cases,8 cases (16.3 %)had infectious disease,18 cases (38.8%) remained unknown.Diagnostic accuracy of medical thoracoscopy was 61.2%.Complications of these patients undergoing medical thoracoscopy were fever (n=8,16.3%) and subcutaneous emphysema (n=7,14.3%).Conclusions Medical thoracoscopy is a standard option for diagnosing pleural effusion.It could be easily managed by physicians.The complications appear more often in aged people.

ABSTRACT

Objective To investigate the relationship between multi-slice spiral CT perfusion imaging and tumor angiogenesis and cell cycle protein D1 (cyclinD1) expression in patients with peripheral lung cancer. Methods Seventy-three patients with peripheral lung cancer underwent 16-slice spiral CTperfusion imaging. The CT perfusion imaging were analyzed for time density curve (TDC), perfusion parametric maps and the respective perfusion parameters (BF, BV, PS, PH, and PHpm/PHa). Correlation between the respective perfusion parameters and immunohistochemical findings of MVD measurement and cyclinD1 expression were evaluated. Statistical method used one factor analysis of variance (One-wayANOVA) and Pearson correlation analysis. Results The shape of TDC of three groups of peripheral lung cancers (adenocarcinoma, squamous cell carcinoma, and other type of cancer) was similar. They showed a steeper slope, was obvious increase of nodular HU value [ peak height of TDC is respectively (44. 87±6. 83), (34. 91 ± 8.05), (40. 66±5. 87) HU ], changed little after reaching peak height, became flat at peak, had a platform. Forty-four patients were cyclinD1 positive expression, the peripheral lung cancers with cyclinD1 positive expression showed significantly higher MVD value than that of the peripheral lung cancers with cyclinD1 negative expression [ respectively (33.88± 14. 81), (23. 17±11.66) band/high magnification, P < 0. 01]. The cancers with cyclinD1 positive expression showed significantly higher PH,PHpm/PHa, BF,BV, PS value than those of the cancers with cyelinD1 negative expression[respectively P <0. 05]. MVD was positively correlated with PH, PHpm/PHa, BF, BV, and PS of the three groups ofcancers with cyclinD1 positive expression. Among, the correlation coefficient (r value) of BV, PS, BF was0. 409, 0. 517,0. 503 respectively(all P < 0. 01). MVD was significantly correlated with them. The r valueof PH, PHpm/PHa is 0. 319,0. 324 respectively(all P < 0. 05). MVD was less correlated with them. MVDwas not correlated with PH, PHpm/PHa, BF, BV, and PS of the three groups of cancers with cyclinD1 negative expression. Conclusions Multi-slice spiral CT perfusion imaging moderately correlated with tumorangiogenesis and reflected MVD measurement and cyclinD1 expression. It provided not only a noninvasive method of quantitative assessment for blood flow patterns of peripheral lung cancer but also an applicable diagnostic method for peripheral lung cancer.

ABSTRACT

We first cloned the dsz operon of Pseudomonas delafieldii R-8 into the expressing plasmid (pPR9TT) to construct the recombinant plasmid pPR-dsz, and then reintroduced it into strain R-8 to obtain a muti-copy dsz operon engineering strain R-8-1. Compared with the wild-type, strain R-8-1 showed a higher desulfurization activity for dibenzothiophene (DBT). Initial rates of DBT removal by strain R-8-1 were 6.25 micromol/g dry cell/h, about 2-fold higher than that for wild-type strain. The recombinant cells were also applied in the desulfurization of diesel. It resulted in a 68% reduction of total sulfur from 310.8 mg/L to 100.1 mg/L, whereas only 53% of sulfur was removed by strain R-8. The stability of pPR-dsz in strain R-8-1 was studied. The results revealed the first obtain a muti-copy dsz operon engineering strain are helpful for further development in biodesulfurization.

ABSTRACT

OBJECTIVE To evaluate the efficacy and safety of moxifloxacin in treating lower respiratory infection.METHODS Totally 102 patients were enrolled in this study,but 3 cases were infixed or weeded.Fifty cases of trial group were given 400mg of moxifloxacin intravenously,once a day for 7-14 days,49 cases of control group were given 400mg of levofloxacin intravenously,once a day for 7-14 days.RESULTS The clinical efficacious rates of moxifloxacin and levofloxacin were 94.0% and 79.6%,respectively.The bacterial clearance rates of moxifloxacin and levofloxacin were 94.3% and 77.8%,respectively.There was significant difference(P0.05) between two groups.CONCLUSIONS Moxifloxacin injection is an effective and safe antibiotics for the treatment of lower respiratory tract infection.

