ABSTRACT

With the rapid development of modern biomedicine, subjective factors such as human emotion and disease experience are diluted. Accurate data, programmed diagnosis and treatment process and other objective factors cut off the effective communication between doctors and patients, aggravating the already tense doctor-patient relationship and intensifying various conflicts in the clinic. In order to improve clinical humanistic care, medical and nursing workers have launched reflections and explorations. Narrative medicine is a new form of medical humanistic practice, while parallel chart, as an important way of its practice, records patients’ subjective experiences and painful feelings. It is of great theoretical significance and clinical value to sinicize narrative medicine and construct and promote parallel chart in Traditional Chinese Medicine (TCM) by studying the research progress of parallel chart in China and its application in TCM.

ABSTRACT

Preschool age(3-6 years old)is a critical period for visual development, and it is crucial to detect and treat visual problems in preschool children as early as possible. Visual acuity charts are important tools for screening visual issues in children. In China, the commonly used charts are the standard logarithmic visual acuity chart and the pediatric optotype chart, while overseas, the Lea, HOTV, and ETDRS visual acuity charts are frequently employed. Numerous studies have reported the measurability, repeatability, and sensitivity of these three charts in diagnosing visual-related problems in children. However, the application of these three charts is relatively limited in China. This article provides a comprehensive review of the design principles, clinical applications, and characteristics of these three visual acuity charts, so as to better understand their applicability and limitations in preschool children, and provide reference for the selection and improvement of vision examination methods in the future.

ABSTRACT

Patient Safety has been a challenge for healthcare facilities worldwide. Ensuring the safety of the patient is the duty of each healthcare professional as emphasised in the Hippocratic oath, 'ldquo;above all, do no harm'rdquo;. Nurses in the healthcare team are pivotal in the provision of patient care. They spend more time with patients than others and are in direct communication with the patient and family, so they are better placed to mitigate harm. Hence nurses need to be vigilant, engaged and invested in patient safety and improvement in quality care. Nursing education and training too should serve as a platform for increased efforts in this direction and include opportunities for continued learning. Discussed in this paper are two simple measures such as '#39;safety cross and run charts'#39; that can be used to promote patient safety by recognising risks and preventing harm or adverse events.

ABSTRACT

Background: Normal human beings can appreciate color in all three of it’s attributes; Hue, intensity, and saturation. Human beings can perceive three primary colors, that is, red, green, and blue. Any defect in appreciation of colors is known as color vision deficiency (CVD). Complete inability to appreciate color is known as color blindness. The genes for red and green cone pigments are found in the q arm of the X chromosome. Hence, red and green CVD are inherited as X-linked recessive diseases. Blue CVD is autosomaly inherited. Blood groups are genetically inherited as well. Although it is a known fact that there is a preponderance of genetic diseases in subjects belonging to a specific blood group, no such study was available in Western Odisha as per our knowledge. This becomes more relevant as consanguineous marriages are a serious social problem in that part of Odisha. Aim and Objectives: Therefore, the study was undertaken to find out the prevalence of CVD among Medical students in Western Odisha and to find out it’s relation, if any, with ABO blood groups, as these two entities are genetically inherited. Record should be kept for future use, especially for counseling at the time of marriage and if necessary, to choose a subject for further study, where color detection does not play an important part. Materials and Methods: Prior permission was taken from the Institutional Ethics Committee to carry out this study. The study was carried out among the 1st–4th year medical students of V.S.S. Medical College and Hospital (n = 690) from August 2022 to October 2022. Ishihara test plates for color vision and agglutination method for ABO blood grouping were employed to get the prevalence of CVD and to get the percentage of subjects belonging to each blood group among those having CVD. The observations were noted carefully. Results: After careful screening, it was seen that 647 (93.76%) were normal, 42 (6.08%) had CVD, and 1 (0.14%) subject was completely color blind. All those affected were males. Distribution of blood group in CVD subjects showed 7 (16.27%) numbers belonging to blood group A, 22 (51.16%) belonging to blood group B, 1 (2.3%) belonging to blood group AB, and 12 (27.9%) belonging to blood group O. The only color blind student belonged to blood group B (2.3%). Conclusion: As medical students have to deal with colors throughout their career, the study helps the subjects to be aware of their condition early in their professional life. It also tried to find out the percentage distribution into ABO blood groups. A larger population should be included to get more accurate results.

