ABSTRACT

ObjectiveTo explore the mechanism of Xiaoyaosan in alleviating lipopolysaccharide (LPS)-induced depressive-like behavior in mice based on the c-Jun N-terminal kinase (JNK) pathway. MethodAfter adaptive feeding, C57BL/6J mice were randomly divided into normal group, model group, minocycline group (intrabitoneal injection, 50 mg·kg-1), fluoxetine group (intragastric administration, 2.6 mg·kg-1), and low-, medium-, and high-dose Xiaoyaosan groups (intragastric administration,6.012 5, 12.025, and 24.050 g·kg-1). After 14 days of administration, the model group and each administration group were intraperitoneally injected with 2 mg·kg-1 LPS, and the normal group was intraperitoneally injected with equal volume of normal saline. Depressive-like behavior in mice was assessed using the open field test and the elevated zero maze test. High-performance liquid chromatography (HPLC) was used to measure the levels of norepinephrine (NE) and epinephrine (E) in the mouse hippocampus. Enzyme-linked immunosorbent assay (ELISA) was performed to determine serum interleukin-1β (IL-1β) levels. Immunohistochemistry was used to measure the protein expression levels of ionized calcium-binding adapter molecule-1 (Iba-1), c-Fos, and c-Jun. Real-time polymerase chain reaction (Real-time PCR) was used to measure mRNA expression levels of IL-1β, c-Jun, c-Fos, and JNK3 in the mouse hippocampus. Protein expression levels of JNK and phosphorylated (p)-JNK in the mouse hippocampus were measured using capillary protein automated protein expression analysis system (Western). ResultCompared with the normal group, the model group exhibited significantly reduced central area residence time, crossing times, and travel distance in the open field (P<0.01), significantly increased serum IL-1β levels (P<0.01), significantly decreased NE and E levels (P<0.05), upregulated mRNA expression of IL-1β, JNK3, and c-Fos, and increased protein expression of Iba-1, c-Fos, and c-Jun (P<0.05, P<0.01). Compared with the model group, the Xiaoyaosan groups showed increased central area residence time and open arm residence time (P<0.05), increased NE and E levels (P<0.01), decreased mRNA expression of IL-1β, JNK3, c-Jun, and c-Fos, and decreased protein expression of Iba-1, c-Fos, JNK, and p-JNK (P<0.05, P<0.01). The minocycline group and the fluoxetine group showed decreased mRNA expression of JNK3, c-Jun, and c-Fos (P<0.05, P<0.01). The minocycline group showed decreased serum IL-1β and p-JNK protein expression (P<0.01). The fluoxetine group exhibited increased NE and E levels and decreased c-Fos protein expression (P<0.01). ConclusionXiaoyaosan can improve depressive-like behavior induced by LPS in mice, and its mechanism may be related to the inhibition of neuroinflammatory responses and the JNK pathway.

ABSTRACT

The present study analyzed and identified the chemical constituents from ethyl acetate(EA) extract of Taxilli Herba with UPLC-Q-Exactive-MS and screened active xanthine oxidase(XO) inhibitors with HPLC. The analysis was performed on an Hypersil GOLD C_(18) reversed-phase column(2.1 mm×50 mm, 1.9 μm), with the mobile phase of water containing 1% formic acid(A) and methanol(B) under gradient elution, the flow rate of 0.3 mL·min~(-1), and the injection volume of 5 μL. ESI source was used for MS and the compounds were collected in positive and negative ion modes. Xcalibur 4.1 was used to analyze the retention time, accurate relative molecular weight, and fragmentation of the compounds. The inhibitory activity of some known compounds on XO was screened by HPLC. Thirty chemical constituents were identified, including phenolic acids and flavonoids by experimental data combined with information of standards, data reported previously, and databases, such as MzCloud and ChemSpider. The activities of 10 chemical components were screened. Gallic acid and naringenin chalcone had strong inhibitory activities on XO with IC_(50) of 57 μg·mL~(-1) and 108 μg·mL~(-1). UPLC-Q-Exactive-MS allows the accurate, rapid, and comprehensive identification of main chemical constituents from Taxilli Herba. Gallic acid and naringenin chalcone may be the active components of XO inhibitors.

