ABSTRACT

RESUMEN Objetivo: Describir los desafíos de la gestión del cuidado de los profesionales de enfermería. Metodología: El presente trabajo investigativo se desarrolla desde un enfoque cuantitativo con una metodología descriptiva con diseño no experimental, la cual se apoya en el análisis documental-bibliográfico. Se organizó un proceso investigativo en donde la población de estudio se basó, primordialmente, en documentos escritos como tesis, revistas arbitradas y artículos científicos. En conclusión: Los desafíos a los que se enfrentan los profesionales de enfermería en la gestión del cuidado, en esta época involucran el aspecto económico, social y político en un mundo en constantes cambios, en donde la tecnología juega un gran papel que permite mejorar los servicios inter hospitalarios como red de comunicación, mejoras en las condiciones laborales, incorporación de nuevo talento humano y disminución en la deficiencia a nivel mundial, fortalecimiento y replanteamiento de los planes de formación y estudio de enfermería.

ABSTRACT

Purpose: The study intends to evaluate the demographic and clinical profile of pediatric patients (0–18 years) attending six vision centers (VCs) of a tertiary eye care facility in south India. Methods: This is a cross?sectional observation study conducted at six major peripheral VCs of a tertiary eye care facility in South India from June 20 to Dec 20. All children presenting at the VCs under a base hospital (BH) in Pondicherry, whose parents gave consent were included. Data were entered by the ophthalmic assistants in the VC and cross checked by the principal investigator at the BH. Results: A total of 250 pediatric patients were brought to the VCs during the study period, predominantly males (60.8%), with a mean age of 8.2 ± 4.5 years (0– 18 years). Half of them were staying within 5 km from the VCs. Most children were escorted by their parents (88%), and the expenditure of travel to the VC was affordable for most of the parents (75%). Also, 53.6% of patients were in their primary school, while schooling had not started for 28% of children. Visual acuity (VA) could be assessed only for school?going children and older children due to lack of age?matched VA assessment tools. Most children (91.3%) had uncorrected VA better than or equal to 6/18 in the better eye, and approximately 3% had VA worse than 6/60. All patients had best corrected VA of 6/6–6/18 after cycloplegic refraction. Most children reported to the VC for allergic conjunctivitis (25%), followed by refractive error (13%) and squint (10.4%). Urgent referral to the BH was made for 47 children. Conclusion: Primary eye care in pediatric population?I (PREPP?I) showed that most children can be treated at the VCs and only one?fifth of the children require active intervention at higher referral centers. Further study on satisfaction of services provided for pediatric patients in these VCs and barriers of not reporting to the BH when referred are considered for the PREPP?II study

ABSTRACT

Resumo Este artigo reflete sobre a compreensão dos profissionais que atuam na Estratégia Saúde da Família acerca das intersecções entre relações raciais e saúde. Para tanto, foram realizadas entrevistas semiestruturadas com profissionais que compõem as equipes de saúde da família em um município de Pernambuco, cujos dados foram analisados à luz das discussões que se debruçam sobre a temática racial. Os resultados apontam para as divergências entre os modelos de saúde presentes no cotidiano, para a invisibilidade da raça/cor na formação e atuação profissional e a branquitude como elemento estruturante das relações raciais. O estudo reafirma a necessidade do campo da saúde coletiva se debruçar sobre a intersecção entre o quesito raça/cor e as relações de cuidado em saúde, bem como o investimento na formação dos profissionais, que inclua a temática racial.


Abstract This paper reflects on the Family Health Strategy (ESF) workers' understanding of racial relationships and health intersections. We conducted semi-structured interviews with ESF teams' professionals in the State of Pernambuco, Brazil, whose data were analyzed in light of the discussions that focused on the racial theme. The results point to discrepancies between different health models in the daily routine, race/skin color invisibility in training and professional work, and whiteness as a structuring element of race relationships. The study reaffirms the need for Public Health to focus on the intersection between the issue of race/skin color and healthcare relationships and invest in the training of professionals regarding race discussions.

