ABSTRACT

The FDA approved a total of 37 new drugs in 2022, including 22 new molecular entities and 15 new biological products. This is the year with the lowest number of new drugs approved by the FDA since 2017. Among these approved drugs, 21 new drugs belong to the "first-in-class" category, accounting for 56% of the total approved drugs, which is the highest ratio in the past 10 years. Among the drugs approved in 2022, there are 5 small molecule kinase modulators, including the tyrosine kinase 2 (TYK2) allosteric inhibitor deucravacitinib, the first oral pyruvate kinase (PK) activator mitapivat, the Janus kinase 1 (JAK1) selective inhibitor abcrocitinib, the JAK2 selective inhibitor pacritinib and the broad-spectrum fibroblast growth factor receptor (FGFR) inhibitor futibatinib. This review briefly describes the discovery background, research and development process, synthesis routes and clinical efficacy and safety of small molecule kinase modulators approved by the FDA in 2022, hoping to provide ideas and methods for further research on kinase modulators.

ABSTRACT

Objective Nowadays, radioactive xenon isotopes, including 131mXe, 133mXe, 133Xe, and 135Xe, are primarily released into the atmosphere through various reactor operation and major accidents of reactors. To improve the online monitoring capability of xenon in nuclear facilities and their gaseous effluents, a highly sensitive online xenon monitoring system was developed to monitor, warn, and alarm the activity concentration of radioactive xenon. Methods The online monitoring system for radioactive xenon gas in nuclear facilities was established using xenon membrane separation and concentration, xenon high-efficiency selective adsorption, and low-background gamma-ray spectrometry analysis methods. Results Under the operation mode of one-hour sampling and one-hour measuring, the minimum detectable activity concentration of the radioactive xenon online monitoring system for 133Xe was approximately (1.43 ± 0.03) Bq/m3. Conclusion This system can be effectively used for online monitoring of xenon activity concentration in nuclear facilities such as nuclear power plants and isotope production reactors, as well as in gaseous effluents. It helps improve the safety level of personnel, the environment, and nuclear facilities.

ABSTRACT

ABSTRACT BACKGROUND: Specific types of violence such as intimate partner sexual violence and intimate partner homicide occur more frequently in rural areas. OBJECTIVE: This study aimed to systematically review the literature on the knowledge and attitudes of rural healthcare providers regarding cases of domestic violence against women. DESIGN AND SETTING: Systematic review developed at Universidade Federal de Uberlândia. METHODS: We conducted an electronic search of six databases, which only included observational studies, regardless of the year, language, or country of publication, except for studies that used secondary data and were exclusively qualitative. Two reviewers performed the selection, data extraction, and risk of bias assessment using a specific Joanna Briggs Institute tool. RESULTS: Six studies met the inclusion criteria. All the studies had a low risk of bias. Approximately 38% of these professionals identified injuries caused by violence in patients. When asked about knowing the correct attitude to take in cases of confirmed violence, between 12% and 64% of rural healthcare providers answered positively; most of them would refer to specialized institutions and promote victim empowerment and counseling. The number of professionals with an educational background in the field ranged from 16% to 98%. CONCLUSIONS: The evident disparity across studies shows that some professionals have suboptimal knowledge and require training to adopt the correct attitude when identifying female victims of domestic violence in clinical practice. ABSTRACT

Objectives: This was a single-subject study, aimed to demonstrate different vocal demand situations that are typical for primary school and teacher's vocal demand response under two acoustical conditions, with and without voice amplification, during five working days. Methods: The long-term voice dosimetry with Vocal Holter Med (PR.O. Voice Srl) was carried out on a 49-year-old female teacher with voice disorders during daily teaching activities. A sound field amplification system (SFAS) PentaClass Runa was installed in the classroom. Voice dosimetry was provided under two different acoustical conditions: without SFAS (2 days) and with SFAS (3 days). Results: Phonation time percentage, sound pressure level (SPL), SPL SD, fundamental frequency (F0), F0 SD, cycle, and distance doses were investigated in seven communication scenarios (lessons, group/individual classes, sports lessons in the gym and schoolyard, breaks, lunch breaks, and other activities). The median scores of all voice parameters differed significantly between different vocal demand contexts. The significant statistical difference in the vocal demand response was in the communication situations with and without SFAS. In addition, the number of children, reverberation time, and ambient air relative humidity impacted voice SPL and the cycle dose. Conclusions: Lessons, sports lessons held in the gym or schoolyard, breaks, and lunch breaks were considered as high vocal demand communication situations requiring higher voice intensity and fundamental frequency, higher phonation time percentage, cycle, and distance doses. Group/individual work and other teacher activities during the day, unrelated to direct work with students, were categorized as low vocal demand communication scenarios.


