ABSTRACT

OBJECTIVE@#To evaluate the feasibility of S2 alar iliac screw insertion in Chinese children using computerized three-dimension reconstruction and simulated screw placement technique, and to optimize the measurement of screw parameters.@*METHODS@#A total of 83 pelvic CT data of children who underwent pelvic CT scan December 2018 to December 2020 were retrospectively analyzed, excluding fractures, deformities, and tumors. There were 44 boys and 39 girls, with an average age of (10.66±3.52) years, and were divided into 4 groups based on age (group A:5 to 7 years old;group B:8 to 10 years old;group C:11-13 years old;group D:14 to 16 years old). The original CT data obtained were imported into Mimics software, and the bony structure of the pelvis was reconstructed, and the maximum and minimum cranial angles of the screws were simulated in the three-dimensional view with the placement of 6.5 mm diameter S2 alar iliac screws. Subsequently, the coronal angle, sagittal angle, transverse angle, total length of the screw, length of the screw in the sacrum, width of the iliac, and distance of the entry point from the skin were measured in 3-Matic software at the maximum and minimum head tilt angles, respectively. The differences among the screw parameters of S2 alar iliac screws in children of different ages and the differences between gender and side were compared and analyzed.@*RESULTS@#In all 83 children, 6.5 mm diameter S2 iliac screws could be placed. There was no significant difference between the side of each screw placement parameter. The 5 to 7 years old children had a significantly smaller screw coronal angle than other age groups, but in the screw sagittal angle, the difference was more mixed. The 5 to 7 years old children could obtain a larger angle at the maximum head tilt angle of the screw, but at the minimum cranial angle, the larger angle was obtained in the age group of 11 to 13 years old. There were no significant differences among the age groups. The coronal angle and sagittal angle under maximum cephalic angle and minimum cranial angle of 5 to 7 years old male were (40.91±2.91)° and (51.85±3.75)° respectively, which were significantly greater than in female. The coronal angle under minimum cranial angle was significantly greater in girls aged 8-10 years old than in boys. For the remaining screw placement angle parameters, there were no significant differences between gender. The differences in the minimum iliac width, the screw length, and the length of the sacral screws showed an increasing trend with age in all age groups. The distance from the screw entry point to the skin in boys were significantly smaller than that of girls. The minimum width of the iliac in boys at 14 to 16 years of age were significantly wider than that in girls at the same stage. In contrast, in girls aged 5 to 7 years and 11 to 13 years, the screw length was significantly longer than that of boys at the same stage.@*CONCLUSION@#The pelvis of children aged 5 to 16 years can safely accommodate the placement of 6.5 mm diameter S2 alar iliac screws, but the bony structures of the pelvis are developing and growing in children, precise assessment is needed to plan a reasonable screw trajectory and select the appropriate screw length.

ABSTRACT

OBJECTIVE@#To summarize the complications of core drilling intramedullary nail in the treatment of femoral shaft closed fracture and explore the treatment strategy.@*METHODS@#From August 2014 to June 2018, a total of 215 patients with closed femoral shaft fractures were treated with closed reduction core drill intramedullary nail, including 129 males and 86 females, aged from 18 to 62 years, with an average of (44.2±10.6) years old. The time from injury to operation was 3 to 21 days. There were 102 cases of AO type A fracture, 82 cases of AO type B fracture and 31 cases of AO type C fracture. The time of operation, the amount of blood loss during operation, the duration of hospitalization, the time of fracture healing and the HSS score of knee joint function at the last follow-up were recorded. The observation of complications included:iatrogenic fracture, core drill broken, core drill twist, postoperative infection, and fracture nonunion.@*RESULTS@#The average operation time was (63.2± 15.6) min and intraoperative blood loss was (150.0±34.5) ml. All the incisions reached grade A healing. Patients were follow up for a mean of (18.5±3.2) months, the average hospital stay was (4.3±1.2) days, and the average fracture healing time was (5.6±2.3) months. At the final follow-up, the average HSS score of knee joint was 90.3±4.7. Related complications occurred in 37 cases (17.2%). The core drill related complications occurred in 13 cases (6.0%), including core drill broken in 5 cases (2.3%), core removal in 1 case and slotting in 4 cases;core drill twist in 8 cases (3.7%). After the core was cut, the core was removed. Similar complicationsof conventional intramedullary nail:iatrogenic fracture was performed in 12 cases (5.6%), including 10 cases of fracture end split and 2 cases of distal perimedullary fracture of intramedullary nail. The patients with cleavage at the fracture end were not treated after judging their stability, and the patients with fracture around the distal end of the intramedullary nail were fixed with auxiliary steel plate during operation;1 case(0.4%) with delayed infection after operation, debridement and external fixation was replaced and healed after bone transfer; fracture nonunion occurred in 11 cases (5.1%), of which 7 cases (3.3%) were hypertrophic nonunion and healed with additional plate. Atrophic nonunion occurred in 4 cases (1.9%), which healed after additional steel plate and bone graft.@*CONCLUSION@#Core drilling intramedullary nail is an effective method for the treatment of closed femoral shaft fracture, and the complications include core drill related complications and conventional intramedullary nail similar complications. Accurate preoperative evaluation, careful operation during operation and early postoperative symptomatic treatment can effectively reduce the occurrence of related complications.

