ABSTRACT

ObjectiveTo investigate the prevalence and epidemiological characteristics of norovirus in adult cases with infectious diarrhea in Putuo District of Shanghai, and provide scientific evidence for the prevention and control of infectious diarrhea caused by norovirus. MethodsFecal samples, clinical information and epidemiological data were collected from January 2013 through December 2019 in surveillance hospitals in Putuo District of Shanghai. Norovirus was examined by real-time fluorescent quantitative reverse transcription polymerase chain reaction (rRT-PCR). ResultsIn 1 389 adult cases with infectious diarrhea, norovirus positive rate was 25.41%, which was significantly higher in male (27.16%) than female (23.89%). Furthermore, in 353 cases positive for norovirus, GⅡ group was the most common (77.98%).The positive rate was highest in the cases aged 30-44 years. Spring, autumn and winter were the seasons with higher incidence of norovirus (September to May). The norovirus-infected cases had more nausea, vomiting, hyperactivity of bowel sounds, and watery stool, compared to the negative cases (P<0.05). ConclusionThe detection rate of norovirus remains high in adult cases with infectious diarrhea in Putuo District of Shanghai, of which GII is predominant. Seasonality may be spring, autumn and winter. Therefore, it warrants the countermeasures, such as surveillance and health education, for prevention and control of norovirus in susceptible population during epidemic seasons.

ABSTRACT

The rapid development of professional technology not only brings great benefits to patients, but also reveals the problem of non-technical skills. Technical competence is not enough to avoid the occurrence of adverse medical events or to get optimal post-operative outcomes. The development of technology is endless, we are desperately in need of non-technical skills, such as situation awareness, decision making, communication and teamwork, leadership. The only way we could achieve in the assistance of the perfect surgical operation with the combination of excellent surgical techniques and solid non-technical skills, and therefore relieve the patients as much as possible.

ABSTRACT

To investigate the effect of Yiqi Wenyang (YQWY) decoction on reversing cardiac hypertrophy induced by the transverse aortic constriction (TAC). Wistar rats aged 7-8 weeks were subjected to TAC surgery and then randomly divided into 4 groups (n = 5/group): Sham group, TAC group, low-dose group and high dose group. After 16-week intragastric administration of YQWY decoction, the effect of YQWY decoction on alleviating cardiomyocyte hypertrophy was examined by transthoracic echocardiography (TTE), hematoxylin/eosin (HE), wheat germ agglutinin (WGA) staining, enzyme linked immunosorbent assay (ELISA), Western blot (WB), immunohistochemistry (IHC) and immunofluorescence (IF), respectively. The results showed significant differences in left ventricle volume-diastole/systole (LV Vol d/s), N-terminal pro-B-type brain natriuretic peptide (NT-proBNP) (P < 0.01), Ejection Fraction (EF), LV mass and fractional shortening (FS) (P < 0.05) between YQWY-treated group and TAC group. HE and WGA staining showed that treatment with YQWY decoction dramatically prevented TAC-induced cardiomycyte hypertrophy. Moreover, the results of WB, IHC and IF indicated that administration of YQWY could suppress the expressions of cardiac hypertrophic markers, which included the atrial natriuretic peptide (ANP), BNP and myosin heavy chain 7 (MYH7) (P < 0.05) and inhibit phosphorylation of GATA binding protein 4 (P-GATA4) (P < 0.05), phosphorylation of extracellular signal-regulated kinase (P-ERK) (P < 0.05), phosphorylation of P38 mitogen activated protein kinase (P-P38) (P < 0.05) and phosphorylation of c-Jun N-terminal kinase (P-JNK) (P < 0.05). Thus, we concluded that YQWY decoction suppressed cardiomyocyte hypertrophy and reversed the impaired heart function, and the curative effects of YQWY decoction were associated with the decreased phosphorylation of GATA4 and mitogen activated protein kinases (MAPKs), as well as the reduced expression of the downstream targets of GATA4, including ANP, BNP, and MYH7.

ABSTRACT

Objective To analyze the prevalence, awareness, treatment, control and self-management of diabetes among the elderly in Midu County of Yunnan province. Methods A stratified random sampling method was used to carry out on-site questionnaire survey and blood glucose measurement among 1,603 older people aged≥60 in Midu County of Yunnan province. Results The overall prevalence, awareness, treatment and control rate of diabetes among the elderly were 10.11%, 56.17%, 75.82%and 37.36%respectively. The rates of self-monitoring of blood glucose, taking medicine prescribed by doctors and hypoglycemic measures in the last two weeks were 43.96%, 80.22% and 98.90% respectively. People with higher average annual income had higher prevalence of diabetes ( <0.05) . The awareness rate of diabetes in women (67.42%) was significantly higher than that in males (42.47%, <0.01). Self monitoring of blood glucose in patients with house toilet (46.99%) in house was higher than those without toilet (12.50%, <0.05). The higher the level of education, the higher the rate of taking medicine prescribed by doctors ( <0.05) . Patients who do not live alone and have toilet in house have a higher rate of weight control (67.07%and 66.27%) than those living alone and without toilet (22.22%and 25%,<0.05).Conclusions The findings indicate a relatively high prevalence of diabetes and low level of self-management of diabetes among older people in Midu County. It should adopt targeted diabetes health education and intervention according to different characteristics of the population so as to improve their level of diabetes self-management.

