The Hedgehog (HH) signaling pathway plays important roles in gastrointestinal carcinogenesis and the gastrointestinal tumor microenvironment (TME). ABSTRACT

INTRODUCTION@#Surgical resection of the primary and metastatic tumour is increasingly recommended in suitable patients with metastatic colorectal cancer (CRC). While the role of metastasectomy is well studied and established in colorectal liver metastasis, evidence remains limited in pulmonary metastases. This systematic review was conducted to examine the current evidence on the role of lung metastasectomy (LUM) in CRC.@*METHODS@#Three databases were systematically searched, to identify studies that compared survival outcomes of LUM, and factors that affected decision for LUM.@*RESULTS@#From a total of 5,477 records, 6 studies were eventually identified. Two papers reported findings from one randomised controlled trial and 4 were retrospective reviews. There was no clear survival benefit in patients who underwent LUM compared to those who did not. When compared against patients who underwent liver metastasectomy, there was also no clear survival benefit. Patients who underwent LUM were also more likely to have a single pulmonary tumour, and metachronous disease.@*CONCLUSION@#The evidence suggests a role for LUM, but is limited by inherent selection bias in retrospective reviews, and the single randomised clinical trial performed was not completed. More prospective studies are required to understand the true effect of LUM on outcomes in metastatic CRC.

ABSTRACT

Objective@#To explore the excellent methods for aesthetic repair of the donor sites of flaps.@*Methods@#From January 2013 to March 2018, 120 patients (94 males and 26 females, aged from 3 to 60 years) were admitted to the Department of Burns of Beijing Jishuitan Hospital. Wounds areas after debridement or removing scar were ranged from 8.0 cm×3.5 cm to 24.0 cm×18.0 cm. Twenty patients with facial and neck scar were repaired with expanded flaps, including 4 scalp flaps, 8 supraclavicular flaps, 4 deltoid flaps, and 4 trapezius myocutaneous flaps. The flaps in ideal donor sites were selected to repair the wounds in 40 patients, including 20 cases of hand wounds or scars repaired with inguinal flaps, 10 children of foot skin defects or scars repaired with cross inguinal skin flap, 10 cases of knee joint wounds repaired with medial or lateral thigh flaps. The optimal flap design was used to repair wounds in 50 patients. Among the patients, wounds of 36 patients were repaired with relaying flaps, including donor sites of free anterolateral thigh flaps of 8 patients repaired with anteromedial thigh perforator flaps and donor sites of free anterolateral thigh flaps of 8 patients repaired with ilioinguinal flaps or superficial abdominal artery flaps, and donor sites of flaps of 20 patients repaired with peroneal perforator relaying flaps. Besides, wounds of 9 patients were repaired with free lobulated anterolateral thigh flaps, and wounds of 5 patients were repaired with modified V-Y propelling latissimus dorsi myocutaneous flaps. The donor sites of flaps were repaired with allogenic acellular dermal matrix combined with autologous split-thickness skin grafts in 10 cases. The areas of the flaps or myocutaneous flaps were ranged from 6.0 cm×4.0 cm to 30.0 cm×20.0 cm. The survival of flap, myocutaneous flap, or skin graft and the repair of donor site after operation and during follow-up were observed.@*Results@#Blood flow obstacle at 0.5 cm to the distal margin of the flap occurred in 1 patient repaired with expanded flap, which were healed after dressing change. Blood supply disorder occurred at the tip of the anteromedial thigh perforator flap of 1 patient repaired by optimal flap design, which were healed completely after second debridement and restitching. The other flaps or myocutaneous flaps survived well. The allogenic acellular dermal matrix and the autologous split-thickness skin graft survived with good color and texture. During follow-up of 3 months to 4 years, the donor sites of flaps had good appearance, only with linear scar and the function recovered well. The donor sites of skin grafts had no scar hyperplasia, only with scattered pigmentation.@*Conclusions@#According to the characteristics of donor sites of flaps, individualized and reasonable design before the operation such as pre-expanding of the flaps, selecting the ideal donor sites, optimization of the flap design or allogenic acellular dermal matrix combined with autologous split-thickness skin graft to repair donor sites of flaps can minimize the damage for function and appearance of donor sites of flaps and achieve aesthetic effects of donor sites of flaps.

