ABSTRACT

OBJECTIVE To explore the efficacy and safety of intra-articular injection of ropivacaine combined with alfentanil for postoperative analgesia in patients who underwent knee arthroscopic surgery. METHODS A total of 60 patients who underwent knee arthroscopic surgery were collected from the Central Hospital of Enshi Tujia and Miao Autonomous Prefecture from March to September in 2022, and then divided into trial group and control group with random number table method, with 30 cases in each group. The control group received intra-articular injection of 0.25% ropivacaine 50 mg, and the trial group received intra-articular injection of 0.25% ropivacaine 50 mg+alfentanil 0.15 μg/kg.The first postoperative remedial analgesia time, the total amount of postoperative remedial drugs, numerical rating scale at rest (NRS-R) scores, numerical rating scale at movement (NRS-M) scores, heart rate, mean arterial blood pressure, and pulse oxygen saturation during exercise at different monitoring time points after surgery, the incidence of adverse drug reactions such as hypotension, respiratory depression, nausea, and vomiting after surgery were compared between 2 groups. RESULTS Compared with the control group, the first postoperative remedial analgesia time was significantly longer in the trial group, and the total amount of postoperative remedial drugs was significantly reduced (P<0.001). The trial group had lower NRS-R and NRS-M scores at each monitoring time point, with statistically significant differences (P<0.001), and there was an interactive effect between time and groups (P<0.001). The changes in heart rate, mean arterial blood pressure, and pulse oxygen saturation of patients in the trial group were relatively small, with no statistically significant differences (P>0.05), and there was no interactive effect between time and groups (P>0.05). There was no statistical significance in the incidence of adverse drug reactions between 2 groups, such as postoperative hypotension, respiratory depression, nausea, vomiting (P>0.05). CONCLUSIONS The intra- articular injection of ropivacaine combined with alfentanil shows good efficacy and safety for post-knee arthroscopic analgesia, and significantly prolongs the analgesic duration of ropivacaine.

ABSTRACT

Superior labrum anterior posterior (SLAP) injury is a major challenge for orthopedic surgeons, due to the poor healing ability of the injured labrum. Although arthroscopic surgery is the gold standard for the treatment of SLAP injury, there are still disputes about the adaptation of different surgical techniques, the choice of anchors during operation, knotted or knotless anchors, and fixation methods. The authors believe that arthroscopic repair of SLAP lesions is effective for young patients with intact glenoid labrum(<35 years old) or with extensive activity, where single and knotless anchor is preferred. For the older patients(≥35 years old) with degeneration and wear of glenoid labrum, biceps tenodesis is more preferable, and interference screw fixation technique is recommended. As for patients with failed SLAP repair, biceps tenodesis can achieve a high success rate as a revision surgery. By review of the relevant literature in recent years, this paper summarizes the adaptation of different surgical methods of arthroscopic treatment of SLAP injury, intraoperative anchoring techniques, fixation methods and other improved surgical techniques.

ABSTRACT

OBJECTIVE@#To explore clinical effect of arthroscopy-assisted rotator cuff tendon transfer in treating irreparable rotator cuff tears (IRCT).@*METHODS@#From May 2015 to May 2018, 23 patients with unrepairable rotator cuff tears were treated with arthroscopy-assisted rotator cuff tendon transfer, and 21 patients were followed up finally, including 8 males and 13 females, aged from 48 to 82 years old with an average of(64.3±9.1) years old;the courses of disease ranged from 6 to 36 months with an average of (14.0±6.4) months. American Rotator and Elbow Surgeons Score(ASES) and Constant-Murley score were used to evaluate clinical efficacy before surgery and at the latest follow-up.@*RESULTS@#All 21 patients were followed up for 36 to 54 months with an average of (39.4±4.4) months. Axillary incision of 1 patient was redness, swelling and exudation after surgery, which healed after 3 weeks of dressing change, and exudate culture was negative. At the latest follow-up, MRI showed partial tearing of the metastatic tendon in 2 patients, but pain and movement of the affected shoulder were still better than before surgery. ASES increased from preoperative (41.0±9.6) scores to the latest follow-up (75.6±14.0) scores, and had statistical difference (t=10.50, P<0.01). Constant-Murley score increased from (49.8±7.1) scores before operation to (67.5±11.6) scores at the latest follow-up (t=11.27, P<0.01).@*CONCLUSION@#Arthroscopic assisted latissimus dorsalis tendon transposition restores physiological and anatomical structure of glenohumeral joint by reconstructing balance of horizontal and vertical couples of shoulder joint, thus achieving the stability of the shoulder joint, relieving shoulder pain and improving shoulder joint function.

