ABSTRACT

Objective:To analyze the clinical application effect of otoendoscopic myringoplasty with tragus perichondrium.Methods:The clinical data of 20 patients with tympanic membrane perforation who received treatment in Suixian Traditional Chinese Medicine Hospital from March 2019 to March 2020 were retrospectively analyzed. Among the 20 patients, 6 patients had traumatic tympanic membrane perforation, and 14 patients had tympanic membrane perforation caused by chronic suppurative otitis media (stationary phase). All patients underwent otoendoscopic myringoplasty with tragus perichondrium. After surgery, the healing of tympanic membrane perforation and the improvement of hearing were evaluated.Results:After surgery, all patients were followed up for 12 months. Traumatic tympanic membrane perforation healed in six patients. The tympanic membrane grafts survived in 12 patients with tympanic membrane perforation caused by chronic suppurative otitis media (stationary phase). One patient had poor postoperative healing and the remaining small perforation healed after secondary treatment under local anesthesia. Healing from tympanic membrane perforation was not achieved in one patient because of perforation caused by otitis media. The healing rate of tympanic membrane perforation caused by chronic suppurative otitis media was 95%. Before surgery, the air conduction pure tone hearing threshold was (42 ± 11) dBHL, and it was (25 ± 10) dBHL 12 months after surgery. There was a significant difference in air conduction pure tone hearing threshold between before and after surgery ( t = 4.00, P < 0.05). No complications such as tragus cartilage infection, skin flap necrosis of external auditory meatus, peripheral facial paralysis, hearing loss, or tinnitus occurred in 20 patients with tympanic membrane perforation. Conclusion:Otoendoscopic myringoplasty with tragus perichondrium leads to a high perforation healing rate, improves healing greatly, has no serious complications, produces minimal trauma, and contributes to a rapid recovery from tympanic membrane perforation.

Resumen Objetivo: Analizar la diferencia en la pérdida auditiva según el consumo de alcohol, en mujeres de entre 20 y 40 años de edad, en la comuna de Temuco, Chile. Metodología: Estudio de corte, donde se evaluaron 30 mujeres consumidoras y no consumidoras de alcohol, divididas en 3 grupos (G): G1 abstemias, G2 consumidoras de 40-59 y G3 consumidoras demás de 60g de alcohol diarios (15, 6 y 9 respectivamente), con un muestreo intencional, contactadas en centros comerciales. Se realizaron pruebas auditivas: audiometría, discriminación de la palabra y potenciales evocados auditivos de tronco cerebral. Resultados: El oído derecho presentó diferencias significativas en las frecuencias medias y discriminación de la palabra en G1 vs. G2 (p = 0,045 y 0,010, respectivamente). El oído izquierdo mostró diferencias significativas en las frecuencias agudas en G1 vs. G3, en las latencias interpeakde las ondas iii-v (tiempo en milisegundo transcurrido entre la onda iii y v posterior al estímulo) en G1 vs. G2 (p = 0,003) y G2 vs. G3 (p = 0,005) a 80 dB; en las latencias interpeaki-iiiy iii-v en G1 vs. G2 (p = 0,039) a 60 dB y en la discriminación de la palabra en G1 vs. G2 (p = 0,009) y G1 vs. G3 (p = 0,013). Conclusiones: En el grupo estudiado existe pérdida auditiva relacionada con un consumo diario a partir de los 40g de alcohol, siendo el oído más afectado el izquierdo, en las frecuencias medias y agudas, en la cóclea y el tronco cerebral.


Abstract Objective: To analyze the relationship between alcohol consumption and loss hearing in women between 20 and 40 years of age in Temuco city. Methodology: Cutting study. We evaluated 30 consuming and non alcoholic women divided into 3 groups (G): G1 abstemies, G2 consumers of 40-59 and G3 consumers of> 60 grams alcohol daily (15, 6 and 9 respectively), intentional sampling contacted in shopping centers. Auditory tests were performed: Audiometry, Discrimination of the Word and Auditory Evoked Potentials of the Brain Stem. Results: In the right ear, there were significant differences in medium frequencies and word discrimination between groups 1 versus 2 (p = 0.045 and 0.010, respectively). In left ear, there were significant differences in the high frequencies between groups 1 versus 3, interpeak latencies of iii-v waves (time elapsed between iii and v waves in millisecond after the stimulus) between groups 1 versus 2 (p = 0.003) and groups 2 versus 3 (p = 0.005) to 80 dB; in interpeak latencies i-iii and iii-vbetween groups 1 versus 2 (p = 0.039) at 60 dB and in word discrimination between groups 1 versus 2 (p = 0.009) and 1 versus 3 (p = 0.013). Conclusion: In the group studied there is hearing loss related to a daily consumption from the 40 grams of alcohol being the most affected ear the left in the medium and high frequencies and affecting to the cochlea and also brainstem.


Resumo Objetivo: O objetivo foi analisar a relação entre consumo de álcool e perda auditiva em mulheres entre 20 e 40 anos. Metodologia: Foi realizado um estudo transversal não experimental em que 30 consumidoras femininas e não o álcool foram divididas em 3 grupos: abstêmios G1, consumidores G2 de 40-59 gramas e consumidores G3> 60 gramas de álcool por dia. Testes auditivos foram realizados: audiometria, discriminação de palavras e potencial evocado auditivo de tronco encefálico. O teste não paramétrico de Mann-Whitney U foi utilizado para a análise estatística. Resultados: Na orelha direita, há diferenças significativas nas frequências médias e discriminação de palavras entre os grupos 1 e 2 (p = 0,045 e 0,010, respectivamente). Na orelha esquerda, há diferenças significativas nas freqüências agudas entre os grupos 1 versus 3, nas latências interpicos das ondas iii-v entre os grupos 1 versus 2 (p = 0,003) e os grupos 2 versus 3 (p = 0,005) a 80dB; nas latências interpicos i-iii e iii-v entre os grupos 1 versus 2 (p = 0,039) a 60 dB e na discriminação de palavras entre os grupos 1 versus 2 (p = 0,009) e 1 versus 3 (p = 0,013). Conclusões: Há uma perda auditiva relacionada ao consumo diário de 40 gramas de álcool, sendo que a orelha mais afetou a esquerda nas freqüências média e aguda, afetando a cóclea e o tronco cerebral.

