ABSTRACT

ABSTRACT BACKGROUND: Down syndrome (DS) is a non-rare genetic condition that affects approximately 1 in every 800 live births worldwide. Further, it is associated with comorbidities, anatomical alterations of the respiratory tract, and immunological dysfunctions that make individuals more susceptible to respiratory infections. OBJECTIVE: To systematize the current scientific knowledge about the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection among individuals with DS. DESIGN AND SETTING: This integrative review was conducted at the Universidade Federal de São Carlos, São Paulo, Brazil. METHODS: This review was conducted in the following databases: the Virtual Health Library (Biblioteca Virtual em Saúde, BVS), PubMed, and Web of Science, using MeSH descriptors. The search included English or Portuguese studies published between January 1, 2020, and October 14, 2022. RESULTS: A total of 55 articles from 24 countries were selected, comprising 21 case-control or cohort studies, 23 case reports or series, and 11 narrative reviews or opinion studies. The articles were grouped into five categories: previous comorbidities, coronavirus disease 2019 (COVID-19) clinical features and evolution, cytokine storm and interleukins, living in institutions as a risk factor, and behavioral actions as a protective factor against SARS-CoV-2 infection. CONCLUSION: Individuals with DS are more susceptible to COVID-19 infection due to variables such as previous comorbidities, immunological factors, and their habitable environments. These aspects confer a higher risk of infection and an unfavorable clinical course. The precise pathways involved in the pathophysiology of COVID-19 in individuals with DS are not clear, thus requiring further studies. Introdução: Diversos estudos têm analisado a possível relação entre a prótese mamária de silicone e sintomas sistêmicos. A remoção das próteses de mama com capsulectomia tem sido indicada na tentativa de melhorar esses sintomas. É necessário que o cirurgião tenha dados embasados na literatura para informar ao paciente se há relação entre retirada de prótese de mama com capsulectomia e melhora dos sintomas, qual a taxa de melhora e por quanto tempo se mantém. Método: Foi realizada pesquisa nos bancos de dados virtuais Cochrane Library e PubMed de janeiro de 1990 até abril de 2023. A busca foi realizada pela combinação de termos livres ("breast implant illness", "breast capsulectomy" e "breast implant explantation") e pelo uso de operadores booleanos para descritores Mesh como [autoimmune diseases (MeSH Terms)] e [breast implant (MeSH Terms)]. Resultados: Foram obtidos 1.203 artigos, sendo 14 selecionados para o estudo, consistindo em 7 artigos de coorte retrospectivo, 3 de coorte prospectivo e 4 caso-controle. A taxa de melhora variou entre 50 e 100% dos casos e o tempo de acompanhamento variou entre 2 meses e 2,7 anos. Diversos tipos de capsulectomia foram realizados nos estudos, com taxas semelhantes de melhora. Conclusão: Há evidências de melhora dos sintomas sistêmicos em pacientes com prótese mamária de silicone submetidas a retirada de prótese de mama com capsulectomia. A melhora dos sintomas persistiu durante o período em que as pacientes foram acompanhadas nos estudos. Estudos mais recentes demonstraram que o tipo de capsulectomia não tem influência na melhora dos sintomas sistêmicos.


Introduction: Several studies have analyzed the possible relationship between silicone breast implants and systemic symptoms. Removal of breast implants with capsulectomy has been indicated in an attempt to improve these symptoms. The surgeon must have data based on the literature to inform the patient whether there is a relationship between the removal of a breast prosthesis with capsulectomy and improvement in symptoms, what is the rate of improvement, and how long it lasts. Method: A search was carried out in the Cochrane Library and PubMed virtual databases from January 1990 to April 2023. The search was carried out using a combination of free terms ("breast implant illness", "breast capsulectomy," and "breast implant explantation") and by using Boolean operators for Mesh descriptors such as [autoimmune diseases (MeSH Terms)] and [breast implant (MeSH Terms)]. Results: 1,203 articles were obtained, 14 of which were selected for the study, consisting of 7 retrospective cohort articles, 3 prospective cohort articles, and 4 case-control articles. The improvement rate varied between 50 and 100% of cases, and the follow-up time varied between 2 months and 2.7 years. Several types of capsulectomies were performed in the studies, with similar rates of improvement. Conclusion: There is evidence of improvement in systemic symptoms in patients with silicone breast implants who underwent breast implant removal with capsulectomy. The improvement in symptoms persisted during the period in which the patients were followed in the studies. More recent studies have demonstrated that the type of capsulectomy does not influence the improvement of systemic symptoms.

ABSTRACT

RESUMEN INTRODUCCIÓN: El espectro de trastornos de neuromielitis óptica (NMOSD) es un grupo de enfermedades desmielinizantes, inflamatorias y autoinmunes, caracterizadas por episodios recurrentes de neuritis óptica y mielitis transversa longitudinal extensa, entre otras manifestaciones clínicas. Su tratamiento crónico se basa en el uso de terapias inmunosupresoras como azatioprina (AZA), micofenolato mofetilo (MFM) o rituximab (RTX). El objetivo del presente estudio es realizar un análisis comparativo de la respuesta al tratamiento con AZA o RTX. MATERIALES Y MÉTODOS: Se realizó un estudio observacional, analítico, retrospectivo, en el cual se incluyeron inicialmente 69 pacientes con diagnóstico confirmado de NMOSD. Tras aplicar los criterios de inclusión y exclusión 59 pacientes fueron incluidos en el análisis final. RESULTADOS: En el grupo de RTX se evidenció una mejoría importante en el estado funcional en comparación con el grupo de AZA, en el que se vio un empeoramiento de este al año de seguimiento. El perfil de seguridad fue similar entre ambos grupos, con una adherencia significativamente superior en el grupo de RTX. DISCUSIÓN: Los hallazgos del presente estudio respecto a las ventajas del uso de RTX sobre AZA se encuentran en concordancia con resultados de estudios previos reportados en la literatura. CONCLUSIONES: Los resultados respaldan el uso de RTX sobre AZA como terapia de mantenimiento para pacientes con NMOSD, al estar asociado principalmente con una mejoría notable en la funcionalidad de los pacientes, al igual que una mayor adherencia al tratamiento.