Objetivos: Este fue un estudio de sujeto único, cuyo objetivo fue demostrar diferentes situaciones de demanda vocal típicas de la escuela primaria y la respuesta vocal de los docentes bajo dos condiciones acústicas, con y sin amplificación de voz, durante cinco días laborables. Métodos: Se llevó a cabo dosimetría vocal a largo plazo con Vocal Holter Med (PR.O. Voice Srl) durante las actividades diarias de enseñanza en una docente de 49 años con trastornos de la voz. Se instaló un sistema de amplificación de campo sonoro (SFAS) PentaClass Runa en el aula. La dosimetría vocal se realizó bajo dos condiciones acústicas diferentes: sin SFAS (2 días) y con SFAS (3 días). Resultados: Se investigaron el porcentaje de tiempo de fonación, el nivel de presión sonora (SPL), SPL SD, la frecuencia fundamental (F0), F0 SD, ciclos y dosis de distancia en siete escenarios de comunicación diferentes (clases, clases grupales/individuales, clases de educación física en el gimnasio y el patio de la escuela, recreos, almuerzos y otras actividades). Las puntuaciones medias de todos los parámetros vocales diferían significativamente entre los diferentes contextos de demanda vocal. La diferencia estadísticamente significativa en la respuesta a la demanda vocal se observó en las situaciones de comunicación con y sin SFAS. Además, el número de niños, el tiempo de reverberación y la humedad relativa del aire ambiente afectaron al SPL de la voz y la dosis de ciclo. Conclusiones: Las lecciones, las clases de educación física en el gimnasio o el patio de la escuela, los recreos y los almuerzos se consideraron situaciones de comunicación de alta demanda vocal, que requerían una mayor intensidad y frecuencia fundamental de la voz, un mayor porcentaje de tiempo de fonación y dosis de ciclo y distancia más altas. El trabajo grupal/individual y otras actividades del profesor durante el día no relacionadas con el trabajo directo con los estudiantes se categorizaron como escenarios de comunicación de baja demanda vocal.

ABSTRACT

Resumen El Museo de la Medicina Mexicana es un lugar dedicado a la divulgación de la historia de la medicina, en especial la de nuestro país. El museo se ubica en el Palacio de la Escuela de Medicina en la ciudad de México. En este trabajo se presentan los antecedentes del Museo de la Medicina Mexicana, elemento clave que fue del Departamento de Historia y Filosofía de la Medicina (DHyFM) de la UNAM en la enseñanza y divulgación de la historia de la medicina. Por ello, indicaremos los antecedentes, información de sus promotores, los trabajos a favor del museo, el origen de la colección museográfica y las primeras exposiciones.


Abstract The Museum of Mexican Medicine is a place dedicated to the divulgation of the history of medicine especially Mexico's history. The museum is located in the Palace of the School of Medicine in Mexico City. In this work we present the background of the Museum of Mexican Medicine, a key element of UNAM's Department of History and Philosophy of Medicine (DHyFM) for teaching and divulgating the history of medicine. Therefore, we will indicate the background, information of its promoters, works for the museum, the origin of the museographic collection and the first exhibitions.

