ABSTRACT

Copper is an essential metal element for human metabolism. Copper homeostasis can lead to inherited disorders of copper metabolism and also play an important role in tumor proliferation. In recent years, great progress has been made in the research and development of radioactive metal nuclide tracers, and molecular imaging quickly becomes a new inspection technology for detecting copper metabolism. 64CuCl 2 carries out PET imaging diagnosis of diseases with abnormal copper metabolism and tumors with high copper metabolism through real-time tracking of changes of copper ions in the body. At the same time, β - decay and electron capture of 64CuCl 2 have a therapeutic effect, and can be directly used for tumor nuclide therapy. Therefore, 64CuCl 2, as an integrated radiopharmaceutical for diagnosis and treatment, is the current research focus of molecular imaging. This article reviews 64CuCl 2 preparation, biological metabolism and its application in the diagnosis and treatment of abnormal copper metabolism diseases and tumors.

ABSTRACT

Objective:To evaluate the effect of intrathecal insulin-like growth factor-1 (IGF-1) on chemotherapy-induced neuropathic pain (NP) in mice.Methods:Forty clean-grade healthy male C57 mice, aged 7-9 weeks, weighing 22-24 g, were divided into 4 groups ( n=10 each) using a random number table method: control group (group C), chemotherapy-induced NP group (group CIPN), low-dose IGF-1 group (group I1) and high-dose IGF-1 group (group I2). In CIPN, I1 and I2 groups, oxaliplatin 5 mg/kg was intraperitoneally injected for 5 consecutive days to establish chemotherapy-induced NP model.Normal saline 0.2 ml was given in group C. After measurement of the pain threshold at 10 days after establishment of the model, IGF-1 0.5 and 1.0 μg were intrathecally injected in group I1 and group I2, respectively.Normal saline 5 μl was intrathecally injected in C and CINP groups.Mechanical withdrawal threshold (MWT) was measured at 3, 5, 8, 10, 11, 13 and 15 days after establishment of the model.After measurement of the pain threshold at 15 days after establishment of the model, the expression of spinal IGF-1, IGF-1receptor (IGF-1R), interleukin (IL)-17A, IL-1β and tumor necrosis factor (TNF)-α was detected, and IGF-1 positive cells were counted using immunofluorescence. Results:Compared with group C, MWT was significantly decreased, the expression of spinal IGF-1 was down-regulated, the count of IGF-1 positive cells was decreased, and expression of IL-17A, IL-1β and TNF-α was up-regulated at 3-25 days after establishment of the model in CINP, I1 and I2 groups ( P<0.05). Compared with group CIPN, MWT was significantly increased at 15 days after establishment of the model in group I1, and MWT was increased, the expression of spinal IGF-1 was up-regulated, the count of IGF-1 positive cells was increased, and expression of IL-17A, IL-1β and TNF-α was down-regulated at 13 and 15 days after establishment of the model in group I2 ( P<0.05). Compared with group I1, the count of IGF-1 positive cells in spinal dorsal horn was increased in group I2 ( P<0.05). There was no significant difference in the expression of spinal IGF-1R among the 4 groups ( P>0.05). Conclusion:Intrathecal IGF-1 can alleviate chemotherapy-induced NP, and the mechanism may be related to inhibiting the inflammatory responses in spinal cord of mice.

ABSTRACT

Objective@#To comprehensively understand the disinfection quality of secondary and primary schools and kindergartens in Wuxi, so as to find out problems and to provide advice for improvement.@*Methods@#Stratified random sampling method was applied in the investigation to select 73 schools and classes. The qualities of room air, surface of object, hand hygiene of staff, tableware, ultraviolet lamp and disinfectant in use (including bacterial contamination and concentration of chlorine-containing) were all tested.@*Results@#A total of 2 563 samples were collected with the total disinfection qualified rate 88.02%. The qualified rates of kindergarten, primary and secondary schools were 87.89%, 90.67% and 85.83% respectively. The rates of Xishan and Jiangyin districts were 75.24% and 75.89% respectively. The quality of urban schools was better than the rural(χ 2=16.57, P<0.01). There was no statistical significance between public and private schools (χ 2=0.01, P=0.92). The rank of qualified rates of different objects was: bacterial contamination (100.00%) > room air (93.13%) > tableware (91.87%) > surface of object (89.40%) > ultraviolet lamp (84.00%) > concentration of chlorine-containing (73.68%) > hand hygiene of staff (73.53%). The quality of secondary schools was lower than kindergartens and primary schools in the aspects of room air and surface of object.@*Conclusion@#The disinfection quality of secondary and primary schools and kindergartens in Wuxi is good in general. More attention should be paid on hand hygiene, concentration of chlorine-containing and ultraviolet lamp. The qualities of room air and surface of object of secondary schools need to be improved.