ABSTRACT INTRODUCTION: Neuromyelitis Optica Spectrum Disorders (NMOSD) is a group of inflammatory, autoimmune, and demyelinating disorders. Its hallmark behavior is characterized by recurrent episodes of optic neuritis and longitudinally extensive transverse myelitis, among other clinical manifestations. Chronic therapy is based primarily in immunosuppressive therapies such as azathioprine (AZA), mycophenolate mofetil (MMF), or rituximab (RTX). The goal of this study is to perform a comparative analysis of response rates to chronic treatment with either AZA or RTX. MATERIALS AND METHODS: A retrospective observational analytic study was designed with an initial cohort of 69 patients with a diagnosis of NMOSD. After application of the inclusion and exclusion criteria a total of 59 patients were finally included in the analysis. RESULTS: The RTX group had an improved functional status when compared to the AZA group; in the latter this feature worsened after a one-year follow-up. There was also a comparable safety profile between the two groups with a significantly greater adherence to RTX regimes. DISCUSSION: The findings of the current study as to the benefits of RTX in comparison to AZA are similar to the results of previous studies. CONCLUSION: These results favor the use of RTX as maintenance treatment of NMOSD, because of its greater benefit mainly in the improvement in functional status of patients, as well as a greater adherence to treatment.

Almost half of infertility cases involve male infertility. Understanding the consequence of a diagnosis of male infertility, as a sole or partial contributor to the couples' infertility, to the mental health of men is required to ensure clinical care meets their psychological needs. The aim of this systematic analysis was to synthesize the evidence regarding whether men diagnosed with male factor infertility experience greater psychological distress than (1) men described as fertile and (2) men in couples with other infertility diagnoses. Online databases were searched using a combination of Medical Subject Headings (MeSH) headings and keywords relating to male infertility and psychological distress. The search yielded 1016 unique publications, of which 23 were included: 8 case-control, 14 prospective cohort, and 1 data linkage studies. Seven aspects of psychological distress were identified depression, anxiety, self-esteem, quality of life, fertility-related stress, general psychological stress or well-being, and psychiatric conditions. Case-control studies reported that men with male factor infertility have more symptoms of depression, anxiety and general psychological distress, worse quality of some aspects of life, and lower self-esteem than controls. When men with male factor infertility were compared to men in couples with other causes of infertility, there were few differences in the assessed aspects of psychological distress. Despite methodological limitations within the studies, this systematic analysis suggests that the experience of infertility, irrespective of its cause, negatively affects men's mental health and demonstrates the need for assisted reproduction technology (ART) providers to consider men undergoing assisted reproduction as individuals with their own unique support needs.

ABSTRACT

Objective:To evaluate the efficacy of endoscopic ultrasound-guided drainage (EUS-GD) and percutaneous drainage (PD) for the treatment of pancreatic pseudocyst (PPC) by meta-analysis.Methods:The key words were pancreatic pseudocyst, endoscopic ultrasonography, percutaneous drainage, endoscopic ultrasound-guided, EUS and EUS-guided, and the databases of CNKI, Wanfang, PubMed and Web of science were searched from the establishment to October 1, 2021 for the retrospective cohort studies on PD, endoscopic ultrasonography and PPC. The article were screened and the quality was evaluated according to the pre-set inclusion and exclusion criteria; and important data were extracted. The software REVMAN5.3 was used for meta-analysis.Results:14 articles were finally included, and a total of 843 patients with PPC were enrolled (485 in the EUS-GD group and 358 in the PD group). Mesh meta-analysis showed that in the incidence of technical success rate, clinical success rate and complication rate, there were no significant differences between PD group and EUS-GD group ( OR=0.69, 95% CI 0.32-1.46; OR=1.31, 95% CI 0.88-1.95; OR=1.01, 95% CI 0.70-1.47, all P value >0.05); but the incidence of the recurrence rate and reintervention rate in EUS-GD group were significantly lower than those in the PD group and the differences were statistically significant ( OR=0.45, 95% CI 0.23-0.85; OR=0.51, 95% CI 0.28-0.93, all P value <0.05). Conclusions:Compared with PD, EUS-GD can significantly reduce recurrence rate and complication rate, and it has more advantages than PD for the treatment of PPC.

Introducción: La telemedicina es una herramienta para los servicios de atención en pediatría, ya que permite el seguimiento de los pacientes pediátricos en el manejo de enfermedades crónicas y de grupos vulnerables; disminuye los costos y mejora el acceso a los servicios de salud. Objetivo: Explorar, por medio de una revisión sistemática de la literatura, la efectividad de la teleconsulta en el manejo de enfermedades crónicas y grupos vulnerables en la población pediátrica. Métodos: Se realizó una revisión sistemática de la literatura, utilizando los términos MESH pediatrics AND telehealth AND primary care AND control care en las bases Medline PubMed, EMBASE, Scopus, Web of Science, Cochrane, Science Direct. Se incluyeron metaanálisis, revisiones sistemáticas, experimentos clínicos controlados, estudios de cohortes, casos y controles, realizados desde 1998 hasta julio de 2021. Se obtuvieron 54 estudios para la revisión final. Resultados: Los principales resultados de la búsqueda arrojaron beneficios a nivel del control de enfermedades crónicas, control posterior al egreso de la unidad de cuidado intensivo, poscirugía pediátrica, cuidado paliativo, enfermedades mentales, acceso a los servicios de salud de población proveniente de regiones apartadas. Se encontró un beneficio para prevenir las visitas al servicio de urgencias. Conclusiones: La telemedicina tiene una diversidad de utilidades e intervenciones como herramienta de la salud. Se ha encontrado evidencia científica robusta para la atención de enfermedades crónicas y/o seguimiento en pediatría. Asimismo, la prestación de servicios en la modalidad de telemedicina en pediatría debe incluirse en los procesos de atención y guías de práctica clínica de la especialidad(AU)