Abstract Objectives: to analyze the nutritional status and temporal trend of the coverage of Brazilian adolescents monitored in the Food and Nutritional Surveillance System in the period 2008-2019. Methods: ecological study of temporal series with SISVAN data collected from 2008 to 2019 with 45,869.266 registration. Body mass index data were collected and the registration coverage of adolescents monitored by the system was calculated. The percentage of annual variation in coverage and nutritional status was estimated using Prais-Winsten regression, with a significance level of 5%. Results: an increase in the prevalence of obesity was observed between 2008 and 2019, during this period thinness remained stable in Brazil. National coverage evolved from 9.3% in 2008 to 19.6% in 2019, with an annual variation of 9% and a statistically significant increase trend. In Brazil, obesity in this age group increased between 2008 and 2019, with an annual variation of 8.7% (CI95% = 7.8-9.6). Conclusion: modifications were observed in nutritional status suggest the occurrence of nutritional transition in the adolescents' population. The percentage of coverage available in the system has growth potential, however, the current scenario impacts on a possible insufficient analysis to support the elaboration and reorientation of public policies.


Resumo Objetivos: analisar o estado nutricional e a tendência temporal da cobertura de adolescentes brasileiros acompanhados no Sistema de Vigilância Alimentar e Nutricional no período de 2008-2019. Métodos: estudo ecológico de séries temporais com dados do SISVAN coletados no período de 2008 a 2019 com 45.869.266 de registos. Foram coletados dados de índice de massa corporal e calculadas as coberturas dos registros dos adolescentes acompanhados pelo sistema. Estimou-se a percentagem da variação anual da cobertura e estado nutricional pela regressão de Prais-Winsten, com nível de significância de 5%. Resultados: observa-se crescimento da prevalência de obesidade entre 2008 a 2019, nesse período a magreza manteve-se estável no Brasil. A cobertura nacional evoluiu de 9,3% em 2008 para 19,6% em 2019, com variação anual de 9% e tendência de aumento estatisticamente significativa. No Brasil, a obesidade nessa faixa etária apresentou um aumento entre os anos de 2008 e 2019, com uma variação anual de 8,7% (IC95%= 7,8-9,6). Conclusão: modificações observadas no estado nutricional sugerem a ocorrência da transição nutricional na população de adolescentes. O percentual de cobertura disponível no sistema tem potencial de crescimento, entretanto, o atual cenário impacta em possível análise insuficiente para subsidiar a elaboração e reorientação de políticas públicas.

ABSTRACT

Objective:To construct an "Internet+nursing service" model for elderly patients at home led by the continuing care service center from the perspective of the Internet, and to provide reference for different types of professional home care services for elderly patients.Methods:From March 2019 to December 2021, relying on the continuing care service center, six links of "discharge preparation, discharge follow-up service, home care service, online diagnosis and treatment, institutional return visit, health guidance" in Wuhan Central Hospital, Tongji Medical College, Huazhong University of Science and Technology were interconnected to construct a closed-loop "Internet+nursing service" model for elderly patients at home. Summarized the workload after the implementation of "Internet+nursing service", and investigated the satisfaction of 149 patients and 112 home sevice nurses.Results:After 2 years and 9 months of implementation, the team provided 11 home care services for elderly patients with diabetes foot dressing change, PICC catheter maintenance, replacement of catheter, wound stoma care and home membrane dialysis etc, and provided 452 home care services for 149 patients. Patient satisfaction was (94.47 ± 3.07) points; the approval of this model by 112 home service nurses was (4.69 ± 0.43) points.Conclusions:The "Internet+nursing service" model for elderly patients at home, which is dominated by the continuous nursing service center, can meet the continuous nursing needs of elderly patients at home. At the same time, expanding nurses′ practice space and enhancing nurses′ professional value will play a positive role in promoting the development of nursing service industry.

Introduction: The purpose of this study was to clarify the relationship between the available service information on small-scale multifunctional in-home nursing care (KANTAKI) and its operational status via text mining.Methods: We obtained nationwide textual information on KANTAKI from the Nursing Care Service Information Disclosure System and the Ministry of Health, Labour, and Welfare, and analyzed the characteristics of the word usage using KH Coder. The number of users and employees and the implementation of services were compared among the facilities that used terms relating to medical dependency and end-of-life care, which are KANTAKI characteristics, and with other facilities.Results: The facilities that used terms relating to medical dependency and end-of-life care showed significantly more users requiring nursing care level 5 and more full-time nursing staff than those not using such terms. Moreover, regarding service provision, the rate of procedures was significantly higher in 11 of 12 items, except for stoma.Conclusion: The facilities that use terms relating to medical dependency and end-of-life care in their information have more users and provide a greater variety of services. In the future, educational support is required to enable facility managers to understand the services and translate them into their work.