ABSTRACT

RESUMO Objetivo Verificar o benefício obtido pela prescrição de ganho acústico baseada nos limiares audiométricos obtidos com tons puros (Warble) e com ruído de faixa estreita (NB). Método Amostra de 30 idosos, com idade igual ou superior a 60 anos, perda auditiva neurossensorial de grau moderado a severo simétrica bilateral de configuração descendente com limiares em 4kHz iguais ou inferiores a 70dBNA. Foram dois grupos. GTP (grupo tom puro): 15 idosos tiveram as próteses auditivas adaptadas com emprego dos limiares obtidos com tom puro e grupo GNB (grupo Narrowband): 15 idosos tiveram as próteses auditivas adaptadas por meio dos limiares obtidos com NB. Os procedimentos realizados antes da adaptação de próteses auditivas e após três meses de uso de amplificação foram: Escala COSI, IPRF (Índice Percentual de Reconhecimento de fala), Relação Sinal/Ruído e análise do tempo de uso do AASI. O Questionário Internacional de Aparelho de Amplificação Sonora Individual (QI-AASI) foi aplicado após três meses. Resultados Os idosos do grupo em que as próteses auditivas foram adaptadas com ganho prescrito com base nos limiares auditivos obtidos com o estímulo Narrow Band apresentaram melhor desempenho nos seguintes testes: IPRF à orelha direita, pontuação total do questionário QI-AASI, escala COSI e maior tempo de uso do AASI em comparação ao grupo GTP. Conclusão Observou-se maior benefício com o uso de próteses auditivas, pela pontuação total do questionário QI-AASI, escala COSI e maior tempo de uso do AASI, no grupo cuja prescrição do ganho acústico baseou-se nos limiares audiométricos obtidos com o Narrowband.


ABSTRACT Purpose To verify the benefit obtained by the prescription of acoustic gain based on the auditory thresholds obtained with pure tones modulated in frequency and with Narrow Band Noise. Methods The sample consisted of 30 elderly people, aged 60 years or over with moderate to severe descending sensorineural symmetrical hearing loss with thresholds at 4kHz equal to or less than 70dBHL. There were two groups. GTP (pure tone group): 15 elderly people had their hearing aids fitted through the auditory thresholds obtained with pure tone and the GNB group (narrow band group): 15 elderly people had their hearing aids fitted through the auditory thresholds obtained with NB. The procedures performed before the fitting of hearing aids and after three months of amplification use were: COSI, WRS (Word Recognition Score), Signal/Noise ratio. The International Outcome Inventory for Hearing Aids (IOI-HA) was applied only after three months of hearing aid fitting. Results The elderly people in the group in which the hearing aids were fitted with a prescribed gain based on the hearing thresholds obtained with the Narrow Band stimulus showed better performance in the following tests: WRS on the right ear, total score of the IOI-HA inventory, COSI and longer use of hearing aids compared to the GTP group. Conclusion There was a greater benefit with the use of hearing aids, due to the total score of the IOI-HA inventory, COSI scale and longer daily use time of hearing aids, in the group whose prescription of acoustic gain was based on the auditory thresholds obtained with narrow band.

Abstract Introduction: The study of the threshold level of cortical auditory response in adults has been investigated in previous studies. Due to maturational issues, little is known about these responses in neonates. Technological advances with automatic analysis devices now allow investigation in specific populations. Thus, new studies are needed to establish the feasibility of using this auditory potential to identify the lowest levels of responses in children. Objective: Verify and compare latency and amplitude in 80 dBnNA and the minimum level of cortical auditory response in term and preterm neonates. Methods: A cross-sectional, comparative study involving 59 neonates, 35 full-term births and 24 preterm births, with positive results in the Neonatal Hearing Screening. The Hearlab system was used to investigate the P1i auditory potential with tone burst stimulus at frequencies of 500, 1000, 2000 and 4000 Hz. The minimum response level search ranged from 80 to 0 dBNA and was detected automatically. The results were compared between groups, evaluating the latency and amplitude in 80 dBNA and the minimum level of cortical auditory response. Results: The mean values obtained for the minimum level of cortical auditory response in term group were 26 ± 8.81; 26.14 ± 6.97; 29 ± 7.65 and 29.43 ± 7.04 dBNA and for preterm neonates of 31.96 ± 10.41; 34.13 ± 11.34; 33.64 ± 11.03 and 37.73 ± 11.92 dBNA, for the frequencies of 500, 1000, 2000 and 4000 Hz, respectively. There was a difference between groups for the latency of P1i at 4000 Hz and the minimum response levels at 500, 1000 and 4000 Hz, with higher values for preterm infants. Conclusion: It was possible to obtain latency and amplitude values at 80 dBnNA and the minimum level of cortical response in term and preterm newborns, with different results between groups, with higher values in those born preterm.


