ABSTRACT

Adrenocortical carcinoma (ACC) with inferior vena cava thrombosis is rare and has a poor prognosis, and the current literature overwhelmingly supports aggressive surgical intervention. This article summarizes the management of a patient with ACC with inferior vena cava thrombosis, and discusses the feasibility of detailed preoperative imaging data and intraoperative ultrasound to assess the superior and inferior boundaries of ACC with inferior vena cava thrombosis, while describing the intraoperative ultrasound-guided surgical planning and procedure for ACC with retrohepatic inferior vena cava tumor thrombus. Furthermore, it also demonstrates that it is feasible to accurately assess the superior and inferior boundaries of ACC with inferior vena cava thrombosis by preoperative multimodal imaging and intraoperative ultrasound, determine the mode of flow blockage during the operation, and obtain radical resection of the tumor.

ABSTRACT

Clonal hematopoiesis with indeterminant potential(CHIP)is defined as the proportion of detectable clonal hematopoietic cells in peripheral blood exceeding 2% and without confirmed hematologic malignancy.CHIP could increase the risk of malignant diseases through changes in DNA damage response, transcriptional programming and epigenetic modification.The incidence of malignant tumors in the blood system is significantly higher in the CHIP patients than healthy person.In addition, CHIP represents a negative factor associated with aging.Recent studies have found that the incidences of infections, anemia, heart failure, thrombotic events, and tumors of the blood system in CHIP carriers were significantly increased.Starting with the epigenetic modifications, phenotypic changes and inflammatory mechanisms of CHIP-related gene mutations, this paper discussed the mechanisms of CHIP-related diseases and possible intervention aimed at aging.

ABSTRACT

The normal ventilatory function is severely impaired by tracheal traumas, stenoses, tumors and some congenital diseases, which could result in tissue hypoxia and endangering the life of the patient. Resection and reconstruction of tracheal lesions is the most effective way to treat these diseases. At present, there is still no long-term safe and reliable method to achieve the reconstruction of long-segment trachea injury in clinical practice, and tissue-engineered trachea may be the solution to this situation. Cartilage, as one of the most important parts of tissue engineered trachea, plays a key role in providing mechanical support and maintaining the integrity of trachea. Tracheal tissue engineering cartilage regeneration process consists of several important parts, including the source of the cartilage cells, tissue engineering scaffold construction strategy and hydrogel composite scaffold material preparation, and the affecting factors of biological activity and application. This article reviews the new strategies of tissue engineered tracheal cartilage regeneration and the existing obstacles in order to provide reference for clinical practice.

ABSTRACT

Objective:To identify the potential pathogenic genes for perioperative neurocognitive disorder (PND) in the patients with digestive system tumors.Methods:The gene expression data of esophageal cancer, gastric cancer, colon cancer, rectal cancer and liver cancer in The Cancer Genome Atlas database were analyzed by bioinformatics analysis method, and the differentially expressed genes in tumor tissues in above-mentioned disease samples were identified compared with para-carcinoma tissues.Secretory proteome differential genes with the same expression trend in digestive system tumors were obtained by comparing with human secretory proteome genes.The correlation between secretomics and PND was determined by comparing with the GeneCards database.Hub genes were identified through PPI network construction and calculation, and the functions and signaling pathways of the above-mentioned differential genes were identified through GO and KEGG enrichment analysis.Results:Compared with para-carcinoma tissues, the expression of 2 640 genes was significantly up-regulated and the expression of 1 423 genes was down-regulated in esophageal cancer tissues; the expression of 3 748 genes was up-regulated and the expression of 908 genes was down-regulated in gastric cancer samples; the expression of 2 684 genes was up-regulated and the expression of 2 678 genes was down-regulated in colon cancer samples; the expression of 2 876 genes was up-regulated and the expression of 2 945 genes was down-regulated in rectal cancer samples; the expression of 1 484 genes was up-regulated and the expression of 723 genes was down-regulated in hepatocellular carcinoma samples.Among them, the expression of the encoding genes of 53 secreted proteins was uniformly up-regulated and the expression of the encoding genes of 20 secreted proteins was uniformly down-regulated in the above tumors.Twenty up-regulated genes and 3 down-regulated genes were associated with PND.PPI network analysis showed that MMP9 was the hub gene.The results of GO and KEGG analysis suggested that differentially expressed genes were mainly related to receptor-ligand activity, cytokine activity and chemokine activity, and were mainly enriched in signaling pathways related to cell cycle and cellular senescence.Conclusions:About 23 differentially expressed genes in digestive system tumors are potentially related to PND, of which MMP9 and other genes may be the hub genes, mainly acting on receptor-ligand binding, regulation of cytokine and chemokine activity, cell cycle, cellular senescence and other related signaling pathways.

