ABSTRACT

Objective@#To analyze the prevalence of elevated blood pressure among primary and middle school students in Anhui Province in 2022, as well as its influencing factors, so as to provide scientific evidence for the development of effective measures to prevent and control elevated blood pressure among children and adolescents.@*Methods@#Using data from the 2022 "Monitoring of Common Diseases and health Influencing Factors of Students" in Anhui Province, a total of 210 360 primary and middle school students aged 7 to 17 with complete information. Statistical analysis, including independent sample t tests, analysis of variance, and Chi square tests, were conducted to compare blood pressure differences and prevalence rates among students with different characteristics. Generalized linear mixed models were established to analyze the influencing factors of elevated blood pressure in children and adolescents.@*Results@#The overall prevalence of elevated blood pressure among primary and middle school students in Anhui Province was 22.3%, higher among high school students (25.6%) and junior high school students ( 23.7%) than elementary school students (20.4%) ( χ 2=528.49, P < 0.01 ); higher among obese(33.3%) and overweight students (24.9%) than that of normal students (18.6%) ( χ 2=3 704.52, P <0.01), higher among rural students (23.1%) than urban students (21.4%) ( χ 2=83.39, P <0.01), and higher in the northern part of Anhui Province (26.9%) than in the central part (21.9%) and southern part (17.6%) ( χ 2=1 777.51, P <0.01). The generalized linear mixed models revealed that girls, high school stage, overweight, obese, living in the northern part of Anhui Province, and living in rural were all positively correlated with elevated blood pressure( OR =1.11,1.25,1.47,1.45,2.24,1.21,1.67, P <0.01).@*Conclusions@#The prevalence of elevated blood pressure among children and adolescents in Anhui Province is the highest in the country. It is recommended that relevant departments promptly formulate corresponding measures to curb the increasing trend of elevated blood pressure and protect the physical and mental health of children and adolescents.

ABSTRACT

Epidermal growth factor receptor (EGFR) mutations are the most common driver genes in the development of non-small cell lung cancer (NSCLC), of which mutations in exons 18-21 are frequent, especially the loss of exon 19 and exon 21 L858R mutation are the most frequent. Other rare gene mutations are rare. Simultaneous occurrence of two or more rare EGFR mutations are extremely rare in lung cancer, and the incidence of EGFR L833V/H835L rare gene compound mutations is very low, and there is little clinical data and evidence of relevant treatment methods. Some EGFR-tyrosine kinase inhibitors (EGFR-TKIs) are effective in treating lung cancer patients with rare gene mutations. In this article, we reported a case of NSCLC patient with a rare gene compound mutation EGFR L833V/H835L, who responded to Afatinib in combination with Anilotinib treatment well after 5 months of treatment, and computed tomography (CT) showed shrinkage of lung lesions. Meanwhile, we also compiled previously reported NSCLC patients with EGFR L833V/H835L rare gene compound mutation and summarized the characteristics of this group of patients and the effect of applying different kinds of EGFR-TKIs treatment.

ABSTRACT

In clinical practice, early diagnosis, accurate assessment and effective management of rare skin diseases are difficult. The Big Data gives rise to the exponential growth of biomedical data, including medical images, multi-omics information and electronic health records. Artificial intelligence (AI), particularly machine learning, has its advantage in processing complex and abundant information. Researches have applied AI in the field of rare skin diseases. In this paper, we briefly describe, discuss, and foresee the research on AI based image data, multi-omics data & text data and AI in assisting rare skin disease drug development, in order to improve the awareness of dermatologist understanding of this field and actively promote the development of AI usage on rare skin diseases.

ABSTRACT

Disfiguring dermatoses are a group of cutaneous diseases that affects the appearance and mental health of patients to various extents. Patients usually have high expectations for the treatment outcome, and there is an urgent need for convenient and accurate non-invasive auxiliary evaluation tools for better diagnosis and treatment. With the development of high-frequency ultrasonography, more and more skin diseases, including disfiguring dermatoses, have been objectively and accurately assessed by this technology. This review summarizes research progress in the application of high-frequency ultrasonography in the diagnosis and treatment of some disfiguring dermatoses.