ABSTRACT

OBJECTIVE@#To study the value of serum gamma-glutamyl transpeptidase (GGT) combined with direct bilirubin (DB) in the diagnosis of biliary atresia.@*METHODS@#A total of 667 infants with cholestasis who were hospitalized and treated from July 2010 to December 2018 were enrolled as subjects. According to the results of intraoperative cholangiography and follow-up, they were divided into biliary atresia group with 234 infants and cholestasis group with 433 infants. The two groups were compared in terms of age of onset, sex, and serum levels of total bilirubin (TB), DB, alanine aminotransferase (ALT), aspartate aminotransferase (AST), total bile acid (TBA), and GGT. A receiver operating characteristic (ROC) curve analysis was performed for indices with statistical significance, and the area under the ROC curve (AUC) and the optimal cut-off value for diagnosis were calculated.@*RESULTS@#The biliary atresia group had a significantly younger age of onset than the cholestasis group (P0.05), while the biliary atresia group had significantly higher serum levels of TB, DB, TBA, and GGT than the cholestasis group (P<0.05). GGT combined with DB had the highest AUC of 0.892 (95% confidence interval: 0.868-0.916) in the diagnosis of biliary atresia. At the optimal cut-off values of 324.0 U/L for GGT and 115.1 μmmol/L for DB, GGT combined with DB had a sensitivity of 79.8% and a specificity of 83.2% in the diagnosis of biliary atresia.@*CONCLUSIONS@#GGT combined with DB has high sensitivity and specificity in the diagnosis of biliary atresia and can be used as an effective indicator for diagnosis of biliary atresia in infants.

Objective@#To evaluate hepatectomy by anterior approach combined with liver hanging maneuver against conventional right hepatectomy in patients with large hepatocellular carcinoma (HCC).@*Methods@#Of the 62 cases, 29 patients underwent anterior approach hepatectomy and 33 did conventional hepatectomy.@*Results@#The overall operative time (t=1.815, P=0.037) and parenchymal transsection time (t=4.591, P=0.000) were longer in the anterior approach group than in the conventional group. More cases got radical resection in the anterior group (χ2=7.280, P=0.007). Log-rank test showed that 1, 3 and 5-year overall survival rate (OS) in the anterior approach group and the conventional group was 86.1%, 50.1%, 32.6% vs 75.8%, 30.3%, 18.9%, respectively (χ2=5.24, P=0.022), 1, 3 and 5-year disease free survival rate (DFS) were 75.3%, 42.1%, 31.0% vs 66.1%, 24.4%, 10.1%, respectively (χ2=4.38, P=0.037), and recurrence rate were 23.6%, 49.5%, 64.8% vs 36.7%, 63.7%, 82.5%, respectively (χ2=5.61, P=0.018).@*Conclusion@#Long term survival of patients with large sized HCC undergoing hepatectomy through the anterior approach combined with liver hanging maneuver is better than that of conventional approach.

ABSTRACT

OBJECTIVE:To evaluate the risk factors of early gastric cancer with lymph node metastasis.METHODS:Clinicopathological data of 220 early gastric cancer cases admitted at Changhai Hospital of Navy Military Medical University from January 2015 to November 2018 were analyzed retrospectively. The statistical analysis was conducted to evaluate gender, age, BMI, personal history, family history, tumor location, pathological features.RESULTS:The difference of lymph node metastasis between male and female(10%vs.22%,χ2=5.469,P=0.019),patients with tumor diameter <2 cm and ≥2 cm(7%vs.21%,χ2=8.375,P=0.004),patients with mucous invasionand submucous invasion(5%vs.28%,χ2=21.455,P<0.001),patients with ulceredand without ulcer(20%vs.10%,χ2=4.151,P=0.042),among patients with differentiated histology(11%, 27%, 12%respectivly,χ2=6.143,P=0.046)were statistically significant. Univariate analysis showed a positive relationship between depth of tumor invasion(OR=0.115,P<0.05)and lymph node metastasis in early gastric cancer.CONCLUSION:Female, tumor diameter≥2 cm, in submucous invasion and mixed histology may be risk factors for lymph node metastasis in early gastric cancer,which need further analysis.