ABSTRACT

Objective To explore the optimal administration route of tranexamic acid (TXA) in shoulder arthroscopic surgery. Methods Patients undergoing arthroscopic rotator cuff repair were randomly divided into four groups: control group (without TXA treatment), intravenous group (TXA was intravenously administered 10 minutes before surgery), irrigation group (TXA was added to the irrigation fluid during subacromial decompression and acromioplasty), and intravenous plus irrigation group (TXA was applied both intravenously and via intra-articular irrigation). The primary outcome was visual clarity assessed with visual analog scale (VAS) score, and the secondary outcomes included irrigation fluid consumption and time to subacromial decompression and acromioplasty procedure. Results There were 134 patients enrolled in the study, including 33 in the control group, 35 in the intravenous group, 32 in the irrigation group, and 34 in the intravenous plus irrigation group. The median and interquartile range of VAS scores for the intravenous, irrigation, and intravenous plus irrigation groups were 2.70 (2.50, 2.86) (Z = -3.677, P = 0.002), 2.67 (2.50, 2.77) (Z = -3.058, P < 0.001), and 2.91 (2.75, 3.00) (Z = -6.634, P < 0.001), respectively, significantly higher than that of the control group [2.44 (2.37, 2.53)]. Moreover, the control group consumed more irrigation fluid than the intravenous group, irrigation group, and intravenous plus irrigation group (all P < 0.05). The intravenous plus irrigation group consumed less irrigation fluid than either the intravenous group or the irrigation group (both P < 0.001). There was no difference in subacromial decompression and acromioplasty operative time among the four groups. Conclusion TXA applied both topically and systematically can improve intraoperative visual clarity, and the combined application is more effective.

ABSTRACT

Objective: To observe the efficacy of mild moxibustion combined with arthroscopic meniscal repair in the treatment of meniscal injury and to explore its action mechanism. Methods: Ninety-eight patients with meniscal injury were divided into a surgery group and a moxibustion plus surgery group by the random number table method, with 49 cases in each group. Both groups received arthroscopic meniscal repair, and the moxibustion plus surgery group was treated with add-on mild moxibustion. The Lysholm score, visual analog scale (VAS) score, and mobility of the affected knee were measured before and after treatment, and transforming growth factor (TGF)-β1 and platelet-derived growth factor (PDGF) levels were measured in the fluid of the affected knee joint. The healing of the meniscus was recorded at a follow-up visit 3 months after the surgery. Results: After treatment, the Lysholm score increased, the VAS score decreased in both groups, and the intra-group differences in both groups were statistically significant (P<0.05). The Lysholm score was higher in the moxibustion plus surgery group than in the surgery group, and the VAS score was lower in the moxibustion plus surgery group than in the surgery group. The differences between groups in both scores were statistically significant (P<0.05). The mobility of the affected knee joint increased in both groups (P<0.05), and it was greater in the moxibustion plus surgery group than in the surgery group (P<0.05). When compared with pretreatment, the levels of TGF-β1 and PDGF in the fluid of the knee joint increased in both groups (P<0.05), and the levels of TGF-β1 and PDGF in the fluid of the affected knee joint were higher in the moxibustion plus surgery group than in the surgery group (P<0.05). The healing of the meniscus in the moxibustion plus surgery group was significantly better than that in the surgery group at the follow-up visit 3 months after the surgery (P<0.05). Conclusion: The effect of mild moxibustion combined with arthroscopic meniscal repair is better than the surgery alone in improving knee function, relieving pain, increasing mobility of the affected knee, and promoting meniscal healing, which may be related to the up-regulation of TGF-β1 and PDGF levels in the fluid of knee joint.