ABSTRACT

Objective:In this study, lengthening over nail (LON) and plating after lengthening (PAL) were compared by the process of treatment and therapeutic effect in the children's femoral lengthening cases,to guide treatment choices; and in the PAL group there was another comparison between different timeline of plate fixation after lengthening to formulate reasonable technical route.Methods:Since January 7, 2014 to September 10, 2018, a total of 21 cases of LON and PAL surgery had been carried out, including 12 males and 9 females. The average age at their operation was 12.43±3.12 years. There were 10 patients in the LON group and 11 patients in the PAL group, in the PAL group 5 cases was synchronous fixation, which means removal of the external fixator and plate fixation would be performed in the same surgery, 6 cases was asynchronous fixation, which means plate fixation would be performed at least 2 weeks later after the removal offixator to reduce the risk of infection. The recorded data includesgender, date of birth, reason of discrepancy and treatment history, age at surgery, lengthening distance, the length of duration with frame, healing time, total duration time and total blood loss of seriesoperations, ROM of the knee joint, complications, etc.Statistical analysis was made by SPSS 22.0 software, and then reasonable treatment strategies were proposed.Results:In the LON group, the medianlengthening distance was 7.10 (6.63, 7.70) cm, the median total duration time was 300 (240, 330) min, the medianspeed of healing was 1.12 (0.78, 1.72) month/cm, complications were found in 3 cases, the median ROM of knee joint was 125.00° (117.50°, 150.00°); in the PAL group, the data respectively were 6.00 (5.00, 8.00) cm, 260 (230, 465) min, 1.00 (0.75,1.71) month/cm, 3 cases, 100.00° (90.00°, 150.00°), the difference between them was not statistically significant (all P>0.05). 2. In the PAL group, the difference of number of surgeries, total duration time, total blood loss, complication rate, healing speed and postoperative function between synchronous and asynchronous fixation was not statistically significant (all P>0.05). Conclusion:They are similarbetween LON and PAL techniques in the process of treatment and therapeutic effect. The surgeons could make choices according to the growth potential of the patients and their own custom operation methods. In the case of the current sample size, there is no statistical difference in the therapeutic effect between the synchronous and asynchronous plate fixation in the PAL method, but the sample size needs to be increased for further study.

ABSTRACT

Objective:In this study, the feasibility of this method was evaluated in children with large distance femoral lengthening cases, and the acute correction and staging surgeries were compared at healing speed. At the same time the key points of surgical techniques would be summarized, so as to clarify the application value of this technology in children patients.Methods:Since July 7 of 2014 to January 16 of 2018, a total of 10 cases of Lengthening Over Retrograde Nail (LORN) surgery have been formulated and carried out, including 9 males and 1 females.The average age of the operation was 13.10±2.18 y. In all cases, the osteotomy point was located in the metaphysis of the distal femur, and the records from the first meeting to the end of follow-up included gender, date of birth, reason of discrepancy and treatment history, age at surgery, lengthing distance, the length of duration with frame, healing time, ROM of the knee joint, complications, et al. To compare the healing time of acute correction and staging surgery. Statistical analysis was made by SPSS 22.0 software, and then reasonable treatment strategies were proposed.Results:The average lengthing distance of 7.07±1.01 cm. The median time of the duration with frame was 8.5 (4,16) months, and the median follow-up time was 39 (34, 54) months. All cases obtained good clinical and imaging healing (1.70±1.10 month/cm vs. 1.16±0.54 month/cm), and all regained to normal walking function (136.67°±20.82° vs. 125.71°±26.37°), without the occurrence of fracture after therapy. There were no statistically significant differences in the speed of healing, range of joint motion or complication rate between acute correction and staging treatment. Conclusion:It is feasible for femoral lengthening surgery assisted by retrograde intramedullary nail in the treatment of femoral shortening deformityin children. The correction of mild angular deformity and lengthening surgery can be performed simultaneously. The osteotomy site was proposed at distal metaphysealof the femur for better quality of osteogenesis and shorter duration for fixator. It is safe to osteotomy and lengthening in the area of enchondroma.