ABSTRACT

Objective To explore cross-talk elimination method in the determination of gross α and gross β activities using a low-background α/β gas-flow proportional counter. Methods A CLB-104 low-background α/β gas-flow proportional counter was used in this study. First, the α threshold was increased to eliminate the cross-talk counting caused by β particles in the α channel. Then, the α-β anticoincidence threshold was reduced to eliminate the cross-talk counting induced by low-energy α particles in the β channel, and β counts were corrected to eliminate the counts induced by internal convention electrons in the β channel. Finally, gross α and gross β activities of non-saline water samples with different activity levels were determined and compared with gross α and gross β activities of the same samples determined on a BH1227 low-background α/β solid scintillation counter, in order to verify effectiveness of the cross-talk elimination method. Results By eliminating the cross-talk counts of β particles in the α channel and the cross-talk counts of α particles in the β channel, and deducting the counts of internal convention electrons in the β channel, the gross α and gross β activities of the same samples determined by CLB-104 were consistent with the values determined by BH1227. Conclusion Cross-talk counts induced by low-energy α particles or β particles can be eliminated by threshold adjustment, and the counts caused by internal convention electrons in the β channel can be eliminated by correction.

ABSTRACT

INTRODUCCIÓN: La vacunación es la única herramienta práctica para detener la expansión y erradicar la pandemia COVID-19, la mayoría de las vacunas han demostrado capacidad de inducir respuestas inmunitarias. Es predecible que existan varias vacunas con distintos niveles de protección por lo que su uso dependerá en cada país de las autoridades sanitarias y recomendaciones de la Organización Mundial de la Salud (OMS). OBJETIVO: Determinar los Eventos Adversos (ESAVIS) de la vacuna Sinopharm administrada en personas adultas en el punto de vacunación masivo de la Red de salud N°4 ESTE de la ciudad de La Paz, meses de abril a octubre gestión 2021. MATERIAL Y MÉTODOS: Se realizó un estudio descriptivo, observacional, muestra de 60 casos, con diagnóstico de Eventos Supuestamente atribuibles a la vacunación y/o inmunización. Los métodos empleados fueron análisis documental, instrumentos usados fichas ESAVIS del Ministerio de Salud y Deportes. Se recogieron datos sociodemográficos, sexo y edad. RESULTADOS: Las personas que presentaron eventos adversos a la Vacuna Sinopharm son del sexo femenino en un 78,3% con un promedio de edad 44 años, con antecedentes patológicos de Hipertensión Arterial Sistèmica en un 5%, el medicamento administrado en un 83,3% es el oxígeno, el síntoma más frecuente son los mareos 58,3%, se presentó más ESAVIS leves en un 98,3%. CONCLUSIÓN: Las personas que recibieron la vacuna Sinopharm presentan ESAVIS Leves.


INTRODUCTION: Vaccination is the only practical tool to stop the expansion and eradicate the COVID-19 pandemic, most vaccines have demonstrated the ability to induce immune responses. It is predictable that there are several vaccines with different levels of protection, so their use will depend in each country on the health authorities and recommendations of the World Health Organization (WHO). OBJECTIVE: To determine the Adverse Events (ESAVIS) of the Sinopharm vaccine administered to adults at the mass vaccination point of the Health Network No. 4 ESTE of the city of La Paz, from April to October, 2021. MATERIAL AND METHODS: A descriptive, observational study was carried out, a sample of60 cases, with a diagnosis of Events Supposedly attributable to vaccination and/or immunization. The methods used were documentary analysis, instruments used ESAVIS files from the Ministry of Health and Sports. Sociodemographic data, sex and age were collected. RESULTS: The people who presented adverse events to the Sinopharm Vaccine are 78.3% female with an average age of 44 years, with a pathological history of Systemic Arterial Hypertension in 5%, the medication administered in 83.3% is oxygen, the most frequent symptom is dizziness 58.3%, more mild ESAVIS occurred in 98.3%. CONCLUSION: People who received the Sinopharm vaccine have Mild ESAVIS.