ABSTRACT

Background@#In consideration of characteristics and functions, extra-cellular signal-regulated protein kinase 5 (ERK5) signaling pathway could be a new target for spinal cord injury (SCI) treatment. Our study aimed to evaluate the roles of ERK5 signaling pathway in secondary damage of SCI.@*Methods@#We randomly divided 70 healthy Wistar rats into five groups: ten in the blank group, 15 in the sham surgery + BIX02188 (sham + B) group, 15 in the sham surgery + dimethyl sulfoxide (DMSO; sham + D) group, 15 in the SCI + BIX02188 (SCI + B) group, and 15 in the SCI + DMSO (SCI + D) group. BIX02188 is a specific inhibitor of the ERK5 signaling pathway. SCI was induced by the application of vascular clips (with the force of 30 g) to the dura on T10 level, while rats in the sham surgery group underwent only T9-T11 laminectomy. BIX02188 or DMSO was intra-thecally injected at 1, 6, and 12 h after surgery or SCI. Spinal cord samples were taken for testing at 24 h after surgery or SCI.@*Results@#Expression of phosphorylated-ERK5 (p-ERK5) significantly increased after SCI. Application of BIX02188 indeed inhibited ERK5 signaling pathway and reduced the degree of spinal cord tissue injury, neutrophil infiltration and proinflammatory cytokine expression, nuclear factor-κB (NF-κB) activation and apoptosis (measured by TdT-mediated 2′-deoxyuridine 5′-triphosphate nickend labeling, expression of Fas-ligand, BCL2-associated X [Bax], and B-cell lymphoma-2 [Bcl-2]). Double immunofluorescence revealed activation of ERK5 in neurons and microglia after SCI.@*Conclusion@#ERK5 signaling pathway was activated in spinal neurons and microglia, contributing to secondary injury of SCI. Moreover, inhibition of ERK5 signaling pathway could alleviate the degree of SCI, which might be related to its regulation of infiltration of inflammatory cells and release of inflammatory cytokines, expression of NF-κB and cell apoptosis.