ABSTRACT

Endoglucanase (EG) is an important component of cellulases and play an important role in cellulose degradation. However, its application is limited due to the low yield of endoglucanase from natural microorganisms. Efficient heterologous expression of endoglucanase is an effective way to solve this problem. To obtain the engineered Saccharomyces cerevisiae for high-yield endoglucanase, endoglucanase gene was cloned from Clostridium cellulovorans, with a total length of 1 996 bp, encoding 440 amino acids, and the complete expression cassette (PαEGC) was constructed with the PGK promoter sequence from Saccharomyces cerevisiae, α-signal peptide sequence from pPIC9K plasmid and CYC1 terminator sequence from pSH65 plasmid by gene splicing by overlap extension PCR (SOE PCR), and the expression vector of endoglucanase in Saccharomyces cerevisiae was constructed by rDNA integration. The relationship between copy number and protein expression was explored. Random multicopy expression of endoglucanase was performed in Saccharomyces cerevisiae. The copy number of endoglucanase was identified by Droplet Digital PCR and explore the relationship between copy number and protein expression.The engineered Saccharomyces cerevisiae of endoglucanase with copy numbers of 1, 3, 4, 7, 9, 11, 15, 16, 19, 21, 22 and 23 were obtained by rDNA integration, respectively. The results showed that when the copy number was 15, the enzyme activity was the highest, namely 351 U/mL. The engineered strain of Saccharomyces cerevisiae for endoglucanase was successfully constructed, which can provide reference for the heterologous expression of other industrial enzymes.

ABSTRACT

OBJECTIVE:To establish an effective technical system to achieve the informatization management in drug produc-tion process,and provide reference for the drug informatization management during production process in China. METHODS:Through analyzing the common problems existing in informatization management in drug production process,above problems were solved by interactive integration marketing system(IIMS)based on mobile internet. Informatization supervision was conducted for the production progress of a batch of drugs in a pharmaceutical production enterprise,and feasibility of the technical system was in-vestigated. RESULTS:There were problems commonly existed in the production process,such as supervision and data collection out of touch,no advanced means,delayed and closed informatization transmission,high cost of informatization collection,hard to develop,etc. IIMS consisted of transit label,informatization collection terminal and monitoring management platform had achieved batch production records paperless data interaction and solved informatization interaction and supervision issues in process of drug quality. In the actual study,IIMS totally supervised 15 links from raw materials purchasing to finished products,and all data were successfully collected. IIMS achieved the real-time remote control of data to make the government easy to regulate,without chang-ing the operational processes,and did not cause economic burden to the enterprises. CONCLUSIONS:The technical system is suit-able for the informatization management in drug production process in pharmaceutical production enterprise,and it can be extended to the scientific research institutes and drug supervision departments in the further.

ABSTRACT

OBJECTIVE:To provide reference for strengthening drug regulation in drug administration department. METH-ODS:Drug regulatory statistical yearbooks during 2011-2015 were collected. Literature analysis,content analysis,comparative analysis and secondary analysis were conducted to analyze and explore the drug production license,business license,advertising ap-proval,protection of TCM varieties,drug complaints,investigation and punishment of drug cases,etc. in statistical yearbook. RE-SULTS & CONCLUSIONS:The number of drug manufacturers and operating enterprises had been increasing year by year,while the retail chain stores in certified enterprises showed tendency to go beyond the single retail pharmacy. Compared with the down-ward trend of number of approved drug advertisements,the number of protected varieties of TCM decreased more obviously. The increase in number of drug complaints did not cause the number of investigated and punished drug cases at the same time,which showed a downward trend. It is suggested to further improve the quality and efficiency of drug regulatory work in China by strict drug production license approval,reforming drug advertising regulation and TCM varieties protection system,intensifying efforts to investigate and punish drug complaints and cases,and playing the supervision of public opinion role of the media and the masses.