ABSTRACT

Objective:To explore the effects of risk grading nursing on the prevention of postoperative deep vein thrombosis (DVT) and quality of life in patients undergoing knee arthroscopic surgery.Methods:A total of 1 140 patients undergoing knee arthroscopic surgery in Beijing Jishuitan Hospital were selected. The 529 cases with routine nursing from January to December 2018 were selected as the control group, while another 611 cases with risk grading nursing from January to December 2019 were selected as the observation group. The incidence of DVT, limbs swelling, quality of life and nursing satisfaction were compared between the two groups.Results:The incidence of DVT and the positive rates of Homan sign and Neuhof sign were 2.78% (17/611), 3.93% (24/611), 4.91% (30/611) in the observation group, and 5.10% (27/529), 10.02% (53/529), 11.72% (62/529) in the control group, the differences were statistically significant ( χ2=4.12, 16.70, 17.72, all P<0.05). The peripheral diameter and body surface temperature at 5 cm above the ankle, 10 cm below the patella and 15 cm above the patella were (21.30 ± 1.72) cm, (34.89 ± 2.75) cm, (46.69 ± 4.58) cm, (33.14 ± 1.40) ℃, (34.45 ± 1.52) ℃, (35.22 ± 1.36) ℃ in the observation group, and (22.88 ± 1.43) cm, (35.91 ± 1.81) cm, (49.18 ± 4.23) cm, (34.39 ± 1.22) ℃ (35.88 ± 1.49) ℃, (35.97 ± 1.31) ℃ in the control group, the differences were statistically significant ( t values were -16.74--7.25, all P<0.05). The scores of general health, physiological function, physiological function, mental health, social function, emotional function, body pain and body vitality were (75.84 ± 5.33), (79.78 ± 5.71), (76.71 ± 4.14), (84.91 ± 4.15), (75.53 ± 4.19), (78.80 ± 6.76), (74.85 ± 3.49), (78.61 ± 3.52) points in the observation group, and (71.39 ± 4.28), (75.44 ± 6.26), (73.05 ± 4.09), (80.15 ± 6.87), (71.66 ± 4.18), (74.89 ± 6.23), (71.14 ± 5.20), (74.66 ± 5.14) points in the control group, the differences were statistically significant ( t values were 10.10-15.57, all P<0.05). The satisfaction scores of nursing communication, safety, nursing technology, nursing, guidance and physical environment were (4.54 ± 0.42), (4.75 ± 0.46), (4.54 ± 0.52), (4.60 ± 0.48), (4.60 ± 0.53), (4.30 ± 0.64) points in the observation group, and (3.93 ± 0.34), (3.98 ± 0.37), (3.96 ± 0.41), (3.93 ± 0.38), (3.85 ± 0.36) (3.74 ± 0.38) points in the control group, the differences were statistically significant ( t values were 17.62-30.82, all P<0.05). Conclusions:Risk grading nursing is beneficial to alleviate postoperative limb swelling in patients after knee arthroscopic surgery, reduce incidence of DVT, improve quality of life and nursing satisfaction.

ABSTRACT

PURPOSE: Although the number of hip arthroscopies is rapidly increasing in non-elderly patients, outcomes of this procedure in middle-aged patients are not well documented or clearly understood. The purpose of this study was to evaluate the clinical and radiological outcomes after hip arthroscopy in middle-aged patients with early osteoarthritis.MATERIALS AND METHODS: This retrospective study analyzed 189 patients with early osteoarthritis of various diagnoses aged 40 years or older who underwent hip arthroscopy between January 2010 and December 2015. Clinical (e.g., modified Harris hip score [mHHS], hip outcome score-activities of daily living [HOS-ADL], visual analogue scale [VAS] for pain, range of motion) and radiological (change of Tönnis grade) outcomes were assessed at a minimum of 3-year follow-up.RESULTS: The mean preoperative and final mHHS and HOS-ADL improved from 61.2 and 60.6 to 79.5 and 81.8, respectively, while the VAS pain score decreased from 6.3 to 3.2 (P<0.001). Although the mean range of internal rotation and flexion increased from 14.2 and 100.7° preoperatively to 30.4 and 110.6° at 1-year postoperatively, they decreased slightly to 27.4 and 105.4° at the final follow-up, respectively. Eight cases (4.2%) underwent revision arthroscopic surgery and three cases (1.6%) were converted to total hip arthroplasty.CONCLUSION: Patients with early-stage osteoarthritis of various diagnoses achieved improved clinical outcomes. Therefore, using hip arthroscopy in middle-aged patients with early osteoarthritis, it is possible to achieve good surgical options.