ABSTRACT

@#Palliative care provides active holistic care for people with serious health-related suffering due to illness, especially near the end of life. In the Asia-Pacific region, there is variability in palliative care services and access to drugs, with a predominant focus on hospitals and cancer. The COVID-19 pandemic also disrupted the development and provision of palliative services. In this situation analysis, palliative services and the impact of the pandemic in five Asian countries (Bangladesh, Brunei, India, Myanmar and Thailand) are described. Overall, palliative care is an underserved specialty despite an obvious need for the services in each locality. While the pandemic significantly interrupted palliative services, palliative care providers adapted by developing guidelines and using teleconferencing. Overall, much work still needs to be done to improve palliative care in the region.

ABSTRACT

BACKGROUND: Postnatal care is given to mothers and newborn babies within 42 days of delivery. It is a period of high maternal and newborn mortality and is also the most neglected in terms of maternal health services in many parts of the world. This study aimed to assess postnatal care and associated factors among mothers who gave birth in the year preceding the survey of the Ayssaeta district. METHODS: A community-based cross-sectional study was conducted among 406 mothers who gave birth in the year preceding the survey from August 02­30, 2020. Bivariable and multivariable logistic regression analyses were done to identify factors associated with postnatal care utilization. RESULTS: Slightly greater than four out of ten mothers have visited postnatal care units at least once. Living in urban areas, giving birth in a health facility, having complications during labor and after, and getting advice during antenatal care visits were associated with higher odds of postnatal care utilization. CONCLUSION: Less than half of the mothers received postnatal care following the delivery of their last child. Living in an urban, place of delivery, experiencing labor and postpartum complications, and receiving postnatal care advice during antenatal care have affected the utilization of postnatal care. Promoting skilled delivery and antenatal care with a focus on rural areas can help mothers learn about postnatal care and increase the number of mothers who use it

Objective:To investigate the quality issues of community home care service in Beijing.Methods:A study on quality of community home care service was conducted from March to May in 2021 in six urban districts of Beijing, and 10 community health service centers were selected as study sites by purpose sampling method. The quality issues of home care service were studied by observing the process of 17 home care services, interviewing medical staff and reviewing relevant medical documents.Results:The study extracted 3 structural quality problems and 6 process quality problems of home care service. The structural quality problems included unclear service scheduling and insufficient qualification of the personnel; lacking special transportations; and lacking home health service standards in some centers. The process quality problems included incomplete material preparation, wearing jewelry during medical procedures, no physical check-up before procedures, not strictly following the aseptic principle in the operation, not properly disposing medical wastes, wrong or missing filling of medical documents.Conclusions:There are some problems in the implementation of community home care service in Beijing, such as insufficient medical personnel and material resources, unclear service scheduling, lack of unified medical service standards, and insufficient quality management. Therefore, these problems should corrected to ensure the quality of community home care services in Beijing.

Resumo A carga global e a prevalência de hipertensão arterial sistêmica (HAS) têm crescido nas últimas duas décadas, especialmente em países de baixa e média renda, representando uma preocupação para as autoridades em saúde. Este estudo analisou a prevalência de HAS entre adultos brasileiros em 2008, 2013 e 2019 e o controle da doença pelos indivíduos em 2013 e 2019. Utilizou-se dados da Pesquisa Nacional por Amostra de Domicílio (2008) e da Pesquisa Nacional de Saúde (2013-2019). Foram calculadas razões de prevalência da doença pelo método de Poisson, ajustado para caraterísticas sociodemográficas. Para os indicadores de cuidados em saúde e organização da atenção primária calculamos proporções estratificadas por sexo, faixa etária, raça e região. Os resultados indicam que as desigualdades regionais persistem, com menores prevalências no Norte e Nordeste e maiores no Sudeste e Sul. Embora os indicadores de acesso e utilização dos serviços de saúde sejam considerados bons, refletindo as melhorias na atenção primária nos últimos anos, ressaltamos a importância da adoção de estratégias multifacetadas para a prevenção e controle da HAS no país.