ABSTRACT

Background and Objectives : Routine surveillance and monitoring studies pose a constant need to update clinicians on prevalent pathogens and rational and empirical treatment in Urinary Tract Infection (UTI). Escherichia coli (E coli) is the most commonly isolated uropathogen globally. Extended-Spectrum ?-Lactamase (ESBL) production and ?-Lactamase Inhibitor Resistance (BLIR) among these pathogens together with their uro-virulence determinants further complicate treatment approaches. This study investigated the clinico-microbiological pattern of UTI and determined the antibiotic sensitivity pattern, the phylogenetic background, and virulence determinants of E coli, the most commonly isolated uropathogen. Methods : Uropathogens isolated by urine culture from community and hospitalized patients were biochemically speciated. Antibiotic susceptibility was tested by Kirby-bauer disk diffusion method. Phylogenetic background and virulence determinants of E coli isolates were identified by PCR. SPSS 16.0 was used for statistical interpretation. Results : 45% of the urine samples showed growth positivity. 44% amongst them were E coli. All isolates were multidrug-resistant. 50% and 40% were ESBL producers and BLIR respectively. Former showed highest resistance to quinolone, fluoroquinolones, cotrimoxazole, and latter were resistant against all drugs tested except nitrofurantoin. Significant correlation existed between the ?-lactams, quinolone, fluoroquinolones, cotrimoxazole (p<0.05) resistance pattern. BLIR and ESBL E coli recorded highest prevalence of pathogenic phylogroup B2 and D respectively. Varied prevalence of fimbrial (fimH, papC, papEF, papG, GII) and toxin genes (iroN, hlyA, cnfI, i ucD, cdtBU) in ESBL, BLIR and non-ESBL isolates were observed. Their distribution was statistically significant (p=0.05). Interpretation and Conclusions : Nitrofurantoin is the drug of choice in empirical treatment of uncomplicated UTI. Aggressive and consistent investigation and health education are highly recommended for effective clinical management in UTI.

ABSTRACT

Objetivo: identificar o grau do conhecimento técnico e científico dos enfermeiros no procedimento intubação traqueal. Método: pesquisa de campo com caráter descritivo-exploratória e quantitativa tendo identificado, através do resultado de um questionário aplicado a 23 profissionais enfermeiros, em um Hospital Geral do Vale do Paraíba, o grau de conhecimento técnico e científico sobre o procedimento de intubação traqueal. A coleta dos dados foi realizada nos meses de setembro e outubro de 2021. Resultados: a enfermagem participa e desempenha papel importante no procedimento de intubação, desde os diagnósticos até a avaliação contínua dos principais sinais e sintomas, estabelecendo um plano de cuidados, intervenções e atividades que permitirão fornecer o melhor atendimento ao paciente. Conclusão: O conhecimento do enfermeiro durante o procedimento de intubação traquel é de extrema importância, visando a qualidade da assistência, e a segurança do paciente.(AU)


Objective: To identify the degree of technical and scientific knowledge of nurses in the tracheal intubation procedure. Method: descriptive-exploratory and quantitative field research, having identified, through the result of a questionnaire applied to 23 nursing professionals, in a General Hospital of the Paraíba Valley, the degree of technical and scientific knowledge about the tracheal intubation procedure. Data were collected in September and October 2021. Results: nursing participates and plays an important role in the intubation procedure, from diagnoses to continuous evaluation of the main signs and symptoms, establishing a care plan, interventions and activities that will allow providing the best patient care. Conclusion: The knowledge of nurses during the traquel intubation procedure is extremely important, aiming at the quality of care, and patient safety.(AU)


Objetivo: identificar el grado de conocimiento técnico y científico de las enfermeras en el procedimiento de intubación traqueal. Método: investigación de campo descriptiva-exploratoria y cuantitativa, habiendo identificado, a través del resultado de un cuestionario aplicado a 23 profesionales de enfermería, en un Hospital General del Valle de Paraíba, el grado de conocimiento técnico y científico sobre el procedimiento de intubación traqueal. Los datos fueron recolectados en septiembre y octubre de 2021. Resultados: La enfermería participa y juega un papel importante en el procedimiento de intubación, desde el diagnóstico hasta la evaluación continua de los principales signos y síntomas, estableciendo un plan de cuidados, intervenciones y actividades que permitan brindar la mejor atención al paciente. Conclusión: El conocimiento de las enfermeras durante el procedimiento deintubación de traquel es extremadamente importante, apuntando a la calidad de la atención y la seguridad del paciente.(AU)