ABSTRACT

The introduction of left ventricular assist device (LVAD) has improved survival rates for patients with end-stage heart failure. Two categories of VADs exist: one generates pulsatile flow and the other produces nonpulsatile continuous flow. Survival is better for patients with continuous-flow LVADs. With improved survival, more of such patients now present for noncardiac surgery (NCS). This review, written for the general anesthesiologists, addresses the perioperative considerations when the patient undergoes NCS. For best outcomes, a multidisciplinary approach is essential in perioperative management of the patient.

Objective@#To analyze the correlation between circumcision and incidence and clearance of male genital HPV infection.@*Methods@#From May to July 2014, 18-55 year old men who had sexual behavior history were recruited from the general population in Liuzhou, Guangxi to set up a cohort. Totally, 113 circumcised and 560 uncircumcised men were enrolled and interviewed using a questionnaire (including information on demographic characteristics and sexual behaviors), then they were followed-up with 6-month interval for 2 times. On each visit, specimens of male external genitalia were collected and genotyped for HPV DNA. The differences of incidence and clearance of genital HPV infections between circumcised and uncircumcised men were analyzed by Log-rank test. Cox regression was used to analyze the relationship between circumcision and incidence and clearance of HPV infection.@*Results@#The median age (P25, P75) of circumcised and uncircumcised men were 28 (24, 35) and 32 (24, 31), respectively. The incidences of any HPV infections were 9.1 (95%CI: 2.4-15.7) and 8.4 (95% CI: 5.6-11.2) per 1 000 person-months (χ2=0.10, P=0.758), respectively. The clearance of circumcised men [136.3 (95%CI: 70.0-202.7) per 1 000 person-months] was higher than that in uncircumcised men [89.6 (95%CI: 65.9-113.3) per 1 000 person-months] (χ2=8.19, P=0.004). In multivariate COX regression analysis, compared with uncircumcised men, circumcised men had higher possibility to clear any HPV infections (HR: 2.41, 95%CI: 1.30-4.46). Compared with men having one sexual partner, people having more than 4 sexual partners had lower possibility to clear any HPV infections (HR: 0.49, 95%CI: 0.25-0.96). Compared with 18-25 years old men, men aged 26-35 years old had higher possibility to clear high-risk HPV infections (HR: 2.14, 95%CI: 1.08-4.23).@*Conclusion@#Circumcised and uncircumcised men had similar incidence of genital HPV infection, whereas, men conducted circumcision and having fewer sexual partners could increase the clearance of genital HPV infections.

ABSTRACT

Objective To explore the effect of preservation of inclined fractured permanent molars by transverse fixation with screw post,and provide a practical method for reserving the inclined fractured permanent molars.Methods Clinical data of 50 patients with inclined fractured permanent molars by transverse fixation with screw post in the Department of Stomatology of Tianjin First Central Hospital between January 2010 to December 2011 were reviewed retrospectively.After fixing the crown with steel-wire binding and root canal treatment,the molars were drilled a hole buccolingually and placed screw post.The flowable light-curing composite resin was used to seal the crack and bond screw post to prevent the relative movement.All treated molars were restored with full crown.All patients were followed up for 10-60 months.The therapeutic effects were observed.Results After 5 year-follow-up,41 cases were cured completely,6 cases were cured clinically,and 3 cases failed treatment.Conclusion The transverse fixation with screw post and bonding technique can preserve the inclined fractured molars to gain chewing function and avoid or reduce the extraction of affected teeth,which provides a better restoration effect.

Objective To demonstrate the relationship between Cdc20 mutation and the promotion of colon cancer via Cdc20loxp/+ APCmin/+ villin-cre+/-compound mutant mice.Methods Cdc20loxp/+ APCmin/+ villin-cre+/-compound mutant mice and APCmin/+ mutant mice were generated by mice mating strategy.The colon tumors of two group mice were compared by phenotypic analysis and histology analysis.Results Phenotypic analysis showed that the number of tumors in Cdc20loxp/+ APCmin/+ villin-cre+/-compound mutant mice group and APCmin/+ mutant mice group was 1.2±0.5 and 1.6±0.5, respectively (t =0.215, P =0.588), and the maximum diameter of tumors was (2.7±0.3) cm and (2.5±0.2) cm, respectively (t =0.568, P =0.575).Pathologic type of Cdc20loxp/+ APCmin/+ villin-cre+/-compound mutant mice was adenocarcinoma, while that of APCmin/+ mice was tubular adenoma.Conclusion Cdc20 carrying a null allele can accelerate the promotion of colon cancer in APCmin/+ mice without influence on the tumor number and size.