Introduction: Telemedicine is a tool for pediatric care services, since it allows monitoring of pediatric patients in the management of chronic diseases and vulnerable groups; lowers costs and improves access to health services. Objective: To explore, through a systematic review of the literature, the effectiveness of teleconsultation in the management of chronic diseases and vulnerable groups in the pediatric population. Methods: A systematic review of the literature was carried out, using the MESH terms pediatrics AND telehealth AND primary care AND control care in Medline PubMed, EMBASE, Scopus, Web of Science, Cochrane, Science Direct databases. Meta-analyses, systematic reviews, controlled clinical trials, cohort, case-control studies, conducted from 1998 to July 2021, were included. Fifty-four studies were obtained for the final review. Results: The main results of the search showed benefits at the level of chronic disease control, control after discharge from the intensive care unit, pediatric post-surgery, palliative care, mental illness, access to health services for populations from remote regions. A benefit was found in preventing visits to the Emergency Department. Conclusions: Telemedicine has a diversity of utilities and interventions as a health tool. Robust scientific evidence has been found for the care of chronic diseases and/or follow-up in pediatrics. Likewise, the provision of services in the telemedicine modality in pediatrics should be included in the care processes and clinical practice guidelines of the specialty(AU)

Abstract Oral health personnel must acknowledge the health needs of sexual and gender minorities. They should consult scientific literature to deepen their knowledge about sexuality, gender identity, general and oral health status, and treatment disparities among LGBTQIA+ people. The aim of this scoping review was to portray the development and current stage of internationally indexed literature approaching the oral health of this population. In this study, the search strategy used consisted of combinations of subject descriptors (MeSH terms), in two concept blocks: LGBTQIAP+ people and oral health. Global literature was searched through Medline (PUBMED), Web of Science, Embase, Lilacs, and Scopus electronic databases, with no language or date restrictions. Records were selected and evaluated by two independent reviewers, under the supervision of three senior reviewers and the inclusion criteria resulted in 189 eligible papers. Since the first study was published in 1974, numbers increased over the decades, reaching 67 (35.4%) in 2010-2019. The most frequently studied populations were North American (42.9%) and European (19.0%) and the most frequent language of publication was English (99.0%). There were 38 open access papers (20.1%). Medical (57.7%) and dental journals (20.1%) predominated. Cross-sectional studies were found more frequently (65.1%), followed by the cohort type (11.1%). Oral manifestation of STI (58.7%) was the topic most frequently addressed. The search for literature approaching the oral health of LGBTQIAP+ people showed evidence of the need to encourage research reported in papers made easily available, with more robust scientific evidence, and on a broader scope of topics, including oral health needs and treatment, and planning of oral health services.

Abstract Objectives: to evaluate the repercussions of motherhood on patients deprived of their liberty. Methods: integrative review based on research in electronic databases: Medline (PubMed), LILACS, SciELO and Nursing Database (BDENF), using the following search strategy: ("Prisons" [Majr] AND "Pregnancy" [ Mesh]), as of 2016. Results: the research ended with 33 bibliographic references, being the highest level of evidence from cohort studies, which demonstrate the precariousness of health care offered to these pregnant women. Three main groups of results were identified: low quality prenatal care, negative maternal and neonatal health indicators in relation to the general population and the emotional issues involved in prison reality during the period of pregnancy and puerperium. Conclusions: prenatal care has flaws that influence, in some way, the maternal and neonatal health indicators. In addition, the precarious structure of prison system directly interferes with the emotional health of these women.


Resumo Objetivos: avaliar as repercussões da maternidade em pacientes privadas da liberdade. Métodos: revisão integrativa a partir de pesquisa nas bases de dados eletrônicas: Medline (PubMed), LILACS, SciELO e Base de Dados de Enfermagem (BDENF), sendo usada como estratégia de busca: ("Prisons"[Majr] AND "Pregnancy"[Mesh]), a partir de 2016. Resultados: a pesquisa finalizou com 33 referências bibliográficas, sendo o maior nível de evidência de estudos de coorte, que demonstram a precarização na assistência de saúde ofertada a essas gestantes. Foram identificados três grupos principais de resultados: pré-natal de baixa qualidade, indicadores de saúde maternos e neonatais negativos e as questões emocionais envolvidas na realidade carcerária durante o período de gestação e puerpério. Conclusões: a assistência pré-natal apresenta falhas que influenciam nos indicadores de saúde maternos e neonatais. Além disso, a estrutura precária do sistema prisional interfere diretamente na saúde emocional dessas mulheres

ABSTRACT

The COVID-19 pandemic has had a strong impact on education, forcing the implementation of distance learning methodologies using information and communication technologies (ICT). The digital textbook is a didactic resource used to guide the training process of students; besides, its interactivity allows systematic work, favoring metacognition and the construction of self-learning. The aim of this paper is to describe, through a descriptive cross-sectional design, the design, review, digitization and implementation phases of a digital textbook about physical agents as a learning resource to support distance education in the training process of physical therapy students enrolled in the physical agents course offered by Universidad Andres Bello in Santiago, Chile, in the context of the human mobility restriction measures implemented due to the COVID-19 pandemic. The textbook consists of thirteen thematic sections and once it was reviewed and validated by peers and by the editorial committee of the university it was digitalized and indexed into the digital resource database of the university, where it was made available to the entire cohort of students enrolled in the physical agents course during the first semester of2020 (n=115).The digital textbook was successfully implemented as a didactic resource to guide the autonomous and distance learning of students during the time human mobility restriction measures were established in Chile due to the pandemic. Future studies should focus on reporting the students' satisfaction and perception of this tool and its effect on learning outcomes in order to obtain effective feedback.