Unhealthy eating behavior caused by stress is common in children and adolescents, which has not been fully clarified. In this paper, studies on the association of psychological stress with general eating behavior, abnormal eating behavior or eating disorders among children and adolescents were reviewed. The epidemiology and influencing factors were elaborated, and the possible physiological mechanism was summarized, in order to provide reference for the future research.

ABSTRACT

The human gut is populated by a complex community of microbiota that coexists with the host to maintain homeostasis.Macrogenomic and metabonomics analysis shows that the imbalance in gut microbiota composition and diversity, some bacterial components, metabolites and the signaling pathways mediated by them are involved in the occurrence and development of a variety of diseases, especially in the elderly.Intestinal microorganisms are one of the important factors affecting age-related diseases and can provide insights on treatment strategies for the elderly.This review focuses on the composition of the intestinal microbiota, interaction between the microbiota and the host, features of the gut microbiota in common geriatric diseases and pathogenic mechanisms.

To summarize the current application of electromagnetic navigation in orthopedics ,summarize its characteristics,and provide a better basis for clinical treatment.The domestic and foreign literatures on the application of electromagnetic navigation in orthopedics were reviewed and summarized.Various navigation systems have been used in various clinical fields.Among them,electromagnetic navigation systems have become more and more widely used in orthopedics ,and their status has become increasingly prominent.There are many navigation and positioning technologies,which should be comprehensively selected according to the actual clinical situation .Electromagnetic navigation has relatively obvious advantages and greater development potential ,and still needs a lot of clinical research to verify.

ABSTRACT

Large or complete defect of trachea is a difficult problem in surgical reconstruction. Tracheal replacement therapy is an effective treatment. Due to the lack of ideal graft materials and poor survival after transplantation, tracheal clinical transplantation still faces enormous challenges. Systematic review clinical translation of circumferential tracheal substitutes. Four primary methodologies have emerged: tracheal allotransplantation, autologous tissue reconstruction, bioprosthetic reconstruction, and tissue engineered reconstruction. The main reason for the failure of current clinical graft are focused on that the lacks the mechanical properties of maintaining lumen patency, poor revascularization, and insufficient regeneration of epithelial cells and chondrocytes. The results show that laboratory work and animal experimental research on revascularization, re-epithelialization and further improvement of the mechanical properties of grafts are particularly important in order to promote the safer, more effective and widely used tissue engineering trachea for clinical diseases.

ABSTRACT

Objective:Resorption can occur after lumbar disc herniation, and Thymic stromal lymphopoietin (TSLP) is considered to be a key factor mediating reabsorption. Studies have found that under the action of inflammatory factors like TNF-α, IL-6, etc,the expression of TSLP in intervertebral disc tissue was increased, and then mononuclear macrophage chemokine-1 (MCP-1) was induced to induce infiltration of macrophage to promote reabsorption. To determine the mechanism, we design the experiment to explore the mechanism of IL-6-mediated JAK/STAT pathway and TGF-β/Smad pathway to promote the reabsorption of intervertebral disc by regulating the expression of TSLP.Methods:Rat bone marrow mesenchymal stem cells (MSC) and rat nucleus pulposus cells (NPC) were cultured in vitro, treating the cells withrat IL-6 recombinant protein, STAT3inhibitor and Smad2/3 inhibitorrespectively, and use real-time quantitative PCR (qRT-PCR) technology to detect the expression of JAK1, STAT3, Smad2, TSLP mRNA under different conditions; Western blot to detect the expression of TSLP, Smad2 and phosphate STAT3 protein; using enzyme-linked immunosorbent assay (Elisa) to detect the expression of TGF-β in two rat cells under the treatment of IL-6.Results:After stimulation ofIL-6 (10 ng/ml or 100 ng/ml) the expression of JAK1in MSC (10 ng/ml: 5.13±1.21; 100 ng/ml: 5.23±0.35; control group: 0.97±0.03), STAT3 (10 ng/ml: 6.50±0.38; 100 ng/ml: 6.74±0.61; control group: 0.87±0.19) was significantly increased, and TSLP also showed high expression in MSC (10 ng/ml: 4.26±0.38; 100 ng/ml: 5.05±0.46; control group: 1.04±0.04).The expression of STAT3 (10 ng/ml: 2.91±0.08; control group: 1.12±0.11), TSLP (10 ng/ml: 7.32±0.37; control group: 1.03±0.03) in NPC also increased. After stimulation of IL-6, the expression of Smad2 increasing in MSC was observed (10 ng/ml: 15.92±0.62; 100 ng/ml: 20.28±0.58; control group: 0.96±0.08), and increased expression of Smad2 in NPC (10 ng/ml: 5.01±0.17; control group: 0.96±0.03). The expression of TSLP in MSC decreased after adding STAT3 inhibitor (BP group, BP group: 0.17±0.01; control group: 0.90±0.09), the expression of TSLP also decreased in NPC (BP group: 0.42±0.11; control group: 0.90±0.11). After adding Smad2/3 inhibitor (SB group), the expression of TSLP in MSC decreased (SB group: 0.33±0.01; control group: 1.02±0.02), and the expression of TSLP in NPC also decreased (SB group: 0.40±0.04; control group: 0.99±0.01).Conclusion:IL-6 up-regulates TSLP expression via the JAK1/STAT3 signaling pathway and promotes prominent intervertebral disc reabsorption. At the same time, IL-6 can activate the TGF-β/Smad2/3 pathway and up-regulate the expression of TSLP, which play a synergistic role in the reabsorption process.