ABSTRACT

Objective To evaluate the policy of human organ transplantation in China, aiming to provide theoretical basis for further optimizing the policy of human organ transplantation. Methods Based on text mining and statistical analysis, seven normative policies of human organ transplantation formulated by national government from 2000 to 2022 were quantitatively evaluated by constructing policy modeling consistency (PMC) with 10 first-level variables and 35 second-level variables. Results Among the seven policies, six were graded as excellent policies and one as perfect policy, with an average PMC index of 8.476. Except X8 policy audience, the scores of other second-level variables of P5 were higher than or equal to the mean. The scores of all second-level variables of P1 were lower than or equal to the mean. P1 and P5 significantly differed in X3 policy timeliness, X4 policy norms and X6 policy tools. P5 was more specific and relatively comprehensive in these aspects, and its score was significantly higher than that of P1. Conclusions Human organ transplantation policies in China are generally excellent, scientific and rational. Health administrative departments at all levels should pay attention to the grasp of policy timeliness, the combination of policy tools, and fully mobilize the initiative and enthusiasm of all policy audience to participate in organ transplantation management when formulating organ transplantation policies.

ABSTRACT

ObjectiveTo present the health status of traditional Chinese medicine (TCM) constitutions more intuitively and comprehensively based on improved radar chart. MethodsParticipants who completed a 26-week comprehensive intervention based on TCM constitution from February 2013 to January 2014 in Zhuhai branch of Guangdong Provincial Hospital of Chinese Medicine were included in the study. They were divided into groups according to gender and age, i.e. young, middle-aged, and elderly male and female groups. TCM constitution scale and health survey short form (SF-36) were used to evaluate the 9 basic TCM constitution types and quality of life at three time points, including pre-intervention (T1), at 13-week intervention (T2), and at 26-week intervention (T3). The improved radar charts were drawn to visually present the comprehensive evaluation results on the health status of 9 TCM constitutions, and graphic features (area S value, perimeter L value) were extracted to construct a comprehensive health index for TCM constitutions (H value). Spearman correlation analysis was used to analyze the correlation between H value and SF-36 total score. ResultsAmong the included 509 participants, there were 45 elderly male, 76 elderly female, 60 middle-aged male, 140 middle-aged female, 53 young male and 135 young female. The radar charts for comprehensive evaluation of TCM constitution health status showed that the total areas for all groups increased at T3 compared to T1, with the most significant increase in the young population. In the middle-aged population, the fan-shaped areas of certain constitutions decreased at T2 than T1. At T3, the radar chart shapes for females were more balanced than males in the same age group. By calculating the features of function graphs, it was found that the S, L, and H values for the elderly population were relatively higher than those for the middle-aged and young population with the same gender, and the young population increased by highest ratio. The values measured at T3 compared to T1 showed average increase of 26% for S value (11% for the middle-aged and 14% for the elderly), 22% for L value (10% for the middle-aged and the elderly each), and 22% for H value (10% for the middle-aged and 9% for the elderly). The female had lower S and L values, as well as higher H value than the male of the same age group measured at T3. The correlation coefficient between the H value of all participants and the total SF-36 score was 0.662 (P<0.01). ConclusionThe comprehensive evaluation model for the health status of TCM constitution based on the improved radar chart constructed in this study can present the health status of TCM constitutions and intervention effectiveness more comprehensively and intuitively. It is suggested to regulate the constitution in pursuit of the dynamic balance of the constitution health status, as well as consider the parts from the whole, and put focus on the balance of nine TCM constitutions.

ABSTRACT

Cardiovascular diseases (CVDs) contribute to most of the potentially preventable burden through early risk assessment. Nurse-led CVD risk assessment is an effective strategy to address the human resource crisis for CVD prevention. An interventional study was conducted in medicine wards of a tertiary care hospital in North India to train nurses in CVD risk assessment and its communication. All bedside nurses (n = 30) of selected wards were enrolled and trained in CVD risk assessment and communication using WHO/ISH risk prediction charts. Once fully trained, each nurse enrolled patients (>40 years of age) from their respective wards to assess and communicate CVD risk. To calculate the reliability of risk assessment, investigator simultaneously assessed CVD risk with nurses. The mean age of nurses was 32.07 ± 6.31 years. The results revealed that training significantly increased the knowledge of nursing personnel (P < 0.001). There was perfect inter?rater reliability agreement (Cohen’s k = 0.929) between nurses and investigators while assessing CVD risk. Nurses demonstrated good communication skills. The study concluded that nurses can be trained successfully in CVD risk assessment and communication. The study recommends the task shifting of CVD risk assessment to nurses after providing proper training

ABSTRACT

Objective: To compare the average birthweights and the weight centiles of the ‘new’ growth charts with the ‘old’ (1974) charts developed in the same unit four decades ago. Methods: Birthweight and gestation data of the eligible 12,355 singleton neonates born between 2009 and 2016 at a level-3 neonatal unit at a public sector hospital were used to develop the new growth chart. We then compared the prevalence of small for gestational age (SGA) and large for gestational age (LGA) classified by the new charts and the old charts, the incidence of short-term adverse outcomes among them, and the diagnostic performance of both the charts to identify the adverse outcomes in a separate validation cohort. Results: The mean birthweights of boys and girls across all gestations were higher by 150-200 g and 100-150 g, respectively, in the new chart. The prevalence of SGA doubled (9.8% vs 4.7%), but LGA decreased by one-third (17.5% vs 25.9%) with the new chart. However, the proportion of SGA and LGA having one or more short-term adverse outcomes, and the diagnostic performance of both the charts to identify neonates with shortterm adverse outcomes, were comparable. Conclusion: There was an upward shift in the birthweights by about 150 g across all gestations in the new chart compared to the old chart developed 40 years ago. The findings imply the need to consider using updated growth charts to ensure accurate classification of size at birth of neonates.