Abstract The global burden and the prevalence of systemic arterial hypertension (SAH) have increased over the last two decades, especially in low- and middle-income countries, and are a concern to health authorities. This study analyzed the prevalence of SAH reported by Brazilian adults in 2008, 2013, and 2019, and individual disease control in 2013 and 2019. Data from the National Household Sample Survey (2008) and National Health Survey (2013-2019) were employed. We calculated the disease's prevalence ratios using Poisson regression, adjusted for sociodemographic characteristics. Regarding health care and PHC organization indicators, we calculated proportions by gender, age group, ethnicity, and region. The results reveal persistent regional inequalities, with lower prevalence in the North and Northeast and higher prevalence in the Southeast and South. While the health care access and use indicators are positive, reflecting PHC improvements in recent years, we highlight the importance of adopting multifaceted SAH prevention and control strategies in the country.

Objetivo: descrever como a terapêutica nutricional domiciliar é realizada no Programa Melhor em casa do Ministério da Saúde e na Saúde suplementar. Método: Estudo transversal, com dados secundários, onde foram selecionados os perfis de profissionais atuantes em atenção domiciliaria no Brasil. A coleta de dados ocorreu de março a junho de 2018, depois de submetido e aprovado pelo Comitê de Ética e Pesquisa. Resultados: Dos 289 brasileiros, 74% eram profissionais atuantes na Assistência domiciliaria. O tipo de Terapia Nutricional realizada foi 67% enteral exclusiva seguida de 33% terapia mista. A prescrição realizada de dieta exclusiva artesanal foi de 9% e de dieta enteral mista 55%. A maioria recebe dieta por gravidade intermitente, seguida de administração em "bolus", gravitacional contínua e controle do gotejamento através da bomba de infusão. Conclusão: Os achados nos dão uma visão panorâmica da terapia nutricional enteral domiciliar no Brasil. A aumento da terapia nutricional domiciliaria se faz necessária especialmente pelo aumento da população idosa e consequentemente de maior presença das doenças crônicas que podem levar a incapacidade, dependência, maior tempo de hospitalização e custos para o sistema de saúde. É fundamental a presença da equipe interdisciplinar, de boas práticas e do acompanhamento das famílias nos domicílios.


Objective: To learn how home nutritional therapy is carried out in the Ministry of Health's Better Home Program and in Supplementary Health. Method: Cross-sectional study, with secondary data, in which the profiles of professionals working in home care in Brazil were selected. Data collection took place from March to June 2018, after being submitted and approved by the Ethics and Research Committee. Results: Of the 289 Brazilians, 74% were professionals working in home care. The type of nutritional therapy performed was 67% exclusive enteral followed by 33% mixed therapy. The prescription of an exclusive handmade diet was 9% and a mixed enteral diet 55%. Most receive intermittent gravity diet, followed by bolus administration, continuous gravitational and drip control through the infusion pump. Conclusion: The findings give us a panoramic view of home enteral nutritional therapy in Brazil. The increase in home nutritional therapy is necessary especially because of the increase in the elderly population and, consequently, the greater presence of chronic diseases that can lead to disability, dependence, longer hospitalization and costs for the health system. The presence of an interdisciplinary team, good practices and monitoring of families at home is essential.

La recolección de datos tuvo lugar de marzo a junio de 2018, luego de ser presentados y aprobados por el Comité de Ética e Investigación. Resultados: De los 289 brasileños, el 74% eran profesionales que trabajaban en la atención domiciliaria. El tipo de terapia nutricional realizada fue 67% enteral exclusiva seguida de 33% terapia mixta. La prescripción de una dieta exclusiva artesanal fue del 9% y una dieta enteral mixta del 55%. La mayoría recibe una dieta de gravedad intermitente, seguida de administración de bolo, control gravitacional continuo y de goteo a través de la bomba de infusión. Conclusión: Los hallazgos nos brindan una visión panorámica de la terapia nutricional enteral domiciliaria en Brasil. El aumento de la terapia nutricional domiciliaria es necesario sobre todo por el aumento de la población anciana y, en consecuencia, la mayor presencia de enfermedades crónicas que pueden derivar en discapacidad, dependencia, mayor internación y costos para el sistema de salud. La presencia de un equipo interdisciplinario, buenas prácticas y seguimiento de las familias en el hogar es fundamental.