OBJECTIVES@#To explore the safety and effectiveness of arthroscopic reconstruction of coracoclavicular ligament by suspensory fixation to manage the acute acromioclavicular joint dislocation.@*METHODS@#From January 2016 to December 2017, 18 cases of acute acromioclavicular joint dislocation were carried out with arthroscopic reconstruction of coracoclavicular ligament by double Endobutton plate suspensory fixation. Anteroposterior view X-ray plain radiographs were obtained on the second day, 6 months and 12 months after the surgery, MRI was performed in 1 year after operation. Meanwhile, subjective and objective scoring were obtained by Vsual Analogue Scale (VAS), Rating Scale of the American Shoulder and Elbow Surgeons (ASES) and University of California at Los Angeles Shoulder Rating Scale (UCLA).@*RESULTS@#All patients were followed up for 12 to 30 months (an average of 18 months). There was no patient with infection, neurovascular injury, loosening and breakage of internal fixation, re-dislocation of acromioclavicular joint, clavicular fracture, coracoid process fracture, etc. Postoperative X-ray showed that all acromioclavicular joints were completely relocated. The follow-up of MRI after 1 year showed no obvious dislocation of acromioclavicular joint and good recovery of acromioclavicular space. Postoperative shoulder joint function, VAS, ASES, UCLA and acromioclavicular distance were significantly improved compared with those before surgery, with statistically significant differences (all <0.05).@*CONCLUSIONS@#Arthroscopic reconstruction of coracoclavicular ligament by suspensory fixation to manage the acute acromioclavicular joint dislocation has the advantages of minimal invasive, rapid functional recovery and less complications and satisfactory early clinical results.

ABSTRACT

ABSTRACT Objective: To evaluate the outcomes of patients that underwent arthroscopic surgery for lateral epicondylitis (LE), after failed conservative treatment. Methods: One hundred four patients with LE treated with arthroscopic debridement of the extensor carpi radialis brevis (ECRB) tendon were enrolled in this retrospective study. They were evaluated using Disabilities of the Arm, Shoulder and Hand (DASH) score, Visual Analogue Scale (VAS) and Short Form Health Survey (SF-36) scale. Mean age at surgery was 46.9 years. Duration of symptoms was 2.1 years (range: 6 m to 10 yrs.). Mean follow-up was 34.4 months (range: 6 to 68 m). Results: Mean postoperative scores were: 20.67 points on the DASH; 1.8 points on the VAS at rest, with 48 cases (46%) without pain, 40 (38%) with mild pain, 13 (13%) with moderate pain and 4 (4%) with severe pain; 4.7 points on the VAS in activity, with 21 (20%) without pain, 21 (20%) with mild pain, 35 (34%) with moderate pain and 27 (26%) with severe pain; and SF-36 was 66.8 points. Of the 23 patients who practiced sports regularly or with higher physical demand from the upper limbs, 17 (74%) were able to return to the same activity at the same level. No significant complications were observed postoperatively, except for 3 (2.8%) cases of postoperative superficial infection. Conclusion: Surgical treatment with arthroscopy for recalcitrant LE is effective and safe, presenting positive outcomes in the studied patients. Level of evidence IV, Case Series.


RESUMO Objetivo: Avaliar os resultados dos pacientes submetidos a tratamento cirúrgico artroscópico da epicondilite lateral (EL) refratária depois da falha no tratamento conservador. Métodos: Estudo retrospectivo que incluiu 104 pacientes submetidos a desbridamento artroscópico do tendão extensor radial curto do carpo (ERCC) para tratamento de EL. Os pacientes foram avaliados pelo escore de DASH, pela classificação visual analógica de dor (EVA) e pelo Short-Form 36 (SF36). A média da idade foi de 46,9 anos (variação de 30 a 69 anos). O tempo de sintomas foi de 2,1 anos (variação de 6 meses a 10 anos). O seguimento médio foi de 34,4 meses (variação de 6 - 68 meses). Resultados: A média dos escores pós-operatórios foi de: 20,67 pontos no DASH; 1,8 pontos no EVA de repouso, sendo 48 (46%) sem dor, 40 (38%) com dor leve, 13 casos (13%) com dor moderada e 4 (4%) com dor intensa; 4,7 pontos no EVA em atividade, sendo 21 (20%) sem dor, 21 (20%) com dores leves, 35 (34%) com dores moderadas e 27 (26%) com dores intensas; e SF-36 de 66,8. Dos 23 pacientes em prática constante de esporte ou com maior demanda física nos membros superiores, 17 (74%) conseguiram retornar ao mesmo nível. Não observamos complicações significativas exceto por 3 (2,8%) casos de infecção pós-operatória superficial. Conclusão: O tratamento artroscópico para EL recalcitrante do cotovelo é eficaz e seguro, apresentando resultados positivos. Nível de Evidencia IV, Série de Casos.