ABSTRACT

To establish the quality standard for Xiaobai capsules. Methods: Paeoniae Radix Alba and Angelicae Sinensis Radix were identified by TLC. The contents of psoralen and isopsoralen were determined by HPLC. Results:The qualitative i-dentification was with high resolution and without interference from the negative substances. The linear range of psoralen was 1. 082-108. 200 μg·ml-1 . The average recovery was 99. 82% with RSD of 1. 16%. The linear range of isopsoralen was 0. 818-81. 800μg· ml-1 . The average recovery was 99. 85% with RSD of 1. 81%. Conclusion:The method can be used in the quality control of Xiaobai capsules.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effects of periacetabular osteotomy (PAO) for developmental dysplasia of the hip in adolescent.</p><p><b>METHODS</b>Twelve hips in 9 adolescent patients who underwent a Bernese periacetabular osteotomy for symptomatic or asymptomatic developmental dysplasia of the hip were analyzed. The average age of the patients at the time of surgery was 12.9 years(11-14 years). The Harris hip score and overall patient satisfaction with surgery were used to assess hip function and clinical results. Plain radiographs were used to assess the correction of the deformity and to observe progression of degenerative changes.</p><p><b>RESULTS</b>The average duration of clinical follow-up was 88.5 (60-136) months. The mean Harris hip score increased from 89 ± 6 preoperatively to 97 ± 3 at the time of the most recent follow-up (t = -6.754, P = 0.000). All patients (12 hips) had an excellent clinical result. The lateral center-edge angle of Wiberg increased from 4° ± 13° preoperatively to 36° ± 7° at the time of the most recent follow-up (t = -11.677, P = 0.000). The acetabular roof obliquity decreased from 28° ± 10° preoperatively to 2° ± 8° at the time of the most recent follow-up (t = 9.038, P = 0.000). The acetabular-head index increased from 54% ± 11% preoperatively to 89% ± 13% at the time of the most recent follow-up (t = -11.137, P = 0.000). The hip center was translated medially. Improvement of cystic degeneration of the acetabulum were found in 4 hips. Remodeling of aspherical uncongruence were found in 5 cases. Crossing sign were found in 2 hips postoperatively without symptoms of impingement.</p><p><b>CONCLUSIONS</b>PAO can provide comprehensive deformity correction and improve hip function in treatment of developmental dysplasia of the hip in adolescence. Some of the patients have improvement of cystic degeneration and remodeling of the hip.</p>

ABSTRACT

Triptolide, a compound extracted from the traditional Chinese medicine preparation of Tripterygium wilfordii Hook F., has been reported to have anti-inflammatory and anti-cancer activities. However, its effect on ovarian cancer invasion is unknown. We observed that MMP7 and MMP19 expression increased in ovarian cancer tissue. Triptolide treatment inhibited the migration and invasion of ovarian cancer cells SKOV3 and A2780 at the concentration of 15 nM. We also observed that triptolide suppressed MMP7 and MMP19 promoter activity in a dose-dependent manner, down-regulating the expressions of these promoters on mRNA and protein level. Moreover, triptolide enhanced E-cadherin expression in ovarian cancer cells. In vivo, triptolide inhibited tumor formation and metastasis in nude mice, and suppressed MMP7 and MMP19 expression; it also enhanced E-cadherin expression in tumor in a dose-dependent manner. Over expression of MMP7 and MMP19, or suppression of E-cadherin expression partially abolished the inhibitory effect of triptolide on invasion of ovarian cancer cells. To summarize, triptolide significantly inhibited the migration and invasion of ovarian cancer cells by suppression of MMP7 and MMP19 and up-regulation of E-cadherin expression. This study shows that triptolide is a good candidate for the treatment of ovarian cancer and reduction of metastasis.