La pandemia por COVID-19 ha tenido un fuerte impacto en la educación, por lo que ha sido necesario implementar metodologías de aprendizaje a distancia basadas en tecnologías de la información y las comunicaciones (TIC). El libro de texto digital es un recurso didáctico utilizado para guiar el proceso de formación de los estudiantes; además, su interactividad permite el trabajo sistemático, lo que favorece la metacognición y la construcción del aprendizaje autónomo. El objetivo de este estudio fue describir, mediante un diseño transversal descriptivo, las fases de diseño, revisión, digitalización e implementación de un libro de texto digital sobre agentes físicos como recurso de aprendizaje para apoyar la educación a distancia en el proceso de formación de estudiantes de fisioterapia inscritos en el curso de Agentes Físicos ofrecido por la Universidad Andrés Bello en Santiago, Chile, en el contexto de las medidas de restricción de movilidad humana implementadas por la pandemia por COVID-19. El libro de texto consiste de 13 secciones temáticas y, una vez revisado y validado por pares y por el comité editorial de la universidad, fue digitalizado e indexado en la base de datos de recursos digitales de la universidad, donde se puso a disposición de toda la cohorte de estudiantes matriculados en el curso durante el primer semestre de 2020 (n=115). El libro de texto digital se implementó satisfactoriamente como recurso didáctico para guiar el aprendizaje autónomo y no presencial de los estudiantes en el periodo en que se instauraron las medidas de restricción de la movilidad humana en Chile debido a la pandemia. Estudios futuros deben centrarse en reportar la satisfacción y percepción de los estudiantes con esta herramienta y su efecto en los resultados del aprendizaje para obtener una retroalimentación efectiva.

Resumen La periodontitis es una enfermedad multifactorial caracterizada por una respuesta inflamatoria desarrollada por el huésped frente a los microorganismos de la biopelícula bacteriana. El proceso localizado en la cavidad oral puede influir en el huésped a nivel sistémico provocando resultados adversos en el embarazo como parto prematuro, hipertensión materna y bajo peso del niño al nacer. Objetivo: Analizar el estado actual del conocimiento sobre los mecanismos probables que vinculan la periodontitis con resultados adversos en el embarazo. Material y métodos: Se efectuó una revisión de la literatura y una búsqueda de artículos publicados durante el periodo comprendido entre los años 2016 y 2021 en las bases de datos MEDLINE, PUBMED, SciELO, LILACS, idioma inglés y/o español. Las búsquedas se realizaron utilizando los siguientes términos: Periodontitis, mediadores de inflamación, complicaciones del embarazo, parto prematuro. Resultados: Se analizaron un total de 24 publicaciones, incluidas revisiones de la literatura, revisiones sistemáticas, estudios de cohorte, estudios de casos y controles y estudio observacionales, que cumplieron con los criterios de inclusión. Conclusión: La periodontitis es un factor de riesgo evitable por lo que se recomienda intensificar las medidas de prevención y tratamiento, en embarazadas y en mujeres en edad fértil.


Abstract Periodontitis is a multifactorial disease characterized by an inflammatory response developed by the host against the microorganisms of the bacterial biofilm. The localized process in the oral cavity can influence the host systemically causing adverse pregnancy outcomes such as premature delivery, maternal hypertension, and low birth weight. Objective: To analyze the current state of knowledge about the probable mechanisms that link periodontitis with adverse pregnancy outcomes. Material and methods: A literature review and a search for articles published during the period between 2016 and 2021 was carried out in the MEDLINE, PUBMED, SciELO, LILACS, English and / or Spanish databases. Searches were conducted using the following terms: Periodontitis, inflammatory mediators, pregnancy complications, preterm delivery. Results: 24 publications were analyzed, including literature reviews, systematic reviews, cohort studies, case-control studies, and observational studies, which met the inclusion criteria. Conclusion: Periodontitis is an avoidable risk factor, therefore it is recommended to intensify prevention and treatment measures in pregnant women and women of childbearing age.

RESUMEN INTRODUCCIÓN: El estado migrañoso es una complicación de la migraña, los pacientes a menudo son hospitalizados para estudios diagnósticos o tratamiento. Una alternativa para el manejo son las salas de infusión, lo que permite minimizar los ingresos hospitalarios y la realización de exámenes innecesarios. OBJETIVO: Determinar la respuesta terapéutica y los factores asociados con esta en pacientes con estado migrañoso tratados en una sala de infusión. METODOLOGÍA: estudio de cohorte retrospectiva cuya población fueron pacientes con estado migrañoso atendidos en sala de infusión entre febrero y agosto del 2019. La información sobre variables sociodemográficas, variable clínicas y el manejo terapéutico se obtuvo a partir de la revisión de historias clínicas. Se evaluó la respuesta con escala análoga del dolor al inicio y al final del tratamiento; los pacientes con mejoría mayor al 50 % se consideraron respondedores. Se evaluaron los factores predictores de la respuesta terapéutica mediante regresión logística. RESULTADOS: Se incluyeron 124 pacientes, con promedio de edad de 36 (DE = 12) años, de los cuales el 82,3 % fueron mujeres. La duración promedio del estado migrañoso fue 8 (DE = 4) días, el 73,4 % de los casos se presentó sin aura y 97 (78,2 %) fueron respondedores. Los factores predictores de buena respuesta fueron el uso de dexametasona (OR 4,3) y no usar lidocaína (OR 6,0). CONCLUSIONES: La mayoría de los pacientes tuvo buena respuesta, lo que sugiere que las salas de infusión constituyen una buena alternativa al manejo intrahospitalario de la migraña. En nuestra experiencia, el esteroide constituye parte fundamental del manejo del estado migrañoso una vez se ha instaurado.