ABSTRACT

La evolución de las técnicas quirúrgicas tradicionales y los modelos de formación de los cirujanos exigen cambios, por esta razón el centro de investigación y entrenamiento en cirugía de mínima invasión (CLEMI) ha desarrollado y aplicado modelos de enseñanza que permiten aprender técnicas quirúrgicas mínimamente invasivas como la artroscopia de hombro. CLEMI propone un modelo basado en simulación impartido en un ambiente controlado, estructurado y progresivo que vaya al ritmo individual de cada uno de los estudiantes. Inicialmente el estudiante encontrará conceptos teóricos de instrumental, equipos y ergonomía, posteriormente en la fase práctica usando un modelo sintético anatómico del hombro y finalmente usando un modelo biológico con el que se realizan ejercicios para desarrollar destrezas exigidas por las técnicas quirúrgicas. El entrenamiento en modelos bajo ambiente controlado disminuye el período de aprendizaje y eleva la competencia del estudiante.


The evolution of traditional surgical techniques and training models for surgeons demand changes. For this reason, the Latin-American centre for training in minimally invasive surgery (CLEMI) has developed and applied teaching models that allow surgeons to learn minimally invasive surgical techniques such as arthroscopy. CLEMI proposes a model based on simulation presented in a controlled, structured, and progressive environment that is adjusted to the individual rhythm of each of the students. The student initially studies the theoretical concepts of the instruments, equipment, and ergonomics. Later, in the practical phase of the instruction, students perform a series of exercises in technical skills. Training and models under controlled environments decrease the learning period and enhance student skills.

El objetivo principal del curso es alcanzar las habilidades mínimas necesarias para realizar con éxito una artroscopia en el paciente.

ABSTRACT

PURPOSE: This study analyzed the treatment effects of drain insertion in the posteromedial portal after an arthroscopic synovectomy and posterior septum resection for pyogenic arthritis. MATERIALS AND METHODS: From June 2005 to December 2016, 56 cases (55 patients, 1 case of bilateral knee) were diagnosed with pyogenic arthritis and arthroscopic treatment was performed. Fourteen patients (25.0%) were identified with causative organisms. The average follow-up period was 12.3 months, and the mean age was 67.8 years old. Twenty-four cases were males and 31 cases were females. Kellgrene–Lawrence grade (K–L grade) I was 6 patients, II was 20 patients, III was 11 patients, and IV was 18 patients. Surgery was performed through six portal. The posterior septum was removed and a drainage tube was inserted from the posterior medial side to the posterior side of the joint. The Lysholm score was used after surgery for a functional evaluation. The K–L grade at the time of admission and at the last follow-up were compared for a radiological evaluation. RESULTS: The mean normalization period of the C-reactive protein was 59.8 days (6–164 days). Intravenous antibiotics were used for an average of 37.1 days. The results of the functional evaluation revealed a mean Lysholm score of 64.5 (30–98) in a total of 56 cases. Two reoperations was performed, but the treatment was completed with same method in all patients. The K–L grade was similar at the time of surgery and at the last follow-up (p>0.05). CONCLUSION: The method of drain insertion in the posteromedial portal after arthroscopic synovectomy and posterior septum resection for pyogenic arthritis is an effective technique that can obtain satisfactory results by inducing proper exudate discharge.

ABSTRACT

OBJECTIVE@#To observe and evaluate the effects of transcutaneous electrical acupoint stimulation (TEAS) at different time points on postoperative analgesia in perioperative period in patients undergoing shoulder arthroscopic surgery, and to explore the optimal time to use TEAS for shoulder arthroscopic surgery.@*METHODS@#A total of 120 patients undergoing unilateral shoulder arthroscopy under general anesthesia, graded withⅠtoⅡaccording to ASA criteria were randomly divided into 3 groups, 40 patients in each one. The patients in the group A were treated with preoperative TEAS at sham acupoints combined with postoperative TEAS at Hegu (LI 4) and Neiguan (PC 6); the patients in the group B were treated with preoperative TEAS at Hegu (LI 4) and Neiguan (PC 6) combined with postoperative TEAS at sham acupoints; the patients in the group C were treated with TEAS at sham acupoints before and after operation. The parameters of TEAS were dilatational wave, 2 Hz/100 Hz in frequency, 30 min. When the resting-state visual analogue scale (VAS) of incision was more than 3 points, the patient-controlled intravenous analgesia (PCIA) pump of sufentanil was administered to maintain the VAS no more than 3 points. The time point when PCIA pump was firstly used, the used dose of sufentanil and effective pressing number of PCIA pump within 24 hours after surgery were recorded. Intraoperative anesthetic doses were recorded in the three groups. The resting-state and task-state VAS were evaluated at 0, 6, 12, 24 hours after surgery; the patient's satisfaction rate and adverse effects were recorded.@*RESULTS@#The time when PCIA pump was firstly used in the group A and the group B was significantly longer than that in the group C, and the used dose of sufentanil and effective pressing number of PCIA pump within 24 hours after surgery in the group A and group B were significantly less than those in the group C (all 0.05). There were no statistically significant difference in VAS score at different postoperative time points and postoperative analgesia satisfaction rate between the two groups (all >0.05).@*CONCLUSION@#Perioperative TEAS could improve the postoperative analgesia in patients undergoing arthroscopic shoulder surgery, delay the time when PCIA pump is firstly used, reduce the dosage of postoperative analgesics and adverse events. Compared before surgery, postoperative TEAS has better analgesia.