ABSTRACT

Background: Teachers are often in dilemma on how to use illustrations effectively in teaching Anatomy using multimedia (for e.g., PowerPoint) slides. 態ackground� is an important element for creating illustration-based PowerPoint slides but is often overlooked when designing such slides. Some general guidelines on the PowerPoint slide background for presenting textual materials are available, but guidelines on the background for presenting illustrations in PowerPoint slides are almost nil. This descriptive type of observational study was part of a broader research that was designed to assess the preferences of Bangladeshi Anatomy teachers towards different types of background of illustration-based PowerPoint slides and their perceived influence on learning.Material & Methods:A survey was conducted among 39 Anatomy teachers from Bangladesh. Seven PowerPoint slides with one same pair of Neuroanatomy illustrations but seven different types of background dealing with the following characteristics were shown to them to collect their preferences and perceived influence on learning: White/Light-coloured homogeneous/Dark-coloured homogeneous; Slightly decorated/Heavily decorated; Related to subject/Not related to subject. Resultsand Conclusion:The survey revealed that most of the participants preferred either light-coloured homogeneous background (around 64%) or white background (around 60%). The least preferred (about 18%) one was the heavily decorated background not related to the subject. Some interesting trends of preference were also observed, including those related to male and female teachers as well as to their length of experience. In most Anatomy teachers� perceptions, learning would be influenced by using their preferred 慴ackgrounds�. Although the results of the present study reveal some insight into the preferences of Anatomy teachers and their perceived influence of different illustration-based PowerPoint slide 慴ackgrounds� on learning, further studies are recommended on larger samples focusing on more specific issues of PowerPoint slide backgrounds before making any conclusive comments on these issues.

ABSTRACT

Background: 態ackground� is an important visual element for creating multimedia (e.g., PowerPoint) slides on which text and illustrations are placed. Although some general guidelines for the background for textual materials on PowerPoint slides are available, detailed and specific guidelines on the background for illustrations are almost lacking. As part of a broader research, this qualitative study was designed to assess the preferences of Anatomy postgraduate students towards different types of background of illustration-based PowerPoint slides and their perceived influence on learning.Material & Methods:An FGD was conducted among eight Anatomy postgraduate students from two medical colleges of Bangladesh. Seven PowerPoint slides with one same pair of Neuroanatomy illustrations but seven different types of background dealing with the following characteristics were shown to them: White/light-coloured homogeneous/Dark-coloured homogeneous; Slightly decorated/Heavily decorated; Related to subject/Not related to subject. The discussion was on which background they liked most and why, and whether they think that the background would enhance students� learning.Resultsand Conclusion:Six broad 憈hemes� emerged from the FGD: 1. No single type of background yielded a unanimous preference. 2. Coloured homogeneous backgrounds seemed to be the most preferred type with no clear-cut difference between the light-coloured and dark-coloured ones. 3. Preferences for white or slightly decorated background was moderate. 4. Among the slightly decorated backgrounds, preference for those related to the subject was greater than for those not related. 5. No preference was shown for the heavily decorated background not related to the subject. 6. Despite some positive comments regarding the influence of slide background on learning, no definitive conclusion could be drawn. While these results provide some baseline answers to the research questions, further studies addressing more specific issues of background and viewer characteristics are recommended.

ABSTRACT

Color in animals responds to selective pressures and mediates the relationship between organism and environment. Reef fishes have the amplest variety of pigment cell types. This color patterns' variety may function as camouflage and be related to spatial use. We tested the hypothesis that the coloration of reef fish relates to water column stratum occupation. We predicted that sedentary animals connected to the background take advantage of background matching or disruptive patterns; more mobile demersal species apply disruptive coloration or motion-dazzle; and that pelagic species tend to have silvery bodies. We classified color patterns and categorized the water column stratum use for the Brazilian reef fishes in FishBase. Our analyses confirmed that irregular contrasting contour breaks, suggestive of disruptive coloration, occurs in benthic species and that silvering color was more prevalent in the pelagic stratum. Our raw data suggested a higher frequency of contrasting regular stripes, typical of motion-dazzle, in demersal species. However, the considerable uncertainty around estimates did not confirm this pattern. Reef fishes coloration is correlated to occupation of different strata in the water column. This can be interpreted as fishes being adapted to these habitats and partially explaining the richness of color patterns among them.(AU)