ABSTRACT

Objective To explore the effect of Qingzao Jiufei Decoction (QJD) and its decomposing agent on the expression of Bax, Bcl-2, and Caspase-3 apoptosis protein in MP infection, in order to determine the effect target of QJD and its decomposing agent. Methods A total of 120 balb/c mice were randomly divided into normal group (group A), model group (group B), QJD group (group C), QJD group I decomposition agent (Group D), QJD group II decomposition agent (Group E), and azithromycin group (Group F), 20 rats in each group. Except the normal group, the other five groups were infected with MP by using the nose drop method. The ultrastructure and apoptosis of lung tissue were observed by transmission electron microscope. The expression of Bcl-2, Bax and Caspase-3 protein in lung tissue was detected by Immunohistochemical SP and Western blot method. The expression of Caspase-3 m-RNA was detected by qPCR method. Results After MP infection, inflammation changes can be observed in the lung tissue of mice, thickening of the alveolar wall, destruction of alveolar epithelial cells, cell infiltration, and changes in the characteristics of apoptosis. The expression of Bcl-2, Bax, Caspase-3 protein in the lung tissue of mice infected with MP was significantly increased, but the ratio of Bcl-2/Bax decreased significantly. Compared with the group B, the expression of Bcl-2 in group C, D, and F increased, the ratio of Bcl-2/Bax increased significantly, and the expression of Bax and Caspase-3 decreased. The difference of expression between group E and group B was not obvious. The results of Caspase3 mRNA detection showed that the expression of group C, group D and group F was lower than that of model group, and the change of group E was not obvious. Conclusion QJD can inhibit the cell apoptosis induced by MP infection, and Bax, Bcl-2 were one of its effect target, in which I decomposition agent plays a major role.

ABSTRACT

<p><b>BACKGROUND</b>Thoracolumbar junction (TLJ) is the transitional area between the lower thoracic spine and the upper lumbar spine. Vertebral compression fractures and proximal junctional kyphosis following spine surgery often occur in this area. Therefore, the study of development and mechanisms of thoracolumbar junctional degeneration is important for planning surgical management. This study aimed to review radiological parameters of thoracolumbar junctional degenerative kyphosis (TLJDK) in patients with lumbar degenerative kyphosis and to analyze compensatory mechanisms of sagittal balance.</p><p><b>METHODS</b>From January 2016 to March 2017, patients with lumbar degenerative kyphosis were enrolled in this radiographic study. Patients were divided into two groups according to thoracolumbar junctional angle (TLJA): the non-TLJDK (NTLJDK) group (TLJA <10°) and the TLJDK group (TLJA ≥10°). Complete spinopelvic radiographic parameters were analyzed and compared between two groups. Pearson or Spearman correlation coefficients and independent two-sample t-test or Mann-Whitney U-test were used.</p><p><b>RESULTS</b>A total of 77 patients with symptomatic sagittal imbalance due to lumbar degenerative kyphosis were enrolled in this study. There were 34 patients in NTLJDK group (TLJA <10°) and 43 patients in TLJDK group (TLJA ≥10°). The median angle of lumbar lordosis (LL) in the NTLJDK or TLJDK groups was 23.40° (18.50°, 29.48°) or 19.50° (13.30°, 24.55°), respectively. The median TLJAs in all patients and both groups were -11.20° (-14.60°, -4.80°), -3.70° (-7.53°, -1.73°), and -14.30° (-17.45°, -13.00°), respectively. In the NTLJDK group, LL was correlated with thoracic kyphosis (TK; r = -0.400, P = 0.019), sacral slope (SS; r = 0.681, P < 0.001), and C7-sagittal vertical axis (r = -0.402, P = 0.018). In the TLJDK group, LL was correlated with TK (r = -0.345, P = 0.024), SS (r = 0.595, P < 0.001), and pelvic tilt (r = -0.363, P = 0.017). There were significant differences in LL, TLJA, TK, SS, and pelvic incidence (PI) between two groups.</p><p><b>CONCLUSIONS</b>Although TLJDK is common in patients with lumbar degenerative kyphosis, it might be generated by special characteristics of morphology and biomechanics of the TLJ. To maintain sagittal balance, pelvis back tilt might be more important in patients with TLJDK, whereas thoracic curve changes might be more important in patients without TLJDK.</p>