ABSTRACT

Objective To study the gender differences in echocardiography in essential hypertensives. Methods Echocardiography measurement was performed in 108 subjects with Grade 1-2 essential hypertension (52 in female patient group, PGf, and 56 in male patient group, PGm). Forty two normotensive subjects (20 in female control group, CGf, and 22 in male control group, CGm) were as controls.Data were obtained by averaging measurements of the traced heart chambers and velocity curves in 5 cardiac cycles. Results Compared with PGm, the following echocardiographic features in PGf were showed:LAID (37.24±5.88 vs 32.14±3.80)mm,P<0.01] and MVa[(84.18±12.13 vs 81.71±12.30)cm/s, P<0. 05] were greater; LVMI [ ( 119.26 ± 22.33 vs 128.17 ± 27.00 ) g/m2 , P<0. 05], EF ( 75.13 % ±6.69% vs 83.00% ±3.68%,P<0. 01), FS (41.67% ±7.99%0 vs 49.03% ± 7.35%, P<0.01), MVe[(68. 28±8.66 vs 73. 73±11.46)cm/s, P<0. 05] and MVe/a(0.83±0. 08 vs 0. 93±0.11, P<0.01)were lower. The differences between CGf and CGm were not significant. Conclusion There are echocardiographic differences between sexes in hypertensives including cardiac structural and functional changes. Hypertensive woman is more susceptible to both cardiac structure damage and cardiac dysfunction.

<p><b>BACKGROUND</b>To study the effect and safety of tissue specific gene therapy of suicide gene for lung adenocarcinoma.</p><p><b>METHODS</b>Retroviral vector of G1CEACDNa contained a carcinoembryonic antigen (CEA) promoter regulated cytosine deaminase expression cassete (CEA/CD). By means of infection of virus, tissue specific expressing vectors and non-specific expressing vectors were transfected into A549 cell, which was CEA-producing lung adenocarcinoma cell line and then xenografted in nude mice, and the anti-tumor effect was evaluated. Then the retrovirus was injected directly into the tumor mass on nude mice, and the sensitivity of the xenograft to G1CEACDNa/5-fluorocytosine (5-FC) and the side effects were observed.</p><p><b>RESULTS</b>(1) After transfected and untransfected A549 cells were implanted into nude mice, there was no difference in tumor formation among all the groups. (2) After 5-FC administration, the tumor transfected with tissue-specific gene displayed a higher sensitivity to the drug than those treated with non-specific in vitro gene-therapy. (3) The tumor-bearing nude mice were randomized in a blind manner based on comparable size to receive the supernatant of recombinant retrovirus G1CEACDNa followed by 5-FC, and significant growth suppression could be observed. (4) Comparing to the group with injection of 5-fluorouracil (5-FU) alone, tissue-specific suicide gene therapy showed lower suppression to bone marrow.</p><p><b>CONCLUSIONS</b>The results suggest that tissue-specific suicide gene therapy may play an important role in individual treatment of lung cancer.</p>

ABSTRACT

Objective:To investigate the potential of differentiating into myocytes of the granulocyte colony stimulating factor(G CSF) and Simvastatin mobilized bone marrow stem cells. Methods:After injection of isoprenaline(ISO) interaperitoneally to develop acute ischemic model, rat bone marrow stem cells were mobilized by G CSF and dextran sulfate sodium salt and migrated to the site of myocardial infarction. Hearts were harvested from 24 hours to 24 weeks after administration of ISO for histopathological examination.Immunohistochemisty?HE and VG stain were used to detect infiltration of CD34 + monocytes and the regeneration of myocytes and the inhibition of fibroatrophy of ischemic myocardium.Results:24 hours after administration of ISO,a large amount of infiltrative monocytes and regenerative myocytes which were CD34 positive expression could be found in the infarct zones of the G CSF treatment group,while majority of the infiltrative inflammatory cells in control group were neutrophils and there was no infiltrative cells and myocytes which were CD34 positive expression,4 weeks after administration of ISO,there were a plenty of scar in control group,but not in the treatment group.Conclusion:Rat bone marrow stem cells were mobilized by G CSF and Simvastatin,and migrated to the site of myocardial infarction,and differentiated into viable cardiomyocytes or vascular endothelial.The heart function remarkably was improved by regeneration of myocytes,inhibition of fibroatrophy and protection of ischemic myocardial structure.

ABSTRACT

Objective This study reports the early and late results of percutaneous transluminal angioplasty (PTA) and intravascular stenting for atherosclerotic stenosis of the iliac artery. Methods From December 1994 to February 2000, we performed iliac artery angioplasty and primary intravascular stent placement in 42 vessel of 31 patients (23 men and 8 women; mean age, 70.5 years). A total of 44 self expanding Wallstents ( Boston Schneider, Inc. ) were deployed. All patients underwent angioplasty and stent placement successfully. Results Clinical follow up was available for all patients at a mean of (36?13) months. All patients with clinical ischemic category (claudication or rest pain) were improved. Angiographic follow up with completed data are available for 20 of 31 patients at a mean follow up of (10?5) months. The iliac artery restenosis rate was 2%(1/42). The patients again underwent balloon angioplasty successfully. Conclusions PTA and primary stenting of the iliac arteries are effective and safety for treating iliac arterial stenosis,and should be the method of first shoice.