A cor nos animais responde a pressões seletivas e media a relação entre organismo e ambiente. Peixes recifais têm a maior variedade de tipos de células de pigmento. Essa variedade de padrões de coloração pode funcionar como camuflagem e estar relacionada ao uso espacial. Nós testamos a hipótese de que a coloração dos peixes recifais está relacionada à ocupação do estrato da coluna d'água. Previmos que animais sedentários conectados ao fundo aproveitam a semelhança ao fundo ou padrões disruptivos; espécies demersais mais móveis aplicam coloração disruptiva ou deslumbramento de movimento; e que espécies pelágicas tendem a apresentar corpos prateados. Classificamos os padrões de coloração e categorizamos o uso do estrato da coluna d'água para os peixes recifais brasileiros no FishBase. Nossas análises confirmaram que quebras de contorno irregulares, sugerindo coloração disruptiva, ocorrem em espécies bentônicas e que a cor prateada prevaleceu no estrato pelágico. Nossos dados brutos sugeriram uma maior frequência de listras contrastantes, típicas do deslumbramento de movimento, em espécies demersais. Mas a considerável incerteza dessas estimativas não confirmou esse padrão. A coloração dos peixes recifais está correlacionada com a ocupação de diferentes estratos da coluna d'água. Isso pode ser interpretado como peixes sendo adaptados a estes habitats, explicando em parte a riqueza de padrões de coloração entre eles.(AU)

ABSTRACT

Objective:To analyze the effect of SBAR (situation, background, assessment, and recommendation) sign-out template in the standardized residency training in a neonatal ward.Methods:Based on SBAR communication mode, we designed and optimized neonatal ward sign-out checklist, and a total of 67 residents were trained for 2 weeks from April 2019 to June 2019, with control group (before training) and observation group (after training). The assessment indicators of sign-out information included sign-out duration, the incidence of sign-out errors, quality assessment scores and shift satisfaction (including sign-out satisfaction and self-evaluation) as well as competencies. A total of 1 553 children's morning shift data were collected, and SPSS 22. 0 was used for t test and chi-square test. Results:In the control group, the sign-out duration was (23.4±4.7) min, the incidence of sign-out errors was (43.6±6.6)%, quality assessment scores were (6.3±0.7) points, the satisfaction degree was (76.5±4.6)%, and the self-evaluation scores were (5.2±2.1) points. While, in the observation group, the sign-out duration was (15.9±3.2) min, the incidence of sign-out errors was (21.1±2.3)%, quality assessment scores were (8.9±0.9) points, the satisfaction degree was (94.1±2.9)%, and the self-evaluation scores were (8.9±0.8) points, with statistically significant difference ( P<0.05). There were statistically significant differences between the two groups in clinical skills and medical service ability [(2.2±0.1) vs. (3.8±0.3)], interpersonal communication ability [(2.6±0.5) vs. (4.2±0.1) points], teamwork ability [(3.1±0.2) vs. (4.6±0.3)], information and management ability [(2.5±0.5) vs. (4.2±0.2)] ( P<0.05). Conclusion:The SBAR template can improve sign-out process and the clinical skills of standardized training residents.

ABSTRACT

【Objective】 To study the demographic characteristics of whole blood donors aged 56 to 60 in Guangzhou, so as to provide reference for smooth implementation of the recruitment and management of elderly blood donors in Guangzhou. 【Methods】 The data of whole blood donors, aged 56 to 60, in Guangzhou Blood Center from July 1, 2016 to June 30, 2021 were retrospectively analyzed, and such characteristics as the number of blood donations, gender, donation frequency, donation volume, occupation and educational background were analyzed. 【Results】 A total of 3 996 blood donors aged 56 to 60, including 3 013 males and 983 females, gave 14 416 donations during July 1, 2016 to June 30, 2021, with the proportion of 400 mL specification at 64.1% (6 999/10 918) of males, which was significantly higher than that of females 57.7% (2 017/3 498). The number of blood donors had been decreasing year by year, but the ratio of men to women was stable.The education background of blood donors were mainly high school, junior college, and undergraduate education, accounting for more than 60% in total, and the proportion of undergraduate and graduate degree trended to grow. Staff, workers, medical staff and civil servants were the majorities, and the proportion of medical staff trended to grow. 【Conclusion】 Male blood donors aged 56 to 60 in Guangzhou showed stronger intention to donate blood, and the proportion of blood donors with better education and medical staff was on the rise.