ABSTRACT

Programmed cell death 4 (PDCD4) is a RNA-binding protein that acts as a tumor suppressor in many cancer types, including colorectal cancer (CRC). During CRC carcinogenesis, PDCD4 protein levels remarkably decrease, but the underlying molecular mechanism for decreased PDCD4 expression is not fully understood. In this study, we performed bioinformatics analysis to identify miRNAs that potentially target PDCD4. We demonstrated miR-181b as a direct regulator of PDCD4. We further showed that activation of IL6/STAT3 signaling pathway increased miR-181b expression and consequently resulted in downregulation of PDCD4 in CRC cells. In addition, we investigated the biological effects of PDCD4 inhibition by miR-181b both in vitro and in vivo and found that miR-181b could promote cell proliferation and migration and suppress apoptosis in CRC cells and accelerate tumor growth in xenograft mice, potentially through targeting PDCD4. Taken together, this study highlights an oncomiR role for miR-181b in regulating PDCD4 in CRC and suggests that miR-181b may be a novel molecular therapeutic target for CRC.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the suitable transfection condition of human epidermal cells (hECs) with human epidermal growth factor (EGF) gene by adenovirus vector (Ad-hEGF) and its effects on the biological characteristics of hECs.</p><p><b>METHODS</b>hECs were isolated from deprecated human fresh prepuce tissue of circumcision by enzyme digestion method and then sub-cultured. hECs of the third passage were used in the following experiments. (1) Cells were divided into non-transfection group and 5, 20, 50, 100, 150, and 200 fold transfection groups according to the random number table (the same grouping method below), with 3 wells in each group. Cells in non-transfection group were not transfected with Ad-hEGF gene, while cells in the latter six groups were transfected with Ad-hEGF gene in multiplicities of infection (MOI) of 5, 20, 50, 100, 150, and 200 respectively. The morphology of the cells was observed with inverted phase contrast microscope, and expression of green fluorescent protein of the cells was observed with inverted fluorescence microscope at transfection hour (TH) 24, 48, and 72. (2) Another three batches of cells were collected, grouped, and treated as above, respectively. Then the transfection rate of Ad-hEGF gene was detected by flow cytometer (n=3), the mass concentration of EGF in culture supernatant of cells was detected by enzyme-linked immunosorbent assay (n=6), and the proliferation activity of cells was detected by cell counting kit 8 (CCK8) and microplate reader (n=6) at TH 24, 48, and 72, respectively. (3) Cells were collected and divided into non-transfection group and transfection group, with 6 wells in each group. Cells in non-transfection group were cultured with culture supernatant of cells without transfection, while cells in transfection group were cultured with culture supernatant of cells which were transfected with Ad-hEGF gene in the optimum MOI (50). CCK8 and microplate reader were used to measure the biological activity of EGF secreted by cells on culture day 1, 3, and 5. (4) Cells were collected and divided into non-transfection group and transfection group, with 12 wells in each group. Cells in non-transfection group were not transfected with Ad-hEGF gene, while cells in transfection group were transfected with Ad-hEGF gene in the optimum MOI (50). The expression levels of cytokeratin 14 (CK14) and CK19 of cells were measured by immunofluorescence staining at TH 24. (5) Cells were collected, grouped, and treated as in (4), with 6 wells in each group. At post scratch hour (PSH) 0 (immediately after scratch), 12, 24, and 48, the migration distance of cells was observed and measured with inverted phase contrast microscope. Data were processed with analysis of variance of factorial design, analysis of variance for repeated measurement, and LSD test.</p><p><b>RESULTS</b>(1) At TH 24 and 48, morphology of cells in each transfection group and non-transfection group were similar. Compared with that in non-transfection group, the cell debris increased significantly in 200 fold transfection group at TH 72. At TH 24, 48, and 72, the expression of green fluorescent protein was not seen in cells of non-transfection group, whereas it increased in cells of transfection group over transfection time. (2) The transfection rate of Ad-hEGF gene of cells in each transfection group increased gradually over transfection time. At TH 72, the transfection rates of Ad-hEGF gene of cells in 50-200 fold transfection groups were all above 90%, while the transfection rates of Ad-hEGF gene of cells in non-transfection group, 5, and 20 fold transfection groups were (0.51±0.20)%, (62.44±6.23)%, and (75.00±5.43)% respectively, which were obviously lower than the rate in 50 fold transfection group [(93.12±2.55)%, with P values below 0.01]. The mass concentration of EGF in culture supernatant of cells in each transfection group increased gradually over transfection time. At TH 72, the mass concentration of EGF in culture supernatant of cells in 50 fold transfection group was obviously higher than that in each of the other groups (with P values below 0.01). The proliferation activity of cells in each group at TH 24 and 48 was similar (with P values above 0.05). At TH 72, the proliferation activity of cells in 200 fold transfection group was obviously lower than that in other groups (with P values below 0.05). (3) On culture day 1, the biological activity of EGF secreted by cells in two groups was similar (P>0.05). On culture day 3 and 5, the biological activity of EGF secreted by cells in transfection group were obviously higher than that in non-transfection group (with P values below 0.01). (4) At TH 24, the expression levels of CK14 and CK19 of cells in transfection group were higher than those in non-transfection group. (5) The width of scratch in two groups was nearly the same at PSH 0. At PSH 12-48, the migration distance of cells in transfection group was obviously longer than that in non-transfection group (with P values below 0.01).</p><p><b>CONCLUSIONS</b>The suitable range of MOI of hECs transfected with Ad-hEGF gene is 50-150, and 50 is the optimum. hECs transfected with Ad-hEGF gene with MOI 50 can effectively express the EGF gene and keep its good abilities of proliferation, differentiation, and migration, as well.</p>