ABSTRACT Objective: The cause of anterior shoulder instability is not fully understood and surgical management remains controversial. The objective of this study was to evaluate the results of patients undergoing arthroscopic Latarjet procedure with endobuttons. Methods: A retrospective study of 26 patients undergoing arthroscopic Latarjet procedure with endobuttons to treat anterior shoulder instability. Patients with previous glenohumeral instability, failure of Bankart procedure or Instability Severity Index Score (ISIS) greater than or equal to 6, were included. Patients were assessed by: DASH, UCLA, Rowe, Visual Analog Scale (VAS) of pain and Short-Form 36 (SF36) scores. Correct position and consolidation of the graft were evaluated. Results: Mean age was 31.5 years (16 to 46). Preoperative duration of symptoms was 1.7 years (1 month to 10 years). Mean follow-up was 14.3 (6 to 24) months. Mean postoperative scores were: 10 points in DASH; 1.6 in VAS, where 23 (88%) patients experienced mild pain and 3 (12%) moderate pain; 89 in Rowe; 32 in UCLA and 78 in SF-36. Positioning of the graft was correct in 25 (96%) cases, and was consolidated in 23 (88%). We had two cases of graft fracture (7%) and postoperative migration (7%). Conclusion: Surgical treatment using arthroscopic Latarjet with endobuttons is safe and effective, producing good functional outcomes in patients. Level of Evidence IV, Case Series.


RESUMO Objetivo: A causa da instabilidade anterior do ombro não é totalmente esclarecida e o tratamento cirúrgico é controverso. O objetivo deste estudo foi avaliar o resultado dos pacientes submetidos à técnica de Latarjet artroscópica com endobuttons. Métodos: Estudo retrospectivo de 26 pacientes submetidos à técnica de Latarjet artroscópica com endobuttons para tratamento de instabilidade anterior do ombro. Foram incluídos pacientes com instabilidade glenoumeral anterior, falha no procedimento de Bankart ou Instability Severity Index Score (ISIS) maior ou igual a 6. Foram avaliados mediante DASH, UCLA, Rowe, Escala Visual Analógica de dor (EVA) e pelo Short-Form 36 (SF36). Avaliamos também a posição correta e a consolidação do enxerto. Resultados: A média de idade foi de 31,5 anos (16 a 46). Os sintomas antes da cirurgia foram de 1,7 anos (1 mês a 10 anos). Seguimento médio de 14,3 (6 - 24) meses. A média dos escores pós-operatórios foi de 10 pontos no DASH; 1,6 pontos na EVA sendo 23 (88%) dores leves e 3 (12%) dores moderadas; Rowe de 89, UCLA de 32; SF-36 de 78. O posicionamento foi correto em 25 (96%) casos e consolidou em 23 (88%). Tivemos 2 casos de fratura (7%) e de migração pós-operatória do enxerto (7%). Conclusão: A cirurgia de Latarjet artroscópica com endobuttons é eficaz e segura, produzindo bons resultados funcionas. Nível de Evidencia IV, Série de Casos.

ABSTRACT

Objective To investigate the effect of arthroscopic surgery treating global pincer femoroacetabular impingement. Methods Global (center-edge angle ≥ 40°) and Focal (center-edge angle 25° ~ 39°) cohorts were based on pre-operative radiographs. Pre-operative and intra-operative findings, surgical procedures, postoperative nonarthritic hip score (NAHS) and satisfaction (5-point Likert scale), complications and conversion arthroplasties were compared. A nested case-control study was also performed. Results The Global cohort consisted of 15 patients of mean age (37.2 ± 11.3) years. Pre-operative NAHS was (51.5 ± 22.7) and (74.1 ± 16.6) at 24 months after operation. The change in NAHS was significant (P = 0.001). Mean satisfaction was (4.2 ± 1.2). There was one total hip arthroplasty (THA) conversion (6.7%), no revision surgeries or complications. The Focal cohort consisted of 125 patients of mean age (39.8 ± 13.5) years. Pre-operative NAHS was (55.5 ± 17.1) and (77.3 ± 18.5) at 24 months after operation. The change in NAHS was significant (P = 0.015). Mean satisfaction was (4.2 ± 1.3). There were eight THA conversions (6.4%), three complications (2.4%) and two revision surgeries (1.6%). Cohort comparisons revealed no statistically significant difference in NAHS (P = 0.301), satisfaction (P = 0.926) and THA conversion rate (P = 0.912). The nested case-control study found mean post-operative change in NAHS was (22.2 ± 6.9) and (21.5 ± 8.8), respectively, at 24 months (P = 0.955). Conclusion The global pincer FAI was safe and effective with arthroscopic surgery, and the effect was similar to that of the local pincer FAI .