ABSTRACT

Objective:To investigate short-term efficacy and safety of subcutaneous injection of dupilumab in the treatment of moderate-to-severe childhood atopic dermatitis (AD) .Methods:A retrospective study was conducted on clinical data from children who were diagnosed with moderate-to-severe AD and subcutaneously injected with dupilumab in Department of Dermatology, Beijing Children′s Hospital, Capital Medical University from March 2021 to August 2021. Changes in the Eczema Area and Severity Index (EASI), itch Numeric Rating Scale (NRS) score, SCORing Atopic Dermatitis (SCORAD) index, and Dermatology Family quality of life Index (DFI) were analyzed before and 4 weeks after the first subcutaneous injection of dupilumab. Adverse events were collected during the first injection to the first follow-up visit at week 4 after the start of treatment. Normally distributed measurement indices were compared by using paired t test, non-normally distributed measurement indices were compared by using signed rank test, and logistic regression analysis was used to evaluate the effects of disease duration, eosinophil counts, IgE levels, personal and family history of allergic diseases on EASI50 (≥ 50% decrease in the EASI score) after dupilumab treatment. Results:A total of 39 children were enrolled in this study, including 21 males and 18 females. Twenty-one patients were aged 2 to < 6 years, 18 were aged 6 to < 18 years, and their median age ( Q1, Q3) was 65.0 (53.0, 111.0) months. Four weeks after the single-dose subcutaneous injection of dupilumab, 18 patients (84.85%) achieved ≥ 50% decrease in EASI score, 13 (60.61%) ≥ 75% decrease in EASI score; 18 (75.76%) experienced a decrease of ≥ 4 points in peak NRS, and 20 (81.82%) ≥ 3 points in peak NRS; the SCORAD score decreased by ≥ 50% in 15 (68.75%) patients, and by ≥ 75% in 7 (18.75%). Neither common adverse events such as conjunctivitis, skin infections, injection site reactions, nor serious adverse events were observed in any of the children from the first injection to the first follow-up visit at week 4. Logistic regression analysis showed no significant effect of the disease duration, eosinophil counts, IgE levels, personal or family history of allergic diseases on EASI50 (all P > 0.05) . Conclusion:A single-dose subcutaneous injection of dupilumab can markedly improve pruritus and severity of skin lesions in children with moderate-to-severe AD, and enhance the family quality of life, with favorable short-term safety.

ABSTRACT

The wind, phlegm, and blood stasis are important pathogenic factors of cough variant asthma in children, and they are also the pathological products in the occurrence and development of this disease. They have typical pathogenic characteristics. The main pathogenesis characteristics of cough caused by wind, phlegm and blood stasis are as follows that external wind attacks the lungs and induces internal wind, phlegm and dampness accumulates in the lungs, and the lungs fail to declare and descend, and blood stasis obstructs the collaterals and stagnation of Qi. The wind, phlegm, and blood stasis have their own pathogenic characteristics, and their cough-causing also have their own pathogenic characteristics and clinical characteristics. Based on the characteristics of wind, phlegm, and blood stasis, the application of medicine based on the differentiation of symptoms and signs can effectively prevent and treat this disease, and provide theoretical basis and treatment ideas for the treatment of cough variant asthma in children with Chinese medicine.

ABSTRACT

ObjectiveTo explore the effect of kinematic alignment on postoperative knee function for patients after total knee arthroplasty (TKA). MethodsFrom June, 2020 to October, 2021, 200 patients undergoing primary TKA in Beijing Chaoyang Hospital were divided into mechanical alignment (MA) group (n = 100) and kinematic alignment (KA) group (n = 100). All the patients accepted comprehensive rehabilitation after operation. They were assessed with Keen Society Score (KSS) before and three months after operation, with Visual Analogue Scale for pain (VAS) before, and three days and three months after operation. The time of first standing, the time of first straight-leg raising more than 30°, and the active range of motion (AROM) of knee before, and one, two and three days, and one and three months after operation were recorded, as well as where to go after discharge. ResultsA total of 96 patients in MA group and 98 in KA group finished the research. The AROM of knee improved more in the KA group than in the MA group after operation (Fgroup = 8.816, P = 0.017), and the incidence going to the rehabilitation institutes was less (χ2 = 6.542, P = 0.011). ConclusionKA may promote the rapid recovery of AROM of knee for patients after TKA, and reduce the needs of institute-based rehabilitation after discharge, to save medical costs.