ABSTRACT

PurposeTo optimize the imaging parameters of clinical MRI scanner in rat pancreas imaging to improve the image quality and to provide better MRI image quality and more economical research method for imaging study of rat pancreas. Materials and Methods Twenty-four healthy male Wistar rats were randomly divided into the conventional sequence (CS) group, the adjustment sequence (AS) group and the optimization sequence (OS) group, with 8 rats in each group. The rats in the CS group were scanned with conventional parameters using a clinical MRI scanner. The principle of parameter adjustment was: parameters associated with T1WI or T2WI imaging quality (TR, TE, slice thickness, NEX, FOV and matrix) was set with four changes, and only one of the six parameters was changed in each scan, image quality was evaluated by two senior radiologists, the parameter corresponded the best image quality evaluated consistently by two radiologists were selected as the optimal imaging parameter, all the optimized parameters were set up step by step in this way which formed the imaging parameters in OS group. The pancreatic signal intensity and signal to noise ratio was compared between CS group and OS group after imaging.Results The optimized sequence parameters in clinical MRI scanner were listed below: T1WI sequence (M3D/FSPGR/15): TR 6 ms, TE 2.5 ms, slice thickness 2.0 mm, NEX 8, FOV 7 cm×7 cm, Matrix 120×120; T2WI sequence (FSE-XL/90): TR 4000 ms, TE 71 ms, slice thickness 2.0 mm, NEX 1, FOV 8 cm×8 cm, Matrix 192×160. The pancreatic SI in T1WI and T2WI sequence of the OS group were significantly higher than those in the CS group (t=5.16 and 3.80,P<0.01), while the pancreatic SNR in T1WI and T2WI sequence of the OS group were significantly higher than those in the CS group (t=5.65 and 3.26,P<0.01).Conclusion The optimized parameters can improve the imaging quality of rat pancreas MRI significantly, thus provide a reference for the related experimental study.