ABSTRACT

This study was purposed to investigate the apoptosis-inducing effect of astragalosides on acute promyelocytic leukemia(APL) cell line NB4 and its mechanism. NB4 cells were treated with different concentrations (200, 300, 400 µg /ml) of astragalosides for 48 h. The cell proliferation was assayed by using CCK-8 method; the cell apoptosis was analyzed by flow cytometry with Annexin V-FITE/PI double staining. The mRNA expression of BCL-2 and the relative activity of BCL-2, NF-κB and caspase-3 were detected by RT-PCR and Western blot, respectively. The results showed that after treated with astragalosides for 48 h, astragalosides inhibited NB4 cell proliferation in concentration-dependent way, the apoptosis rate of NB4 cells gradually elevated from 4.69% to 40.85% with the increasing of astragalosides concentration. Simultaneously, the mRNA expression of BCL-2 was down-regulated, Western blot analysis showed that the protein expression levels of BCL-2 and NF-κB decreased after astragalosides treatment, while caspase-3 protein expression level increased. It is concluded that the molecular mechanism of the astragalosides-induced apoptosis in NB4 cells may be associated with down-regulation of the expression of BCL-2 and NF-κB, finally the relative activity of caspase-3 activated.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effects and apoptosis of intrathecal injection of Methylprednisolone Sodium Succinate (MPss) for acute spinal cord injury (SCI) in New Zealand rabbits.</p><p><b>METHODS</b>Seventy-two healthy New Zealand rabbits were used for the procedure and were randomly divided into two groups: SCI group and SHAM group, which was both divided into 6 subgroups, such as the vehicle group, the MPss intrathecal injection groups (1.5 mg/kg, 3.0 mg/kg, 6.0 mg/kg group), the MPss intravenous injection group and the combined injection group. TARLOV score was tested daily to evaluate the motor function. The rabbits were sacrificed 7 days after the surgery and the thoracic spinal cord sections and the sacral sections where MPss was injected were harvested for HE and TUNEL staining. Two-Factors Repeated Measures analysis of variance for TARLOV scores tested at various times and One-Way ANOVA analysis of variance for data between groups were used.</p><p><b>RESULT</b>Seven days after surgery in SCI group, there was no statistical difference between the TARLOV scores of intrathecal injection of MPss 3.0 mg/kg group, 6.0 mg/kg group and MPss intravenous injection group (P > 0.05), which were all better than the vehicle group (F = 4.762, P < 0.05). Referring to the lymphocyte infiltration at the injury site in SCI group, there was statistical difference between MPss intrathecal injection 6.0 mg/kg group (1.33 ± 0.21) and the vehicle group (2.67 ± 0.21) (F = 5.793, P < 0.05) and no statistical difference between intrathecal injection of MPss 6.0 mg/kg group and MPss intravenous injection group (P > 0.05). As for the lymphocyte infiltration at the intrathecal injection site in SHAM group, there was statistical difference between MPss intrathecal injection 6.0 mg/kg group (2.50 ± 0.55) and the vehicle group (0.50 ± 0.55) (F = 17.333, P < 0.05). TUNEL staining in SCI group showed statistical difference between MPss intrathecal injection 6.0 mg/kg group (6.3 ± 1.5) and the vehicle group (20.3 ± 2.2) (F = 71.279, P < 0.05).</p><p><b>CONCLUSIONS</b>Intrathecal injection of MPss can improve the functional recovery of lower limb and decrease apoptosis of neuron cells, which can provide same effects as the traditional intravenous injection of MPss in New Zealand rabbits.</p>

ABSTRACT

Fungal pathogens represent major problems for human health and agriculture. As eukaryotic organisms, fungi share some important features with mammalian cells. Therefore, current anti-fungal antibiotics often can not distinguish between fungi and mammalian cells, resulting in serious side effects in mammalian cells. Accordingly, there is strong impetus to develop antifungal alternatives that are both safe and effective. The E1 family of colicin are channel-forming bacteriocins produced by Escherichia coli, which are bactericidal only to E. coli and related species. To target the channel-forming domain of colicin to fungal cell membrane, we engineered a sexual mating pheromone of Candida albicans, α-factor pheromone to colicin Ia. A peptide was constructed consisting of an α mating pheromone of C. albicans fused to the channel-forming domain of colicin Ia to create a new fusion protein, pheromonicin-CA (PMC-CA). Indirect immunolabeling showed that the PMC-CA bound to fungal cells and inhibited growth in the laboratory and field. In the field, the protective activity of pheromonicin against rice blast disease was significantly greater, on a molar basis, than that of triazoles, tricyclazole or isoprothiolane. These results suggest that fusion peptides may be of value as fungicidal agents under agricultural conditions.