Resumo Introdução: A investigação do nível mínimo de resposta auditiva cortical tem sido alvo de diferentes estudos em adultos. Devido a questões de maturação, pouco se sabe sobre essas respostas em recém-nascidos. Com o avanço tecnológico, dispositivos de análise automática surgiram com o objetivo de retomar essa avaliação em populações específicas. Assim, novos estudos são necessários para verificar a viabilidade do uso desse potencial auditivo na obtenção de níveis mínimos de respostas na criança. Objetivo: Verificar e comparar latência e amplitude em 80 dBnNA e o nível mínimo de resposta auditiva cortical em recém-nascidos a termo e pré-termo. Método: Estudo transversal, comparativo, envolvendo 59 neonatos, 35 nascidos a termo e 24 pré-termos, com resultados positivos na triagem auditiva neonatal. O sistema Hearlab foi utilizado para investigar o potencial auditivo P1i com estímulo tone burst nas frequências de 500, 1000, 2000 e 4000 Hz. A busca do nível mínimo de resposta variou de 80 a 0 dBNA e foi detectado automaticamente. Os resultados foram comparados entre os grupos, avaliando a latência e amplitude em 80 dBNA e o nível mínimo de resposta auditiva cortical. Resultados: Os valores médios obtidos para o nível mínimo de resposta auditiva cortical no grupo nascido a termo foram 26 ± 8,81; 26,14 ± 6,97; 29 ± 7,65 e 29,43 ± 7,04 dBNA e para recém-nascidos pré-termos foram de 31,96 ± 10,41; 34,13 ± 11,34; 33,64 ± 11,03 e 37,73 ± 11,92 dBNA, para as frequências de 500, 1000, 2000 e 4000 Hz, respectivamente. Houve diferenc¸a entre os grupos para a latência de P1i em 4000 Hz e os níveis mínimos de resposta em 500, 1000 e 4000 Hz, com valores maiores em Pré-termos. Conclusão: Foi possível obter valores de latência e amplitude em 80 dBnNA e o nível mínimo de resposta cortical em recém-nascidos a termo e Pré-termos, com resultados diferentes entre osgrupos, com valores maiores em pré-termos.

ABSTRACT

Objective The tests of three types of auditory evoked potentials (AEPs) were performed on normal young adults, to understand the frequency characteristics of different testing methods and the relationship between response threshold and pure tone audiometry threshold of different methods, and to discuss the forensic value of 3 types of AEPs to evaluate hearing function. Methods Twenty normal young adults were selected, their standard pure tone audiometry threshold, short-term pure tone audiometry threshold and the response threshold of 3 types of AEPs (tone burst-auditory brainstem response, 40 Hz auditory event-related potential and slow vertex response) at 0.5 kHz, 1.0 kHz, 2.0 kHz and 4.0 kHz were recorded. The relationship between the response threshold and standard pure tone audiometry threshold, short-term pure tone audiometry threshold of 3 types of AEPs at different frequencies as well as the differences between different types of AEPs were analyzed. Results The short-term pure tone audiometry threshold was higher than the standard pure tone audiometry threshold at each frequency. The response threshold and standard pure tone audiometry threshold of the 3 types of AEPs all had a certain correlation, and the response threshold of the 3 types of AEPs was higher than short-term pure tone audiometry threshold and standard pure tone audiometry threshold at each frequency. The differences in the differences between the response threshold and standard pure tone audiometry threshold of the 3 types of AEPs at different frequencies had statistical significance. Linear regression mathematical models were established to infer the standard pure tone audiometry threshold (hearing level) from response threshold (sound pressure level) of 3 types of AEPs of normal young adults. Conclusion When using response threshold of different types of AEPs to estimate pure tone audiometry threshold, conversion and correction are needed. Combined use of different types of AEPs could improve the accuracy of hearing function evaluation.

ABSTRACT Purpose This study aimed to evaluate the effects of complete external ear canal occlusion on hearing thresholds with aging. The goal was to decide which tuning fork is more appropriate to use for the contralateral occlusion test (COT), in individuals of different ages. Methods Forty-two normal hearing subjects between 21 and 67 years were divided into three age groups (20-30 years, 40-50 years, and 60-70 years). Participants underwent sound field audiometry tests with warble tones, with and without ear canal occlusion. Each ear was tested with the standard frequencies (250, 500, 1000, and 2000 Hz). The contralateral ear was suppressed by masking. Results Hearing thresholds showed an increase as the frequency increased from 20.85 dB (250 Hz, 20-30 years group) to 48 dB (2000 Hz, 60-70 years group). The threshold differences between occlusion and no occlusion conditions were statistically significant and increased ranging from 11.1 dB (250 Hz, 20-30 years group) to 32 dB (2000 Hz, 20-30 years group). We found statistically significant differences for the three age groups and for all evaluations except to 500 Hz difference and average difference. The mean hearing loss produced by occlusion at 500 Hz was approximately 19 dB. We found no statistically significant differences between right and left ears and gender for all measurements. Conclusion We conclude that the use of the 512 Hz tuning fork is the most suitable for COT, and its use may allow clinicians to distinguish mild from moderate unilateral conductive hearing loss.


RESUMO Objetivo O objetivo deste estudo foi avaliar o efeito da oclusão completa do canal auditivo externo nos limiares auditivos, em indivíduos de idades distintas, para apurar qual o diapasão mais adequado na realização do teste de oclusão contralateral (TOC). Método 42 indivíduos normo-ouvintes (21-67 anos) foram divididos em três grupos etários (20-30, 40-50 e 60-70 anos). Os participantes foram avaliados com testes de audiometria tonal liminar em campo livre, com e sem oclusão completa do canal auditivo externo. Cada ouvido foi testado para as frequências 250, 500, 1000 e 2000 Hz. No ouvido contralateral, foi realizado mascaramento, para evitar a ocorrência de audição contralateral. Resultados Verificou-se aumento dos limiares auditivos, diretamente proporcional ao aumento da frequência (desde 20.85 até 48 dB). A diferença nos limiares auditivos entre a condição de oclusão e de não oclusão foi estatisticamente significativa em todas as frequências e aumentou de forma diretamente proporcional com a frequência (desde 11.1 até 32 dB). Foram também encontradas diferenças estatisticamente significativas para os três grupos etários, em todos os parâmetros, exceto na diferença a 500 Hz e na diferença total média. A perda auditiva média resultante da oclusão aos 500 Hz foi de 19 dB. Não se encontraram diferenças estatisticamente significativas entre o ouvido direito e o esquerdo, e entre o gênero. Conclusão A utilização do diapasão de 512 Hz é a mais adequada para o TOC. A sua utilização pode permitir aos clínicos, em ambiente de consulta e de forma rápida, a distinção entre perda condutiva de grau leve a moderada.