ABSTRACT

Adipocyte plasma membrane associated protein (APMAP) is a novel integral membrane protein, widely expressed in many organizations in humans. It promotes the differentiation of preadipocytes to mature adipocytes, so as to maintain normal physiological metabolism function in adipocytes, and plays a major role in adipocytes differentiation. Nowadays, some progress has been made in its role of inflammation and consequently in pathogenesis of gestational diabetes mellitus and other diseases. This paper gives a brief review about the structure and function of APMAP and sums up progress of the current studies.

ABSTRACT

<p><b>BACKGROUND</b>Early surgical intervention is required for the primary congenital glaucoma (PCG). There are currently few reports on the surgical outcomes in infants with PCG. This study aimed to evaluate the efficacy and safety of trabeculotomy and the postoperative visual outcomes in Chinese newborns with PCG within 4 weeks of birth.</p><p><b>METHODS</b>A total of 21 eyes of 12 patients with PCG who underwent primary trabeculotomy within 4 weeks of birth were retrospectively studied. Preoperative and postoperative intraocular pressure (IOP), corneal clarity and diameter, axial length and optic disc cupping, visual acuity and postoperative refractive error, success rates, and complications were evaluated. Kaplan-Meier survival analysis was applied to evaluate the success rates.</p><p><b>RESULTS</b>The mean follow-up time was 46.9 ± 34.4 months (range: 12-122 months). The postoperative IOP was significantly lower than the preoperative IOP at all of the follow-up visits (P < 0.001). The complete success rates for all eyes at 1, 2, 3, and 5 years postoperatively were 90.5%, 85.7%, 85.7%, and 85.7%, respectively. The IOPs of the three patients who needed antiglaucomatous medications postoperatively were also well controlled. At the last visit, the cornea became clear, and the cup-to-disc ratio decreased significantly (P = 0.01) although the horizontal corneal diameter did not change significantly (P = 0.11). Visual acuities were able to be recorded in eight eyes at the last visit, among which six eyes had a best-corrected visual acuity of 20/40 or better. There were no severe intraoperative or postoperative complications.</p><p><b>CONCLUSIONS</b>Trabeculotomy proves to be a safe and effective treatment in reducing IOP in this group of Chinese newborns with PCG. The outcomes of vision function were satisfactory in most of the patients.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the association between C825T polymorphism of G protein beta3 subunit (GNB3) gene and different Hilit types of essential hypertension (EH) in the Uygur nationality of Xinjiang.</p><p><b>METHODS</b>According to Uygur medical theories, EH patients (as the EH group) and non-EH patients (as the control group) were assigned to four Hilit groups. The C825T polymorphism of GNB3 was detected in 161 EH patients and 379 non-EH subjects of different Hilit types by using polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) to explore the difference of the genotypes and allelic frequencies and hypertension.</p><p><b>RESULTS</b>(1) In Xinjiang Uygur population, the distribution frequencies of GNB3 C825T polymorphism were in accordance with Hardy-Weinberg (chi2 = 0.871, P = 0.647). (2) There was no statistical difference in the distribution frequencies of three genotypes and two alleles of GNB3 between the EH group and the control group (P > 0.05). (3) There was statistical difference in distribution frequencies of three genotypes between the abnormal Sapra and non-abnormal Sapra group (the sum of abnormal Sewda, abnormal Kan, and abnormal Balhem) (chi2 = 6.905, P = 0.032), especially between the abnormal Sapra and abnormal Balhem groups (chi2 = 10.404, P = 0.006), but there was no statistical difference in distribution frequencies of alleles between the two groups (P > 0.05). (4) In 161 EH patients, there was statistical difference in the distribution frequencies of three genotypes and two alleles between the abnormal Sapra and non-abnormal Sapra group (chi2 = 9.034, P = 0.011; chi2 = 4.701, P = 0.03).</p><p><b>CONCLUSIONS</b>Both TT genotype and T allele of GNB3 C825T polymorphism might not be associated with EH patients in Xinjiang Uygur populations. However, they were correlated with hypertension patients of non-abnormal Sapra, indicating the pathogeneses of EH with different Hilit types might be different.</p>