Methods A total of 125 male Wistar rats were randomly allocated to five groups according to the treatments during 8 weeks: normal control (group A, n=25), periodontitis(group B, n=25), calcitonin (group C, prepared in sterile saline at 2 mg/L, and 2μg/kg was administered daily, s.c. , n=25), 1,25 vitamin D (group D, prepared in corn oil daily at a concentration of 2 mg/L, and 2μg/kg was administered daily, p.o. , n=25), 1,25 vitamin D plus calcitonin (group E, n=25). The experimental model of periodontitis was induced by ligating floss around mandibular first molars with orthodontic wires in B,C,D and E groups. Five rats from each group were sacrificed, and the specimens were prepared at 2, 4, 6 and 8-week. The probing depth (PD) and alveolar bone level were observed. The serum bone-specific alkaline phosphatase (BALP) and osteocalcin (OC) were measured by enzyme-linked immunosorbent assay (ELISA) at 8-week in each group. Results The values of PD were significantly lower after 4, 6 and 8 weeks in E group than those of B, C and D groups (P<0.05). The alveolar bone loss was significantly lower after 6 and 8 weeks in group E compared with that of B, C and D groups (P<0.05). The serum levels of BALP and OC were significantly higher after 8 weeks in E group than those of B, C and D groups (P<0.05). Conclusion The present study suggests that 1, 25 vitamin D and calcitonin can partially inhibit the alveolar bone loss induced by periodontitis. Especially, the application of both is more effective than either drug treatment alone.

ABSTRACT

Objective To explore the influence of different lengths and contents of glass-fiber post on the fracture resistance of teeth, and to investigate the optimal length and content of glass-fiber post used for restorative treatment. Meth-ods Forty-two intact single canal mandibular premolars were collected from orthodontic patients and were divided into three experimental groups (A, B and C). All the teeth were endodontically treated after cut off the crowns from the 2 mm above cemento-enamel junction. The post lengths were10 mm in A group, 8 mm in B group, and 6 mm in C group.There were two subgroups for each experimental group (A1, A2, B1, B2, C1, C2 ), 7 teeth for each subgroup.The 42%glass fiber post was used in A1, B1 and C1 groups. The 75%glass fiber post was used in A2, B2 and C2 groups. All of groups were restored with resin cores and metal crowns. The fracture resistance was measured with universal testing machine and recorded the root frac-ture pressure data and the mode of all the teeth. Results The fracture resistance was significantly higher in group A than that of group B and group C (P<0.05), and the fracture resistance was significantly higher in group B than that of group C (P<0.05). The fracture resistance was significantly higher for 75%glass fiber post than that of 42%glass fiber post (P<0.05). Conclusion Within a certain range, the longer the length of glass fiber post, the greater the tooth fracture resistance. The fracture resistance of fiber content is better for 75%glass fiber post than that of 42%glass fiber post.

ABSTRACT

Objective To investigate the effective endodontic irrigation protocols on smear layer removal from curved canals. Methods Forty extracted mandibular first molars with curved mesial root canal were selected and decoronat-ed. Mesiobuccal root canal was instrumented to F3 using handed ProTaper system, and randomly divided into four groups ac-cording to final irrigation protocols: syringe irrigation (group A), #30K file agitation (group B), passive ultrasonic irrigation (PUI, group C) and combination of #30K file agitation and PUI (group D). The ultrasonic file was located within 0.5 mm coro-nal from start of the curvature. All root canals were mesio-distally split, snicked at the start of the curvature and 2 mm apical-ly from the start of the curvature, and divided into three sections. They wereⅠ,ⅡandⅢfrom coronal to apical. The smear layer of each section of root canal was examined under scanning electron microscope (SEM). Results Group D presented small amounts of scattered or thin smear layers and obtained the lowest score at each section of root canal wall. At sectionⅠandⅡ, there was no significant difference in smear layer score between group D and group C. The smear layer scores were both significantly lower in group D and group C than those of group A and group B (P<0.05). At sectionⅢ, the smear layer score was significantly lower in group D than that of group A, group B and group C, and which was significantly lower in group B and group C than that of group A (P<0.05). Conclusion When ultrasonic file was located coronal from the start of the curvature, K file, which was the same size as dimension of apical portion preparation to agitate and PUI can remove smear layer from curved canals efficiently.

