ABSTRACT

OBJECTIVE@#To evaluate the therapeutic effect of the combined treatment with balance acupuncture therapy and exercise re-learning rehabilitation therapy and the impact on serum cAMP and cGMP in the patients with hemiplegia of cerebral ischemic stroke.@*METHODS@#A total of 90 patients of hemiplegia of cerebral ischemic stroke were randomized into an observation group and a control group, 45 cases in each one. All of the patients in the two groups received health education, diet guidance, routine symptomatic treatment as well as exercise re-learning rehabilitation therapy. Additionally, in the observation group, balance acupuncture therapy was applied, in which, the acupoints on the aspect of the human body, on the governor vessel and bladder meridian were adopted in the morning and those on the aspect of the human body, on the conception vessel and kidney meridian were stimulated in the afternoon. In the control group, the regular acupuncture was given. In the two groups, both acupuncture and rehabilitation therapies were given 5 days a week, 2 week-treatment as one course and totally 2 courses were required. Separately, before and after treatment, the score of Fugl-Meyer assessment (FMA) and the score of Chinese stroke scale (CSS) were recorded, the levels of cyclic adenosine monophosphate (cAMP) and cyclic guanosine monophosphate (cGMP) detected in serum and the clinical therapeutic effect were evaluated in the two groups.@*RESULTS@#After treatment, FMA score was increased in the patients of either of the groups as compared with that before treatment (<0.01) and CSS score decreased as compared with that before treatment (<0.01). After treatment, FMA score in the observation group was higher than that in the control group (<0.01) and CSS score was lower than the control group (<0.01). After treatment, the level of serum cAMP of the patients in either of the groups was increased as compared with that before treatment (<0.01) and that of cGMP decreased as compared with that before treatment (<0.01). After treatment, the level of cAMP in the observation group was higher than that in the control group (<0.01) and that of cGMP was lower than the control group (<0.01). The total effective rate was 93.3% (42/45) in the observation group, better than 73.3% (33/45) in the control group (<0.01).@*CONCLUSION@#The balance acupuncture therapy combined with exercise re-learning rehabilitation effectively improves the motor function of the affected limb, relieves injury and regulate the levels of serum cAMP and cGMP in the patients with hemiplegia of ischemic stroke.

ABSTRACT

The hierarchical diagnosis and treatment system is an important measure for rational allocation of medical resources, promotion of equalization of basic medical services, improving the patients' disorderly medical treatment and guiding patients' hospital choice behavior. This paper defines the utility function of rational medical treatment of patients affected by multiple factors such as the personal disposable income, the age,the severity of the self-induced illness and the decision-makers' education level, and considered the outpatient price, service capacity and other factors of medical institutions. The present study constructed the incomplete information dynamic game model based on fictitious play method. It provided the quantitative analysis of the statistical data of Qinhuangdao and obtained the patients distribution in medical institution. The rationality of the model was verified by comparing the simulation with actual data, and showed the choice is more reasonable between the primary health care and general hospital. Therefore, the emphasis of optimizing the medical service resources in hierarchical diagnosis and treatment system is to establish the treatment system of integrated medical service for patients'on-demand and on-demand medi-cal services. At the same, various incentives should be used to enhance patients'enthusiasm for primary health care.

ABSTRACT

Objective By taking rehabilitation as the control, to observe the effect of abdominal and wrist-ankle acupuncture plus rehabilitation in restoring the motor and neurological function in hemiplegia due to ischemic cerebral stroke, and to optimize the treatment protocol. Method By adopting a randomized controlled clinical design, a hundred patients diagnosed with hemiplegia after ischemic cerebral stroke were randomized into a treatment group (50 cases) and a control group (50 cases) regarding their visiting sequence. The patients all received basic symptomatic treatment to control blood pressure, improve cerebral circulation, stabilize vital signs and prevent complications. In addition, the treatment group received abdominal and wrist-ankle acupuncture plus kinesiotherapy; the control group received rehabilitation training. The two groups were evaluated for therapeutic efficacy after 2 courses of treatment. Before and after the intervention, the recruited patients were tested by Fugl-Meyer Assessment (FMA), China Stroke Scale (CSS) and Berg Balance Scale (BBS), and the level of homocysteine (Hcy) was also detected. Result The FMA, CSS and BBS scores all showed significant changes after the intervention in the two groups (P<0.05); after the treatment, the FMA, CSS and BSS scores in the treatment group were significantly different from those in the control group (P<0.05), and there were significant between-group differences in comparing the score differences of FMA,CSS and BBS (P<0.05). The two groups both showed decrease of Hcy, (11.68±4.53)μmol/L in the treatment group versus (13.53±4.01)μmol/L in the control group, and the between-group difference was statistically significant (P<0.05). Conclusion Compared to rehabilitation alone, abdominal and wrist-ankle acupuncture plus kinesiotherapy can produce a more significant efficacy in treating hemiplegia due to ischemic cerebral stroke.