Introdução: A audição, sentido que nos permite receber a informação sonora, se apresenta como um dos facilitadores para uma interação social eficaz. Um déficit nessa função acarreta prejuízo no processo comunicativo, devendo ser diagnosticado e tratado precocemente. Atualmente dispomos de meios subjetivos e objetivos para avaliar o sistema auditivo. A audiometria tonal é a forma subjetiva mais aplicada para medir esse sentido. Entretanto, em populações impossibilitadas de responder subjetivamente, aplicam-se técnicas objetivas, como o Potencial Evocado Auditivo de Estado Estável. Ambos pesquisam respostas auditivas do sujeito sob teste, utilizando técnicas distintas. Objetivo: Avaliar adultos jovens, sem queixas auditivas, pesquisando a sensibilidade auditiva e, a partir de indicadores estatísticos, calcular as médias das diferenças das respostas obtidas nesses procedimentos, verificando se há correlação entre eles. Método: Pesquisa de respostas auditivas via Audiometria Tonal e Potencial Evocado Auditivo de Estado Estável, nas frequências de 500, 1000, 2000 e 4000 Hz, em 30 sujeitos normo-ouvintes. Resultados: Ao calcularmos a média das diferenças entre as frequências testadas nos dois procedimentos, elas variaram de 10,47 a 18,22, não havendo forte correlação em nenhuma delas, deixando dúvidas na frequência de 1000 Hz. Conclusão: Podemos dizer que as médias das diferenças entre os valores obtidos nos dois procedimentos foram razoavelmente elevadas, principalmente em 500 Hz e, em menor proporção, para 4 kHz. Os valores obtidos nos dois exames, embora não tenham apresentado forte correlação, apresentaram-se discretamente melhores para 500 Hz. Resultados de 1000 Hz não nos permitem afirmar, inclusive, se existe alguma correlação entre os testes.


Introduction: Hearing is the sense which allows us to receive auditory information; therefore it facilitates efficient social interaction. Any loss in this function damages the communicative process, so it must be diagnosed and treated as soon as possible. Nowadays, both objective and subjective hearing tests are available. Pure Tone Audiometry is the most widely used subjective way to evaluate this sense. However, for those who are unable to respond subjectively, we can try objective techniques like the Auditory Steady State Response. Both will search for patients' minimum hearing responses, applying different approaches. Objective: To evaluate normal-hearing young adults with no hearing complaints by analyzing their hearing sensibility and, based on statistical indicators, calculate the average of the differences between the results obtained in both procedures in order to verify the existence of correlation between them. Method: We studied the hearing responses from 30 normal-hearing subjects through Pure Tone Audiometry and Auditory Steady State Response at 500, 1000, 2000 and 4000 Hz. Results: When we calculated the average of the differences between the tested frequencies, they ranged from 10,47 to 18,22 with no strong correlation, except at 1000 Hz whose results were uncertain. Conclusion: We concluded that the average of the differences of hearing values obtained in Pure Tone Audiometry and Auditory Steady State Response were reasonably elevated mainly at 500 Hz, and at a lower level at 4000 Hz. Although the scores obtained in both tests had not shown strong correlation, they were slightly better at 500 Hz. The results for 1000 Hz are inconclusive regarding any existing correlation between these two tests.


Introducción: La audición, sentido que nos permite recibir la información sonora, se presenta como uno de los facilitadores para una interacción social eficaz. Un déficit en esta función acarrea perjuicio en el proceso comunicativo, lo que debe ser diagnosticado y tratado lo antes posible. Actualmente disponemos de medios subjetivos y objetivos para evaluar el sistema auditivo. La audiometría tonal es la forma subjetiva más aplicada para medir ese sentido. Sin embargo, en poblaciones imposibilitadas de responder subjetivamente, se aplican técnicas subjetivas como el Potencial Evocado Auditivo de Estado Estable. Ambas investigan las respuestas auditivas del sujeto bajo test, utilizando técnicas distintas. Objetivo: Evaluar a adultos jóvenes, sin quejas auditivas, investigando la sensibilidad auditiva y, a partir de los indicadores estadísticos, calcular los promedios de la diferencias de las respuestas obtenidas en esos procedimientos, y comprobar si existe correlación entre ellos. Método: Pesquisa de respuestas auditivas vía Audiometría Tonal y Potencial Evocado Auditivo de Estado Estable en las frecuencias de 500,1000, 2000 y 4000 Hz, en 30 sujetos oyentes normales. Resultados: Tras el cálculo del promedio de las diferencias entre las frecuencias testadas en los dos procedimientos, se pudo verificar una variación de 10,47 a 18,22, donde no se ha registrado fuerte correlación en ninguna de ellas, dejando duda en la frecuencia de 1000 Hz. Conclusión: Podemos decir que los promedios de las diferencias entre los valores obtenidos en los dos procedimientos fueron razonablemente elevados, sobre todo en 500 Hz y, en menor proporción, para 4kHz. Los valores que se obtuvieron en los dos exámenes, aunque no hayan presentado fuerte correlación, se presentaron discretamente mejores para 500 Hz. Resultados de 1000 Hz no nos permiten afirmar, incluso, si hay alguna correlación entre los testes.

RESUMO Objetivo Avaliar os limiares auditivos nas altas frequências, buscando comparar as respostas entre as orelhas, verificar a correlação do nível de audibilidade com o aumento da idade e analisar as respostas por frequência para adultos normo-ouvintes. Método Este estudo foi prospectivo, quantitativo e transversal, com amostra por conveniência. Fizeram parte da amostra 60 sujeitos, com idade entre 18 e 58 anos, média de idade de 25,8 anos, com limiares auditivos dentro dos padrões de normalidade de 250 a 8000 Hz e normalidade de resultados nas medidas de imitância acústica. Para a pesquisa das altas frequências foi utilizado o equipamento de modelo AS10 HF da marca Interacoustics com fones de ouvido do tipo KOSS R/80 e com limiares expressos em dBNPS. Resultados Quanto aos limiares de audibilidade há um aumento para a orelha direita, com significância estatística em 10, 11 e 14 kHz e uma tendência à significância em 13 kHz, assim como a partir da frequência de 14 kHz há um aumento progressivo diretamente proporcional à frequência bilateralmente e quanto maior for a idade dos indivíduos, maiores serão os limiares para todas as frequências. Conclusão Os limiares de audibilidade de altas frequências tornam-se maiores com um aumento progressivo proporcional ao avanço de frequência e idade de indivíduos normo-ouvintes com valores mais elevados para a orelha direita.