ABSTRACT

Objective To investigate the influence of perioperative continuing aspirin therapy on tooth extraction surgery in elderly patients with coronary heart disease.Methods A total of 926 patients with coronary heart disease who required tooth extraction were enrolled in this study.Among them,241 patients discontinued aspirin therapy before tooth extraction,685 patients continued aspirin therapy before tooth extraction.The entering rate of operation was evaluated.100 patients continuing aspirin and 100 patients discontinuing aspirin were randomly selected.Systemic pressure,diastolic pressure and heart rate before and after tooth extraction were detected.100 matched healthy elderly patients were selected as normal control group.Multivariate logistic regression analysis was used to estimate the risk for tooth extraction.Results The entering rate of operation was 67.9% (131 cases) in 193 patients with angina pectoris discontinuing aspirin therapy and 82.1 % (312 cases) in 380 patients with angina pectoris continuing aspirin therapy (x2 =14.77,P<0.01).The entering rate of operation was 41.7% (20 cases) in 48 patients undergoing coronary revascularization and discontinuing aspirin therapy,and 80.7% (312 cases) in 305 patients undergoing coronary revascularization and continuing aspirin therapy (x2=33.95,P<0.01).The changes in systolic blood pressure before versus after dental extraction had a significant difference between the discontinuing aspirin group and control group [(15.9±5.5) mmHg vs.(12.2±4.7)mmHg,P<0.05,1 mmHg=0.133 kPa],while had no significant difference between the continuing aspirin group and control group [(13.6±4.5) mmHg vs.(12.2±4.7) mmHg,P>0.05].There were no significant differences in changes in diastolic blood pressure and heart rate in the intraoperative period between the patients and controls (P>0.05).The bleeding rate in patients continuing aspirin therapy was not significantly different as compared with control group (P> 0.05).Multivariate logistic regression analysis revealed that perioperative discontinuing aspirin therapy remained the second significant risk factor for tooth extraction.Conclusions Preoperative administration of aspirin should be continued in dental extraction surgery in elderly patients with coronary heart disease.

ABSTRACT

BACKGROUND:Basic fibroblast growth factor is a pluripotent cytokine that can promote the proliferation of mesodermal and neuroectodermal cells. OBJECTIVE:To observe the effect of basic fibroblast growth factor in human periodontal ligament cells cultured in vitro. METHODS:Human periodontal ligament cells at passage 5 were inoculated into the 96-wel plates at the density of 1×108/L, and were randomly divided into four groups. The cells were cultured inα-MEM containing 15%fetal bovine serum and 0, 1, 10, 100μg/L basic fibroblast growth factor, respectively. At 1, 3, 5, 7 days of the culture, the cellproliferation was determined, and the activity of alkaline phosphates was detected at 1 and 7 days. RESULTS AND CONCLUSION:There were significant differences in the proliferation of human periodontal ligament cells among the four groups (F=6.586, P=0.024). As the increase of the basic fibroblast growth factor concentrations, the absorbance value was gradual y increased and reached the peak in 100μg/L basic fibroblast growth factor group (P<0.05). The alkaline phosphatase activity in basic fibroblast growth factor groups was lower than that of the control group (P=0.000), the higher the concentration was, the lower activity was (P<0.05). Results show that basic fibroblast growth factor can promote the proliferation of human periodontal ligament cells and inhibit the activity of alkaline phosphatase, and the effect is concentration-dependent.