ABSTRACT

<p><b>OBJECTIVE</b>To explore clinical significance of rotational axis of distal femur on Chinese adults in total knee arthroplasty (TKA).</p><p><b>METHODS</b>There were 86 Chinese adults (106 normal knees) including 47 males (53 knees) and 39 females (53 knees), 54 knees were on left and 52 on right. The CT scan was employed in the distal femur. The scan direction was aligned to be on the plane perpendicular to the mechanical axis of the femoral. The CT images of cross sections across lateral and medial femoral epicondyle were moved to personal computer, lateral angle between anterior posterior line (APL) and surgical transepicondylar axis (STEA) (ATA),lateral angle between posterior condylar line (PCL) and APL (APA), angle between perpendicularity of APL and PCL (A-PA), posterior condylar angle (PCA), condylar twist angle (CTA), angle between clinical transepicondylar axis (CTEA) and STEA (CSA) were measured. These values were divided into different groups according to gender and side, the values of CTA, PCA, A-PA, angle PT (varus of tibia plateau), constant 3, ATA, APA and constant 90° were compared by statistically. A-PA and PCA, and CTA were analysed statistically with the liner regression, the relationship among CTEA, STEA ,PCL, APL and PLP were performed to assess by liner regression.</p><p><b>RESULTS</b>ATA was (89.79 ± 1.22)°, APA was (84.84 ± 1.83)°, A-PA was (5.16 ± 1.83)°, PCA was (4.80 ± 1.23)°, CTA was (8.23 ± 1.40), CSA was (3.45 ± 0.68)°. All the parameters had no differences on sex and side, but CSA had difference on male and female. There was no difference among angle PT, PCA, A-PA. There was significant difference in CAT, constant 30 and angle PT, PCA,A-PA. There was no difference between ATA and constant 90°, but there was difference between APA and constant 90°. There was relativity between PCA and CTA, and also PCA and A-PA, CTA and A-PA. There was significant relativity between STEA and CTEA, between STEA and APL, between STEA and PCL, and also between APLP, APL and PCL, but there was no significant relativity between PCL and CTEA.</p><p><b>CONCLUSION</b>TKA for Chinese, the section of femoral posterior condyle should be external 5° to obtain the optimum rotational orientation. The property is different entirely between STEA and CTEA, the rotational alignment is not performed according to parallel to the CTEA in distal femur. Among STEA, APL, PCL, the STEA is the most reliable mark ofrotational alignment of the distal femur, and the PCL is the less reliable mark.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To detect the spatial distribution characteristics of iodine in drinking water of residents in Shandong province with spatial autocorrelation analysis.</p><p><b>METHODS</b>The county-based study set Shandong province as a research site. A total of 108 164 water samples from 140 counties were collected. The drinking water iodine data in county-level city between 2008 to 2010 were obtained from Shandong Institute of Prevention and Control for Endemic Disease and was merged with an electronic map to build a spatial database. Global and local Moran's I index were calculated, respectively, and spatial autocorrelation and cluster range of iodine distribution in drinking water in Shandong province were studied by SaTScan software.</p><p><b>RESULTS</b>All counties were further grouped according to the "criteria of delimitation for IDD endemic areas" and "determination and classification of the areas of high water iodine and the endemic areas of iodine excess goiter", and 90 counties were iodine deficiency (< 10 µg/L), 31 were iodine suitable (10 - 150 µg/L), and 19 (> 150 µg/L) were high iodine. For the overall study area, the iodine distribution in drinking water in Shandong province existed spatial autocorrelation (Moran's I = 0.52, Z = 7.4, P < 0.01). For the local scale, the drinking water iodine in 18 counties of Dezhou, Liaocheng and Heze city in western Shandong province was clustered, the local Moran's I were between 0.22 - 1.00 (P < 0.01), which were all high-high clusters, indicating the positive spatial correlation. Spatial analysis using SaTScan software detected two cluster areas including 20 counties, which the centers located in Xiajin and Dingtao county, the cluster radiuses were 57.47 km and 65.58 km respectively. The analysis results were consistent with the results of local spatial autocorrelation.</p><p><b>CONCLUSION</b>There are apparent spatial autocorrelation and strong spatial heterogeneity existed in the iodine distribution in drink water in Shandong province.</p>