ABSTRACT INTRODUCTION: Status migrainosus is a complication of migraine, patients are often hospitalized for diagnostic studies and/or treatment. An alternative for the management, are the outpatient infusion center, minimizing hospital admissions and unnecessary examinations. OBJECTIVE: To determine the therapeutic response and associated factors in patients with migraine status treated in an infusion room. METHODOLOGY: Retrospective cohort study whose population was migraine patients attended in an infusion room between February and August 2019. Information on sociodemographic, clinical, and therapeutic management variables was obtained from the review of medical records. The response was evaluated with an analogous pain scale at the beginning and at the end of the treatment; Those patients with improvement greater than 50 % were considered responders. Predictive factors of therapeutic response were evaluated using logistic regression. RESULTS: 124 patients were included, with an average age of 36 (SD = 12 years), 82.3 % were women. The average duration of the migraine state was 8 (SD = 4 days), 73.4 % of the cases were without aura and 97 (78.2 %) were responders. The predictors of good response were the use of dexamethasone (OR: 4.3) and not using lidocaine (OR: 6.0). CONCLUSIONS: Most of the patients had a good response, which identified the outpatient infusion center as a good alternative to inpatient treatment of headache. In our experience, the steroid constitutes a fundamental part of the management of the status migrainosus once it has been established.

Abstract Introduction Platinum-based chemotherapeutics play an important role in the treatment of cancer at different levels and are the most cited ototoxic agents when scientific evidence is analyzed. Objective To present scientific evidence based on a systematic literature review, PRISMA, in order to systematize information on the ototoxic effects of using antineoplastic drugs. Methods For the selection of studies, the combination based on the Medical Subject Heading Terms (MeSH) was used. The Medline (Pubmed), LILACS, SciELO, SCOPUS, WEB OF SCIENCE and BIREME databases were used, without restriction of language, period, and location. Evaluation of the quality of the articles was carried out, which included articles with a minimum score of 6 in the modified scale of the literature. The designs of the selected studies were descriptive, cohort, and cross-sectional, which were related to the research objective. Results Three articles were included in this systematic review. The ototoxicity caused by cisplatin alone varied from 45% to 83.3%, while that caused by the use associated with carboplatin varied from 16.6% to 75%. There was a significant variation in the cumulative doses of these antineoplastic agents, both in isolated and in combination. Auditory changes, especially at high frequencies, were evident after completion of treatment. Conclusion Auditory changes after the use of platinum-based antineoplastic drugs were found, however, there was an important heterogeneity regarding the frequency of ototoxicity and the cumulative dose of the drugs used.


Resumo Introdução Os quimioterápicos à base de cisplatina desempenham papel importante no tratamento do câncer em diversos níveis e são os agentes ototóxicos mais citados quando analisadas evidências científicas. Objetivo Apresentar evidências científicas com base em revisão sistemática da literatura (PRISMA), com o intuito de sistematizar informações sobre os efeitos ototóxicos do uso de medicamentos antineoplásicos. Método Para a seleção dos estudos foi usada a combinação baseada no Medical Subject Heading Terms (MeSH). Foram usadas as bases de dados Medline (Pubmed), Lilacs, SciELO, Scopus, Web of Science e Bireme, sem restrição de idioma, período e localização. Foi feita avaliação da qualidade dos artigos, na qual se incluíam artigos com nota mínima 6 na escala modificada da literatura. Os desenhos dos estudos selecionados foram do tipo descritivo, coorte e transversal, os quais estavam relacionados com o objetivo da pesquisa. Resultados Três artigos foram admitidos para esta revisão sistemática. A ototoxicidade causada pela cisplatina isoladamente variou de 45% a 83,3%, enquanto a causada pelo uso associado com a carboplatina variou de 16,6% a 75%. Verificou-se uma variação significativa nas doses cumulativas desses antineoplásicos, tanto no uso isolado quanto em combinação. Alterações auditivas, principalmente em altas frequências, foram evidenciadas após finalização do tratamento. Conclusão Alterações auditivas após o uso de medicamentos antineoplásicos à base de platina foram constatadas, porém verificou-se uma importante heterogeneidade quanto à frequência da ototoxicidade e a dose cumulativa das drogas usadas.