ABSTRACT

Objective:To explore the value and efficacy of the risk model based on the metabolic parameters of 18F-FDG PET-CT in predicting epidermal growth factor receptor (EGFR) gene mutations in non-small cell lung cancer (NSCLC). Methods:This retrospectives study reviewed 105 NSCLC patients who were tested for EGFR gene expression and underwent 18F-FDG PET-CT exam prior to treatment from Jan 2017 to June 2018 in our hospital. The patients were divided into EGFR mutations group ( n=40) and EGFR wild type group ( n=65). The differences between the different groups were analyzed in several clinical characteristics and three metabolic parameters based on 18F-FDG PET-CT, including the maximal standard uptake value (SUV max), metabolic tumor volume (MTV), total lesion glycolysis (TLG) of the primary tumor. Multivariate logistic regression analysis was performed to identify predictors of EGFR mutations, and the risk prediction model and nomogram graph were constructed. Diagnostic efficiency of the model was done by the receiver operating characteristics (ROC) curve analysis, and the Calibration plot was performed by Hosmer-Lemeshow (H-L) test to evaluate the calibration scale of the model. Results:There were statistically significant differences in gender, smoking status, serum CEA level, length of tumor, pathological types, TTF-1 and NapsinA expression between the EGFR mutant groups and EGFR wild-type groups (all P<0.05). The MTV and TLG of EGFR mutation group were 4.4 (4.5,37.1) cm 3 and 46.6 (21.2,118.2), respectively. The MTV and TLG of EGFR wild type group were 7.4 (3.2,13.5) cm 3 and 95.4 (26.4,345.1), respectively. The MTV and TLG of EGFR mutation group were significantly lower than those of EGFR wild type group ( Z=-2.452, P=0.014; Z=-2.379, P=0.017). ROC curve analysis showed area under the curve (AUC) predicted by SUV max, MTV and TLG for EGFR mutations was 0.597, 0.643 and 0.639, respectively. Multivariate analysis demonstrated that gender, length of tumor, SUV max and MTV were independent predictors of EGFR mutations, with the odds ratio (OR) values (95 %CI) as 3.811 (1.508-9.629), 1.679 (0.899-3.136), 0.928 (0.848-1.015) and 0.924 (0.865-0.986), respectively. The predictive model and nomogram graph was established, with the sensitivity, specificity, positive predictive value, negative predictive value and AUC of 80.0%, 66.2%, 68.8%, 75.3% and 0.775 (0.687-0.864), respectively. The H-L test showed the model had excellent accuracy (χ 2=3.872, P=0.869). Conclusion:The risk model based on the metabolic parameters of 18F-FDG PET-CT has a good performance in predicting the mutations of EGFR gene in patients with NSCLC.

ABSTRACT

Compared with conventional imaging, 18F-fluorodeoxyglucose positron-emission tomography compared with computed tomography (18F-FDG PET/CT) possesses higher sensitivity and specificity in the diagnosis of solitary pulmonary nodule (SPN) and lung cancer. From the perspective of health economics, PET/CT is more suitable strategy for diagnose of SPN with intermediate probability of malignancy, and has good health economics value in preoperative staging diagnosis and follow-up after radiotherapy and chemotherapy of lung cancer. The evaluation method, effect index and comparison method used in the health economics research of PET/CT for lung cancer diagnosis and treatment are cost-effect analysis, life year and incremental cost-effect ratio, respectively. Case tracking and follow-up was a means of early studies on PET/CT health economics, and in recent years mathematical models are used in most studies.