ABSTRACT

OBJECTIVE: To study the pharmacokinetics of intravenous busulfan in Chinese adult patients undergoing allogeneic hematopoietic stem cell transplantation and its correlation with clinical outcome. METHODS: Blood specimens of 34 patients were collected following the first, fifth and seventh dose of an intravenous administration of busulfan at 1.6 mg·kg-1 every 12 hours for 4 days. The concentrations of busulfan in blood plasma were determined by LC-MS. The pharmacokinetic parameters were calculated according to non-compartment model by WinNoLin statistical software. RESULTS: The pharmacokinetics behavior after dose 1 and dose 7 of intravenous busulfan in 34 patients were all fit to a one compartment model. The main pharmacokinetic parameters were as follow: CL (4.1±1.8), (4.6±1.9) mL·min-1·kg-1; Vd (1.3±0.5), (1.2±0.5) L·kg-1; AUC0-12 (1218.5±351.0), (1501.2±444.3)μmol·min·L-1; AUC0-∞ (1694.8±741.7), (1882.2±754.1) μmol·min·L-1; cav(1.8±0.6), (2.2±0.7) μmol·L-1. AUC and cav of dose 7 were higher than dose 1 (P<0.05). The patient's body weight negatively correlated with CL (P<0.05). CONCLUSION: The pharmacokinetics profiles of twice-daily intravenous busulfan are fit to a one compartment model. The pharmacokinetics of busulfan varied in different doses and patients. Therapeutic drug monitoring (TDM) of busulfan will be important to clinical treatment.

ABSTRACT

Inactivation and microbial regrowth of penicillin-, ampicillin-, cefalexin-, tetracycline-, chloramphenicol-, and rifampicin-resistant bacteria were studied to explore risks associated with selection and regrowth of antibiotic-resistant bacteria after PAA disinfection. The results showed that after exposure to 20 mg/L PAA for 10 min, inactivation of ampicillin-resistant bacteria reached 2.3-log, which was significantly higher than that of total heterotrophic bacteria with a decrease of 2.0-log. In contrast, inactivation of tetracycline- resistant bacteria was significantly less efficient, reaching only 1.1-log. Chloramphenicol-and tetracycline-resistant bacteria, as well as total heterotrophic bacteria regrew more than 10 fold compared to those in the untreated wastewater sample with 22 h stilling culture after exposure to 2 or 5 mg/L PAA as for 10 min. Selection and potential regrowth of tetracycline-and chloramphenicol-resistant bacteria are potential risks when utilizing PAA disinfection, which may induce the spread of specific antibiotic-resistant bacteria in reclaimed water.

ABSTRACT

<p><b>OBJECTIVE</b>To study the effects of quercetin, a flavonoid, on the learning and memory ability of 3-day-old neonatal rats with hypoxic-ischemic brain white matter damage (WMD).</p><p><b>METHODS</b>Sixty 3-day-old Sprague-Dawley rats were randomly divided into four groups: control, WMD model, and quercetin treatment groups (20 and 40 mg/kg). There were 15 rats in each group. Rats in the WMD model and the two quercetin treatment groups were subjected to right common carotid artery ligation followed by 2 hrs of exposure to 8% O2 to induce periventricular white matter injury. After the operation quercetin was administered daily in the two quercetin treatment groups for 6 weeks. Six weeks later, Morris water maze and open-field tests were carried out to test memory and learning ability as well as behavior and cognition.</p><p><b>RESULTS</b>From the second day of training, escape latency in the Morris water maze test was more prolonged in the WMD model group than in the control group (P<0.01). The escape latency in the two quercetin treatment groups was shortened significantly compared with the WMD model group (P<0.05). The WMD model group crossed the original platform fewer times compared with the control and quercetin treatment groups (P<0.05). The open-field test indicated that the number of rearings increased and time spent in the centre was extended in the WMD model group compared with the control group. Compared with the WMD model group, the number of rearings was significantly reduced (P<0.05) and time spent in the centre was significantly shortened in the quercetin treatment groups (P<0.05).</p><p><b>CONCLUSIONS</b>Quercetin treatment can improve memory and learning ability as well as cognitive ability in neonates with WMD, suggesting that quercetin protects against WMD resulting from hypoxia-ischemia.</p>