ABSTRACT

  Objective  To summarize the clinical and genetic features of children with autosomal dominant and recessive hyperimmunoglobulin E syndrome (HIES).  Methods  HIES patients were studied at the dermatology department of Beijing Children's Hospital, Capital Medical University were collected, from January 2013 to December 2021, diagnosed by both clinical manifestation and genetic assessment. The general data were summarized, the clinical and genetic characteristics were analyzed, and the similarities and differences between autosomal dominant HIES (AD-HIES) and autosomal recessive HIES (AR-HIES) were compared.  Results  A total of 7 children with HIES were studied, including 3 cases of AD-HIES and 4 cases of AR-HIES. There were 4 males and 3 females. All children had recurrent eczema-like lesions, recurrent skin and pulmonary infections, and elevated serum IgE and eosinophil levels. The differences between AD-HIES and AR-HIES mainly include: the main cutaneous infection in 3 children with AD-HIES were bacterial infections (such as abscess and impetigo), while in 4 children with AR-HIES, cutaneous infections were mostly severe viral infection (such as verruca vulgaris and molluscum contagiosum). There were pulmonary parenchymal changes (such as pneumatoceles, cyst and atelectasis) in 3 children with AD-HIES, whilst there were no similar changes in the lungs of 4 children with AR-HIES; 75% of children with AR-HIES had allergic diseases (including asthma and food allergy), while there were no reports of allergic diseases in children with AD-HIES. As for manifestations outside of immune system, AD-HIES was more likely to appear facial dysmorphism(such a broad nasal bridge and a high-arched palate). Furthermore, the incidence of tumor in AR-HIES was higher than that in AD-HIES. AD-HIES was mainly caused by the mutation of STAT3 gene, and AR-HIES was mainly caused by the mutation of DOCK8 gene. We reported two new mutation sites of DOCK8 gene c.1798-2A > T and c.874G > A in two cases, respectively.  Conclusions  For children with clinical manifestations of recurrent eczema-like lesions, repeated infection and significant increase in serum IgE levels, HIES should be suspected, and genetic screening should be carried out to make definite diagnosis and classification, to achieve better long-term management and improve prognosis.

ABSTRACT

OBJECTIVE@#To evaluate the potential effectiveness of different screening strategies for cardiovascular diseases prevention in a community-based Chinese population from economically developed area of China.@*METHODS@#Totally 202 179 adults aged 40 to 74 years without cardiovascular diseases at baseline (January 1, 2010) were enrolled from the Chinese electronic health records research in Yinzhou (CHERRY) study. Three scenarios were considered: the screening strategy based on risk charts recommended by the 2020 Chinese guideline on the primary prevention of cardiovascular diseases in Chinese adults aged 40-74 years (Strategy 1); the screening strategy based on the prediction for atherosclerotic cardiovascular disease risk in China (China-PAR) models recommended by the 2019 Guideline on the assessment and management of cardiovascular risk in China in Chinese adults aged 40-74 years (Strategy 2); and the screening strategy based on the China-PAR models in Chinese adults aged 50-74 years (Strategy 3). According to the guidelines, individuals who were classified into medium- or high-risk groups after cardiovascular risk assessment by the corresponding strategies would be introduced to lifestyle intervention, while high-risk population would take medication in addition. Markov model was used to simulate different screening scenarios for 10 years (cycles), using parameters mainly from the CHERRY study, as well as published data, Meta-analyses and systematic reviews for Chinese populations. The life year gained, quality-adjusted life year (QALY) gained, number of cardiovascular disease events/deaths could be prevented and number needed to be screened (NNS) were calculated to compare the effectiveness between the different strategies. One-way sensitivity analysis on uncertainty of cardiovascular disease incidence rate and probabilistic sensitivity analysis on uncertainty of distributions for the hazard ratios were conducted.@*RESULTS@#Compared with non-screening strategy, QALYs gained were 1 433 [95% uncertainty interval (UI): 969-1 831], 1 401 (95%UI: 936-1 807), and 716 (95%UI: 265-1 111) for the Strategies 1, 2, and 3; and the NNS per QALY in the above strategies were 141 (95%UI: 110-209), 144 (95%UI: 112-216), and 198 (95%UI: 127-529), respectively. The Strategies 1 and 2 based on different guidelines showed similar effectiveness, while more benefits were found for screening using China-PAR models in adults aged 40-74 years than those aged 50-74 years. The results were consistent in the sensitivity analyses.@*CONCLUSION@#Screening for cardiovascular diseases in Chinese adults aged above 40 years seems effective in coastal developed areas of China, and the different screening strategies based on risk charts by the 2020 Chinese guideline on the primary prevention of cardiovascular diseases or China-PAR models by the 2019 Guideline on the assessment and management of cardiovascular risk in China may have similar effectiveness.