Low back pain is becoming an important factor that affects people's quality of life today, and the social losses caused by lowback pain are hugeevery year. Lumbar disc herniation (LDH) is one of the main diseases that cause low back pain. The mechanism of lumbar disc herniation in the biomedical science is still controversial. Inflammatory factor is a cytokine secreted by tissue cells and involved in mediating the inflammatory response. Studies have shown that some factors stimulated by the extrusive nucleus pulposus, like inflammatory factors, degeneration-related genes and downstream expression products, can cause the degeneration of intervertebral disc. IL-1, IL-6, TNF-α, MMPs, and TGF-β have become the hot topicin disc degeneration. Signaling pathway is the main pathway for inflammatory factors to participate in the regulation of various biochemical reactions in cells. The inflammatory factors interact with different proteins to activate or inhibit different pathways, thereby achieving regulation of the cell cycle, regulates gene expression, induces immune inflammatory response, and apoptosis. Research on the role of various inflammatory factors in the body and related molecular signaling pathways will help us understand the mechanism of LDH. Most of the experimental studies only focus on the influence of a certain cytokine or single pathway on intervertebral disc degeneration, but different inflammatory factors and their signaling pathways often crosstalk with each other through special channels, forming a complex and precise signal transduction regulation network jointly regulates various physiological or pathological processes in the body, and the occurrence of disease is often accompanied by multiple factors. Studying the effect of a single signal network on the disease cannot fully explain the cause of the disease and related clinical manifestations. Therefore, clarifying the role of various inflammatory factors in IDD and exploring and analyzing the ways in which each factor regulates each other will provide ideas for understanding the mechanism of lumbar degeneration and exploring new methods for preventing and treating LDH in the future.

ABSTRACT

This article introduces the development history of palliative medicine at home and abroad, investigates the existing problems of palliative medicine teaching in geriatrics, explores the improvement of geriatric palliative medicine education, and elaborates adopting theoretical teaching and clinical practice, including PBL teaching, case teaching and narrative medicine teaching, to carry out palliative medical education for geriatrics graduate students. The results showed that 67.9% of the students have increased the learning sense of palliative medicine, 92.5% of them think that palliative medicine is very important, and 75% of them think that they have enhanced their ability of language expression and communication with patients with advanced diseases, namely, the ability of end-of-life care.

ABSTRACT

Tissue engineering provides an effective treatment for tracheal reconstruction. The key to maintain airway function and ensure long-term survival of tracheal grafts is how to achieve epithelial cell regeneration, chondrocyte regeneration, and angiogenesis of the transplanted trachea. Growth factors are signaling molecules that direct cell development by providing biochemical cues for stem cell proliferation, migration, and differentiation. They play an important role in tissue engineering trachea regeneration. It is particularly important to design a delivery system that provides optimal activity, stability and adjustability for growth factors. This article summarizes the types, characteristics, functions of growth factors applied in the field of tissue engineering trachea, and the value of growth factors in tissue engineering trachea orthotopic transplantation aimed for promoting clinical transformation of tissue engineered trachea.

