ABSTRACT

RESUMEN Las úlceras solitarias de ciego (USC) son infrecuentes y no se registran aún en la literatura de la Ar gentina. Existen informes de casos y pequeñas series en otros países. Son de etiología desconocida y diagnóstico diferencial amplio. En este caso se resolvió mediante una hemicolectomía derecha supra selectiva por laparotomía de emergencia.


ABSTRACT Solitary cecal ulcers (SCUs) are rare and no cases have been published in Argentina. Few cases or series have been reported in other countries. The etiology is unknown and many other diseases must be ru led out before making the diagnosis. This case was solved with selective right hemicolectomy through laparotomy in an emergency basis.

ABSTRACT

A úlcera solitária benigna do ceco (USBC) é uma doença rara que normalmente se apresenta como uma massa abdominal ou por sangramento digestivo baixo. O diagnóstico definitivo se dá pelo exame anatomopatológico da peça cirúrgica. Os autores reportam caso de paciente jovem que procurou a Unidade de Pronto-Socorro do Hospital Unimar - Marília com quadro clínico, exame físico e testes laboratoriais compatíveis com o diagnóstico de apendicite aguda. Foi indicada laparotomia com abordagem abdominal pela incisão de McBurney. Durante inventário da cavidade peritoneal, foi constatada massa palpável em região cecal e apêndice normal. Uma vez que não se conseguiu afastar uma neoplasia maligna do ceco, optou-se pela hemicolectomia direita seguindo preceitos oncologicos os, O diagnóstico de USBC se deu pelo exame histológico do espécime cirúrgico. Assim como descrito neste caso clínico, usualmente o diagnóstico de USBC se dá pelo exame histológico. Além disso, cerca de 60% a 80% dos pacientes se apresentam inicialmente com quadro clínico de apendicite aguda. O diagnóstico clínico da USBC por meio de uma colonoscopia é muito raro. Devido a isso, o tratamento, na grande maioria das vezes, é a hemicolectomia direita,uma vez que durante o intraoperatório não é possível afastar um câncer do ceco.

ABSTRACT

Aspirin and non-steroidal anti-inflammatory drugs (NSAIDs) are widely prescribed drugs, and they are known to be responsible for many cases of gastroduodenal ulcerations by inhibiting prostaglandin synthesis via blocking the cyclo-oxygenase production. Colonic side effects of these drugs are rare, but they are increasingly being reported to be due to the popularization of colonoscopy and the new formulations of drugs such as enteric coated pills or slow release pills. There is currently no consensus for making the clinical diagnosis and administering the proper therapy for drug-induced colonopathy in Korea. We experienced the patient who had multiple colonic ulcers after ingesting large amounts of aspirin and NSAIDs. In near future, we hope to determine the clinical and endoscopic features of drug associated colon injury.

ABSTRACT

PURPOSE: Non-specific ulceration of colon is a rare disease of unknown etiology. To establish correctly the diagnosis of nonspecific colon ulcer preoperatively is difficult, but with more wide spread use of colonoscopy and complementary diagnostic aids, this lesion may be accurately diagnosed more often. The presentating symptoms were mainly lower abdominal pain, which mimic such conditions as acute appendicitis, diverticulitis, intestinal obstruction, and colon cancer. If its serious complications (perforation, abscess formation, or uncontrolled bleeding) were developed, resection of ulcerated segment or more extensive colectomy was recommended. The purpose of this study was to establish correctly the diagnosis of nonspecific colon ulcer preoperatively and to identify definitive treatment of complicated colon ulcer. METHODS: 6 cases, surgically treated as non-specific colon ulcer at Chosun University hospital from January 1995 to December 1999 were studied retrospectively. RESULTS: The ages of the patient ranged from 35 to 70 years; the ratio of male to female is 2: 1. The main clinical symptoms were abdominal pain (6 cases), nausea and vomiting (4 cases), hematochezia (3 cases), constipation (2 cases) and palpable mass (1 case). The preoperative diagnosis was generalized peritonitis (2 cases), colon cancer (2 cases), acute appendicitis (1 case), mechanical obstruction (1 case). 3 cases had past medication history, one was anti-hypertensive drug and the two were nonsteroidal anti-inflammatory drug. The preoperative diagnostic study were simple abdomen (6 cases), abdominal CT (6 cases), colon study (3 cases), abdominal sonograph (4 cases) and selective angiogram (1 cases), The location of ulcer were cecum (2 cases), sigmoid colon (2 cases), transversecolon (1 case) and descending colon (1 case). The methods of operation were hemicolectomy (3 cases), segmental resection (1 case) and anterior resection (2 case). There was one death, but 5 cases were completely recovered. CONCLUSIONS: Non-specific ulcer of the colon is not detected until complicated by bleeding, perforation, or obstruction. The patient who has chronic abdominal pain and rectal bleeding can be diagnosed preoperatevely by colonoscopy and colon study. The recommended therapy of complicated cases is resection of the ulcerated segment or more extensive colectomy. Non-specific colonic ulcer should be suspected as one of colonic disease in the patients with complications.