INTRODUCTION@#Studies showed that those who tested positive for COVID-19 have a 65% risk for a psychiatric disorder, while those undergoing isolation or quarantine are put at risk for anxiety and depression. The objective of this study was to appraise studies that determine the risk factors for psychiatric disorder post-COVID-19 infection.@*METHODS@#All cross-sectional and cohort studies from 2019 onwards that had COVID-19 survivors that developed anxiety, depression and/or post-traumatic stress disorder (PTSD), were included. Medline, Cochrane Library and ClinicalKey were searched using MeSH terms including “COVID-19”, “depression”, “anxiety”, “post-traumatic stress disorder”, and “risk factor”. The risk of bias was assessed using the Newcastle-Ottawa scale. The data extracted from the studies were characteristics of the participants, risk factors, outcome measures and outcomes.@*RESULTS@#Four cohort and four cross-sectional studies involving 1438 COVID-19 survivors who developed depression, anxiety and/or depression were included. The risk factors that were statistically significant were 1) female sex (RR = 1.86; 95% CI 1.06, 2.04; Z = 2.32; p = 0.02) for depression, 2) having family members infected with COVID-19 (RR = 1.56; 95% CI 1.32, 1.85; Z= 5.17; p = <0.01) for depression, 3) steroid administration during hospital admission (RR = 1.62; 95% CI 1.07, 2.47; Z = 2.26; p = 0.02) for anxiety and 4) female sex (RR = 2.13; 95% CI 1.16, 3.91; Z = 2.45, p = 0.01) for PTSD.@*CONCLUSION@#Female sex increases the risk of depression and PTSD. A family history of COVID-19 increases the risk of depression. Steroid administration is a risk factor for anxiety.

Objective:To evaluate the effect of maternal depressed mood at pregnancy and postpartum on the risk of emotional or behavioral disorders of offspring by meta-analysis. Methods:The following Mesh words and free words were searched in 7 online databases, including the PubMed, Embase, Web of Knowledge, PsycINFO, Cochrane, WanFang databases and China National Knowledge Infrastructure from January 1, 2000 to October 31, 2020: " maternal" AND " depression" AND " child OR offspring" AND " neuropsychology" . According to the inclusion and exclusion criteria, case-control and cohort studies reporting the effect of maternal depressed mood during pregnancy or postpartum on the risk of emotional or behavioral disorders of offspring were reviewed. Meta-analysis was performed by RevMan 5.3. Results:Fourteen studies involving 3 914 in the case group and 17 016 in the control group were included.Children whose mother with depressed mood during pregnancy or postpartum had 2.03 times risk of emotional or behavioral disorders than those whose mothers without depressed mood ( OR=2.03, 95% CI: 1.55-2.65). Both depressed mood at pregnancy and postpartum could increase the incidence of emotional or behavioral disorders in children, but there was no significant difference between these two periods ( Z=-0.371, 95% CI: 0.796-1.168). Moreover, the effect of maternal depressed mood on emotional or behavioral disorders in offspring could last to the preschool and school period, and the children in the school period may have higher incidence of emotional or behavioral disorders than those during the preschool period ( Z=-2.340, 95% CI: 0.643-0.962). Conclusions:Maternal depressed mood can increase the incidence of emotional or behavioral disorders in offspring, which are long-lasting and do not decrease with age.

Resumen ANTECEDENTES: Las hemorragias durante el primer trimestre del embarazo son un problema que se atiende en la práctica clínica obstétrica diaria. Las conductas de atención suelen estar protocolizadas en casi todas las instituciones; sin embargo, la evidencia científica actual sugiere que algunas de esas conductas no tienen una evidencia sólida que las apoye. OBJETIVO: Revisar la bibliografía reciente y resumir las principales recomendaciones para la atención de casos de hemorragia en el primer trimestre del embarazo. METODOLOGÍA: Estudio retrospectivo basado en la búsqueda bibliográfica de artículos en inglés que contuvieran los términos MeSH: "First Trimester", "Hemorrhage", "Bleeding", "Ectopic Pregnancy" "Abortion", "Incomplete Abortion", "Miscarriage", "Early Pregnancy Loss", "Threatened Abortion" y "Gestational Trophoblastic Disease". Se incluyeron ensayos clínicos controlados, estudios de casos y controles, estudios de cohorte prospectivos y retrospectivos, guías de práctica clínica, protocolos, revisiones sistemáticas y metanálisis incluidos en la base de datos PubMed de 2014 a 2021. RESULTADOS: Se encontraron 54 artículos completos, de los que se descartaron 38 por duplicidad en la información, falta de pertinencia o no actualizados. Al final, solo se incluyeron 16 artículos para la revisión narrativa. CONCLUSIONES: Las hemorragias durante el primer trimestre, si bien son frecuentes no dejan de ser un reto diagnóstico por su amplio espectro de manifestaciones clínicas y causas. Siempre es necesario correlacionar la evaluación clínica completa con los hallazgos ecográficos y las concentraciones de β-hCG y descartar las causas no obstétricas del sangrado, independientemente de la sospecha diagnóstica inicial.


Abstract BACKGROUND: Hemorrhage in the first trimester of pregnancy is a problem seen in daily obstetric clinical practice. Care behaviors are usually protocolized in almost all institutions; however, current scientific evidence suggests that some of these behaviors do not have strong evidence to support them. OBJECTIVE: To review the recent literature and summarize the main recommendations for the care of cases of hemorrhage in the first trimester of pregnancy. METHODOLOGY: Retrospective study based on a literature search of English language articles containing the MeSH terms: "First Trimester", "Hemorrhage", "Bleeding", "Ectopic Pregnancy", "Abortion", "Incomplete Abortion", "Miscarriage", "Early Pregnancy Loss", "Threatened Abortion" and "Gestational Trophoblastic Disease". Controlled clinical trials, case-control studies, prospective and retrospective cohort studies, clinical practice guidelines, protocols, systematic reviews and meta-analyses included in the PubMed database from 2014 to 2021 were included. RESULTS: We found 54 complete articles, of which 38 were discarded due to duplicity in information, lack of relevance or not updated. In the end, only 16 articles were included for narrative review. CONCLUSIONS: First trimester hemorrhage, although frequent, is still a diagnostic challenge due to its wide spectrum of clinical manifestations and causes. It is always necessary to correlate the complete clinical evaluation with ultrasound findings and β-hCG concentrations and to rule out nonobstetric causes of bleeding, regardless of the initial diagnostic suspicion.