ABSTRACT

OBJECTIVE@#To measure the expression pattern of STAT2 in cervical cancer initiation and progression in tissue sections from patients with cervicitis, dysplasia, and cervical cancer.@*METHODS@#Antibody against human STAT2 was confirmed by plasmids transient transfection and Western blot. Immunohistochemistry was used to detect STAT2 expression in the cervical biopsies by using the confirmed antibody against STAT2 as the primary antibody.@*RESULTS@#It was found that the overall rate of positive STAT2 expression in the cervicitis, dysplasia and cervical cancer groups were 38.5%, 69.4% and 76.9%, respectively. The STAT2 levels are significantly increased in premalignant dysplasia and cervical cancer, as compared to cervicitis (P< 0.05). Noticeably, STAT2 signals were mainly found in the cytoplasm, implying that STAT2 was not biologically active.@*CONCLUSIONS@#These findings reveal an association between cervical cancer progression and augmented STAT2 expression. In conclusion, STAT2 increase appears to be an early detectable cellular event in cervical cancer development.

ABSTRACT

Breast cancer stem cells are a group of undifferentiated cells with self-renewal and multidifferentiation potential. Chemotherapeutic and radiotherapeutic resistance, hypoxic resistance, high tumorigenicity, high cell invasion, and metastatic abilities are characteristics of these cells, which are responsible for breast cancer recurrence. Therefore, the correct sorting and identification of breast cancer stem cells is a primary step for research in this field. This article briefly describes the recent progress on sorting and identification technologies for breast cancer stem cells. Sorting technologies include the side population technique, technologies that depend on cell surface markers, ALDEFLUOR assays, and in situ detection. Identification technologies include mammosphere cultures, limited dilution in vitro, and in-vivo animal models. This review provides an important reference for breast cancer stem cell research, which will explore new methods for the treatment of patients with breast cancer.

ABSTRACT

<p><b>BACKGROUND</b>Population based epidemiologic study on the main diseases and birth status of liveborn neonates remains scarce in China, especially in rural areas where a large number of neonates are born. The aim of this study was to establish an epidemiological basis of live births in Julu County, a representative of the northern and mid-western parts of China in terms of demography, disease pattern and women and children's health care infrastructure.</p><p><b>METHODS</b>The perinatal data of all live births were prospectively collected in three participating county-level hospitals from September 1, 2007 to August 30, 2008.</p><p><b>RESULTS</b>There were 5822 live births in these hospitals. Among all live births, 53.7% were male and 4.5% were born prematurely. Mean (SD) birth weight (BW) was (3348 ± 503) g. The low (< 2500 g) and very low BW (< 1500 g) infants accounted for 3.8% and 0.5% of the total births, with 6.5% as small for gestational age and 2.8% as multi-births. Cesarean section rate was 30.2%, of which 68.6% were elective. There were 745 infants (12.8% of the live births) admitted to local neonatal wards within 7 days of postnatal life, in which 48.3% and 19.3% were due to perinatal asphyxia and prematurity, respectively. The incidences of perinatal aspiration syndrome, transient tachypnea and respiratory distress syndrome were 4.9%, 0.6% and 0.5%, respectively. Neonatal mortality was 7.6‰ (44/5822), with 16 in delivery room and 28 in neonatal ward before discharge.</p><p><b>CONCLUSIONS</b>This study provided a population-based perinatal data of live births and neonatal mortality in a northern China county with limited resources. Neonatal disorders related to perinatal asphyxia remain a serious clinical problem, which calls for sustained education of advanced neonatal resuscitation and improvement in the quality of perinatal-neonatal care.</p>

ABSTRACT

In this study, we explored the operation options for middle cranial fossa arachnoid cysts(MCFAC). One hundred and forty-nine patients who were operated for a symptomatic MCFAC between 1993 and 2006 in our hosptial were analyzed. Follow-up time ranged from 1 y to 14 y(mean=5.4 y). All these patients were divided into three subgroups according to Galassi classification.Long-term outcome and complications were studied respectively. Fenestration (F) resulted in a more favorable long-term outcome and less complication for cysts of types Ⅰ and Ⅱ, whereas a favorable outcome was noted in type Ⅲ patients who underwent cysto-peritoneal shunting (S). We are led to conclude that Fenestration is suitable for cysts of types Ⅰ and Ⅱ (Galassi classification),cysto-peritoneal shunting is better for cysts of type Ⅲ.