ABSTRACT

Objective To explore the efficacy of microvascular decompression (MVD)on trigeminal neuralgia and the anatomical characteristics of the responsible blood vessels. Methods The clinical data of 25patients with idiopathic trigemine neuralgia(ITN) undergoing MVD were analyzed retrospectively. The origins of the responsible blood vessels and sites of compression at the trigeminal root were observed and determined.Meanwhile, the anatomical characteristics of the responsible blood vessels and the efficacy of MVD were investigated. Results The offending vessels which were responsible for ITN were identified at root exit zone (REZ) in 21 cases (84%).The most common responsible vessel was the superior cerebellar artery in 13 cases (52%),followed by anterior inferior cerebellar artery in 6 cases (24%).Seventy six percent of the offending vessels were arteries and 8% were rock vein.No vascular compression but evident increase in the thickness of arachnoid mater covered by the trigeminal nerve was observed in 4 cases ( 16% ).All patients were relieved for the acute pain and the overall cure rate was 96%.Conclusion MVD is a safe, microinvasive and effective surgical modality for ITN.The key for better outcomes and less complications of MVD is to improve microsurgical techniques and deal with all the involved vessels.

ABSTRACT

ObjectiveTo observe the changes of fluoride content in serum, bone and urine after rats were exposed to single fluoride, single aluminum or fluoride combined with aluminum and to investigate the effects of different doses of aluminum on fluoride accumulation and excretion in rats. Methods Male Wistar rats were randomly divided into 9 groups based on 3 × 3 factorial design.Different doses of fluoride(NaF,0,50,200 mg/L)and(or) aluminum(AlCl3,0,100,200 mg/L) were administered to rats in each group by drinking water. The rats took food and water ad libitum during the experimental period. After feeding for 18 weeks, rats with obvious dental fluorosis were determined as successful establishment of animal model. The fluoride content in the serum, bones and urine were measured. Results Fluoride affected the fluoride content in serum, bones and urine(F=166.74,577.81,160.96,all P < 0.01 ).The interaction of fluoride and aluminum on serum, bone and urinary fluoride were statistically significant (F =7.95,5.13,6.94,all P < 0.01 ).When the fluoride level was 50 mg/L,the serum fluoride contents were [ (0.08 ± 0.03) and (0.08 ± 0.02) mg/L] in the aluminum levels of 0 and 100 mg/L groups, which was higher than that of the aluminum level of 200 mg/L group[ (0.04 ± 0.01)mg/L,F=7.14,5.78.all P< 0.05].The bone fluoride content in the 0 mg/L aluminum level group[ (1996.53 ± 383.73) mg/kg] was higher than that of the 100 and 200 mg/L groups[(1252.51 ± 189.08),( 1160.63 ± 129.63) mg/kg,F=20.54,24.56,all P < 0.01 ].When the fluoride level was 200 mg/L,the bone fluoride contents were decreased with the increasing doses of aluminum[ (4668.70 ± 887.67),(3920.30 ± 528.31 ),(3297.64 ± 396.04) mg/kg].Between any two groups, the differences were statistically significant (F =15.59,52.31,14.38,all P < 0.01 ).When the fluoride level was 50 mg/L,the urinary fluoride content in the 0 mg/L aluminum level group[ (34.054 ± 9.30)mg/L] was higher than that of the 100,200 mg/L groups[( 14.81 ± 6.32),(14.67 ± 3.42) mg/L,F =25.30,24.32,all P < 0.01 ].When the fluoride level was 200 mg/L,the urinary fluoride contents in the 0,100 mg/L aluminum level groups[ (57.14 ± 21.38),(51.75 ± 8.39)mg/L] were higher than that of the 200 mg/L group[(34.839 ± 9.30) mg/L,F=30.04,20.31,all P < 0.01 ].ConclusionsAluminum is an antagonist of fluoride. The antagonism could be enhanced as the dose of aluminum increased. In this study, aluminum could effectively counteract the absorption of fluoride in rat model when the ratio of fluoride to aluminum is 1 ∶ 2.