ABSTRACT Purpose Evaluate high-frequency auditory thresholds, seeking to compare responses between the ears, to verify the correlation between hearing level and aging and analyze frequency responses in normal hearing adults. Methods This is a prospective, quantitative, transversal study conducted with a convenience sample. Study participants were 60 individuals aged 18 to 58 years (mean=25.82) with auditory thresholds within normality standards (250-8000 Hz) and normal results in acoustic immittance measurements. High-frequency pure-tone hearing thresholds were determined using an Interacoustics AS10HF audiometer with electrodynamic high-fidelity KOSS R/80 headphones, with thresholds expressed in dBNPS. Results Hearing thresholds showed an increase for the right ear with statistical significance at the 10, 11 and 14 kHz frequencies and a tendency to significance at 13 kHz. As of the 14 kHz frequency, a progressive increase directly proportional to the frequency was observed bilaterally, with the thresholds increasing proportionally to age advancement for all frequencies. Conclusion High-frequency auditory thresholds progressively increase proportionally to frequency and age advancement in normal hearing individuals, with higher values ​​for the right ear.

ABSTRACT

OBJECTIVE To compare the difference of octave band CE-Chirp ABR response threshold and behavioral auditory thresholds in children with hearing loss, and to explore the clinical value of octave band CE-Chirp ABR. METHODS Twenty-one children (40 ears) with hearing loss were selected. The octave band CE-Chirp ABR response threshold and behavioral auditory hearing threshold were measured in quiet environment. The results of two different test methods were compared. RESULTS The frequencies of octave band CE-Chirp ABR response thresholds and behavioral auditory thresholds were statistically analyzed. The correlation coefficients at different frequencies were 0.693, 0.830, 0.836 and 0.845, respectively, and the P values were <0.05. CONCLUSION There is good correlation between octave band CE-Chirp ABR response and behavioral auditory hearing measurement. Octave band CE-Chirp ABR, as an objective audiometry technique, has good stability and reliability for the objective response of hearing loss, and can better reflect the hearing level.

ABSTRACT

Objective:To investigate the relationship between clinical effects of acupuncture for elderly patients with sensorineural deafness and ear distending sensation.Methods:A total of 120 elderly patients with sensorineural deafness were randomly divided into a comprehensive treatment group,an acupuncture group and a Western medicine group,with 40 cases in each group.The acupuncture group received acupuncture treatment,the Western medicine group received oral mecobalamin tablets and the comprehensive treatment group received acupuncture plus acupoint injection and auricular acupoint sticking.The values of pure tone hearing threshold test of the three groups were observed before and after treatment,and the relationship between clinical effects and ear distending sensation was compared.Results:The total effective rate of the comprehensive treatment group was 82.5% versus 67.0% in the acupuncture group and 62.5% in the Western medicine group.The inter-group comparisons showed statistically significant differences in the comprehensive treatment group versus the Western medicine group (P<0.01) and the acupuncture group (P<0.05).In the comprehensive treatment group,there were 23 cases (57.5%) with ear distending sensation,the clinical total effective rate was 86.9%;there were 17 cases (42.5%) without the sensation,the clinical total effective rate was 76.5%.In the acupuncture group,there were 24 cases (60.0%) with ear distending sensation,the clinical total effective rate was 71.0%;there were 16 cases (40.0%) without the sensation,the clinical total effective rate was 63.0%.In the Western medicine group,there were 21 cases (52.5%) with ear distending sensation,the clinical total effective rate was 66.7%;there were 19 cases (47.5%) without the sensation,the clinical total effective rate was 57.9%.The total effective rate of patients with ear distending sensation were higher than the rates of those without ear distending sensation in the three groups,but the differences were insignificant (all P>0.05).Conclusion:The comprehensive therapy is one of the effective methods to treat elderly patients with sensorineural deafness.In the three groups of elderly patients with sensorineural deafness,the relief of ear distending sensation and the hearing loss were basically simultaneous,and the hearing recovery in the patients with ear distending sensation may be slightly better than that in those without ear distending sensation.Nevertheless,further research is needed.

BACKGROUND AND OBJECTIVES: The aim of this study was to evaluate the usefulness of speech mapping based on real ear measurement for routine hearing aid fitting and to compare functional gains for evaluating subjective satisfaction of hearing aid users. SUBJECTS AND METHOD: Twenty two participants with bilateral symmetric (< 10 dB HL difference) sensorineural hearing loss were enrolled in this study. All participants were fitted unilateral hearing aids with speech mapping using the National Acoustic Laboratories-Nonlinear 2 formula. After the initial fitting, patients were followed with the 2nd and 3rd fitting at two weeks and six weeks, respectively, and measured for aided pure tone average (PTA), aided speech discrimination score (SDS), the difference between target gain and real ear insertion gain (REIG) using speech mapping and subjective satisfaction via Korean Adaptation of the International Outcome Inventory for Hearing Aids (K-IOI-HA) questionnaire before further fitting was performed. We analyzed correlation of each parameter at 2nd and 3rd fitting with the K-IOI-HA score. RESULTS: Every sequential aided PTA and SDS at 2nd fitting and 3rd fitting were significantly improved after repeated hearing aid fitting (all p < 0.01). In the correlation analysis between K-IOI-HA and each parameter, the aided PTA and aided SDS did not show significant correlations with subjective satisfaction from the 2nd and 3rd fittings. But the difference between the target gain and R EIG in speech mapping showed significant negative correlations with the satisfaction scores at various speech level (r=–0.609 to –0.709, all p < 0.01). CONCLUSION: Speech mapping using real ear measurement was useful to expect subjective satisfaction of hearing aid users and it would be a valuable tool for fine tuning to achieve individual preferences.