@#Introduction: Paediatric Palliative care has been a relatively new specialty in Malaysia. It serves to cater hospital and community support for children with life-limiting illnesses (LLI). The aim of the study is to describe the clinical characteristic of Paediatric Palliative Care (PPC) cases in the North East of Peninsular Malaysia. Methods: Children with LLI were identified through 2 databases captured by the ‘Sayangku’ Homecare team and Paediatric Palliative Care (PPC) team. These databases include the details on clinical diagnosis, time of referral and outcome. Children included were those referred from November 2012 to March 2020 to PPC service. The number of new cases referred per annum (2019) was compared to the crude estimation of prevalence. Results: A total of 216 children were included. About half of the children with LLI (50.5%) were from group 4 and about a quarter from group 1 (27%) of the ACT/ RCPCH classification. Approximately one third of our cohort (31%) had died, majority from oncology causes. There were 116 neurology cases, 60 oncology cases and further 48 cases were the remaining cases in our databases. More than one third (37.5%) of the patients were in the age range of 11-18-year-old. The estimated cases under PPC was only 7.7% from the estimated annual prevalence of LLI. Conclusion: The current data showed that many of the LLI cases in our area have gone underreported. There is a need to disseminate information and awareness among the medical fraternity on the patient’s eligibility for the PPC service.

Objective@#To evaluate the degree of trust of patients in community traditional Chinese medicine(TCM)physicians, based on the TCM service capability promotion program, and to analyze the impacts by TCM prevention and health care service.@*Methods@#1 391 patients at 22 primary health service centers in 3 cities from Zhejiang province were investigated using the Chinese version of Wake Forest Physician Trust Scale(WFPTS)in September, 2017. Quantitative and qualitative variables were described by mean±SD, and ratio respectively. Differences were compared by t test, ANOVA and Kruskal-Wallis test.@*Results@#Chinese version of WFPTS had good reliability and validity in measuring community TCM physicians. The average scoring of patients′ trust in community TCM physicians was 38.00±6.42, while their benevolence and competence were 19.14±3.41, 18.82±3.50 respectively.For patients with different demographic characteristics, their quantity and the way to acquire and utilize TCM prevention and health care service had different degrees of trust in community TCM physicians. Patients′ gender, age, awareness and utilization of TCM prevention and health care service, etc. affect the degree of trust in community TCM physicians.@*Conclusions@#Patients have higher degree of trust in community TCM physicians, and improving the rate of awareness and utilization of TCM prevention has positive impacts on the degree of trust in these physicians. It is proposed that health knowledge education be strengthened and skills of community TCM physicians′ capacity be enhanced through various ways.

Objetivo: avaliar a percepção do paciente acerca do atendimento da equipe multiprofissional do Serviço de Atendimento Móvel de Urgência. Metodologia: trata-se de uma pesquisa de campo, qualitativa e descritiva, realizada por meio de entrevistas, com perguntas abertas a usuários do Serviço de Atendimento Móvel de Urgência no município de Piripiri-PI, no mês de novembro de 2018. Participaram da pesquisa 10 usuários. O estudo obedeceu aos princípios éticos e legais que rege as Resoluções 466/12 do Conselho Nacional de Saúde, foi aprovado pelo Comitê de Ética em Pesquisa com o parecer de nº 2.868.027. Resultados: observou-se que o tempo de espera e burocracia no atendimento telefônico, influenciou negativamente na avaliação do serviço. Também ficou claro que há falta de conhecimento da população a respeito do assunto, o que influencia a percepção dos usuários sobre a presença de humanização do serviço. Conclusão: a humanização atrelada à assistência pré-hospitalar não tem sido devidamente efetuada pelos profissionais, no entanto é importante para promoção de melhor qualidade no atendimento e consequente satisfação dos usuários.


Objective: to evaluate the patient's perception regarding the care provided by the multidisciplinary team of the Mobile Emergency Care Service. Methodology: this is a qualitative and descriptive field research, conducted through interviews, with open questions to users of the Mobile Emergency Care Service in Piripiri-PI in November 2018. Participated in the research ten users. The study complied with the ethical and legal principles governing Resolutions 466/12 of the National Health Council, was approved by the Research Ethics Committee with the opinion No. 2,868,027. Results: it was observed that the waiting time and bureaucracy in the telephone service, negatively influenced the evaluation of the service. It was also clear that there is a lack of knowledge of the population on the subject, which influences the perception of users about the presence of humanization of the service. Conclusion: humanization linked to prehospital care has not been properly performed by professionals, however it is important to promote better quality care and consequent user satisfaction.