Introducción. La diabetes mellitus (DM) es una de las enfermedades crónicas más comunes, siendo una de las causas principales de mortalidad de la población mun-dial. La dermatoglifia es empleada como instrumento para el hallazgo de ciertos aspectos biológicos en diferentes poblaciones. Objetivo. Reconocer, a partir de una revisión sistemática, la relevancia que tiene la dermatoglifia como medio de hallazgo de diabetes mellitus. Metodología. Revisión de literatura científica en bases de datos como Sciencedi-rect, PubMed, Scopus y BVS, en las cuales se extrajo información después de co-rrer la ecuación de búsqueda con términos MESH. Posteriormente, fueron revisados para incluir aquellos artículos relacionados con DM y dermatoglifia. Para todo el proceso se siguió la metodología PRISMA, evaluando los artículos con la escala de sesgo de Cochrane y el nivel de evidencia y recomendación con escala SIGN. Resultados. Después de una revisión de 475 artículos, se incluyeron ocho artículos, y al ser evaluados fueron clasificados cinco artículos 2+ y tres 2-, con nivel de reco-mendación D. Conclusión. La dermatoglifía es una herramienta útil como medio de detección de la DM. Sin embargo, es necesario realizar estudios de cohortes para demostrarlo en di-ferentes poblaciones, como la colombiana. En ese sentido, se encuentra que el 70% de los artículos revisados demuestran que la predicción de hallazgo de DM es efectiva. Sin embargo, el 30% de las investigaciones dentro de esta revisión no consideran que sea una herramienta lo suficientemente óptima para descubrir la patología en la población.


Introduction. Diabetes mellitus (DM) is one of the most common chronic diseases, be-ing one of the main causes of mortality in the world population. Dermatoglyphics is used as an instrument for the discovery of certain biological aspects in different populations.Objective. To recognize, by doing a systematic review, the relevance of dermato-glyphics as a means of finding diabetes mellitus. Methodology. Review of scientific literature in databases such as Sciencedirect, PubMed, Scopus, and VHL, in which information was extracted after running the search equation with MESH terms. After that, they were revised to include those articles related to DM and dermatoglyphics. For the entire process, the PRISMA methodology was followed, evaluating the articles with the Cochrane bias scale and the level of evidence and recommendation with the SIGN scale. Results. After a review of 475 articles, eight articles were included, and when evalu-ated, five articles were classified as 2+ and three as 2-, with a level D recommendation. Conclusion. Dermatoglyphics is a useful tool as a means of detecting DM. However, it is necessary to carry out cohort studies to demonstrate this in different populations, such as the Colombian population. 70% of the reviewed articles show that the prediction of DM finding is effective. Nevertheless, 30% of the investigations within this review do not consider it to be a sufficiently optimal tool to discover the pathology in the population

RESUMEN INTRODUCCIÓN: La prevalencia de deterioro cognitivo leve (DCL) y demencia se ha descrito recientemente como alta en Colombia, debido al aumento de la población adulta mayor y a su baja escolaridad. Aun no es claro el mecanismo por el cual la diabetes mellitus (DM) o la hipertensión arterial (HTA), en ausencia de accidente cerebrovascular (ACV), aumentarían el riesgo de DCL o demencia. OBJETIVO: Revisar en la literatura la posible asociación de HTA y DM con el riesgo de progresión a DCL. MÉTODOS: Se realizó una búsqueda en las bases de datos de las publicaciones entre los años 2007 y 2017, de cohortes y casos y controles de adultos de 65 años o más con HTA o DM y que evaluaran como desenlace DCL o demencia. RESULTADOS: La revisión arrojó doce artículos que cumplieron con los criterios de selección. Seis artículos reportaban relación entre HTA y riesgo de presentar deterioro cognitivo; de estos, cinco artículos presentaban como desenlace demencia vascular (DVa) y el otro tanto demencia tipo Alzheimer (DTA) como DVa. Además, informan sobre el riesgo de presentar demencia según las cifras de presión arterial sistólica y diastólica. De los seis estudios de cohortes incluidos, cinco encontraron una relación significativa entre DM y progresión a DCL o DTA. CONCLUSIÓN: Los pacientes con HTA tienen mayor riesgo de DVa, seguido por demencia mixta. La DM se asocia con una mayor frecuencia de DTA, en especial si la DM aparece en edades tempranas.


SUMMARY INTRODUCTION: A high prevalence of mild cognitive impairment (MCI) and dementia has been described recently in Colombia, due to the increase of the older adult population with low level of schooling. It is still unclear the mechanism by which and high blood pressure (HBP), in the absence of stroke, increase the risk of MCI or dementia. OBJECTIVE: To review the possible association of HBP and diabetes with the risk of progression to cognitive impairment. METHODS: We searched databases from 2007 to 2017, and case-control or cohort studies on adults aged 65 years or older with HBP or diabetes in whom cognitive functions were evaluated over time. RESULTS: The search yielded 12 articles that met the selection criteria. Six articles report the relationship between hypertension and the risk of developing dementia; of these, five for vascular dementia (VDa) and one for both Alzheimer dementia (AD) and VDa as the outcome. They also report on the risk of developing dementia according to systolic and diastolic blood pressure. Of the six cohort studies included, five found a statistically significant relationship of diabetes and progression to MCI or AD. CONCLUSION: Patients with hypertension have an increased risk of VDa, followed by mixed dementia. DM is associated with a higher frequency of AD, especially if diabetes appears at an early age.