ABSTRACT

OBJECTIVES: To measure 125 Hz pure-tone thresholds in patients with low frequency sensorineural hearing loss (LFHL) and vertigo and to evaluate the necessity of 125 Hz thresholds for assessment of LFHL with vertigo. METHODS: Pure tone audiometry including 125 Hz was performed in 25 dizzy patients with LFHL ≤500 Hz and 25 age-matched subjects with normal hearing. Patients with sudden sensorineural hearing loss and vertigo were excluded. Comparison of 125 Hz between LFHL and control groups, and comparison of 125 Hz and other frequencies in LFHL group was made. RESULTS: Mean pure-tone thresholds at 125 Hz in LFHL group (41.7±7.5 dB) was higher than that in normal controls (12.8±6.4 dB). Three (12%) patients had normal thresholds at 125 Hz in LFHL group, whereas all subjects showed normal at 125 Hz in control group. None with average hearing thresholds at 250 and 500 Hz ≥35 dB had normal threshold at 125 Hz. There was a significant correlation between 125 Hz and other low frequencies in LFHL group (250 Hz; r=0.79, 500 Hz; r=0.66). CONCLUSIONS: Not every patient of LFHL with vertigo has abnormal hearing threshold at 125 Hz, although all subjects with normal hearing is within normal limits at 125 Hz. Measurement of 125 Hz pure-tone threshold is highly recommended when a mild LFHL exists.

ABSTRACT

OBJECTIVE To explore the relationship between the clinical characteristics of acoustic neuroma and the number ofT lymphocytes in tumor tissues.METHODS Clinical data of 65 patients diagnosed with VS were retrospectively collected.Peripheral blood samples of the patients were collected and the number of T lymphocytes were counted by flow cytometry.Immunohistochemistry was carried out with the fresh tumor samples compared with 8 normal cranial nerve controls.The expression level of each immune protein was analyzed its correlation with clinical characters was explored.RESULTS The CD3+,CD4+ and CD8+ T lymphocytes in the peripheral blood are not significantly correlated with clinical data.In the tumor tissues,the number of CD4+ and CD8+ T lymphocytes are much larger than those in normal cranial nerves,and negatively correlated with preoperative hearing.The number of CD8+ T lymphocytes has a negative relationship with postoperative facial nerve function.In addition,CD4 and CD8 expression levels in the tumor samples from patients younger than 40 are much higher than those in elder patients.Foxp3 is not detected in the normal cranial nerves.The number of Foxp3+ T lymphocytes is negatively correlated with preoperative hearing.CONCLUSION The CD4+ and CD8+ T lymphocytes mediated cellular immunity play an important role in the development of acoustic neuroma,as well as the reservation and restoration of nerve function.

ABSTRACT

Objective To evaluate the therapeutic effects of early intratympanic steroid of different concentrations and types combined with systemic steroid on severe idiopathic sudden sensorineurl hearing loss(ISSNHL).Methods We reviewed 95 patients with unilateral ISSNHL from January 2012 to June 2016 in the Department of Otolaryngology of the People's Hospital of Changshou and according to the steroid treatment methods,Patients were divided into systemic steroid therapy alone group(group A,30 cases),early intratympanic steroid of low concentration combined with systemic steroid group (group B,32 cases) and early intratympanic steroid of high concentration combined with systemic steroid group (group C,33 cases).Results Total effective rates of the group A,group B and group C were 46.67 %,56.25 %,78.79%.Group C was significantly higher than that of group A and group B(P<0.05);the average pure tone threshold improvement was (46.67±9.32) in group C which was better than group A (20.53±8.83) and group B(25.19± 7.46),the differences were stastically significant (P<0.01).Conclusion Severe ISSNHL should be treated by intratympanic steroid of high concentration combined with systemic steroid as early as possible in order to rescue the hearing,the efficacy of which is much better than other steroid treatments so far.

ABSTRACT

Objective To compare the ear baric function between 4000m altitude chamber test with inhaling air and 6900m altitude chamber test with inhaling pure oxygen.Methods Eleven healthy male volunteers attended two tests as two groups by self-comparison. As the air group the volunteers inhaled air at 4000m, while as the pure oxygen group they inhaled pure oxygen at 6900m altitude, and the time interval between the two tests was more than two weeks. During the test, the volunteers breathed air or pure oxygen at random for 1h, and then were exposed at a speed of 20m/s to the target altitude for 5min. Hereafter they were sent back to the ground at the same speed. The changes of subjective symptoms, degree of tympanic congestion, acoustic immitance index and pure-tone auditory threshold were recorded before and after the test. The acoustic impedance index and pure-tone threshold were statistically analyzed.ResultsFour volunteers (4 ears) in air group and 7 volunteers (7 ears) in pure oxygen group reported ear pain in altitude chamber exposures, respectively. The pain-triggering altitude was higher in the pure oxygen group. Immediately after tests, there were 3 (3 ears) and 5 volunteers (5 ears) with Ⅲ degree congestion of the tympanic membrane in the two groups respectively. Four volunteers (6 ears) developed gradually aggravated hemorrhages after altitude exposure. And the tympanic membrane congestion difference between groups was statistically significant at 3 and 24h after tests (P<0.01). The type A tympanogram appeared in 11 (15 ears) and 11 (14 ears) volunteers respectively immediately after tests. The increase of static compliance value was significantly greater in pure oxygen group than in air group immediately after tests (P<0.05), the decrease of middle ear pressure was more significant in pure oxygen group than in air group at 3 and 24h after tests (P<0.05). Both the two altitude exposure tests resulted in eustachian tube dysfunction. At 3 and 24h after the tests, the increase of individual frequency pure-tone threshold was significantly higher in pure oxygen group than in air group (P<0.05).Conclusion Breathing pure oxygen and lifting height could increase the screening degree of ear baric function test in hypobaric chamber, and have greater influence on degree of tympanic congestion, acoustic immittance and pure-tone auditory threshold in 24 hours.