ABSTRACT

Objectives@#The coronavirus disease 2019 (COVID-19) pandemic has set a precedent for the fastest-produced vaccine as a result of global collaboration and outreach. This study explored Malaysians’ acceptance of the COVID-19 vaccine and its associated factors. @*Methods@#A cross-sectional anonymous web-based survey was disseminated to Malaysian adults aged ≥18 years old via social media platforms between July 10, 2020 and August 31, 2020. @*Results@#In the analysis of 4,164 complete responses, 93.2% of participants indicated that they would accept the COVID-19 vaccine if it was offered for free by the Malaysian government. The median out-of-pocket cost that participants were willing to pay for a COVID-19 vaccine was Malaysian ringgit (MYR) 100 (interquartile range [IQR], 100) if it was readily available and MYR 150 (IQR, 200) if the supply was limited. Respondents with a low likelihood of vaccine hesitancy had 13 times higher odds of accepting the COVID-19 vaccine (95% confidence interval [CI], 8.69 to 19.13). High perceived risk and severity were also associated with willingness to be vaccinated, with adjusted odds ratios of 2.22 (95% CI, 1.44 to 3.41) and 2.76 (95% CI, 1.87 to 4.09), respectively. Age and ethnicity were the only independent demographic characteristics that predicted vaccine uptake. @*Conclusion@#Public health strategies targeting perceived risk, perceived susceptibility and vaccine hesitancy could be effective in enhancing vaccine uptake.

ABSTRACT

Upon his untimely death, Michal Boym (1612-1659) left behind an enormous written legacy; covering a diverse range of interests in fields such as Chinese linguistics, cartography, botany, zoology, philosophy and medicine. This article attempts to examine Boym's De indiciis morborum ex linguae coloribus & affectionibus (The signs of disease on the tongue, colors and affections), a translation of a previously unidentified Chinese text. The text specifically deals with the subject of tongue diagnosis, in relation to contagious diseases that were still raging in the 17th century China. Arriving to China at the end of the Ming Dynasty (1368-1644), Boym was able to personally witness the early development of tongue examination as an independent specialty; he recognized its significance and tried to incorporate it into his ultimately unfinished Chinese medical corpus. Boym's work eventually facilitated the assimilation of tongue observation in European medicine during the 19th century.

ABSTRACT

Objective:To investigate the effect of low molecular weight heparin (LMWH) on the inflammatory response of PC12 cells induced by oxygen glucose deprivation (OGD) and its related mechanism.Methods:The PC12 cells were cultured in vitro were randomly divided into sham(control) group, OGD group, LMWH group and blocking agent group. The latter group was divided into six groups: Eritoran+ OGD group, LMWH+ Eritoran+ OGD group, ST2825+ OGD group, LMWH+ ST2825+ OGD group, pyrrolidinedithiocarbamate (PDTC)+ OGD group and LMWH+ PDTC+ OGD group. OGD cell model was established. Cell counting kit-8 (CCK-8) assay was used to detect cell activity. The expressions of toll-like receptor 4 (TLR4), MyD88 and nuclear factor κB (NF-κB) mRNA and protein were detected by real time polymerase chain reaction (qRT-PCR) and Western blot. The concentration of interleukin (IL)-1β, IL-6, tumor necrosis factor-α(TNF-α) and S100β were determined by enzyme linked immunosorbent assay (ELISA). Results:The cell activity of OGD group was significantly lower than that of control group on the first, second, third day ( P<0.05). Compared with OGD group, the activity of LMWH group was increased on the second, third day ( P<0.05), but lower than that of control group ( P<0.01). The mRNA expression of TLR4, MyD88 and NF-κB was significantly increased in OGD group compared with the control group ( F=144.9, F=710.5, 79.51, P<0.01). Compared with OGD group, the mRNA expression of TLR4, MyD88 and NF-κB were significantly decreased after treatment with LMWH ( P<0.01), and the specific inhibitor of TLR4, MyD88 and NF-κB enhanced the anti-inflammatory effect of LMWH. The protein expression of this pathway was consistent with that of the gene. The concentration of IL-1β, IL-6, TNF-α and S100β in OGD group was significantly higher than control group ( P<0.05). After treatment with LMWH, the concentrations of inflammatory factors and S100β were significantly decreased compared with OGD group ( P<0.01). When hinder TLR4, MyD88 and NF-κB respectively by Eritoran, ST2825 and PDTC, the concentrations of inflammatory factors and S100β were significantly decreased, but it was still higher than control group ( P<0.05). Conclusions:OGD can cause pathological damage of PC12 cells, including high expression level of S100β and aggravation of inflammatory reaction. LMWH can improve cell activity, down-regulate inflammatory reaction degree and protect the cells. Using inhibitors of TLR4/MyD88/NF-κB pathway to inhibit the corresponding target, the up-regulation of inflammatory factors by OGD can be inhibited in varying degrees. These suggested that LMWH may regulate inflammatory reaction of PC12 cells induced by OGD through TLR4/MyD88/NF-κB pathway.

