ABSTRACT

Objective@#To investigate the clinical significance of serum low-density lipoprotein receptor-related protein 6 (LRP6) level in patients with acute myocardial infarction (AMI).@*Methods@#One hundred and fifty patients with AMI were selected as the AMI group and 150 patients with suspected coronary heart disease without coronary artery stenosis were selected as control group from January 2017 to December 2018 in Quzhou People′s Hospital of Zhejiang. The serum LRP6 levels were determined by Western blot. The serum B-type brain natriuretic peptide (BNP) and cardiac troponin I (cTnI) levels were determined by enzyme-linked immunosorbent assay (ELISA).@*Results@#The levels of total cholesterol (TC), triglyceride (TG) and low-density lipoprotein cholesterol (LDL-C) in AMI group were higher than those in the control group [(4.42 ± 0.79) mmol/L vs. (3.79 ± 0.82) mmol/L, (1.52 ± 0.33) mmol/L vs. (1.37 ± 0.38) mmol/L, (3.15 ± 0.34) mmol/L vs. (2.91 ± 0.28) mmol/L], and the level of high-density lipoprotein cholesterol (HDL-C) was lower than that in control group [(0.95 ± 0.26) mmol/L vs. (1.21 ± 0.33) mmol/L], and there were significant differences (P<0.05). The level of serum LRP6 and left ventricular ejection fractionin in AMI group were lower than those in control group [0.12 ± 0.03 vs. 0.38 ± 0.07, (53.27 ± 6.89)% vs. (66.82 ± 7.35)%], and the BNP and cTnI levels were higher than those in control group [(78.16 ± 5.27) ng/L vs. (7.13 ± 1.24) ng/L, (125.83 ± 3.26) ng/L vs.(0.71 ± 0.24) ng/L], and there were significant differences (P<0.05). The serum LRP6 level was negatively correlated with LDL-C, BNP, cTnI and SYNTAX scores (r=- 0.587, - 0.523, - 0.542, - 0.583, P<0.05), and was positively correlated with left ventricular ejection fraction (r=0.515, P<0.05).@*Conclusions@#Serum LRP6 level is decreased in patients with AMI. Serum LRP6 is closely related to the severity of AMI and the extent of coronary artery disease.

ABSTRACT

Objective To investigate the levels of N-terminal B-type brain natriuretic peptide (NT-proB-NP) ,homocysteine (Hcy) and high-sensitivity C reactive protein (hs-CRP) in serum of patients with acute coronary syndromes and clinical significance .Methods Twenty hundred patients with acute coronary syn-dromes were enrolled in our hospital from March 2016 to February 2017 .Fifty patients with healthy subjects were selected as control .Serum samples were collected and immunocytochemistry Serum levels of NT-proB-NP ,Hcy and hs-CRP were measured by cyclic enzyme method and immunoturbidimetric turbidimetric assay . Results The levels of NT-proBNP ,Hcy and hs-CRP in serum of patients with acute coronary syndromes were significantly higher than those of healthy subjects .The positive rate of serum test was 91 .50% .The more seri-ous the disease and the higher the number of coronary lesions The levels of NT-proBNP ,Hcy and hs-CRP in patients with acute coronary syndromes were higher .Conclusion The combination of three serum markers coronary artery syndrome in the clinical diagnosis and treatment has important reference value .

ABSTRACT

Heart failure is a serious and terminal stage of various cardiovascular diseases,is one of the most important heart diseases,and it also the most common reason for hospitalization.In recent years,studies on new markers in the diagnosis,treatment and prognosis of heart failure have made great progress,such as B-type brain natriuretic peptide can partially reflect hemodynamic imbalance,troponin can indicate cardiomyocyte injury,soluble ST2 and galectin-3 indicate myocardium remodeling and fibrosis,C-reactive protein,cystatin C and procalcitonin,and copeptin have significant prognostic value for outcomes,microRNA in the regulation of gene expression influence heart failure.

ABSTRACT

Objective To explore the relationship between serum soluble human matrix lysine 2 (sST2) with chronic heart failure(CHF) and its clinical value for diagnosis and prognosis of CHF .Methods 60 cases of CHF and 60 cases of non-CHF were selected as the CHF group and control group respectively ,and the CHF group was divided into sST2 low level group and sST2 high level group according to the diagnostic threshold .The ELISA method was used to detect the serum sST 2 level of each group .The CHF group were followed up for 6 months .Then the influence of sST2 on CHF prognosis survival rate was observed .Results There was no statistical difference in age ,gender ,body mass index ,basic disease history ,basic medication situation and blood lipid indexes between the CHF group and control group(P>0 .05);serum brain natriuretic peptide(BNP) level in the CHF group was obviously higher than that in the control group(P<0 .01);serum sST2 levels in the CHF group and control group were (55 .08 ± 3 .98)ng/mL and (10 .46 ± 0 .72)ng/mL ,the difference was statistically significant (P<0 .01) .Serum sST2 was positively correlated with BNP(r=0 .4606 ,P<0 .01) ,moreover 95% CI was 0 .3066-0 .5911 .When the critical value was 0 .5303 ,the area under curve ,95% CI ,sensitivity ,specificity and likelihood ratio of sST 2 combined BNP detection were 0 .9362 ,0 .8853 -0 .9877 , 85 .00% (73 .43% -92 .90% ) ,98 .33% (91 .06% -99 .96% ) and 50 .00 respectively .The survival curve had statistical difference between the sST2 low level group and sST2 high level group(P=0 .0149) .Conclusion Serum sST2 can be used as a new biomarker for the diagnosis and prognosis of CHF ,and its combined with BNP may have better diagnostic value.

ABSTRACT

Objective To observe the clinical significance and variation of plasma B-type brain natriuretic peptide (△BNP) levels before and after treadmill exercise test (TET) in women with coronary heart disease(CHD). Methods Sixty-one women with suspected coronary heart disease were selected (except for heart failure, blood BNP levels were normal). All patients underwent coronary angiography, were divided into two groups, 25 cases in normal group and 36 cases in coronary heart disease group. Plasma BNP levels were measured before and after the TET test, and data were compared between two groups after one week.Furthermore, the levels of△BNP before and after TET in patients with different degrees of coronary artery lesions were compared. The diagnostic value of the levels of△BNP in coronary heart disease was evaluated by the re-ceiver operating characteristic curve (ROC). Results The plasma BNP level was significantly increased after TET (44.20± 14.53) than that before the test (32.82±10.72) in coronary heart disease group (P<0.01). There were significant differences in levels of△BNP before and after TET in patients with different degrees of coronary lesions. The levels of△BNP were sig-nificantly higher in multi-vessel disease group than those of single and double lesion group (P<0.05). The level of△BNP was significantly higher in double lesion group than that of single lesion group (P < 0.05). When the level of △BNP=7.3 ng/L, the sensibility value and specificity value for the diagnosis of CHD was 82.4%and 81.5%. The area under curve (AUC) was 0.900, and 95%confidence interval was 0.796-0.962. Conclusion The detection of△BNP before and after TET has an important clinical significance in the diagnosis and evaluation of coronary heart disease in women patients.