Introducción: Diversos estudios evidencian que la audiometría de alta frecuencia cumple un importante rol en la detección temprana de pérdida auditiva; sin embargo, en Chile aún no se dispone de una normativa para su aplicación clínica. Objetivo: Normalizar los umbrales audiométricos de alta frecuencia (9-20 kHz) en normoyentes entre 8 y 23 años y 11 meses pertenecientes a la ciudad de Chillán. Material y método: No experimental, tipo descriptivo y corte transversal. Se evaluaron a 259 sujetos normoyentes, estudiantes de enseñanza básica, media y universitaria. A la muestra se le aplicó una audiometría de alta frecuencia, empleando un audiómetro (Gradson Stadler) GSI61 con fonoauriculares (Sennheiser) HDA 200. Resultados: Los umbrales auditivos incrementaron su valor según la edad y frecuencia. No se encontraron diferencias significativas según sexo. Se observaron diferencias significativas interaural en 9, 10 y 14 kHz. Los valores de los umbrales se presentaron en percentiles, considerando como normalidad aquellos ubicados entre el P10 y P75. Conclusión: Esta norma podría ser utilizada como referencia de normalidad para pesquisar tempranamente a sujetos en riesgo de pérdida auditiva y tomar las acciones sanitarias más pertinentes a cada caso en particular, empleando equipamiento similar.


Introduction: There are several studies showing that high frequency audiometry serves the important purpose in hearing loss early detection; nonetheless, Chile does not have a standard regulation for clinical implementation of the previously mentioned exam. Aim: To standardize high frequency thresholds (9-20) in patients with normal hearing in between the ages of 8 and 23 years and 11 months old from Chillan city. Material and method: Non-experimental, descriptive and held in a transversal lapse of time. 259 normal-hearing subjects were evaluated, belonging to elementary, middle, high school and higher -university- levels of education. All subjects underwent a high-frequency audiometry for which was used a (Gradson Stadler) GSI61 audiometer, with (Sennheiser) HDA 200 headphones. Results: It was observed that high-frequency thresholds tended to increase their figures according to age and frequency requirements. No meaningful differences -regarding male or female subjects- were found, although there were note worthy differences in 9, 10 and 14 kHz frequencies. The numbers of the thresholds were presented as percentiles, considering 'normal' those located in between P10 and P75. Conclusion: This regulation can be used as a point of reference for normality in order to early detect subjects having a hearing loss risk, and to also take health-related action suitable for each particular case, by using similar equipment to the one described in this study.

BACKGROUND AND OBJECTIVES: To analyze the 125 Hz pure-tone thresholds in patients with acute low frequency sensorineural hearing loss (LFHL) and to investigate the value of 125 Hz thresholds for the assessment of LFHL. SUBJECTS AND METHOD: Hearing tests including 125 Hz pure-tone were performed in 91 patients with acute LFHL ≤500 Hz and in 46 subjects with normal hearing. Patients with sudden sensorineural hearing loss or Meniere's disease were excluded. Inter-group and intra-group comparison of 125 Hz was made between LFHL and the control groups. RESULTS: There was a significant difference of mean pure-tone thresholds at 125 Hz between the acute LFHL and the normal groups (39.8±8.9 vs. 14.3±6.7 dB). Eight (8.8%) patients in the LFHL group showed normal thresholds at 125 Hz, but all other subjects were normal at 125 Hz in the control group. None with the average hearing thresholds at 250 and 500 Hz ≥40 dB had normal threshold at 125 Hz. There was a significant correlation between 125 Hz and other low frequencies in the LFHL group (250 Hz; r=0.81, 500 Hz; r=0.63). CONCLUSION: Not all patients with acute LFHL show abnormal hearing threshold at 125 Hz although every subject with normal hearing is within the normal limits at 125 Hz. Threshold assessment should be made at 125 Hz when a mild LFHL exists in the conventional pure tone audiometry.

ABSTRACT INTRODUCTION: One of the problems observed in pure-tone audiometry tonal has been the variation in test results of a same individual, particularly at frequencies of 4 kHz, 6 kHz and/or 8 kHz. Improper placement of headphones is one of the factors that can cause alterations in results. OBJECTIVE: To compare differences in auditory thresholds using earphones positioned by the examiner and by the worker. METHODS: Clinical and experimental study conducted in 2009, with 324 workers aged between 19 and 61 years, with a mean of 33.29 years and mean exposure time of 7.67 years. All subjects were familiar with audiometry procedures. Auditory thresholds were obtained at frequencies of 0.25-8 kHz, with earphones positioned by the examiners, and at frequencies of 4, 6 and 8 kHz, with earphones placed by workers in a comfortable position, following the examiner's instructions. The thresholds obtained in these two situations were compared. RESULTS: The three frequencies exhibited better responses with earphones placed by the workers themselves (p < 0.001). At a frequency of 8 kHz a greater difference was found (p < 0.001), with a mean of 13.89 dB and standard deviation of 6.07 dB. CONCLUSION: Earphone placement by the workers themselves under supervision of the examiner results in improved mean auditory thresholds at frequencies of 4, 6 and 8 kHz, the last one significantly higher than the other two.