ABSTRACT

Benign metastasizing leiomyoma (BML) often occurs in women with a history of uterine leiomyoma at the childbearing age. At the same time, BML cases with multiple sites of metastatic lesions are extremely rare. A BML patient with multiple metastases of uterine leiomyoma in two lung and lumbar spine after surgery was admitted to Department of Spinal Surgery, Second Xiangya Hospital, Central South University in September 2017. Positron emission tomography/computed tomography (PET/CT) can perform a whole body examination for BML patients, which can find metastases in many parts of the body such as lung, abdominal cavity, pelvic cavity and spine. The PET/CT results are helpful to a comprehensive diagnosis. The imaging and clinical features of BML are now explored in combination with the characteristics of the case and relevant literature reports.

ABSTRACT

Objective To evaluate the role of spinal cord tumor necrosis factor receptor-associated factor 6 (TRAF6)/nuclear factor kappa B (NF-κB)signaling pathway in the development of diabetic neuropathic pain (DNP)in rats.Methods Clean-grade healthy adult male Sprague-Dawley rats,aged 2 months,weighing 180-230 g,in which IT catheters were implanted,were used in this study.Streptozotocin 60 mg/kg was intraperitoneally injected after IT catheterization to establish the model of DNP.Twelve DNP rats were divided into 2 groups (n =6 each) by a random number table method:DNP group and DNP plus TRAF6 inhibitor group (group DTR).Another 6 age-matched Sprague-Dawley rats were used as normal control group (group NC).The rats in group DC and group DTR received IT injection of dimethyl sulfoxide 10 μl and TRAF6 inhibition 10 μg,respectively,once a day for 7 consecutive days starting from day 21 after establishing the model.The mechanical paw withdrawal threshold (MWT) was determined before establishing the model (T1),on 7,14 and 21 days after establishing the model (T2-4),and on 1,4 and 7 days after IT injection (T5-7).The rats were sacrificed after the last MWT measurement,and the L3-5 segments of the spinal cord were removed for determination of the expression of TRAF6 and NFκB p65 by Western blot.Results Compared with group NC,the MWT at T3-7 in group DC and at T3-6 in group DTR was significantly decreased,and the expression of spinal TRAF6 and NF-κB p65 was up-regulated in DC and DTR groups (P<0.05).Compared with group DC,the MWT was significantly increased at T6-7,and the expression of spinal TRAF6 and NF-κB p65 was down-regulated in group DTR (P < 0.05).Conclusion Spinal cord TRAF6/NF-κB signaling pathway is involved in the development of DNP in rats.

ABSTRACT

To determine the clinicopathological and imaging features in 18F-fluoro-2-deoxyglucose (18F-FDG) positron emission tomography and computed tomography (PET/CT) for paraganglioma of testis, and to increase the diagnostic accuracy.
 Methods: A case of paraganglioma of testis with multiple lymph node and lung metastasis were reported. PET/CT and pathological findings in the case were retrospectively analyzed.
 Results: The patient presented with high blood pressure, high level of catecholamine, and urinary vanillylmandelic acid. The patient underwent 18F-FDG PET/CT, which showed the features including the right testis nodule with a star lesion nearby, the right spermatic cord, the lymphadenopathy of bilateral inguinal and retroperitoneum, the posterior basal segment of right lung nodule, and a lot of brown adipose tissues (BAT) in the whole body with intense FDG uptake. 18F-FDG PET/CT showed that the intense FDG uptake of the BAT disappeared after the excision of the right testis and metastasis of paraganglioma.
 Conclusion: PET/CT shows great value in localization diagnose, clinical staging and curative evaluation. PET/CT plays a helpful role in revealing the BAT with 18F-DG avidity in the patients with paraganglioma with elevated blood pressure, high level catecholamine, and urinary vanillylmandelic acid.

18F-fluorodeoxyglucose (FDG) PET/CT allows the functional information of PET to be fused with the accurate anatomical intormation from CT,it demonstrates unique advantages in lung cancer diagnosis,clinical staging,therapeutic evaluation and prognostic stratification.The metabolic parameters from 18F-FDG PET/CT include standardized uptake value,metabolic tumor volume,total lesion glycolysis and so on.This review summarizes the progress,problems and trends about 18F-FDG PET/CT parameters which are currently used in lung cancer.