ABSTRACT

Objective:To conduct a position evaluation of functional departments directors by means of Hay guide-chart profile (Hay) and international position evaluation (IPE) for heads of public hospital departments, and a comparative analysis of the results, for reference in reforming the performance-related pay system of the functional departments. Methods:From July to August of 2021, positions of ten functional department directors of a public tertiary general hospital were selected, and expert groups within and out of the hospital were invited for a position evaluation using both Hay and IPE. Cronbach α and intraclass correlation coefficient were used in the reliability and consistency evaluation. Technique for order preference by similarity to ideal solution (TOPSIS) was conducted to compare the differences between results of both methods, with the position value ranked by quartiles. Results:22 position evaluation forms were issued and 20 were recovered (12 for in-hospital experts and 8 for extramural experts). In Hay evaluation, the scores of medical affairs department and hospital head office were higher, averaging 757.30 and 727.21 respectively, and those of logistics management department were the lowest, being 279.94. In-hospital experts scored lower than those extramural experts on the posts of directors of scientific research management department, Party committee office and medical insurance department, while the scores of the directors of the hospital head office were higher than those extramural experts. In IPE evaluation, the position values of hospital head office and medical affairs department were the highest, averaging 680.25 and 621.00 respectively, and the score of the logistics management department was the lowest, being 365.05. In-hospital experts scored lower than those extramural experts on the posts of directors of Party committee office, scientific research management department and logistics management department, while the scores of the directors of the hospital head office were higher than those of extramural experts. The Cronbach α coefficients of position value scoring of functional department heads evaluated by two evaluation methods were 0.943 (Hay) and 0.800 (IPE) respectively. The hospital head office and medical affairs department ranked the first quartile, the Party committee office and nursing department ranked the second, while the medical insurance department and logistics department ranked the third and fourth respectively. In addition, the ranking differences of the information technology department, human resource department, financial department and scientific research management department were within one quartile. Conclusions:The position evaluation results of Hay and IPE are consistent, but the former is more sensitive. Ranking of position values by quartiles via TOPSIS can support the reform of performance-related pay system of functional departments of public hospitals.

ABSTRACT

Objective:To study the feasibility of control charts in the quality management of hospital statistical data as a reference for improving such management. Methods:Main business indicators of the main campus and some business indicators of the affiliated campus of a tertiary general hospital from January to May 2022 were selected. K-S test and chi-square goodness of fit test were used to test data in its statistical daily report, analyzing whether the data conform to the specific distributions. Then appropriat control chart were used according to the data type and distribution type. Minitab 21.1 software was used to draw the hospital data quality control chart, and data quality was monitored by analyzing the distribution of data points in the control chart. Results:The test found that the number of admissions, ultrasound examinations and emergency department visits in the main campus, and CT examinations in the affiliated campus, conformed to normal distribution, and single value control charts were applied. The number of ambulance trips in the main campus and the affiliated campus conform to Poisson distribution, and the defect number control chart was applied. The number of inpatient deaths in the main campus conform to a geometric distribution, and a rare event control chart was applied. The volume of admissions and ultrasound examinations in the main campus were mostly influenced by other factors, and the single-value selective control chart was used to further determine the cause of abnormal data distribution. The results of the control chart analysis showed that, there were no abnormal points in the data distribution of admissions, color ultrasound volume and in-hospital deaths in the main campus, two abnormal points in the CT examination volume in the affiliated campus. The control charts for the number of emergency department visits in the main campus and the number of ambulance trips in the main and affiliated campuses each had one outlier. It was verified that one anomaly in the volume of CT examinations in the affiliated campus and one anomaly in the number of ambulance trips in the affiliated campus were caused by data errors, while the other data were correct. Conclusions:It is feasible to use control charts to monitor the quality of hospital data, which can be used as a quality management tool to assist the quality management of hospital data.