ABSTRACT

Drug reactions with eosinophilia and systemic symptom (DRESS) are a severe drug eruption, which is characterized by fever, rash, lymphadenopathy, hematological system abnormalities and involvement of internal organs.It also has such clinical characteristics as delayed onset, relapsing symptoms and reactivation of human herpesvi-ruses.The clinical characteristics of DRESS make it different from other drug reactions, with high clinical heterogeneity, variability and unpredictability.DRESS in children has some unique characteristics that are different from adults, such as culprit drugs, rash characteristics, and involved systems and organs.Moreover, the course of disease is shorter, the severity is milder, the morbidity and mortality are lower, and the prognosis is better compared with adults.In this paper, the clinical characteristics and progress in diagnosis and treatment of DRESS in children are reviewed.

ABSTRACT

Objective:To investigate the clinical effect of Chaihu Shugansan combined with abdominal acupuncture on depression caused by chronic pain,and to explore its mechanism. Method:A total of 97 patients with depression caused by chronic pain were randomly divided into control group (49 cases) and observation group (48 cases). Patients in both groups received routine western medicine treatment,including necessary psychological intervention and taking paroxetine. Control groupobservation groupcontrol group Patients in control group were treated with Xiaoyaowan,and patients in observation group were treated with Chaihu Shugansan combined with abdominal acupuncture. Both groups were treated for 6 weeks. The levels of serum neurotransmitters,cytokines and Hamilton depression rating scale(HAMD) before and after treatment were compared between two groups<bold>.</bold> Result:There was no significant difference in HAMD scores of the two groups before treatment and the HAMD scores of two groups after treatment were significantly lower than those before treatment (<italic>P</italic><0.05),and the HAMD scores in observation group were significantly lower than those in the control group (<italic>P</italic><0.05). There was no significant difference in the levels of serum 5-hydroxytryptamine (5-HT),norepinephrine (NE),and brain-derived neurotrophic factor (BDNF) between two groups before treatment. After treatment,the levels of serum 5-HT,NE,and BDNF in two groups were significantly higher than those before treatment (<italic>P</italic><0.05),and the levels in observation group were significantly higher than those in control group (<italic>P</italic><0.05). There was no significant difference in the levels of serum interleukin-6 (IL-6),interleukin-1<italic>β</italic> (IL-1<italic>β</italic>) and tumor necrosis factor-<italic>α</italic> (TNF-<italic>α</italic>) before treatment. After treatment,the levels of serum IL-6,IL-1<italic>β</italic> and TNF-<italic>α</italic> were significantly lower than those before treatment (<italic>P</italic><0.05),and the levels in observation group were significantly lower than those in control group (<italic>P</italic><0.05). Conclusion:On the basis of psychological intervention and paroxetine administration,the combination of Chaihu Shugansan and abdominal acupuncture exerts their respective advantages. It treats both symptoms and root causes of depression,relieves the degree of depression,reduces the classification of depression,and regulates the levels of neurotransmitter and cellular inflammatory factors,and inhibits inflammatory response. The clinical effect is significant.