ABSTRACT

Objective To investigate the clinical efficacy and nursing strategy of alprostadil and edaravone in the treatment of acute cerebral infarction (ACI). Methods 60 patients with ACI were randomly divided into observation group and control group with 30 cases in each group,the two groups were treated with Alprostadil Combined with edaravone, while the control group received routine care. The observation group received early rehabilitation nursing intervention on the basis of the control group nursing, the clinical efficacy, neurological deficit (NIHSS) score, Barthel index (BI) and complication rate were compared between the two groups before and after the nursing. Results The total effective rate and complication rate of the observation group were 93.33 and 10.00% respectively, compared with 76.67% and 23.33% in the control group (P< 0.05);after nursing, the NIHSS score and BI index of the observation group were significantly better than that of the control group (P<0.05). Conclusion For ACI patients, given alprostadil combined with edaravone treatment and given early nursing intervention can significantly improve the clinical efficacy, improve the degree of neurologic impairment, improve the ability of daily life of patients and reduce the incidence of complications, worthy of promotion.

ABSTRACT

Objective:To review the dual-time-point 18F-fluorodeoxyglucose (FDG) positron emission tomography/X-ray computed tomography (PET-CT) imaging for a patient with primary ureteric lymphoma (PUL),and explore the potential ofFDG PET-CT on diagnosis,staging and evaluation of treatment in patients with PUL.Methods:Complete clinical and imaging data of one patient with PUL was reported.The relevant literatures from 1997 to 2016 were reviewed,and the imaging features of uriteric lymphoma were analyzed.Results:The patient presented with microhematuria,a soft-density mass with moderate enhancement in the mid left ureter,and a luminal stenosis with the scope inserted 5 cm into the left stoma,but no mass was found.The patient was pathologically diagnosed as follicular lymphoma (stage IE confirmed by whole body FDG PET-CT).Conclusion:PET-CT may be useful in diagnosis,clinical stage and therapy assessment in patients with PUL.

ABSTRACT

OBJECTIVE@#To explore the 2-[fluorine-18]-fluoro-2-deoxy-D-glucose (¹⁸F-FDG) positron emission tomography/computed tomography (PET-CT) manifestations and functions in patients with primary hyperparathyroidism (pHPT). 
@*METHOD@#The diagnosis of 1 case of double parathyroid adenomas secondary to pPTH was reported and the related literature was reviewed.
@*RESULTS@#PET-CT showed two nodus shadow with slightly lower density compared with muscle, clear boundary and uniform density. Simultaneously, images also showed multiple enhanced FDG uptake of bone lesions in skeletal system. The maximum standard uptake values for ¹⁸F-FDG in the destructive bone lesions were more than 10. 
@*CONCLUSION@#¹⁸F-FDG PET-CT is a well-established imaging technique for the diagnosis in pHPT. It not only can clearly visualize systemic lesions and accurately assess the extent of the disease, but also can accurately localize parathyroid adenoma. After resection of parathyroid adenoma, FDG PET-CT imaging can be useful to see the response to treatment and osteoblastic changes in brown tumor.