ABSTRACT

Objective:To analyze the current status of body composition and development patterns of children and adolescents aged 6-17 years in Guangzhou. Methods:This was a cross-sectional study involving 8 169 school students from 3 elementary schools and 3 middle schools in Guangzhou from March to December 2019.The fat-free mass (FFM) and fat mass (FM) were measured by the bioelectrical impedance analysis. The fat-free mass index (FFMI) and fat mass index (FMI) were calculated via the height standardization. T test was used to compare quantitative variables between groups. The growth pattern of body composition was described using the Hattori chart. Results:A total of 4 431 boys (54.24%) and 3 738 girls (45.76%) were involved in this study.FFM and FM both increased with age between boys and girls.Except for boys aged 11 years, FFM in boys were significantly higher than that in girls with the same age (all P<0.05). In the age of 7-10 years, FM in boys were significantly higher than that in girls with the same age, while it was significantly higher in girls aged 12 years and older than that of boys at the same age (all P<0.05). The Hattori chart showed that the difference in body composition between genders occurred after 11 years old.In contrast to girls, increases in the weight and body mass index (BMI) in boys were mainly attributed to the FFM development. Conclusions:The development of FFM and FM in children and adolescents varies with age, accompanied with the gender-specific features.FFM in boys is higher than that of girls at the same age.The weight gain in boys is mainly attributed to the development of fat-free tissues, and thus the utility of BMI may lead to the overestimation of obesity.

ABSTRACT

Objective:To explore the use of Delphi method and superiority chart to construct flipped classroom teaching index evaluation system. Methods:Delphi method was applied to conduct expert consultation, and the superiority chart was used to calculate the weight of each index of the flipped classroom teaching evaluation system. SPSS 17.0 was used to statistically analyze the importance assignment of indicators. Results:After two rounds of correspondence consultation, the evaluation system of flipped classroom teaching mode was established, with 3 first-level indicators with weights of (0.32, 0.56 and 0.12); 6 second-level indicators with weights of (0.45, 0.55, 0.56, 0.44, 0.55 and 0.45) and 20 third-level indicators. Conclusion:Delphi method can not be substituted for other methods of mathematical statistics. In terms of weight determination, compared with analytic hierarchy process (ahp), the superiority chart does not need to establish judgment matrix, or to conduct consistency test. This method is simple to operate, time-saving and labor-saving, and is worthy of extensive promotion.

ABSTRACT

Objective: To define the alert levels for the total number of COVID-19 cases derived by using quantile functions to monitor COVID-19 outbreaks via an exponentially weighted moving-average (EWMA) control chart based on the first hitting time of the total number of COVID-19 cases following a symmetric logistic growth curve. Methods: The cumulative distribution function of the time for the total number of COVID-19 cases was used to construct a quantile function for classifying COVID-19 alert levels. The EWMA control chart control limits for monitoring a COVID-19 outbreak were formulated by applying the delta method and the sample mean and variance method. Samples were selected from countries and region including Thailand, Singapore, Vietnam, and Hong Kong to generate the total number of COVID-19 cases from February 15, 2020 to December 16, 2020, all of which followed symmetric patterns. A comparison of the two methods was made by applying them to a EWMA control chart based on the first hitting time for monitoring the COVID-19 outbreak in the sampled countries and region. Results: The optimal first hitting times for the EWMA control chart for monitoring COVID-19 outbreaks in Thailand, Singapore, Vietnam, and Hong Kong were approximately 280, 208, 286, and 298 days, respectively. Conclusions: The findings show that the sample mean and variance method can detect the first hitting time better than the delta method. Moreover, the COVID-19 alert levels can be defined into four stages for monitoring COVID-19 situation, which help the authorities to enact policies that monitor, control, and protect the population from a COVID-19 outbreak.

ABSTRACT

【Objective】 To explore the establishment of individualized prediction model of recurrence after percutaneous endoscopic lumbar discectomy (PELD) in patients with lumbar disc herniation (LDH). 【Methods】 We selected 124 LDH patients treated with PELD in Department of Orthopedics, The First Affiliated Hospital of Xi’an Jiaotong University, from January 2017 to January 2020 as the research subjects. Their clinical data were retrospectively analyzed, and the independent risk factors affecting PELD recurrence in the LDH patients were screened by univariate analysis and Logistic regression analysis, respectively; the correlation histogram prediction model was established. 【Results】 Age, history of diabetes, course of disease, work intensity and IDDG were the risk factors for the recurrence of PELD in LDH patients (P<0.05). Based on the risk factors screened out, the prediction model of the histogram was established, and the model was verified. The results showed that the C-index of the modeling set and the validation set was 0.944 (95% CI: 0.902-0.963) and 0.969 (95% CI: 0.911-0.978), respectively. The correction curves of both groups were well fitted with the standard curves. The areas under the ROC curve (AUC) in the two groups were 0.944 and 0.969, respectively, which proved that the model had good prediction accuracy. 【Conclusion】 LDH patients have many independent risk factors for recurrence after PELD, and the model based on risk factors with good predictive ability can be useful in preoperative evaluation, appropriate patient selection, and decrease of recurrence rate after PELD.