ABSTRACT

Objective:To investigate and analyze the basic occupational situation of Chinese radiotherapists and to provide references for the development and construction of the radiation therapist team in China.Methods:Between May 7, 2021 and June 15, 2021, this survey was conducted using a self-designed questionnaire, which was distributed and collected in the form of online survey to conduct a statistical analysis of the basic situation of radiotherapists in 30 provincial-level administrative regions in China.Results:A total of 2 928 valid questionnaires were collected from 30 provincial-level administrative regions. The ratio of male to female and average age of the participants were 69∶31 and 35 years (18-65), respectively. According to the survey result, 84.7% of the participants came from Tertiary A-level hospitals. In terms of educational background, the participants with doctoral, master, undergraduate, junior-college, and technical-secondary degrees accounted for 0.2%, 6.9%, 76.6%, 14.9%, and 1.4%, respectively. In terms of title, full senior technologists, associate senior technologist, technologist-in-charge, technologist, and technician in the participants accounted for 0.5%, 5.8%, 32.5%, 52.2%, and 8.2%, respectively. The participants with years of employment of 0-, 5-, 10-, 20-y accounted for 33.74%, 25.79%, 25.51%, and 14.96%, respectively. The majors of the participants included medical imaging/imaging technology (45.9%), clinical medicine (19.1%), and biomedical engineering (11.9%).Conclusions:Chinese radiotherapists tend to be younger and are rapidly increasing in number. Employers should attach importance to the career planningand continuing education of radiotherapists to improve their technical skills, in order to fully satisfy the demand for radiotherapists in the new era.

ABSTRACT

Objective:To explore the differences and current situation of scientific research background and occupational cognition of enrollees in MD-PhD programs.Methods:A questionnaire survey was conducted among 158 students (40 males and 118 females) who were enrolled in MD-PhD programs in a medical college during 2017 to 2020. SPSS 24.0 and Fisher’s exact test were used for statistical analysis.Results:Over 50% of enrollees in 2019 and 2020 had at least 2-year experience in basic medical research before enrollment. The research background of MD-PhD program enrollees was continuously improving from 2018 to 2020, and Fisher's exact chi-square P value was 0.02. Since 2018, the proportion of enrollees who planned to seek for administrative or education-related jobs in medical institutions had reduced year by year ( P=0.01). Conclusion:In view of the scientific research problems and professional cognitive tendencies of the new students of the MD-PhD programs, certain relevant guidance suggestions are put forward in this study.

RESUMEN El cáncer gástrico es una de las principales causas de muerte por neoplasias en el mundo. Las guías de prácticas clínicas actuales ofrecen modelos de tratamiento que involucran a la cirugía, radioterapia, quimioterapia, inmunoterapia y terapia dirigida a receptores de crecimiento específicos, sin embargo, los aspectos genéticos y de la biología molecular no siempre son tenidos en cuenta en la práctica médica. El objetivo de la presente revisión es articular los aspectos actuales más relevantes de la genética y la biología molecular en relación con el cáncer gástrico, para integrarlos en las guías clínicas de pacientes y familiares con el diagnóstico o con riesgo de padecer este tipo de neoplasia. Para ello se revisaron los avances genéticos y los síndromes relacionados con el cáncer gástrico, clasificaciones moleculares e implicaciones en su manejo. Se utilizaron las bases de datos Google Scholar, Elsevier y PubMed en los últimos 10 años, seleccionándose los de mayor importancia a texto completo desde el punto de vista cualitativo. La integración de antecedentes personales y familiares con elementos genéticos precisan ser tenidos en cuenta en el manejo, diagnóstico y prevención de esta enfermedad. Se sugiere la inclusión de elementos genéticos y moleculares en toda guía de práctica clínica.