ABSTRACT

Objective To investigate the effects of simulated high-altitude hypoxia on the detection rate and endotoxin level of Porphyromonas gingivalis (Pg) of subgingival bacterial plagues in rabbit periodontitis models. Methods Forty male rabbits were randomly divided into four groups, namely, normoxia control group (group A1), normoxia experimental group (group A2), hypoxia control group (group B1), and hypoxia experimental group (group B2). Each group included 10 rabbits. Periodontitis models was established in groups A2 and B2 combined by ligating both lower central incisors with steel ligature and feeding periodontitis diets, and then the animals were housed in a hypoxia chamber (simulating 5000m altitude, 23h per day). Groups A1 and A2 were raised normal diet in normoxia environment. After eight weeks, the rabbit periodontitis model was evaluated by observing radiographic features of the X-ray films and histopathologic changes under a light microscope. Subgingival plague sample from periodontal pockets on both lower central incisors were collected for isolation, culture and identification of Pg, and for detection of the endotoxin level. Results The histopathologic observation and X-ray examination results showed that the periodontitis of rabbits in group B2 was significantly more severe than that in group A2. The detection rates of Pg in group A1, A2, B1 and B2 was 0%, 50%, 55% and 95% (P<0.05). Pg detection rate and endotoxin level were higher in group B2 (95%, 0.46 0.04EU/ml) than in group A2 (50%, 0.38x 0.02EU/ml, P<0.05). Conclusions The process speed and damage degree of periodontitis in hypoxic environment is higher than that in normoxic environment. Moreover, the hypoxic environment is more suitable in the colonization of Pg with higher endotoxin level in subgingival plague.

ObjectiveTo evaluate the effect of Bone mesenchymal stem cells(BMSCs) intraarticular injection on the pathological development of collagen-induced arthritis(CIA) rats.MethodsCIA Wistar rats were divided into 2 groups:the saline injection control group and the BMSCs injection group.BMSCs were isolated from normal rats and cultured in vitro till P2 generation, which were injected into the affected the ankle joint consequently. After 2,4,8,12 weeks of processing, the following indexes were observed:arthritis score and X-ray.Twelve weeks later, all of the rats were sacrificed and the following histological indexes were evaluated:cartilage destruction and cartilage extracellular matrix secretion.Paired samples t-test was used for the statistical comparison.ResultsAll the arthritis indexes of the BMSCs group were much lower.than those of the saline control group. The arthritis scores were 3.18±0.62 vs 3.84±0.35 (at week 2),3.45±0.28 vs 5.96±0.48(at week 4),3.86±0.23 vs 6.75±0.36(at week 8),4.23±0.43 vs 7.86±0.66 (at week 12) respectively.X-ray scores were 2.04±0.21 vs 2.72±0.15(at week 2),2.52±0.47 vs 4.06±0.38 (atweek 4),3.56±0.29 vs 4.35±0.36 (at week 8),3.73±0.43 vs 4.86±0.62(at weekl2) respectively and the histological evaluation indexes were 2.34±0.22 vs 3.52±0.55 (at week 12).ConclusionBMSCs injection may inhibit the pathological development of CIA rats, and it may be a promising approach for the treatment of rheumatoid arthritis.

ABSTRACT

<p><b>BACKGROUND</b>Ahmed glaucoma valves (AGV) has been used for decades, but there is no detailed report about the efficacy of AGV in Chinese glaucoma patients. This study aimed to compare the intraocular pressure (IOP) lowering efficacy and side effects of S-2 polypropylene and PF-7 silicone AGV implantation in Chinese refractory glaucoma patients.</p><p><b>METHODS</b>Patients were divided into S-2 model AGV group and FP-7 model AGV group. The complete and qualified surgical success rate, change of IOP, number of anti-glaucoma medications used and postoperative complications were recorded and analyzed.</p><p><b>RESULTS</b>Average follow-up time was comparable between two groups. IOP was reduced from (37.9 ± 12.7) mmHg preoperatively to (17.3 ± 5.3) mmHg at the last follow-up in S-2 group and reduced from (39.9 ± 14.4) mmHg to (17.7 ± 4.9) mmHg in FP-7 group. Anti-glaucoma medications were reduced from 3.8 ± 0.2 to 1.5 ± 0.2 in S-2 group, and 3.5 ± 0.2 to 0.7 ± 0.2 in FP-7 groups. The cumulative success rates were comparable in two groups, which were 61.2% and 72.1% in S-2 group and FP-7 group respectively. When IOP reduction criteria was used, complete success rates were 30.6% and 51.2% for S-2 and FP-7 groups, and qualified success rates were 86.1% and 92.7% separately. In both groups, the major complication was hypotony, and the previous trabeculectomy of patients was the major risk factor for surgery failure.</p><p><b>CONCLUSIONS</b>In this short-term retrospective study, S-2 AGV is showed at least as effective as FP-7 AGV in IOP reduction, but associated with higher rate of complications. Previous trabeculectomy is a principle risk factor for AGV implantation failure. These clinical outcomes are important for converting use of the FP-7 silicon AGV in Chinese refractory glaucoma patients.</p>