ABSTRACT

<p><b>OBJECTIVE</b>To establish a new method for the identification of Schisandra sphenanthera and S. chinensis.</p><p><b>METHOD</b>Random amplified polymorphic DNA-Sequence characterized applied region (RAPD-SCAR) method was applied to screen primers.</p><p><b>RESULT</b>Screening from 100 primers, only 2 random primers, which can be used to identify S. sphenanthera and S. chinensis accurately with a good reproducibility. It worked to fit them into sequence characterized applied region.</p><p><b>CONCLUSION</b>RAPD-SCAR can be used to identify S. sphenanthera and S. chinensis accurately.</p>

ABSTRACT

BACKGROUND: The determination of the normal acetabular spatial contour, the pattern of its curvature transformation, and adaptability between acetabulum and femoral head plays an important role in the description of spatial relationship of the hip joint the management and evaluation of hip disorders, especially developmental dysplasia of the hip.OBJECTIVE: Based on spiral threa-dimensional CT data of normal hip joints, to create isoline map of each acetabulum and femoral head by a well-developed and cooperating programmed computer software so as to determine the normal acetabular spatial contour and the pattern of its curvature transformation.METHODS: Three-dimensional CT data of 56 normal hip joints, aged from 6 to 17 years, were collected. After three-dimensional simulation and surface shade display (SSD) reconstruction by a well-developed VxWork software, the isoline map was created using contours map in Gecmorphology, and each isoline map of the normal acetabulum and femoral head was analyzed via a specific computer software (Terrain). RESULTS AND CONCLUSION: The isoline array of normal acetabulum revealed the modality of some concentric "C" or oval "O",with at least one close "O" at the central zone, indicating that there existed dome shape at the superior zone of acetabulum. The modality of sparse isolines around dome illustrated larger curvature radius and more slowgoing change of spatial contour transformation of this zone, while the dense isolines on the bottom zone Of acetabulum represented a small curvature radius,steep shape, and rapid changing profile of this zone. The isoline array of normal femoral head was multiple concentric "O" with different space between each line. The congruence isolines of acetabular dome and superior surface of femoral head emerged as the similar trend of geomorphological characteristics, revealing that both the spatial contour and the curvature pattern between acatabulum and femoral head are well-adaptable to each other. Results show that the isolines of acatabulum and femoral headcan stersoscopicly visualize the spatial contour and curvature pattern of the topographic surface of the hip joint.

ABSTRACT

BACKGROUND:The etiological factor of pediatric avascular necrosis of the femoral head(Legg-Calv?-Perthe disease) is unclear right now,and the disease typing,treatment and prognosis outcome are also different.Thus a simple,practical and reliable clinical classification is necessary to guide treatment and estimate result.OBJECTIVE:To summarize and analyze the criteria commonly used for Legg-Calv?-Perthe disease in clinical typing as well as their advantage and shortcoming,which provides evidences for the treatment and instruction.RETRIEVAL STRATEGY:A computer-based search of Medline database was undertaken to identify the articles regarding the classification criteria for Perthes disease,published between January 1970 and September 2007,with the key items of "Perthes,classification" in English.After the primary check,the full-texts of the remained articles were looked for.Inclusion criteria:①X-ray,bone scanning and MRI classification criteria for Perthes disease.②the study of the correlation between the classification reliability and the prosthesis.Exclusion criteria:repeated researches.Totally 76 articles about the classification criteria for Perthes disease were retrieved,and 32 of them were in accordance with the inclusion criteria.LITERATURE EVALUATION:Among 32 included articles,24 ones concerned with X-ray examination,4 with bone scanning image,and 4 with MRI image.DATA SYNTHESIS:There were many related researches to the clinical classification of Legg-Calv?-Perthe disease,but uniform conclusion is absent.A reliable and unified clinical classification is beneficial for the orthopaedic physicians to find out the risk factors at early stage,and take therapeutic measures as soon as possible,so as to prevent the malformation of the femoral head.Ideal classification criteria with good reliability and consistency,are closely related to the disease prosthesis,and should be used at early stage.CONCLUSION:X-ray typing is commonly used in the clinical classification for Legg-Calv?-Perthe disease,but it is difficult to apply at early stage.Bone scanning can forecast results,and also induce irradiation.Due to good sensitivity and high resolution,MRI has an earlier effect on detecting necrosis and collapse,and is potential for further development and application,however,it also calls on a long-term clinical follow-ups.