ABSTRACT

Objective:To observe the activation of microglia in hippocampus of depressed and anxious mice induced by maternal separation with acute restraint stress and the expression of interleukin-1<italic>β</italic>(IL-1<italic>β</italic>),interleukin-6(IL-6),tumor necrosis factor-<italic>α</italic>(TNF-<italic>α</italic>), investigating the mechanism of Wenyang Jieyu prescription in treating anxiety and depression. Method:Eighty four male C57BL offspring were randomly divided into control group, acute restraint stress group and model group on postnatal day 0(PD0). Maternal separation combined with acute restraint stress was used to prepare anxious and depressed model mice, dividing the model mice into model group, Wenyang, Jieyu, Wenyang Jieyu and fluoxetine group according to random number table method. During the period of PD21-PD90, the control, acute restraint stress and model mice were fed with normal diet, with the other groups fed with corresponding medicine mixed diet. The Wenyang, Jieyu and Wenyang Jieyu groups were given 5.85, 12.03 and 16.71 g·kg<sup>-1</sup>·d<sup>-1</sup> respectively. The fluoxetine group was given 2.60 mg·kg<sup>-1</sup>·d<sup>-1</sup>. Open field, zero maze test and social interaction tests were used to evaluate the anxiety and depression of model mice. The expression of Iba-1 in hippocampal microglia was detected by immunohistochemistry(IHC). The mRNA expression of IL-1<italic>β</italic>, IL-6, TNF-<italic>α</italic>, Iba-1 and glucocorticoid receptor(GR)were detected by Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR). Result:Compared with the control group, total movement distance and time spent in central zone in 5 min of the model mice significantly decreased(<italic>P</italic><0.01), time spent in opened arm and total movement distance decreased significantly(<italic>P</italic><0.05,<italic>P</italic><0.01), investigation time during testing and training increased significantly(<italic>P</italic><0.01). The expression of Iba-1 protein and mRNA,IL-1<italic>β</italic>,IL-6,TNF-<italic>α</italic> mRNA significantly increased(<italic>P</italic><0.01), the expression levels of GR mRNA significantly decreased(<italic>P</italic><0.01). The result of IHC staining showed that microglia were over activated. Compared with the model group, total movement distance and time spent in central zone in 5 min of mice in the Wenyang Jieyu and fluoxetine group significantly increased(<italic>P</italic><0.05,<italic>P</italic><0.01).Time spent in opened arm significantly increased(<italic>P</italic><0.01). Investigation time during testing and training significantly decreased(<italic>P</italic><0.05,<italic>P</italic><0.01). The expression of Iba-1 protein and mRNA,IL-1<italic>β</italic>,IL-6,TNF-<italic>α</italic> mRNA significantly decreased(<italic>P</italic><0.05,<italic>P</italic><0.01). The expression of GR mRNA increased significantly(<italic>P</italic><0.05,<italic>P</italic><0.01). IHC staining showed the microglia recovered. Time spent in opened arm of mice in the Wenyang group and Jieyu group significantly increased (<italic>P</italic><0.01), time spent investigating during testing decreased significantly (<italic>P</italic><0.05), the expression levels of Iba-1 protein and mRNA,IL-6 mRNA significantly decreased (<italic>P</italic><0.05,<italic>P</italic><0.01). The expression of GR mRNA of mice in the Wenyang group significantly increased (<italic>P</italic><0.05), the expression of TNF-<italic>α </italic>mRNA significantly decreased (<italic>P</italic><0.05). Total movement distance of mice in the Jieyu group increased significantly (<italic>P</italic><0.01), time spent investigating during training decreased significantly (<italic>P</italic><0.05),the expression level of IL-1<italic>β </italic>mRNA significantly decreased (<italic>P</italic><0.05). IHC staining showed that microglia recovered partly in both groups. Conclusion:The comprehensive curative effect and pharmacological action of Wenyang Jieyu prescription were better than Wenyang prescription and Jieyu prescription. Wenyang Jieyu prescription can treat anxiety and depression in maternal separation and acute restraint stress mice, its possible mechanism may be related to the decreased activation of microglia, down-regulation of IL-1<italic>β</italic>,IL-6,TNF-<italic>α</italic> expression and up-regulation of GR expression.