ABSTRACT

BACKGROUND: Ultrasound-guided interscalene brachial plexus block (US-ISB) has been reported to be effective postoperative analgesia for arthroscopic shoulder surgery. Although considered rare, various neurological complications have been reported. We retrospectively evaluated 668 patients for post operation neurological symptoms including hemidiaphragmatic paresis and post-operative neurologic symptoms after US-ISB. METHODS: We performed a retrospective chart review of 668 patients undergoing shoulder surgery with single-shot US-ISB from January 2010 to May 2015. The general anesthesia prior to the US-guided ISB procedure was standardized by expert anesthesiologists. Neurological postoperative complications were evaluated at 48 hours, about 2 weeks, 1 month, 3 months, 6 months, and up to resolution after operation. RESULTS: Three patients (0.4%) developed hemidiaphragmatic paresis (HDP), which were likely US-ISB associated and improved within 1 day. Two patients developed sensory symptoms, also likely US-ISB associated; one was paresthesia at the tip of the thumb/index finger, which resolved within 2 weeks, and the other was hypoesthesia involving the posterior auricular nerve, which resolved within 6 months. Motor and sensory symptoms which were not likely associated with US-ISB were hypoesthesia and pain (n = 28, 4.6%) and motor weakness (n = 2, 0.3%). CONCLUSIONS: Incidence of HDP and neurological complications, respectively 0.4% and 0.3%, related to transient minor sensory symptoms occurred after US-ISB for arthroscopic shoulder surgery but the complications improved spontaneously. Therefore, we confirm that the US-ISB procedure with low volumes of local anesthetics is an acceptable technique with a low rate of HDP and neurological complications.

Objective@#To evaluate the necessity of arthroscopic suture repair for the stable ramp lesion.@*Methods@#From August 2008 to April 2012, 91 patients of consecutive cases were diagnosed as anterior cruciate ligament (ACL) injury associated with stable ramp lesion in Department of Sports Medicine Service of Beijing Jishuitan Hospital. All of the diagnosis was confirmed during arthroscopic surgeries. The patients were randomized into suture repair group and control group. The ACL was reconstructed using auto grafts of hamstring. MRI was used for evaluation of the healing of the ramp lesion during the follow-up period. The stability of the knee joint was evaluated using KT-1000 device. The clinical functional outcomes were analyzed with Lysholm and International Knee Documentation Committee(IKDC) score. t test and χ2 test were used to analyze the data, respectively.@*Results@#There were 73 patients (80%) were available for final follow-up. The follow-up period was 24 to 72 months (mean (38.9±16.1) months). The suture repair group included 40 patients, while the control group included 33 patients. There were no statistically difference in age(t=0.458, P>0.05), gender(χ2=0.222, P>0.05), time interval from injury(t=0.643, P>0.05) and associated injuries(χ2=0.658, P>0.05). At the final follow-up, the healing rate of ramp lesion in suture repair group was 97.5% (95% of complete healed, 2.5% of partial healed, and 2.5% of non-healed), in control group the healing rate was 93.9% (87.8% of complete healed, 6.1% of partial healed, and 6.1% of non healed), there were no significant differences between the two groups (χ2=0.446, P>0.05). There was no statistically significant difference of postoperative knee stability (t=-0.510, P>0.05). The healing status of ramp area from MRI evaluation had statistically significant correlation with the ACL stability (χ2=11.919, P<0.01). For the analysis of postoperative Lysholm scores and IKDC scores, there was no significant difference between the two groups (t=-0.166, P>0.05; t=0.032, P>0.05).@*Conclusions@#Stable ramp lesion could be well healed after arthroscopic treatment of all-inside suture repair as well as refreshment only. For patients with ACL injury and stable ramp lesion, there was no significant difference between the results of MRI evaluation in the early postoperative stage.The postoperative healing status of ramp area had statistically significant correlation with the stability of knee joint.