ABSTRACT

Objective Minimally invasive treatment of orthopedic diseases is the general direction of future development of medicine.This study was designed to observe the effect of manual reduction combined with percutaneous kyphoplasty (MR+PKP) in the treatment of fresh osteoporotic vertebral compression fractures ( OVCF) in elderly patients. Methods Sixty OVCF patients aged 60-86 ( mean 72.3) years were randomly assigned to 2 groups of e-qual number to be treated by MR+PKP and PKP alone, respectively. Comparisons were made between the two groups of patients in the op-eration time, volumeand permeability of the bone cement injected,changes of the Cobb angle,restoration of the anterior height of the compressed vertebral bodies,pre-and post-operative Visual Analogue Scale ( VAS) pain scores, OswestryDisability Indexes ( ODIs) , and other differences observed before and aftersurgery. Results Op-erations were performed successfully in all the 60 cases.In the MR+PKP group, the mean operation time was 61 min, the mean volume of bone cement injected was 5.1mL with qualified distribution, and bone cement leakage occurred in 1 case without adverse reaction. Statistically significant differences were found in the pre-and post-operativeanterior height of the compressed vertebral bodies, Cobb an-gle, VAS scores, and ODIs (P<0.05).Compared with the PKP control, MR+PKP achieved a significant increase at 3 days and 3 months after surgery in the anterior height of the compressed vertebral bodies ([22.4±1.4] vs [26.8±8.1] mm and [21.4±4.2] vs [26.5±7.2]mm, P<0.05), and a decrease in the Cobb angle ([8.6±2.7] vs [8.1±2.1]°and [9.0±2.3] vs [8.3±1.8]°, P<0.05) as well as remarkably reduced VAS scores (4.1±2.2vs 3.1±2.0, P<0.05)and ODIs (23.0±3.1vs25.6±3.3, P<0.05) at 3 d postopera-tively. Conclusion MR+PKP, with its advantages of effective pain-relief, improvement of the height of compressed vertebral bodies, and reduction of bone cement leakage,is better than PKP alone for the treatment of OVCF in elderly patients.

OBJECTIVE@#To analyze why lung benign nodules or masses are misdiagnosed as lung cancer in positron emissin tomography-computed tomography (PET-CT).@*METHODS@#We retrospectively analyzed the imaging, clinical and pathological data of 18 patients with benign lung nodules or masses misdiagnosed as lung cancer from May 2012 to August 2013.@*RESULTS@#18F-fluorodeoxyglucose (FDG) uptake was found in all 18 patients [maximum standardized uptake value (SUVmax) of FDG ranged from 1.2-12.2, mean=4.99]. SUVmax of FDG was greater than 2.5 in 13 patients, and delayed scan was not performed in 17. The contrast enhanced CT scan and high resolution CT (HRCT) was not performed in all the patients. The diameter was greater than 3 cm in 4 patients in CT. There were 5 patients locating in contact with the pleural surface and including clear subpleural fat clearance. Other radiographic signs included regular shape and smooth margin in 7 patients, calcification in 4, satellite opacities in 3, and long spiculation in 1. Neglecting the CT features was the main cause of misdiagnosing the lung benign nodules or masses as lung cancer in PET-CT, while lacking of contrast enhanced CT scan, HRCT and PET-CT delayed scan also led to misdiagnosis.@*CONCLUSION@#We should learn to apply the contrast enhanced CT scan, HRCT and PET-CT delayed scan and analyze the CT and PET imagings to avoid the misdiagnosis of lung benign lesions. In-depth study of other tracers is a new direction to prevent the misdiagnosis.

ABSTRACT

Objective To investigate the effect of Ulinastatin preconditioning on hyperthermia induced rat skeletal muscle injury and the possible mechanism. Methods Cultured skeletal muscle cells were divided into 7 groups: control group (Group C), as normal culture; hyperthermia treated group (Group H), in which cells were exposed to 42℃for 2 hours then transfered to normal culture; ulinastatin preconditioned group (Group U), which were only preconditioned 1 hour by the final concentration of ulinastatin at 1 250 U/mL; and four different concentration of ulinastatin preconditioned+hyperthermia treated groups (Group Ⅰ, Ⅱ, Ⅲ, Ⅳ), in which cells were preconditioned 1 hour by the final concentration of ulinastatin at 312 , 625, 1250, 2 500 U/mL, respectively. And then, all the four groups were exposed to 42℃for 2 hours. MTT assay and LDH leakage were performed to study the cytotoxicity. Superoxide dismutase (SOD) activity and malondialdehyde (MDA) formation were measured to reflect the activity of antioxidase. Results Compared with Group H, MTT value in ulinastatin preconditioned group in GroupⅠ, Ⅱ,Ⅲ, Ⅳsignificantly increased (P0.05). Conclusion Ulinastatin preconditioning may have protective effect against hyperthermia induced skeletal muscle injury and it may be related to its ability of inhibiting radical production and promoting radical scavenging.