ABSTRACT

Objective:To observe the activation of microglia and the expression of inflammatory factors in hippocampus of mice with depression-like behavior after mother-infant separation (MS) combined with lipopolysaccharide (LPS) stress, and to explore the possible anti-depression mechanisms of Wenyang (WY), Jieyu (JY), and Wenyang Jieyu (WYJY) prescriptions from the perspective of warming Yang and relieving depression. Method:Seventy offspring mice were randomly divided into a normal group (<italic>n</italic>=10), a LPS stress group (<italic>n</italic>=10), and a modeling group (<italic>n</italic>=50). After undergoing 8 h·d<sup>-1 </sup>mother-infant separation during postnatal day 5 (PD<sub>5</sub>)–PD<sub>14</sub>, mice in the modeling group were further divided into the MS + LPS group, WY group, JY group, WYJY group, and fluoxetine (FLU) group, with 10 in each group. The birth date of the offspring mice was recorded as PD<sub>0</sub>. The mice in the normal, LPS, and MS + LPS groups were fed a normal diet during PD<sub>21</sub>–PD<sub>90</sub>, while those in the other groups were treated with the mixtures of corresponding drugs and feed, followed by seven-day intraperitoneal injection of LPS since PD<sub>91</sub> for inducing depression. The depression-like behavior of mice in each group was detected in the open-field, O-maze, and social interaction tests. The protein expression of microglia-specific ionized calcium-binding adaptor molecule 1 (Iba-1) in the hippocampus was assayed by immunohistochemistry, and the mRNA expression of interleukin-1<italic>β </italic>(IL-1<italic>β</italic>), interleukin-6 (IL-6), tumor necrosis factor-<italic>α </italic>(TNF-<italic>α</italic>), Iba-1, and glucocorticoid receptor (GR) by real-time fluorescence quantitative PCR (Real-time PCR). Result:Compared with the normal group, the LPS group exhibited significantly reduced residence time at the central area within 5 min (<italic>P</italic><0.01) and shortened total exercise distance (<italic>P</italic><0.01). In the MS + LPS group, the open-arm activity time and the total activity distance decreased significantly (<italic>P</italic><0.01, <italic>P</italic><0.05), whereas the training, discrimination and exploration time increased significantly (<italic>P</italic><0.01). The expression of Iba-1 in hippocampal CA1 region of mice in the LPS and MS + LPS groups was remarkably elevated (<italic>P</italic><0.01). Compared with the LPS group, the MS + LPS group displayed significantly prolonged distance of 5-min exercise (<italic>P</italic><0.05), increased training, discrimination and exploration time (<italic>P</italic><0.05, <italic>P</italic><0.01), and up-regulated Iba-1 expression in hippocampal CA1 area (<italic>P</italic><0.01). As revealed by comparison with the MS + LPS group, both the total 5-min exercise distance (<italic>P</italic><0.01) and the training and discrimination time (<italic>P</italic><0.01, <italic>P</italic><0.05) of mice in each administration group was significantly shortened. The discrimination and exploration time of mice in the JY, WYJY, and FLU groups was significantly reduced (<italic>P</italic><0.01), and the expression of Iba-1 in hippocampal CA1 region of mice in each administration group was significantly down-regulated (<italic>P</italic><0.01). Conclusion:The warming Yang and relieving depression method helps to inhibit the occurrence and development of depression due to its efficacy in activating microglia in hippocampus of depression mice and lowering the expression of IL-1<italic>β</italic>, IL-6, and TNF-<italic>α</italic>.

ABSTRACT

Objective:To seed for stable time window of the integrated disease-syndrome animal model based on the counterevidence from Chinese medicinal prescriptions, and to verify syndrome stability and reliability. Method:A model of depression was established by exposing rats to chronic unpredictable mild stress (CUMS), followed by body weight measurement, sugar water test, behavioral test, and brain 5-hydroxytryptamine(5-HT) detection. The identification of liver depression and spleen deficiency syndrome was conducted after the equivalent transformation of human clinical symptoms into macroscopic representations of rats. Based on the dynamically collected macroscopic representation scale, Xiaoyaosan was used to reversely verify the stability and reliability of the integrated disease-syndrome animal model of depression due to liver depression and spleen deficiency. Result:The sugar water consumption and the number of crossings and the total movement distance in the open field test of 16-week-old rats in the CUMS (eight weeks of CUMS) group were significantly lower than those in the normal group (<italic>P</italic><0.05). According to the immunohistochemical results, the 5-HT content in hippocampal area CA2 of rats in the CUMS group was also significantly lowered as compared with that in the normal group(<italic>P</italic><0.05),which indicated that depression was successfully modeled. The liver depression and spleen deficiency syndrome was present in 14-week-old rats (six weeks after CUMS)of the CUMS group, and the number of rats experiencing the liver depression and spleen deficiency syndrome reached the peak in the 16th week (eight weeks after CUMS),accounting for 70% of the total number. Thereafter, the number decreased gradually. The syndrome scores of the 14-, 16-, 18-, 20-, and 22-week-old rats in the Xiaoyaosan group were reduced by 66.6%, 70.7%, 54.8%, 50.4%, and 44.8%, which were graded as effective, marked effective, effective, effective, and effective, respectively. Conclusion:The age of 14-16 weeks(six to eight weeks after CUMS) is considered the stable and reliable time window for depression due to liver depression and spleen deficiency.