ABSTRACT Gastric cancer is one of the leading causes of death from neoplasms in the world. Current clinical practice guidelines offer treatment models that involve surgery, radiotherapy, chemotherapy, immunotherapy and therapy directed at specific growth receptors, however, genetic and molecular biology aspects are not always taken into account in medical practice. The objective of this review is to articulate the most relevant current aspects of genetics and molecular biology in relation to gastric cancer, to integrate them into the clinical guidelines of patients and relatives with the diagnosis or at risk of suffering from this type of neoplasia. For them, genetic advances and syndromes related to gastric cancer, molecular classifications and implications in their management were reviewed. The databases Google Scholar, Elsevier, and PubMed were used in the last 10 years, selecting the most important full-text from a qualitative point of view. The integration of personal and family history with genetic elements needs to be taken into account in the diagnostic management and prevention of this disease. The inclusion of genetic and molecular elements in all clinical practice guidelines is suggested.

OBJECTIVE:To pr ovide reference for the future pharmacoeconomic evaluation. METHODS :Four typical cases of pharmacoeconomic evaluation showed the special situation that “not cost-effective at a zero price ”,defined the concept of “net value”to assist in theoretical analysis ,and summarized the possible causes of this situation . RESULTS & CONCLUSIONS :The system(ATPS):an overview and advances in its applica background treatment cost other than the target drug was too high ,the patient ’s quality of life during survival was too low ,the threshold of willingness to pay of incremental cost-effectiveness ratio was too low ,the cost of risk events during extended survival was too high or the health loss was too high ,and the price of the control scheme was too low ,which might affect the results of economic evaluation ,and even le d to the dilemma of pharmacoeconomic evaluation of “not cost-effective at a zero price ”. Pharmacoeconomic evaluation discusses the economy of intervention programs rather than the economy of drugs. The uneconomic results caused by various reasons need to be treated objectively ;theoretically,all“relevant”and“irrelevant”costs should be included in the calculation category in pharmacoeconomic evaluation ;pharmacoeconomics mainly solves the problem of “cost performance”comparison among several treatment schemes ,and the medical and health field often needs to face problems such as ethics and fairness. Therefore ,researchers may need to jump out of the framework of pharmacoeconomics analysis and conduct in-depth discussion from a higher and broader perspective.

ABSTRACT

Traditional Chinese medicine (TCM) has been an indispensable source of drugs for curing various human diseases. However, the inherent chemical diversity and complexity of TCM restricted the safety and efficacy of its usage. Over the past few decades, the combination of liquid chromatography with mass spectrometry has contributed greatly to the TCM qualitative analysis. And novel approaches have been continuously introduced to improve the analytical performance, including both the data acquisition methods to generate a large and informative dataset, and the data post-processing tools to extract the structure-related MS information. Furthermore, the fast-developing computer techniques and big data analytics have markedly enriched the data processing tools, bringing benefits of high efficiency and accuracy. To provide an up-to-date review of the latest techniques on the TCM qualitative analysis, multiple data-independent acquisition methods and data-dependent acquisition methods (precursor ion list, dynamic exclusion, mass tag, precursor ion scan, neutral loss scan, and multiple reaction monitoring) and post-processing techniques (mass defect filtering, diagnostic ion filtering, neutral loss filtering, mass spectral trees similarity filter, molecular networking, statistical analysis, database matching, etc.) were summarized and categorized. Applications of each technique and integrated analytical strategies were highlighted, discussion and future perspectives were proposed as well.

ABSTRACT

Antibiotic resistance is a major global concern and challenge in the 21st century. Enterobacteriaceae are one of the important pathogens of hospital-acquired infections. With the increasing use of antibiotics in clinical practice, a variety of drug-resistant Enterobacteriaceae, especially multidrug-resistant Enterobacteriaceae have emerged, posing an increasingly serious threat to human health. Bacteria can acquire antibiotic resistance by mutation or horizontal transfer of antibiotic resistance genes, and it is often possible to predict the corresponding resistance phenotype from known mechanism. However, recent findings suggest that genetic background and environmental factors could alter the expression of specific resistance genes and that a given genotype does not always generate the expected resistance phenotype. The genotype-phenotype segregation greatly hampers our ability to predict antibiotic resistance phenotype from a genetic perspective. In this review, we explore the genetic and environmental regulation of the expression of antibiotic resistance genes in a variety of Enterobacteriaceae, with the aim to provide scientific evidence for genetic prediction of antibiotic resistance phenotype and clinical guidance on drug use.