ABSTRACT

Objective To compare effectiveness,performance,and complications between ultrasound-guided selective cervical nerve root block and interscalene brachial plexus block for patients undergoing arthroscopic surgery in perioperative period.Methods Seventy patients scheduled for arthroscopic surgery,25 males and 45 females,aged 18-75 years,were randomly divided into two groups.They were given either selective cervical nerve root block (group S,n =35) or interscalene brachial plexus block (group ISB,n=35).In group S,C5 and C6 nerve roots were given 0.5% ropivacaine 5 ml respectively;In group ISB,patients were given 0.5% ropivacaine 10 ml under ultrasound guidance.The primary outcome:VAS score and forearm modified Bromage scale (MBS) score were recorded at 4,12 and 24 hours after surgery;Secondary outcomes:cumulative tramadol consumption,the patients' satisfaction rate and adverse effects were recorded.Results The VAS scores in group S was significantly lower than that in group ISB at 12 hours after surgery (1.7±0.8 vs 3.6±0.7,P<0.05).The forearm MBS scores in group S was significantly higher than that in group ISB 4 hours after surgery (P<0.01).Compared with group ISB,the amount of tramadol consumption was lower at 24 hours after surgery [(37.5±35.9) mg vs (112.5±43.5) mg,P<0.05)].The satisfaction rate of group S was higher than group ISB (88% vs 56%,P<0.05).There was no significant difference in side effects between the two groups.Conclusion In arthroscopic surgery,the selective cervical nerve root block is superior to the brachial plexus block.

ABSTRACT

PURPOSE: The purpose of this study is to analyze the causative factors that affect the recurrence and positive culture results of pyogenic knee arthritis after arthroscopic treatment. MATERIALS AND METHODS: A total of 69 patients, who were followed-up for more than one year after arthroscopid lavage and debridement for pyogenic knee arthritis, between February 2011 and February 2015, were retrospectively reviewed for analysis. We analyzed the factors affecting the recurrence rate, preoperative C-reactive protein (CRP) level, neutrophil count in synovial fluid, growth of bacteria in culture, and normalization of CRP level, and also analyzed potential factors that may influence the treatment results and recurrence. RESULTS: The recurrence rate was significantly higher in the culture-positive group (p=0.014). Culture-positive patients had low preoperative CRP (p=0.004), shorter duration for normalization of CRP (p=0.037), shorter duration of hospital stay (p=0.001) and intravenous antibiotics use (p=0.021). The duration for normalization of the CRP level had was associated with symptom-to-treatment interval (p=0.005), neutrophil count in synovial fluid (p=0.007), and growth of bacteria in the culture (p=0.001). There were 16 cases (23.2%) culture-positive cases and 15 cases (21.7%) recurrent cases. CONCLUSION: According to our study, early diagnosis and management affect rapid recovery of the CRP level in pyogenic arthritis of the knee. Preoperative CRP and leukocyte counts in the joint fluid are of diagnostic value in acute infection. However, it should not be overlooked that their diagnostic value is limited as a single factor as they may have low preoperative values. Therefore preoperative CRP and leukocyte counts in the joint fluid should be considered in conjunction with symptoms and intra-articular fluid manifestations. Bacteria growth in the culture are meaningful factors in an evaluation for postoperative recovery and recurrence.

ABSTRACT

BACKGROUND: The beach chair position (BCP) can cause significant hypotension. Epinephrine is used to prolong the duration of local anesthetics; it is also absorbed into blood and can exert systemic effects. This study determined the effects of epinephrine mixed with ropivacaine for an interscalene block (ISB) on hemodynamic changes related to BCP. METHODS: Patient data collected from March 2013 to August 2014 were used retrospectively. We divided the patients into three groups: 1) ISB only, 2) I+G (general anesthesia after ISB without epinephrine), and 3) I+E+G (general anesthesia after ISB with epinephrine). Mean blood pressure (MBP) and heart rate (HR) were measured for 30 minutes at 5-minute intervals. RESULTS: The study analyzed data from 431 patients. MBP tended to decrease gradually in the groups I+G and I+E+G. There were significant differences in MBP between the groups I+G and I, and between the groups I+G and I+E+G. Group I+E+G showed a significant increase in HR compared with the other two groups. CONCLUSIONS: ISB with an epinephrine mixture did not prevent hypotension caused by the BCP after general anesthesia. HR increased only in response to the epinephrine mixture. A well-planned prospective study is required to compare hemodynamic changes in that context.