ABSTRACT

OBJECTIVE: To study the effects of glycyrrhizic acid on the pharmacokinetics of nifedipine in rats. METHODS: Rats were randomly divided into experimental group and control group, with 10 rats in each group. Experimental group was given glycyrrhizic acid 5 mg/kg and control group was given 0.5% CMC-Na (sodium carboxymethylcellulose) solution, once a day, for 14 consecutive days. On 14th day after 30 min of intragastric administration, both groups were given nifedipine 3 mg/kg intragastrically. Blood samples 0.5 mL were collected from intraocular vein plexus before and at 0.25, 0.5, 0.75, 1, 1.5, 2, 3, 4, 6, 8, 12 and 24 h after intragastric administration. The concentration of nifedipine was determined by HPLC using diazepam as internal standard. The determination was performed on ODS-C18 column with mobile phase consisted of methanol-water (62 ∶ 38,     V/V,pH adjusted to 4.5 with acetic acid) at the flow rate of 1.0 mL/min. The column temperature was 30 ℃. The detection wavelength was set at 238 nm, and sample size was 20 μL. The pharmacokinetic parameters were calculated with Winonlin 6.0 software, and statistical analysis was performed by t-test. RESULTS: The main pharmacokinetic parameters of the experimental group and the control group were as follows as tmax was (1.40±0.15), (1.50±0.01) h; cmax was (0.15±0.03), (0.29±0.09)      mg/L; t1/2 was (4.70±1.17), (5.20±1.38) h; AUC0-24 h were (1.00±0.10), (1.89±0.37) mg·h/L; AUC0-∞ was (1.00±0.16), (1.98±0.32) mg·h/L; MRT was (6.76±0.64), (6.60±1.36) h, respectively. Compared with control group, cmax, AUC0-24 h and AUC0-∞ of nifedipine were decreased significantly in experimental group, with statistical significance (P<0.05). CONCLUSIONS: Glycyrrhizic acid can reduce the bioavailability of nifedipine in rats. It is suggested that the dosage of nifedipine should be increased in order to achieve effective blood concentration.

With the development of precision medicine in the field of tumor,thanks to the latest screening technology,the demand for real-time monitoring of tumor is increasingly high.Traditional tissue biopsy is not suitable for repeated due to clinical problems such as puncture clinical risk and tumor heterogeneity.Ultrasound,endoscopy and other imaging examinations are highly subjective,and it is difficult to detect small lesions,and increasingly unable to meet the requirements of real-time monitoring.However,circulating tumor DNA,with its advantages of simple sample acquisition,small trauma,repeatability and real-time monitoring efficacy,has an important application prospect in tumor diagnosis and treatment.Based on the latest reports at home and abroad,this paper reviews the research progress of circulating tumor DNA in the early diagnosis,treatment and prognosis of esophageal cancer.

ABSTRACT

Tissue engineering is a comprehensive discipline that combines materials science, life sciences, and engineering to repair, and improve and preserve damaged tissues or organs through cell or tissue reconstruction. In recent years, with the rapid development of tissue engineering technology, tissue engineering trachea has gradually become a new approach to tracheal replacement therapy. However, due to the slender and periodic distribution of the blood vessels supplying the trachea, tracheal grafts cannot obtain sufficient blood supply to maintain its demand, making its vascularization problem one of the major obstacles to the development of tissue engineering trachea. In the construction of tissue engineering trachea, the vascularization strategy of seed cells, tracheal scaffold and growth factors have gradually become the focus of research. In this paper, the current researches on tissue engineering tracheal vascularization were reviewed.

Methods We collected the new rural cooperative medical system ( NRCMS ) data of Funan county ( with "Countywide Medical Community" introduced in 2015, along with Capitation Payment) and Yingshang county (without"Countywide Medical Community"), both in Anhui province, from 2014 -2016. With such data, a longitudinal comparison was made on the performance of Funan "Countywide Medical Community" before and after, and a horizontal comparison on Funan county and Yingshang county regarding the merits and weakness of the Community. Results 2014-2016 witnessed a year-by-year decline of the outside-county proportion of the expenditure, man-time of inpatients, hospitalization expenses and hospitalization reimbursement by NRCMS in Funan county, and an increase of such proportion within the county. Furthermore, these proportions of Funan county were better than those of Yingshang county, proving "Countywide Medical Community" a success. Conclusions The performance of the combination of "Countywide Medical Community" and "Capitation Payment" proved a success.