ABSTRACT: Allergic reactions are host immune responses to endogenous or exogenous antigens, which can result in local and systemic problems. Among the main allergens are the dental materials used in orthodontics, which faces some challenges with regard to biocompatibility with oral tissues.This study aimed to analyze in the literature which materials used in orthodontics are related to the appearance of oral and perioral allergic reactions, as well as to investigate the most prevalen t manifestations.An integrative review was carried out with articles published between 2010 and 2020 on the PubMed, Embase, and ScienceDirect on-line database. For this, MeSH descriptors and synonyms were used following two semantic fields: "Hypersensitivity" in association with "Orthodontic Appliances", the selection of studies counted with the stages of identifica tion, screening, eligibility and inclusion. Randomized clinical trials, cross-sectional or cohort clinical trials; Patients with alle rgic reactions affecting the oral or perioral region due to contact with orthodontic material, were included in the review.From a to tal of 549 articles only 04 were selected for the study. In accordance with what has been analyzed in the evidence of these studies, periodontal changes such as gingival inflammation as well as erythema, edema, papules and blisters were the main reactions described. Furthermore, in all articles there was an association with nickel and in only one with chromium. It is possible to i nfer that inflammatory lesions are frequent manifestations on the use of nickel in orthodontic patients.


RESUMEN: Las reacciones alérgicas son respuestas inmunes del huésped a antígenos endógenos o exógenos, los cuales pueden provocar problemas locales y sistémicos. Entre los principales alérgenos se encuentran los materiales dentales utilizados en la ortodoncia, la cual se enfrenta a algunos desafíos respecto a la biocompatibilidad de los tejidos orales. Este estudio tuvo como objetivo analizar en la literatura que materiales utilizados en ortodoncia están relacionados con la aparición de reacciones alérgicas orales y periorales, así como investigar las manifestaciones más prevalentes. Se llevó a cabo una revisión con artículos publicados entre 2010 y 2020 en las bases de datos PubMed, Embase y ScienceDirect. Para ello, se utilizaron descriptores y sinónimos de MeSH siguiendo dos campos semánticos: "Hipersensibilidad" en asociación con "Aparatos de Ortodoncia", la selección de estudios contó con las etapas de identificación, cribado, elegibilidad e inclusión. Ensayos clínicos aleatorizados, ensayos clínicos transversales o de cohortes; en la revisión se incluyeron pacientes con reacciones alérgicas que afectaron a la regiones oral o perioral debido al contacto con material de ortodoncia. De un total de 549 artículos sólo cuatro fueron seleccionados para el estudio. De acuerdo con lo que se analizó en la evidencia, las principales reacciones descritas fueron cambios periodontales, como inflamación gingival, eritema, edema, pápulas y ampollas. Además, en todos los artículos hubo una asociación con níquel y solamente en uno con cromo. Es posible inferir que las lesiones inflamatorias son manifestaciones frecuentes sobre el uso de níquel en pacientes de ortodoncia.

SUMMARY OBJECTIVE: To verify the scientific evidence on the association between Autistic Spectrum Disorder and Central Auditory Processing Disorder in children, aiming to answer the following research question: What is the association between Autistic Spectrum and Alteration of Auditory Processing in Children? METHODS: Studies were chosen through the combination based on the Medical Subject Heading Terms (MeSH): [(auditory processing) and (children) and (autism) and (neurological disorders)]. The MEDLINE (PubMed), LILACS, and SciELO databases were used. The analyzed papers covered a ten-year period, from 2010 to 2020. We selected descriptive, cross-sectional, cohort, and case studies. We evaluated the quality of the papers, which had a minimum score of six in the modified scale of the literature. RESULTS: 126 papers were retrieved after the exclusion phase, and 17 of them followed the inclusion criteria. Only two papers answered the guiding question with audiological results. CONCLUSIONS: Patients diagnosed with autistic spectrum disorder may have disturbance central auditory processing, considering that changes were found both in absolute and interpeak latencies in the brainstem evoked response audiometry, as well as in latency and laterality of the N1c wave amplitude. In addition, there were changes in the assessment behavioral auditory processing. Thus, disturbance central auditory processing is common in children with autistic spectrum disorder.

Abstract Either bites or stings of venomous animals comprise relevant public health problems in tropical countries. Acute kidney injury (AKI) induced by animal toxins is related to worse prognostic and outcomes. Being one the most important pathways to induce AKI following envenoming due to animal toxins, inflammation is an essential biological response that eliminates pathogenic bacteria and repairs tissue after injury. However, direct nephrotoxicity (i.e. apoptotic and necrotic mechanisms of toxins), pigmenturia (i.e. rhabdomyolysis and hemolysis), anaphylactic reactions, and coagulopathies could contribute to the renal injury. All these mechanisms are closely integrated, but inflammation is a distinct process. Hence, it is important to improve our understanding on inflammation mechanisms of these syndromes to provide a promising outlook to reduce morbidity and mortality. This literature review highlights the main scientific evidence of acute kidney injury induced by bites or stings from venomous animals and their inflammatory mechanisms. It included observational, cross-sectional, case-control and cohort human studies available up to December 2019. Descriptors were used according to Medical Subject Headings (MeSH), namely: Acute kidney injury or Venom and Inflammation on Medline/Pubmed and Google Scholar; Kidney disease or Acute kidney injury on Lilacs and SciELO. The present review evidenced that, among the described forms of renal inflammation, it can occur either directly or indirectly on renal cells by means of intravascular, systemic and endothelial hemolysis, activation of inflammatory pathway, as well as direct action of venom cytotoxic components on kidney structures.