ABSTRACT

[Objective]To investigate the effect and complications of treating radial neck fractures in children with elastic stable intramedullary nail(ESIN).[Method]A retrospective study was applied in 35 children(5~13 years) with displaced radial neck fractures,that were corrected with ESIN in our hospital from 2005 to 2008.[Result]All cases were successfully followed up,with an average time of 7.5 months.Ninteen cases were evaluated as excellent,6 cases were good,the excellent and good rate was 68%.Avascular necrosis was found in 9 cases,including 4 cases of radial head bulk formation,1 case showed ossification and delayed union was proved in 2 cases.[Conclusion]Treating the radial neck fracture with ESIN is a simple and minimally invasive method,the overall effect is good while the complications are limited.

ABSTRACT

Objective To develop converting rule from international classification of disease (ICD) codes of trauma to abbreviated injury scale (AIS) values so as to provide clues for a simple and convenient trauma scoring way. Methods The corresponding set relations were made for ICD-9CM codes and AIS-90 codes with similar code meanings in the trauma diseases. Based on medical theories and knowledge, 8 indeces closely associated with injury severity were worked out for each ICD-9CM code of trauma diseases by using the database of discharge abstracts of trauma patients. Discriminant analysis was used to set up corresponding relations of ICD-9CM codes with AIS values. Results Seven indices were selected for discriminant function. Corresponding relations of all ICD-9CM codes of trauma diseases with AIS values was made through nonparametric discriminant method. ABSTRACT

Experiments in vitro demonstrated for the first time that an EtOAc extract of roots of Rubus crataegifolius Bge.can markedly inhibit the for mation of lipid peroxide in rat brain,heart. liver and kidney tissucs. When administered intragastrically for 15 successive days,the EtOAc extract markedly inhibited the formation of lipid peroxide in plasma and tissues of liver and brain of mice. The above results showed that the EtOAc extract from roots of R. crataegifolius Bge.had anti-lipid peroxidation effect.

ABSTRACT

Objective To develop a new comprehensive trauma score method based on international classification of diseases(ICD) and evaluate the effectiveness of its use in cerebral injured patients.Methods The model of the new trauma score method was constructed by referring to those of some certain comprehensive trauma score methods generally used around the world.Receiver operating characteristics(ROC) analysis was employed to evaluate the effectiveness of outcome prediction for 355 cerebral injured patients using the new developed trauma score method.Statistical comparisons were performed between the AUC for the new trauma score values and that of NISS values derived from two professional raters.Results The area under the curve(AUC) for the new trauma score values was as large as 0.925 4,whose 95% CI ranged from 0.863 4 to 0.987 5,and is statistically different from 0.5(P

ABSTRACT

Objective To investigate the effects of hyaluronan(HA) on phagocytosis of macrophages and to explore the mechanism.Methods CD44 of macrophages was labeled with immunochemical method.In phagokinetic track assay,the areas without gold particles around the cells were measured with an image analyzer,shape of the cells was viewed under a scanning electron microscope.In fluorescent bead phagocytosis assay,the phagocytosed beads were conunted,the distribution of intracellular F-actin was viewed.In HA-coated bead phagocytosis assay,the phagocytosed beads were counted,shape of the cells was viewed with scanning electron microscope,the level of the intracellular Ca~(2+) was analyzed with confocal laser scanning microscope when the beads were phagocytosed.After treatment with anti-CD44 antibody,the changes of phagocytosis of the cells were examined.Results CD44 expressed on macrophages.The phagocytosing cells were round,their lamellipodia and filopodia were short.The membrane of the cells formed cup-like invagination when they engulfed particles and beads.In HA group,the phagocytosed gold particles and fluorescent beads increased.There was rich F-actin in the area where the fluorescent bead was engulfed.In HA-coated group,number of the phagocytosed HA-coated beads was great,the level of the intracellular Ca~(2+) in the area where the bead was engulfed increased significantly.After treatment with anti-CD44 antibody,the phagocytosed gold particles,fluorescent beads and HA-coated beads decreased.Conclusion HA promotes phagocytosis of macrophages,CD44 mediates effects of HA on phagocytosis of the cells.