ABSTRACT

BACKGROUND:The application of anterior cervical plate for anterior cervical discectomy and fusion wil induce some complications such as dysphagia after treatment. OBJECTIVE:To observe the clinical efficacy of the new implant Zero-P system for cervical spondylosis patients and its effect on reducing complications after treatment. METHODS:A total of 51 cervical spondylosis cases underwent anterior cervical discectomy and fusion using Zero-P system. Neurological function was evaluated by neck/arm visual analogue scale, neck disability index and Japanese Orthopaedic Association scores pretreatment, at 3 days, 3, 6 months, 1, 2 years post-treatment. The incidence of postoperative dysphagia was evaluated using dysphagia score. Anteroposterior&lateral and dynamics X-ray films were used to evaluate the efficacy of graft fusion and internal fixation-related complications. RESULTS AND CONCLUSION:The patients were fol owed up for 6-30 months, averagely 15.4 months. After treatment, their incision achieved stage I healing. Pain was relieved and muscle force was restored in al patients after treatment. Visual analogue scale and neck disability index scores were apparently improved. Improvement rate of Japanese Orthopaedic Association score was 85.7%. At 2 days after treatment, seven patients experienced mild and moderate dysphagia. During fol ow up, implant subsidence was not observed. No complications, such as screw loose, breakage or fixator displacement, were found. Results suggested that the new cervical stand-alone anterior fusion device in anterior cervical discectomy and fusion procedure for the treatment of cervical spondylosis has offered an excellent clinical outcome. Its design has the advantages of cervical interbody fusion and anterior cervical plate. There were less relevant complications.

ABSTRACT

Objective:To optimize 3.0T MRI diagnosis indicator in breast cancer and to select the best MRI scan program. Methods:Totally 45 patients with breast cancers were collected, and another 35 patients with benign breast tumor served as the control group. All patients underwent 3.0T MRI, including T1-weighted imaging (T1WI), fat suppression of the T2-weighted imaging (T2WI), diffusion weighted imaging (DWI), 1H magnetic resonance spectroscopy (1H-MRS) and dynamic contrast enhanced (DCE) sequence. With operation pathology results as the gold standard in the diagnosis of breast diseases, the pathological results of benign and malignant served as dependent variables, and the diagnostic indicators of MRI were taken as independent variables. We put all the indicators of MRI examination under Logistic regression analysis, established the Logistic model, and optimized the diagnosis indicators of MRI examination to further improve MRI scan of breast cancer. Results:By Logistic regression analysis, some indicators were selected in the equation, including the edge feature of the tumor, the time-signal intensity curve (TIC) type and the apparent diffusion coeffcient (ADC) value when b=500 s/mm2. hTe regression equation was Logit (P)=-21.936+20.478X6+3.267X7+21.488X3. Conclusion:Valuable indicators in the diagnosis of breast cancer are the edge feature of the tumor, the TIC type and the ADC value when b=500 s/mm2. Combining conventional MRI scan, DWI and dynamic enhanced MRI is a better examination program, while MRS is the complementary program when diagnosis is diffcult.

ABSTRACT

ObjectiveTo study the application of endovascular treatment of the acute ruptured intracranial aneurysms with the hydrocoil. Methods Fifteen patients ( treatment group ) with 17 acute ruptured intracranial aneurysms were treated with the hydrocoils.In the same period,18 patients (control group) with acute ruptured intracranial aneurysms were treated with the bare platinum coil.Results In treatment group,16 aneurysms received dease embolization,and one aneutysm had subtotal embolization.In control group,11 aneurysms achieved dense embolization,7 aneurysms had subtotal embolization.There was a significant difference between the two groups ( x2 =5.402,P < 0.05 ). After follow-up by DSA from 6 months to 1.5 years,no recurrence was showed in the treatment group,while recurrence occurred in 5 cases in the control group aneurysm,with a significant difference between the two groups ( x2 =5.510,P <0.05 ).ConclusionThe hydrocoil is an effective therapy for the endovascular treatment of intracranial aneurysms with improving the packing density of intracranial aneurysm embolization and reducing the recurrence rate.