ABSTRACT

Objective To compare the short-term efficacies of laparoscopic adjustable gastric banding (LAGB) and laparoscopic sleeve gastrectomy (LSG) in the treatment of obesity and type 2 diabetes mellitus,and to investigate the relationship between the body weight loss and the decrease of glucose of the 2 treatment methods.Methods The clinical data of 40 patients with obesity combined with type 2 diabetes mellitus who were admitted to the Changhai Hospital of the Second Military Medical University from January 2010 to December 2011 were retrospectively analyzed.There were 14 patients treated by LAGB (LAGB group) and 26 by LSG (LSG group).The body weight loss and the decrease of glucose at postoperative year 1 of the 2 groups were compared.The differences in the body weight and glucose before and after operation within groups were compared using the paired t test,and the differences in the body weight and glucose between the 2 groups were compared using the multiple analysis of variance.The correlation between the body weight loss and the decrease of glucose was analyzed using the linear regression analysis.Results Laparoscopic surgery was successfully done without conversion to open surgery or intraoperative complications.The operation time and volume of blood loss were (69 ± 16)minutes and (31 ± 14)mL in the LAGB group,(120 ± 15) minutes and (148 ± 48) mL in the LSG group.Complications including postoperative malnutrition,electrolyte disturbance,delayed gastric emptying,bleeding,anastomotic leakage did not occurr in the 2 groups.Two patients were complicated with abdominal incision fat liquefaction,and were cured by symptomatic treatment.(1) The body weight,body mass index (BMI) and waistline had a decrease trend.The body weight,BMI and waistline in the LAGB group were decreased from (117 ± 28)kg,(40 ± 8)kg/m2 and (118 ± 15) cm before operation to (94 ± 28) kg,(33 ± 8) kg/m2 and (92 ± 15) cm at postoperative week 48.The body weight,BMI and waistline in the LSG group were decreased from (119 ± 25)kg,(42 ± 6)kg/m2 and (123 ± 14)cm before operation to (74± 16)kg,(26± 4)kg/m2 and (86 ± 13)cm at postoperative week 48.The EWL had an increase trend in the 2 groups.The EWL in the LAGB group was increased from 7% ± 2% at postoperative week 1 to 53%± 24% at postoperative week 48,and the EWL in the LSG group was increased from 11% ± 4% at postoperative week 1 to 90% ± 20% at postoperative week 48.There were significant differences in the changes of body weight,BMI,waistline and EWL between the 2 groups (F =60.660,74.490,57.650,90.020,P < 0.05).(2) The levels of glycosylated hemoglobin,fasting glucose,fasting insulin,HOMA-IR,triglyceride,total cholesterol,low-density lipoprotein and high density lipoprotein in the LAGB group were decreased from 8.1%± 0.8%,(8.4±0.6)mmol/L,(21±8)μmol/L,7.9 ±2.9,(1.68±0.50)mmol/L,(6.0±1.1)mmol/L (4.1 ± 0.8) mmol/L,(1.09 ±0.15)mmol/L to 6.4% ±0.8%,(6.3 ±0.3) mmol/L,(10 ± 3) μmol/L,2.7 ±0.9,(1.04 ± 0.09) mmol/L,(4.3 ± 0.8) mmol/L,(2.3 ± 0.4) mmol/L,(1.22 ± 0.09) mmol/L at postoperative week 48 ; the levels of glycosylated hemoglobin,fasting glucose,fasting insulin,HOMA-IR,triglyceride,total cholesterol,low-density lipoprotein and high density lipoprotein in the LSG group were changed from 7.9% ± 1.0%,(9.0±1.0)mmol/L,(21 ±9) μ mol/L,8.5 ±3.5,(2.09 ± 0.70) mmol/L,(6.0 ± 1.2)mmol/L,(3.9 ± 1.1) mmol/L,(1.06 ± 0.21) mmol/L before operation to 5.1% ± 0.8%,(5.2 ± 0.4) mmol/L,(4 ± 1)μmol/L,0.9±0.2,(1.22±0.17)mmol/L,(4.2±0.8)mmol/L,(2.3 ±0.6)mmol/L,(1.30±0.13)mmol/L at postoperative week 48.There was a decrease trend of the glycosylated hemoglobin,fasting glucose,fasting insulin,HOMA-IR,trigluceride,total cholesterol and low density lipoprotein and a increase trend of the high density lipoprotein in the 2 groups.There were significant differences in the levels of glycosylated hemoglobin,fasting glucose,fasting insulin,HOMA-IR,triglyceride,total cholesterol,low-density lipoprotein and high density lipoprotein between the 2 groups (F =57.650,74.270,36.750,42.960,10.870,30.650,32.560,11.490,P <0.05).The levels of glucose of the LAGB group at postoperative month 1 and 3 were influenced by BMI (b =0.543,0.753,P < 0.05),while the levels of glucose of the LAGB group did not be influenced by BMI at postoperative month 6 and year 1 (b =0.130,0.222,P >0.05).The levels of glucose of the LSG group did not be influenced by BMI at postoperative month 1,3,6 and year 1 (b =0.185,0.035,0.212,0.126,P >0.05).Conclusions The efficacy of LSG is superior to LAGB for the treatment of obesity combined with type 2 diabetes mellitus.The efficacy of LAGB is correlated with the change of body weight,while the efficacy of LSG does not correlated with the change of body weight.