ABSTRACT

Intrahepatic cholangiocarcinoma (ICC) is an important liver malignancy next only to hepatocellular carcinoma, accounting for 15%-20% of primary liver cancer. In recent years, the incidence rate of ICC tends to increase globally; however, due to its insidious onset, high degree of malignancy, and strong invasive ability, most patients are in the advanced stage when attending the hospital and thus miss the most appropriate timing for surgery. With the continuous development of next-generation sequencing, the treatment of ICC gradually develops towards the direction of individualization and precision. This article introduces the basic research advances in the pathogenesis, molecular typing, and early diagnosis of ICC and reviews the clinical translational research of ICC in recent years, so as to provide new ideas for the treatment and clinical research of ICC.

Bolbostemma paniculatum is a commonly used Chinese medicinal material effective in clearing heat, removing toxin, eliminating phlegm, and alleviating swelling. The anti-tumor activity it possesses makes it a research hotspot. At present, 76 compounds have been isolated from B. paniculatum, including triterpenoids, sterols, alkaloids, anthraquinones, organic acids, etc., with anti-tumor, antiviral, and immunosuppressive pharmacological activities. This study reviewed the research on the chemical constituents and pharmacological effects of B. paniculatum over the past 20 years, aiming to provide a scientific basis for the research on the pharmacodynamic material basis and promote the development and utilization of B. paniculatum.

ABSTRACT

Objective To compare the performance of contrast-enhanced ultrasound(CEUS)and ultrasound(US)in the differential diagnosis between cholesterol polyps and gallbladder adenomas. Methods A total of 136 patients with gallbladder polyp lesions(GPLs)and undergoing cholecystectomy in the First Medical Center of Chinese PLA General Hospital from January 2019 to October 2020 were retrospectively analyzed.All the patients underwent US and CEUS examinations before cholecystectomy.US and CEUS images of cholesterol polyps and gallbladder adenomas were compared for the evaluation of the performance of CEUS in the diagnosis of gallbladder adenomas. Results The 136 cases of GPLs included 95 cases of cholesterol polyps and 41 cases of gallbladder adenomas.Cholesterol polyps and gallbladder adenomas showed significant differences in the maximum size of GPLs(

ABSTRACT

OBJECTIVE@#To explore the operative method and clinical effect of lateral mini plate and Kirschner wire in the treatment of distal humeral metaphysis junction fracture in children.@*METHODS@#From January 2015 to December 2018, 21 cases of distal humeral diaphyseal metaphyseal junction fracture were analyzed retrospectively, including 12 males and 9 females, aged 2 to 10 years with an average age of 4.5 years, and the time from injury to operation was 6 hours to 7 days. The imaging data showed that the fracture line was located at the junction of the distal humerus and metaphysis. There were 10 oblique fractures, 8 transverse fractures and 3 comminuted fractures. The operation methods were open reduction, lateral mini plate and Kirschner wire assisted internal fixation, and the improved Flynn elbow joint scoring standard was used to evaluate the clinical effect.@*RESULTS@#All the 21 children were followed up for 8 to 24 months, with an average of 13 months. The healing time was 6 to 8 weeks, with an average of 7.2 weeks. There were no complications such as fracture displacement, cubitus varus and ulnar nerve injury. According to the improved Flynn elbow joint scoring standard, 19 cases were excellent and 2 cases were good.@*CONCLUSION@#The treatment of distal humeral metaphyseal junction fracture in children is different from that of supracondylar fracture of humerus.

ABSTRACT

Purpose@#The purpose of this study was to assess the amount of variance in the coping strategies of patients with brain tumors that could be accounted for by resilience. @*Methods@#This cross-sectional survey involved 95 patients who had experienced surgical, chemotherapy, or radiotherapy therapies for their brain tumors at least 1 month before data collection. The investigator collected data using the scales of the Ways of Coping Checklist-Revised and Resilience Scale. Data were analyzed using descriptive statistics, t tests, analysis of variance, Pearson product–moment correlation, and hierarchical multiple regression. @*Results@#The results revealed that resilience was significantly positively associated with patients' problem-focused coping (r = .65, p < .001) and total coping (r = .49, p < .001). In addition, resilience accounted for 27% (R2inc = .27, p < .001) and 16% ((R2inc = .16, p < .001) of the distinct variances in predicting patients’ problem-focused coping and total coping. @*Conclusion@#The current results provide evidence to support the importance of resilience in shaping the coping strategies of relevant patients. As resilience shows a crucial element in patient coping with brain tumors, health team members should develop and employ appropriate strategies to improve the resilience of patients with brain tumors.