This paper analyzes the development status and existing problems of informational TCM health care services for the elderly, and proposes development strategies for TCM health care services, including "Health Care Plus Intelligent Management, Platform Plus Tourism Support, Medical Network Integration, and Smoothening the Industry Chain" for the new era of "Internet Plus", with the purposes of realizing real-time disease detection and intelligent diagnoses, meeting the senior health care needs, responding proactively to the call of the aging population and promoting the development and revitalization of TCM in the field of senior care.

ABSTRACT

BACKGROUND: Falls are the most frequent adverse events reported in hospitals. The aim of this study was to investigate the incidence rate and characteristics of falls in patients who used comprehensive nursing care service in National Health Insurance Service Ilsan Hospital. METHODS: Incidence rate of falls was investigated in patients using comprehensive nursing care service, from July 2013 to Jun 2017 and compared with those not using this service. The characteristics and risk factors for falls, and fall-related injuries were obtained. RESULTS: Among the 62,445 patients who used the comprehensive nursing care service for 4 years, total of 672 falls were reported. The incidence rate of falls per 1,000 patients-day was 1.15. The percentage of fall-related injuries was 26.9% and that of major injury was 2.2%. Although the incidence rate of all falls was slightly higher in patients using comprehensive nursing care service than those not using this service, falls-related injuries were not correlated with the implementation of this service. CONCLUSION: The falls could be more frequently detected and reported in comprehensive nursing care service, but there was no difference in fall-related injuries.

ABSTRACT

BACKGROUND: Although the importance of pharmaceutical care service has been growing to meet the needs of customers with the improvement of clinical and humanistic outcomes, there was not a systematic strategy to promote research in Korea. The aim of this study was to suggest the core agendas for pharmaceutical care services research and development (R&D) considering priorities. METHODS: Based on desk researches, we developed R&D agendas for the needs of improving pharmaceutical care services in the area of institution, community, and public health. To determine the priority of agendas in developing pharmaceutical care service, analytic hierarchy process (AHP) analysis was performed by the 14 experts. Criteria and subcriteria were assessed for significance by pairwise comparisons. Then, agendas were evaluated for importance according to each subcriteria, and rank ordered considering the weight calculated by multiplying the importance scores of the criteria and the subcriteria. RESULTS: We derived 25 agendas including 13 for institutional pharmaceutical care service, 8 for community pharmaceutical care service, and 4 for public-health related pharmaceutical care service. AHP model was constructed based on 4 criteria and 8 subcriteria by a hierarchical structure. From the AHP survey, the ‘Development of pharmaceutical care service for metabolic and chronic disease’ agenda accounted for the highest priority. CONCLUSION: We have developed the R&D agendas of the pharmaceutical care service which should be promoted. The results should be utilized by the government to nationally support the development of the standards and relevant regulations related to pharmaceutical care services in Korea.

ABSTRACT

PURPOSE: The purpose of this study was to explore and analyze the latest trends in domestic research related to integrated nursing care service and to present a practical direction for future research.METHODS: This study was focused on domestic nursing research related to integrated nursing care service as published in journals until March 20, 2019. After reviewing the quality of the papers, 34 papers were finally selected for analysis. In order to analyze the overall trends, the analysis was reviewed by year, source, research design, and subjects, and content analysis was used to identify trends by topic.RESULTS: From the content analysis, the top categories were divided into nurses' view and patients' view, with both nurses' view and patients' view for the subcategories of ‘input’, ‘process’ and ‘output’ respectively. From both the nurse and patient view, the sub-category ‘input’ included ‘service environment’, ‘process’ and ‘service throughput’ and for the ‘output’ subcategory, three subjects, ‘performance’, ‘service-related perception change’ and ‘social psychological change’ were derived from the nurses' view, and two subjects ‘performance’ and ‘service-related perception change’ were derived from the patients' view.CONCLUSION: In order for future studies to provide practical basic data for stable settlement and improvement of integrated nursing care service, it is necessary to develop and study various aspects without bias, and in a balanced and integrated manner.