RESUMO INTRODUÇÃO: Um dos problemas observados na audiometria tonal é a variação nos resultados de testes de um mesmo indivíduo, sobretudo nas frequências de 4, 6 e/ou 8 kHz. A colocação indevida dos fones é um dos fatores que podem causar alterações nos resultados. OBJETIVO: Comparar as diferenças nos limiares auditivos com os fones posicionados pelo examinador e pelo paciente. MÉTODO: Estudo clínico e experimental realizado, em 2009, com 324 trabalhadores, com idade entre 19 e 61 anos, média de 29,33 anos e tempo médio de exposição ao ruído de 7,67 anos; todos familiarizados com os procedimentos da audiometria. Os limiares auditivos foram obtidos nas frequências de 0,25 a 8 kHz, com os fones colocados pelos examinadores; e nas frequências de 4, 6 e 8 kHz com os fones colocados pelos trabalhadores em posição de conforto, sob orientação do examinador. Os limiares obtidos nas duas situações foram comparados. RESULTADOS: As três frequências apresentaram melhores respostas com os fones colocados pelos próprios indivíduos (p < 0,001). Na frequência de 8 kHz foi encontrada a maior diferença (p < 0,001), com média de 13,89 dB e desvio padrão de 6,07 dB. CONCLUSÃO: A colocação dos fones de ouvido pelos próprios trabalhadores, sob supervisão dos examinadores, resulta na melhora dos limiares auditivos médios nas frequências de 4, 6 e 8 kHz, sendo esta última significativamente maior que as demais.

RESUMO Objetivo: Caracterizar os Potenciais Evocados Auditivos de Longa Latência (PEALL) (P1, N1, P2, N2, P300) em campo sonoro, em crianças audiologicamente normais, bem como verificar a estabilidade destes potenciais. Métodos: Trata-se de um estudo prospectivo, longitudinal, composto por 17 crianças audiologicamente normais, na faixa etária de 6 a 13 anos de idade, com limiares de audibilidade dentro da normalidade. Foram captados os PEALL P1, N1, P2, N2, P300 com estímulos de fala e tone burst, em três momentos de avaliação: avaliação inicial (M0), três meses após a avaliação inicial (M3) e nove meses após a avaliação inicial (M9). Resultados: Foi observada diminuição dos valores de latência dos componentes P1 (M0xM3 / M0xM9 / M0xM3xM9) e P2 (M0xM9) e aumento no valor de amplitude do P300 (M0xM3), quando obtidos com estímulo de fala, e diminuição no valor de latência do P300 (M0xM9), obtido com estímulo tone burst. Conclusão: Foi possível identificar os componentes do PEALL na maioria dos indivíduos. Os componentes P1, N1, P2, N2 (tone burst) e N1 e N2 (fala) não sofreram modificações em latências e amplitudes entre os diferentes momentos de avaliação, sugerindo estabilidade deste potencial no período de nove meses. O P300 demonstrou ser um componente mais sensível a esse intervalo de tempo entre as avaliações, pois sofreu modificações indicativas de maturação do sistema nervoso auditivo central. As latências de todos os componentes obtidos com estímulo de fala foram maiores do que com tone burst, demonstrando que estímulos diferentes geram respostas corticais distintas.


ABSTRACT Purpose: To characterize the Long-Latency Auditory Evoked Potentials (LLAEP) (P1, N1, P2, N2, P300) with acoustic stimuli presented in sound field system in children with normal hearing, as well as to verify the stability of these potentials. Methods: This prospective, longitudinal study comprised 17 children with ages between 6 and 13 years and hearing thresholds within normal. The LLAEP P1, N1, P2, N2, P300 were recorded with speech stimuli and tone burst presented in sound field system, in three different moments: initial assessment (M0), three months after the initial assessment (M3), and nine months after the initial assessment (M9). Results: When the speech stimuli was used, there was a decrease in the latency values of components P1 (M0xM3/M0xM9/M0xM3xM9) and P2 (M0xM9), as well as an increase in the amplitude of P300 (M0xM3) over time. With the tone bust stimuli, it was verified a decrease in the latency values of P300 (M0xM9) over time. Conclusion: it was possible to identify the components of the LLAEP in most individuals. The P1, N1, P2, N2 (tone burst) and N1 and N2 (speech) did not change their latencies and amplitudes between the different moments of evaluation, suggesting stability of this potential in the period of 9 months. The P300 was the component most sensitive to the time intervals considered, since it presented modifications over time that indicated maturation of the central auditory nervous system. The latencies of all components obtained with speech stimulus were higher than with tone bursts, indicating that different stimuli generate different cortical responses.

ABSTRACT

Objetivo Caracterizar a variabilidade teste-reteste com fones supra-aurais e fones de inserção e comparar os limiares auditivos tonais entre os transdutores.Métodos Foram selecionados 74 indivíduos de ambos os gêneros, com limiares auditivos normais e curvas timpanométricas do tipo A. Todos os indivíduos foram submetidos à anamnese, meatoscopia, audiometria tonal por via aérea, ora com o fone supra-aural, ora com o fone de inserção, e logoaudiometria. Após esta primeira avaliação, foi realizada a medida de imitância acústica. Por fim, os procedimentos de audiometria tonal foram repetidos.Resultados O fone supra-aural produziu maior variabilidade teste-reteste, quando comparado com o fone de inserção. Já na comparação dos transdutores, verificou-se, tanto na primeira, quanto na segunda testagem, que as respostas mostraram-se ora semelhantes, ora favorecendo um ou outro transdutor.Conclusão Há maior variabilidade teste-reteste com o fone supra-aural do que com o fone de inserção. No entanto, foram verificados melhores limiares na condição reteste, para ambos os transdutores.


Purpose To characterize the test-retest variability with supra-aural and insert earphones and compare the pure tone thresholds between the transducers.Methods Seventy-four individuals of both genders with normal hearing and tympanometric curve of type A were selected. All subjects underwent anamnesis, meatoscopy, air conduction pure tone audiometry, either with the supra-aural earphone, or with insert earphone and speech audiometry. After this initial evaluation Acoustic Impedance Testswere performed. Finally, pure tone audiometry procedures were repeated with both transducers.Results Supra-aural earphone produced higher test-retest variability when compared to the insert earphones. Comparing a transducer with each other, the responses were sometimes similar and sometimes favoring one or the other transducer considering the first and second evaluations.Conclusion There is a greater test-retest variability with supra-aural earphones comparing with insert earphones. However, better thresholds were obtained in the retest condition for both transducers.
